Thermoregulation during anesthesia – general or regional – is often significantly impaired. Any shifts from a body core temperature of 36.6°c±0.2°c results in either hyperthermia or hypothermia, causing pathophysiologic reactions. Hyperthermic disorders are very dangerously and critical for patients. Hyperthermia is caused by a variety of clinical states. It is important to differentiate controlled and uncontrolled hyperthermia and evaluate the underlying cause. In this paper we discuss most frequently pathologic state, monitoring of hyperthermic disorders and adequate clinical management. Temperature control occurs by feedback mechanisms operating through the preoptic area of the hypothalamus. Heat-sensitive neurons in this area increase their rate of firing during artificial heating. Receptors in the skin, spinal cord, abdominal viscera, and central veins primarily detect cold and provide feedback to the hypothalamus signaling an increase in heat production. The hypothalamus maintains body temperature at 37.1 C (98.78 F). Stimuli that begin to change the core temperature result in drastic changes in heat loss or production. Even a mild elevation in brain temperature may be detrimental to the hypoxic, ischemic and injured brain. Hyperthermic states may be caused by a wide variety of clinical disorders which are divided into two major groups: 1) Controlled hyperthermia – resulting from a deviation of thermoregulatory set points and thresholds, 2) Uncontrolled hyperthermia – resulting from impaired thermoregulatory responses or excessive heat production. Numerous studies have found pyrexia to be associated with increased mortality and morbidity after stroke. Early fever is associated with a poor Glasgow Coma Scale score in traumatic brain injury patients. Patients with subarachnoid hemorrhage are at increased risk for cerebral ischemia due to vasospasm. Hyperthermia may potentially worsen this vasospasm mediated brain injury. Several studies have shown hyperpyrexia to be an independent risk factor, predicting worse outcomes in TBI, SAH and ischemia. Blood in the cerebrospinal fluid induces fever in experimental models, and temperature is thus a likely marker for the primary severity of the hemorrhage.

The inability of any patient to maintain normal body temperature is indicative of a vast number of potentially serious disorders, including infections, neoplasms, shock, toxic reactions, and environmental exposures. Neurosurgical patient are especially vulnerable to temperature changes. More frequently, intraoperative thermal dysregulation results in hypothermia. Our chapter relates different ethiology, pathophisiology and clinical strategy of intraoperative hypothermia in neurosurgical patients.

The goal of endovascular repair of IAs is the occlusion of the proximal feeding arteries or obliteration of the aneurismal sac. Destruction of the sac while sparing the parent artery can be difficult, and can lead to distal embolism and rupture. The anesthesiologist must always be prepared for potential complications. This article provides a brief description of the principles of anesthesia for endovascular repair of intracranial aneurysms, as well as the relevant epidemiological, pathophysiological, diagnostic and therapeutic points. The goal of anesthesia in endovascular IA repair is to maintain an adequate hemodynamic profile and avoid decreases in cerebral perfusion pressure. Cerebral ischemia, increased intracranial pressure (ICP), and hemodynamic instability are intraoperative concerns.

While surgical approach and exposure are considered by surgeons, frequently, the importance of properly positioning the patient, accounting for the clinical situation and co-morbidities, is overlooked. This is particularly true in many neurosurgical procedures, which require prolonged anesthesia and immobilization. Traditionally, it is the anesthesiologists’ responsibility to look after the well being of the patient, and maintain an adequate physiological environment, promoting full recovery. In this review, we analyze the different positions common in neurosurgery considering and balancing between surgical exposure, hemodynamic implications, patient comfort and neurological outcomes. The latter should be given particular attention, as it might be difficult to differentiate between neurological deficits which are a result of the patient’s position, and those that are part of the underlying condition or surgical complication. Some positions interfere with standard monitoring or necessitate temporary interruption of or ventilation and continuous IV treatment. These as should be taken into account as well, reducing the interruption to the minimum. Finally, several complications are associated with specific positions and the approach to these complications is discussed. The review has been divided into two separate parts. In the previous issue of the journal we discussed the problems, related to head, neck and body positioning including supine and lateral position. Here we offer you the second part of the review, discussing benefits and problems related to prone and sitting position and specific complications including venous air embolism, blindness in prone position and pneumocephalus.

Early treatment is essential, yet diagnosis and management can be challenging. 85% of cases of SAH result from ruptured cerebral aneurysms, typically from berry aneurysms. At the time of the initial bleed, there is a critical reduction in cerebral blood flow (CBF) as the regional intracranial pressure (ICP) increases and approaches the systemic arterial pressure. The expanding mass effect of the hemorrhage and subsequent brain edema and hydrocephalus contributes to the acute rise in ICP. If IPC does not decrease rapidly after the initial sudden increase, death usually follows. Anesthesiologists, together with the neurosurgeon, play a vital role in the management of SAH. This article provides a brief description of the principles of anesthesia in the treatment of SAH, as well as the relevant epidemiological, pathophysiological, diagnostic and therapeutic points. The aim in anesthesia management is to prevent aneurysm rupture or rebleeding, and to avoid factors that may promote cerebral ischemia, vasospasm, cardiopulmonary dysfunction, hydrocephalus or electrolyte disturbances.
