El crecimiento urbano incontrolado en América Latina y el Caribe ha dado lugar a mayor presencia de habitantes en barrios pobres, infraestructuras y servicios inadecuados y sobrecargados, dando lugar a ciudades fragmentadas. La presente revisión utilizó técnicas de cartografía científica y sistemática de investigación para examinar 1486 documentos indexados en Scopus® sobre el desarrollo de la fragmentación urbana en América Latina y el Caribe; para el análisis de la información se utilizó Bibliometrix® y VOSviwer® para facilitar la comprensión de los datos. Los objetivos de la revisión son documentar el tamaño, el crecimiento y la distribución de esta literatura en la región, identificar sus principales autores y fuentes de publicación y resaltar la estructura intelectual emergente. Como parte de los resultados, se identifica que, a pesar de la urgencia de documentar y abordar los retos de la fragmentación urbana, se encontró que la mayoría de los estudios son de autoría procedente de solo ciertos países de la región. La revisión aporta tendencias investigativas para que se desarrollen estudios que favorezcan el desarrollo y las discusiones futuras en relación con su impacto sobre los sistemas socio-ecológicos, su relación con el desarrollo sustentable, y su incidencia con enfoque diferencial poblacional.

Introducción: Teniendo en cuenta la repercusión del tinnitus en la calidad de vida de los pacientes, este estudio busca conocer las características específicas de esta población. Objetivo: Describir las características sociodemográficas y clínicas de los pacientes con diagnóstico de tinnitus que asistieron ambulatoriamente a la consulta de tinnitus de la Clínica ORLANT en la ciudad de Medellín. Materiales y métodos: Estudio retrospectivo de pacientes referidos por primera vez a la consulta especializada de tinnitus en el centro de especialistas en Otorrinolaringología y Otología, clínica ORLANT, en un año. Se obtuvo información demográfica y se recopilaron datos de las historias clínicas. Resultados: 61 pacientes fueron incluidos. El 59% de sexo masculino, edad media de 52,5 años. La mediana del tiempo de evolución fue 24 meses, como características clínicas 54,1% presentó tinnitus bilateral, 78,7% refirió tono agudo y 59% intensidad moderada, como síntomas asociados 67,2% refirió sensación de giro y 80,3% desequilibrio o inestabilidad. El 23% tenía hipertensión arterial, 5 pacientes recibieron ototóxicos, 34,4% tuvo exposición crónica al ruido y 8,2% hipoacusia súbita. El 21,3% tenía diagnóstico de trastorno psiquiátrico. El 65,5% diagnóstico de hipoacusia, 52,5% bilateral y 54,1% de tipo neurosensorial Conclusión: Las características del tinnitus más comunes fueron la presentación bilateral, duración mayor de 3 meses, tonalidad aguda y de intensidad moderada. Encontrando una representación similar de algunas características en la población de otras áreas geográficas. Importancia clínica: conocer las particularidades del paciente con tinnitus, hace un acercamiento para plantear medidas de prevención y tratamiento específicamente dirigidas

Over half the world’s population is at risk for viruses transmitted by Aedes mosquitoes, such as, dengue and Zika. The primary vector, Aedes aegypti, thrives in urban environments. Despite decades of effort, cases and geographic range of Aedes-borne viruses (ABV) continue to expand. Rigorously proven vector control interventions that measure protective efficacy against ABV diseases is limited to Wolbachia in a single trial in Indonesia, and do not include any chemical intervention. Spatial repellents, a new option for efficient deployment, are designed to decrease human exposure to ABV by releasing active ingredients into the air that disrupt mosquito-human contact. A parallel, cluster-randomized controlled trial was conducted in Iquitos, Peru to quantify the impact of a transfluthrin-based spatial repellent on human ABV infection. From 2,907 households across 26 clusters (13 per arm), 1,578 participants were assessed for seroconversion (primary endpoint) by survival analysis. Incidence of acute disease was calculated among 16,683 participants (secondary endpoint). Adult mosquito collections were conducted to compare Ae. aegypti abundance, blood-fed rate and parity status through mixed effect difference-in-difference analyses. The spatial repellent significantly reduced ABV infection by 34·1% (1-sided 95% CI lower limit, 6·9%; 1-sided p-value=0·0236, z=1·98). Aedes aegypti abundance and blood-fed rates were significantly reduced by 28·6% (95% CI 24·1%, ∞); z=-9·11) and 12·4% (95% CI 4·2%, ∞); z=-2·43), respectively. Our trial provides the first conclusive statistical evidence from a pre-planned cluster randomized controlled clinical trial with a pre-defined effect size on the primary endpoint that was appropriate powered to prospectively quantify and statistically test for a difference in the impact of a chemical intervention, in this case a spatial repellent, to reduce the risk of ABV transmission compared to a placebo.Significance StatementVector interventions are needed for Aedes-borne viral diseases (dengue, Zika, chikungunya, and yellow fever) prevention, but their application is hindered by the lack of evidence proving they prevent infection or disease. Our research reports the first conclusive statistical evidence from a pre-planned, prospective cluster-randomized, controlled clinical trial (cRCT) of significant protective efficacy (34.1% hazard estimate) against human Aedes-borne virus (ABV) infection by a chemical-based vector control intervention, the most commonly used intervention category among all ABV World Health Organization recommendations. A previous trial against malaria in Indonesia indicated a positive trend but did not detect a significant effect. Results from our ABV study will help guide public health authorities responsible for operational management and world-wide prevention of ABV, and incentivize new strategies for disease prevention.

Vehicular communications hold the promise of disrupting mobility services and supporting the mass adoption of future autonomous vehicles. Regulators have set aside specific spectrum at the 5.9 GHz band to support Intelligent Transport Systems (ITS) safety applications, for which a world-wide adoption of a standardized radio technology is a key factor to deliver on this promise. Two technologies are currently positioned to begin its commercial path, IEEE 802.11p and LTE-PC5 Mode-4. The main differences between these technologies lie on the design of their channel access mechanisms. This paper provides an analysis of the impact that the Medium Access Control (MAC) mechanisms included in 802.11p and LTE-PC5 Mode-4 will have on the performance of the applications using the Cooperative Awareness Service, applying two new application-level metrics used by safety applications: Neighborhood Awareness Ratio and Position Error. We have found that, even with an equivalent physical layer performance, the MAC layer of LTE-PC5 Mode-4 will mostly outperform the MAC layer of IEEE 802.11p (or its not yet ready enhanced version 802.11bd). However, IEEE 802.11p/bd results in slightly better vehicle positioning accuracy at lower distances.

