The progressive increase in HIV infection among older adults requires constant research and monitoring, given that geriatric syndromes associated with HIV comorbidities have become an important public health problem. We reported this systematic review according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), and it has a central question: Is the incidence of cognitive impairment higher in older patients living with HIV than in their seronegative peers? The following databases were searched for this review: MEDLINE/PubMed, EMBASE, LILACS, Web of Science, and Scopus. The inclusion criteria were studies whose samples were ≥ 50% patients aged ≥ 50 years, with and without HIV, and a main outcome related to the incidence of cognitive impairment. Only cohort studies with follow-up lasting ≥ 24 months were considered. Three reviewers independently screened the documents for eligibility criteria, extracted the data, assessed the risk of bias (Newcastle-Ottawa Scale), and evaluated the quality of evidence. A narrative synthesis was prepared. In total, 10 798 trials were screened, 8884 were excluded, 14 were analyzed, and 5 were included in this review. Only 1 applied cognitive assessment tests; the rest used secondary data from the medical records. Most found that the incidence of cognitive disorders was higher among older people living with HIV, which highlights the need for public policies aimed at primary and secondary prevention strategies. Recorrendo a uma escrita seca e fragmentária, Gonçalo M. Resumo Fundamento: O no-reflow (NR) é caracterizado por uma redução aguda no fluxo coronário que não é acompanhada por espasmo coronário, trombose ou dissecção. O índice prognóstico inflamatório (IPI) é um novo marcador que foi relatado como tendo um papel prognóstico em pacientes com câncer e é calculado pela razão neutrófilos/linfócitos (NLR) multiplicada pela razão proteína C reativa/albumina. Objetivo: Nosso objetivo foi investigar a relação entre IPI e NR em pacientes com infarto do miocárdio com supradesnivelamento do segmento ST (IAMCSST) submetidos a intervenção coronária percutânea primária (ICPp). Métodos: Um total de 1.541 pacientes foram incluídos neste estudo (178 com NR e 1.363 com refluxo). A regressão penalizada LASSO (Least Absolute Shrinkage and Select Operator) foi usada para seleção de variáveis. The ergogenecity of caffeine (CAF) and pre-exercise music have been studied extensively in male and mixed-gender groups, but there is limited information on their synergistic effects in females. This study assessed the effects of combining a low dose of CAF with warm-up music on taekwondo specific performance, perceived exertion (RPE), and psychological aspects in female athletes. In a double-blinded, randomized, placebo-controlled crossover study, 16 female taekwondo athletes (M ± SD; age: 17.69 ± 0.60 years) performed the taekwondo-specific agility test (TSAT), 10 s frequency speed of kick test (FSKT-10s) and its multiple version (FSKT-mult) under the following conditions: 1) no supplement with music (NS+M); 2) no supplement without music (control); 3) CAF without music (CAF+NM), 4) placebo without music (PL+NM), 5) CAF with music (CAF+M), 6) PL with music (PL+M). After each test, athletes rated their RPE, feeling scale (FS), felt arousal scale (FAS) and physical enjoyment (PACES). CAF+M induced better physical performance than other conditions on the FSKT-10s, FSKT-mult, and more desirable psychophysiological responses including RPE post_TSAT, RPE post_FSKT-10s, PACES post_FSKT-10s, FS, FAS post_warm-up, FAS post_FSKT-10s and FAS post_FSKT-mult (all p < 0.05). Except for PL+M, CAF+M also elicited more desirable responses than other conditions for FAS post_TSAT (p < 0.05). Furthermore, CAF+M and CAF+NM conditions enhanced TSAT, PACES post_TSAT, RPE post_FSKT-mult greater than the other conditions (p < 0.05). Preceding warm-up music stimulus with a low dose of CAF may be a more efficient strategy to enhance physical performance and affective valence in female taekwondo athletes compared with using either strategy in isolation.

Resumo Fundamento: A isquemia com artéria coronária não obstrutiva (INOCA) é uma doença cardíaca isquêmica que inclui principalmente disfunção microvascular coronariana e/ou vasoespasmo coronariano epicárdico devido à disfunção vascular coronariana subjacente e pode ser observada mais comumente em pacientes do sexo feminino. O índice de inflamação imunológica sistêmica (SII, relação plaquetas × neutrófilos/linfócitos) é um novo marcador que prediz resultados clínicos adversos na doença arterial coronariana (DAC). Objetivo: Este estudo tem como objetivo investigar a relação entre INOCA e SII, um novo marcador associado à inflamação. Métodos: Um total de 424 pacientes (212 pacientes com INOCA e 212 controles normais) foram incluídos no estudo. ABSTRACT Objective: To determine the role of the IL8 rs4073 polymorphism in predicting the risk of central nervous system (CNS) toxicity in patients receiving standard pharmacological treatment for multidrug-resistant tuberculosis (MDR-TB). Methods: A cohort of 85 consenting MDR-TB patients receiving treatment with second-line antituberculosis drugs had their blood samples amplified for the IL8 (rs4073) gene and genotyped. All patients were clinically screened for evidence of treatment toxicity and categorized accordingly. Crude and adjusted associations were assessed. Results: The chief complaints fell into the following categories: CNS toxicity; gastrointestinal toxicity; skin toxicity; and eye and ear toxicities. Symptoms of gastrointestinal toxicity were reported by 59% of the patients, and symptoms of CNS toxicity were reported by 42.7%. With regard to the genotypes of IL8 (rs4073), the following were identified: AA, in 64 of the study participants; AT, in 7; and TT, in 11. A significant association was found between the dominant model of inheritance and CNS toxicity for the crude model (p = 0.024; OR = 3.57; 95% CI, 1.18-10.76) and the adjusted model (p = 0.031; OR = 3.92; 95% CI, 1.13-13.58). The AT+TT genotype of IL8 (rs4073) showed a 3.92 times increased risk of CNS toxicity when compared with the AA genotype. Conclusions: The AT+TT genotype has a tendency to be associated with an increased risk of adverse clinical features during MDR-TB treatment.

