Objective: To explore the effect of disposable endodontic treatment combined with crown repair and its effect on chewing function. Methods: 80 patients with cryptic cleft dental pulp disease admitted to our hospital from January 2018 to January 2023 were selected and divided into observation group and Matched group, with 40 patients in each group. The Matched group patients underwent crown restoration after multiple root canal treatments, while the observation group patients underwent crown restoration after a single root canal treatment. After 3 months of treatment, the clinical efficacy of the two groups of patients was compared, and record all patients treatment success rates. The chewing function, pain level, inflammatory factors in gingival crevicular fluid, and expression levels of alkaline phosphatase (ALP) were compared between the two groups of patients before and after 3 months of treatment. Finally, their plaque index (PLI) was compared, Changes in periodontal health indicators related to bleeding index (BI) and probing depth of gingival sulcus (PD). Results: The total effective rate and treatment success rate of the observation group were higher than those of the matched group (P<0.05); Post-treatment, the chewing function score of the two groups of patients increased, and the observation group was higher than the matched group. The pain level score decreased, and the observation group was lower than the matched group (P<0.05); Post-treatment, the levels of PD, BI, and PLI in both groups of patients decreased, and the observation group was lower than the matched group (P<0.05); Post-treatment, the two patients had lower levels of IL-6, CRP, TNF-α, and ALP, while the observation group was lower than the matched group(P<0.05). Conclusion: The combination of one-time root canal therapy and crown restoration has a significant therapeutic effect on cracked pulp disease, and can improve the long-term success rate of treatment, enhance chewing function, alleviate periodontal pain, reduce periodontal inflammation, alleviate disease severity, and improve periodontal health. BACKGROUND: Bacterial infection is an important cause of infection of the dental pulp and periapical tissue. Vital pulp conservation and regeneration of the dental pulp are becoming new clinical strategies for the treatment of pulp and periapical diseases. Resolvin is a lipid mediator derived from omega-3 polyunsaturated fatty acids. It promotes the resolution of tissue inflammation and the recovery of inflammatory conditions. OBJECTIVE: To review the research progress in the effect of resolvin in inflammation regulation and its application in the inflammation of dental pulp, periapical and periodontal tissues, providing evidence for inflammatory recovery and regeneration of the dental pulp. METHODS: PubMed, VIP, WanFang and CNKI were searched for relevant literatures published from January 2014 to June 2020 using the keywords of “resolvin, effect, signaling pathway” in Chinese and English, respectively. Thirty-five articles regarding resolvin and its application were included and analyzed. These databases were also searched using the keywords of “resolving, pulptitis, periapical, periodontitis, dental” in Chinese and English, and 31 articles were included for analyzing the application of resolving in different tissues and diseases, and another 3 articles were further explained of the literature content. RESULTS AND CONCLUSION: In this article, we systematically review the types and effects of resolvin, and detail the role and mechanism of resolvin in inflammatory diseases of the dental pulp, periapical and periodontal tissues. BACKGROUND: Since the application of transcriptome sequencing technology has made remarkable achievements in the research of melanoma and breast cancer, transcriptome sequencing technology has become a hot research method in scientific research and widely used in the field of stomatology. OBJECTIVE: To review the development of transcriptome sequencing technology and its application in various disciplines of stomatology through searching, screening and reading literature. METHODS: A search of CNKI, CBM and PubMed was performed for relevant literature published from 2015 through 2020. The search terms were “transcriptome, sequencing technology, RNA-seq, microarray, oral cancers, OSCC, periodontal, caries, pulp disease, tooth development, DPSCs, PDLSCs, orthodontics, implant” in English and Chinese, respectively. According to the inclusion and exclusion criteria, 72 literatures were included to review the development of transcriptome sequencing technology and its application in the field of stomatology. RESULTS AND CONCLUSION: In the development of transcriptome sequencing technology, RNA-seq technology has the greatest advantage in scientific research, because of its high accuracy, high throughput and low price. In the field of stomatology, this technology has been used in the research of oral squamous cell carcinoma, periodontitis, dental pulp disease, tooth regeneration, orthodontics and dental implantation, and has achieved some achievements. However, the