Background: The prognosis of HIV infection dramatically improved after the introduction of triple antiretroviral therapy 25 years ago. Herein, we report the impact of further improvements in HIV management since then, looking at all hospitalizations in persons with HIV (PWH) in Spain. Methods: A retrospective study using the Spanish National Registry of Hospital Discharges. Information was retrieved since 1997-2018. Results: From 79 647 783 nationwide hospital admissions recorded during the study period, 532 668 (0.67%) included HIV as diagnosis. The mean age of PWH hospitalized increased from 33 to 51 years (P < 0.001). The rate of HIV hospitalizations significantly declined after 2008. Comparing hospitalizations during the first (1997-2007) and last (2008-2018) decades, the rate of non-AIDS illnesses increased, mostly due to liver disease (from 35.9 to 38.3%), cardiovascular diseases (from 12.4 to 28.2%), non-AIDS cancers (from 6.4 to 15.5%), and kidney insufficiency (from 5.4 to 13%). In-hospital deaths occurred in 5.5% of PWH, declining significantly over time. Although most deaths were the result from AIDS conditions (34.8%), the most frequent non-AIDS deaths were liver disease (47.1%), cardiovascular events (29.2%), non-AIDS cancers (24.2%), and kidney insufficiency (20.7%). Conclusion: Hospital admissions in PWH significantly declined after 2008, following improvements in HIV management and antiretroviral therapy. Non-AIDS cancers, cardiovascular events and liver disease represent a growing proportion of hospital admissions and deaths in PWH.

Chronic hepatitis C virus (HCV) infection is major cause of decompensated cirrhosis and liver cancer. The advent of curative new antiviral therapies since year 2015 has dramatically improved the prognosis of HCV patients. The real-life clinical benefits at country level of these therapies have not yet been assessed. This is a retrospective study of all hospitalizations in Spain including HCV as diagnosis using the Spanish National Registry of Hospital Discharges. Information was retrieved from 1997 to 2019. From 81,482,509 nationwide hospital admissions recorded during the study period, 1,057,582 (1.29%) included HCV as diagnosis. The median age of HCV hospitalized patients was 54-years-old. Males accounted for 63.2% of cases. Most HCV admissions recorded chronic hepatitis C whereas acute hepatitis C was reported in less than 3%. In-hospital death occurred in 6.4% of HCV admissions. Coinfection with HIV or hepatitis B virus was seen in 14.8% and 6.4%, respectively. Patients hospitalized with HIV-HCV coinfection represented 14.8% of cases and were on average 17 years younger than HCV-monoinfected individuals. The rate of HCV hospitalizations significantly increased until 2005, and then stabilized for one decade. A significant reduction was noticed since 2015. However, whereas the proportion of HCV-associated hepatic decompensation events declined since then, liver cancer diagnoses increased. In conclusion, hospital admissions of HCV individuals significantly declined in Spain since 2015 following a wide prescription of new oral direct-acting antivirals. This reduction was primarily driven by a fall of hepatic decompensation events whereas HCV-related liver cancer continues rising.

OBJECTIVES: Although only 10% of persons infected with human T-lymphotropic virus type 1 (HTLV-1) may develop virus-associated illnesses over their lifetime, missing the earlier diagnosis of asymptomatic carriers frequently leads to late presentation. METHODS: A nationwide HTLV-1 register was created in Spain in 1989. We examined the main demographics and clinical features at the time of the first diagnosis for more than three decades. RESULTS: A total of 428 individuals infected with HTLV-1 had been reported in Spain until the end of 2021. Up to 96 (22%) individuals presented clinically with HTLV-1-associated conditions, including subacute myelopathy (57%), T-cell lymphoma (34%), or Strongyloides stercoralis infestation (8%). Since 2008, HTLV-1 diagnosis has been made at blood banks (44%) or clinics (56%). Native Spaniards and Sub-Saharan Africans are overrepresented among patients presenting with HTLV-1-associated illnesses suggesting that poor epidemiological and/or clinical suspicion, which led to the late presentation are more frequent in them than carriers from Latin America (LATAM) (31.7% vs 20.4%, respectively; P = 0.015). CONCLUSION: HTLV-1 infection in Spain is frequently diagnosed in patients presenting with characteristic illnesses. Although screening in blood banks mostly identifies asymptomatic carriers from LATAM, a disproportionately high number of Spaniards and Africans are diagnosed too late at the time of clinical manifestations. Expanding testing to all pregnant women and clinics for sexually transmitted infections could help to unveil HTLV-1 asymptomatic carriers.

Hepatitis delta virus (HDV) is a defective agent that only infects individuals with hepatitis B virus (HBV). Around 5-10% of chronic hepatitis B patients worldwide are superinfected with HDV, which means 15-25 million people. Hepatitis delta is the most severe of all chronic viral hepatitis, leading to cirrhosis, liver cancer and/or transplantation in most patients. Despite it, many HDV patients remain undiagnosed. The only treatment available until recently was peginterferon alfa, with poor results and significant side effects. The recent approval of bulevirtide, a lipopeptide that blocks HBV/HDV entry, has revolutionized the field. Another drug, lonafarnib, already approved to treat progeria, is expected to be available soon as HDV therapy. Since there is no cell reservoir for the HDV RNA genome, hypothetically viral clearance could be achieved if complete blocking of viral replication occurs for a minimum time-frame. This is what happens in hepatitis C using direct-acting antivirals, with achievement of cure in nearly all treated patients. We envision the cure of hepatitis delta using combination antiviral therapy. Given that sexual and parenteral transmission routes are the most frequent for the acquisition of HBV and HDV -and shared with HIV infection-, HBV/HDV and HIV coinfection is common. The clinical outcome of hepatitis delta is worst in the HIV setting, with more frequent liver complications. Since most persons infected with HIV are on regular health care follow-up, we propose that HIV-HDV patients should be prioritized for moving forward new and potentially curative treatments for hepatitis delta.