Abstract Objective It was aimed to compare visceral adiposity index (VAI) levels in patients with normal bone mineral density (BMD), osteopenia, and osteoporosis. Methods One hundred twenty postmenopausal women (40 with normal BMD, 40 with osteopenia, and 40 with osteoporosis) between the ages of 50 to 70 years were included in the study. For females, the VAI was calculated using the formula (waist circumference [WC]/[36.58 + (1.89 x body mass index (BMI))]) x (1.52/High-density lipoprotein [HDL]-cholesterol [mmol/L]) x (triglyceride [TG]/0.81 [mmol/L]). Results The time of menopause from the beginning was similar in all groups. Waist circumference was found to be higher in those with normal BMD than in the osteopenic and osteoporotic groups (p = 0.018 and p < 0.001, respectively), and it was also higher in the osteopenic group than in the osteoporotic group (p = 0.003). Height and body weight, BMI, blood pressure, insulin, glucose, HDL-cholesterol, and homeostasis model assessment-insulin resistance (HOMA-IR) levels were similar in all groups. Triglyceride levels were found to be higher in the normal BMD group, compared with the osteoporotic group (p = 0.005). The level of VAI was detected as higher in those with normal BMD, compared with the women with osteoporosis (p = 0.002). Additionally, the correlation analysis showed a positive correlation between dual-energy X-ray absorptiometry (DXA) spine T-scores, WC, VAI, and a negative correlation between DXA spine T-scores and age. Conclusion In our study, we found higher VAI levels in those with normal BMD, compared with women with osteoporosis. We consider that further studies with a larger sample size will be beneficial in elucidating the entity.

Objectives: This review will determine whether the incidence of cognitive impairment in HIV patients aged ≥ 50 years is greater than that of their HIV-negative peers. It will include studies that evaluate seropositive patients compared to and an unexposed control group. Study design: Cohort studies with follow-up ≥ 24 months will be included. Three reviewers will independently screen for eligibility criteria, extract data, and assess the risk of bias in the included studies, as well as evaluate the overall quality of evidence. A narrative synthesis will be prepared according to synthesis without meta-analysis guidelines. Expected results: We expect to find correlations between older age, HIV, and cognitive impairment. Relevance: The association of geriatric syndromes and HIV is becoming an important field of study. Increased life expectancy accompanied by an active sex life is contributing to this public health problem. Abstract One-year-old transplants of ‘Olinda Valencia orange’, ‘Murcott Tangerine’, and ‘Limoneira 8A Lisbon’ genotypes were grafted on ‘Macrophylla’ rootstock and subjected, along with ‘Macrophylla’ rootstock, to 24 h of the night chilling (nocturnal) at 20/4 °C±1 (day/night) for three days, (8 h per day), followed by three days of recovery at 30/25 °C±1 (day/night). The results showed that leaf diffusion resistance (LDR), proline, malondialdehyde (MDA), peroxidase (POX), and superoxide dismutase (SOD) increased significantly in all genotypes, while there was a significant decrease in photosynthetic apparatus, polyphenol oxidase (PPO), and catalase (CAT) under nocturnal chilling stress. Under the chilling, Macrophylla rootstock had the highest concentrations of proline and SOD, whereas it had the lowest MDA and total chlorophyll concentrations. Noticeably, the enzymatic antioxidant profiles were enhanced after being recovered. Moreover, ‘Murcott’ had the highest LDR, MDA concentration, thickness of lamina, midvein, palisade tissue, spongy tissue, and dimension of midvein vascular bundle. Meanwhile, it had the lowest stomatal conductance (GS) and transpiration rate (E). Similar results were obtained with ‘Valencia’ genotype. In contrast, the ‘Limoneira 8A’ genotype under the chilling had the lowest levels of LDR, antioxidant enzymes (POX, CAT, and PPO), thickness of lamina, midvein, palisade tissue, spongy tissue, and dimension of midvein vascular bundle.

Resumo O cacau é uma importante atividade sociocultural e económica em São Tomé e Príncipe (STP) e o maior contribuinte para o PIB e para as exportações do país. No entanto, os pequenos produtores ligados a esta cadeia de valor, pertencentes a duas cooperativas, enfrentam vários problemas e desafios relacionados à continuidade da produção de cacau biológico, com algumas vantagens relacionadas ao superior valor de mercado superior e sabor fino ou, à sua substituição por outras atividades mais produtivas e rentáveis. Este trabalho teve como objetivo analisar o comportamento face ao risco dos produtores de cacau biológico de STP no contexto das suas estratégias de subsistência, através da aplicação de um questionário a uma amostra de 230 famílias rurais. Os resultados mostraram não haver diferenças de governança entre as duas cooperativas. Um modelo teórico foi usado para determinar as relações entre as componentes de risco e as estratégias de subsistência. Os resultados mostram que as perceções de risco são mais importantes do que as atitudes de risco na decisão de estratégias de mitigação de risco. Além disso, as perceções desempenham um papel mediador entre as atitudes de risco dos agricultores e as estratégias de gestão de risco adotadas por eles. Estes resultados defendem medidas de política orientadas para as perceções de risco dos agricultores, a fim de implementar estratégias de gestão de risco bem-sucedidas.

