Dapsone has been used in the treatment of bullous pemphigoid (BP) for over 50 years. According to the 2015 European Recommendations, the treatment of choice in BP are potent topical corticosteroids. In this article, we present data from the literature on the mechanism of action, efficacy and safety of dapsone in BP. After an extensive search of the PubMed database, we identified one systemic review and 7 nonrandomized clinical trials assessing the efficacy of dapson in BP. Data from the literature indicate that monotherapy with dapsone may be effective in 20-45.4% of BP cases. Combined treatment with dapsone and topical potent corticosteroids increases the efficacy up to 63-76%. The combination of oral corticosteroids and dapsone induces remission in 92-100% of cases. According to many authors, dapsone may be an effective and safe corticosteroid- sparing drug. Still there is a need for randomized controlled trials assessing the effectiveness of different treatment options in BP cases. [ABSTRACT FROM AUTHOR]

The article presents a case study of a 17-year-old girl with endogenous hypercortisolism. She was diagnosed with primary pigmented nodular adrenocortical disease (PPNAD) in early and predominant symptoms associated with anxiety-depressive disorders. She was administered with antidepressant drug for endogenous hypercortisolism. She underwent total adrenalectomy to withdraw of all symptoms of Cushing's syndrome.

The relationship between the eye and psoriasis has been recognized for decades, but the precise ocular manifestations in patients with psoriasis and psoriatic arthritis which occur in approximately 10% of patients are only recently coming to light. Psoriatic eye changes include conjunctivitis, dry eye, and uveitis. Uveitis has been reported more frequently in patients with arthritis (7-25%), but sometimes in psoriatic patients without arthritis. Dermatologists have to remember about side effects of the therapy. Care should be taken when prescribing corticosteroids because of corticosteroid induced glaucoma, retinoids for ocular manifestations including photophobia, conjunctivitis, keratitis, and dry eye. PUVA therapy does not substantially increase risk of cataract, while psoralens and corticosteroid therapy can have an influence on the development of opacification. Therefore during PUVA therapy patients should use eye protection to prevent increasing risk of cataract and corneal lesions. Untreated ocular disease may have serious consequences, therefore dermatologists should be aware of the possibility of eye changes in psoriatic patients. We recommend regular surveillance of these patients for visual changes and eye symptoms. Collaboration between ophthalmologists and dermatologists is essential to optimize disease management. Asthma is a common disease and its incidence increases around the world, in both developed and developing countries. There are, however, effective medicines, limiting the possibility of the development of exacerbations. The most important drugs are inhaled glucocorticosteroids, preferably used in combination therapy with LABA and also in the form of combination preparations. Another group of drugs - antileucotrienes confirmed the validity of their use in combination therapy with inhaled glucocorticosteroids. Shall also apply biologic drugs. Introduction. Chronic hand dermatitis (CHD) is a common, chronic disease and is associated with significant loss of quality of life. Due to different etiologies, no standard commonly accepted therapy is available. The first-line treatment, topical corticosteroids, is not always fully effective. Recently alitretinoin proved to be very promising therapy for chronic hand dermatitis, however, it is still not readily available. Objective. Presentation of the results of therapy with local PUVA in refractory, long-lasting chronic hand dermatitis. Material and methods. Fifty-four patients (39 females and 15 males) aged from25 to 75 years with long-lasting (mean 7.7 years) CHD refractory to the previous standard therapy were included. PUVA therapy was performed with orally administered 8-methoxypsoralen (∼0.8 mg/ kg) or after topical application of 0.1% Oxsoralen ointment. Irradiated with UVA were only affected body areas (palms/palms and soles). Initial UV doses ranged, depending on the way of psoralen application, from 0.5 J/cm² to 1.0 J/cm², and were increased every 2 to 5 irradiations by 0.25 J/cm². The therapy was conducted 3 times a week until a satisfactory result was obtained. Ten patients were additionally treated with acitretin (20-30 mg/day). Results. Complete or almost complete