This thesis describes the oral health of community-dwelling older people who visit a dental practice as seen from the perspective of both the older person and the dentist. In addition, the oral health care provided by dentists and the barriers they may perceive are described. The older people who still visit the dentist are relatively healthy, well-educated, and dentate. As they grow older, they face increasing health problems, increased medication use and oral health issues. The age of 75 seems to be a tipping point. Dentists are mainly providing curative care, focusing on preserving the dentition. In the assessment of oral health, older people appeared to assess their own oral health more positively than the dentist. The barriers in the provision of oral health care were mainly related to health problems, misunderstood behaviour, and poor daily oral hygiene by the older people. Earlier identified barriers appeared to be related to knowledge, practice conditions and financial reimbursement for the work.In het proefschrift wordt de mondgezondheid beschreven van thuiswonende ouderen die de tandartspraktijk bezoeken. Zowel gezien vanuit het perspectief van de oudere als van de tandarts. Daarnaast wordt de door tandartsen verleende mondzorg en de barrières die zij daarbij kunnen ervaren in kaart gebracht. Vooral relatief gezonde, hoger opgeleide ouderen met een eigen dentitie bezoeken de tandarts. Naarmate deze ouder worden, krijgen zij te maken met toenemende gezondheidsproblemen, meer medicijngebruik en problemen met de mondgezondheid. Uit het onderzoek bleek dat de leeftijd van 75 jaar een omslagpunt lijkt te zijn voor de mondgezondheid. De mondgezondheid werd door de oudere zelf positiever beoordeeld dan door de tandarts. De mondzorgverlening door de tandarts was vooral gericht op curatie en behoud van de dentitie. De barrières in de mondzorgverlening door tandartsen hingen vooral samen met gezondheidsproblemen, onbegrepen gedrag en slechte dagelijkse mondverzorging van de oudere. Uit eerder onderzoek bleek dat barrières bij tandartsen samenhangen met kennis, praktijkomstandigheden en de financiële vergoeding van het werk.

Apicale wortelresorptie is een biologisch proces dat op gang komt wanneer een orthodontische kracht op een gebitselement wordt uitgeoefend en lokale necrose van het parodontale ligament optreedt. Macrofagen zorgen ervoor dat het necrotische weefsel wordt verwijderd. Hierdoor kunnen differentiërende osteoclasten en aan het vrijgekomen tandoppervlak hechten en dan wortelresorptie veroorzaken. Er bestaat veel onzekerheid onder mondzorgverleners hoe om te gaan met klinisch relevante apicale wortelresorptie (≥ 2 mm) tijdens of na een orthodontische behandeling. Om meer inzicht te krijgen en de kwaliteit van zorg te verbeteren werd vanuit de Nederlandse Vereniging van Orthodontisten een klinische richtlijn ontwikkeld. Er zijn aanbevelingen geformuleerd voor het diagnosticeren van apicale wortelresorptie, mogelijke risicofactoren en het behandelbeleid om in de praktijk adequaat op dit probleem te kunnen reageren. Macrophages remove the necrotic tissue. In this way, differentiating osteoclasts can both attach to the now available dental surface and can then provoke root resorption. There is considerable uncertainty among dental practitioners on how to deal with clinically relevant apical root resorption (≥2 mm) during or after orthodontic treatment. To increase understanding and to improve the quality of care, the Dutch Association of Orthodontists has developed a clinical practice guideline. Recommendations have been formulated for the diagnosis of apical root resorption, possible risk factors and treatment management in order to respond adequately to this problem in practice.

In this questionnaire study, the attitude of dentists and students regarding the provision of oral care to palliative patients was investigated. The extent to which they would like to be involved in the care for this patient group was also investigated. The results showed that both research groups had relatively little affinity with palliative patients. In general, however, they do consider oral care to be important for this group and believe a dentist can play a role in the quality of life. About one third of both research groups, nevertheless, preferred not to be involved in the provision of oral care often. Besides, when it comes to providing oral care in nursing homes or at patients' homes, approximately one third of the respondents were not very willing or not willing at all to make house calls. Dentists and students are aware of the importance of oral care in the palliative stage of life, but they do not (yet) want to be responsible for the oral care themselves.