Resumo Fundamento A hipertensão causa inflamação subendotelial e disfunção na aterosclerose resultante. A espessura média-intimal da carótida (EMIC) é um marcador útil de disfunção endotelial e aterosclerose. A razão ácido úrico/albumina (RUA) emergiu como um novo marcador para prever eventos cardiovasculares. Objetivo Nosso objetivo foi investigar a associação da RUA com a EIMC em pacientes hipertensos. Método Duzentos e dezesseis pacientes hipertensos consecutivos foram incluídos neste estudo prospectivo. Todos os pacientes foram submetidos a ultrassonografia de carótida para classificar baixos (EMIC < 0,9 mm) e altos (EMIC≥0,9 mm) grupos de EMIC. Pretende-se, a partir de Uma menina está perdida no seu século à procura do pai, de Gonçalo M. Tavares, aprofundar a compreensão sobre a noção de inefável e confirmar, a partir da leitura desse romance, a possibilidade de um dizer sem palavras capaz de expressá-lo. Assumindo que o inefável é um sentido (in)exprimível ao infinito e que fica por dizer (e significar) e por redizer (e ressignificar) de modo interminável (JANKÉLÉVITCH, 2018), não é possível convertê-lo em palavras. Por isso, começa-se por considerar o vazio inefável desse romance tavariano como uma forma decisiva para dar sentido aos sentidos que extrapolam à lógica da palavra escrita. Ao considerá-lo como forma determinante para a justa expressão do inefável, torna-se imprescindível deter-se sobre ele, pois à medida que é interpretado, amplia-se a compreensão do inefável verificado na obra. Introduction: Echocardiography (echo) is the primary non-invasive imaging modality for the assessment of congenital heart disease (CHD). Computed tomography angiography (CTA) also has potential to examine the anatomy of complex heart anomalies as well as extracardiac involvement. Objectives: The aim of this study is to determine the impact of new CTA technology in the diagnosis of CHD and to compare echo and CTA in terms of diagnostic accuracy. Methods: Forty-five patients who underwent preoperative echo and CTA assessment in the intensive care unit were included in this study. The results were assessed for three main types of CHD (cardiac malformations, cardiac-major vessel connections and major vessels). The main groups were also divided into subgroups according to surgical features in order to assess them more objectively. Imaging methods were compared for diagnostic accuracy, sensitivity and specificity, while surgical findings were accepted as the gold standard. Results: Patients’ median age and weight were two months (three days-eight years) and 12 kg (2.5-60 kg), respectively. In 45 operated cases, 205 subgroup malformations were assessed. Diagnostic accuracy was significantly greater in echo (echo vs. CTA: 98.4% and 96.2% [chi-square=6.4, p=0.011]). During surgery, 84 cardiac malformations (echo vs. CTA: 97.4% and 95.1% [chi-square=4.9, p=0.03]), 47 cardiac-major vessel connections (echo vs. CTA: 98.3% and 95.4% [chi-square=7.5, p=0.03]), and 74 major vessel malformations (echo vs. CTA: 96% and 98% [chi-square=1.8, p=0.48]) were confirmed. Conclusion: Echocardiography and CTA are imaging methods with high diagnostic accuracy in children with CHD. The use of echocardiography together with CTA, especially for the visualization of extracardiac anatomy, provides additional information for clinicians. Resumo: Introdução: A ecocardiografia é a principal modalidade de imagem não invasiva na avaliação das cardiopatias congénitas (CC). A angiotomografia computorizada (ATC) tem também potencial para examinar as anomalias morfológicas cardíacas complexas, assim como do envolvimento extracardíaco. Objetivos: Determinar o impacto de avanços tecnológicos da ATC no diagnóstico de CC e comparar a ecocardiografia e a ATC em termos de precisão diagnóstica. Métodos: Foram incluídos neste estudo 45 doentes submetidos a avaliação por ECO e por ATC pré operatória (n=45) em unidade de cuidados intensivos. Os resultados foram avaliados nos três principais tipos de grupos de CC (malformações cardíacas, conexões vasculares anómalas em nível cardíaco e malformações dos grandes vasos). Estes grupos foram também divididos em subgrupos de acordo com as características das cirurgias, de modo a serem avaliados mais objetivamente. Os métodos de imagem foram comparados para precisão diagnóstica, sensibilidade e especificidade, enquanto os resultados cirúrgicos foram aceites como gold standard. Resultados: A média da idade e do peso dos doentes foi de dois meses (3 dias-8 anos) e 12 kgt (2,5-60 kg), respetivamente. Em 45 casos operados, foram avaliadas 205 malformações de subgrupos. A precisão diagnóstica foi significativamente superior na ECO (ECO versus ATC; 98,4% & 96,2%, (X2=6,4, p=0,011). Durante as 84 intervenções, foram confirmadas 84 malformações cardíacas (ECO versus ATC; 97,4% & 95,1% (X2=4,9, p=0,03), 47 conexões vasculares anómalas em nível cardíaco (ECO versus ATC; 98,3 & 95,4% (X2=7,5, p=0,03), 74 malformações dos grandes vasos (ECHO versus ATC; 96% & 98% (X2=1,8, p=0,48). Conclusão: A ecocardiografia e a ATC são métodos de imagem com elevado rigor diagnóstico em crianças com CC. A utilização da ecocardiografia juntamente com a ATC, especialmente na avaliação da anatomia extracardíaca, fornece informação adicional para os clínicos.

Resumo Fundamento A nova doença por coronavírus (COVID-19) pode levar a uma enfermidade grave e causar a morte. Sabe-se que a COVID-19 afeta o sistema cardiovascular. A detecção precoce da progressão para um estágio grave da doença que afeta o sistema cardiovascular pode desempenhar um papel crítico no tratamento da COVID-19. Objetivos Explorar a possível relação entre a pneumonia por COVID-19 e os achados de strain do ventrículo direito no eletrocardiograma (ECG). Métodos Foi realizado um estudo retrospectivo de 141 pacientes hospitalizados com COVID-19. A correlação de Spearman e as análises de regressão logística foram aplicadas para avaliar as relações entre as manifestações de strain ventricular direito na ECG e os níveis de biomarcadores e outros achados laboratoriais e de imagem do tórax. O nível de significância foi considerado estabelecido como p < 0,05. Resultados Os sinais de ECG de estresse ventricular direito foram significativamente mais frequentes e os níveis de fibrinogênio, PCR e ferritina foram significativamente mais elevados em pacientes com COVID-19 com níveis elevados de hs-cTnI, procalcitonina e dímero-D. A análise univariada mostrou que existem relações significativas entre a presença de pneumonia bilateral, a maioria dos sinais eletrocardiográficos de strain ventricular direito e lesão cardíaca e biomarcadores inflamatórios e trombóticos. A análise multivariada revelou que o supradesnivelamento do segmento ST em V1 e padrão S1Q3T3 são preditores independentes de lesão cardíaca ( odds ratio =0,23; IC95%, 0,06 a 0,90; p=0,035) e níveis elevados de procalcitonina ( odds ratio =0,19; IC 95%, 0,06 a 0,62; p=0,006), respectivamente. Conclusão Os achados do presente estudo sugerem que a dano cardíaco direito é prevalente na COVID-19. Além disso, nosso estudo demonstra o valor clínico do ECG na avaliação e monitoramento de pacientes com pneumonia por COVID-19.

