The purpose of the study is to examine the degree of knowledge and practice of personal finance of the non-teaching staff at a private university and to determine if the degree of knowledge of the respondents in terms of the variables under personal finance has a significant relationship to the degree of practice. The formulated questions were addressed using a descriptive-correlational approach. Fifty (50) non-teaching staff were chosen to participate in the data collection using simple random sampling and then examined the gathered data using the weighted mean and Pearson correlation coefficient. The study’s findings showed that the non-teaching staff who participated have a modest knowledge and practice of the indicators considered under personal finance. On the one hand, the study revealed that the degree of knowledge and practice of budgeting, saving, spending, insurance, and investment correlate, resulting in rejecting the null hypothesis. On the other hand, the degree of knowledge and practice of emergency fund and debt management showed no correlation that led the researchers not to reject the null hypothesis. The researchers concluded that non-teaching staff generally have sufficient understanding and execution of budgeting, saving, spending, emergency fund, debt management, insurance, and investment, yet in terms of the degree of the practice of setting emergency funds and investing money, have both obtain poor responses. Furthermore, recommendations and an action plan were provided to help them achieve financial robustness.

Vitamin D deficiency is associated with an increased risk of prostate cancer mortality and is hypothesized to contribute to prostate cancer aggressiveness and disparities in African American populations. The prostate epithelium was recently shown to express megalin, an endocytic receptor that internalizes circulating globulin-bound hormones, which suggests regulation of intracellular prostate hormone levels. This contrasts with passive diffusion of hormones that is posited by the free hormone hypothesis. Here, we demonstrate that megalin imports testosterone bound to sex hormone-binding globulin into prostate cells. Prostatic loss of Lrp2 (megalin) in a mouse model resulted in reduced prostate testosterone and dihydrotestosterone levels. Megalin expression was regulated and suppressed by 25-hydroxyvitamin D (25D) in cell lines, patient-derived prostate epithelial cells, and prostate tissue explants. In patients, the relationships between hormones support this regulatory mechanism, as prostatic DHT levels are higher in African American men and are inversely correlated with serum 25D status. Megalin levels are reduced in localized prostate cancer by Gleason grade. Our findings suggest that the free hormone hypothesis should be revisited for testosterone and highlight the impact of vitamin D deficiency on prostate androgen levels, which is a known driver of prostate cancer. Thus, we revealed a mechanistic link between vitamin D and prostate cancer disparities observed in African Americans.Significance:These findings link vitamin D deficiency and the megalin protein to increased levels of prostate androgens, which may underpin the disparity in lethal prostate cancer in African America men.

Introduction: When analyzing the current situation in Spain, derived from our current lifestyles far from following a Mediterranean lifestyle, there is an alarming prevalence of diabesity, which increases the global risk of suffering from cardiovascular disease (CVD) and decreases the quality of life related to health (QLRH). In order to guarantee, beyond glycemic control of patients with type 2 diabetes (DM2), adequate control of cardiovascular risk factors (CVRF) in DM2, it will be necessary to carry out a community intervention focused on the application of education programs focused on promoting the acquisition of healthy eating habits through the Mediterranean diet (DMED). This dietary pattern, together with physical exercise, has been shown to contribute to improving the QLRH of patients. Objective: To analyze the quality of life related to health, present in patients with poorly controlled type 2 diabetes, to determine the possible relationship between this and the degree of adherence to the Mediterranean diet and to examine whether there are differences between the sexes. Material and methods: Observational descriptive study in 93 patients diagnosed with DM2 with poor glycemic control (1Ac ≥ 7%), carried out in various health centers in Albacete and Cuenca, in which the baseline relationship between adherence to DMed and the HRQoL. Background The association of several immune-mediated inflammatory diseases (IMIDs) in the same patient is well known. IMID, especially joint and cutaneous diseases, are frequently diagnosed in patients with inflammatory bowel disease (IBD). However, the degree of knowledge that patients with IBD have about the coexistence of other IMIDs is little studied. Our aim was to evaluate the accuracy of self-reporting of IMID by patients with IBD. Methods Prospective, unicentric study that included patients attended in person at the IBD Unit of the Central University Hospital of Asturias (Spain) between August 2020 and December 2021. Patients were invited to participate in the study, and, after signing the informed consent, they answered a questionnaire about the presence or not of 50 IMIDs (self-reported diagnosis). The diagnosis of an IMID was confirmed in the medical records of each patient (reference diagnosis). Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy (proportion of subjects correctly classified) of self-reporting was calculated. Statistical analyses were performed with R software version 4.0.2. Results A total of 1,621 patients were included. Forty-four percent of patients had ulcerative colitis, 53% had Crohn's disease and 3% had an IBD-unclassified, 50% were men, and the age at diagnosis of IBD was 38±15.2 years. Seven hundred and twelve patients were receiving mesalazine, 67 corticosteroids, 394 immunomodulators and 532 biologics. Six hundred and twenty-seven (39%) patients were diagnosed with at least one IMID, 177 (10.9%) with two, 49 (3%) with three, and 17 (1%) with more than three. Sixteen percent of patients had a joint IMID and 15% a cutaneous IMID. Self-reporting of IMID by IBD patients has a sensitivity, specificity, PPV, NPV and accuracy of 0.75 (95% CI 0.73-0,78), 0.89 (95% CI 0.86-0.91), 0.92 (95% CI 0.89-0.93), 0.69 (95% CI 0.66-0.73) and 0.81 (95% CI 0.79-0.83), respectively. Table 1 shows the results by IMID groups (joint, cutaneous, digestive, endocrinological, ocular, others), and Table 2 shows the results of the most frequent IMIDs. Conclusion Eighty-one percent of patients with IBD are able to correctly identify the coexistence or not of IMIDs. It was observed that in the case of joint IMIDs there is a significant percentage of patients who do not know whether or not they suffer from one of these diseases.

