This article considers the development of N. O. Lossky’s concept of intuitivism in the context of the translation of Kant’s works and revision of Kant’s epistemology. The development of Lossky as an independent philosopher required overcoming Kant’s scepticism, which was not consistent with the ideal-realist’s belief in the cognoscibility of transsubjective world given us in the original. The means necessary to overcome it determined the key characteristics of the new system of intuitivism, which emerged in a discussion of Kant’s epistemological system. The author identifies the key themes of transcendental philosophy that drew special attention of Lossky. These include the fundamental logical issues of the essence of logical connection, the nature of judgement and inference, and that of the synthetic and analytic. The article addresses the interpretation of the concept of ‘intuition’ by Lossky and the influence of this interpretation on the translation of Kant’s Anschauung.The author identifies a number of reasons that inspired the intuitivist to translate the CPR. Fol¬lowing Kant’s philosophy, Lossky had sufficient arguments in the game played in the field of his teacher and opponent, the Russian Kantian A. I. Vvedensky; moreover, he proved the inconsistency of Vvedensky’s interpretation of transcendental philosophy. It is shown that the experience of translating CPR and the critical analysis of Kant’s epistemology helped Lossky to overcome the complications that emerged in the history of philosophy and develop his own complete and organic worldview. Moreover, there arose an objective need for a new CPR translation, since some intellectuals were not satisfied with that made by N. M. Sokolov. The concepts of style and topos in their cultural studies interpretation by Yu. V. Rozhdestvensky are applied to the analysis of some fragments of N. O. Lossky's philosophical concept pertaining to the features of Russian culture in general and the originality of Russian philosophy in particular. The topic is developed through comparative digressions into the field of anthropological aspects of I. Kant's philosophy.

The goal of the article is analysis of logistics costs in production of semiconductor materials using example of two Ukrainian enterprises. The article studies influence of logistics management and logistics costs upon formation of the final cost value (price) of a commodity (service). It gives an assessment of logistics costs of Ukrainian semiconductor enterprises and establishes its structure by types of main expenditure items: material, transport, production and storehouse. It establishes the generalised quantitative structure of logistics costs of Ukrainian semiconductor enterprises with various forms of ownership under conditions of a situational growth of cost value of products and reduction of profitability of production, caused by common crisis tendencies in economy. Prospects of further studies in this direction are analysis of costs in production of semiconductor products and establishment of the specific feature of their grouping and classifying from the point of view of logistics and justification of the model of assessment of cost value of products, which takes into account mutually contradictory influence of direct logistics costs and logistics management upon the final result.

Objective: To investigate the efficacy and safety of Aviandr in the treatment of anxiety in patients with adjustment disorders after COVID-19., Material and Methods: A multicenter prospective open-label study included 109 patients of both sexes aged 18 to 65 years (70 women, 39 men, average age - 41.4±13.18 years) with a leading complaint of anxiety (Hamilton scale score, HAM-A ≥18 - ≤24), which arose after acute coronavirus infections. Clinical manifestations had to meet the diagnostic criteria F43.2 ICD-10. The drug Aviander was prescribed 20 mg 2 times a day for 4 weeks. At the end of taking the drug, patients were monitored for another 1 week (a delayed follow-up visit). Psychopathological, statistical and parametric research methods were used using standardized HAM-A, Montgomery-Asberg scales (MADRS), visual analog asthenia scale (VASH-A), Sheehan Disability Scale (SDS), digital character substitution test (DSST), general clinical impression scale (CGI)., Results: Data from 109\110 patients were analyzed to evaluate efficacy\safety. Aviandr was administered 20 mg 2 times daily for 4 weeks. Patients were followed for 1 week (delayed follow-up visit) at the end of treatment. Reducing the intensity of anxiety on the HAM-A scale was - 14.2±4.92 or 69.4±22.66% by the end of treatment. The response rate to therapy (responders are patients with a decrease in the total score on the HAM-A ≥50%) was 83.49%. Remission was achieved (sum of HAM-A scores ≤7) by the end of treatment 68.81% of patients, and 79.8% of patients at the follow-up visit. Significant changes were obtained on the MADRS, VAS-A, SDS and DSST scales. According CGI 45.9% of patients had «much improved» and 43.1% of patients had «very much improved» by the end of treatment; 58.7% of patients had «much improved» and of 33.9% patients had «very much improved» at the follow-up visit. 38 adverse events were reported in 27 (24.55%) patients during the study. A definite association with study drug was reported between 5 mild adverse events in 4 (3.64%) patients. No subjects withdrew from the study due to an adverse event. Positive dynamics (reduction of anxiety symptoms, decrease in asthenia) persisted after discontinuation of the study drug. No cases of withdrawal syndrome were observed., Conclusion: According to the results of the study, the anxiolytic, antidepressant, antiasthenic and pro-cognitive effects of Aviandr were observed. An increase in the social activity of patients was observed.

