BACKGROUND: Household contact investigation is an important strategy to identify individuals with tuberculosis (TB) exposure, infection, and disease, including those who may benefit from tuberculosis preventive therapy (TPT). Data in children exposed to rifampin-resistant TB are limited. METHODS: In preparation for and to inform the feasibility of an interventional trial, household contacts (HHC) of adults with pulmonary rifampin-resistant TB from high TB-burden countries were evaluated in a cross-sectional study. 9 páginas, 4 figuras, 1 tabla., Background: Molecular diagnostics are considered the most promising route to achieving rapid, universal drug susceptibility testing for Mycobacterium tuberculosiscomplex (MTBC). We aimed to generate a WHO endorsed catalogue of mutations to serve as a global standard for interpreting molecular information for drug resistance prediction. Methods: A candidate gene approach was used to identify mutations as associated with resistance, or consistent with susceptibility, for 13 WHO endorsed anti-tuberculosis drugs. 38,215 MTBC isolates with paired whole-genome sequencing and phenotypic drug susceptibility testing data were amassed from 45 countries. For each mutation, a contingency table of binary phenotypes and presence or absence of the mutation computed positive predictive value, and Fisher's exact tests generated odds ratios and Benjamini-Hochberg corrected p-values. Mutations were graded as Associated with Resistance if present in at least 5 isolates, if the odds ratio was >1 with a statistically significant corrected p-value, and if the lower bound of the 95% confidence interval on the positive predictive value for phenotypic resistance was >25%. A series of expert rules were applied for final confidence grading of each mutation. Findings: 15,667 associations were computed for 13,211 unique mutations linked to one or more drugs. 1,149/15,667 (7·3%) mutations were classified as associated with phenotypic resistance and 107/15,667 (0·7%) were deemed consistent with susceptibility. For rifampicin, isoniazid, ethambutol, fluoroquinolones, and streptomycin, the mutations' pooled sensitivity was >80%. Specificity was over 95% for all drugs except ethionamide (91·4%), moxifloxacin (91·6%) and ethambutol (93·3%). Only two resistance mutations were classified for bedaquiline, delamanid, clofazimine, and linezolid as prevalence of phenotypic resistance was low for these drugs. Interpretation: This first WHO endorsed catalogue of molecular targets for MTBC drug susceptibility testing provides a global standard for resistance interpretation. Its existence should encourage the implementation of molecular diagnostics by National Tuberculosis Programmes. Objective. To compare the epidemiology of antimicrobial resistance in bacteria isolated from inpatient and outpatient samples in Ecuador. Methods. A secondary analysis was done of data on bacteria isolated from inpatient and outpatient samples. Data were taken from the 2018 national antimicrobial resistance surveillance database of the National Reference Center for Antimicrobial Resistance. The variables included were: age, sex, inpatient versus outpatient setting, type of specimen, bacterial species identified, pattern of resistance to antibiotics, and geographic area. Results. Data from 57 305 bacterial isolates were included in the study: 48.8% were from hospitalized patients, 55.7% were from women, and 60.1% were from patients older than 45 years. Urine (42.9%) and blood (12.4%) were the most common clinical samples. Overall, 77.1% of bacterial isolates were gram-negative (83% and 71% in outpatients and inpatients, respectively). The most common gram-positive and gram-negative species were Staphylococcus aureus and Escherichia coli, respectively. Antimicrobial resistance levels were high (up to 80% for some antimicrobial drugs), and were higher in hospitalized patients compared with outpatients. A variety of carbapenemases were found to confer resistance to carbapenems (antibiotics of last resort) in gram-negative bacteria. Conclusions. The study findings provide an important baseline on antimicrobial resistance in Ecuador. This will allow the strengthening of guidelines of the surveillance system, the creation of public policies for standardization of laboratory methodologies, the proper handling of information, and the development of empirical therapy guidelines based on local epidemiology.