Dentists and dental specialists are qualified to prescribe drugs. In this study, we assessed and compared the pharmacotherapeutic knowledge and skills of final year dental students, dentists and dental specialists in the Netherlands. In 2017, a random sample of these three groups was invited to complete an assessment. The knowledge assessment comprised 40 multiple choice questions covering often prescribed drugs. The skills assessment comprised three patient cases for which participants had to write a treatment plan. For the knowledge assessment, the response rates were 26 (20%) dental students, 28 (8%) dentists and 19 (19%) dental specialists, and for the skills assessment the response rates were 14 (11%) dental students, eight (2%) dentists, and eight (8%) dental specialists. On average, all three groups had inadequate knowledge scores (smaller 80%) and only a small proportion (smaller 30%) of their treatment plans was assessed as correct. These results suggest that dental students, dentists and dental specialists lack prescribing competence, which could be caused by poor pharmacotherapy education during under- and postgraduate dental training.

Worldwide and also in the Netherlands there is an increasing human papillomavirus (HPV)-attributable oropharyngeal cancer incidence. This type of cancer is more common at a younger age than the classical oropharyngeal cancer, which is usually caused by smoking and excessive drinking. Most often, the tumor occurs in the tonsils and at the base of the tongue. This study explored the level of knowledge of dentists about oropharyngeal cancer, its association with the human papillomavirus and their willingness to play a role in prevention. 7.364 digital questionnaires were sent to oral health-care providers and data from 607 dentists was analyzed. 48% of knowledge questions were answered correctly by more than half of the respondents. Significantly more female caregivers were aware of the relationship between HPV and oropharyngeal cancer and the availability of an HPV vaccine. Respondents considered it important that the relationship between HPV and oropharyngeal cancer is discussed with patients and that a protocol is developed that is useful in screening for mouth and throat cancer.Wereldwijd en ook in Nederland is er een toename van de incidentie van humaan papillomavirus geassocieerde orofarynxkanker. Deze kanker komt op jongere leeftijd vaker voor dan de klassieke orofarynxkanker die meestal wordt veroorzaakt door roken en overmatig alcoholgebruik. De tumor komt vooral voor in de tonsillen en de tongbasis. In dit onderzoek is gekeken naar de kennis van tandartsen over orofarynxkanker, de associatie ervan met het humaan papillomavirus en de bereidheid om te adviseren over preventieve maatregelen. Er werden 7.364 digitale vragenlijsten naar mondzorgverleners verstuurd en data van 607 tandartsen werden geanalyseerd. Van de kennisvragen werd 48% door meer dan de helft van de respondenten correct beantwoord. Significant meer vrouwen waren op de hoogte van de associatie tussen het virus en orofarynxkanker en de beschikbaarheid van een vaccin. Respondenten vonden het belangrijk dat de relatie tussen humaan papillomavirus en orofarynxkanker met patiënten wordt besproken en wensten een protocol dat bruikbaar is bij de screening op mondholte- en keelkanker.

Coinciding with an increasing tendency of professionals to cross national borders, there is also an increasing need to harmonise curricula. In dentistry in Europe, this tendency is undiminished. By defining international curriculum requirements for dentists, international harmonisation can be strengthened. In the Netherlands, too, harmonisation with European standards has taken place in establishing final achievement levels for dentistry curricula, as defined in a national framework. In a working environment where oral health professionals are confronted with rapid change, for example, in the division of responsibilities, future caregivers should have an optimal educational preparation for their competences, skills and expertise.Tegelijk met een toenemende mobiliteit van beroepsbeoefenaren over de landsgrenzen, ontstaat een groeiende behoefte aan harmonisering van opleidingen. Binnen de tandheelkunde is dit in Europa onverminderd het geval. Door het formuleren van internationale kaders voor opleidingseisen voor tandartsen wordt de onderlinge afstemming versterkt. Ook in Nederland vindt afstemming plaats met de Europese kaders bij het vaststellen van eindtermen voor de tandheelkundeopleidingen, zoals vastgelegd in een Raamplan. Terwijl de beroepsbeoefenaren in de Nederlandse mondzorg volop in beweging zijn, denk aan taakherschikking, dienen de opleidingen de toekomstige zorgverleners op de best mogelijke wijze voor te bereiden op hun bevoegdheden, bekwaamheid en deskundigheid.