Aim: To identify the features of the course of COVID-19 in patients with type 2 diabetes mellitus (T2DM), depending on the intake of hypoglycemic therapy at the prehospital stage, in conjunction with the functional state of the kidneys., Materials and Methods: A retrospective analysis of 291 case histories of patients with COVID-19 and T2DM hospitalized in the infection department of Semashko Regional Clinical Hospital from January to December 2021, including the main clinical and laboratory parameters. Results. Among hospitalized patients with COVID-19, patients with T2DM had a higher mortality rate. An analysis of the case histories of deceased patients with COVID-19 and T2DM showed that at admission, body mass index (BMI), C-reactive protein, and creatinine were higher than those of survivors and amounted to BMI - 33 [30; 39] and 33 [28; 36] kg/m3; p=0.039, C-reactive protein - 77 [47.5; 106.0] and 57 [27.0; 89.0] mg/l; p=0.015, in terms of creatinine level - 89 [70.0; 144.0] and 82 [66.0; 101.0] µmol/l; p=0.039, respectively. It was found that in the second week of hospitalization in the group of deceased patients with COVID-19 and T2DM, the creatinine level was statistically significantly higher than in surviving patients and amounted to 94.5 [71.5; 141.0] and 72.5 [57.0; 88.0] µmol/L; p<0.001, respectively. The probability of death in hospitalized patients with type 2 COVID-19 and T2DM depended on BMI and creatinine levels at the second week of hospitalization. Patients with prehospital correction of hyperglycemia dipeptidyl peptidase-4 inhibitors (iDPP-4)/ glucagon-like peptide-1 receptor agonists (agGLP-1)/ sodium-glucose co-transporter 2 inhibitors (iSGLT-2) had significantly lower creatinine levels at week 2 of hospitalization., Conclusion: In patients with moderate to severe COVID-19 with concomitant T2DM, special attention should be paid to the combination of high BMI and creatinine in the second week of hospitalization, which is a prognostically unfavorable predictor of death in such patients.

