1. Anti–IL-4R versus anti–IL-5/5R after anti–IL-5/5R failure in asthma: An emulated target trial.

Abstract

[Display omitted] Switching biologics is now common practice in severe eosinophilic asthma. After insufficient response to anti–IL-5 or 5 receptor (anti–IL-5/5R), the optimal switch between an anti–IL-4R mAb (interclass) or another anti–IL-5/5R drug (intraclass) remains unknown. We sought to compare the effectiveness of these 2 strategies in asthma control in patients with severe eosinophilic asthma and insufficient response to an anti–IL-5/5R mAb. We emulated a target randomized trial using observational data from the Recherche sur les AsthMes SEvèreS (RAMSES) cohort. Eligible patients were switched to an anti–IL-4R mAb or another anti–IL-5/5R drug after insufficient response to an anti–IL-5/5R mAb. The primary outcome was the change in Asthma Control Test score at 6 months. Among the 2046 patients in the cohort, 151 were included in the study: 103 switched to an anti–IL-4R mAb and 48 to another anti–IL-5/5R. At 6 months, the difference in Asthma Control Test score improvement was not statistically significant (mean difference groups, 0.82 [−0.47 to 2.10], P =.213). The interclass group exhibited greater cumulative reduction in oral corticosteroid dose (P inter-intra , −1.05 g [−1.76 to −0.34], P =.041). The interclass group had a better effect, although not significantly, on reducing exacerbations (Δ inter-intra , −0.37 [−0.77 to 0.02], P =.124) and increasing lung function (FEV 1) (126.8 mL [−12.7 to 266.4], P =.124). After anti–IL-5/5R mAb insufficient response, switching to dupilumab demonstrated similar improvement in Asthma Control Test scores compared with intraclass switching. 4. Eosinophils and tissue remodeling: Relevance to airway disease.

Abstract

The ability of human tissue to reorganize and restore its existing structure underlies tissue homeostasis in the healthy airways, but in disease can persist without normal resolution, leading to an altered airway structure. Eosinophils play a cardinal role in airway remodeling both in health and disease, driving epithelial homeostasis and extracellular matrix turnover. Physiological consequences associated with eosinophil-driven remodeling include impaired lung function and reduced bronchodilator reversibility in asthma, and obstructed airflow in chronic rhinosinusitis with nasal polyps. Given the contribution of airway remodeling to the development and persistence of symptoms in airways disease, targeting remodeling is an important therapeutic consideration. Indeed, there is early evidence that eosinophil attenuation may reduce remodeling and disease progression in asthma. This review provides an overview of tissue remodeling in both health and airway disease with a particular focus on eosinophilic asthma and chronic rhinosinusitis with nasal polyps, as well as the role of eosinophils in these processes and the implications for therapeutic interventions. Areas for future research are also noted, to help improve our understanding of the homeostatic and pathological roles of eosinophils in tissue remodeling, which should aid the development of targeted and effective treatments for eosinophilic diseases of the airways. 9. Essential role of smooth muscle Rac1 in severe asthma-associated airway remodelling

Abstract

BackgroundSevere asthma is a chronic lung disease characterised by inflammation, airway hyperresponsiveness (AHR) and airway remodelling. The molecular mechanisms underlying uncontrolled airway smooth muscle cell (aSMC) proliferation involved in pulmonary remodelling are still largely unknown. Small G proteins of the Rho family (RhoA, Rac1 and Cdc42) are key regulators of smooth muscle functions and we recently demonstrated that Rac1 is activated in aSMC from allergic mice. The objective of this study was to assess the role of Rac1 in severe asthma-associated airway remodelling.Methods and resultsImmunofluorescence analysis in human bronchial biopsies revealed an increased Rac1 activity in aSMC from patients with severe asthma compared with control subjects. Inhibition of Rac1 by EHT1864 showed that Rac1 signalling controlled human aSMC proliferation induced by mitogenic stimuli through the signal transducer and activator of transcription 3 (STAT3) signalling pathway. In vivo, specific deletion of Rac1 in SMC or pharmacological inhibition of Rac1 by nebulisation of NSC23766 prevented AHR and aSMC hyperplasia in a mouse model of severe asthma. Moreover, the Rac1 inhibitor prevented goblet cell hyperplasia and epithelial cell hypertrophy whereas treatment with corticosteroids had less effect. Nebulisation of NSC23766 also decreased eosinophil accumulation in the bronchoalveolar lavage of asthmatic mice.ConclusionThis study demonstrates that Rac1 is overactive in the airways of patients with severe asthma and is essential for aSMC proliferation. It also provides evidence that Rac1 is causally involved in AHR and airway remodelling. Rac1 may represent as an interesting target for treating both AHR and airway remodelling of patients with severe asthma.