Background: Hepatitis delta is the most aggressive form of chronic viral hepatitis. We examined the clinical burden, epidemiological features and time trends for hepatitis delta patients hospitalized in Spain during the last two decades. Methods: Retrospective, observational study using the Spanish National Registry of Hospital Discharges. Information was retrieved since 1997 to 2018. Results: From a total of 79 647 783 nationwide hospital admissions recorded during the study period, 5179 included hepatitis delta as diagnosis. The overall hospitalization rate because of hepatitis delta was 6.5/10(5), without significant yearly changes. In-hospital death occurred in 335 (6.6%) patients. Acute hepatitis and cirrhosis were recorded in 46.5 and 33.5% of hepatitis delta hospitalizations, respectively. Acute hepatitis delta predominated until 2007 (55.9%) whereas cirrhosis increased since then (39.4%). Hepatic decompensation events and liver cancer accounted on average for 16 and 8% of hospitalizations, increasing significantly over time. Coinfection with HIV and hepatitis C virus (HCV) were recognized in 24 and 31.2% of hepatitis delta patients, respectively. All hepatitis C, HIV and injection drug use declined significantly since 2008. Conclusion: The rate of hepatitis delta in patients hospitalized in Spain is low and has remained stable over two decades. However, hepatitis delta-related decompensation events and liver cancer are on the rise. The association of hepatitis delta with injection drug use, HIV and HCV has declined among recently hospitalized hepatitis delta patients.

The medical demand imposed by COVID-19 has distracted proper care of other illnesses. Herein, we report the impact on new diagnoses of HTLV-1, HTLV-2, and HIV-2 in Spain, where these infections are mostly driven by immigration flows from endemic regions. As expected, case reporting declined for all three retroviral infections with respect to prior years. Furthermore, late presentations were more common. The two major reasons for these observations were significant declines in the arrival of foreigners from endemic regions and a shift in medical resources to prioritize COVID-19.

Objectives This ongoing Phase-2, randomised, placebo-controlled, double-blind study evaluated the efficacy, safety and pharmacokinetics of intravenous belimumab in childhood-onset systemic lupus erythematosus (cSLE). Methods Patients (5 to 17 years) were randomised to belimumab 10 mg/kg intravenous or placebo every 4 weeks, plus standard SLE therapy. Primary endpoint: SLE Responder Index (SRI4) response rate (Week 52). Key major secondary endpoints: proportion of patients achieving the Paediatric Rheumatology International Trials Organisation/American College of Rheumatology (PRINTO/ACR) response using 50 and '30 alternative' definitions (Week 52), and sustained response (Weeks 44 to 52) by SRI4 and Parent Global Assessment of well-being (Parent-global). Safety and pharmacokinetics were assessed. Study not powered for statistical testing. Results Ninety-three patients were randomised (belimumab, n=53; placebo, n=40). At Week 52, there were numerically more SRI4 responders with belimumab versus placebo (52.8% vs 43.6%; OR 1.49 (95% CI 0.64 to 3.46)). PRINTO/ACR 30 alternative (52.8% vs 27.5%; OR 2.92 (95% CI 1.19 to 7.17)) and PRINTO/ACR 50 (60.4% vs 35.0%; OR 2.74 (95% CI 1.15 to 6.54)) responses were more frequent with belimumab than placebo, as were sustained responses for SRI4 (belimumab, 43.4%; placebo, 41.0%; OR 1.08 (95% CI 0.46 to 2.52)) and Parent-global (belimumab, 59.1%; placebo, 33.3%; OR 3.49 (95% CI 1.23 to 9.91)). Serious adverse events were reported in 17.0% of belimumab patients and 35.0% of placebo patients; one death occurred (placebo). Week-52, geometric mean (95% CI) belimumab trough concentration was 56.2 (45.2 to 69.8) mu g/mL. Conclusion The belimumab intravenous pharmacokinetics and benefit-risk profile in cSLE are consistent with adult belimumab studies and the 10 mg/kg every 4 weeks dose is appropriate.

We present a detailed analysis of the gaseous component of the Si K edge using high-resolution Chandra spectra of low-mass X-ray binaries. We fit the spectra with amodified version of the ISMabs model, including new photoabsorption cross sectionscomputed for all Si ionic species. We estimate column densities for Si i, Si ii, Si iii, Si xii and Si xiii, which trace the warm, intermediate temperature and hot phases of the Galactic interstellar medium. We find that the ionic fractions of the first two phases are similar. This may be due to the physical state of the plasma determined by the temperature or to the presence of absorber material in the close vicinity of the sources. Our findings highlight the need for accurate modeling of the gaseous component before attempting to address the solid component., Comment: 5 pages, 4 figures

The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has hit health-care systems and societies in an unprecedented manner. In 1981, the first cases of AIDS were reported and wide diagnostic testing helped to characterize high-risk groups and the global burden of the epidemic. With Coronavirus Disease (COVID)-19, everything has happened too fast and both cases and fatalities are huge but still uncertain in most places. Diagnostic testing of active and past SARS-CoV-2 infections needs to expand rapidly, ideally using rapid tests. COVID-19 deaths are highly concentrated in the elderly population, with a large proportion of fatalities being "with" rather than "by" SARS-CoV-2 infection. They are often the result of inadequate health care due to overwhelming demands. To date, there is no specific therapy for SARS-CoV-2 infection. Several antivirals are being tested clinically, including remdesivir, at this time the most promising. For others such as lopinavir/ritonavir, neither significant virological nor clinical benefit has been shown. Given the characteristic pulmonary cytokine storm underlying the pathogenic mechanism of severe COVID-19 pneumonia and acute respiratory distress, antiinflammatory agents are being investigated. The benefit of orticosteroids, hydroxychloroquine, etc., is limited. Monoclonal antibodies targeting different pro-inflammatory cytokines, such as tocilizumab, an anti-interleukin 6 agent, are being tried with encouraging results. Ultimately a protective vaccine will be the best response for controlling the COVID-19 pandemic.