Objective: Evaluation of the efficacy and safety of Levroso Long (fixed dose combination (hereinafter referred to as «FDC») diphenhydramine and melatonin, 25 mg + 3 mg and FDC diphenhydramine and melatonin 50 mg + 3 mg, respectively, capsules with modified release, NovaMedica Innotech LLC) in comparison with diphenhydramine medicines 50 mg (Diphenhydramine tablets, DALHIMFARM OJSC, Russia) and 3 mg melatonin (Melaxen tablets, Unipharm Inc., USA) in patients with insomnia., Materials and Methods: Material and methods. A multicenter open randomized comparative clinical trial was conducted in parallel groups. The study included 312 patients with an insomnia diagnosis verified in accordance with the DSM-IV criteria (ICD code F51.0-10) and an insomnia severity index (ICI) >8 points. The patients were randomized into 4 groups and took the following medications for 10 days: patients of group 1 - Levroso Long (25 mg diphenhydramine and 3 mg melatonin), group 2 - Levroso Long combination drug (50 mg diphenhydramine and 3 mg melatonin), Group 3 -Diphenhydramine 50 mg, 4 th - Melaxen 3 mg. A comparative assessment was performed on the 7 th and 10 th days of therapy according to the primary endpoint - ICI, and secondary endpoints - the results of the assessment based on the Leeds Sleep Assessment Questionnaire, the Pittsburgh Sleep Quality Questionnaire, the Scale of the overall clinical impression of improvement and the analysis of the profile of adverse events (NA)., Results: The FDC of diphenhydramine 25 mg + melatonin 3 mg was more effective than diphenhydramine 50 mg, and FDC diphenhydramine 50 mg + melatonin 3 mg was more effective than diphenhydramine 50 mg or melatonin 3 mg on both primary and secondary endpoints. In a safety assessment, the highest incidence of adverse events (48.7%) in the diphenhydramine 50 mg group was significantly lower in groups 1 (29.5%), 2 (12.8%) and 4 (1.3%). All reported adverse events in the FDC groups were mild or moderate in severity., Conclusion: The study showed that Levroso Long in two dosages surpassed monotherapy with 50 mg of Diphenhydramine (50 mg) or Melaxen (3 mg) in patients with insomnia in terms of efficacy parameters. The benefit/risk ratio of the studied drug is favorable compared to monotherapy.

Aim: To assess the safety and efficacy of Remaxol, solution for infusion, compared with parenteral form of S-adenosyl-L-methionine, in the treatment of patients with intrahepatic cholestasis syndrome accompanying chronic diffuse liver diseases of various etiology., Materials and Methods: In a multicenter open-label comparative study of the safety and efficacy of Remaxol (inosine + meglumine + methionine + nicotinamide + succinic acid) 317 patients aged 18 to 65 years were randomized into 2 groups: patients of the experimental group (n=168) received intravenous Remaxol, solution for infusion, 400 ml, and patients of the control group (n=149) Heptral (S-adenosyl-L-methionine) 800 mg. The duration of treatment was 10 days. The primary efficacy endpoint was the proportion of patients who responded to therapy, as demonstrated by dynamics of laboratory parameters of liver functional status: decrease in gamma glutamyl transpeptidase level by 40%, and/or alkaline phosphatase level by 30%, and/or decrease total bilirubin level by 30% from baseline by the end of the treatment course., Results: The proportion of responders was 51% in the Remaxol group vs. 44.9% in the Heptral group (p=0.303); the lower limit of the one-sided 95% confidence interval for the difference in the proportions of responders was -4.01%, which exceeds the non-inferiority margin pre-defined by the study protocol, thus, the non-inferiority hypothesis was proven, i.e. Remaxol at a dose of 400 ml/day demonstrates similar efficacy to Heptral at a dose of 800 mg/day in patients with intrahepatic cholestasis syndrome associated with chronic diffuse liver diseases. Similar positive trends in the levels of transaminases, total bilirubin and the severity of pruritus were revealed in both treatment groups. We did not reveal statistically significant between-group differences in the frequency of adverse events definitely related to the study treatment., Conclusion: Administration of Remaxol as a part of the pathogenetic therapy of patients with intrahepatic cholestasis syndrome who need hepatoprotection is justified.