L’asthme touche 6 à 12% des femmes enceintes. Le mauvais contrôle de l’asthme et les exacerbations sont associés à des complications obstétricales et néonatales. Les corticoïdes inhalés et bronchodilatateurs ont fait la preuve de leur sécurité et de leur efficacité pour prévenir les exacerbations d’asthme, qui doivent être gérées rapidement ; leur maintien pendant la grossesse est crucial. Le sevrage tabagique, les vaccinations, la prise en charge du reflux et de la rhinite contribuent au contrôle de la maladie. La consultation préconceptionnelle est l’occasion de réévaluer le traitement.

13. Kyste hydatique pulmonaire autochtone

Abstract

L’échinococcose kystique, ou hydatidose, encore appelée kyste hydatique, est une parasitose cosmopolite présente essentiellement dans les régions d’élevage ovin et bovin. Cette anthropozoonose est liée au développement tissulaire d’une hydatide d’un tænia échinocoque, Echinococcus granulosus, retrouvé à l’état adulte dans le tube digestif des canidés. En France, cette cestodose larvaire représente essentiellement une maladie d’importation développée par des patients originaires de zones endémiques telles que l’Afrique de l’Est et du Nord, l’Amérique du Sud ou l’Asie. Cependant, des formes autochtones, bien que rares, persistent encore. Nous décrivons ici le cas d’un patient francilien de 39 ans, non fumeur, admis aux urgences pour des douleurs thoraciques, associées à des sueurs et des frissons. L’interrogatoire retrouve la notion d’un nodule pulmonaire lobaire inférieur droit découvert 20 ans auparavant sur une radiographie thoracique, jamais exploré ni surveillé. La tomodensitométrie thoracique montre deux volumineuses opacités kystiques avec images feuilletées endokystiques, dont une rompue dans la plèvre avec épanchement pleural droit. Cette suspicion radiologique d’échinococcose kystique pulmonaire fissurée est confirmée par une sérologie hydatidose positive. La réunion de concertation pluridisciplinaire retient l’indication de segmentectomie basale droite élargie à un patch diaphragmatique, associée à un traitement par albendazole. Le diagnostic sera confirmé par les données parasitologiques et anatomopathologiques. Dans cet article, nous aborderons les caractéristiques macroscopiques et microscopiques de cette parasitose rare en France métropolitaine et nous discuterons des éléments de la prise en charge à l’état frais permettant de limiter l’essaimage parasitaire.

15. Les exacerbations d’asthme chez l’adulte : prévenir et guérir

Abstract

Les exacerbations d’asthme sont encore responsables de 900 morts par an en France, dont la plupart sont jugées évitables. Elles ont un impact majeur sur la qualité de vie des patients et sont responsables d’une bonne partie du coût socioéconomique de l’asthme en France. Les exacerbations d’asthme peuvent en partie être prévenues par la corticothérapie inhalée en traitement de fond, à la dose adaptée au niveau de contrôle des symptômes et la prise en charge des facteurs aggravants. Tout patient devrait bénéficier d’un plan d’action écrit lui permettant de débuter au plus tôt la corticothérapie orale lors d’une exacerbation. Le traitement associe des bronchodilatateurs de courte durée d’action et une corticothérapie systémique. L’antibiothérapie systématique n’a pas sa place dans le traitement des exacerbations. Le patient doit être revu précocement au décours d’une exacerbation d’asthme afin de revoir l’ensemble de son suivi.

16. Effectiveness of bronchial thermoplasty in patients with severe refractory asthma: Clinical and histopathologic correlations.