clearing of skin lesions was obtained in 24 patients (44.4%). In a further 15 patients there was observed significant improvement; thus in 48 (88.9%) patients a satisfactory effect of the therapy was noted. Addition of acitretin enabled very good results to be obtained for these patients. Significant side effects were observed only after topical application of psoralen. They corresponded to typical adverse effects of PUVA, i.e. mild to moderate phototoxic reaction, but they did not lead to discontinuation of the therapy. All patients were very happy with the treatment option used. Conclusions. Local PUVA seems, at the moment, the most effective treatment option for chronic hand dermatitis. [ABSTRACT FROM AUTHOR]

Introduction: Granuloma annulare is a benign chronic skin disease with unknown etiology. Aim of the study was the clinical characterisation of the disseminated form of granuloma annulare, including also comorbidities and applied therapies. Material and methods: A total of 53 patients with the disseminated form of granuloma annulare were included into the study. Based on the available medical documentation we assessed the clinical morphology and localization of skin lesions, coexistence of systemic diseases and treatment modalities used by the patients. All results were analyzed statistically. Results: Granuloma annulare was more often found in women (women/men ratio: 3.1:1). The highest occurrence of the disseminated form of granuloma annulare was found in the sixth decade of patients' life. Papules (39.6%) and erythemas (22.6%) were the most commonly observed types of skin lesions. They were most commonly located on the lower extremities (77.4%), upper extremities (69.8%) and the trunk (62.3%). Arterial hypertension and diabetes were the most commonly found comorbidities in the analyzed patients (32.1% and 20.8%, respectively). Among the various treatment options, topical corticosteroids and photo- or photochemotherapy were most frequently used (in 77.4% and 56.6% of patients, respectively). Conclusions: The disseminated form of granuloma annulare occurs more commonly in women and in patients older than 50 years. The morphology of skin lesions in granuloma annulare may vary sometimes making the correct diagnosis difficult. [ABSTRACT FROM AUTHOR]

Introduction: Nephrotic syndrome (NS) seems to be the most common manifestation of childhood primary glomerulopathies. In childhood the treatment of NS is based on corticosteroids. About 80% of patients show positive response to the therapy, the remaining 20% become steroid-resistant. Aim of the study: The aim of the study was to compare a group of steroid-sensitive and steroid-resistant children, with new-onset NS, to find out clinical features differentiating both groups prior to corticosteroid therapy. Material and methods: Retrospective chart analysis was done to find out children with new-onset NS admitted to the Division of Pediatric Nephrology in Zabrze between 1986-2005. According to the response to initial corticosteroid therapy, patients entered either steroid-sensitive or primary steroid-resistant group. Certain clinical features in both groups were compared. Results: 178 children entered the study: 44 (24.7%) steroid-resistant, 134 (75.3%) steroid-sensitive. There was a significant difference in age at onset: median and interquartile range for steroid-resistant patients was 5.6 (2.2-11.7) years vs steroid-sensitive patients - 2.7 (2.0-4.0) years (p=0.0005). There were no differences in the frequency of hypertension and impaired renal function in both groups. The only difference in clinical characteristics was significantly higher frequency of hematuria: 28 (63.6%) vs 43 (32%) (p=0.0002) and significantly higher concentration of blood urea nitrogen (BUN): 5.6 (3.8-9.0) mmol/l vs 4.6 (3.2-6.8) mmol/l (p=0.02) in steroid-resistant patients at onset. Conclusions: Primary steroid-resistant NS is characterized by significantly higher age at onset, higher frequency of hematuria and higher concentration of BUN at presentation. [ABSTRACT FROM AUTHOR]