Human immunodeficiency virus type 2 (HIV-2) was isolated in AIDS patients in 1986. Around 1-2 million people are infected worldwide. The virus is less transmissible than HIV-1, being sexual contacts the most frequent route of acquisition. In the absence of antiretroviral therapy, most HIV-2 carriers will develop AIDS; however, it takes longer than in HIV-1 infection. There is no global pandemic caused by HIV-2, as the virus is largely confined to West Africa. Due to historical ties, HIV-2 is also prevalent in Portugal and its former colonies in Brazil, India, Mozambique, and Angola. Other European countries with hundreds to thousands of HIV-2 infections are France, Belgium, and Spain. A few hundred have been reported in North America. mostly in West African foreigners. Globally, HIV-2 infections are steadily declining. Although CD4 declines occur more slowly in HIV-2 than in HIV-1 patients. the CD4 recovery with antiretroviral treatment is smaller in the former. HIV-2 is naturally resistant to non-nucleoside reverse transcriptase inhibitors (NNATIs) and some protease inhibitors. In contrast. HIV-2 is susceptible to all NRTIs and integrase inhibitors. Drug resistance in HIV-2 may develop earlier than in HIV-1 and select for mutations at distinct sites. Misdiagnosis of HIV-2 in patients wrongly considered as HIV-1 positive or in those dually infected may result in treatment failures with undetectable HIV-1RNA. Given the relatively large number of West Africans migrated to the European Union and North America. HIV-2 infection either alone or as coinfection with HIV-1 should be excluded at least once in all HIV-seroreactive persons. This should be stressed in the face of atypical HIV serological profiles, immunovirological disconnect (CD4 cell count loss despite undetectable HIV-1 viremia), and/or high epidemiological risks (birth in or sex partners from HIV-2 endemic regions). Superinfection with any HIV variant may occur in persons infected with the other. since there is no cross-protection. Thus, earlier antiretroviral therapy is warranted for either HIV-1 or HIV-2, given that it would protect from each other superinfection in persons at risk.

Objective: To examine the clinical burden and disease spectrum, as well as time trends for human T-cell leukemia virus type 1 (HTLV-1) and HTLV type 2 (HTLV-2) hospital admissions. Design: Retrospective, observational study using the Spanish National Hospital Discharge Database. Methods: Information for the diagnostic codes HTLV-1 and HTLV-2 using theInternational Classification of Diseases, Ninth Revision, Clinical Modification(ICD-9-CM) was retrieved from the national public registry since 1997--2015. Results: From a total of 66 462 136 nationwide hospital admissions recorded during the study period, 135 included HTLV diagnosis, being HTLV-1 in 115 (85.2%) and HTLV-2 in 20 (14.8%). The overall hospital admission rate because of HTLV was 2.03 per million, without significant yearly changes. First admissions represented 104 (77%) whereas 31 (23%) were re-admissions. The median in-hospital stay for HTLV patients was 9 days. In-hospital death occurred in 11 (8.1%). The median age of individuals with HTLV admission was 48 years and 60 (44.4%) were women. HTLV was recorded as the main diagnosis in 20%. The most frequent clinical conditions recorded alongside HTLV diagnosis were myelopathy (61; 45.2%), leukemia/lymphoma (30; 22.2%), solid organ transplantation (14; 10.4%) and child delivery (7; 5.2%). Conclusion: The rate of HTLV diagnosis in hospitalized patients in Spain is low, roughly of two per million admissions. Despite continuous large immigrant flows from HTLV-1 endemic areas, no significant rising in hospitalizations because of HTLV-1 associated illnesses were noticed during the last two decades. Classical clinical complications of HTLV-1 infection, such as myelopathy and lymphoma account for more than two-thirds of cases.

Background In collaboration with state, tribal, local, and territorial health departments, CDC established the U.S. Zika Pregnancy Registry (USZPR) in early 2016 to monitor pregnant women with laboratory evidence of possible recent Zika virus infection and their infants. Methods This report includes an analysis of completed pregnancies (which include live births and pregnancy losses, regardless of gestational age) in the 50 U.S. states and the District of Columbia (DC) with laboratory evidence of possible recent Zika virus infection reported to the USZPR from January 15 to December 27, 2016. Birth defects potentially associated with Zika virus infection during pregnancy include brain abnormalities and/or microcephaly, eye abnormalities, other consequences of central nervous system dysfunction, and neural tube defects and other early brain malformations. Results During the analysis period, 1,297 pregnant women in 44 states were reported to the USZPR. Zika virus-associated birth defects were reported for 51 (5%) of the 972 fetuses/infants from completed pregnancies with laboratory evidence of possible recent Zika virus infection (95% confidence interval [CI] = 4%-7%); the proportion was higher when restricted to pregnancies with laboratory-confirmed Zika virus infection (24/250 completed pregnancies [10%, 95% CI = 7%-14%]). Birth defects were reported in 15% (95% CI = 8%-26%) of fetuses/infants of completed pregnancies with confirmed Zika virus infection in the first trimester. Among 895 liveborn infants from pregnancies with possible recent Zika virus infection, postnatal neuroimaging was reported for 221 (25%), and Zika virus testing of at least one infant specimen was reported for 585 (65%). Conclusions and implications for public health practice These findings highlight why pregnant women should avoid Zika virus exposure. Because the full clinical spectrum of congenital Zika virus infection is not yet known, all infants born to women with laboratory evidence of possible recent Zika virus infection during pregnancy should receive postnatal neuroimaging and Zika virus testing in addition to a comprehensive newborn physical exam and hearing screen. Identification and follow-up care of infants born to women with laboratory evidence of possible recent Zika virus infection during pregnancy and infants with possible congenital Zika virus infection can ensure that appropriate clinical services are available.

