2. Evaluation of PD-L1 Expression in Colorectal Carcinomas by Comparing Scoring Methods and Their Significance in Relation to Clinicopathologic Parameters.

Abstract

Background: This study aims to evaluate PD-L1 expression in colorectal carcinomas (CRCs) by using the tumor proportion score (TPS) and the combined positive score (CPS), and to investigate whether there is a correlation with clinicopathologic features. Methods: A cross-sectional study was conducted that included samples from patients with colorectal adenocarcinoma treated with colon resection and rectal resection after neoadjuvant radio- and chemotherapy at the Department of Abdominal Surgery at Požega Hospital in the period from 2017 to 2022. The study included 102 tumor tissue samples from patients after resection and the pathohistological diagnosis of adenocarcinoma. Results: In our study, the PD-L1 positivity rate after the TPS was 42 (41%) samples, and after the CPS, 97 (95%) of them (p < 0.001). The positive expression of PD-L1 in tumor cells using the TPS method showed a statistically significant association with adenocarcinoma (TPS ≥ 10–50% and ≥50%). There were significantly more that were moderately differentiated, with TPS ≥ 50%, and those poorly differentiated had values ≥ 10–50%. There were significantly more patients with a status of more than one positive lymph node with TPS values ≥ 10–50%. Patients without metastases in the lymph nodes are significantly more likely to have CPS values > 50%, compared with other lymph node statuses. Conclusions: These results suggest that the total number of PD-L1-expressing cells, including tumor and immune cells, is a more sensitive biomarker than the number of PD-L1-expressing tumor cells alone in CRC. Abstract

Patient satisfaction is a crucial measure of the quality of healthcare, especially with regard to nursing services in hospital settings. Understanding and increasing patient satisfaction with nursing care is critical to improving overall healthcare and ensuring positive patient experiences during their hospital stay. The aim of this research was to evaluate the psychometric properties of the Croatian version of the Satisfaction with Nursing Care Quality Questionnaire (PSNCQQ), test the reliability and validity of the tool after translation, and investigate differences in patient satisfaction based on demographic variables, as well as their contribution to satisfaction with the quality of nursing care. After translation and adaptation, the Croatian version of the PSNCQQ was applied to 350 hospitalized patients (average age 51.19 years (range: 18–87)), of whom 194 (55.4%) were men and 156 (44.6%) were women. The results showed high internal consistency (Cronbach's α = 0.977) and confirmed the one-factor structure of the questionnaire, explaining 70.64% of the total variance. Confirmatory factor analysis supported the unidimensional model, showing strong fit indices (χ2 = 583.047, df = 149, p < 0.001, GFI = 0.861, AGFI = 0.818, NFI = 0.936, TLI = 0.946, CFI = 0.955, RMSEA = 0.080, PCLOSE < 0.001). In conclusion, the validation of the PSNCQQ in the Croatian language increases resources for evaluating and improving the quality of nursing care. This research lays the foundation for future studies and practical applications aimed at improving patient satisfaction and nursing care outcomes in Croatia, but there are also limitations to this study, including its one-institution scope, the possible influence of factors outside the current treatment on satisfaction, and the lack of comparison with objective clinical indicators. Abstract

The purpose of this research was to examine whether demographic variables, personality traits, and workplace variables (working in shifts, job tenure, and perceived organizational justice) contribute the most to the prediction of job satisfaction in nurses. The survey included 161 nurses. The instruments used in this research were as follows: the Demographic Data Questionnaire, the Perceived Organizational Justice Scale, the Job Satisfaction Scale, and the NEO five-factor inventory. The study findings indicated that age, health status, distributive justice, and procedural justice positively contribute to job satisfaction among nurses, while neuroticism contributes negatively. Older nurses, those in better health, those who are satisfied with the organization's decision-making process, and those who feel adequately rewarded for their contributions tend to be more satisfied with their jobs. Conversely, nurses with a higher level of the neuroticism personality trait tend to be less satisfied with their job. The strongest predictors of job satisfaction among nurses were found to be health status, the personality trait of neuroticism, and distributive and procedural justice, with the age of nurses being slightly less powerful but still significant. Abstract

