Abstract

•In this real-world clinical MS setting, using icobrain for analysis of brain atrophy and our quality control criteria, we found comparable variance on brain volume changes for same-scanner and different-scanner MRI pairs.•A higher number of different-scanner MRI pairs did not pass quality control and had poorer alignment similarity.•Applying strict selection criteria for volumetric analyses in real-world settings, and longitudinal imaging within scanners, remain imperative to reduce brain atrophy variability.

Abstract

Background: Magnetic Resonance Imaging (MRI) has become part of clinical routine for diagnosis of neurodegenerative brain disorders. Acquisitions are performed at multiple centers, using multiple imaging systems and is therefore subjected to intra‐ and inter‐scanner variability. Detailed analysis of brain volumetry differences between MRI systems and scan‐rescan acquisitions can provide useful information to correct for different MRI scanner effects in future multi‐center longitudinal studies. Method: The study population (n=10) consisted of five cognitively healthy controls and five patients belonging to different stages of the AD continuum. Subjects underwent brain MRI acquisitions on three different MRI systems with harmonized scan parameters: Philips Achieva dStream 1.5T, Philips Ingenia 3T and GE Discovery MR750w 3T. Sequences were harmonized by keeping acquisition parameters similar (Table 1). To test intra‐scanner variability, each participant underwent two subsequent MRI scans (between‐scan repositioning time 3‐5 minutes) per imaging system, repeated on three different MRI systems within two hours to test inter‐scanner variability. A total of 60 MRI scans were acquired for downstream comparative analysis. Brain volumes, including whole brain, (cortical) gray matter, hippocampal volumes, lateral ventricles, frontal, temporal and parietal cortices were computed with icobrain dm (v5.0). Coefficients of variation (CV), absolute volume differences (AVD), Dice similarity coefficients (DSC) and intraclass correlation coefficients (ICC) were computed to assess within‐ and between‐scanner agreement. Result: The intra‐scanner CVs over all volumes were 0.948% for Discovery MR750w, 1.050% for Achieva and 1.152% for Ingenia. The inter‐scanner CVs were 3.016% for all‐scanner comparisons, 2.044% for Ingenia vs. Achieva, 3.191% for Achieva vs. GE and 3.326% for Ingenia vs. GE. The AVDs and ICCs were in line with the CV values. The intra‐scanner DSCs (mean±SD) over all volumes were 0.917±0.011 for Discovery MR750w, 0.917±0.010 for Achieva and 0.917±0.015 for Ingenia. The inter‐scanner DSCs were 0.898±0.017 for all‐scanner comparisons, 0.912±0.012 for Ingenia vs. Achieva, 0.888±0.013 for Achieva vs. Discovery MR750w and 0.893±0.015 for Ingenia vs. Discovery MR750w (Table 2, 3, Figure 1). Conclusion: The variability results show an excellent agreement (CV<2%) for within‐scanner and a good agreement (CV<5%) for between‐scanner comparisons in terms of clinical relevance, indicating that MRI acquisition harmonization is beneficial for automated MRI volumetry software. Enrolling traumatic brain injury (TBI) patients with an inability to provide informed consent in research is challenging. Alternatives to patient consent are not sufficiently embedded in European and national legislation, which allows procedural variation and bias. We aimed to quantify variations in informed consent policy and practice.

7. Analysis of structural and functional connectivity MRI biomarkers in Alzheimer's disease: Neuroimaging / Multi‐modal comparisons.

Abstract

Background: MRI allows detecting functional and structural connectivity changes in Alzheimer's disease (AD). However, the relative importance of the corresponding MRI‐biomarkers is not fully understood. Moreover, several of these connectivity changes are not only driven by AD but also by healthy aging, which can limit the use of these biomarkers. Therefore, we looked at how 31‐different MRI biomarkers change due to aging and the presence of AD. Furthermore, we trained a set of machine‐learning‐models to distinguish AD from control subjects. The models were used to rank the features in terms of classification‐performance and analyze the impact of including age. Method: 20 AD patients and 20 age and gender‐matched healthy controls underwent MRI scanning (3T GE), including T1‐weighted, diffusion‐MRI, and resting‐state‐fMRI (rsfMRI). Whole‐brain (WB), white matter (WM), gray matter (GM), cortical GM and left and right hippocampi volumes were measured using T1‐images and icobrain. rsfMRI between regions of the default mode network was assessed using group independent component analysis. Median diffusivity and kurtosis values in GM and WM were determined. T‐tests between groups and ANOVA analysis using a Generalized‐Linear‐Model that included age and AD‐status as covariates were conducted for each feature. For assessing the ability of each feature to separate AD subjects from controls, a Support‐Vector‐Machine (SVM) model was trained using each feature separately and each feature with age. Performance was measured using 10‐fold cross‐validation. Result: Figure 1 shows the table with the ANOVA results. Generally, rsfMRI features showed only a significant association with disease‐status, whereas structural and diffusion‐derived features showed both an association with age and disease‐status. Figure 2 shows the classification performance for the different SVM models. Regarding classification performance, the models trained with the total volume of the hippocampus achieved the highest performance (accuracy:77%, sensitivity:64%, specificity:89%). Despite several of the features changing with both age and AD‐status, the inclusion of age in the SVM‐models didn't provide an advantage in classification performance. Conclusion: Multiple MRI‐biomarkers are able to detect structural and functional connectivity changes associated with AD. From those, the hippocampi volumes are the ones that better distinguish AD patients from cognitively‐healthy subjects. [ABSTRACT FROM AUTHOR]