Abstract

Background The effectiveness of bronchial thermoplasty (BT) has been reported in patients with severe asthma, yet its effect on different bronchial structures remains unknown. Objective We sought to examine the effect of BT on bronchial structures and to explore the association with clinical outcome in patients with severe refractory asthma. Methods Bronchial biopsy specimens (n = 300) were collected from 15 patients with severe uncontrolled asthma before and 3 months after BT. Immunostained sections were assessed for airway smooth muscle (ASM) area, subepithelial basement membrane thickness, nerve fibers, and epithelial neuroendocrine cells. Histopathologic findings were correlated with clinical parameters. Results BT significantly improved asthma control and quality of life at both 3 and 12 months and decreased the numbers of severe exacerbations and the dose of oral corticosteroids. At 3 months, this clinical benefit was accompanied by a reduction in ASM area (median values before and after BT, respectively: 19.7% [25th-75th interquartile range (IQR), 15.9% to 22.4%] and 5.3% [25th-75th IQR], 3.5% to 10.1%, P < .001), subepithelial basement membrane thickening (4.4 μm [25th-75th IQR, 4.0-4.7 μm] and 3.9 μm [25th-75th IQR, 3.7-4.6 μm], P = 0.02), submucosal nerves (1.0 ‰ [25th-75th IQR, 0.7-1.3 ‰] immunoreactivity and 0.3 ‰ [25th-75th IQR, 0.1-0.5 ‰] immunoreactivity, P < .001), ASM-associated nerves (452.6 [25th-75th IQR, 196.0-811.2] immunoreactive pixels per mm 2 and 62.7 [25th-75th IQR, 0.0-230.3] immunoreactive pixels per mm 2 , P = .02), and epithelial neuroendocrine cells (4.9/mm 2 [25th-75th IQR, 0-16.4/mm 2 ] and 0.0/mm 2 [25th-75th IQR, 0-0/mm 2 ], P = .02). Histopathologic parameters were associated based on Asthma Control Test scores, numbers of exacerbations, and visits to the emergency department (all P ≤ .02) 3 and 12 months after BT. Conclusion BT is a treatment option in patients with severe therapy-refractory asthma that downregulates selectively structural abnormalities involved in airway narrowing and bronchial reactivity, particularly ASM, neuroendocrine epithelial cells, and bronchial nerve endings. Abstract

Il est licite d’évaluer la pertinence de la recherche destinée à identifier des biomarqueurs et à les confronter à la routine clinique. Au-delà de l’effet de mode, le développement actuel de biomarqueurs dans toutes les pathologies est probablement lié aux progrès considérables des sciences cliniques et biologiques, les plateformes biologiques et les biobanques s’étant appuyées sur des capacités nouvelles à constituer des bases de données longitudinales robustes sous formes de registres ou de cohortes. Le plus délicat, si étrange que cela puisse paraître, reste de poser la bonne question, c’est-à-dire de correctement identifier pour quelle zone de la problématique clinique posée par la pathologie un biomarqueur pourrait être utile. Les spécificités de l’asthme, en tant que maladie chronique, semblent effectivement indiquer qu’il existe de réels besoins non couverts. La variabilité dans le temps, la chronicité, la faiblesse des arguments diagnostiques, et l’hétérogénéité de la présentation clinique impliquent effectivement de disposer d’aides supplémentaires diagnostiques et pronostiques, potentiellement pour l’aide à la prise en charge et l’ajustement thérapeutique, mais aussi pour l’identification phénotypique et/ou endotypique déjà utilisée dans les premières initiatives de médecine personnalisée. Dans l’asthme, le volume expiré maximal pendant une seconde (VEMS) est un biomarqueur disponible depuis longtemps, et on peut raisonnablement penser qu’il couvre déjà d’assez nombreux secteurs. Aussi, il semble opportun d’identifier les caractéristiques nécessaires pour qu’un biomarqueur puisse voir le jour dans l’asthme, ceux dont on dispose déjà, et identifier les méthodes potentielles pour en mettre de nouveaux en évidence. Pour cela, nous proposons une revue de la littérature récente sur le sujet.

