BACKGROUND Although the evidence is expanding, in mental health care shared decision making (sdm) is not widely applied. Moreover, little is known about the use of routine outcome monitoring (rom) and eHealth in sdm. AIM PhD research on the added value of sdm using rom and eHealth for patients and clinicians in mental health care. METHOD Three studies: 1. a literature research and a cross-sectional study on decisional conflict; 2. rom implementation research and a cluster randomised trial on shared decision making using rom (Breakthrough program); 3. a cluster randomised trial on shared decision making during the intake (regional). RESULTS The interventions did not lead to reduced decisional conflict for all patient groups. Decisional conflict gives insight into the patient's perspective on the quality of the decision making process and decisions being made. Only patients with depression, who participated in the national trial, reported less decisional conflict. This trial did not show a higher level of sdm, but did show increased usage of rom in clinical practice. Although the regional trial showed no results on decisional conflict, the application of sdm and treatment outcomes improved. CONCLUSION sdm in mental health care needs further improvement. We recommend investigating how to support patients better, taking into account the role that suits them.

Doel Zorggebruik onderzoeken bij patiënten met een depressie, en nagaan welke demografische kenmerken en ernstkenmerkenpatiënten die vroegtijdig gebruikmaken van zorg, patiënten die afwachten met zorggebruik en patiënten die geen gebruikmakenvan zorg van elkaar onderscheiden. Opzet Longitudinaal, observationeel en prospectief onderzoek.MethodeWe gebruikten gegevens van de ‘Netherlands mental health survey and incidence study-2’ (NEMESIS-2). Patiënten die eendepressie rapporteerden in de 12 maanden voorafgaand aan de ‘baseline’-meting, werden geïncludeerd. Hun zorggebruik indiezelfde periode werd gerapporteerd. Na 3 jaar keken we opnieuw naar hun zorggebruik. Om verschillen te onderzoeken tussenvroege zorggebruikers (in het jaar dat ze een depressie hadden gebruikten ze ook zorg), afwachtende en niet-zorggebruikersgebruikten we een multinomiale logistische regressieanalyse. Resultaten Meer dan de helft van de respondenten was een vroege zorggebruiker (62%). Vroege zorggebruikers hadden ernstigere enpersisterende depressieve symptomen en vaker geen partner dan niet-zorggebruikers (dat wil zeggen: noch zorg in jaar waarindepressie begon, noch in 3 jaar daarna). De meerderheid van de niet-zorggebruikers (89%) was hersteld na 3 jaar. Afwachtendezorggebruikers (geen zorg in jaar waarin depressie begon, wel in 3 jaar daarna) hadden relatief geringe klachten en rapporteerdenvaker een nieuwe of aanhoudende depressieve episode na 3 jaar dan vroege zorggebruikers. Conclusie Zorggebruik door patiënten met een depressie lijkt vooral samen te hangen met ernstkenmerken en niet zozeer met demografischekenmerken zoals leeftijd, opleidingsniveau of geslacht.

Personalised medicine wordt beschouwd als één van de grote beloftes voor de geneeskunde (FIGUUR 1; Nationale Wetenschapsagenda 2016). De gedachte is dat geneeskundige interventies (preventie, cure en care) niet effectief zijn bij een kwart tot twee derde van de patiënten. Dit geldt ook voor behandelingen die als effectief te boek staan omdat ze in grote klinische trials, gemiddeld genomen, effect hebben.

background In clinical practice, psychiatrists experience difficulties in treating depression and anxiety in patients with a non-Western cultural background. Is this the result of differences in symptom profile, or are different risk factors involved? aim To present a review of the findings in population studies on the prevalence, symptoms, risk factors and protective factors for depressive disorders and anxiety disorders in Turkish and Moroccan minorities, compared to the Dutch majority population. METHOD A systematic review of the literature. results Eighteen publications were selected, based on nine datasets.The prevalence of depressive disorders and anxiety disorders was increased in the Turkish minority population, compared to the Dutch native population.The symptom profile was highly comparable between minority and majority groups, as well as association of the risk factors neuroticism and physical impairments with depression. Discrimination was a risk factor, and participation in the Dutch society a protective factor in the minority groups. In all ethnic groups, active coping and positive religious coping were associated with less depressive symptoms. conclusion The increased prevalence of depressive and anxiety disorders in Turkish-Dutch patients calls for new preventive and treatment initiatives in this population.These initiatives need to address subgroup- specific risk factors, in particular discrimination and lack of social participation, as well as the common risk factors that apply to all population groups.