Psoriasis is a chronic inflammatory skin disease affecting about 2-3% of general population. Within the last years better understanding of psoriasis etiology resulted in rapid development of new and effective treatment modalities, mainly biological drugs. In the era of biologics, however, topical therapy still represents a valuable method of psoriasis treatment, especially in patients with milder disease. Also in patients who require systemic therapy, topical treatment may be of interest, as it may shorten the time necessary to obtain clinical improvement and reduce the cumulative dose of systemic drugs. Here, we have reviewed data about the use of fixed combination of calcipotriol and betamethazone dipropionate as the topical treatment of psoriasis. Taking into account available results of clinical trials it seems that the fixed combination of calcipotriol and betamethasone dipropionate should be recommended as a first line treatment in patients with psoriasis requiring topical therapy. Sarcoidosis is an idiopathic granulomatous disease. Many organs could be involved, but in majority of cases sarcoidosis affects lung and intrathoracic lymph nodes. Pulmonary fungal infections are the important problem in immunocompromised patients. We describe a case of 47-year-old woman with sarcoidosis. The patient was treated many years with corticosteroids and had secondary cavity recognized 2 years ago. Main symptoms were high fever, intensive cough with sputum, weigh loss and hemoptysis. After unsuccessful treatment with antibiotics we recognized candidiasis and semiinvasive aspergillosis. Bronchoscopy showed typical picture of bronchi candydiasis. CT revealed aspergilloma in cavity and positive sputum culture and improvement after treatment confirmed diagnosis. [ABSTRACT FROM AUTHOR]

Lyell's syndrome (toxic epidermal necrolysis) -- is a severe life-threatening disease of the skin and mucous membranes, demonstrating the extensive loss of epidermis, often developing after exposure to certain medications. Skin lesions are similar to those found in burn disease. A Nikolsky sign is easily demonstrated by applying lateral pressure to bullae. Lesions may include mucous membranes of the mouth, gastrointestinal tract, genitals, conjunctiva and epithelium of the respiratory tract. Mortality of Lyell Syndrome is 30-40%. Recent reports demonstrated the efficacy of treating a disease with cyclosporine A. We present the case of 32-year-old woman admitted to the Department of Skin Diseases of Provincial Hospital in Poznan with diagnosis of the Lyell Syndrome. On admission lesions of mucous membranes of the mouth, genitals and conjunctiva were present. Widespreading erythematous macules appeared over a period of hours. Skin lesions underwent dynamic progression. Epidermolysis was observed in large areas due to rupture of large blisters (Nikolsky sign positive). The intravenous treatment with high doses of corticosteroids and cyclosporine A was introduced. The patient has remained under the care of the Dermatology Clinic. The follow up revealed few hyperpigmented spots, patient's general condition was good, there was no evidence of side effects. Early implementation of immunosuppressive therapy supported by general multidirectional treatment resulted in complete withdrawal symptoms of the disease. [ABSTRACT FROM AUTHOR]

Psoriasis vulgaris is one of the most common skin conditions, that causes significant morbidity and impairment of quality of life. The mainstay treatment for the majority of the patients is topical therapy, but can be complex and time consuming. A rising first-line treatment modality for psoriasis vulgaris is the two-compound ointment containing calcipotriol and betamethasone dipropionate -- vitamin D analog and a potent corticosteroid. This innovative formulation preserves the activity and bioavailability of the two components, and many multicenter clinical studies encompassed more than 6000 patients have demonstrated that It has a greater tolerability, efficacy, and a rapid onset of action compared with its individual ingredients or tacalcitol. A 30-year-old male patient has been reported, who was admitted because of multiple, symmetric, confluent brownish papules localized primarily on the shoulders, and gradually extended to the arms, back and front of chest. Lesions developed over a period of 10 years and were accompanied by severe pruritus. Based on the clinical presentation and the results of histological examination, showing positive staining for amyloid, a lichen amyloidosus was diagnosed. The measurement of nitric oxide is a simple and noninvasive diagnostic test. In patients with asthma the increase in concentration of exhaled nitric oxide (FENO) has been demonstrated. In these patients FENO correlates well with other inflammatory process indicators related to eosinophilic infiltration; it is a sensitive index to inflammatory process, quickly responding to treatment or exacerbation of a disease. It should be emphasized that FENO is particularly useful in monitoring the therapy with the use of inhaled corticosteroids. The increased concentration of FENO in patients with allergic rhinitis might