Gluten is the complex of water-insoluble proteins, gliadin (prolamin) and glutenin (glutelin) fraction from wheat, rye, and barley. Because of its properties (water binding and viscosity yielding) is widely used food additive. Gliadin is very important in the context of coeliac disease (CD), non-coeliac gluten sensitivity (NCGS), gluten ataxia (GA) and dermatitis herpetiformis (DH). CD is characterized by villus atrophy of the small intestinal mucosa caused by the interaction of the gliadin and the mucosa of the small intestine in susceptible persons as an immune disorder. NCGS presents patients whose symptoms have disappeared with gluten withdrawal who do not have typical CD antibodies and do not suffer from lesions in the duodenal mucosa. GA occurs later in life with or without enteropathy and is associated with cerebellum affection by CD antibodies causing clinical features of gait and limb ataxia, ocular signs of cerebellar dysfunction. DH is uncommon cutaneous eruption associated with gluten sensitivity it is diagnosed after biopsy by immunofluorescence of granular or speckled IgA deposits in an area of perilesional skin. The gluten-free diet leads to complete regression of symptoms in all this entities. Despite the fact that self-reported gluten sensitivity (GS) is increasing over the past years there is no scientific evidence of benefits of gluten withdrawal in gluten non associated diseases. Researches show that the public perception of gluten is causing increase in the number of people who erroneously believe they are sensitive to it.

Abstract

Funkcionalna dispepsija (FD) definira se kao prisutnost postprandijalne punine, rane sitosti, epigastričnog žarenjaili boli u odsutnosti druge sistemske, metaboličke ili organske bolesti. FD je podijeljena u sindrom postprandijalnogdistresa (PDS) i sindrom epigastrične boli (EPS) sa značajnim preklapanjem s gastroezofagealnom refluksnom bolesti (GERD) i sindromom iritabilnog crijeva (IBS). Trenutačna preporučena terapija su antisekretorni lijekovi (PPI ili H2RA), no ponavljanje simptoma često je nakon prestanka terapije. Prokinetički lijekovi kao što su metoklopramid i domperidon posebno su djelotvorni u PDS, ali poput antisekretornih lijekova, dugoročna terapija nije preporučljiva. Acotiamid može biti učinkovit kao prokinetik. Antidepresivi (SSRI) pokazuju neke rezultate, kao i bizmutove soli i simetikon, ali su potrebni daljnji adekvatni pokusi i istraživanja. Iz ovog su istraživanja isključeni različiti brojevi kineskih, korejskih i iranskih bilja. Alginati daju učinke u GERB, ali učinci FD-a su upitni. Podaci pokazuju da je SWT5 djelotvoran kao prokinetik, osobito s IBS preklapanjem. Ulje metvice i kima te anisa, ekstrakt lišća đumbira i artičoke imaju bolju učinkovitost od placeba u liječenju funkcionalne dispepsije. 9. Prevention of diversion colitis

Abstract

Background: diversion of the faecal stream by colostomy leads to inflammation in the defunctioned segment, known as diversion colitis. Colostomy closure has a high rate of stricture formation and needs to be delayed in these patients. Diversion colitis represents an inflammatory state resulting from a nutritional deficiency in the lumen of the colonic epithelium. Therefore, it can potentially benefit significantly from local application of polysaccharide fibers. Objectives and research questions: determine whether the enema with polysaccharide fibers is effective in preventing the development of diversion colitis. Design: prospective multicentre randomised controlled trial. Inclusion Criteria: patients >18 years old who have undergone colostomy formation will be randomized to treatment with fiber enema or placebo. Exclusion criteria: patients with IBD ; patients allergic to any of the preparations used in the study. Primary outcome parameters: incidence of diversion colitis after colostomy formation evaluated by endoscopic biopsies. Secondary outcome parameters: postoperative complications after colostomy closure. 10. Synchronous caecal small-cell neuroendocrine carcinoma and adenocarcinoma of the rectum

Abstract

Small-cell neuroendocrine colon carcinoma is a rare entity with a usually poor prognosis. An 80- year-old female had colon cancer surgery due to synchronous tumour of the rectum and caecum. Pathohystological analysis of the caecal tumour showed trabecular and solid clusters, relatively uniformed small to middle sized epithelial cells, deficient cytoplasm and there were a great number of mitosis with larger areas of necrosis in the connective tissue. The immunohistochemistry was positive for chromogranin A. The caecal tumour was diagnosed as a small-cell neuroendocrine carcinoma. In addition, the rectal tumour showed microscopic findings consistent with stage IIA adenocarcinoma. The immunohistochemical panel showed that the tumour was negative for neuroendocrine markers. There were no clinical findings suggestive of hormone hypersecretion. Cancer métastasés were not found. Postoperative chemotherapy was applied. The patient is still alive, in good general condition and with no signs of tumour progression.

