41. T-06 Intérêt du dosage de l’adénosine désaminase (ADA) dans la tuberculose pleurale

Abstract

Objectifs: Évaluer l’utilité diagnostique du dosage pleural d’ADA en France. Patients et méthodes: Un ADA selon la méthode colorimétrique de Giusti (seuil de positivité > 24 UI/l) a été réalisé chez 85 patients admis de façon consécutive pour une pleurésie sérofibrineuse (critères de Light) entre le 1er janvier 2000 et le 31 décembre 2007. Les patients étaient classés en 2 groupes : groupe 1 : pleurésie tuberculeuse, groupe 2 : pleurésie non tuberculeuse. La sensibilité (Se), la spécificité (Sp) et les valeurs prédictives positives (VPP) et négatives (VPN) étaient respectivement calculées avec un seuil d’ADA fixé à 50 UI/l. Résultats: Les diagnostics étiologiques retenus chez les 85 patients étaient les suivants : 30 pleurésies tuberculeuses, 55 pleurésies non tuberculeuses (purulentes n = 23, parapneumonique n = 14, néoplasiques n = 12, divers n = 6). Le taux médian d’ADA du groupe des pleurésies tuberculeuses était significativement plus élevé que celui du groupe des pleurésies non tuberculeuses (132 vs 15 UI/l ; p < 0,05). 4 faux positifs étaient mis en évidence (3 pleurésies purulentes, 1 lymphome). Aucun faux négatif n’était individualisé. Les performances diagnostiques globales étaient les suivantes : Se 100 %, Sp 90 %, VPP 88 % et VPN 100 %. Discussion: En dépit de performances diagnostiques élevées (sensibilité et spécificité > 93 %) validées en zone de forte endémie tuberculeuse, le dosage de l’ADA pleurale n’est pas recommandé par les conférences de consensus américaines (2000) et françaises (2004). Notre étude confirme son excellente sensibilité. La spécificité altérée par la constatation de faux positif dans les pleurésies purulentes est en pratique améliorée par la prise en compte de la cytologie du liquide pleural et des données microbiologiques. Devant une pleurésie lymphocytaire, sa forte VPN permet d’éliminer une origine tuberculeuse même en zone de faible prévalence. [Copyright &y& Elsevier]

Abstract

Current chemotherapy-induced nausea and vomiting management guidelines recommend taking into account the emetogenic potential of the chemotherapy employed as well as individual risk factors to such effects. We performed an interventional prospective study to assess the impact of an innovating therapeutic optimization strategy. The latter combines current guidelines application to a specific consultation in order to individualize the treatment. This study included 170 patients and covered a total of 1,746 days of various chemotherapies. Among these patients, 86.5% never vomited and 53.8% never had any nausea or vomiting. These results seem generally better than the ones found in the literature with all kinds of chemotherapies. Regarding them, we have attempted to highlight the determining criteria for a successful antiemetic treatment.

Abstract

Introduction: An acute viral pericarditis may reveal a congenital pericardial abnormality., Case History: We report the case of a young man of 29 years in whom the development of rapidly progressive dyspnoea and fever led to the echocardiographic diagnosis of a pericardial tumour. The thoracic CT scan showed a mass arising in the superior mediastinum with no evidence of spread. Surgical exploration allowed the excision of a soft mutilobular mass adherent only to the aorta. Histological examination revealed an intrapericardial bronchogenic cyst., Conclusion: After a review of bronchogenic cysts we point out the properties of this rare intra-pericardial localisation, one of which is the frequently observed secretion of CA 19-9.