Objective: To determine the maximum bite force (MBF) in oral submucous fibrosis (OSMF) patients and to compare them with that of healthy subjects. Material and Methods: Twenty patients who were clinically confirmed, as OSMF and 20 healthy controls matched for age, gender, and number of intact functional teeth were included in this study. For each subject, age, gender, weight, height and body mass index (BMI) were recorded. The MBF registration was carried out by the two evaluators, who were previously calibrated. Bite force was measured in the first molar region using a force transducer occlusal force meter for each subject seated at the upright position, with Frankfort's plane nearly parallel to the floor, and no head support. The Student’s independent t-test was used to determine the statistical significance in relation to mean height, weight, BMI and the presence of number of intact teeth and MBF between the healthy subjects and OSMF individuals. A comparison of grades of OSMF with all variables was carried out by one-way ANOVA test. Results: No significant difference was found in mean age, mean height, weight, BMI and the presence of the number of intact teeth between healthy individuals and OSMF patients. The mean MBF in healthy subjects was 628.23 ± 24.39 N and 635.47 ± 31.22 N in OSMF patients. Even though the healthy subjects reported a higher MBF than OSMF patients did, the difference was statistically non-significant. With regards to sides, no significant difference was observed in mean MBF in healthy subjects and OSMF patients on the right (p=0.7818) and left side (p=0.6154). Conclusion: The healthy subjects reported higher MBF values than OSMF patients did and the difference was statistically non-significant.

Objective: To determine the frequency and characteristics of mesiodens in Indian school children. Material and Methods: 1232 radiographs of patients attending the Department of Oral Medicine and Radiology of the age group of 4-15 years studied for the present retrospective study. The intraoral periapical radiographs and occlusal radiographs of the premaxilla were examined to determine the presence of mesiodens. The data regarding the presence of mesiodens, number, position, location on the arch, shape and associated complications were recorded. Results: Eleven patients had 14 mesiodens, with a frequency of 0.8%. The ratio of boys to girls was 1.2:1. Maximum mesiodens observed in the age group of 7-9 years. Most of the mesiodens were conical in shape and found on the palatal side. Fifty percent of the mesiodens were unerupted. The majority of patients (57.1%) had vertically positioned mesiodens. Midline diastema was the most common complication due to mesiodens (66.7%). Conclusion: Routine check-up during the primary dentition and mixed dentition stages helps for early detection of mesiodens and thus preventing complications. Early identification is useful for planning comprehensive management, initiating proper consultation and referral for evaluation and treatment.

The aim of this study is to examine the relationship between body mass indexes and nutritional habits of national cycling athletes. In 2020-2021, 20 elite/national cyclists, including 10 sprinters and 10 hill climbers competing in the Turkey Cycling Spindle Team, participated voluntarily. The averages of age, height and body mass indexes of the cyclists participating in the study were respectively for sprinters; 23.40±2.68 years, 1.76±0.04 meters and 23.31±1.68 kg/m2, while for hill climbers it is 24.10±3.64 years, 1.76±0.05 meters and it was determined 21.39±1.02 kg/m2. The data obtained in the study were analyzed in computer environment. Spearman Correlation analysis was applied for the research results. In the study, it was seen that the BMI values of the athletes were normal and the sub-headings of their eating habits were above the average. In addition, no relationship was found between BMI and eating habits and total scores.

Introduction: Structural and electrophysiological changes play a critical role in the development of atrial fibrillation (AF). Although the pathophysiology of paroxysmal AF (PAF) has not been fully elucidated, oxidative stress (OS) and DNA damage appear to be important triggers. Thus far, no studies have investigated the relationships among total oxidant status (TOS), DNA damage, and PAF. The goal of this study was to assess TOS and DNA damage in patients with PAF. Methods: This cross-sectional study included 56 patients with PAF and 31 healthy controls. OS was assessed based on TOS, total antioxidant capacity (TAC), and oxidative stress index (OSI). The level of DNA damage was assessed using 8-hydroxy-2′-deoxyguanosine (8-OHdG). Results: There were no significant differences between the groups in terms of baseline characteristics. However, patients with PAF had significantly higher high-sensitivity C-reactive protein (p=0.018), TOS (p=0.001), OSI (p=0.001), and 8-OHdG (p=0.019) levels, compared with the control group. Multivariate logistic regression analysis showed that serum TOS level (odds ratio: 1.608; 95% confidence interval [CI]: 1.188-2.176, p=0.002) was the only independent predictor of PAF. ABSTRACT Background: Roux-en-Y gastric bypass (RYGB) has been the choice of bariatric procedure for patients with symptomatic reflux - and is known to be effective in reducing the need for anti-reflux medication postoperatively. However, a small number of RYGB patients can still develop severe reflux symptoms that require a surgical intervention. Aim: To examine and describe the patient population that requires an anti-reflux procedure after RYGB evaluating demographics, characteristics, symptoms and diagnosis Methods: A retrospective chart review was performed on 32 patients who underwent a hiatal hernia repair and/or Nissen fundoplication after RYGB Jul 1st, 2014 and Dec 31st, 2019. Patients were identified using the MBSAQIP database and their electronic medical records were reviewed. Results: Most patients were female (n=29, 90.6%). The mean age was 52.8 years and the mean body mass index (BMI) was 34.1 kg/m2 at the time of anti-reflux procedure. Patients underwent the anti-reflux procedure at a mean of 7.9 years after the RYGB procedure. The mean percentage of excess BMI loss during the time between RYGB and anti-reflux procedure was 63.4%. Conclusions: Female patients with a significant weight loss may develop a severe reflux symptoms years after RYGB. Complaints of reflux after RYGB should not be overlooked. Careful follow-up and appropriate treatment (including surgical intervention) is needed for this population.