current research on the same kind of disease does not reflect the relationship between the various studies, and the research content is limited. Objective: To investigate the effects of single and repeated root canal dredging and filling on the pain degree, masticatory function and inflammatory factors of patients with dental pulp disease. Methods: A total of 119 patients with dental pulp disease who were admitted to our hospital from February 2017 to May 2019 were selected, they were randomly divided into group A (n=59, repeated root canal dredging and filling treatment) and group B (n=60, single root canal dredging and filling treatment). The pain degree, total effective rate, masticatory function and inflammatory factor level of the two groups were compared, and the complications of the two groups were recorded. Results: The total clinical effective rate of group B was 93.33% (56/60), which was higher than that of group A 74.58% (44/59) (P<0.05). 2 weeks after treatment, the biting force and chewing efficiency of the two groups increased, and those of group B were higher than those of group A (P<0.05). 2 weeks after treatment, the gingival index and visual pain simulation score (VAS) of the two groups decreased, and those of group B were lower than those of group A (P<0.05). The serum levels of interleukin -6 (IL-6), tumor necrosis factor-α(TNF-α), C-reactive protein (CRP) of the two groups decreased at 2 weeks after treatment, and the levels of group B were lower than those of group A (P<0.05). The incidence of postoperative complications of group B was lower than that of group A (P<0.05). Conclusion: Single root canal dredging and filling can effectively improve the clinical symptoms of patients with dental pulp disease, with definite curative effect. Objective To investigate the pulp prognosis and influence factors of dentin-fractured teeth. Methods Case records of dentin-fractured teeth treated with indirect pulp capping and with more than 2 years follow-up were collected. The data related to trauma information and pulp prognosis were recorded and the influence factors were analyzed by Logistic regression. Results This study consisted of 205 cases with 242 dentin-fractured teeth. Among 242 teeth, pulp necrosis occurred in 78 teeth (32.2%) and pulp calcification occurred in 3 teeth (1.2%). The success rate of indirect pulp-capping on dentin-fractured teeth was 67.8%. During observation period, 141 out of 177 young permanent teeth completed rout development. The extent of fracture close to pulp was an indicator for pulp prognosis (P=0.001). Conclusion The prognosis of indirect pulp-capping on dentin-fractured permanent teeth is good. Successful endodontic treatment depends on the effectiveness of the cleaning and shaping of the root canal system. Due to the complexity of the structure of root canal system and the particularity of root canal infections, the equipment and technology currently available for root canal preparation hinder the ability to adequately clean and shape the root canal system. PURPOSE: To investigate the relationship between diabetes mellitus and pulposis and the pathogenesis of diabetes mellitus pulposis, via comparing the pulp tissue conditions and ultramicrostructures of capillary vessels in pulp tissues of diabetes mellitus patients and normal control. METHODS: Pulp tissues were obtained from clinically extracted human third molars with impaction (control group, 30 cases) and intact third molars with impaction (non-insulin . dependent diabetes patients) whilst without pulposis symptom (diabetic group, 30 cases). The pathological changes of two groups' pulp tissues were evaluated under light microscope and electron microscopy. The thickness of pulp capillary basement membrane and their anionic sites were quantitatively analyzed by electron microscopy and special staining. Statistical analysis was performed using SAS6.12 software package. RESULTS: The tricked blood capillary wall was observed in diabetic pulp under light microscope. The thickened blood capillary wall and constrictive lumen were observed in diabetic pulp under electron microscope, accompanied with vascular endothelial cell swelling and structural destruction and thickened basement membrane (P<0.01). The decrease of anionic sites of capillary basement membrane was observed in diabetic pulp under specially stained electron microscope (P<0.01). CONCLUSIONS: Inflammatory reaction existed in the pulp of intact teeth of DM patients without symptoms. The changes observed in diabetic vessels may occur in the dental pulp. Bioactive materials is defined as substances that elicit special chemical bonding at the interface between tissues and the material. In recent years, more and more bioactive materials have emerged in endodontic treatment. The present article introduces the definition, classification and application of bioactive materials in endodontics. Combined with evidence-based medicine and bioactive materials, optimum treatment recommendations for various endodontic diseases are provided and the development trend of bioactive materials in endodontics is also discussed.