Objetivo: describir los principales factores asociados a la mortalidad en una cohorte de pacientes hospitalizados por neumonía por SARS-CoV-2 en un hospital público de Lima, Perú. Materiales y métodos: estudio de cohorte retrospectivo con muestreo no aleatorio por conveniencia. Se incluyó pacientes adultos con sospecha clínica o confirmados para SARS-CoV-2 con pruebas rápidas y/o moleculares. Se identificaron variables demográficas, clínicas, de laboratorio, tomográficas y de tratamiento. Se emplearon técnicas de estadística descriptiva, análisis bivariado y de regresión de Poisson para determinar el riesgo relativo (RR) ajustado. Resultados: se incluyó 122 pacientes, 70,5% del sexo masculino, edad promedio de 55,8 años, con antecedentes de obesidad (25,4%) e hipertensión arterial (HTA 13,1%). El análisis multivariado de los factores clínicos identificó que la edad (RR ajustado [RRa] 1,03; IC 95%: 1,00-1,06, p=0,021), el índice de masa corporal (IMC, RRa 1,03; IC 95%: 1,01-1,05; p=0,006), la HTA (RRa 1,68; IC 95%: 1,09-2,56; p=0,017), el índice de PaO2/FiO2 (RRa 0,99; IC 95%: 0,99-1,00; p

Setting The detection of multidrug-resistant tuberculosis (MDR-TB) using rapid drug susceptibility testing (DST) has increased steadily in recent years in Peru, from 9216 tests in 2010 to 27 021 tests in 2015. Research examining the impact of rapid DST on treatment outcomes is required. Objective To evaluate the association between rapid DST use (nitrate reductase assay, microscopic observation drug susceptibility assay [MODS] and GenoType® MTBDRplus) and treatment outcomes and mortality in MDR-TB patients in Peru. Design Retrospective cohort study of patients diagnosed with pulmonary MDR-TB between 2010 and 2013 (with treatment outcomes up to December 2015) using the electronic registry of the Peruvian National TB Programme. Results A total of 2671 MDR-TB patients were included; the median age was 27 years, 2.8% were co-infected with the human immunodeficiency virus. Use of rapid DST was associated with a 40% increase in the adjusted odds of treatment success (aOR 1.40, 95%CI 1.19-1.64) and a 54% reduction in mortality (aOR 0.46, 95%CI 0.33-0.64). Higher treatment success rates were driven by MODS and GenoType® MTBDRplus testing (aORs for unsuccessful outcomes respectively 0.68 and 0.66). Conclusion The use of rapid DST (MODS and MTBDRplus) to diagnose MDR-TB was associated with a reduction in the odds of death and a substantial increase in the odds of treatment success.