Identifying climate refugia is key to effective biodiversity conservation under a changing climate, especially for mountain-specialist species adapted to cold conditions and highly threatened by climate warming. We combined species distribution models (SDMs) with climate forecasts to identify climate refugia for high-elevation bird species (Lagopus muta, Anthus spinoletta, Prunella collaris, Montifringilla nivalis) in the European Alps, where the ecological effects of climate changes are particularly evident and predicted to intensify. We considered future (2041-2070) conditions (SSP585 scenario, four climate models) and identified three types of refugia: (1) in-situ refugia potentially suitable under both current and future climate conditions, ex-situ refugia suitable (2) only in the future according to all future conditions, or (3) under at least three out of four future conditions. SDMs were based on a very large, high-resolution occurrence dataset (2901-12,601 independent records for each species) collected by citizen scientists. SDMs were fitted using different algorithms, balancing statistical accuracy, ecological realism and predictive/extrapolation ability. We selected the most reliable ones based on consistency between training and testing data and extrapolation over distant areas. Future predictions revealed that all species (with the partial exception of A. spinoletta) will undergo a range contraction towards higher elevations, losing 17%-59% of their current range (larger losses in L. muta). Abstract: News of unaccompanied minors (UMs) from Central America entering the United States–Mexico border have inundated political and social platforms as policies are discussed to address this crisis. The hope for a future away from violence, poverty, and fear drives these minors into embarking on an uncertain journey to the United States. The dangers these children encounter on their long travels along with previous distressing experiences from their home country put a devastating toll on their psychological and overall well-being. Their anguish continues as these youth are placed in transitional settings (i.e., shelters) while awaiting to be released into their new communities. While psychological care is often available at these settings, the complex realities of these children call for culturally and trauma-responsive guidelines to address their unique psychosocial needs. This conceptual paper reviews existent literature on the mental health needs of Central American youth in the United States–Mexico border as well as relevant theories and principles, such as the evidence-informed psychological first aid (PFA), which has shown to help Central American youth in the immediate aftermath of disasters and other crises. The aim of this manuscript was to adapt and expand on the core elements of PFA and utilize its theoretical basis to (1) enhance the psychological health of Central American UMs in transitional settings and (2) equip mental health professionals working in the United States–Mexican border or other US mental health professionals to address the immediate psychological needs of UMs from a culturally sensitive and trauma-informed approach.