8. Histological Differences of the Vascular Wall Between Sites With High and Low Prevalence of Intracranial Aneurysm.

Abstract

Intracranial aneurysms (IAs) develop more often on bifurcations compared with the rest of the circle of Willis (CoW). We investigated histological differences between 2 high IA prevalence sites (anterior communicating artery [AcomA] and basilar tip) and 2 corresponding low IA prevalence sites (anterior cerebral artery [ACA] and basilar artery [BA]) using histological sections of 10 CoWs without IAs. Medial defect density in the AcomA was 0.24 medial defects/mm compared with 0.02 for the A1 part and 0.03 for the A2 part of the ACA. In the basilar tip we found 0.15 medial defects/mm compared with 0.14 in the BA. Vascular smooth muscle cells (VSMCs) were more often disorganized in both high-prevalence sites (AcomA: 10/10, basilar tip: 5/10) compared with low-prevalence sites (both ACA and BA: 1/10). Intima thickening was more severe in the high-prevalence sites. Vascular wall thickness was not significantly different between high- and low-prevalence sites, but had a larger variance in high- compared with low-prevalence sites (AcomA vs ACA: p = 6.8E-12, basilar tip vs BA: p = 0.02). Disorganized VSMCs at high-prevalence sites likely result in a higher susceptibility to hemodynamic stress, leading to more vascular remodeling (such as intima thickening), which could increase the likelihood of IA formation.

9. Volumetric brain changes in MOGAD: A cross-sectional and longitudinal comparative analysis.

Abstract

• The information regarding the pattern and amount of brain atrophy in MOGAD is currently very limited. • We compared cross-sectional brain volume measures in clinically stable MOGAD to MS, NMOSD and HC, and longitudinal trajectories of brain volume changes in a subset of clinically stable MOGAD, MS and NMOSD patients. • We found a distinct pattern of brain atrophy in MOGAD, suggesting that a subclinical neurodegenerative process may occur in this rare condition. • Our findings may shed light on the distinct pathogenic mechanisms underlying the various neuroinflammatory CNS diseases. Relatively little is known about how global and regional brain volumes changes in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) compare with Multiple Sclerosis (MS), Neuromyelitis optica spectrum disorder (NMOSD), and healthy controls (HC). To compare global and regional brain volumes in MOGAD, MS, NMOSD, and HC cross-sectionally as well as longitudinally in a subset of patients. We retrospectively reviewed all adult MOGAD and NMOSD patients with brain MRI performed in stable remission and compared them with MS patients and HC. Volumetric parameters were assessed using the FDA-approved icobrain software. adjusted for age and sex. Twenty-four MOGAD, 47 NMOSD, 40 MS patients, and 37 HC were included in the cross-sectional analyses. Relative to HC, the age-adjusted whole brain (WB) volume was significantly lower in patients with MOGAD (p=0.0002), NMOSD (p=0.042), and MS (p=0.01). Longitudinal analysis of a subset of 8 MOGAD, 22 NMOSD, and 34 MS patients showed a reduction in the WB and cortical gray matter (CGM) volumes over time in all three disease groups, without statistically significant differences between groups. The MOGAD group had a greater loss of thalamic volume compared to MS (p=0.028) and NMOSD (p=0.023) and a greater loss of hippocampal volumes compared to MS (p=0.007). Age-adjusted WB volume loss was evident in all neuroinflammatory conditions relative to HC in cross-sectional comparisons. In longitudinal analyses, MOGAD patients had a higher thalamic atrophy rate relative to MS and NMOSD, and a higher hippocampal atrophy rate relative to MS. Visual evoked potentials (VEP) index visual pathway functioning, and are often used for clinical assessment and as outcome measures in people with multiple sclerosis (PwMS). VEPs may also reflect broader neural disturbances that extend beyond the visual system, but this possibility requires further investigation. In the present study, we examined the hypothesis that delayed latency of the P100 component of the VEP would be associated with broader structural changes in the brain in PwMS. We obtained VEP latency for a standard pattern-reversal checkerboard stimulus paradigm, in addition to Magnetic Resonance Imaging (MRI) measures of whole brain volume (WBV), gray matter volume (GMV), white matter volume (WMV), and T2-weighted fluid attenuated inversion recovery (FLAIR) white matter lesion volume (FLV). Correlation analyses indicated that prolonged VEP latency was significantly associated with lower WBV, GMV, and WMV, and greater FLV. VEP latency remained significantly associated with WBV, GMV, and WMV even after controlling for the variance associated with inter-ocular latency, age, time between VEP and MRI assessments, and other MRI variables. VEP latency delays were most pronounced in PwMS that exhibited low volume in both white and gray matter simultaneously. Furthermore, PwMS that had delayed VEP latency based on a clinically relevant cutoff (VEP latency ≥ 113 ms) in both eyes had lower WBV, GMV, and WMV and greater FLV in comparison to PwMS that had normal VEP latency in one or both eyes. The findings suggest that PwMS that have delayed latency in both eyes may be particularly at risk for exhibiting greater brain atrophy and lesion volume. These analyses also indicate that VEP latency may index combined gray matter and white matter disturbances, and therefore broader network connectivity and efficiency. Background Brain volume assessment has become an important outcome within research and treatment of multiple sclerosis (MS). MSmetrix (Icometrix) has been developed specifically for patients with MS. The original healthy, ‘normal’ population used for determination of the acceptable brain volume data consisted mainly of Europeans.Methods In the present study, results from 295 completely normal magnetic resonance imaging scans from Brazilian subjects were plotted against the original results in MSmetrix.Results With regard to whole brain volume, only 2% of the healthy adult subjects from Brazil were below the 5th percentile of the graph, and for grey matter volume, only 3% of the subjects were below the 5th percentile of MSmetrix standards.Conclusion MSmetrix may be used across different populations.