Determine the incidence of loss to follow-up (LTFU) in patients treated for rifampicin-resistant tuberculosis (RR-TB) or multidrug-resistant tuberculosis (RR/MDR-TB), and the factors associated with this discharge status in Ecuador.Retrospective cohort study of patients with RR/MDR-TB who followed the World Health Organization's 18-24-month treatment regimen in 2014 and 2015, as reported by the Ministry of Health of Ecuador. The incidence of LTFU was determined, and clinical and epidemiological manifestations of cases discharged as LTFU were compared with those discharged as successfully treated. Survival was analyzed with Cox regression in order to evaluate factors associated with LTFU.Of 328 cases, 270 (82.3%) were analyzed because they had a reported discharge status. Discharge as LTFU accounted for 39.6% of cases, and as successfully treated, 50.4%. The risk factors associated with LTFU were: previous discharge as LTFU in a previous TB episode [hazard ratio (HR): 2.96 (1.53-5.73),The incidence of LTFU in patients with RR/MDR-TB in Ecuador is above the average for the Region of the Americas. The three identified factors support implementation of shorter regimens and patient-centered care, in line with the End TB Strategy.Determinar o percentual de perda de seguimento de pacientes tratados para tuberculose resistente à rifampicina (TB-RR) ou tuberculose multirresistente a medicamentos (TB-MR) e os fatores associados à interrupção do tratamento no Equador.Estudo de coorte retrospectivo de casos de pacientes com TB-RR/TB-MR tratados em 2014 e 2015 com o esquema farmacológico de 18 a 24 meses de duração da Organização Mundial da Saúde (OMS) que foram notificados ao Ministério da Saúde do Equador. Foi determinado o percentual de perda de seguimento e foram comparadas as características clínicas e epidemiológicas dos casos de interrupção do tratamento por perda de seguimento e daqueles com alta por sucesso no tratamento. Uma análise da sobrevida com o modelo de regressão de Cox foi realizada para avaliar os fatores associados à perda de seguimento.De 328 casos registrados, 270 (82,3%) foram incluídos na análise por terem tido sua interrupção ou alta notificadas. Houve interrupção por perda de seguimento em 39,6% dos casos e alta por sucesso no tratamento em 50,4%. Os fatores de risco associados à perda de seguimento foram: história de perda de seguimento em tratamento anterior de TB, razão de riscos (O percentual de perda de seguimento de pacientes com TB-RR/TB-MR no Equador está acima da média da Região das Américas. Os três fatores identificados no estudo reforçam ser necessário implementar esquemas de tratamento mais curtos e prestar atenção centrada no paciente, segundo as recomendações da Estratégia pelo Fim da Tuberculose.

Background Household contacts (HHCs) of individuals with multidrug-resistant tuberculosis (MDR-TB) are at high risk of infection and subsequent disease. There is limited evidence on the willingness of MDR-TB HHCs to take MDR-TB preventive therapy (MDR TPT) to decrease their risk of TB disease. Methods In this cross-sectional study of HHCs of MDR-TB and rifampicin-resistant tuberculosis (RR-TB) index cases from 16 clinical research sites in 8 countries, enrollees were interviewed to assess willingness to take a hypothetical, newly developed MDR TPT if offered. To identify factors associated with willingness to take MDR TPT, a marginal logistic model was fitted using generalized estimating equations to account for household-level clustering. Results From 278 MDR-TB/RR-TB index case households, 743 HHCs were enrolled; the median age of HHCs was 33 (interquartile range, 22–49) years, and 62% were women. HHC willingness to take hypothetical MDR TPT was high (79%) and remained high even with the potential for mild side effects (70%). Increased willingness was significantly associated with current employment or schooling (adjusted odds ratio [aOR], 1.83 [95% confidence interval {CI}, 1.07–3.13]), appropriate TB-related knowledge (aOR, 2.22 [95% CI, 1.23–3.99]), confidence in taking MDR TPT (aOR, 7.16 [95% CI, 3.33–15.42]), and being comfortable telling others about taking MDR TPT (aOR, 2.29 [95% CI, 1.29–4.06]). Conclusions The high percentage of HHCs of MDR-TB/RR-TB index cases willing to take hypothetical MDR TPT provides important evidence for the potential uptake of effective MDR TPT when implemented. Identified HHC-level variables associated with willingness may inform education and counseling efforts to increase HHC confidence in and uptake of MDR TPT.

BACKGROUND: In Peru, a treatment approach for extensively drug-resistant tuberculosis (XDR-TB) incorporating World Health Organization Group 5 drugs and patient-centred care has achieved 65% success. To extend this approach to pre-XDR-TB patients, we evaluated this population separately. OBJECTIVE: To assess programmatic management of pre-XDR-TB. METHOD: Retrospective study using the official national registry from 2011 to 2014. Cases were separately evaluated according to resistance to fluoroquinolones (FQs) (pre-XDR-F) or to second-line injectables (SLIs) (pre-XDR-I). RESULTS: Of 610 pre-XDR-TB patients, 120 (20%) had pre-XDR-F and 490 (80%) had pre-XDR-I. Pre-XDR-F cases were older (34 years vs. 28 years, P < 0.001) and a higher proportion had previously received two or more regimens (70% vs. 38%, P < 0.001). Among the 452 patients who started treatment in 2011-2013, treatment success was 43.3%, 26.5% were lost to follow-up, 12.1% died and 13.7% failed treatment. Success was higher in pre-XDR-I (48.5%) than pre-XDR-F (21.4%) patients. History of previous treatment (OR 2.23, 95%CI 1.52-3.38) and pre-XDR-F (OR 2.39, CI 1.18-4.83) were associated with unsuccessful outcomes. CONCLUSION: Programmatic management of pre-XDR-TB has not been successful, particularly in pre-XDR-F patients, with lower rates of success than those achieved in the same setting for XDR-TB. The strategy used for XDR-TB should be extended to pre-XDR-TB patients in Peru.