Objective To comparatively analyze the clinical efficacy of conservative treatment for vital teeth after tooth fractures and dead dental pulp. Methods Vertically fractured teeth were divided vital teeth group and dead dental pulp group, and were given direct intraoral bonding, and complete coronal restoration were given four weeks later. Six months and twelve months after treatment, patients were reviewed, and graded according to clinical manifestations and periodontal status, and the data were statistically analyzed. Results The results of six minths follow-up showed that the success rate in half a year had no significantly difference between two methods ( P > 0.05) . The results of one year follow-up showed that the success rate in one year was higher in vital teeth group than that in dead dental pulp group (P< 0.05) . Caries-derived dental pulp diseases are characterized with high incidence and serious endangerment. Considering the complexity of the pulpal infection and the limitation of the pulp self-repairing capability, it is still an urgent problem that how to eradicate infection and to promote tissue regeneration subsequently for dental clinicians. This review discusses and prospects on the pathogenesis, diagnosis and treatment strategies of caries-derived dental pulp diseases, so as to provid a reference on diagnosis and treatment of such diseases for the clinicians.

A case of unusual root canal morphology was presented to demonstrate anatomic variations in mandibular second molars. This case report presented the endodontic therapy of a mandibular second molar with three canals in the mesial root. A mesial middle canal was found between the mesiobuccal and mesiolingual canals. This report indicated the importance of looking for additional canals and unusual canal morphology, because knowledge of their existence might occasionally enable clinicians to treat a case successfully that otherwise to fail. The development of an expert consensus on vital pulp therapy can provide practical guidance for the improvement of pulp damage care in China. Dental pulp disease is a major type of illness that adversely affects human oral health. Pulp capping and pulpotomy are currently the main methods for vital pulp therapy. Along with the development of minimal invasion cosmetic dentistry, using different treatment technologies and materials reasonably, preserving healthy tooth tissue, and extending tooth save time have become urgent problems that call for immediate solution in dental clinics. This paper summarizes the experiences and knowledge of endodontic experts. We develop a clinical path of vital pulp therapy for clinical work by utilizing the nature, approach, and degree of pulp damage as references, defense and self-repairing ability of pulp as guidance, and modern technologies of diagnosis and treatment as means.

Dental pulp tissue plays a role in forming dentin, providing nutrition, conducting pain, and generating protective responses to environmental stimuli. Bacterial infection is the main cause of pulp disease, where histopathological changes are the histological basis for determining the choice of treatment and the evaluation of therapeutic effect. Thus, particular attention should be given to eliminate infection, as well as preserve and maintain pulpal health in teeth that show reversible or limited pulpal injuries. Vital pulp therapy, especially its indications and prognostic factors, has been a research hotspot that often causes confusion among clinicians. In this paper, we briefly introduce the confusion and solution for vital pulp therapy in terms of indications, pulp condition assessment, infection elimination, and capping material selection. In addition, we develop a clinical pathway and an operation normalization of vital pulp therapy to better perform the therapy.

Combined the elaboration on periodontic-endodontic lesion in the textbook Periodontics with the deficiencies existed in the clinical and teaching work and demonstrated the understanding on the type of the combined periodontic-endodontic lesion, and suggested the viewpoint of no sub-type of combined periodontic-endodontic lesion. Only regard the type of pulp disease that induced by periodontal disease as genuine combined periodontic-endodontic lesion.