Om het niveau van farmacotherapeutische kennis en vaardigheden van laatstejaars tandheelkundestudenten en tandartsen(-specialisten) in Nederland te onderzoeken en te vergelijken werd in 2017 een steekproef van deze 3 groepen uitgenodigd om een kennistoets en een vaardighedentoets te maken. De kennistoets bestond uit 40 meerkeuzevragen over veel voorgeschreven geneesmiddelen. De vaardighedentoets bestond uit 3 patiëntcasussen waarvoor een behandelplan gemaakt moest worden. In totaal maakten 26 (20%) tandheelkundestudenten, 28 (8%) tandartsen en 19 (19%) tandarts-specialisten de kennistoets en rondden 14 (11%) tandheelkundestudenten, 8 (2%) tandartsen en 8 (8%) tandarts-specialisten de vaardighedentoets af. Gemiddeld hadden de 3 groepen een onvoldoende kennisscore (kleiner 80%) en werd slechts een klein deel (kleiner 30%) van hun behandelplannen als correct beoordeeld. Deze resultaten suggereren dat tandheelkundestudenten en tandartsen(-specialisten) onvoldoende in staat zijn adequaat medicamenten voor te schrijven. Een verklaring hiervoor zou onvoldoende farmacotherapie-onderwijs tijdens de tandheelkundeopleiding en nascholing kunnen zijn.

The realisation that oral care for elder patients is very desirable and also necessary is only a few decades old. The earliest research was carried out in 1978. In the Netherlands and Flanders, research on oral care for the elderly began a few years later: the first doctoral degree in the field of gerodontology was awarded at the University of Nijmegen in 1989, after which more followed. In 2005, the BENECOMO was established, a Belgian-Dutch consortium in which geriatric dentists and specialists in healthcare for the elderly cooperate closely in research. A consequence of the cooperation has been, among other things, that gerodontology is now a broad professional field, which takes into consideration the importance of biomedical, psychological and social factors and a multidisciplinary approach, aiming at prevention.Het besef dat mondzorg voor de oudere patiënt zeer gewenst en bovendien noodzakelijk is, is pas enkele decennia oud. Het eerste onderzoek werd gedaan in 1978. In Nederland en Vlaanderen kwam het onderzoek naar mondzorg voor ouderen enkele jaren later van de grond: aan de universiteit van Nijmegen vond de eerste promotie op het vakgebied van de gerodontologie plaats in 1989, waarna er meer volgden. In 2005 werd het BENECOMO opgericht, een Belgisch-Nederlands consortium waarin geriatrisch-tandartsen en specialisten ouderengeneeskunde nauw samenwerken in onderzoeken. De samenwerking heeft onder andere ertoe geleid dat gerodontologie nu een breed vakgebied is dat rekening houdt met een toenemend belang van biomedische, psychologische en sociale factoren en een multidisciplinaire aanpak met aandacht voor preventie.