KDM5B (lysine[K]-specific demethylase 5B) is frequently upregulated in various human cancers including prostate cancer. KDM5B controls H3K4me3/2 levels and regulates gene transcription and cell differentiation, yet the contributions of KDM5B to prostate cancer tumorigenesis remain unknown. In this study, we investigated the functional role of KDM5B in epigenetic dysregulation and prostate cancer progression in cultured cells and in mouse models of prostate epithelium–specific mutant Pten/Kdm5b. Kdm5b deficiency resulted in a significant delay in the onset of prostate cancer in Pten-null mice, whereas Kdm5b loss alone caused no morphologic abnormalities in mouse prostates. At 6 months of age, the prostate weight of Pten/Kdm5b mice was reduced by up to 70% compared with that of Pten mice. Pathologic analysis revealed Pten/Kdm5b mice displayed mild morphologic changes with hyperplasia in prostates, whereas age-matched Pten littermates developed high-grade prostatic intraepithelial neoplasia and prostate cancer. Mechanistically, KDM5B governed PI3K/AKT signaling in prostate cancer in vitro and in vivo. KDM5B directly bound the PIK3CA promoter, and KDM5B knockout resulted in a significant reduction of P110α and PIP3 levels and subsequent decrease in proliferation of human prostate cancer cells. Conversely, KDM5B overexpression resulted in increased PI3K/AKT signaling. Loss of Kdm5b abrogated the hyperactivation of AKT signaling by decreasing P110α/P85 levels in Pten/Kdm5b mice. Taken together, our findings reveal that KDM5B acts as a key regulator of PI3K/AKT signaling; they also support the concept that targeting KDM5B is a novel and effective therapeutic strategy against prostate cancer.Significance:This study demonstrates that levels of histone modification enzyme KDM5B determine hyperactivation of PI3K/AKT signaling in prostate cancer and that targeting KDM5B could be a novel strategy against prostate cancer.

Compelling evidence has accumulated for the role of oxidative stress on the endothelial cell (EC) dysfunction underlying acute coronary syndromes. However, understanding the metabolic determinants of EC dysfunction has been hampered by the scarcity of appropriate cell models. Here, we have generated and phenotypically characterized EC derived from thrombectomy specimens in patients with acute myocardial infarction (AMI). We have found that AMI-derived endothelial cells (AMIECs), but not control EC from health coronary arteries, display impaired growth, migration and tubulogenesis. These phenotypic abnormalities were accompanied with metabolic abnormalities including augmentation of reactive oxygen species (ROS) and glutathione intracellular content, along with diminished glucose consumption coupled to increased lactate production. In AMIECs, the protein levels of 6-phosphofructo-2-kinase/fructose-2,6-bisphosphatase type 3, PFKFB3, were downregulated, while those of PFKFB4 were upregulated, suggesting a shunting of glycolysis towards the pentose phosphate pathway (PPP) in the pathological ECs. PPP overactivation was further supported by upregulation of G6PD in AMIECs, the key enzyme in the oxidative branch of the PPP, which supplies the bulk of NADPH reducing equivalents necessary for the reduction/turnover and lipid synthesis.. Further, the glutaminolytic enzyme glutaminase (GLS) was upregulated in AMIECs, providing a mechanistic explanation for the observed increase in glutathione content. Finally, AMIECs had higher mitochondrial membrane potential than control ECs, which, together with high ROS levels, suggest a highly coupled mitochondrial activity in patient ECs. We suggest that high proton coupling underlies the abnormally high production of ROS, balanced by PPP-driven glutathione turnover, as a primary, cell-autonomous abnormality driving EC dysfunction in AMI.

Robotic manipulation in semi-structured and changing environments requires systems with: a) perception and reasoning capabilities able to capture and understand the state of the environment; b) planning and replanning capabilities at both symbolic and geometric levels; c) automatic and robust execution capabilities. To cope with these issues, this paper presents a framework with the following features. First, it uses perception and ontology-based reasoning procedures to obtain the Planning Description Domain Language files that describe the manipulation problem at task level. This is used in the planning stage as well as during task execution in order to adapt to new situations, if required. Second, the proposed framework is able to plan at both task and motion levels, intertwining them by incorporating geometric reasoning modules to determine some of the symbolic predicates needed to describe the states. Finally, the framework automatically generates the behavior trees required to execute the task. The proposal takes advantage of the ability of behavior trees to be edited during run time, allowing adaptation of the action plan or of the trajectories according to changes in the state of the environment. The approach allows for robot manipulation tasks to be automatically planned and robustly executed, contributing to achieve fully functional service robots.