Objective: To assess the efficacy and safety of the drug ampasse in the treatment of patients with chronic cerebrovascular disorders (CCVD)., Materials and Methods: A multicenter, randomized, double-blind, placebo-controlled, confirmatory study of the efficacy and safety of ampasse (phase III) was conducted in 124 patients aged 50 to 75 years. The main group (MG) - 62 patients, received the test drug ampasse, solution for intravenous administration, 5 mg/ml, at a dose of 5 ml (25 mg), intravenously bolus slowly, the duration of treatment was 15 days. Control group (CG) - 62 patients, received comparison drug: placebo (0.9% sodium chloride-5 ml)., Results: All 124 patients fully completed the procedures and visits, there were no dropouts from the study. The proportion of patients who reached the primary endpoint (an increase in the score by 2 or more points on the MoCA scale) was 83.87% in MG and 22.58% in CG, that is, the efficacy of therapy in MG was 61.29% higher than in CG ( p <0.001), and good tolerability of the drug was proved. The secondary endpoint is an increase in quality of life (QOL) on the SF-36 V2 scale on Day 31. In MG, there was a statistically significant improvement in all indicators of QOL compared to the baseline. When assessing the safety spectrum, the proportion of patients who had adverse events was 14.52% in MG and 8.06% in CG ( p =0.395)., Conclusion: Ampasse has a positive effect on cognitive functions and QOL, does not increase the frequency of adverse events in patients with CCVD compared to placebo, does not cause significant side effects, and is well tolerated by patients.

mRNA transport from the nucleus to the cytoplasm is an essential step of eukaryotic gene expression. A pre-mRNA molecule undergoes modification, such as 5'-capping, splicing, and 3'-end processing, in the nucleus. The molecule being modified interacts with a large number of proteins and, thus, mRNP particles are formed. The binding of factors involved in nuclear export also occurs during transcription and mRNA processing. We have shown that the functioning of TREX-2, an mRNA export complex, is restricted to the nucleus. We used the method of RNA coprecipitation that enables the selective extraction of RNA-protein complexes from samples to show that the transcription elongation complex TREX interacts with mRNA of the β-tubulin 56D gene over the entire length of the molecule. The capping protein Cbp80 reacted both with the cap structure and with a specific part of the coding mRNA of the β-tubulin 56D gene. The TREX-2 complex that mediates mRNA export from the nucleus to the cytoplasm is bound to the same part of the coding sequence. Thus, we identified a common binding site for all of the complexes under investigation on the mRNA of β-tubulin 56D. Co-immunoprecipitation reactions performed with S2 cell extracts revealed interactions between the components of complexes involved in transcription elongation, maturation, and export of mRNA. The model of molecular folding for the mRNP particle involving the mRNA of β-tubulin 56D has been proposed.

Barrett’s esophagus is considered as a predictor of esophageal adenocarcinoma with multistage neoplastic progression at present time. The research assessed an expression of microRNA-21 in 25 patients with different degree of metaplasia and dysplasia of mucous coat of esophagus. The level of expression of microRNA depended on the presence and degree of expression of neoplastic changes of mucous coat of esophagus. The expression rising of microRNA was noted in patients with columnar-celled metaplasia and intraepithelial neoplasia and in case of esophageal adenocarcinoma. The treatment strategy could be determined by the method of estimation of the level of microRNA expression in biopsy material from mucous coat of esophagus in patients with gastroesophageal reflux disease and using other criteria.

RNA-binding proteins (RBPs) play an important role in regulating gene expression at the posttranscriptional level, including the steps of pre-mRNA splicing, polyadenylation, mRNA stabilization, mRNA export from the nucleus to the cytoplasm, mRNA localization, and translation. RBPs regulate these processes primarily by binding to specific sequence elements in newly synthesized or mature transcripts. While many RPBs are known to recognize certain nucleotide sequences in RNA, information is insufficient for others. In particular, RBPs often compete for RNA binding or interact with RNA cooperatively. Hence, it is of importance to study the RNA-protein interactions in vivo. Numerous methods have been developed to identify the target nucleotide sequences of RBPs. The methods include the electrophoretic mobility shift assay (EMSA), systematic evolution of ligands by exponential enrichment (SELEX), RNA pull-down assay, RNA footprinting, RNA immunoprecipitation (RIP), UV-induced crosslinking immunoprecipitation (CLIP) and its variants, and measurement of the level for newly synthesized transcripts. Each of the methods has its limitation, and several methods supplementing each other should be employed in order to detect the RNA sequence to which a protein binds.

