Medicalization represents an increasingly significant form of social control. Emergent evidence suggests that workplace managers take up medicalized practices and discourses to produce a compliant labor force, but this phenomenon has received limited sociological attention. This paper extends prior theories of medicalization to investigate therapeutic management in the low-wage workplace. I draw upon eight months of ethnographic fieldwork in Disability Works, a nonprofit job training program for people with mental illnesses, and interviews with other providers and advocates within this field. Disability Works harnesses therapy, psychiatry, and "softer" therapeutic practices such as mindfulness meditation, sleep hygiene, and positive affirmations to produce its workforce. This paper identifies two dimensions of therapeutic management: (1) it aims to inculcate work norms at the level of client-workers' embodied dispositions, and (2) it aims to transform structural problems into individual ones. • Conditional reimbursement policies are increasingly viewed as promising instruments for expensive yet uncertain technologies and pharmaceuticals. • There is paucity of empirical research investigating how conditional reimbursement policies play out in practice. • We study two cases of conditional reimbursement in the context of Dutch healthcare and identify three key tensions: proceduralism versus improvisation, steering professionals versus providing leeway, and involving patients as data subjects versus legitimate stakeholders. • The notion of 'epistemic entanglements' highlights how normative and scientific dimensions are interwoven in reimbursement decisions. HTA agencies are experimenting with conditional reimbursement approaches allowing greater flexibility to cope with epistemic uncertainties generated by new health technologies and pharmaceuticals lacking evidence. While some research into promises and effects of conditional reimbursement is conducted, little empirical research investigates how such policies play out in practice. In this paper, we analyze two cases of conditional reimbursement in the Netherlands. Case studies were purposively selected. We conducted document analysis combined with semi-structured in-depth interviews (n = 28). We analyzed both case studies together through initial thematic analysis and additional abductive analysis. Results were verified through data triangulation. We performed a member check in which we presented our preliminary analysis during a reflection meeting with key stakeholders. We identified three tensions in the practices of CED-schemes: proceduralism versus improvisation, steering professionals versus providing leeway, involving patients as data subjects versus legitimate stakeholders. These tensions explicate several sources of epistemic uncertainties that extend beyond methodological and more well-known socio-political pressures such as from industry on regular reimbursement decision-making process. We note the importance of improvisation work, of normative considerations, and of epistemic hierarchies. We postulate that the emerging uncertainties within the practice of CED-schemes are to an extent unavoidable as they emerge from the necessarily interactive and normative nature of human relations. We conceptualize this with the notion of 'epistemic entanglements', which highlights how normative and scientific dimensions are interwoven in reimbursement decisions. As epistemic uncertainties are difficult to reduce and tame in practice the need for a more reflexive and inclusive approach to conditional reimbursement decision-making becomes apparent. New technologies and personalized medicines can have great health benefits for patients, but often not enough knowledge is available to assess whether these innovations live up to their expectations. At the same time, despite this uncertainty, governments need to make a decision about whether or not to reimburse these innovations. Many countries are experimenting with forms of conditional reimbursement: temporarily reimbursement of technologies until more evidence is collected regarding (cost-)effectiveness in practice. Although much literature addresses potential benefits and drawbacks of conditional reimbursement policies, few studies look into how such policies play out in practice. In our research we study two examples of conditional reimbursement practices. We show conditional reimbursement practices lead to three new tensions. The paper focuses on the relevance of Kant's anthropologically oriented idea of the pragmatic use of reason for specific theoretical approaches in sociology. As I show in the first part, Kant's explicit presence in 20th-century sociology does not refer much to his anthropology and specifically to its cornerstone – the pragmatic use of reason which establishes a subtle connection between the theoretical and practical functions of reason. As an instrument for gaining systematic knowledge about the social world and ourselves as beings both passively and actively involved in this process, Kant's pragmatic use of reason serves a specific form of the theoretical use of reason. At the same time, it embodies a kind of practical reasoning concerning the "general welfare" in the social sphere. Building on the key arguments in the first part, I then address the question of whether we can view Kant's pragmatic approach as a possible third way for sociology today, beyond the simplifying opposition of 'theoretical' normativity and 'realistic' empiricism, and whether this third way can help us in clearing specific sociological issues. Here, I focus on two examples, namely the use of Kant's notion of "unsocial sociability" in Ralf Dahrendorf's conflict theory and on the criticism of Kant's cosmopolitanism in Ulrich Beck's reformed sociology of cosmopolitanism. Instead of diminishing with the spectacular advancement of medical expertise in the country , unqualified biomedical practice in India has been strengthened by the growth of the pharmaceutical production in the twenty first century. In public health discourse, the view that the informal health practitioners have