An 82-year-old woman experienced profound hypokalaemia while being treated with mifepristone for hypercortisolism and chlortalidone for leg swelling. She was initially referred to endocrinology for bilateral adrenal adenomas and started on medical therapy, including mifepristone and later chlortalidone. After developing symptoms and presenting to the emergency department, her potassium levels were found to be critically low. Treatment involved discontinuing mifepristone, starting potassium supplementation, and stabilizing her potassium levels with spironolactone, along with receiving ketoconazole for hypercortisolism.