Objective: To investigate the association of sluggish cognitive tempo (SCT) with autistic traits (ATs) and anxiety disorder symptoms among children with attention-deficit/hyperactivity disorder (ADHD). Methods: A total of 195 children with a DSM-5 diagnosis of ADHD were included. The Barkley Sluggish Cognitive Tempo Scale (BSCTS) was used to measure SCT symptoms. Other study measures included the Autism Spectrum Quotient (AQ), Screen for Child Anxiety and Related Disorders (SCARED), Turgay DSM-IV Disruptive Behavior Disorders Rating Scale (T-DSM-IV-S), and Conners’ Teacher Rating Scale (CTRS-R). Results: The frequency of SCT was 30.3% (n=59) in the whole group. Those with SCT had higher total AQ and SCARED scores. Significant associations and correlations were also found between SCT and certain subscores of AQ and SCARED. According to the linear regression model, the total score and social skills, attention switching, and imagination scores of AQ, as well as generalized anxiety and panic/somatic scores of SCARED and the total and inattention scores of parent T-DSM-IV, were predictive of SCT total score (p < 0.05). Conclusions: SCT is associated with ATs and anxiety disorders. Children with ADHD and SCT symptoms should be screened for such conditions.

ABSTRACT Backgrounds: Hepcidin is related to the pathogenesis of chronic renal failure anemia, which is considered a chronic inflammatory state as well as HCV infection. IL-6 stimulates the release of hepcidin from the liver, suppresses intestinal iron uptake, and releases iron from internal stores. Method: To detect the association between IL-6 gene polymorphism and anemia markers, 80 hemodialysis (HD) patients [40 negative HCV HD patients and 40 positive HCV HD patients] were studied by routine chemistry and complete blood count, in addition to the assessment of serum hepcidin, iron parameters [serum iron and serum ferritin], and hepatitis C markers. IL-6 polymorphism -174G/C was determined by MS-PCR, while IL-6 polymorphisms -597G/A and -572 G/C were detected by PCR-SSP. Results: Hepcidin was non-significantly elevated in HCV-positive compared with HCV-negative hemodialysis patients. A statistically significant difference was detected between the negative and positive HCV HD patients in frequencies of IL-6 -174 G/C and -597 G/A (P≤ 0.01 and P≤ 0.001, respectively). On the other hand, a non-significant difference was reported between negative and positive HCV HD patients in the frequencies of IL-6 -572 G/C. Conclusions: Our study indicated that IL-6 -174 G/C and -597 G/A polymorphisms may play a role in HCV susceptibility in HD patients. Additional prospective studies on a larger population are needed to confirm our findings.

Em decorrência disso, Joseph Walser passa por um processo de esvaziamento de sentidos acelerado pelo ritmo implacável das máquinas e pela violência da guerra. Logo, a convivência com o vazio torna-se inevitável. É fato, portanto, que para este romance o vazio demonstra ser um eixo de extrema signifi cação. Assim, o presente artigo propõe uma refl exão sobre a modalização do vazio nesse romance, especialmente, aquele caracterizado pela falta e o que se apresenta enquanto possibilidade de sentido. Para auxiliar essa refl exão, recorre-se aos estudos de Santiago Kovadloff (2003), David Le Breton (1991); (2018), entre outros.

O presente artigo objetiva apresentar o modelo de organização da experiência educativa segundo a pedagogia do problema de Michel Fabre (1948-), filósofo francês, com foco na problematização das questões sociais polêmicas. Diante dos desafios de um mundo problemático que interroga os sujeitos educativos em relação ao conteúdo, ao modo e ao tempo das relações ensino-aprendizagem, cabe à Educação fornecer ao aluno e desenvolver junto a ele duas instrumentalidades metafóricas interdependentes que atinem à organização bussolar e cartográfica da experiência. A bússola representa a funcionalidade cognitiva e metodológica que estrutura formalmente o processo de problematização. O suporte cartográfico constitui o substrato material e cultural desse processo, a permitir a localização, a orientação e a discriminação de limites dos itinerários educativos a serem percorridos pelo discente, com o auxílio do docente. Na problematização das questões sociais polêmicas, a associação de múltiplas coordenadas éticas, científicas, políticas, econômicas e culturais aos saberes racionais e estáveis ensinados e reproduzidos na escola altera o conjunto das condições e dos dados mobilizados para a construção ou para a resolução do problema. O tratamento dessas questões complexas enfrenta obstáculos reducionistas contra os quais se propõe uma estratégia educativa metodológica respeitosa da pluralidade de posições argumentadas e divergentes. Em um ethos escolar democrático e avesso a injuntividades normativas, tampouco receptivo a relativismos que impeçam a abertura dialógica da experiência fundada na intersubjetividade, educar para a problematização da vida em sociedade implica o dimensionamento regulatório e prudencial dos fatos e dos valores nela imersos. Esse exercício, a ser realizado conjuntamente pelos sujeitos educativos, propicia ao discente, diferentes possibilidades de percursos e de escolhas conducentes à autoconstituição ética, mister indeclinável neste mundo problemático.