be associated with the higher risk of asthma. The elevated values of FENO in patients with chronic obstructive pulmonary disease may indicate coexisting asthma. The measurement of exhaled nitric oxide should be used more extensively in diagnostics and monitoring of the treatment of patients who suffer from respiratory diseases, especially those with asthma. Atopic dermatitis (AD) is a chronic, recurrent pruritic skin disease. Its pathogenesis is complicated with the contribution of immunologic and non-immunologic factors. Ectodermal defect in patients with AD is genetically conditioned and consists in the presence of skin barrier dysfunction. That is why the main elements of topical treatment in AD are agents restoring the skin barrier and anti-inflammatory preparations, both equally important. Atopiclair® is non-steroid topical preparation with anti-inflammatory and cytoprotective properties. It is a hydrolipid cream containing glycyrrhetinic acid, vitis vinifera extract, telmestine, piroctone olamine, shea butter and hyaluronic acid. The main indication for that preparation is mild to moderate atopic dermatitis. The results of available clinical trials show that Atopiclair® significantly reduces itch, inflammation and severity of skin changes in the course of AD, as well as reduces the need of corticosteroid applications. The preparation is well tolerated and has a good safety profile. It can be used even in the youngest patients. It seems that Atopiclair® stays between emollients and mild or moderate topical steroids in the therapeutic approach to AD. [ABSTRACT FROM AUTHOR]

Epidermal nevi are rare congenital lesions of ectodermal origin characterized by the certain deployment of a quantity or composition of normal skin elements (hamartoma). Genetic defect and pathogenesis are mostly unknown. Lesions usually locate on the trunk or the limbs along the Blaschko lines but are meet also on the face and neck. Nevi usually grow with the patient and are solid, without yielding to change, and regression. Rare cases with the development of benign or malignant tumors are described. In some patients lesions may require surgical removal for cosmetic reasons. Excision, coagulation, cryotherapy, laser therapy, treatment with topical retinoids and keratolitic ointments give periodic improvement. Treatment of nevi with inflammatory sings includes the addition of the following: steroid ointments, intralesional corticosteroid injections, takrolimus ointment. In the presence of widespread nevi orall retinoids can be used (side effects!). In some cases it is necessary to do the specific diagnostics and multispecialistic treatment because the nevi may be accompanied by a variety of signs -- epidermal nevus syndroms. These are rare congenital disorders of the nerveous system and the skin characterized by the presence of epidermal nevi in combination with cerebral, ocular, bone, renal, less frequent cardiac changes. Depending on the clinical picture and the genetic ground various syndroms are distinguished: sebaceous nevus, comedonicus nevus, Becker nevus, Phacomatosis pigmentokeratotica, Proteus and congenital hemidysplasia, unilateral ichtyosiform erythroderma and limb defects syndrome (CHILD syndrome). In view of the possible neurological, orthopedic, ocular and internal organs complications children with epidermal nevi syndroms should be covered with the specific diagnostics including CNS imaging studies, the skeleton, internal organs and ocular examinations. Introduction: Primary cutaneous T- cell lymphomas (CTCL) comprises a heterogeneous group of non-Hodgkin's lymphomas, accounting for two-thirds of all cases of cutaneous lymphoma. The majority of CTCL is mycosis fungoides (MF). CTCL have been characterized by clinical and pathological differences what plays a pivotal role in the treatment efficacy and prognosis. Aim of the study: The retrospective analysis of the treatment results of patients with CTCL treated in Department of Dermatology Medical University in Gdañsk between 1997 and 2008 with reference to WHO-EORTC 2006 recommendations. Material and methods: We analyzed the files of 60 patients with cutaneous T-cell lymphomas and parapsoriasis en plaque between 17 and 94 years old (sex ratio men to women was 0,96:1). The treatment was strictly adjusted to the stage of disease and general condition of patient. We also took into consideration the possible side effects of each used method. We modified the therapeutic approach in case of relapses with reference to the current recommendations. Results: Very good results of treatment with topical corticosteroids and