Background. Although only 8%-10% of persons infected with human T-cell leukemia virus type 1 (HTLV-1) may develop virus-associated diseases lifelong, misdiagnosis of asymptomatic infected carriers frequently leads to late diagnoses. Methods. A nationwide HTLV-1 register was created in Spain in 1989. A total of 351 infected persons had been reported by the end of 2017. We examined all new HTLV-1 diagnoses during the last decade and compared their clinical presentation. Results. A total of 247 individuals with HTLV-1 infection had been reported in Spain since year 2008. The incidence has remained stable with 20-25 new diagnoses yearly. Women represented 62%. Only 12% were native Spaniards, most of whom were foreigners from Latin America (72.5%). Up to 57 (23%) individuals presented clinically with HTLV-1-associated conditions, including subacute myelopathy (n = 24; 42.1%), T-cell lymphoma (n = 19; 33.3%), or Strongyloides stercoralis infestation (n = 8; 14%). Human T-cell leukemia virus type 1 diagnosis had been made either at blood banks (n = 109; 44%) or at clinics (n = 138; 56%). It is interesting to note that Spaniards and especially Africans were overrepresented among patients presenting with HTLV-1-associated illnesses, suggesting that misdiagnosis and late presentation are more frequent in these populations compared to Latin Americans. Conclusions. Given that 23% of new HTLV-1 diagnoses in Spain are symptomatic, underdiagnosis must be common. Although screening in blood banks mostly identifies asymptomatic Latin American carriers, a disproportionately high number of Spaniards and Africans are unveiled too late, that is, they already suffer from classic HTLV-1 illnesses.

Background: Human retroviruses HIV and HTLV share transmission routes. HIV widely spread in Spain during the 80 s through injection drug use and sex, and nowadays HIV rates in Spain account for one of the largest in Europe. In contrast, HTLV-1 is not endemic in Spain, despite hosting huge numbers of migrants from highly endemic regions. Herein, we report the rate and main features of the HIV-HTLV co-infected population in Spain. Methods: A national registry exists in Spain for HTLV since year 1989. Data from standardized case report forms and one centralized lab repository were reviewed, especially for the subset with HTLV-HIV co-infection. Results: Up to December 2018, a total of 369 individuals with HTLV-1 had been diagnosed in Spain. 64% of the population were females, and Latin American individuals accounted for 64.5%. Classical HTLV-associated illnesses were found in 12.7% (myelopathy) and 7.6% (leukemia). HIV coinfection was found in 12 (3.2%). Of those, 3 patients (25%) were female and 39 (75%) were of non Spanish origin. All but two harbored HIV-1 subtype B, being non-B variants found in the two West Africans. Exposure had been sexual in most cases, being 4 homosexual men. Seven HTLV-HIV co-infected patients had developed AIDS and two had developed myelopathy. There was no evidence for increased HTLV-1 clinical pathogenicity due to HIV coinfection. Conclusion: HIV coinfection is infrequent (< 5%) among HTLV-1 carriers in Spain. More than half of co-infected patients come from Latin America. Sexual contact is the most frequent risk behavior, being MSM one third of cases. Late diagnosis explains the high rate (9/12) of clinical manifestations in our HIV-HTLV co-infected population.

Background: Whereas HIV-1 has spread globally, HIV-2 is mainly found in West Africa where dual HIV-1/HIV-2 coinfection is nowadays uncommon. Herein, we report the rate, main characteristics, and treatment outcomes of all dually infected patients living in Spain. Methods: We identified retrospectively all persons coinfected with HIV-1 recorded at the Spanish HIV-2 registry. Dual infection had been confirmed using PCR in plasma and/or cells, and/or using discriminatory serological tests. Results: From a total of 373 individuals with HIV-2 recorded at the Spanish registry, 34 (9.1%) were coinfected with HIV-1. Compared with HIV-2 monoinfected persons, dually infected patients were more often male (67.6%), presented with lower median CD4(+) cell counts (204 cells/mu l), and had developed more frequently AIDS events (26.5%). Although 61.7% came from West Africa, 6 (17.6%) were native Spaniards. HIV-1 non-B subtypes were recognized in 75% of coinfected patients, being the most prevalent CRF02_AG. At baseline, 45% of dually infected patients had undetectable plasma HIV-2 RNA. After a median follow-up of 32 (13-48) months on antiretroviral therapy, dually infected patients achieved undetectable viremia in 85% for HIV-1, in 80% for HIV-2; and in 70% for both viruses. Median CD4(+) cell counts reached up to 418 cells/mu l. Conclusion: Roughly 9% of individuals with HIV-2 infection living in Spain are coinfected with HIV-1. Overall, 70% of dually infected patients achieved viral suppression for both viruses under antiretroviral therapy. Given the relatively large population of West Africans living in Spain and the continuous migration flow from HIV-2 endemic areas, HIV-1/HIV-2 coinfection should always be excluded at first diagnosis in all HIV-seroreactive persons. HTLV-1 infection is a neglected disease, despite infecting 10-15 million people worldwide and severe illnesses develop in 10% of carriers lifelong. Acknowledging a greater risk for developing HTLV-1 associated illnesses due to immunosuppression, screening is being widely considered in the transplantation setting. Herein, we report the experience with universal HTLV testing of donors and recipients of solid organ transplants in a survey conducted in Spain.

Background: HIV-2 is a neglected virus despite estimates of 1-2 million people being infected worldwide. The virus is naturally resistant to some antiretrovirals used to treat HIV-1 and therapeutic options are limited for patients with HIV-2. Methods: In this retrospective observational study, we analysed all HIV-2-infected individuals treated with integrase strand transfer inhibitors (INSTIs) recorded in the Spanish HIV-2 cohort. Demographics, treatment modalities, laboratory values, quantitative HIV-2 RNA and CD4 counts as well as drug resistance were analysed. Results: From a total of 354 HIV-2-infected patients recruited by the Spanish HIV-2 cohort as of December 2017, INSTIs had been given to 44, in 18 as first-line therapy and in 26 after failing other antiretroviral regimens. After a median follow-up of 13 months of INSTI-based therapy, undetectable viraemia for HIV-2 was achieved in 89% of treatment-naive and in 65.4% of treatment-experienced patients. In parallel, CD4 gains were 82 and 126 cells/mm(3), respectively. Treatment failure occurred in 15 patients, 2 being treatment-naive and 13 treatment-experienced. INSTI resistance changes were recognized in 12 patients: N155H (5), Q148H/R (3), Y143C/G (3) and R263K (1). Conclusions: Combinations based on INSTIs are effective and safe treatment options for HIV-2-infected individuals. However, resistance mutations to INSTIs are selected frequently in failing patients, reducing the already limited treatment options.