The bariatric surgery is the most effective way of obesity treatment. The article describes an experience of placement of intragastric balloon as an additional method of treatment to diet and medicamentous therapy in one group of patients. At the same time, there was the other group, where intragstric balloons were placed before bariatric surgery as measure of preoperative preparation.

The authors measured the changes of hemodynamics in 72 patients. It was also estimated a blood oxygenation and volume of liquid sectors of the organism in different degree of obesity before and after liposuction of the large volume. It was shown, that this operation facilitated to an improvement of respiratory lung function due to changes of pulmonary circulation.

Periodontal pockets microflora features by aggressive and chronic periodontal disease were studied by means of polymerase chain reaction (PCR) and mass spectrometry. The study included 14 patients aged 25-70 years without somatic pathology. The pilot study has shown the high prevalence rate in obligate periodontal pathogens in periodontal pockets revealed by PCR diagnostics, at the same time mass spectrometry has revealed the presence of other pathogens which had not been primarily included into PRC diagnosis. The aim of this study was to investigate the expression of TNF-alpha produced in the kidney in patients with diabetic nephropathy (DN) type 2 diabetes and its impact on the development of glomerular and tubulointerstitial tissue changes in these patients. The survey was conducted in 49 elderly patients (mean age 66.53 +/- 3.50 years) with type 2 diabetes complicated by the development of DN. The study included patients with serum creatinine less than 0.13-0.14 mmol/l and in the initial stage of chronic renal failure in the level of serum creatinine less than 0.20 mmol/l. Diabetes duration was 17.89 +/- 0.44 years, and the duration of diabetic nephropathy was 1.60 +/- 0.44 years. Light and immunofluorescence microscopy of the renal biopsy specimens obtained by needle lifetime biopsy was performed for all patients. Morphological changes in the tissue were assessed in accordance with the latest international classification of diabetic nephropathy. In addition to light and immunofluorescence microscopy the expression of TNF-alpha in the glomerulus and interstitial tissue by monoclonal antibodies labeled Fitc ("Dako", Germany) were determined in all patients. The location of TNF-alpha expression in the glomeruli (capillary loops, mesangial matrix, glomerular capsule) and in the interstitial tissue (urinary epithelium convoluted tubule basement membrane of urinary tubules, interstitial cells) were estimated. Correlation analysis of the influence of the expression of TNF-alpha on the expression of morphological changes of tissues revealed that a maximum of cytokine production in the glomeruli at the stage IIa class cytokine affected only the development of segmental glomerular sclerosis. With the decrease in TNF-alpha production in the glomeruli you can observe the progression of histological changes--periglomerulyarny sclerosis development, builds thickening of the glomerular basement membrane; mesangial expansion goes from mild to severe and affects more than 25% of mesangial matrix (IIb stage), nodular lesions of Kimmelstil-Wilson are forming. Interstitial produce of TNF-alpha remains relatively high in the epithelium of the urinary tubules and in the interstitial cells of the kidney regardless of the class of diabetic nephropathy and influence the development of tubulointerstitial fibrosis.

The contemporary ideas on endocrine function of the adipose tissue and its role in pathogenesis of diabetes mellitus and insulin resistance associated with it and the metabolic syndrome are shown in the review. A change in the life style (excessive and irrational nutrition, insufficient physical loading, psychological disorders) and also the reduction of genetic and immunologic controlling mechanisms contribute to the development of obesity, that is now considered as low-grade inflammation. An increased number of small size adipocytes and macrophages of the adipose tissue begin to secrete an increase number of proinflammatory adipocytokines and chemokines that result in the inflammatory and metabolic stress accompanied by the stimulation of signal pathways, leading to increased insulin requirement, on the one hand, and promoting to the beta-cell death, on the other hand. The role of some adipocytokines such as IL-6, TNF-alpha, leptin, adiponectin, visfatin and resistin was demonstrated in these processes.