Introduction: Refractory cases of alopecia areata (AA) may be considered a therapeutic challenge. Intralesional methotrexate (IL-MTX) has been used in other dermatological diseases rather than AA. Likewise, its topical use as an immunosuppressant drug may be of interest for the control of the lymphoid infiltrate in AA. On the other hand, the use of fractional ablative laser is supported in literature as an alternative or complementary treatment in AA, whilst the generation of columns of thermal damage may favour the migration of cells and cytokines that are beneficial. Case Presentation: In this paper, we present 2 cases in which IL-MTX and ablative fractional CO2 laser were combined with excellent outcomes. Conclusion: Previous research encompasses a total of 23 patients. Most patients presented with patchy AA. The doses administered ranged from 2.5 to 50 mg with an average frequency of 3 weeks. On average, most patients required a minimum of 3 sessions. One case employed 1% topical methotrexate ointment. Adverse local events were mild and transient. In conclusion, the concomitant application of these treatments has not been reported previously. Specific recommendations relating to the appropriate dosing of the drug, frequency of administration, and requirements for analytical control studies should be determined in further studies.

A partir de inquietações produzidas pela existência do que considero que é uma fronteira entre Brasil e América Latina, focalizarei como são significados os vários aspectos do funcionamento da fronteira nos dizeres de sujeitos que habitam e transitam a divisa geográfica e política traçada entre Brasil e os países de Uruguai, Argentina, Paraguai e Bolívia. Um aspecto crucial, embora não exclusivo, consistirá em observar a constituição de subjetividades específicas e qual tipo de relação essas travam com as línguas.

La Incidencia Lesional (IL) se define como el número de lesiones por 1.000 horas de exposición. El objetivo fue determinar la IL y los posibles factores que pudieran tener alguna influencia en las lesiones en los jugadores de la selección española de fútbol masculina absoluta de 2008 a 2015. Durante este periodo se convocaron 85 jugadores. El tiempo de exposición fue de 10.450 horas de las que 8.800 correspondían a entrenamiento y 1.650 horas a competición. El IL fue 7,46 lesiones por 1.000 horas de exposición correspondiendo el 76,9% a partidos de competición y el resto a entrenamientos. Las lesiones musculares fueron las más frecuentes (52,4%). La severidad de la lesión estaba relacionada con la posición del jugador y con el mecanismo lesional. En conclusión, la selección española absoluta de fútbol presentó un IL en la media de otros equipos de la élite del fútbol mundial.

Background The modified thrombolysis in cerebral infarction (mTICI) scale is a widely used and validated qualitative tool to evaluate angiographic intracerebral inflow following endovascular thrombectomy (EVT). We validated a machine-learning (ML) algorithm to grade digital subtraction angiograms (DSA) using the mTICI scale. Materials and methods We included angiograms of identified middle cerebral artery (MCA) occlusions who underwent EVT. The complete DSA sequences were preprocessed and normalized. We created three convolutional neural networks to classify DSA into two outcomes, low- (mTICI 0,1,2a) and high-grade (mTICI 2b,2c,3). Results We included a total of 234 angiograms in this study. The area under the receiver operating characteristic was 0.863 (95% CI 0.816-0.909), 0.914 (95% CI 0.876-0.951), and 0.890 (95% CI 0.848-0.932) for the anteroposterior (AP), lateral (L), and combined models, respectively, when dichotomizing outcomes into low and high grade. The models' area under the precision-recall curve was 0.879 (95% CI 0.829-0.930), 0.906 (95% CI 0.844-0.968), and 0.887 (95% CI 0.834-0.941) for the AP, L, and combined models. Conclusion In complete cerebral DSA, our angiography-based ML strategy was able to predict mTICI scores following EVT rapidly and reliably for MCA occlusions.