Tuberculosis (TB) is the number one infectious disease killer and exemplifies the most neglected of them. Drug-susceptible TB presents with high mortality especially in atypical forms, disproportionally affecting immunosuppressed and vulnerable populations. The drug-resistant TB (DR-TB) epidemic, a world crisis, is sustained and increased through person-to-person transmission in households and the community. TB diagnostics and treatment in recent years are highly evolving fields. New rapid molecular tests are changing the perspectives in diagnosis and resistance screening. Also, new drugs and shorter regimens for DR-TB are appearing. For the first time in recent history, a large number of randomized control trials are incoming. Areas covered: This article reviews most TB advances including new diagnostic tests, drugs, and regimens and outlines upcoming drug trials while disclosing the potential gaps the in development of patient-centered systems and current organizational challenges leading to a delay in the uptake of these innovations. Expert commentary: Innovations are occurring, but not many are implemented on a wide scale in developing countries. TB health systems and staff are not getting updated in parallel. More efforts and funds are needed not only to implement current novelties but also to research for future solutions to eliminate TB.

Objetivo. Determinar la incidencia de pérdida en el seguimiento (PEES) en pacientes tratados por tuberculosis (TB) resistente a rifampicina o multidrogorresistente (TB-RR/MDR) y los factores asociados a esta condición de egreso en Ecuador. Métodos. Estudio de cohorte retrospectivo de pacientes con TB-RR/MDR tratados con el esquema de 18 a 24 meses de la Organización Mundial de la Salud en 2014 y 2015 notificados al Ministerio de Salud del Ecuador. Se determinó la incidencia de PEES y se compararon las características clínicas y epidemiológicas de los casos egresados como PEES versus los egresados como éxito de tratamiento. Se analizó la sobrevida con regresión de Cox para evaluar factores asociados a PEES. Resultados. De 328 casos, 270 (82,3%) fueron analizados porque tuvieron condición de egreso notificada. El egreso como PEES fue 39,6% y el éxito de tratamiento 50,4%. Los factores de riesgo asociados a PEES fueron: antecedente de egreso como PEES en episodio previo de TB, cociente de riesgos instantáneos (HR, por sus siglas en inglés): 2,96 (1,53-5,73), P < 0,001; adicción al alcohol o drogas, HR: 2,82 (1,10-7,23), P = 0,031 y tener diagnóstico por la prueba Xpert® (TB-RR), HR: 1,53 (1,0-2,35), P = 0,048. Del total de PEES, 43% ocurrió después de nueve meses de tratamiento. Conclusión. La incidencia de PEES en pacientes con TB-RR/MDR en Ecuador está por encima del promedio en la Región de las Américas. Los tres factores identificados refuerzan la implementación de regímenes acortados y atención centrada en el paciente, siguiendo la Estrategia Fin a la Tuberculosis.

