Abstract

Aims Left ventricular (LV) twist and untwist play a major role in LV mechanics. We sought to acquire new pathophysiological insights in cardiomyopathies (CM) studying LV twist dynamics by speckle tracking imaging (STI). Methods and results Standard echo-Doppler and STI study were performed in 67 CM patients divided in four age- and sex-matched subgroups: 18 with apical hypertrophic cardiomyopathy (Group A); 20 with asymmetrical hypertrophic cardiomyopathy (Group B); 15 with dilated cardiomyopathy (Group C); 14 with LV non-compaction (Group D). As controls, 34 age- and sex-matched normal subjects were studied. Compared with control group, all CM Groups showed significantly lower longitudinal, circumferential, and radial myocardial deformations ( P < 0.05). LV twist was correlated with ejection fraction (EF; r = 0.62; P < 0.0001). Furthermore, all CM patients had a significantly lower twist rate ( P < 0.05) and delayed onset of untwist ( P < 0.01). Of interest a significant correlation was found between isovolumic relaxation time and untwist onset ( r = 0.485, P < 0.0001). In addition, a significant correlation was found between longitudinal deformations and the onset of untwist (strain: r = 0.46, P = 0.0001; strain rate: r = 0.33, P = 0.0056) and between longitudinal strain rate and twisting rate ( r = −0.38; P = 0.0015). Conclusion (i) All CM patients show an impairment of longitudinal, circumferential, and radial myocardial deformations; (ii) LV peak twist is impaired only in CM with reduced EF but preserved in those with normal or increased EF; (iii) LV twist is prolonged and untwisting onset is delayed in all CM, suggesting that a mechanical adaptation to subclinical systolic abnormalities might induce, by a prolonged LV twist, the early onset of diastolic dysfunction.

Abstract

Aims To assess the value of two-dimensional (2D) strain in assessing regional myocardial function along the atrial wall. Methods and results We studied 20 patients late after successful percutaneous atrial septal defect (ASD) closure. The analysis was performed for atrial longitudinal peak systolic strain on the interatrial septum, in correspondence of the device, and on the lateral wall of the left atrium. The speckle tracking indexes demonstrated almost the absence of any deformation on the Amplatzer ASD occluder, a bulky non-contractile element, passively moved by global heart motion. This study in a simple clinical model demonstrates that 2D strain is not influenced by global heart motion and tethering from adjacent segments and can also be used to study the regional atrial function. Moreover, both acquisition and post-processing times of 2D strain were very short, and the reproducibility was very good. Conclusion All these above-mentioned characteristics make the 2D strain a tool fully compatible with the clinical scanning, able to provide additional clinical information.

Abstract

and hemodynamic assessment. Results: Out ol the 38 pts. (mean age: 594-12 years) 24 had CP, and 14 RCM; 20 pts. (53°/°) had sinus rhythm, and 18 (47%) atrial fibrillation. Mean pulmonary wedge pressure (PCW) was 21 4-6 mmHg, E was 1054-39 cm/s, deceleration time of E was 1414-37 ms without a significant difference between RCM and CR Mean pulmonary arterial pressure (PAP) was 284-8 mm Hg with a significant dilference between RCM and CP (334-10 vs. 264-4 mm Hg; p 8 crn/s in 20/24 CP pts (83%). The 4 CP pls. with E'< 8 cngs either showed severe pericardial calcification, systolic dyslunclion, or post-radiation syndrome. Pts. with RCM showed a significantly higher E/E'-Ralio (septal MA: 264-9 vs. 124-8 cm/s; p< 0.001). Conclusion: TDI analysis of MA motion demonstrated decreased systolic and diastolic velocities in pts. with RCM and allows a differentiation between pts. with RCM and CP independent ol cardiac rhythm. The diagnostic value ol TDI analysis in CP can be limited in pts. with severe pericardial calcifications, systolic dyslunction and post-radiation syndrome.

Abstract

Background: The echocardiographic evaluation of myocardial systolic function in pts with hypertrophic cardiomyopathy (HCM) is still unsatisfactory. This disease has been shown to exhibit LV relaxation and filling abnormalities despite normal LV ejection fraction (EF). However, EF only detects radial motion and the normal value of systolic function described in HCM pts may be due to a relative insensitivity of the used technique. Strain (S) and Strain Rate (SR) imaging is a new technique able to quantify both radial and longitudinal regional myocardial deformations and potentially is more sensitive compared to 2D grey scale imaging. Methods: We studied 25 HCM pts (aged 16±3 yrs) using standard grey scale echocardiography and S/SR imaging. Regional peak systolic longitudinal function was assessed from the apical views while regional peak systolic radial function was evaluated from the parasternal views. We studied also the time to peak systolic deformation (from the onset of ECG Q wave to the peak systolic S) for both radial and longitudinal function, in the mid segment of posterior wall from the apical 3 chamber view and the parasternal view, respectively. Data were compared with that of 33 age comparable healthy subjects. Results: All pts showed a normal LVEF. Radial peak systolic S and SR in HCM pts were comparable to healthy subjects. Abstract