22. Computed Tomography Assessment of Airways Throughout Bronchial Tree Demonstrates Airway Narrowing in Severe Asthma.

Abstract

Rationale and Objectives To analyze airway dimensions throughout the bronchial tree in severe asthmatic patients using multidetector row computed tomography (MDCT) focusing on airway narrowing. Materials and Methods Thirty-two patients with severe asthma underwent automated (BronCare software) analysis of their right lung bronchi, with counts of airways >3 mm long arising from the main bronchi (airway count) and bronchial dimension quantification at segmental and subsegmental levels (lumen area [LA], wall area [WA], and WA%). Focal bronchial stenosis was defined as >50% narrowing of maximal LA on contiguous cross-sectional slices. Severe asthmatics were compared to 13 nonsevere asthmatic patients and nonasthmatic (pooled) subjects (Wilcoxon rank tests, then stepwise logistic regression). Finally, cluster analysis of severe asthmatic patients and stepwise logistic regression identified specific imaging subgroups. Results The most significant differences between severe asthmatic patients and the pooled subjects were bronchial stenosis (subsegmental and all bronchi: P < .002) and WA% ( P < .0003). Stepwise logistic regression retained WA% as the only explanatory covariable ( P = .002). Two identified clusters of severe asthmatic patients differed for parameters characterizing airway narrowing (airway count: P = .0002; focal bronchial stenosis: P = .009). Airway count was as discriminant as forced expiratory volume in 1 second/forced vital capacity ( P = .01) to identify patients in each cluster, with both variables being correlated ( r = 0.59, P = .005). Conclusions Severe asthma–associated morphologic changes were characterized by focal bronchial stenoses and diffuse airway narrowing; the latter was associated with airflow obstruction. 27. Clinical and histopathologic predictors of therapeutic response to bronchial thermoplasty in severe refractory asthma.

Abstract

Phenotypes and endotypes predicting optimal response to bronchial thermoplasty (BT) in patients with severe asthma remain elusive. Our aim was to compare the clinical characteristics and hallmarks of airway inflammation and remodeling before and after BT in responder and partial responder patients with severe asthma refractory to oral steroids and to omalizumab. In all, 23 patients with severe refractory asthma were divided into BT responders (n = 15) and BT partial responders (n = 8), according to the decrease in asthma exacerbations at 12 months after BT. Clinical parameters were compared at baseline and 12 months after BT, and hallmarks of airway inflammation and remodeling were analyzed by immunohistochemistry in bronchial biopsy specimens before and 3 months after BT. At baseline, the BT responders were around 8 years younger than the BT partial responders (P =.02) and they had a greater incidence of atopy, higher numbers of blood eosinophils (both P =.03) and IgE levels, higher epithelial IFN-α expression, and higher numbers of mucosal eosinophils and IL-33–positive cells (P ≤.05). A reduction in blood eosinophil count, serum IgE level, type 2 airway inflammation, and numbers of mucosal IL-33–positive cells and mast cells associated with augmented epithelial MUC5AC and IFN-α/β immunostaining was noted after BT in responders, whereas the numbers of mucosal IL-33–positive cells were augmented in BT partial responders. Most of these changes were correlated with clinical parameters. Subepithelial membrane thickening and airway smooth muscle area were similar in the 2 patient groups at baseline and after BT. By reducing allergic type 2 inflammation and increasing epithelial MUC5AC and anti-viral IFN-α/β expression, BT may enhance host immune responses and thus attenuate exacerbations and symptoms in BT responders. Eosinophilic granulomatosis with polyangiitis (EGPA) (Churg–Strauss syndrome) is a peculiar hybrid condition of a systemic antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis and a hypereosinophilic disorder with frequent lung involvement that occurs in people with asthma. This review focuses on areas of evidence or persistent uncertainty in the classification, epidemiology, clinical presentation, diagnosis, prognosis and management of EGPA and attempts to identify clues to the mechanisms in the development or course of the disease.