espanolLa ocurrencia de heladas en zonas templadas de Mexico afecta seriamente el volumen y el valor de la produccion agricola, ocasionando importantes perdidas economicas para los productores y desabasto de alimentos. Las heladas tardias y tempranas son altamente peligrosas para el desarrollo de muchos cultivos, por lo que es importante desarrollar modelos sensibles de estimacion de riesgos que permitan caracterizar los regimenes locales de ocurrencia de heladas para tomar decisiones acertadas en la planeacion de la produccion. Este trabajo tiene como objetivo estimar las fechas de ocurrencia de heladas tardias y tempranas, asi como los periodos libres de heladas, mediante la estimacion de la probabilidad de ocurrencia desde un enfoque de eventos extremos. Se usan datos de temperatura minima diaria registrada en 77 estaciones meteorologicas del Estado de Mexico entre 1980 y 2010, tomados del CLICOM (Servicio Meteorologico Nacional, 2014). Se propone emplear una distribucion de valores extremos para modelar las fechas de ocurrencia de las ultimas heladas primaverales y las primeras heladas otonales, ya que esta familia de distribuciones presenta mayor sensibilidad en el analisis de colas para estimacion de riesgos. Los resultados se presentan en mapas de riesgo en terminos de probabilidad de ocurrencia de heladas. Se concluye que la distribucion Weibull presento el mejor ajuste a los datos de acuerdo con el Criterio de Informacion de Akaike (AIC), y permitio caracterizar satisfactoriamente el regimen de ocurrencia de heladas en el Estado de Mexico. EnglishThe occurrence of frosts in temperate zones of Mexico seriously affects the volume and value of agricultural production, causing significant economic losses for producers and food shortages. Late and early frosts are highly dangerous for the development of many crops, in this sense it is important to develop sensitive risk estimation models to characterize local frost occurrence regimes to make sound decisions in production planning. This paper aims to estimate the dates of occurrence of late and early frosts, as well as the frost-free periods, by estimating the probability of occurrence from the approach of an extreme event. Daily minimum temperature data recorded in 77 meteorological stations in Mexico State between 1980 and 2010 are used, taken from CLICOM (National Meteorological Service, 2014). It is proposed to use a distribution of extreme values to model the dates of occurrence of the last spring frosts and the first autumn frosts since this family of distributions presents greater sensitivity in the tails analysis for risk estimation. The results are presented on risk maps in terms of the probability of frost occurrence. It is concluded that the Weibull distribution presented the best fit to the data according to the Akaike Information Criterion (AIC), which has enabled successfully characterize the regime of frost occurrence in Mexico State.

Metastatic pheochromocytoma and paraganglioma (PPGL) have poor prognosis and limited therapeutic options. The recent advent of immunotherapies showing remarkable clinical efficacies against various cancer types offers the possibility of novel opportunities also for metastatic PPGL. Most PPGLs are pathogenically linked to inactivating mutations in genes encoding different succinate dehydrogenase (SDH) subunits. This causes activation of the hypoxia-inducible factor 2 (HIF2)-mediated transcriptional program in the absence of decreased intratumoral oxygen levels, a phenomenon known as pseudohypoxia. Genuine hypoxia in a tumor creates an immunosuppressive tumor microenvironment. However, the impact of pseudohypoxia in the immune landscape of tumors remains largely unexplored. In this study, tumoral expression of programmed death-ligand 1 (PD-L1) and HIF2α and tumor infiltration of CD8 T lymphocytes (CTLs) were examined in PPGL specimens from 102 patients. We assessed associations between PD-L1, CTL infiltration, HIF2α expression, and the mutational status of SDH genes. Our results show that high PD-L1 expression levels in tumor cells and CTL tumor infiltration were more frequent in metastatic than nonmetastatic PPGL. However, this phenotype was negatively associated with SDH mutations and high HIF2α protein expression. These data were validated by analysis of mRNA levels of genes expressing PD-L1, CD8, and HIF2α in PPGL included in The Cancer Genome Atlas database. Further, PD-L1 and CD8 expression was lower in norepinephrine than epinephrine-secreting PPGL. This in silico analysis also revealed the low PD-L1 or CD8 expression levels in tumors with inactivating mutations in VHL or activating mutations in the HIF2α-coding gene, EPAS1, which, together with SDH-mutated tumors, comprise the pseudohypoxic molecular subtype of PPGL. These findings suggest that pseudohypoxic tumor cells induce extrinsic signaling toward the immune cells promoting the development of an immunosuppressive environment. It also provides compelling support to explore the differential response of metastatic PPGL to immune checkpoint inhibitors. © 2022 The Authors. Vitamin D (VD) serum levels, and keratinocytic basal expression of vitamin D receptor (VDR) before treatment of actinic keratoses (AK) have been previously reported as possible biomarkers of the response of AK to treatments. We intended to evaluate the association between these and other serum and immunohistochemical parameters with the response of AK to treatment with topical ingenol mebutate (IM). Twenty-five patients with AK on the head were treated with topical IM 0.015% gel once daily for 3 days. Biopsies were taken at baseline and 6 weeks after treatment. Immunohistochemical staining was performed for VDR, P53, Ki67, Aurora B, Survivin and β-catenin. Basal serum 25(OH)D levels were determined. IM was more effective for KIN I and II AKs than in KIN III, and histological responders showed significantly higher serum VD levels (30.278 [SD 8.839] ng/mL) than nonresponders (21.14 [SD 7.079] ng/mL, p = 0.023). In addition, mean basal expression of VDR (45.63 [SD 16.105] %) increased significantly (57.92 [SD 14.738] %, p = 0.003) after treatment with IM. A significant decrease after treatment in the expression of several markers of aggressiveness and progression to squamous cell carcinoma, namely P53, Ki-67, aurora B kinase and survivin, was also observed. Our results support a relationship between VD status and the response of AK to treatment with topical IM, suggesting that its previous correction to proper serum levels in VD-deficient patients could improve the response of AK to the treatment.