Introduction: Stent restenosis remains a clinical challenge for patients with ischemic heart disease, since it is associated with repeated coronary interventions as well as higher hospitalization rates and medical costs. Inflammation plays a significant role. Although an association between stent restenosis, increased C-reactive protein (CRP) and decreased albumin levels has been previously reported, no studies have investigated the ability of the CRP/albumin ratio to predict stent restenosis. Methods: This retrospective study included 448 patients who had previously undergone primary percutaneous coronary intervention and who were referred for subsequent reintervention due to recurrence of anginal symptoms. The study population was divided into two groups based on whether the patient had developed stent restenosis. They were then stratified into three groups according to their CRP/albumin ratio. Results: Out of 448 patients, stent restenosis was observed in 24.5% (n=110), as determined by coronary angiography. Patients with stent restenosis had a higher CRP/albumin ratio, greater platelet distribution width (PDW), higher CRP levels, and lower levels of both high-density lipoprotein (HDL) cholesterol and serum albumin. The CRP/albumin ratio (OR: 2.289, 95% CI: 1.056-4.959; p=0.036), stent diameter, PDW and HDL cholesterol levels were found to be independent predictors of stent restenosis. A ROC curve comparison demonstrated that the CRP/albumin ratio was a better predictor of restenosis than either albumin and CRP individually, but it was not better than PDW and HDL cholesterol. Conclusion: As a novel inflammation-based risk score, the CRP/albumin ratio may be an easily accessible marker for assessment of stent restenosis risk. Resumo: Objetivo: A reestenose de stent permanece como um desafio clínico em doentes com cardiopatia isquémica. A reestenose de stent está associada a intervenções coronárias recorrentes bem como a aumento de internamentos e de custos médicos. A inflamação tem um papel significativo. Embora a associação entre reestenose de stent e o aumento dos níveis de proteína C-reativa (PCR) e a diminuição dos níveis de albumina tenham sido previamente registados, nenhum estudo pesquisou a possibilidade da PCR/albumina prever um diagnóstico de reestenose de stent. Métodos: Este estudo retrospetivo incluiu 448 doentes que tinham sido previamente submetidos a intervenção coronária percutânea primária e que foram referidos para reintervenção posterior devido a ressurgimento dos sintomas de angina. A população do estudo foi dividida em dois grupos com base no facto do doente ter desenvolvido reestenose de stent. Posteriormente, foram estratificados em três grupos de acordo com as relações PCR/albumina. Resultados: De um grupo de 448 doentes, a reestenose de stent foi observada em 24,5% (n = 110), conforme determinado por avaliação através de angiografia coronária. Os doentes com reestenose de stent apresentaram uma relação PCR/albumina mais elevada, variação maior do diâmetro das plaquetas (DP), níveis mais elevados de PCR e níveis mais baixos de colesterol de lipoproteínas de alta densidade (HDL) e de albumina sérica. Os níveis da relação PCR/albumina (OR: 2,289, IC 95%:1,056 – 4,959; p = 0,036), o diâmetro do stent, a variação maior do DP e o colesterol HDL foram considerados fatores preditores independentes da restenose de stent. A comparação das curvas ROC demonstrou que a relação PCR/albumina constituiu um fator preditor melhor do que a albumina e a PCR, isoladamente, não sendo, no entanto, melhor do que o DP e o colesterol HDL. Conclusão: Como novo score de risco baseado em inflamação, a relação PCR/albumina pode ser um marcador facilmente acessível para a avaliação do risco de reestenose de stent. O objetivo deste estudo é examinar a relação entre o bem-estar psicológico e a percepção do sucesso dos alunos do ensino médio que praticam esportes. A amostra do estudo consiste de um total de 420 estudantes, 140 do sexo feminino e 280 do sexo masculino, que estudam em escolas de ensino médio. Neste estudo, "Formulário de Informações Pessoais" preparado pelo pesquisador, foi utilizada a escala de bem-estar psicológico desenvolvida por Diener et al. (2009) para medir o bem-estar sócio-psicológico e cuja versão turca e, o estudo de validade e confiabilidade foi realizado pela Telef em 2013, e a percepção do inventário de sucesso (Child Version - BAE-Ç) desenvolvida por Roberts et al. (1998) e cuja adaptação ao turco foi feita por Kazak Çetinkalp (2006). Como resultado, não há relação entre as orientações de ego dos indivíduos e seu bem-estar psicológico; no entanto, mostrou que há uma relação positiva significativa entre as orientações de tarefa e o bem-estar psicológico no nível de confiança.

With the objective to evaluate different frequencies of supplementation on the performance and ingestive behavior of Nellore calves kept on Urochloa brizantha cv. Marandu pasture, 56 animals with initial average body weight (BW) of 174.5 kg were submitted to the following treatments: no supplement and 1x, 3x, and 7x supplementation per week. The supplement was provided in the amount of 3.5 g kg-1 of BW day-1. BW and evaluation of behavior were assessed every 28 days. The randomized block's design with two repetitions was used. The analyses of variance were carried out by the GLM procedure in the SAS software, and the means were compared by the Tukey’s test at 5% significance. The supplementation increased the average daily gain (ADG; P ? 0.05); however, it did not differ between frequencies (0.005, 0.190, 0.183, and 0.177 kg head-1 day for the no supplement treatment and 1x, 3x, and 7x per week, respectively). The animals in the no supplement treatment showed longer grazing time (P ? 0.05) compared to those receiving the supplement. However, results did not differ between frequencies (10.44, 8.57, 8.32, and 8.59 hours day-1 for no supplement treatments and 1x, 3x, and 7x per week, respectively). Thus, the frequency of supplementation can be reduced to once a week without reducing animal performance.

ABSTRACT This study was carried out to determine the effects of coating with chitosan film and storing at different periods (7, 14, and 21 days) of broiler breeder eggs on growth performance and carcass characteristics of the chicks. The present study was arranged as three different storage periods (7, 14, and 21 days) and coating or not the eggs with chitosan film. In total, 1800 hatching fertilized eggs were used. These eggs were divided into six groups with 100 eggs in each and 600 eggs in each replication. A total of 751 chicks obtained from the hatching were used as material in this study. As a result, all chicks in the coated chitosan groups were alive during the 42-day growth period. The average hatching weight was determined as 42.7±0.1 g. The mean body weight (BW) of chicks on the 42nd day was determined as 2541.8±12.3 g in all groups. The effect of repetition on weekly BW and body weight gain (BWG) was found to be significant in the growth period. The differences between the groups for the BW were significant on day 1. While the weekly BWG varied, the growth performance was similar in the growth period in all groups. The differences between the groups in terms of slaughter weight and carcass characteristics were insignificant. It was determined that coating broiler eggs with chitosan and storing them in different periods does not have significantly negative effect on growth performance and carcass characteristics.

Abstract: The objective of this work was to evaluate the use of both single- and triple-row production systems in two drip-irrigated sweet corn cultivars under dry Mediterranean climate conditions. A two-year field experiment (2017 and 2018) was carried out in clay loam soil. The following three combinations spacing between crop rows and drip-line lateral spacing, with three replicates for each cultivar, were applied: single rows at 75 cm spacing, with one drip-line lateral spacing for each crop row; single rows at 75 cm row spacing, with one drip-line lateral spacing for three crop rows; and triple rows, 37.5 cm apart, on 225 cm centers, with one drip-line lateral spacing for each triple row. The responses of both cultivars were similar. In addition, husked cob yield and irrigation water use efficiency (IWUE) significantly reduced as the drip-line lateral spacing increased in single rows. Yield loss was 35.2% in relation to the 75 cm spacing. However, when the triple-row system with 225 cm drip-line lateral spacing was adopted, yield and IWUE were noticeably improved, and the yield loss was moderated to 16%, due to the improvement in soil water conditions in the triple rows. For improved yield and IWUE, the combination of triple rows with the 225-cm drip-line lateral spacing is an efficient drip-irrigated planting pattern for sweet corn production in dry Mediterranean climate conditions.