There is enough evidence to declare tuberculosis as an occupational disease among healthcare workers. In Peru, there are regulations granting employment rights regarding tuberculosis as an occupational disease, such as healthcare coverage for temporary or permanent disability. However, these rights have not been sufficiently socialized. This study presents information on the risk of acquiring tuberculosis in the workplace, and a review of the evidence to declare tuberculosis as an occupational disease among health care workers, presenting the current Peruvian law related. Existe evidencia suficiente para declarar a la tuberculosis como enfermedad ocupacional en diversos profesionales especialmente entre los trabajadores de salud. En el Perú están normados y reglamentados los derechos laborales inherentes a la tuberculosis como enfermedad ocupacional, como la cobertura por discapacidad temporal o permanente. Sin embargo, estos derechos aún no han sido suficientemente socializados. En este trabajo se presenta información sobre el riesgo de adquirir tuberculosis en el lugar de trabajo, se revisan las evidencias para declarar a la tuberculosis como enfermedad ocupacional en trabajadores de salud y se presenta la legislación peruana vigente al respecto.

We present the case of a full-term female newborn, whose mother died seven days postpartum from multi-organ failure due to severe dengue confirmed by NS1 antigen detection and positive IgM. The newborn did not have any complication, but at the fourth day of life she developed fever, jaundice, signs of plasma leakage, thrombocytopenia, hepatomegaly, ascitis, and others signs of systemic inflammation response syndrome. She fully recovered with supportive treatment. The RT-PCR test of a peripheral blood sample revealed a positive result for the dengue virus serotype 2, confirming the first case of neonatal dengue reported in Peru.

OBJETIVOS: Determinar la prevalencia de anticuerpos contra Trypanosoma cruzi en puerperas y la posible transmision congenita de la enfermedad de Chagas en Arequipa, Peru, una zona donde esta enfermedad es endemica. METODOS: Se estudio a las puerperas que dieron a luz entre diciembre de 2001 y julio de 2002 en tres hospitales (dos urbanos y uno rural) y cuatro centros de salud (tres rurales y uno urbano) del departamento de Arequipa, Peru. El estudio serologico comprendio el tamizaje de todas las puerperas para detectar anticuerpos contra T. cruzi mediante inmunofluorescencia indirecta (IFI); la prueba de inmunoadsorcion enzimatica (ELISA) y la titulacion de anticuerpos IgG por IFI se usaron como pruebas confirmatorias. A las puerperas con seropositividad y a sus recien nacidos se les realizo la prueba de deteccion de anticuerpos IgM mediante IFI y se evaluo la presencia de infeccion mediante xenodiagnostico (evaluada a los 30 y 60 dias) y el micrometodo de Freilij. Los resultados se analizaron segun la presencia del vector y de casos de enfermedad de Chagas en los lugares de nacimiento y de residencia de las puerperas. Dos neonatologos evaluaron clinicamente a los recien nacidos para detectar anomalias y signos de enfermedad de Chagas congenita. RESULTADOS: La prevalencia general de enfermedad de Chagas en las 3 000 puerperas estudiadas fue de 0,73%; fue mayor en dos centros de salud ubicados en zonas rurales (2,2% en El Pedregal y 4,1% en La Joya) (P = 0,018) y la enfermedad estuvo asociada con el contacto directo previo con el vector (P < 0,05) y con el haber nacido en una zona considerada endemica (P < 0,01). Cuatro de las 20 puerperas con seropositividad (20%) tuvieron resultados positivos en el xenodiagnostico. Ninguna conocia su estado de portadora de la infeccion y no se observaron sintomas o signos caracteristicos de la enfermedad de Chagas aguda o cronica. En ninguna puerpera se detectaron anticuerpos IgM y solo un neonato (nacido de una madre sin parasitemia) presento un titulo de IgM de 1/8, pero en los controles posteriores no se detectaron anticuerpos IgM o IgG. No se detectaron parasitos en la sangre de los neonatos por ninguno de los dos metodos empleados. De los 20 neonatos evaluados, uno tenia microcefalia y hepatoesplenomegalia y aunque tenia anticuerpos especificos IgG contra T. cruzi al nacer, estos desaparecieron a los dos meses; el crecimiento y el desarrollo de los demas recien nacidos fueron normales. CONCLUSION: La prevalencia de enfermedad de Chagas en puerperas del departamento de Arequipa, Peru, es baja. No se encontraron casos de transmision congenita intrauterina. Se recomienda disenar estudios de deteccion prenatal que permitan evaluar a un mayor numero de madres y en el que participen tambien las mujeres que dan a luz en sus domicilios.