Magnetic resonance imaging (MRI) data from multiple sclerosis (MS) patients treated in real-world settings are important for understanding disease-modifying therapy effects, including no evidence of disease activity (NEDA) assessment. This longitudinal, retrospective, single-cohort analysis assessed MRI and clinical disease outcomes in patients with relapsing-remitting MS treated with natalizumab for up to 5 years in Prague, the Czech Republic. The primary study endpoint was the proportion of patients free of new or enlarging fluid-attenuated inversion recovery (FLAIR) lesions after at least 2 years of natalizumab treatment. Secondary endpoints included percentage brain volume change over time, the number of new T1-hypointense lesions that persisted for ≥6 months, FLAIR and T1-hypointense lesion volume change over time, and the proportion of patients with NEDA-3 (defined as no relapses, no confirmed disability worsening, and no new or enlarging FLAIR lesions). A total of 193 patients were included in the study. During year 1 of natalizumab treatment, 78.9% of patients had no new or enlarging FLAIR lesions and 79.5% had no new T1 lesions. These proportions increased in years 2–5, with ≥98.0% of patients free of new or enlarging FLAIR lesions and ≥98.8% free of new T1 lesions. During year 1 on natalizumab, 52.2% of patients achieved NEDA-3; this proportion increased to ≥69.2% in years 2–5. This study provides additional evidence that long-term MS disease activity, as measured by both MRI activity and NEDA-3, is well-controlled in patients treated with natalizumab in real-world settings. Abstract

ABSTRACTPediatric medulloblastomas are the most frequently diagnosed embryonal tumors of the central nervous system. Current therapies cause severe neurological and cognitive side effects including secondary malignancies. Cellular immunotherapy might be key to improve survival and to avoid morbidity. Efficient killing of tumor cells using immunotherapy requires to overcome cancer-associated strategies to evade cytotoxic immune responses. Here, we examined the immune response and immune evasion strategies in pediatric medulloblastomas. Cytotoxic T-cells, infiltrating medulloblastomas with variable activation status, showed no correlation with overall survival of the patients. We found limited numbers of PD1+T-cells and complete absence of PD-L1 on medulloblastomas. Medulloblastomas downregulated immune recognition molecules MHC-I and CD1 d. Intriguingly, expression of granzyme inhibitors SERPINB1 and SERPINB4 was acquired in 23% and 50% of the tumors, respectively. Concluding, pediatric medulloblastomas exploit multiple immune evasion strategies to overcome immune surveillance. Absence of PD-L1 expression in medulloblastoma suggest limited or no added value for immunotherapy with PD1/PD-L1 blockers.

40. Mindfulness Training among Individuals with Parkinson’s Disease: Neurobehavioral Effects

Abstract

Objective. To investigate possible neurobehavioral changes secondary to a mindfulness based intervention (MBI) training for individuals living with Parkinson’s disease (PD). Background. In the context of complementary medicine, MBIs are increasingly being used for stress reduction and in patient populations coping with chronic illness. The use of alternative and complementary medicine may be higher in patients with chronic conditions such as PD. However, behavioral effects of mindfulness training in PD have not yet been reported in the literature and this points to an unmet need and warrants further examination. Methods. A total of 27 out of 30 PD patients completed a randomized controlled longitudinal trial. Questionnaires and the UPDRS I–IV were obtained at baseline and 8-week follow-up. Results. Significant changes after the MBI were found including a 5.5 point decrease on the UPDRS motor score, an increase of 0.79 points on Parkinson’s disease questionnaire (PDQ-39) pain item, and a 3.15 point increase in the Five Facet Mindfulness Questionnaire observe facet. Conclusions. To the best of our knowledge, this is the first quantitative analysis of neurobehavioral effects of MBI in PD.