A combined three-dimensional quantitative structure-activity relationship (QSAR) modeling and molecular docking studies were carried out on the 64 indole derivatives and was accomplished to profoundly understand the structure-activity correlation of indole-based inhibitors of the HCV NS5B polymerase against HCV. Genetic function approximation (GFA) of Material studio software version 8 was used to perform the QSAR study while Autodock vina version 4.0 of Pyrx software was used for molecular docking studies of the selected indole derivatives. The optimum model builds exhibited statistically significant results: squared correlation coefficient (R2) of 0.760, adjusted squared correlation coefficient (R2 adj) value of 0.708, Leave one out (LOO) cross-validation coefficient value of 0.634 and the external validation (R2 pred) of 0.621. Molecular docking study of the indole derivative with 1G8Q as the protein target revealed that the best binding affinity with the docking scores of -9.4 kcal/mol formed hydrophobic interaction and H-bonding with amino acid residues of HCV NS5B polymerase. The QSAR model generated and molecular docking results proposed that the model had a good level of stability, strength, and predictability at internal and external validation, and the physicochemical parameters are to be analyzed when designing new indole derivatives agent with better activity against the 1G8Q target site.

ABSTRACT This study was designed to evaluate the effects of adding kefir on body measurements, giblet weight, and gut morphological patterns of geese through drinking water. A total of 54 birds were divided into three groups, each comprising 18 birds, and each group was further divided into three subgroups containing six birds each. One group served as the control group, while the other two groups were given kefir-treated drinking water at the ratios of 2.5 and 7.5%, respectively. No significant effects of kefir supplementation on body measurements, spleen weight, bursa weight, and gut morphology were observed. It is concluded that kefir may be used in poultry feed with an inclusion rate of 7.5% without imposing any adverse effect on the measurements of body structures, weights of visceral organs, and gut morphology.

Abstract: Background: Rosacea is a chronic facial skin disease associated with excessive inflammatory response to various triggers. Although some studies have supported the increased risk of cardiovascular diseases in rosacea, it has not been completely accepted. Objective: We aimed to investigate epicardial fat thickness and carotid intima-media thickness as cardiovascular risk predictors in rosacea patients. Methods: We conducted a cross-sectional study including 40 rosacea patients and 40 controls. Demographic data, epicardial fat thickness, carotid intima-media thickness, lipid parameters, biochemical parameters, presence of insulin resistance, and presence of metabolic syndrome of the participants were recorded. Results: Forty rosacea patients (31 female and 9 male) and 40 controls (30 female and 10 male) were enrolled in the study. Objective: This study aimed at determining the effect of isotretinoin on olfactory function in patients who have acne and are otherwise healthy. Methods: Forty-five patients (aged 25-40 years) with acne were included in the study. All patients underwent a rhinological examination. Olfactory function was assessed by the Sniffin' Sticks Test. The test was assessed at baseline and in the third month of isotretinoin treatment. Results: Isotretinoin improved the performance of patients in the olfactory test. Abstract Hereditary leiomyomatosis and renal cell carcinoma (HLRCC) is an autosomal dominant manifestation of cutaneous and uterine leiomyomas together with renal cancer due to autosomal dominant germline mutations of fumarate hydratase gene. A twenty-year-old female patient presented with type-II segmental piloleiomyoma and increased menstruation due to uterine leiomyomas, with a history of bilateral nephrectomy performed at 13 and 16 years of age for type 2 papillary renal cell carcinoma. This case represents one of the very early onsets of hereditary leiomyomatosis and renal cell carcinoma syndrome. As genetic anticipation for renal cancer is a well-documented entity for HLRCC syndrome, early recognition is crucial for both the patient and her family in order to provide appropriate counseling and initiation of surveillance.

Justificativa e Objetivos:Sabe-se que a sífilis adquirida, no cenário epidemiológico gaúcho, configura-se como doença de extrema importância devido a sua alta prevalência e ascensão. A partir disso, o seguinte trabalho visa revisar as medidas utilizadas na assistência dos pacientes adultos e idosos com diagnóstico de sífilis adquirida e sua epidemiologia no município de Porto Alegre/RS. Conteúdo: Segundo dados publicados em 2016 pelo Ministério da Saúde, o crescimento da infecção por sífilis em indivíduos adultos e idosos está em grande expansão no Brasil. Através das conquistas pelos idosos nas últimas décadas, o prolongamento da vida sexual é ponto merecedor de destaque, além do incentivo à socialização e a falta de informação sobre saúde sexual. Abstract: Background: c-Kit is a proto-oncogene that encodes tyrosine kinase receptor (CD117). Mean platelet volume (MPV) is a useful marker, providing information on platelet function and diameter. Objective: To investigate c-Kit expression and intensity in patients with Kaposi's sarcoma (KS) and to investigate the relation between Ki-67 proliferation and MPV. Methods: A total of 32 patients, diagnosed with classic cutaneous KS, were included in this study. We reevaluated the histopathological reports with the preparations, confirmed the diagnosis and then determined the patients' histopathological stages. c-Kit expression and Ki-67 proliferation were investigated immunohistochemically in KS cases, while MPV in all cases was checked. Results: Although c-Kit expression was detected in 22 cases (68.8%), it was not expressed in 10 cases (31.2%). We detected 8 cases with + (25%), 6 with ++ (18.8%) and 8 with +++ (25%). Ki-67 expression was 5.0% (min-max 1.0-20.0). Relapse was observed in 5 cases (15.6%) out of 32. There was positive correlation between c-Kit expression and MPV (rs=0.598, p