Tuberculosis (TB) is the first cause of death by an infectious agent in the world, the incidence in the population is declining very slowly and drug resistance is currently considered an international crisis. In Peru, the recent TB Prevention and Control Act in Peru (Law 30287) declares the fight against TB of national interest. In recent years, the Ministry of Health’s (MINSA) National Health Strategy for the Prevention and Control of Tuberculosis (ESNPCT) has achieved significant progress in the control of this disease; however, challenges still remain to be addressed. This article reviews the epidemiological situation of TB in Peru, systematizes the progress achieved during the management of the ESNPCT team between the years 2011 and 2015 from the biomedical approach, public management and social determinants of health, also posing challenges to achieving TB control under law 30287 and the “End of TB” strategy of the World Health Organization (WHO).

Objetivos. Implementar un sistema para el diagnóstico remoto de tuberculosis y multidrogorresistencia (MDR) usando el método Microscopic-Observation Drug Susceptibility Assay (MODS) en el Laboratorio de Micobacterias del Centro de Excelencia en Tuberculosis de Trujillo (CENEX-Trujillo). El sistema incluyó una variante de un algoritmo de reconocimiento de Mycobacterium tuberculosis recientemente reportado a partir de imágenes digitales de cultivos MODS de muestras de esputo. Materiales y métodos. Se optimizó un algoritmo de reconocimiento por medio de un reentrenamiento del modelo estadístico basado en imágenes digitales de cultivos MODS provenientes del Laboratorio de Micobacterias del CENEX-Trujillo. Se obtuvieron imágenes de 50 cultivos MODS positivos de pacientes con sospecha de tuberculosis multidrogorresistente entre enero y octubre de 2012 en el CENEX-Trujillo. Resultados. La sensibilidad y la especificidad en objetos, para reconocer cordones de tuberculosis fueron de 92,04% y de 94,93% respectivamente. La sensibilidad y la especificidad en foto, para determinar un campo positivo a tuberculoisis fueron 95,4% y de 98,07% respectivamente. Conclusiones. Los resultados demostraron la factibilidad de la implementación de telediagnóstico en lugares remotos, el cual puede contribuir con la detección temprana de tuberculosis multidrogorresistente mediante el método MODS Objectives. To implement a system for remote diagnosis of tuberculosis and multidrug resistance (MDR) using the Microscopic-Observation Drug Susceptibility Assay (MODS) method in the Mycobacteria Laboratory, Trujillo Center of Excellence in Tuberculosis (CENEX-Trujillo). The system included a variant of an algorithm for recognition of Mycobacterium tuberculosis recently reported from digital images of MODS cultures of sputum samples. Materials and methods. The recognition algorithm was optimized using a retraining statistical model based on digital images of MODS cultures from CENEX-Trujillo. Images of 50 positive MODS cultures of patients with suspected multidrug-resistant tuberculosis were obtained between January and October 2012 in the CENEX-Trujillo. Results. The sensitivity and specificity to recognize strings of tuberculosis were 92.04% and 94.93% respectively using objects. The sensitivity and specificity to determine a positive tuberculosis field were 95.4% and 98.07% respectively using pictures. Conclusions. The results demonstrated the feasibility of the implementation of telediagnostics in remote locations, which may contribute to the early detection of multidrug-resistant tuberculosis by MODS method