The Netherlands, like other Western countries, shows an obvious demography of aging, which is associated with many challenges. People age differently, after all. Some remain vital until very old age, while others become frail and disabled much earlier in life. Because of the indicated demographic trend, morbidity is expected to increase and with it, the number of older people needing care. 'Aging in place' has become a central phenomenon in healthcare policies. This means that older people should be empowered to lead a meaningful life in their own living environment as long as possible, even when they are frail and care dependent. Therefore, in the future, most care for older people will actually occur at home. This article aims to present a meaningful care model for the older people. Starting from the 'definition discussion' about frailty and a revaluation of the concept of' resilience, a basis will be provided for a sustainable, proactive and personalised elderly care, close to the living environment of older people, in which dentists and other oral health professionals play an important role as well.Mensen verouderen verschillend. Waar de ene persoon tot op hoge leeftijd vitaal blijft, wordt een andere al veel vroeger in het leven getroffen door gebreken. In het ouderenzorgbeleid is aging in place een basisfenomeen geworden. Dit betekent dat ouderen zolang mogelijk in staat gesteld moeten worden om een betekenisvol leven te leiden in hun eigen woonomgeving, ook als ze kwetsbaar en hulpbehoevend zijn. Hierdoor zal de meeste zorg voor ouderen in de toekomst daadwerkelijk in de eigen woonomgeving plaatsvinden. Vanuit het fenomeen ‘kwetsbaarheid’ (frailty) en het concept van de ‘veerkracht’ (resilience) wordt een basis gelegd voor een duurzame, proactieve en persoonsgerichte ouderenzorg, dichtbij de leefomgeving van de oudere zelf en waarin ook de tandarts en andere mondzorgverleners een belangrijke rol spelen.

The interuniversity Progress Test in Medicine is a measuring instrument consisting of about 200 test items, to measure the development of medical students' knowledge during their studies. The Progress Test consists of 4 tests per academic year. Future versions of the Progress Test will likely be based on computerised administration of the test. One of the main goals of the Progress Test is to counter the so-called 'learningto-the-test' effect. Additional benefits are that students and teachers get detailed feedback about command of knowledge and where omissions in knowledge or the curriculum exist at multiple institutions. A Progress Test for dental education in the Netherlands can easily be conceived because the goals of the Progress Test fit well with the common framework (Raamplan Tandheelkunde) shared among the current three dental programmes. The current 'Overalltoets' (general test) of ACTA (academic centre for dentistry Amsterdam) could form a foundation for starting a Progress Test for dental education.De interuniversitaire VoortgangsToets Geneeskunde (iVTG) is een meetinstrument dat bestaat uit ongeveer 200 vragen om de kennisontwikkeling van studenten Geneeskunde tijdens hun studie te meten. De iVTG bestaat uit 4 toetsen per studiejaar. Toekomstige versies van de iVTG zullen waarschijnlijk gebaseerd zijn op geautomatiseerde toetsafname via de computer. Een van de hoofddoelen van de iVTG is om het zogenoemde learning to the test-effect tegen te gaan. Bijkomende voordelen zijn dat studenten en docenten gedetailleerde feedback krijgen over de beheersing van kennis en waar lacunes in kennis of het curriculum bij meerdere instellingen voorkomen. Een voortgangstoets voor het tandheelkundig onderwijs in Nederland is goed denkbaar, omdat de doelstellingen van een voortgangstoets goed aansluiten bij het gemeenschappelijke kader zoals beschreven in het Raamplan Tandheelkunde, dat gedeeld wordt door de huidige 3 tandheelkundeopleidingen. Een basis voor het starten van een voortgangstoets voor de studie tandheelkunde zou de huidige ‘Overalltoets’ van het ACTA kunnen vormen.