Drug-induced inflammatory bowel disease (IBD) is a clinical entity on the rise due to the frequent use of immunomodulatory therapy. Here we report the case of Crohn's disease due to Ocrelizumab, a humanized anti-CD20 monoclonal antibody approved for the treatment of multiple sclerosis. The exact mechanism by which anti-CD20 antibodies can trigger IBD is unknown, but since IBD and multiple sclerosis are processes included within the spectrum of immunomediated diseases, we could suggest that Ocrelizumab could trigger IBD in genetically predisposed patients.

Reasons for Living (RFL) are a construct allows identifying the reasons for not committing suicide. These reasons are based on significant aspects of life, on the commitment to some ideals which may inhibit the impulse of committing suicide. The present study aims to describe the RFL in a sample of chronic schizophrenia patients. Explore if RFLs are different among male and female patients and the possible relation with clinical the variables: duration of illness, suicide attempts and hospitalizations. Method: Seventy patients with schizophrenia were assessed. The application of Reasons for Living Inventory (RFLI) and the structured interview for clinical and socio-demographic data was performed by a Psychology master’s degree research assistants and the lead investigator. Results: The sample mean score was 3.8/ 3.8 cut-off point under which the presence of suicide risk is significant. Patients indicated as most important RFLs those included in the Survival and coping beliefs and Responsibility to family domains. Non-significant differences between groups were observed. No association was found between clinical data and RFLI scores. Conclusion: Considering that the sample of the present study obtained high scores in Survival and coping beliefs and low scores in Fear of suicide as RFLs, thus reflecting an opposite response pattern to other high suicidal risk populations. We suggest that this construct could represent a protective factor for schizophrenia patients, including chronic patients with previous suicide attempts and high hospitalization rates, which were common variables, observed in clinical sample.

The proposed approach takes into account physical constraints of the robot, as the maximum velocity and the maximum acceleration that each joint can reach, and has been implemented in C++ as a general tool that can be used with different robots.

Esta premisa se convirtió en una realidad ya que muchos arquitectos (también interioristas y particulares) usaron sus muebles para viviendas, entradas en edificios y pisos muestra. Su catálogo se componía de elementos de mobiliario en madera, recuperando el trabajo de los racionalistas locales anteriores a la guerra civil. Con rasgos mediterráneos y referencias al mueble popular se desmarca del estilo funcionalista. Aresta también demuestra su sensibilidad por las apuestas expositivas y publicitarias que, usando los lenguajes del arte conceptual, promueve una imagen de empresa, que poco tenia que ver con los recursos comunicativos habituales del sector. En la experiencia que representa Aresta queda implícita la responsabilidad social del mueble y del diseño. Un componente ideológico muy evidente en arquitectura y a veces no tanto en lo que a mobiliario se refiere.