PUVA-therapy in early stages of mycosis fungoides were observed. Besides we had satisfactory experience in almost all cases with X- ray radiation (tumoriform stage of the disease) and metothrexate (all stages of CTCL). The systemic chemotherapy (2-Chlorodeoxiadenosine, 2-CdA) was administered in patients in stage IIB as well as IVB. This aggressive approach didn't give finally satisfactory results in both groups. The results of treatment with bexaroten (MF IIB and III) seems to be promising in spite of dramatical side effects and the progression (to stage IV) observed in one case. Conclusions: Therapeutic approach in CTCL should be adjusted to the stage of the disease. According to our results we confirmed the conclusions of other authors that aggressive therapy in patients with intermediate stage of disease shouldn't be recommended because it doesn't improve the prognosis. On the other hand systemic chemotherapy extends the life-span just slightly in stages IVA and IVB of mycosis fungoides. We suppose that inefficacy of the treatment at our Department can be related to the deficiency of the latest agents such as immunomodulators, rexinoid: bexaroten in early stages of the disorder and TSEB (Total Skin Electron Beam Therapy). Resistance to glicocorticosteroids means lack of sensitivity of tissues to steroids which may be present as an inborn generalised form and as a secondary form caused by tissues pathology. During the treatment of lymphoproliferative diseases glucocorticosteroid resistance is a highly important prognostic factor. Reaction to steroid treatment may be associated with normal function of steroid receptor encoded by NR3C1 gene. The observed therapeutic effect may depend on the total amount of this receptor in a cell as well as on the existence of different receptor isoforms with higher or lower ability to activate transcription or on posttranscriptive modifications. These phenomena are probably not all of the influencing factors. However, it is crucial to determine thoroughly the relations between them in order to be able to use optimal treatment in pediatric patients with lymphoproliferative diseases in near future. The Global Initiative for Asthma (GINA) guidelines aim at improving asthma control and preventing future risk. For patients with moderate to severe asthma an inhaled corticosteroid (ICS) or an ICS/long-acting β2-agonist (LABA) combination with a short-acting β2-agonist (SABA) as reliever is recommended. Despite the availability of effective maintenance therapies, a large proportion of patients still fail to achieve guideline-defined asthma control, and overuse of SABA reliever medication at the expense of CS is commonly observed. New simplified treatment approaches may offer a solution and assist physicians to achieve overall asthma control. One such treatment approach, which is recommended in the GINA guidelines, is budesonide/formoterol for both maintenance and reliever therapy. This treatment strategy significantly reduces the rate of severe asthma exacerbations compared with ICS/LABA plus SABA and achieves equivalent daily symptom control compared with higher doses of ICS/LABA plus separate SABA for relief. These benefits are achieved at a lower overall steroid load, and budesonide/formoterol maintenance and reliever therapy is well tolerated in patients with moderate to severe asthma. Intratympanic injection of corticosteroids or gentamicin is an attractive approach to treatment of Méniere's disease. Corticosteroids injected into the inner ear are readily absorbed across the round window membrane to reach inner ear fluids at concentrations higher than those obtainable by oral or intravenous administration. Intratympanic corticosteroid treatment allows avoiding undesirable side effects that are often associated with systemic application of those drugs. Soon after its introduction, the intratympanic gentamicin therapy fell into disfavour because of the high rate of sensorineural hearing loss. Reductions in the frequency of intratympanic gentamicin injections have improved the preservation of hearing without compromising vertigo control. Otologists commonly use intratympanic treatment, but there is no standard protocol, either for corticosteroid or gentamicin intratympanic therapy. This article reviews the basic information on the pharmacokinetics, treatment protocols and complications for both corticosteroid and gentamicin local therapy in Méniere's disease. Asthma exacerbations can be defined as episodes of progressively worsening of symptoms such as dyspnea, cough and wheezing with concomitant deterioration of the