to be punished and abolished has been countered by the recommendation that they could be trained and incorporated in primary health care where public health amenities are inadequate. The quality of care provided by the informal health care practitioners has also been subject to clinical assessment based on standardized patient vignettes. Based on a sociological approach, this paper examines the time line of chronically ill patients under lived conditions to arrive at an understanding of the role of informal health practitioners in long term treatment and highlights the setbacks. This paper draws on 253 household surveys from two villages in Madhya Pradesh, in depth interviews with four unqualified practitioners in the area, twenty five unstructured interviews of chronic patients, twenty five structured interviews on the cases of untimely death and FGDs with health workers in 2021. Informal health care practitioners offer consultation cum dispensing of medicines and are the primary source of biomedical care in the remote study area without any public transport. But they are 'for profit' economic actors who are ill-equipped to handle chronic diseases. What sets them aside from the qualified private doctors in the town is their social obligation to balance their profit motive with the ethics of proximity and neighborly ties with the villagers amidst whom they reside. The control of rail signalling is known to be highly dependent on local knowledge and local factors. It is also known to be highly cognitive in its nature involving a constant balancing of system performance within the constraints of safety. In the current paper, data generated through field work with signallers were used to understand the role of local knowledge, set against the background of an existing Local Knowledge Framework (Pickup et al., 2013) that was proposed to help determine the contents and mechanisms behind local knowledge in rail signalling. The field work included interviews with signallers and operations managers along with observations of signaller work. The results showed that the local knowledge framework needs to be expanded to include aspects related to the general public at user worked crossings and level crossings. In addition, the analysis highlights some of the issues with the transmission of local knowledge. The paper then discusses some of the gaps in the current framework, highlighting the importance not only of local knowledge for specific functions of signalling, but how these interact to support trade-offs to balance performance with safety. As systematic reviews become more common in the social sciences journal literature, it is important for social science librarians to be familiar with this methodology. Knowledge of characteristics of systematic reviews that have been published in the journal literature can help to inform librarians as they guide researchers to adopt good practices. This paper analyzes a collection of 164 systematic reviews gleaned from the International Bibliography of Social Sciences database for the period 2017 to 2019 from journals in the fields of anthropology, business and economics, communications, education, political science, psychology, social sciences (comprehensive), and sociology. The methodology of each review was checked to answer questions about reporting of keywords and search terms, reporting of inclusion/exclusion criteria, time period searched, external guidelines referenced, initial number of studies retrieved and number of studies included in the review, and number and names of databases searched. Based on ethnographic fieldwork among men who have sex with men and transgender women in Tanzania, this article explores the various types of work that may go into enrolment into PrEP programming and using pre-exposure prophylaxis (PrEP). PrEP protects against HIV acquisition and is widely touted as an essential tool in 'ending AIDS by 2030'. While taking PrEP has often been portrayed as 'just taking a pill a day' in public health campaigns, a striking observation during fieldwork was that enrolling in PrEP programming and adhering to PrEP involved a wide range of tasks. Inspired by this fieldwork experience and the literature on sociology of work, more specifically illness work and patient work, we started to think of these tasks as work. This paper identifies the range of tasks that PrEP users in Dar es Salaam had to perform as part of their enrolment and usage of PrEP. We provide a description of these tasks, organised into three categories of work that we refer to as (a) readying work, (b) user work, and (c) social navigation work that jointly make up what we propose to call biomedical prevention work. Medicalization is increasingly recognized as a bidirectional process, with patients and their families as agents. The paper considers the specific case of the medicalization of autism in Italy, from the point of view of parents of autistic people with different levels of support needs. Through reporting and comparing results of two independently conducted qualitative studies, this paper aims to analyze how parents embrace and resist the medicalization of autism in their everyday lives and in healthcare contexts. Both studies involved participant-observation with services that targeted autistic people and interviews with parents, professionals, and autistic people. Results show that parents of autistic people both embrace and resist medicalization. While parents (sometimes ambivalently) accept the responsibilization inherent in their engagement with interventions (a sort of "therapeutization" of life) and reject lay expertise by deferring to experts' knowledge, they also resist the application of medical labels, language and practices in various ways in their everyday lives. Both embracing and resisting medicalization can be useful for achieving overarching social goals of being a good parent, helping their children, and pursuing respect and social harmony. Medicalization derives not only from the cultural dominance of medical discourses, which seems to incorporate resistance to medicalization stances, but also from the absence of