Aims.Within the framework of compact-object accretion disks, we calculate plasma environment effects on the atomic structure and decay parameters used in the modeling of K lines in lowly charged iron ions, namely Fe II–Fe VIII.Methods.For this study, we used the fully relativistic multiconfiguration Dirac–Fock method approximating the plasma electron–nucleus and electron-electron screenings with a time-averaged Debye-Hückel potential.Results.We report modified ionization potentials, K-threshold energies, wavelengths, radiative emission rates, and Auger widths for plasmas characterized by electron temperatures and densities in the ranges 105 − 107K and 1018 − 1022cm−3. In addition, we propose two universal fitting formulae to predict the IP and K-threshold lowerings in any elemental ion.Conclusions.We conclude that the high-resolution X-ray spectrometers onboard the future XRISM and ATHENA space missions will be able to detect the lowering of the K edges of these Fe ions due to the extreme plasma conditions occurring in the accretion disks around compact objects.

In most accreting black-hole systems the copious X-rays commonly observed from the inner-most regions are accompanied by a reflection spectrum. The latter is the signature of energetic photons reprocessed by the optically thick material of an accretion disk. Given their abundance and fluorescence yield, the iron K-shell lines are the most prominent features in the X-ray reflected spectrum. Their line profiles can be grossly broadened and skewed by Doppler effects and gravitational redshift. Consequently, modeling the reflection spectrum provides one of the best methods to measure, among other physical quantities, the black-hole spin. At present the accuracy of the spin estimates is called into question because the data fits require very high iron abundances: typically several times the solar value. Concurrently no plausible physical explanation has been proffered for these black-hole systems to be so iron rich. The most likely explanation for the supersolar iron abundances is model shortfall at very high densities ($>10^{18}$ cm$^{-3}$) due to atomic data shortcomings in this regime. We review the current observational evidence for the iron supersolar abundance in many black-hole systems, and show the effects of high density in state-of-the-art reflection models. We also briefly discuss our current efforts to produce new atomic data for high-density plasmas, which are required to refine the photoionization models., 7 pages, 5 figures. RESUMEN La interacción genotipo x ambiente estima el comportamiento diferencial que presentan los clones cuando se les evalúa en diferentes ambientes y permite determinar los clones con mayor estabilidad. El objetivo de la presente investigación fue estimar la magnitud de la interacción genotipo x ambiente para el rendimiento de catorce clones avanzados de papa con pulpa pigmentada y dos variedades comerciales (Amarilis y Canchán), en dos localidades productoras de papa en Cutervo, Perú, durante dos años. Para analizar los datos obtenidos se usó el análisis combinado de varianza y el modelo de efectos principales aditivos e interacción multiplicativa (AMMI). El análisis AMMI reveló que el efecto del ambiente, genotipo y la interacción genotipo x ambiente, explican el 13,15%, 26,03% y 60,82% de la suma de cuadrados total en el rendimiento de tubérculos de papa, respectivamente; por tanto, la mayor parte fue aportada por el efecto de la interacción genotipo-ambiente. Los primeros dos componentes principales fueron altamente significativos (p < 0,01) y capturaron el 45,58% y 38,61% de la suma de cuadrados total de la interacción genotipo x ambiente en el rendimiento. El biplot muestra a los clones CIP302304.15, CIP302295.32 y Amarilis INIA registraron valores de 980,64, 959,86 y 1001,35 g planta-1, respectivamente, con rendimiento superior a la media general y una baja interacción con los ambientes evaluados, mostrando una mayor estabilidad de rendimiento de papa con pulpa pigmentada.

Since hepatitis C virus (HCV) and human T-lymphotropic virus (HTLV) share transmission routes, dual infection could be frequent. In Spain, HTLV underdiagnosis is highlighted by the high proportion of patients presenting either with tropical spastic paraparesis (TSP) or adult T-cell leukemia (ATL) at first diagnosis. We examined whether the renewed efforts for expanding HCV testing may provide a sentinel population that might selectively be targeted to unveil asymptomatic HTLV carriers. The presence of anti-HTLV antibodies was examined in 3,838 consecutive individuals with reactive HCV serology attended during the last three years at 13 hospitals distributed across the Spanish geography. Overall 71% were male and the median age was 41-years old. Foreigners represented 9% of the study population. A total of 50 individuals (1.3%) were seroreactive for HTLV, being 30 confirmed as HTLV-2 and two as HTLV-1 (0.12%). The remaining 18 had indeterminate Western blot patterns. Most individuals with HTLV-2 and HTLV indeterminate serology were HIV-positive, former injection drug users and native Spaniards. In contrast, the two HTLV-1 infections were found in men coming from Brazil and the Dominican Republic, respectively. In summary, the overall prevalence of HTLV infection in individuals living in Spain seropositive for HCV is 1.3%, more than 10-fold greater than in general outclinics in Spain. However, immigrants from HTLV-1 endemic regions and former injection drug users with HTLV-2 infection are by far the major contributory groups in HCV patients. Therefore, testing for HTLV in newly diagnosed HCV individuals would not contribute much to improve late HTLV diagnosis in Spain.

HIV type 2 (HIV-2) is a neglected virus despite estimates of 1-2 million people infected worldwide. HIV-2 is less efficiently transmitted than HIV-1 by sex and from mother to child. Although AIDS may develop in HIV-2 carriers, it takes longer than in HIV-1-infected patients. In contrast with HIV-1 infection, there is no global pandemic caused by HIV-2, as the virus is largely confined to West Africa. In a less extent and due to socioeconomic ties and wars, HIV-2 is prevalent in Portugal and its former colonies in Brazil, India, Mozambique and Angola. Globally, HIV-2 infections are steadily declining over time. A total of 338 cases of HIV-2 infection had been reported at the Spanish HIV-2 registry until December 2016, of whom 63% were men. Overall 72% were sub-Saharan Africans, whereas 16% were native Spaniards. Dual HIV-1 and HIV-2 coinfection was found in 9% of patients. Heterosexual contact was the most likely route of HIV-2 acquisition in more than 90% of cases. Roughly one-third presented with CD4 cell counts less than 200 cells/µl and/or AIDS clinical events. Plasma HIV-2 RNA was undetectable at baseline in 40% of patients. To date, one-third of HIV-2 carriers have received antiretroviral therapy, using integrase inhibitors 32 individuals. New diagnoses of HIV-2 in Spain have remained stable since 2010 with an average of 15 cases yearly. Illegal immigration from Northwestern African borders accounts for over 75% of new HIV-2 diagnoses. Given the relatively large community of West Africans already living in Spain and the continuous flux of immigration from endemic regions, HIV-2 infection either alone or as coinfection with HIV-1 should be excluded once in all HIV-seroreactive persons, especially when showing atypical HIV serological profiles, immunovirological disconnect (CD4 cell count loss despite undetectable HIV-1 viremia) and/or high epidemiological risks (birth in or sex partners from endemic regions).

