Abstract

Much has been learned in recent years about the mechanisms by which breastfeeding improves child health and survival. However, there has been little progress in using these insights to improve pediatric care. The aim of this study was to review all clinical studies of lactoferrin (LF) in children in an effort to determine which interventions may improve pediatric care or require further research. We conducted a systematic and critical review of published literature and found 19 clinical studies that have used human or bovine LF for different outcomes: iron metabolisms and anemia (6 studies), fecal flora (5 studies), enteric infections (3 studies), common pediatric illnesses (1 study), immunomodulation (3 studies), and neonatal sepsis (1 study). Although the efficacies have varied in each trial, the main finding of all published studies is the safety of the intervention. Protection against enteric infections and neonatal sepsis are the most likely biologically relevant activities of LF in children. Future studies on neonatal sepsis should answer critically important questions. If the data from these sepsis studies are proven to be correct, it will profoundly affect the treatment of low birth weight neonates and will aid in the reduction of child mortality worldwide. Au cours des dernieres annees, on en a appris beaucoup sur les mecanismes par lesquels l'allaitement ameliore la sante et la survie de l'enfant. Cependant, il y a eu peu de progres quant a l'utilisation de ces connaissances pour ameliorer les soins pediatriques. Cette etude vise a passer en revue toutes les etudes cliniques portant sur la lactoferrine chez les enfants, afin de determiner quelles sont les interventions qui peuvent ameliorer les soins pediatriques ou necessitent des recherches plus approfondies. Nous avons realise une revue systematique et critique de la litterature publiee et nous avons trouve 19 etudes cliniques dans lesquelles la lactoferrine bovine ou humaine avait ete utilisee a differentes fins : le metabolisme du fer et l'anemie (6 etudes), la flore fecale (5 etudes), les infections enteriques (3 etudes), les maladies pediatriques courantes (1 etude), l'immunomodulation (3 etudes) et la sepsie neonatale (1 etude). Malgre une efficacite variable selon les essais, la principale conclusion tiree de toutes ces etudes publiees etait la securite de l'intervention. La protection contre les infections enteriques et la sepsie neonatale constituent probablement les activites les plus pertinentes biologiquement de la lactoferrine chez les enfants. Si les donnees decoulant des etudes portant sur la sepsie s'averent justes, cela affectera profondement le traitement des nouveaux-nes de faible poids et contribuera a la reduction de la mortalite infantile dans le monde. Abstract

Here, we determined the effect of bovine lactoferrin (bLF) on the minimum inhibitory concentration (MIC) of ampicillin and trimethoprim-sulfamethoxazole in Shigella. Using a microdilution method, the MIC was determined in the presence or absence of bovine lactoferrin (10 mg/mL) on 88 Shigella strains (56 Shigella flexneri,15 Shigella boydii,13 Shigella sonnei, and 4 Shigella dysenteriae) previously isolated from peruvian children Key words: Shigella, resistance, lactoferrin, antibiotics. Nous determinons l'effet de la lactoferrine bovine (bLF) sur la concentration inhibitrice minimale (CIM) de l'ampicilline et du couple trimethoprime-sulfamethoxazole envers Shigella. En utilisant une methode de microdilution la CIM a ete determinee en presence ou en absence de bLF (10 mg/mL) sur 88 souches de Shigella (56 S. flexneri,15 S. boydii,13 S. sonnei et 4 S. dysenteriae) isolees precedemment chez des enfants peruviens de moins de deux ans. Un changement de valeur de CIM de deux fois ou plus etait considere significatif. Soixante-sept souches (76 %) etaient hautement resistantes a l'ampicilline; la presence de bLF diminuait la valeur de CIM de l'amplicilline sur le tiers des souches. Cette diminution des valeurs de CIM s'observait surtout sur les souches moins resistantes. La presence de bLF amenait sept souches resistantes a l'ampicilline (8 %) a l'interieur des limites permettant de les considerer comme sensibles. Quant au trimethoprime- sulfamethoxazole, 93 % des isolats y etaient hautement resistants, les valeurs de CIM n'etant diminuees par la bLF que sur 4 isolats seulement (5 %). Aucune souche resistante au trimethoprime-sulfamethoxazole ne devenait sensible en presence de lactoferrine. La diminution de CIM en presence de bLF semble dependre du mecanisme d'action de chaque antibiotique. Des etudes in vivo sont necessaries pour evaluer le potentiel de la bLF comme adjuvant au traitement antibiotique des souches de Shigella resistantes. Abstract