continuity and coordination of services, particularly in the Italian context of public (but increasingly privatizing) health and welfare services. • Parents accept the responsibilization inherent in medicalization and resist it. • Both embracing and resisting medicalization is useful for achieving social goals. • Medicalization is due to the cultural dominance of medicine. • Medicalization also derives from the absence of continuity of services. • Cultural and biopolitical context impact on how people engage with medicalization. How can we analyze the institutional reactions to shocking events producing abrupt and substantial changes? To deal with this question, the paper assumes an interdisciplinary perspective by combining geography and sociology. After defining the foundational concepts of shocking events and institutions, the paper suggests that the outcomes of such events in respect of institutional frameworks should be analyzed at the territorial level as 'spaces of rupture and reconfiguration'. This paper provides an examination of the process used by ordinary courts in Chile when making legal capacity determinations. It provides an up-to-date account of various aspects of law and procedure related to legal capacity in this jurisdiction. Also, by drawing on semi-structured interviews with judges, this paper examines judicial understandings of legal capacity decision-making focusing on problems related to the medicalisation of mental disability and the participation of persons with disabilities in legal capacity procedures. • The term 'industry' can be understood in a variety of ways. • Simple understandings of 'industry' miss out on the complexity of relations. • Systemic and social understandings of 'industry' provide more opportunities. Use of the term 'alcohol industry' plays an important role in discussions of alcohol and public health. In this paper, we examine how the term is currently used and explore the merits of alternative conceptualisations. We start by examining current ways of referring to 'alcohol industry' in public health and then explore the potential for organisational theory, political science, and sociology to provide alcohol research with more inclusive and nuanced conceptualizations. We identify, and critique, three conceptualisations based on purely economic understandings of industry: literal, market , and supply-chain understandings. We then examine three alternative conceptualizations based on systemic understandings of industry: organizational, social-network, and common-interest understandings. In examining these alternatives, we also identify the extent to which they open up new ways of approaching the levels at which industry influence is understood to operate in alcohol and public health research and policy. Each of the six understandings of 'industry' can play a role in research but their utility depends on the question being asked and the breadth and depth of the analysis being undertaken. However, for those intending to engage with a broader disciplinary base, approaches grounded in the systemic understandings of 'industry' are better positioned to study the complex nexus of relationships that contribute to alcohol industry influence. Classic studies of medical education have examined how professional socialization reproduces the prevailing professional culture, as well as how students actively negotiate their place in educational processes. However, sociological research has not re-examined student culture in light of structural transformations in medical education, such as the introduction of new assessment types and their use as modes of commensuration. In this paper, we examine data from two studies of online forums where medical trainees and applicants to medical school discuss their experiences preparing for tests of professional skills, including judgment, empathy, and communication. Examining how medical students talk about these tests on such forums allows us to understand the meaning-making processes at work as students negotiate the commensuration processes such tests enable. We examine how these negotiations take place in online forums, where participants confront common challenges, form common perspectives, and share common solutions, all hallmarks of student culture. Through qualitative analysis, we find that online communities are spaces where students grapple with these new forms of commensuration, interrogate the standards and quantifications that underlie them, and collectively negotiate how to approach these assessments. Using the case of online forum communities, our findings advance past work on student culture in medical sociology by theorizing student culture as an extra-organizational phenomenon that spans multiple career stages. Medicalization is a central topic of concern in the sociology of disability and of health and illness. In this paper, I examine how medicalization is inequitably applied and circulates in the context of schools, specifically in serving students with educational disabilities. My aim is to advance understandings of medicalization through this case. Using a mixed-methods design, I first show, descriptively, how race and gender intersectionally predict educational disability status in a dataset of all Wisconsin public school students. Next, I examine how racial and gender disparities in disability status are produced at the micro level, using interviews with 27 Wisconsin teachers, including in-depth discussions of 73 individual students that were struggling academically or behaviorally. My quantitative findings show variation by race, gender, and disability category: White children have higher probability of special education receipt than comparable children of color for academic difficulties, but lower probability for behavioral difficulties, and girls have lower probability than comparable boys overall. My interview data suggest that these disparate outcomes reflect stratified medicalization processes, in which institutional constraints, status beliefs, and cultural discourses of race and gender shape both stratified noticing of schooling difficulties and stratified interpretation