We are concerned with improving the diagnostic potential of the K lines and edges of elements with low cosmic abundances that are observed in the X-ray spectra of supernova remnants, galaxy clusters and accreting black holes and neutron stars. Since accurate photoabsorption and photoionization cross sections are needed in their spectral models, they have been computed for isoelectronic sequences with electron number $12\leq N\leq 18$ using a multi-channel method. Target representations are obtained with the atomic structure code AUTOSTRUCTURE, and ground-state cross sections are computed with the Breit--Pauli $R$-matrix method (BPRM) in intermediate coupling, including damping (radiative and Auger) effects. The contributions from channels associated with the 2s-hole $[2{\rm s}]\mu$ target configurations and those containing 3d orbitals are studied in the Mg and Ar isoelectronic sequences. Cross sections for the latter ions are also calculated in the isolated-resonance approximation as implemented in AUTOSTRUCTURE and compared with BPRM to test their accuracy. It is confirmed that the collisional channels associated with the $[2{\rm s}]\mu$ target configurations must be taken into account owing to significant increases in the monotonic background cross section between the L and K edges. Target configurations with 3d orbitals give rise to fairly conspicuous unresolved transition arrays in the L-edge region, but to a much lesser extent in the K-edge which is our main concern; therefore, they have been neglected throughout owing to their computationally intractable channel inventory, thus allowing the computation of cross sections for all the ions with $12\leq N\leq 18$ in intermediate coupling with BPRM. Human T-lymphotropic virus type 1 (HTLV-1) infection is a neglected disease despite roughly 15 million people are chronically infected worldwide. Lifelong less than 10% of carriers develop life-threatening diseases, mostly a subacute myelopathy known as tropical spastic paraparesis (TSP) and a lymphoproliferative disorder named adult T-cell leukemia (ATL). HTLV-1 is efficiently transmitted perinatally (breastfeeding), sexually (more from men to women) and parenterally (transfusions, injection drug user (IDU), and transplants). To date there is neither prophylactic vaccine nor effective antiviral therapy. A total of 327 cases of HTLV-1 infection had been reported at the HTLV-1 Spanish registry until December 2016, of whom 34 had been diagnosed with TSP and 25 with ATL. Overall 62% were Latin American immigrants and 13% were persons of African origin. The incidence of HTLV-1 in Spain has remained stable for nearly a decade with 20-25 new cases yearly. Of the 21 newly diagnosed HTLV-1 cases during year 2016, one was a native Spaniard pregnant woman, and four presented with symptomatic disease, including three with ATL and one with TSP. Underdiagnosis of HTLV-1 in Spain must be high (iceberg model), which may account for the disproportionate high rate of symptomatic cases (almost 20%) and the late recognition of preventable HTLV-1 transmissions in special populations, such as newborns and transplant recipients. Our current estimate is of 10 000 persons living with HTLV-1 infection in Spain. Given the large flux of immigrants and visitors from HTLV-1 endemic regions to Spain, the expansion of HTLV-1 screening policies is warranted. At this time, it seems worth recommending HTLV testing to all donor/recipient organ transplants and pregnant women regardless place of birth. Although current leukoreduction procedures largely prevent HTLV-1 transmission by blood transfusions, HTLV testing of all first-time donors should be cost-effective contributing to unveil asymptomatic unaware HTLV-1 carriers.

Summary Background Some children with juvenile idiopathic arthritis either do not respond, or are intolerant to, treatment with disease-modifying antirheumatic drugs, including anti-tumour necrosis factor (TNF) drugs. We aimed to assess the safety and efficacy of abatacept, a selective T-cell costimulation modulator, in children with juvenile idiopathic arthritis who had failed previous treatments. Methods We did a double-blind, randomised controlled withdrawal trial between February, 2004, and June, 2006. We enrolled 190 patients aged 6–17 years, from 45 centres, who had a history of active juvenile idiopathic arthritis; at least five active joints; and an inadequate response to, or intolerance to, at least one disease-modifying antirheumatic drug. All 190 patients were given 10 mg/kg of abatacept intravenously in the open-label period of 4 months. Of the 170 patients who completed this lead-in course, 47 did not respond to the treatment according to predefined American College of Rheumatology (ACR) paediatric criteria and were excluded. Of the patients who did respond to abatacept, 60 were randomly assigned to receive 10 mg/kg of abatacept at 28-day intervals for 6 months, or until a flare of the arthritis, and 62 were randomly assigned to receive placebo at the same dose and timing. The primary endpoint was time to flare of arthritis. Flare was defined as worsening of 30% or more in at least three of six core variables, with at least 30% improvement in no more than one variable. We analysed all patients who were treated as per protocol. This trial is registered, number NCT00095173. Findings Flares of arthritis occurred in 33 of 62 (53%) patients who were given placebo and 12 of 60 (20%) abatacept patients during the double-blind treatment (p=0·0003). Median time to flare of arthritis was 6 months for patients given placebo (insufficient events to calculate IQR); insufficient events had occurred in the abatacept group for median time to flare to be assessed (p=0·0002). The risk of flare in patients who continued abatacept was less than a third of that for controls during that double-blind period (hazard ratio 0·31, 95% CI 0·16–0·95). During the double-blind period, the frequency of adverse events did not differ in the two treatment groups. Adverse events were recorded in 37 abatacept recipients (62%) and 34 (55%) placebo recipients (p=0·47); only two serious adverse events were reported, both in controls (p=0·50). Interpretation Selective modulation of T-cell costimulation with abatacept is a rational alternative treatment for children with juvenile idiopathic arthritis. Funding Bristol-Myers Squibb.