[Background & aims]: The Portfolio and Dietary Approaches to Stop Hypertension (DASH) diets have been shown to lower cardiometabolic risk factors in randomized controlled trials (RCTs). However, the Portfolio diet has only been assessed in RCTs of hyperlipidemic patients. Therefore, to assess the Portfolio diet in a population with metabolic syndrome (MetS), we conducted a longitudinal analysis of one-year data of changes in the Portfolio and DASH diet scores and their association with cardiometabolic risk factors in Prevención con Dieta Mediterránea (PREDIMED)-Plus trial. [Methods]: PREDIMED-Plus is an ongoing clinical trial (Trial registration: ISRCTN89898) conducted in Spain that includes 6874 older participants (mean age 65 y, 48% women) with overweight/obesity fulfilling at least three criteria for MetS. Data for this analysis were collected at baseline, six months and one year. Adherence to the Portfolio and DASH diet scores were derived from a validated 143-item food frequency questionnaire. We used linear mixed models to examine the associations of 1-SD increase and quartile changes in the diet scores with concomitant changes in cardiometabolic risk factors. [Results]: After adjusting for several potential confounders, a 1-SD increase in the Portfolio diet score was significantly associated with lower HbA1c (β [95% CI]: −0.02% [−0.02, −0.01], P < 0.001), fasting glucose (−0.47 mg/dL [−0.83, −0.11], P = 0.01), triglycerides (−1.29 mg/dL [−2.31, −0.28], P = 0.01), waist circumference (WC) (−0.51 cm [−0.59, −0.43], P < 0.001), and body mass index (BMI) (−0.17 kg/m2 [−0.19, −0.15], P < 0.001). A 1-SD increase in the DASH diet score was significantly associated with lower HbA1c (−0.03% [−0.04, −0.02], P < 0.001), glucose (−0.84 mg/dL [−1.18, −0.51], P < 0.001), triglycerides (−3.38 mg/dL [−4.37, −2.38], P < 0.001), non-HDL-cholesterol (−0.47 mg/dL [−0.91, −0.04], P = 0.03), WC (−0.69 cm [−0.76, −0.60 cm], P < 0.001), BMI (−0.25 kg/m2 [−0.28, −0.26 kg/m2], P < 0.001), systolic blood pressure (−0.57 mmHg [−0.81, −0.32 mmHg], P < 0.001), diastolic blood pressure (−0.15 mmHg [−0.29, −0.01 mmHg], P = 0.03), and with higher HDL-cholesterol (0.21 mg/dL [0.09, 0.34 mg/dL, P = 0.001]). Similar associations were seen when both diet scores were assessed as quartiles, comparing extreme categories of adherence. [Conclusions]: Among older adults at high cardiovascular risk with MetS, greater adherence to the Portfolio and DASH diets showed significant favourable prospective associations with several clinically relevant cardiometabolic risk factors. Stereotactic body radiotherapy (SBRT) and stereotactic radiosurgery (SRS) are essential tools in radiation oncology. In Spain, the use of these techniques continues to grow as older linear accelerators (linacs) are replaced with modern equipment. However, little is known about inter-centre variability in prescription and dose heterogeneity limits. Consequently, the SBRT-Spanish Task Group (SBRT-SG) of the Spanish Society of Radiation Oncology (SEOR) has undertaken an initiative to assess prescription and homogeneity in SRS/SBRT treatment. In the present study, we surveyed radiation oncology (RO) departments to obtain a realistic overview of prescription methods used for SBRT and SRS treatment in Spain. A brief survey was developed and sent to 34 RO departments in Spain, mostly those who are members of the SEOR SBRT-SG. The survey contained seven questions about the specific prescription mode, dose distribution heterogeneity limits, prescription strategies according to SRS/SBRT type, and the use of IMRT–VMAT (Intensity Modulated Radiation Therapy–Volumetric Modulated Arc Therapy). Responses were received from 29 centres. Most centres (59%) used the prescription criteria D95% ≥ 100%. Accepted dose heterogeneity was wide, ranging from 107 to 200%. Most centres used IMRT–VMAT (93%). This survey about SRS/SBRT prescription and dose heterogeneity has evidenced substantial inter-centre variability in prescription criteria, particularly for intended and accepted dose heterogeneity. These differences could potentially influence the mean planning target volume dose and its correlation with treatment outcomes. The findings presented here will be used by the SEOR SBRT-SG to develop recommendations for SRS/SBRT dose prescription and heterogeneity.