Approximately 36% of the Dutch population has insufficient or limited health literacy. This disproportionately concerns patients with low socioeconomic status (SES), the elderly and migrants, but also highly-educated patients. Dentists are often unaware of this. A patient with limited health literacy might have trouble with a dentist's explanation or with reading written information. A dentist may not immediately notice this, but can be aware of signals indicating it. Such signals can vary from not completely filling out a questionnaire to wrongly interpreting instructions or systematically not showing up to appointments. The dentist needs to take this into account in his use of language and adapt any informational matter to make it understandable for everyone. Co-workers such as receptionists also need to be informed about signs indicating limited health literacy and how best to communicate with these patients.Bij ongeveer 36% van de Nederlanders is sprake van onvoldoende of beperkte gezondheidsvaardigheden. Dit komt overwegend voor bij patiënten met een lage sociaal-economische status, ouderen en migranten maar ook bij hoger opgeleiden. Tandartsen zijn zich hier vaak niet van bewust. Een patiënt met beperkte gezondheidsvaardigheden kan moeite hebben met het begrijpen van uitleg die de tandarts geeft of met het lezen van schriftelijke informatie. De tandarts merkt dat misschien niet direct in het contact maar kan wel signalen opvangen die daarop duiden. Die signalen kunnen variëren van het niet volledig invullen van een vragenlijst tot het verkeerd opvolgen van adviezen of stelselmatig niet op de afspraken verschijnen. De tandarts moet daar in zijn taalgebruik rekening mee houden en kan informatiemateriaal zo aanpassen dat het voor iedereen begrijpelijk is. Ook kunnen praktijkmedewerkers zoals balie-assistenten ingelicht worden wat signalen kunnen zijn van beperkte gezondheidsvaardigheden en hoe ze het beste kunnen communiceren met deze patiënten.

By means of a brief online questionnaire with 12 statements about the organisation, quality and impact of oral care in the Netherlands, the readers of the Netherlands Journal of Dentistry (NTVT) were asked to express their opinions on a number of important subjects concerning oral care in the Netherlands with respect to health economic matters. A total of 237 readers (61% men, 39% women) completed the online questionnaire. 70% of them were working as dentists and had been active in a practice for between 31 and 40 years. According to the study, a shift from curing to prevention was considered to be necessary. Most of the respondents also thought inequality in oral health in the Netherlands is increasing and people avoid going to the dentist due to the associated costs. In conclusion, most oral care providers appear to be reasonably positive about Dutch oral care. Attention for prevention, appreciation of oral health and the reduction of inequality in oral care continue to be necessary.Door middel van een korte online-enquête met daarin 12 stellingen over de organisatie, kwaliteit en impact van de mondzorg in Nederland werden lezers van het Nederlands Tijdschrift Voor Tandheelkunde gevraagd naar hun mening over een aantal belangrijke thema’s binnen de Nederlandse mondzorg met betrekking tot gezondheidseconomische kwesties. Een totaal van 237 lezers vulden de online-vragenlijst in (61% man, 39% vrouw). Hiervan werkte 70% als tandarts en was tussen de 31 en 40 jaar werkzaam in de praktijk. Geconstateerd werd dat een verschuiving van curatie naar preventie in de mondzorg nodig werd geacht. Ook waren de respondenten veelal van mening dat de ongelijkheid in mondgezondheid in Nederland toeneemt en dat mensen een bezoek aan de tandarts mijden vanwege de daarmee samenhangende kosten. Concluderend kan worden gesteld dat de meeste mondzorgverleners redelijk positief lijken te zijn over de Nederlandse mondzorg. Aandacht voor preventie, de waardering van mondgezondheid en het tegengaan van ongelijkheid in de mondzorg blijft nodig.