Streptococcus pneumoniae upper respiratory infections and pneumonia are often treated with macrolides, but recently macrolide resistance is becoming an increasingly important problem. The 13-valent pneumococcal conjugate vaccine (PCV13) was introduced in the National Immunization Program of Peru in 2015. This study aimed to evaluate the temporal evolution of macrolide resistance in S. pneumoniae isolates collected in five cross-sectional studies conducted before and after this vaccine introduction, from 2006 to 2019 in Lima, Peru. A total of 521 and 242 S. pneumoniae isolates recovered from nasopharyngeal swabs from healthy carrier children < 2 years old (2 carriage studies) and samples from normally sterile body areas from pediatric patients with invasive pneumococcal disease (IPD) (3 IPD studies), respectively, were included in this study. Phenotypic macrolide resistance was detected using the Kirby-Bauer method and/or MIC test. We found a significant increase in macrolide resistance over time, from 33.5% to 50.0% in carriage studies, and from 24.8% to 37.5% and 70.8% in IPD studies. Macrolide resistance genes [ erm (B) and mef (A/E)] were screened using PCR. In carriage studies, we detected a significant decrease in the frequency of mef (A/E) genes among macrolide-resistant S. pneumoniae strains (from 66.7% to 50.0%) after introduction of PCV13. The most common mechanism of macrolide-resistant among IPD strains was the presence of erm (B) (96.0%, 95.2% and 85.1% in the 3 IPD studies respectively). Macrolide resistance was more common in serotype 19A strains (80% and 90% among carriage and IPD strains, respectively) vs. non-serotype 19A (35.5% and 34.4% among carriage and IPD strains, respectively). In conclusion, S. pneumoniae macrolide resistance rates are very high among Peruvian children. Future studies are needed in order to evaluate macrolide resistance trends among pneumococcal strains, especially now after the COVID-19 pandemic, since azithromycin was vastly used as empiric treatment of COVID-19 in Peru. Abstract

Background : Antibiotic-resistant Escherichia coli can colonize the intestinal tract of healthy children, causing concern when antibiotic resistance is related to the presence of transferable mechanisms, such as extended-spectrum 13-lactamases (ESBLs). Materials and Methods: Fecal samples from 41 healthy children from two villages of rural Peru were cultured on ceftriaxone-disks. ESBL production was confirmed with double disk synergy. In all ESBL-produced isolates, antibiotic susceptibility to 12 antibacterial agents was established by disk diffusion, while clonal relationships were determined by repetitive extragenic palindromic-polymerase chain reaction (REP-PCR). Presence of ST131 was determined using PCR. Results:Ceftriaxone-resistant microorganisms were recovered from 39 samples belonging to 22 out of41 children (53.7°/o). Of these, 80 ceftriaxone-resistant and two ceftriaxoneintermediate E. coli from inside ceftriaxone-halos were confirmed as ESBL-producers. All isolates were multidrug-resistant. Abstract

To determine the effect of bovine lactoferrin on prevention of late-onset sepsis (LOS) and neurodevelopment delay.Randomized, double-blind, controlled trial in neonates with a birth weight of 500-2000 g in 3 neonatal units in Lima, Peru, comparing bovine lactoferrin 200 mg/kg/day with placebo administered for 8 weeks. The primary outcome was the first episode of culture-proven LOS or sepsis-associated death. Neurodevelopment delay was assessed by the Mullen Scales at 24 months corrected age.Of the 414 infants enrolled, 209 received bovine lactoferrin and 205 received placebo. LOS or sepsis-associated death occurred in 22 infants (10.5%) in the bovine lactoferrin group vs 30 (14.6%) in the placebo group; there was no difference after adjusting for hospital and birth weight; hazard ratio 0.73 (95% CI, 0.42-1.26). For infants with birth weights of1500 g the hazard ratio was 0.69 (95% CI, 0.39-1.25). The mean age-adjusted normalized Mullen composite score at 24 months was 83.3 ± 13.6 in the bovine lactoferrin group vs 82.6 ± 13.1 in the placebo group. Growth outcomes and rehospitalization rates during the 2-year follow-up were similar in both groups, except for significantly less bronchiolitis in the bovine lactoferrin group (rate ratio, 0.34; 95% CI, 0.14-0.86).Supplementation with bovine lactoferrin did not decrease the incidence of sepsis in infants with birth weights of2000 g. Growth and neurodevelopment outcomes at 24 months of age were similar. Neonatal bovine lactoferrin supplementation had no adverse effects.ClinicalTrials.gov: NCT01525316.