Purpose: We developed a transthoracic echo simulator that can measure psychomotor skill in echo to assist in training as well as for certification of competence. The simulator displays cine loops on a computer in response to the user scanning a mannequin with a mock transducer. The skill metric is the deviation angle between the image acquired by the user and the anatomically correct plane for the specified view. We sought to determine whether the simulator-based test could distinguish levels of expertise. Methods: Attendees at an echo course or at the annual meeting of the Swedish Heart Association were invited to take a 15 min test on the simulator. On the test, the user scanned the mannequin and acquired 4 views: parasternal long axis (pLAX) in patient 1, apical 4 chamber (a4c) and aLAX in patient 2, and pLAX in patient 3. Scan time was limited to 15 min. Attendees were asked regarding current work status, position, and experience with echo assessed from duration in years and procedure volume in the past 12 months. Results: Of the 61 participants there were 22 sonographers, 2 nurses, and 37 doctors who were all in practice except 1 doctor who was a resident. The data of nurses was combined with that of sonographers because their procedure volume was nearer to that of sonographers (850 ± 599 tests/yr) than doctors (312 ± 393, p < 0.001). Doctors and non-doctors had similar duration of experience (9 ± 8 vs. 12 ± 11 yrs, p=NS). The test was not completed by 12 participants (18%) but unfamiliarity with the simulator may have contributed because the deviation angle for pLAX dropped between the first and third patients (23 ± 11 to 18 ± 10 degrees, p<0.020). The average deviation angle over the 4 views was slightly lower for sonographers than for doctors (26 ± 11 vs. 30 ± 14 degrees, p=NS). The deviation angle for pLAX (55 ± 37 degrees) was higher than for a4C (17 ± 22 degrees) or either pLAX view (p<0.00001). pLAX was the only view whose deviation angle correlated significantly with experience and only with procedure volume (r=-0.302, p=0.025). Conclusions: The results of this study demonstrate that the skill metric employed, angle of deviation between the plane of an acquired view and the plane of the anatomically correct image for that view, can distinguish the relative experience of sonographers and doctors in practice. Simulation-based testing provides objective and quantitative assessment of the psychomotor skill of image acquisition and may be of value in certification of trainees and in maintenance of certification examination of practicing sonographers and doctors.

Abstract

Aims To assess the value of two-dimensional (2D) strain in assessing regional myocardial function along the atrial wall. Methods and results We studied 20 patients late after successful percutaneous atrial septal defect (ASD) closure. The analysis was performed for atrial longitudinal peak systolic strain on the interatrial septum, in correspondence of the device, and on the lateral wall of the left atrium. The speckle tracking indexes demonstrated almost the absence of any deformation on the Amplatzer ASD occluder, a bulky non-contractile element, passively moved by global heart motion. This study in a simple clinical model demonstrates that 2D strain is not influenced by global heart motion and tethering from adjacent segments and can also be used to study the regional atrial function. Moreover, both acquisition and post-processing times of 2D strain were very short, and the reproducibility was very good. Conclusion All these above-mentioned characteristics make the 2D strain a tool fully compatible with the clinical scanning, able to provide additional clinical information. [ABSTRACT FROM AUTHOR]

Abstract

Takotsubo cardiomyopathy (TTC) is a distinct clinical entity characterized by the presence of transient left ventricular wall dysfunction without significant culprit obstructive coronary artery disease. Invasive coronary angiography and ventriculography are the ‘gold standard’ for definitive diagnosis, with an integrated multi-modality imaging approach offering advantages in various clinical scenarios. Echocardiography is a widely available, first-line, non-invasive imaging technique appropriate both in emergency setting to confirm diagnosis, assess for various potential acute complications, and in serial follow-up to track myocardial recovery. Cardiac magnetic resonance (CMR) may be helpful to discriminate TTC from other acute cardiac syndromes with troponin elevation and ventricular dysfunction. Echocardiography, CMR, and nuclear imaging may also provide new insights into possible underlying pathophysiological mechanisms, and myocardial 123I-metaiodobenzyl-guanidine imaging may have a role for retrospective diagnosis in the subacute phase of late-presenting cases. The potential diagnostic role of coronary computed tomography angiography in the emergency room requires a further study.