Resumen: Introducción: La alta prevalencia de la patología del antepié genera largas listas de espera quirúrgica (LEQ). Hemos detectado un considerable número de pacientes que renuncian a la cirugía, lo que crea una importante distorsión en nuestra actividad y un elevado gasto de recursos. Nuestro objetivo es estudiar los factores relacionados con estas renuncias, así como comparar con otras patologías de alta prevalencia y tratamiento quirúrgico ambulatorio: síndrome del túnel carpiano (STC) y la meniscopatía interna (MI). Material y métodos: Estudio retrospectivo de las renuncias a la intervención sobre 2.399 pacientes incluidos en LEQ de la Unidad de Pie y Tobillo de nuestro centro para cirugía del antepié, entre enero/2014 y marzo/2018, ambos inclusive. Resultados: Hemos encontrado 389 renuncias, lo que supone un 16,22% de las inclusiones en LEQ, siendo un 84,83% mujeres. Las patologías con mayor tasa de renuncia han sido: metatarsalgia de Morton (24%) y hallux rigidus (20,16%). La patología más frecuente, el hallux valgus, registra un 15,96% de renuncias que se producen mayoritariamente entre los seis y nueve meses. En el STC y la MI, el índice de renuncias ha sido del 17,42 y del 8,92% respectivamente, con mayores tasas de renuncia en los tres primeros meses. Conclusiones: La renuncia a una intervención programada sobre el antepié registra una alta frecuencia en nuestro medio, que puede relacionarse con factores como el tipo de patología, historia natural, respuesta a intervenciones ortopédicas, el tiempo en LEQ, y otros no determinados sobre los que debemos profundizar, para racionalizar y establecer prioridades en nuestras LEQ. Abstract: Introduction: The high prevalence of forefoot pathology generates long surgical waiting lists (SWL). We have detected a considerable number of patients who withdraw surgery, which creates an important distortion in our activity and high expenditure of resources. Our objective is to study the factors related to these resignations, as well as, compare them with other pathologies of high prevalence and ambulatory surgical treatment: carpal tunnel syndrome (CTS) and internal meniscopathy (IM). Material and methods: Retrospective study of the surgical cancellations on 2,399 patients included in the SWL of the Foot and Ankle Unit of our center for forefoot surgery, between January/2014 and March/2018, both included. Results: We have found 389 renunciations, which represent 16.22% of the inclusions in SWL, with 84.83% of women. The pathologies with the highest rate of resignation have been Morton metatarsalgia (24%) and hallux rigidus (20.16%). The most frequent pathology, hallux valgus, records 15.96% of resignations that occur mostly between 6 and 9 months. In the CTS and IM, the resignation rate has been 17.42 and 8.92%, respectively, with higher resignation rates in the first 3 months. Conclusions: The withdrawal of a scheduled intervention on the forefoot registers a high frequency in our environment, which can be related to factors such as the type of pathology, its natural history, response to orthopedic interventions, time in LEQ, and other non-specific ones on which we must investigate, to rationalize and establish duties in our SWL.

Background: Utilization of stereotactic radiosurgery (SRS) for brain metastases (BM) has become the technique of choice as opposed to whole brain radiation therapy (WBRT). The aim of this work is to evaluate the feasibility and potential benefits in terms of normal tissue (NT) and dose escalation of volumetric modulated arc therapy (VMAT) in SRS metastasis treatment. A VMAT optimization procedure has therefore been developed for internal dose scaling which minimizes planner dependence. Materials and methods: Five patient-plans incorporating treatment with frame-based SRS with dynamic conformal arc technique (DA) were re-planned for VMAT. The lesions selected were between 4–6 cm 3 . The same geometry used in the DA plans was maintained for the VMAT cases. A VMAT planning procedure was performed attempting to scale the dose in inner auxiliary volumes, and to explore the potential for dose scaling with this technique. Comparison of dose-volume histogram (DVH) parameters were obtained. Results: VMAT allows a superior NT sparing plus conformity and dose scaling using the auxiliary volumes. The VMAT results were significantly superior in NT sparing, improving both the V 10 and V 12 values in all cases, with a 2–3 cm 3 saving. In addition, VMAT improves the dose coverage D 95 by about 0.5 Gy. The objective of dose escalation was achieved with VMAT with an increment of the Dmean and the Dmedian of about 2 Gy. Conclusions: This work shows a benefit of VMAT in SRS treatment with significant NT sparing. A VMAT optimization procedure, based on auxiliary inner volumes, has been developed, enabling internal dose escalation.

The aim of this article «Analysis and quantifying of Primary teaching in León during the second half of the 19th century» is to analyze the situation of elementary education teachers in León during the aforementioned period, and not only quantifying their number across the different states (Tables), but also attending to other questions such as their training, payments, qualification, school infraestructure, pupils, etc.; such a treatment will help to approach a more objetive vision of the primary teaching staff evolution in León during the second half of the preceding century.

According to unemployment statistics, the Spanish Second Republic had to confront one of the hardest problems of this period, due to most of people would get involved in. The solutions from public institutions needed the support of private enterprises, whose primary purpose would be reduced the urban unemployment. From October 1934, the shocking truth would show other face. A critical social confrontation atmosphere broke down the weak compromise in order to solve this problem.The tension is clearly detectable from January to July 1936 in León. In the middle of it, the called "Elections of Frente Popular" turned more deep the social fracture. Thus, the town council had to head alone the battle against work-crisis in those seven months, although many troubles did not allow a satisfactory evolution of applied initiatives.