Objective To explore the relationship between psychiatric care consumption after a compulsory admission and the probability of a repeat compulsory admission. Design Prospective cohort study with a 5year followup. Method Mental health care consumption was registered for 460 patients admitted compulsorily under the Dutch Psychiatric Care (Compulsory Admissions) Act by the Amsterdam Emergency Psychiatry service between 15 September 2004 and 14 September 2006, with sociodemographic and clinical data as independent variables. Results There was no repeat compulsory admission in 63% of the cohort. The odds ratio for repeat compulsory admission only decreased in the fourth year (odds ratio (OR) 0.64; 95% CI 0.450.92). Repeat compulsory admission was associated with high treatment continuity (χ2 p ≤ 0.001) and a high level of care consumption during the followup period (χ2 p ≤ 0.001). Compulsory admission was predicted on the basis of: high care consumption in the five years prior to inclusion (OR 2.61; 1.444.73), aged younger than 35 years at outset (OR 1.65; 1.082.52), living alone at the time of inclusion (OR 1.68; 1.222.33), and a history of compulsory admission (OR 1.56; 1.032.35). Conclusion Twothirds of the patients were not readmitted compulsorily. The probability of a repeat admission of this kind fell only after four years. Patients who underwent a repeat compulsory admission proved to be the ones who had been treated most intensively, yet this intense treatment did not prevent a higher probability of readmission. When not taking the quality and nature of care and social integration of patients into account, attempts to reduce treatment dropout on the one hand and more intense treatments on the other may not have an effect on reducing the probability of a repeat compulsory admission.

BACKGROUND: There is increasing attention to the costs and efficacy of health care and in particular to psychiatry. To weigh the costs and benefits appropriately, it is important to be able to compare effects. AIM: To describe the efficacy of psychiatric treatment compared to treatment in general medicine. METHOD: Narrative review. RESULTS: The average effect size of psychiatric treatment was 0.49 and that of general medicine treatment 0.45. The effect size of psychotherapy varied from 0.3 - 0.84, but in a meta-analysis, corrected for publication bias, the effect size average amounted to only 0.39. CONCLUSION: The average efficacy of psychiatric treatments is similar to the efficacy of treatments in general medicine. However, there is a large variation in efficacy within the two treatment areas. The variation is even larger in the treatment results for individual patients. The main objective of 'personalised medicine' is to tailor the type of treatment to the particular symptoms of the individual patient so that he or she will not be exposed to types of treatment that are not strictly relevant to the patient's symptoms or illness. In view of the limited and less-than-perfect results of treatment and medication in the entire medical field, 'personalised medicine' seems to be a goal worth striving for in order to improve treatment outcomes.

De vergrijzing van onze bevolking maakt dat er een grotere groep ouderen met schizofrenie ontstaat. Dat deze mensen specifieke zorgbehoeften hebben lijdt geen twijfel. Het Amsterdamse soul-onderzoek zocht uit welke behoeften dit zijn en hoe je hierin kunt voorzien

Achtergrond Bewoners van verzorgingshuizen vormen een risicogroep voor het ontwikkelen van een depressieve en/of angststoornis. Deze stoornissen komen veel voor en hebben een grote impact op het welzijn en functioneren van de betrokkene, maar worden vaak slecht herkend. Wij vroegen ons af of een stepped-care programma ter preventie van depressieve en angststoornissen in verzorgingshuizen haalbaar en effectief zou kunnen zijn. Methoden In een pragmatische gerandomiseerde gecontroleerde trial vergeleken we de effecten van het programma met gebruikelijke zorg in veertien verzorgingshuizen. In totaal deden 185 bewoners mee, die op de Centre for Epidemiologic Studies Depression Scale (CES-D) minstens 8 punten scoorden, niet leden aan een depressieve of angststoornis en ook niet aan een ernstige cognitieve stoornis. De deelnemers kregen een stepped-care preventieprogramma (n = 93) of gebruikelijke behandeling (n = 92). De deelnemers aan het preventieprogramma kregen eerst een afwachtend beleid en als ze niet opknapten achtereenvolgens een zelfhulpinterventie, een psychologische life-review en een verwijzing naar de huisarts. Onze primaire uitkomstmaat was de incidentie van een depressieve stoornis of angststoornis gedurende het jaar na inclusie. Resultaten De incidentie van depressie en angst samen nam niet af door de interventie: de gecombineerde incidence rate ratio (IRR) was 0,50 (95%-betrouwbaarheidsinterval (BI) 0,23 tot 1,12). Ten opzichte van de gebruikelijke zorg bracht het preventieprogramma wel het indicentierisico voor depressie omlaag (IRR 0,26; 95%-BI 0,12 tot 0,80) maar niet dat voor angst (IRR 1,32; 95%-BI 0,48 tot 3,62). Conclusie De resultaten suggereren dat het toegepaste steppedcare preventieprogramma bij ouderen in verzorgingshuizen wel helpt tegen depressie, maar niet tegen angst.

Depression and pain are common disorders among nursing home residents. Residents with depression report more pain than residents without depression and residents with pain more frequently suffer from depression than residents without pain. More severe depression is associated with more pain and more severe pain is associated with more depression. Presence of pain hinders the recognition of depression and negatively effects treatment of depression. Presence of depression in turn negatively effects the treatment of pain. The high prevalence of pain, of depression and of the unhappy marriage of pain and depression among nursing home residents, asks for a systematical screening of these residents on depression and pain.

Psychische gezondheid is essentieel voor het welbevinden en functioneren van mensen. In verschillende grote steden in de wereld is men zich gaan realiseren dat de mentale gezondheid van haar inwoners meer vraagt dan goed toegankelijke behandelprogramma’s in de ggz. Onder de naam Thrive hebben besturen van steden als New York, Londen en ook Amsterdam daarom een beweging op gang gebracht ter bevordering van de mentale gezondheid.