In this study, the prevalence of musculoskeletal symptoms among dentists and dental students in the Netherlands was inventoried by means of 2 corresponding web surveys. 196 (25% response) dentists and 359 (40% response) dental students participated. 80% of the dentists and 95% of the dental students reported to have suffered from pain in muscles and joints in the last 12 months. In particular, they mentioned complaints of the neck, the shoulders and the lower back. Dental students also frequently reported complaints of the upper back. Many physical complaints were more common among females than male students. In contrast, no gender difference was observed for dentists, but regarding this group stress proved to be an important risk factor for developing musculoskeletal complaints. Given the negative impact on the profession, further research into the prevention of musculoskeletal disorders is urgently needed, in particular about the efficiency of (postgraduate) education and occupational briefing in that domain.In dit onderzoek werd de prevalentie van musculoskelettale klachten onder tandartsen en tandheelkundestudenten in Nederland geïnventariseerd door middel van 2 overeenkomstige web-enquêtes. Hieraan namen 196 (25% respons) tandartsen en 359 (40% respons) studenten deel. Van de tandartsen gaf 80% en van de studenten gaf 95% aan de afgelopen 12 maanden last te hebben gehad van spieren en gewrichten, waarbij vooral nek, schouders en onderrug werden genoemd. Studenten meldden daarnaast ook vaak klachten in de bovenrug. Door vrouwelijke studenten werden veel lichamelijke klachten vaker genoemd dan door de mannelijke studenten. Bij tandartsen werd daarentegen geen geslachtsverschil waargenomen, maar bleek ervaren stress een belangrijke risicofactor op het ontwikkelen van musculoskelettale klachten. Gezien de negatieve effecten op de beroepsuitoefening is nader onderzoek naar de preventie van musculoskelettale aandoeningen dringend gewenst, in het bijzonder wat betreft de doelmatigheid van (postacademisch) onderwijs en beroepsmatige voorlichting op dat gebied.

When faced with the question whether a particular treatment is indicated, dentists have to ask themselves to what extent it fits within a health care goal. If it does not, then the indication is usually rejected because it is in conflict with professional standards. Dentists who nevertheless consider such an indication have to bear the responsibility to make clear to the patient not only the general risks of a certain treatment but also the additional risks associated with the hazardous indication.Bij de vraag of een bepaalde behandeling is geïndiceerd moeten tandartsen zich afvragen in hoeverre de behandeling past binnen een zorgdoel. Is dit niet het geval, dan wordt de indicatie doorgaans afgewezen omdat deze in strijd is met de professionele standaard. Op tandartsen die een dergelijke indicatie toch overwegen, rust de plicht de patiënt niet alleen op algemene risico’s van een bepaalde behandeling te wijzen, maar ook op de extra risico’s in verband met de risicovolle indicatie.

Oral healthcare is to an ever-larger extent delivered by a team or network. Members of such a network often seem to be unsure about the rules that govern the distribution of tasks. The Individual Healthcare Professions Act (Dutch: Wet BIG) in which this distribution is defined, makes use of very specific terms. Clarity concerning these terms is a prerequisite to be able to reflect, in a subsequent contribution, on the realisation of the prerequisites that are applicable to working in a team or network.Steeds vaker wordt mondzorg binnen een team of een netwerk geleverd. Leden van een dergelijk netwerk blijken vaak onzeker over de regels die gelden voor de verdeling van taken. De Wet op de beroepen in de individuele gezondheidszorg waarin deze verdeling is geregeld, gebruikt heel specifieke termen. Duidelijkheid over deze termen is een voorwaarde om in een latere bijdrage te kunnen reflecteren op het operationaliseren van de voorwaarden die gelden voor werken in een team of een netwerk.

In het jaar 2015, zo is de verwachting, worden er in het BIG-register meer buitenlandse tandartsen ingeschreven dan dat er jonge Nederlandse tandartsen afstuderen. Voorts verwacht men dat de instroom van buitenlandse tandartsen in Nederland ook in de toekomst nog zal blijven toenemen. Het is van velen van hen onduidelijk wat het niveau van hun kennis en vaardigheden is, of zij zich tijdelijk of blijvend in Nederland vestigen of zij genoeg kennis hebben van zowel de Nederlandse wet - en regelgeving als van de specifieke manier van samenwerken met mondhygiënisten in de Nederlandse mondzorgpraktijken en of zij de Nederlandse taal voldoende beheersen.