11. Short-term outcomes after open and combination of open and laparoscopic approach to advanced rectal cancer resection

Abstract

Aim: The aim of the present study was to analyse the short-term outcomes after open and combination of open and laparoscopic approach to advanced rectal cancer resection. Method: Of 26 patients with a T4 rectal cancer, 11 received initial laparoscopic approach with vascular ligation and splenic flexure mobilization, followed by en bloc resection through a medial suprapubic incision. The incision length was approximately 10–15 cm, depending on size of the tumour. The rest of the patients had an open approach through 2–3 fold longer, traditional laparotomy. Tumours were 3– 10 cm in diameter. Results: From a retrospectively collected database: length of hospital stay, wound infection, postoperative pain and analgesic consumption, volume of blood loss and blood transfusion, and the time until return of bowel movement were decreased in the mini-laparotomy group compared with the traditional laparotomy group. Operation costs, anastomotic leakage, and intestinal obstruction were similar in the two groups. A longer operating time in patients undergoing initial laparoscopy was observed. Conclusion: Combination of initial laparoscopy followed by mini-laparotomy can potentially offer all the benefits of a minimally invasive approach and achieve safe and feasible en bloc resection of advanced rectal cancer.

Abstract

The piezoelectric properties of some natural crystals and polymers can also be used in surgery. For this purpose, a prototype of an endoscopic instrument was constructed with piezoelectric material attached to its working end with the aim of recognizing pulsating blood vessels during laparoscopic surgery. To test the properties of the new instrument in laboratory conditions, simulated blood circulation was used with the possibility of changing pressure and frequency. The instrument was tested in the pressure range of 40-180 mm Hg at constant frequency of 72/min and frequency range of 36-130 beats per minute at constant pressure of 120 mm Hg. Test results showed that the instrument with certainty recognized a pulsating “blood vessel” in the expected pressure ranges and at different blood pump frequencies. Given the piezoelectric material’s very small dimensions and flexible form, it can be installed at the working end of most standard laparoscopic instruments and thus significantly increase certainty in the recognition of arteries during surgery, which would reduce the possibility of their injury or accidental ligation.

13. Utility of preoperative colonoscopy in localization of colorectal cancer

Abstract

Background: Erroneous localization of colorectal carcinomas puts the patients at risk for inappropriate use of adjuvant therapy, inadequate operative approach, prolonged surgery or missing the lesion. Material and Methods: A retrospective chart review was conducted on a total of 146 patients who had complete endoscopic and CT reports available and had undergone surgery for colon or rectal malignancies between January 2007 and January 2013. The purpose of this study was twofold: (a) to determine the accuracy of colonoscopy and CT scanning in localizing colorectal tumors and (b) to examine the utility of preoperative colonoscopy performed by surgeon. Results: Tumor location was accurately determined via colonoscopy in 80.5% of cases and erroneously in 19.5% of cases. CT scan confirmed colonoscopic localization in 59.6% of cases, and did not detect known tumors in 40.4% of cases. Of the tumors erroneously located by colonoscopy, 2.7% were accurately localized by CT scan. 48.4% of the patients with undetected tumors on CT scans, underwent second (preoperative) colonoscopy, performed by surgeon. The location of the tumor as determined by preoperative colonoscopy differed from the location noted on referral colonoscopy in 14.6% of patients. Tumor localization was determined correctly in all patients with preoperative colonoscopy and the difference in the accuracy of tumor localization between the patients which underwent two colonoscopies and those which did not, was statistically significant. The average size of the tumors not detected by CT was 2.98 cm. Large (>4 cm) tumors were rarely missed. Conclusions: Identifying localization of colorectal tumors by two complementary methods is essential before resection is undertaken. Preoperative colonoscopy, performed by surgeon, would decrease the likelihood of mislocalization of the small lesions detected on the first colonoscopy. No conflict of interest.