of those difficulties as medicalized conditions. • White children and boys are more likely than comparable peers to receive special education for higher-status disabilities. • Children of color and boys are more likely than comparable peers to receive special education for lower-status disabilities. • These inequalities are explained by stratified definitional processes of medicalization by teachers. • Institutional constraints and cultural discourses of race and gender shape stratified medicalization processes. • These processes include stratified noticing and stratified interpretation of students' difficulties. Diet is understood to be one of the most important influences on public health and chronic disease, and is particularly implicated in the so-called 'obesity epidemic'. Yet interventions aiming to improve the population's dietary habits have failed to translate into widespread health improvements. Simultaneously, the knowledge landscape has become increasingly contentious, with fat activism challenging dominant approaches to how obesity is framed and addressed. This paper is based on 24 ethnographic interviews, and explores the work of health practitioners promoting therapeutic carbohydrate restriction ('low-carb' diets) for people with metabolic health conditions. Drawing on Michel Callon's study of technological innovation, I show practitioners engaging in 'on-the-job sociology'—situated sociological work to justify, and forge a space for, innovative dietary intervention. These innovators employ physiological explanations of hormones, satiety (or hunger), and pleasure (or shame), supported with personal experience, to emphasise material connections between particular eating habits and the sustainability of dietary improvement in everyday life. They resist fat activist influence on healthcare practice (that has resulted in practitioners avoiding conversations about diet, fatness and health), as well as the more extensively critiqued practices of health promotion. Deflecting blame/shame from individuals, innovators spotlight the role of the food industry in undermining public understandings of food and physiology, and dietary improvement that is achievable and sustainable. Through on-the-job sociology, innovators forge a space to engage patients in collaborative dietary experimentation and improvement. Hospitals remain the most common location of death in a significant number of countries, and specialist palliative care is positioned as a crucial resource for improving hospital care for those nearing end of life. Little is known, however, about a substantive aspect of this work: how hospital palliative clinicians anticipate and organize a patient's dying trajectory. In this paper I draw from a larger original ethnographic research study of palliative specialists in two Canadian hospitals. Abductive analysis resulted in framing their work as affective labour, both reflecting and re-creating a larger affective economy shaping the affective states of everyone involved in the provision and uptake of care. I articulate six analytically ideal outcomes of clinicians' affective labour that organize end of life, including: 1) proactive co-authorship of disease trajectory; 2) mutual acknowledgement of a dying trajectory; 3) naturalizing direction and outcome of care, 4) ensuring a minimum of social disruption, 5) identification as compassionate and efficient care providers, and 6) increased specialist knowledge and interventions. In so doing, clinicians' practices become understandable as labour to meet a diversity of – at times conflicting – individual, organizational, and societal mandates that necessarily include, but extend well beyond, the patient and her immediate social network. This is the first study to consider hospital palliative care as an affective economy, and presents a theoretically innovative and empirically grounded model to advance new ways of conceptualizing hospital palliative care. A comprehensive understanding of human sociality needs to embrace the coevolution of genes and culture. Recent advances in biological research about niche construction by organisms, and the development of the concepts of social niche and ethodiversity, can be integrated into a common approach to understand this coevolution, which implies the interaction between sociology and ecology in an integrative framework of knowledge. In this paper the authors propose such inclusive biosociological and heuristic framework to improve the understanding of the evolution of social niche construction. In addition, it allows a better understanding of the concept of sociotype in non-human organisms and explains some aspects of the social or presocial behavior through the concept of ethodiversity. In the last two decades there has been a large and simultaneous expansion of delivery-based national housing programs in the global South. This paper draws on an investigation of living conditions in Ciudad del Bicentenario, a large-scale housing project on the outskirts of Cartagena. Based on a sample of seventy-five lengthy semistructured interviews with low-income residents and ten other stakeholders, we reached three paradoxical findings that reveal the high costs of free housing: the immediate improvement of overall quality of life but the impoverishment of economic prospects; the dual social condition of the residents who felt socially and economically isolated but also shared a widespread perception of being 'revueltos' or scrambled within the community of Ciudad del Bicentenario itself; and the growth of economic and physical informality in a project intended to counter the informal settlements in the inner city. These paradoxes reflect more than simple processes of impoverishment of living conditions: they demonstrate how this type of public policy systematically complexifies the daily life of relocated families. Sociologically, these findings reveal a new type of peri-urban poverty in the global South. We argue that the productive informalization of the formal housing projects constitutes a central, but previously understudied, aspect of what it means to develop equitable housing.