expiratory airflow. The main goals of therapy are reversal of bronchoconstriction and prevention of hypoxemia. The most effective bronchodilators are selective beta2-agonists with rapid onset of action. Treatment of asthma attacks should be introduced without any delay. Measurement of peak expiratory flow rate (PEF), which is the best objective parameter evaluating severity of asthma exacerbation and response to therapy, should be performed before beta2-agonists have been introduced. The preferred way for delivery of beta2-agonists is either by nebulization or by metered dose inhaler equipped with a spacer. Beta2-agonists are administered at high doses, which are several folds greater than dose used routinely by a patient. In severe asthma exacerbations corticosteroids should be introduced. They are administered either orally or parenterally. In a small proportion of patients addition of anti-cholinergic medication is associated with significant improvement of lung function. Improvement in lung function evaluated as increase in PEF after the first hour of intensive therapy is considered the best prognostic factor. [ABSTRACT FROM AUTHOR]

The results of the GAPP (Global Asthma Physician and Patient Survey) study aimed to estimate the asthma perception by the adult asthmatics and their doctors were presented and analysed. GAPP survey including 16 countries from all over the world took into account mainly the patients' anxiety about the side effects of corticosteroid therapy. Introduction: One of the recent breakthroughs in the treatment of refractory uveitis is biologic therapy. Biologic drugs have a more powerful immunosuppresive influence upon the course of the disease than conventional immunosuppressants. Aim of the study is the evaluation of infliximab therapy in idiopathic chronic uveitis. Case report: Biologic therapy was implemented in a 12-year-old patient with idiopathic chronic uveitis and concomitant progressive deterioration of vision. The child had a history of unaffected therapy with corticosteroids and immunosuppressant drugs. The infiximab was used according to the established protocol for autoimmune diseases. During the course of treatment not only the vision acuity was stabilized and organ of vision improved, but intraocular pressure was normalized as well. During the follow-up no recurrence of uveitis occurred. There were no adverse effects of biological therapy in our patient. Conclusions: Biologic therapy appeared to be an effective approach to the childhood sight-threatening chronic uveitis. [ABSTRACT FROM AUTHOR]

Background: We present a case of anaphylactic shock induced by celery ingestion in a 28-year old woman with pollinosis during allergen (50% birch, 50% grass) immunotherapy. Case Report: A female patient, aged 28 was admitted to the clinic due to a serious anaphylactic reaction. The event took place 15 min after ingesting fresh celery. She recovered after routine treatment with adrenaline, corticosteroids and antazoline. Conclusions: Our case shows the possibility of simultaneous occurrence of hypersensitivity to inhaled allergens and food. P-phenylodiamine is a dye called "black henna". During the last few years the increase of allergy to this allergen is observed. The higher frequency of contact dermatitis diagnosis to corticosteroids topical drugs is also an alarming phenomenon. The authors present the case of a 9-year-old boy with tree pollen allergy diagnosis. Allergy to p-phenylodiamine complicated by allergy to topical corticosteroids was diagnosed only after a few-years treatment at specialist centers and after 4 hospitalizations. Dermatophyte fungal infections of the skin are the most common clinical entities in dermatological practice, although it sometimes represent a significant diagnostic and therapeutic problem. T. rubrum, belonging to the antropophilic dermatophytes, is the most common isolated pathogen in human mycoses of skin and nails in Poland. Usually causes athlete's foot and onychomycosis, rarely fungal infection of hands. The specificity of tinea corporis caused by T. rubrum is the presence of multiple changes on the trunk. We present a case of 66-year-old man with type II diabetes and receiving injections of corticosteroids due to joint pain, in whom extensive development of fungal infection of the skin was observed. The disease initially was mistakenly diagnosed as diffuse eczema and treated with corticosteroids. We suggest to performe mycological examination in each case with not clear clinical picture to rule out fungal infection and avoid erroneous therapeutic intervention. [ABSTRACT FROM AUTHOR]