ABSTRACT BACKGROUND AND OBJECTIVES: Laparoscopic surgery has become a popular surgical tool when compared to traditional open surgery. There are limited data on pediatric patients regarding whether pneumoperitoneum affects cerebral oxygenation although end-tidal CO2 concentration remains normal. Therefore, this study was designed to evaluate the changes of cerebral oxygen saturation using near-infrared spectroscope during laparoscopic surgery in children. METHODS: The study comprised forty children who were scheduled for laparoscopic (Group L, n = 20) or open (Group O, n = 20) appendectomy. Hemodynamic variables, right and left regional cerebral oxygen saturation (RrSO2 and LrSO2), fraction of inspired oxygen, end-tidal carbon dioxide pressure (PETCO2), peak inspiratory pressure (Ppeak), respiratory minute volume, inspiratory and end-tidal concentrations of sevoflurane and body temperature were recorded. All parameters were recorded after anesthesia induction and before start of surgery (T0, baseline), 15 min after start of surgery (T1), 30 min after start of surgery (T2), 45 min after start of surgery (T3), 60 min after start of surgery (T4) and end of the surgery (T5). RESULTS: There were progressive decreases in both RrSO2 and LrSO2 levels in both groups, which were not statistically significant at T1, T2, T3, T4. ABSTRACT Forty male Naemi lambs were used to evaluate the effect of total mixed rations (TMR) alone or in combination with supplementation of alfalfa hay offered at different schedules on growth, carcass characteristics, and meat quality. Lambs at the age of three months, with an average weight of 28.85±1.09 kg, were randomly selected and distributed into four different feeding systems, 10 lambs each, as follows: TMR diet (control); TMR plus 100 g alfalfa hay; TMR plus 200 g alfalfa hay every two days; and TMR plus 300 g alfalfa hay every three days. The TMR and fresh drinking water were offered ad libitum. Lambs in the treated groups (with alfalfa hay) had significantly higher body weight, body weight change, dry matter intake, and feed conversion ratio compared with the control. Similarly, slaughter weight, carcass weight (hot and cold), and internal organs weight significantly increased in the treated groups, except for dressing weight percentage, which improved only in the treatment with TMR plus 300 g alfalfa hay. By contrast, mesentery fat, back fat, and body wall fat decreased in treated groups. Meat quality characteristics indicated that cooking loss and pH values did not differ between the control and treated groups; however, meat colour (lightness, yellowness, and redness) and pH (1 and 24 h) improved in post-slaughter treated groups. Furthermore, no difference was found in texture profile analysis (hardness, springiness, cohesiveness, and chewiness), although the shear force decreased significantly in treated groups. Addition of alfalfa hay offered at the present plan may not only improve the growth and carcass characteristics, but also enhance the meat quality of Naemi lambs. From the labour and economic point of view, TMR plus 300 g alfalfa hay every three days may be adopted while taking into account the management decision.

Abstract Objective: To determine the prevalence of malocclusion and orthodontic treatment need in a large sample of Central Anatolian adolescents and compare them with European-other nations' adolescents. Methods: The sample included 1125 boys and 1204 girls aged between 12 and 16 years with no previous orthodontic treatment history. Occlusal variables examined were molar relationship, overjet, overbite, crowding, midline diastema, posterior crossbite, and scissors bite. The dental health (DHC) and aesthetic components (AC) of the Index of Orthodontic Treatment Need (IOTN) were used as an assessment measure of the need for orthodontic treatment for the total sample. Results: The results indicated a high prevalence of Class I (34.9%) and Class II, Division 1 malocclusions (40.0%). Moreover, increased (18%) and reduced bites (14.%), and increased (25.1%) and reversed overjet (10.%) were present in the sample. Conclusion: Using the DHC of the IOTN, the proportion of subjects estimated to have great and very great treatment need (grades 4 and 5) was 28.%. However, only 16.7% of individuals were in need (grades 8-10) of orthodontic treatment according to the AC.

Abstract This research is focused on the effect of porosity on the friction-wear properties of automotive brake pads. Waste Tire Dust (WTD) was used as a new friction material in brake pads. Newly formulated brake pad materials with five different components have been produced by conventional techniques. In the experimental studies, the change of the friction coefficient, the temperature of the friction surface, the specific wear rate, and the hardness, density and porosity were measured. In addition, the micro-structural characterizations of brake pads are determined using Scanning Electron Microscopy (SEM). The mean coefficient of friction, porosity and specific wear are increased due to a WTD rate increases, on the other hand, hardness and density are decreased. As a result, WTD can be considered as an alternative to revalorize this kind of waste products in the brake pads and the amount of porosity of the brake pad affected the friction coefficient and wear behavior of the pad.

ABSTRACT OBJECTIVE: To present the results of our cases of Dupuytren's disease treated with regional selective fasciectomy in light of the literature. METHODS: Patients diagnosed with Dupuytren's contracture and surgically treated with regional selective fasciectomy at our institution with adequate follow-up data were included in the study. All patients were routinely followed after surgery to assess results and complications. QuickDASH scoring was used to evaluate the patients and recurrences and complications were recorded. RESULTS: Twenty-one hands of 19 patients (13 males, 6 females) who underwent surgery and received adequate follow-up were retrospectively evaluated. Mean patient age was 65.8 (range: 41 to 86) and the mean follow-up period was 48.2 months (range: 24 to 86). Fourteen (66.6%) hands had excellent results, five (23%) hands had good results and two (9.4%) had fair results. The mean QuickDASH score for the patients at the final follow-up was 6.58 (range: 0 to 20.4). CONCLUSION: Our study results demonstrated that regional selective fasciectomy is a reliable and efficient method to treat Dupuytren's disease with low rates of complications and recurrence and the technique can be considered the gold standard. Entre eles se podem contar os discursos filosóficos, sociológicos e religiosos. As relações intertextuais ocorrem tanto no domínio verbal quanto nos domínios do silêncio. Elas se apresentam de forma explícita, por vezes, e de modo mais velado, por outras. O objetivo fulcral deste trabalho consiste na articulação das interfaces entre literatura, trauma, silêncio e vazio à luz dos estudos sobre intertextualidade, especialmente de Júlia Kristeva. Valendo-se dos expedientes do silêncio e do vazio, afloram as memórias de dor, sofrimento e morte. Os vazios rondam as personagens de Jerusalém e encontram-se repletos de silêncios. Tais silêncios potencializados revelam a existência de diferentes discursos. Convocam-se estudos relacionados ao silêncio, desenvolvidos por Eni P. Orlandi, para dar suporte às discussões. Quanto à problemática do vazio, recorre-se principalmente aos apontamentos de Gilles Lipovetsky.