Aim: to study the level of circulating proinflammatory (IL-1alpha, IL-1beta, IL-6, alpha-TNF, IFN-gamma) and anti-inflammatory (IL-4, IL-10) cytokines and chemokines (IL-8, IL-16) in preclinical development of type 1A diabetes mellitus (T1DM) in children., Subjects and Methods: An examination was made in 450 children who had normal blood glucose levels and a burdened history of positive or negative Langerhans islet autoantibodies (LIAA): IAA, GADA, and 1A-2A over time until the clinical manifestations of DM1 emerged. The levels of the cytokines and chemokines were determined by ELISA and the titer of LIAA was by radioimmunoassay., Results: Long before T1DM was clinically diagnosed, most children with normal blood glucose levels and LIAA had elevated levels of the cytokines IL-1alpha, IL-6, and alpha-TNF and the chemoattractants IL-8 and IL-16 with lower IL-4 concentrations as compared with the similar indices in children without LIAA and controls. After the disease manifested, the magnitude of changes in the indices under study reduced in the majority of children with LIAA, which may suggest that the autoimmune process subsides after destruction of most beta-cells., Conclusion: The elevated levels of IL-6, IL-16, alpha-TNF, and the chemokine IL-8 with the lower blood content of the cytokine IL-4 were long before the development of DM1 in children with normal blood glucose level in the presence of LIAA, which should be borne in mind while developing the immune mechanisms specifically directed against block, which participate by means of cytokines in beta-cell destruction, as well as methods for preventing the development of T1DM in subjects with LIAA.

The authors determined interleukin-16 (IL-16) content in blood serum of patients distributed into three groups using an immunoenzymic method (ELISA). The first group consisted of patients with type 2 DM and metabolic syndrome (MS); the second group with type 2 DM and without MS, the third group - with MS and without T2DM. The control group consisted of normoglycemic subjects without MS signs who were distributed into two subgroups: 1) with excessive weight; 2) with normal weight. A significant increase in IL-16 concentration in blood serum was noted in patients with T2DM associated with MS (249,5+/-75,3 pg/ml) versus patients with MS without T2DM (130,5+/-41,2 pg/ml), and versus patients with T2DM without MS (69,5+/-35,6 pg/ml, P<0,05) and without obesity (77,4+/-11,6 pg/ml, P<0,05). This increase correlated with abdomen volume (r=0,4, P<0,05) and triglyceride level (r=0,4, P<0,05).

The content of IL16 in blood serum of children with type I diabetes mellitus and healthy normoglycemic children with genetic burden of diabetes mellitus and positive to autoantibodies against the islets of Langerhans (GADA and IAA) was determined by the immunoenzyme method ELISA. Healthy normoglycemic children with genetic burden but negative to GADA and IAA and healthy normoglycemic children without genetic burden but negative to GADA and IAA have been observed. The conducted examinations established that a statistically significant increase in the content of IL-16 and its decrease in developed diabetes mellitus patients have been noted to have place among children with genetic burden to diabetes, positive to GADA and IAA in comparison with control groups.

The authors have summarized literature data and results of own studies on autoantibodies and beta-cells of Langergan's islands of the pancreas (ICA, GADA, IA-2A and IAA). It may help predicting the development of I type diabetes mellitus even in practically healthy individuals before they present clinically evident disease. The history of the discovery of these autoantibodies, their immunological properties, comparative characteristics, sensibility and specificity and their application in clinical practice to forecast the development of I type diabetes mellitus in different populations of children and adolescents as well as to more exactly carry out differential diagnostics of DM type I and DM type II in difficult cases in adults is presented in the article. Having diabetes-associated autoantibodies as a diagnostic tool allows to diagnose a separate subtype of diabetes mellitus--autoimmune diabetes mellitus of adults (LADA) and it is very important to choose a right way of the treatment.