To the Editor: While there have been impressive gains in the global control of tuberculosis (TB) over the past two decades, TB remains a leading cause of death and efforts to decrease its burden have been limited by the rise of drug resistant strains [1]. As drug-resistant TB remains exceedingly difficult and costly to treat, more research is needed to identify areas for improving primary prevention of TB. The risk of TB transmission is increased whenever there is overcrowding, poor ventilation and exposure to an infected individual, and public transport has been identified as a potential setting with increased risk for TB transmission [2]. Indeed, recent research demonstrates the fraction of rebreathed air on public transport is mathematically correlated with a higher risk of contracting TB [3]. Previous investigations using cross-sectional data in Lima, Peru, have demonstrated that community, rather than household, transmission may account for up to 70% of incident infections [4]. Studies conducted in Lima found an increased risk of TB infection among individuals who rode minibuses [5] and those who worked on public transport [6]. However, these studies were limited by misclassification of TB diagnosis, imprecise time variables and wide confidence intervals. The objective of our study was to assess the association between use of public transportation and active TB using a detailed transportation questionnaire, multiple control groups and improved TB diagnostics. En el Perú están normados y reglamentados los derechos laborales inherentes a la tuberculosis como enfermedad ocupacional, como la cobertura por discapacidad temporal o permanente. Sin embargo, estos derechos aún no han sido suficientemente socializados. En este trabajo se presenta información sobre el riesgo de adquirir tuberculosis en el lugar de trabajo, se revisan las evidencias para declarar a la tuberculosis como enfermedad ocupacional en trabajadores de salud y se presenta la legislación peruana vigente al respecto.

The Genotype®MTBDRplus molecular test is a method that allows identification of the most frequent mutations associated with resistance to major first-line antituberculosis drugs, Isoniazid (INH) and Rifampicin (RFP). The aim of this study was to evaluate the performance of the molecular test with culture and smear- positive sputum samples. We evaluated 95 cultures and 100 sputum samples with resistance profiles previously determined by the reference method "Agar Plate Proportions" (APP). The molecular test from cultures showed a sensitivity of 100 %, 97,5 % and 96,97 % for RIF, INH and MDR respectively while from sputums the sensitivity was 95,65 %, 96,77 % and 95,24 % for RIF, INH and MDR respectively. We conclude that the molecular test Genotype®MTBDRplus is a very useful tool to detect resistance to isoniazid and rifampicin simultaneously (MDR-TB) in up to 72 hours from sputum samples or cultures.

We present the case of a full-term female newborn, whose mother died seven days postpartum from multi-organ failure due to severe dengue confirmed by NS1 antigen detection and positive IgM. The newborn did not have any complication, but at the fourth day of life she developed fever, jaundice, signs of plasma leakage, thrombocytopenia, hepatomegaly, ascitis, and others signs of systemic inflammation response syndrome. She fully recovered with supportive treatment. The RT-PCR test of a peripheral blood sample revealed a positive result for the dengue virus serotype 2, confirming the first case of neonatal dengue reported in Peru.

To determine the prevalence of antibodies against Trypanosoma cruzi in puerperal women and to assess possible congenital transmission of Chagas' disease in the department of Arequipa, Peru, where the disease is endemic.Women who had given birth between December 2001 and July 2002 in three hospitals (two urban and one rural) and four health centers (three rural and one urban) of the department of Arequipa, Peru, were studied. The serological study included screening all the puerperal women in order to detect antibodies against T. cruzi through indirect immunofluorescence (IIF), with confirmatory testing done with enzyme-linked immunosorbent assay (ELISA) testing and the titration of immunoglobulin G (IgG) antibodies by IIF. IIF tests to screen for immunoglobulin M (IgM) antibodies were done with the seropositive women and their newborns, and infection was evaluated through xenodiagnosis (evaluated at 30 and 60 days) and the direct micromethod of Freilij et al. The results were analyzed in terms of the presence of the vector and of cases of Chagas' disease in the places where the puerperal women had been born and where they were living. Two neonatologists clinically evaluated the newborns in order to detect abnormalities and signs of congenital Chagas' disease.The overall prevalence of Chagas' disease in the 3 000 puerperal women studied was 0.73%. Prevalence was highest in two health centers located in rural areas (2.2% in El Pedregal and 4.1% in La Joya) (P=0.018). The disease was associated with previous direct contact with the vector (P0.05) and with having been born in an area considered endemic (P0.01). Four (20%) of the 20 seropositive puerperal women were also positive by xenodiagnosis. However, none of the women was aware of her infectious carrier state, and none showed the characteristic symptoms or signs of acute or chronic Chagas' disease. IgM antibodies were not detected in any of the puerperal women. One neonate (whose mother did not have evidence of parasitemia) presented an IgM titer of 1/8, but in later controls neither IgM nor IgG antibodies were detected. Parasites were not detected in the blood of the neonates by either of the two testing methods used. Of the 20 neonates evaluated, one presented microcephaly and hepatosplenomegaly; although the child had specific IgG antibodies against T. cruzi at birth, the antibodies were not present at the age of two months. The growth and development of the other 19 newborns were normal.The prevalence of Chagas' disease in puerperal women of the department of Arequipa, Peru, is low. No cases of intrauterine congenital transmission were found. We recommend carrying out studies on prenatal detection that evaluate more mothers and in which women who give birth at home also participate.

