Objective To demonstrate the major components and molecular mechanism of Shenling Baizhu San in treating primary osteoporosis by using LC-MS and single-cell RNA sequencing analysis. Methods LC-MS technology was used to identify the components of Shenling Baizhu San and its targets were predicted. The transcriptome and scRNA data related to POP were obtained, the differentially expressed genes (DEGs) and cell subpopulations were analyzed, and weighted gene co-expression network analysis (WGCNA) was further performed on DEGs to obtain the hub genes and construct PPI network. Finally, virtual molecular docking was used to verify the components and core targets. Results After LC-MS identification and screening, 10 components and 414 potential targets were obtained. Differential and WGCNA analysis respectively obtained 843 POP-DEGs and 1976 hub targets, and 19 core targets were obtained. Single-cell sequencing analysis found that Macrophage, Monocyte, T cells, B cellexisted in POP sample. The enrichment results showed that the core targets were involved in cellular senescence, osteoclast differentiation, MAPK signaling pathway. Finally, the virtual molecular docking results showed that the identified components could form a stable docking with the POP core targets. Conclusion Shenling Baizhu San can regulate cell senescence, immune function and osteoclastic differentiation through multiple components, multiple targets, multiple cells and multiple pathways to treat POP. In recent years, molecular signaling pathways related to bone metabolism has become a hot topic. Studies have revealed that signaling pathways associated with osteoporosis include Wnt/β-catenin, RANKL/RANK/OPG, NF-κB, PPAR-γ, PTH, MAPK, PI3K/Akt, Hedgehog and Notch signaling pathways. However, there has been no study concentrated on ERα-AMPK-Sirt1 signaling pathway. Estrogen receptor alpha (ERα), AMP-activated protein kinase (AMPK) and Sirtuin 1(Sirt1) can regulate the function of osteoblast or osteoclast by ligand binding, phosphorylation and deacetylation, respectively. Studies have indicated that ERα can directly increase the activity of LKB1, while LKB1 is the most important upstream protein kinase of AMPK, as a result, AMPK is activated. In addition, the mutual promotion of AMPK and Sirt1 can also modulate the autophagy or apoptosis of osteoblast; all these suggested that ERα-AMPK-Sirt1 signaling pathway may play an important role in the pathogenesis of osteoporosis. To sum up, it is necessary to start the research of ERα-AMPK-Sirt1 signaling pathway in order to better guide the development of new drugs and clinical practice. [ABSTRACT FROM AUTHOR]

Objective To evaluate the scientific quality of the patient-reported outcome instrument for primary osteoporosis. Methods We retested 52 primary osteoporosis patients and 92 healthy volunteers to evaluate the difficulty, discrimination, reliability, validity and feasibility of the OPPRO instrument by using the classical test theory and modern item response theory methods. Results The IRT analysis showed that the difficulty and discrimination of the OPPRO instrument are within acceptable parameters. For reliability, the Cronbach's coefficient of the whole instrument is 0. 801, the coefficient of the main symptoms aspect of the instrument is 0. 907, the systemic performance aspect 0. 780, the psychological aspect 0. 849, the social aspect 0. 807, the performance in daily activities aspect 0. 799,and the performance satisfaction aspect 0. 877. All of these results represented a good internal consistency of the instrument. The Pearson correlation coefficient of the retest reliability of the instrument is 0. 925, the Kendall correlation coefficient is 0. 759, both of them suggests a good retest reliability. For validity, the structural validity of the results indicates that the multidimensional structure of the instrument meets to our professional expectations. For the feasibility analysis,the sampling survey of the primary osteoporosis patients showed that the total completion rate of the instrument is 90. 34% and the time required to complete is about 15 minutes. Conclusion By using the classical statistical testing and the item response theory to analyze the items of the patient-reported outcomes of primary osteoporosis, we found that the OPPR instrument has good difficulty, discrimination, reliability, validity and feasibility. The study was conducted strictly in standard steps and took consideration of Chinese culture. The scope of the research, the methodology and the reporting methods are of a high quality, which represents good innovation, scientific quality and as well as clinical significance.