A twenty-year-old man was referred to an orthodontist for orthodontic re-treatment of dental element 32 by his dentist and a periodontist. At the buccal surface of the dental element a significant gingival resorption had occurred and the root of the element was located outside of the processus alveolaris buccally. This case will be presented at the thematic tour 'Door de beugel: team up' on 27 October 2015, which is organised by the Nederlandse Vereniging van Tandartsen (Dutch Dentists' Society) and will be held at 4 different locations in the Netherlands. It is known that a patient's complaint can have a serious impact on the well-being of dentists. But little is known on the nature and the extent of this impact. That's why in 2013 an anonymous survey was conducted among 955 dentists and dental specialists, for whom in the period mid-2008 to mid-2013 a complaint was handled through the Complaint Committee of the Royal Dutch Dental Association (KNMT). Altogether, 413 (43%) of them participated in the study. 71% of the respondents reported that the complaint had influenced their professional practice. 52% reported that it had (also) influenced their attitude towards colleagues and patients, and 60% (also) experienced an impact on their mental and/or physical condition. Altogether, 68% reported that they had unpleasant feelings and 75% stated that the treatment relationship with the patient was terminated as a result of the complaint.Bekend is dat een klacht van een patiënt voor tandartsen impact kan hebben op hun welbevinden. Maar over de aard en de omvang daarvan is weinig bekend. Om hierop meer zicht te krijgen is in 2013 een anonieme schriftelijke enquête uitgevoerd onder 955 tandartsen en tandartsspecialisten, van wie in de periode medio 2008 tot en met april 2013 een klacht is afgehandeld via de KNMT-klachtenregeling. In totaal deden er 413 (43%) van hen aan het onderzoek mee. Van de ondervraagden meldde 71% dat de klacht heeft doorgewerkt in hun persoonlijke beroepsuitoefening. Bij 52% was dat (ook) het geval voor hun houding ten opzichte van collega’s en patiënten en bij 60% (ook) voor de mentale of fysieke gesteldheid. Alles bijeengenomen gaf 68% aan last te hebben gehad van onprettige gevoelens en meldde 75% dat de behandelrelatie met de klager (patiënt) als gevolg van de klacht is beëindigd.

In 2013, to evaluate the functioning of the complaint procedure of the Royal Dutch Dental Association (KNMT), an anonymous, written survey was conducted among 955 dentists and dental specialists, for whom in the period mid-2008 to mid-2013 a complaint was handled. 413 (43%) participated in the study. 51% of them reported that the complaint was successfully mediated by the Regional Mediation Boards and the Specialist Mediation Board and 17% that the patient had withdrawn the complaint. For the remaining 32% the complaint was handled by the Central Complaint Committee, who determined that 13% was (partially) substantiated, 17% was not substantiated and 2% was settled. Generally, 79% of the dentists are (mostly) positive about the procedure of mediation, while 8% are neutral and 13% are dissatisfied about it. With regard to the complaints procedure this was 60%, 11% and 29% respectively. The judgment of the dentists about the Regional Mediation Boards and the Specialist Mediation Board and about the Central Complaints Committee is influenced by the success of the complaints procedure and by the impact of the complaint that was experienced. The emotional support of dentists during the complaints trajectory is one of the points requiring attention.In 2013 is ter evaluatie van de KNMT-klachtenregeling een anonieme schriftelijke enquête uitgevoerd onder 955 tandartsen en tandartsspecialisten tegen wie in de periode medio 2008 tot mei 2013 een klacht is afgehandeld. De respons bedroeg 413 (43%). Bij 51% was de klacht succesvol bemiddeld door de Regionale Bemiddelingsraden en de Specialisten Bemiddelingsraad; bij 17% trok de patiënt de klacht in. De overige 32% ging langs de Centrale Klachtencommissie, die de klacht bij 13% (deels) gegrond verklaarde, bij 17% ongegrond en bij 2% alsnog schikte. Alles bijeen genomen was 79% van de desbetreffende tandartsen (overwegend) positief, 8% neutraal en 13% ontevreden over de klachtbemiddeling. Voor de klachtprocedure was dit respectievelijk 60%, 11% en 29%. Het oordeel over de Regionale Bemiddelingsraden en de Specialisten Bemiddelingsraad en dat over de Centrale Klachtencommissie wordt beïnvloed door het succes van klachtprocedure en door de ervaren impact van de klacht. De emotionele ondersteuning van tandartsen tijdens en na het klachttraject is een van de aandachtspunten.