Producción Científica, Background:HIV-2 infection is characterized by low plasma viraemia and slower progression to AIDS in compari-son with HIV-1 infection. However, antiretroviral therapy in patients with HIV-2 is less effective and often fails toprovide optimal CD4 recovery.Methods:We examined viral tropism in persons with HIV-2 infection enrolled in the HIV-2 Spanish cohort. Viraltropism was estimated based on V3 sequences obtained from plasma RNA and/or proviral DNA.Results:From a total of 279 individuals with HIV-2 infection recorded in the Spanish national register, 58 V3sequences belonging to 42 individuals were evaluated. X4 viruses were recognized in 14 patients (33%).Patients with X4 viruses had lower median CD4+cell counts than patients with R5 viruses [130 (17 – 210) versus359 (180 – 470) cells/mm3;P¼0.007]. This was true even considering only the subset of 19 patients on antiretro-viral therapy [94 (16 – 147) versus 184 (43 – 368) cells/mm3;P¼0.041]. In multivariate analysis, significant differ-ences in CD4+cell counts between patients with X4 and R5 viruses remained after adjusting for age, gender,antiretroviral therapy and viral load.Conclusions:The presence of X4-tropic viruses in HIV-2 infection is associated with low CD4+cell counts, regard-less of antiretroviral treatment. Reactions of OsO4 with HSR (R=C6F5, C6F4H-4,) in refluxing ethanol afford [Os(SC6F5)3(SC6F4(SC6F5)-2)] (1) and [Os(SC6F4H-4)3(SC6F3H-4-(SC6F4H-4)-2)] (2), which involve the rupture of C-F bonds. At room temperature, the compound [Os(SC6F5)3(PMe2Ph)2] or [Os(SC6F5)4(PMe2Ph)] reacts with KOH(aq) in acetone, giving rise to [ Os(SC6F5)(SC6F4(SC6F4O-2)-2)(PMe2Ph)2] (3), through a process involving the rupture of two C-F bonds, while the compound [Os(SC6F4H)4(PPh3)] reacts with KOH(aq) in acetone to afford [Os(SC6F4H-4)2(SC6F3H-4-O-2)(PPh3)] (4), which also implies a C-F bond cleavage. Single-crystal X-ray diffraction studies of 1, 2, and 4 indicate that these compounds include five-coordinated metal ions in essentially trigonal-bipyramidal geometries, whereas these studies on the paramagnetic compound 3 show a six-coordinated osmium center in a distorted octahedral geometry. 19F, 1H, 31P{1H}, and COSY 19F-19F NMR studies for the diamagnetic 1, 2, and 4 compounds, including variable-temperature 19F NMR experiments, showed that these molecules are fluxional. Some of the activation parameters for these dynamic processes have been determined.

HIV-2 and HTLV-1 infections are globally less frequent than those produced by HIV-1, the classical AIDS agent. In Spain and up to the end of 2014, a total of 310 cases of HIV-2, 274 of HTLV-1, and 776 of HTLV-2 infections had been reported. No cases of HTLV-3 or HTLV-4 infections have been identified so far in Spain. Most persons infected with HIV-2 or HTLV-1 acknowledge epidemiological risk factors for contagion, such as originating from or living in endemic regions and/or having had sexual partners from those areas. However, risk factors could not be recognized in up to 20-25% of carriers in Spain. Thus, it seems worth keeping a high level of clinical suspicion in order to identify earlier these neglected human retroviral infections, since diagnostic procedures and antiviral treatment are specific for each of these agents. Trabajo presentado en el 8th European Meeting on Solar Chemistry and Photocatalysis, celebrado en Salónica (Grecia) del 25 al 28 de Junio de 2014., Despite regulation straightening, air pollutants concentration is one of the biggest problems that modern society faces, increasingly aware of the danger associated. Solar assisted heterogeneous photocatalysis has become an interesting alternative to degradate NOx and VOCs. In this context, the photoefficiency of four TiO2 coated cements (TiO2-Cem) in Rhodamine B (RhB) and NOx photodegradation tests and the effect of a SiO2 interlayer (SiO2-TiO2-Cem) on the mechanical and photocatalytic properties has been analyzed. Although a good photodegradation was always achieved, the home-made titania (TEA) showed the best global behavior in both series., This work have been supported by the Spanish Plan Nacional I +D+I throught the projet CTM2010-14883/TECNO.

En la práctica odontológica, cada vez son más comunes las alteraciones con sintomatología dolorosa en la región maxilofacial que no son apropiadamente diagnosticadas, a causa del desconocimiento de síndromes craneofaciales y cervicofaríngeos, como el síndrome de Eagle. Este último se describe como el dolor orofacial relacionado con la elongación de la apófisis estiloide y/o calcificación del ligamento estilohioideo. La aparición de una patología asociada a tal estructura, está acompañada de otros síntomas como: disfonía, disfagia, dolor faríngeo, glositis, otalgia, tonsilitis, dolor facial, cefalea, odinofagia, dolor en la articulación temporomandibular e imposibilidad de realizar movimientos laterales del cuello. El presente artículo es una revisión bibliográfica cuyo objetivo consistió en describir los aspectos generales del Síndrome de Eagle y la aplicabilidad de ayudas imagenológicas actuales en su identificación y diagnóstico. Se realizó una búsqueda de literatura en bases de datos como: MEDLINE (Ovid), Pubmed, Science Direct, y scirus empleando descriptores como: Eagle syndrome, cone beam, styloid process, Eagle syndrome in dentistry. A partir de la búsqueda se seleccionaron 37 artículos. Finalmente se pudo concluir que entre los diversos métodos diagnósticos, el que permite un análisis detallado de los patrones radiográficos de síndrome de Eagle, es la tomografía computarizada de haz cónico (Cone Beam Coputed Tomography).

Transmission of HIV-1 with drug resistance mutations (DRMs) in Spain remains stable around 13%. However, the profile of recent HIV-1 seroconverters has experienced significant changes.

Compatible monokaryotic strains of Agaricus bisporus ATCC 36975 and 36976 and the resulting dikaryon of their mating were grown in a liquid medium, and their respective cell walls were prepared. Significant differences were not found in the gross chemical composition of the three hyphal types. However, the neutral carbohydrate composition of the complete walls and their fractions was found to be somewhat different in each strain. More consistent differences were encountered in the chemical structure of the distinct polysaccharidic wall fractions in the three types of organisms. Some of these structural wall differences can be considered as characteristic markers for differentiating the mono- and dikaryotic types of A. bisporus.