The purpose of the study was to investigate the condition of immunity (blood lymphocyte immune phenotype and ultrastructure) in healthy children with a family background of type 1 diabetes mellitus (DM 1) having or not having diabetes-associated autoantibodies (DAAB). The subjects of the study were divided into three groups. Group 1 consisted of 90 children with a family background of DM 1 (first line relatives had DM 1), DAAB- (GADA, IA-2A, and IAA) positive or negative; group 2 consisted of 51 children with newly revealed DM 1; group 3 included 45 healthy controls, normoglycemic DAAB-negative children with no family background of DM 1. GADA, IA-2A, and IAA titers were measured using radioimmunoassay. The immune phenotype of lymphocytes (CD3+, CD4+, CDr8, CD20+, and CD56+ cells) were studied using flow cytometry (FACS-analysis); their ultrastructure was studied by means of electron microscopy. The study found a significantly lower total number of T-lymphocytes (CD3+ cells), T-helpers/inductors (CD4+ cells), and natural killer cells (CD56+ cells and large granule-containing lymphocytes) in the DAAB-positive children vs. the DAAB-negative ones and especially the controls. In the DAAB-positive children, electron microscopy found distinct changes in the ultrastructure of CD4+ lymphocytes and large granule-containing lymphocytes (CD56+ cells), which evidences changes in the secretory and cytostatic function. Such changes in the number and ultrastructure of these lymphocyte subpopulations are found in patients with newly revealed DM 1. Thus, immune changes happen in the organism of a healthy person a long time before clinical manifestations of DM 1 develop; these changes reflect a concealed autoimmune process in Langerhans islets. Detection of DAAB plays a significant role not only in studying poorly understood pre-diabetes nature, but also in the development of new, scientifically based methods of its prevention and treatment.

Long-term experiments have provided conditions for the optimal conditions for reproduction of vaccine strains of poliomyelitis, measles, tick-borne and Japan encephalitis on the continuous cell lines. This makes it possible to solve one of the most urgent problems of modern biotechnology, namely to refuse to use primary cell cultures in vaccinology and to apply a more accessible, safe, and reference biological substrate that are stable cell lines.

Considerable decrease in chemokine interleukin-16 level has been detected in blood serum of children with the first diagnosed in life diabetes mellitus type I.

A renal function was studied in children with dismetabolic nephropathy and renal inflammation before and after spa treatment with low-mineral water from the spring "Gornovodnoye". Drinking the water resulted in intensification of 24-h diuresis in increasing proportion of sodium and chlorine ions concentrations and decreasing proportion of calcium and magnesium ions concentrations. A membrane-stabilizing action of Gornovodnenskaya mineral water reduced oxaluria and uraturia. An effective scheme of balneotherapy is proposed.

Intraaortic balloon counterpulsation was implemented in 11 patients with acute myocardial infarction and 1 patient with unstable angina. All patients had severe multivessel coronary artery disease. In 9 patients counterpulsation was used in conjunction with percutaneous transluminal coronary angioplasty, in 3 - with thrombolytic therapy. During hospitalization 2 patients died of progressing heart failure, while significant improvement of hemodynamic parameters occurred in other patients. Thus intraaortic balloon counterpulsation used in combination with angioplasty and thrombolytic therapy is an easily accessible highly effective method of treatment of cardiogenic shock in patients with acute myocardial infarction

Content of different cytokines (IF alpha, TNF alpha, IL-1 alpha, IL-4, IL-6 and IL-10) was examined in the blood serum in two groups of healthy children-siblings with type 1 diabetes mellitus with and without revealed insulin autoantibodies against pancreatic islets (GADA, IA-2A and IAA) by enzyme-like immunosorbent assay (ELISA). In the group of patients with two and more revealed autoantibodies the higher indices in the number of IF alpha, TNF alpha and IL-6, and the decrease in the level of IL-4 comparing with the group of children with negative reaction to diabetes associated autoantibodies were more often observed.