In this paper the opinions from dental hygienists and dentists are described concerning the question whether patients could visit the dental hygienist without being seen by a dentist first (access without referral). Eight of ten dental hygienists have a positive attitude towards the idea of access without referral. Dentists were much more sceptical about this idea. There are different opinions about which treatment, what kind of patients, which age groups etcetera a dental hygienist could treat without referral from the dentist. When access without referral becomes relevant, pilot studies are advised before implementing the concept of access to the dental hygienist without referral from the dentist.

The opinions of professional dental organisations, the government (Ministry of Health), patient organizations and the medical assurance companies are collected on the question whether patients could visit the dental hygienist without being seen by a dentist first. The general opinion is that this could be possible for those treatments for which the dental hygienist are fully qualified. However, the professional organizations of the dentists claim that the dentist should always be responsible for the patient register and thereby the first responsible for the dental health of the patients. Pilot projects are advised to study how the various dental workers can most efficiently work together.

Regulations which have been established by the professional community or by an organization of which a dentist is a member (self-regulation) have a more significant legal effect on dental care than many dentists realize. A dentist can only deviate from these regulations if he does so when motivated by the patient's best interests. Self-regulation can be relevant at the level of the professional community, the organization or the individual dentist. Self-regulation should satisfy a number of criteria if it is to be applicable to an entire professional community, specifically: it must be sufficiently authoritative, it must harmonize with existing laws and regulations and it must be easily accessible to both professionals and patients. The application of self-regulation is sometimes limited to a specific group, for example dentists who have signed a working agreement with a certain health insurance provider or who work in a certain institution. Self-regulation can entail liability not only on the part of the healthcare professional but also, in theory, on the part of those who establish the regulations.

Each generation likes to see itself as unique and thinks that the conditions under which people live and work are also unique. This position does not, for the most part, apply to dentists. Two questions which are now under discussion were also under discussion in the past. In the first case, this has to do with whether dentistry should be seen especially as a profession in which skill and competence are central, or as a medical discipline in which science (too) is central. The second question which was posed in the past as it is now is whether the dentist is seen especially as a salesman, albeit as a healthcare provider. Both questions are of the greatest significance for the image of dentists and also perhaps indirectly for the future of the professional group. Indeed, the answer to these questions has determined in the past as well as now the image of dentists and the degree to which the government has believed that dentists could be replaced by less highly educated non-dentists. For that reason, a clear answer to these questions ought to be formulated. It is possible that the answers given to these same questions in the past will be helpful in formulating answers now.

Disciplinary law can be seen as an element in the system that protects the quality of dental care and in that way helps to maintain the high level of trust which the public has in the profession. A disciplinary court case is regarded by dentists as very taxing. This is in part a result of unfamiliarity with the procedure. The procedure is also feared because of the publicity it attracts. The publicity is partly inherent in the goal of the disciplinary law. To some extent the publicity seems to be taking on the character of 'trial by media'. Questions can be raised with respect to this latter development, regarding, for example, the use of anonymity by journalists; the impact of publication, which is partially dependent on other news-worthy events; and the impact of the publication of reprimands versus the most severe sanction.

The extent of the responsibility to provide care which a dentist bears depends on whether a patient is registered with the dentist's practice. A dentist has the freedom not to register a patient with his practice, provided he does not violate the anti-discrimination laws or a cooperative professional agreement which the dentist has signed. No conditions are applied if a patient wishes to withdraw his registration, or a registration is withdrawn by mutual agreement. If a dentist wishes to terminate the treatment agreement unilaterally, this can only be done if serious reasons for doing so can be identified. The reasons for termination which will be accepted by disciplinary boards as serious depend very much on the conditions of the specific case. In cases of the unilateral termination of a treatment agreement on the part of a dentist, given the serious reasons for doing so, it is usually necessary for a dentist to take into consideration various requirements for careful conduct.