Abstract

We report five cases of asthma unmasked by anti-tumor necrosis factor (TNF)-α-blocking drugs. Asthma symptoms appeared within an average of 4 months (range 1-24 months) after starting the anti-TNF-α treatment for inflammatory disease. The patients did not appear to be predisposed to asthma except for one patient who had asthma during childhood. Four patients stopped anti-TNF-α-blocking drugs with an improvement of symptoms within 1 to 5 months. In the patient with a history of childhood asthma, respiratory symptoms recurred when another anti-TNF-α therapy was started. Asthma control was achieved with inhaled steroids, allowing anti-TNF-α treatment to continue. The biotherapy was maintained for the fifth patient in association with inhaled steroids. The pathophysiologic mechanisms are unknown but are probably more complex than the T helper 1/T helper 2 imbalance suggested in the literature, and further studies are required.

Dupilumab, a fully human monoclonal antibody, blocks the shared receptor component for interleukins-4/13, which are key and central drivers of type 2 inflammation. Phase 3 VENTURE (NCT02528214) and TRAVERSE open-label extension (NCT02134028) evaluated dupilumab 300 mg vs placebo every 2 weeks for 24 weeks (VENTURE) and dupilumab only for an additional 48 to 96 weeks (TRAVERSE) in patients with oral corticosteroid (OCS)–dependent severe asthma.

L’asthme est une maladie inflammatoire chronique des voies aériennes qui doit être traitée par des corticoïdes inhalés pour éviter les exacerbations. Les bronchodilatateurs de courte durée d’action sont des traitements d’appoint qui ne traitent que le bronchospasme. Ils doivent toujours être prescrits en association avec un corticoïde inhalé en traitement de fond. Différents schémas d’administration des corticoïdes inhalés sont proposés, adaptés en fonction de la sévérité de l’asthme et de la dose de corticoïdes inhalés nécessaire. Chacun nécessite un plan d’action adapté pour la gestion des exacerbations. 44. Spontaneous Hemomediastinum Complicating Steroid-Induced Mediastinal Lipomatosis

Abstract

Spontaneous hemomediastinum is a rare event, occurring inassociation with bleeding disorders, intratumoral bleeding, orfollowing an abrupt increase in intrathoracicpressure. We report the case of a patient with systemic lupuserythematosus, nephrotic syndrome, and renal failure, in whommediastinal lipomatosis (ML) developed following increasedcorticosteroid therapy. Anticoagulant therapy likely precipitated amassive spontaneous hemomediastinum secondary to diffuse hemorrhage ofmediastinal fat, which required emergency decompressive surgery. Steroid-induced ML is common and usually well tolerated, but clinicians should be aware of its potential risk of bleeding whenassociated with anticoagulant therapy. This case further emphasizes thebleeding complications of treatment with low-molecular-weightheparin in patients with renal failure.

Abstract

Background Asthma is a complex disease with heterogeneous features of airway inflammation and remodeling. The increase in airway smooth muscle (ASM) mass is an essential component of airway remodeling in patients with severe asthma, yet the pathobiological mechanisms and clinical outcomes associated with ASM enlargement remain elusive. Objective We sought to compare ASM area in control subjects and patients with mild-to-moderate or severe asthma and to identify specific clinical and pathobiological characteristics associated with ASM enlargement. Methods Bronchial biopsy specimens from 12 control subjects, 24 patients with mild-to-moderate asthma, and 105 patients with severe asthma were analyzed for ASM area, basement membrane thickness, vessels, eosinophils, neutrophils, T lymphocytes, mast cells, and protease-activated receptor 2 (PAR-2). In parallel, the levels of several ASM mitogenic factors, including the PAR-2 ligands, mast cell tryptase, trypsin, tissue factor, and kallikrein (KLK) 5 and KLK14, were assessed in bronchoalveolar lavage fluid. Data were correlated with asthma severity and control both at inclusion and after 12 to 18 months of optimal management and therapy. Results Analyses across ASM quartiles in patients with severe asthma demonstrated that patients with the highest ASM quartile (median value of ASM area, 26.3%) were younger (42.5 vs ≥50 years old in the other groups, P ≤ .04) and had lower asthma control after 1 year of optimal management ( P ≤ .006). ASM enlargement occurred independently of features of airway inflammation and remodeling, whereas it was associated with PAR-2 overexpression and higher alveolar tryptase ( P ≤ .02) and KLK14 ( P ≤ .03) levels. Conclusion Increase in ASM mass, possibly involving aberrant expression and activation of PAR-2–mediated pathways, characterizes younger patients with severe asthma with poor asthma control.