Background Peru holds the fourth highest burden of tuberculosis in the Americas. Despite an apparently well-functioning DOTS control program, the prevalence of multidrug resistant tuberculosis (MDR-TB) continues to increase. To worsen this situation, cases of extensively drug resistance tuberculosis (XDR-TB) have been detected. Little information exists about the genetic diversity of drug-susceptible vs. MDR-TB and XDR-TB. Methods Cryopreserved samples of XDR strains from 2007 to 2009 (second semester), were identified and collected. Starting from 227 frozen samples, a total of 142 XDR-TB strains of Mycobacterium tuberculosis complex (MTBC; 1 isolate per patient) were retained for this study. Each strain DNA was analyzed by spoligotyping and the 15-loci Mycobacterial Interspersed Repetitive Unit (MIRU-15). Results Among the 142 isolates analyzed, only 2 samples (1.41%) could not be matched to any lineage. The most prevalent sublineage was Haarlem (43.66%), followed by T (27.46%), LAM (16.2%), Beijing (9.15%), and X clade (1.41%). Spoligotype analysis identified clustering for 128/142 (90.1%) isolates vs. 49/142 (34.5%) with MIRUs. Of the samples, 90.85% belonged to retreated patients. The drug resistant profile demonstrated that 62.67% showed resistance to injectable drugs capreomycin (CAP) and kanamycin (KAN) vs. 15.5% to CAP alone and 21.8% to KAN alone. The SIT219/T1 and SIT50/H3 were the most prevalent patterns in our study. The spoligoforest analysis showed that SIT53/T1 was at the origin of many of the T lineage strains as well as a big proportion of Haarlem lineage strains (SIT50/H3, followed by SIT47/H1, SIT49/H3, and SIT2375/H1), as opposed to the SIT1/Beijing strains that did not appear to evolve into minor Beijing sublineages among the XDR-TB strains. Conclusion In contrast with other Latin-American countries where LAM sublineage is the most predominant, we found the Haarlem to be the most common followed by T sublineage among the XDR-TB strains.

Objetivo: Determinar las susceptibilidad a diversos antibacterianos de tres categorías de S. aureus según su sensibilidad a la meticilina. Materiales y Métodos: Se aislaron 76 cepas de S. aureus que colonizaban pacientes y personal de salud de tres servicios del Hospital Honorio Delgado de Arequipa, de los cuales 36 fueron sensibles a meticilina (MSSA), 15 con susceptibilidad "borderline" (BORSA) y 25 fueron resistentes a la meticilina (MRSA). Se les sometió a antibiograma a 14 antibacterianos. Resultados: El único antibiótico al cual todas los aislamientos fueron sensibles fue vancomicina. Los MRSA fueron resistentes a todos los b-lactámicos excepto imipenem que obtuvo 64% de susceptibilidad. Los BORSA mantuvieron un 80% de sensibilidad a cefalotina y un 100% al imipenem.