Antibodies against the herbicide chlorsulfuron have been raised and characterized. Enzyme immunoassays (EIAs) for chlorsulfuron, involving labeled antigen or labeled antibodies, have been developed. The kinetics of antigen-antibody interactions in the EIA systems developed has been studied. Both systems exhibit equal sensitivity (1 ng/ml). The values of the coefficient of variation (CV), determined within the range of chlorsulfuron concentrations 1-100 ng/ml to be measured by the systems, are not in excess of 8%. The possibility of using glucose oxidase as a label in EIAs for chlorsulfuron has been demonstrated. Lack of cross-reactivity with a series of sulfonyl- and arylurea derivatives and triazines makes it possible to recommend the EIA systems developed for chlorsulfuron determination in the environment.

A patient with pulmonary hypertension and focal liver cirrhosis was hospitalized on the 12-th week of pregnancy because of threatened abortion and died after 2 days of hospital stay during which symptoms of portal and pulmonary hypertension progressed, decompensation of pulmonary heart and hepatic cellular insufficiency ensued and syndrome of disseminated intravascular coagulation developed.

Type II diabetes mellitus is the most common type of the condition. But little is yet known about mechanisms responsible for its development in persons with different body masses. We studied blood plasma levels of insulin and assessed sensitivity to it in 32 patients with freshly detected type II diabetes mellitus with normal and above normal body weight. Of these, 20 subjects were obese, the other 12 being not fat; the control group comprised 30 persons obese and non-obese, with n = 15 and 15 respectively. Recordable in patients was a marked elevation of blood plasma level of insulin accompanied by decrement of the peripheral tissue sensitivity to it in obese diabetic patients while in those persons with normal body mass there was a substantial reduction in the production of insulin, which differences may indicate different pathogenesis of development of type II diabetes mellitus in persons with different body masses.

Epidemiologic studies helped to disclose peculiarities of thyroid diseases prevalence in several districts of Rostov Province. High prevalence of thyroid diseases appeared to be proportional to degree of anthropogenic accumulation of industrial waste in the area. Intensity of the prevalence was independent on iodine deficiency. The materials collected prove technogenic factors to play an important role in development of thyroid diseases, so the prophylactics should consider not only correction of iodine intake.

Review of the literature and case report concern renal damage in hereditary osteo-onychodysplasia (nail-patella syndrome). This syndrome is a rare genetic disease inherited by the autosome-dominant type. It arises because of underdevelopment of tissues of mesodermal and ectodermal origin and is characterized by hypoplasia or absence of patella, ulnar dysplasia, nail plate dystrophy, growths on the iliac bones, renal affection. The disease was detected in early childhood. Renal affection presented with urinary syndrome with a concurrent nephrotic syndrome later on followed by moderate arterial hypertension and progression to renal failure. Renal biopsy revealed the picture of focal segmentary glomerular hyalinosis typical for this disorder. Because immunodepressants were thought inadequate, the treatment included lovastatin and inhibitors of angiotensin-converting enzyme to inhibit non-immune mechanisms of the disease progression.

The article is devoted to investigation of the Helicobacter pylori (HP) infection in 14 patients after resection of the stomach for ulcers of the gastroduodenal zone. Different methods were used for the assessment of the state of the gastric stump and gastroenteric anastomosis in the postoperative period. In most cases different degree of inflammatory alterations of the gastric mucosa were detected. In 2 cases (14.3%) peptic ulcers of the anastomosis developed. Changes to the gastric stump of 11 patients were associated with the HP infection which involved the mucosa of the anastomosed intestine in 8 patients. The data obtained point to possible participation of HP pathogenesis of the alterations detected after the stomach resection. The question of the expediency of a specific antibacterial therapy in the postoperative period and in treatment of certain kinds of postgastroresectional syndromes is raised.
