This paper presents a micromachined thermal convective accelerometer with low power and high reliability. This accelerometer comprises a heater and two thermistors. The central heater elevates the temperature of the chip above ambient levels while the symmetrically arranged thermistors monitor the temperature differentials induced by acceleration. The heater and thermistors are fabricated on a glass substrate using a standard micro-electromechanical systems (MEMS) process flow, and the fabricated sensor is installed on a rotation platform and a shaking table experimental setup to perform the experiment. The results indicate that the sensor has the capability to measure accelerations surpassing 80 m/s2, with an approximate linear sensitivity of 110.69 mV/g. This proposed thermal convective accelerometer offers promising potential for various applications requiring precise acceleration measurements in environments where low power consumption and high reliability are paramount. As the body's largest organ, the skin is an integrated multisensory system with self‐healing ability and helps stabilize body temperature. It is herein, inspired by natural skin, a wearable patch made from porous polydimethylsiloxane (PDMS) skeleton, poly(vinyl alcohol) (PVA) hydrogel, and silicon oxide (SiO2) particles, offers a combination of self‐healing properties, along with hybrid radiative and evaporative cooling mechanisms, designed for electromyographic (EMG) signal detection and human‐machine interaction. The patch has both high mid‐infrared (MIR) emittance (96%) and visible to near‐infrared (visNIR) reflectance (80%), coupled with efficient water evaporation from the PVA hydrogel, resulting in a hybrid cooling power of 180 W m−2. It obtains a temperature drop of ≈7.7 °C using this patch under a solar intensity of ≈700 W m−2. Furthermore, the patch demonstrates self‐healing ability with ultrafast recovery of electrical conductivity (1 s) and a self‐healing efficiency (≈71%) of fracture strain. Thus, the wearable patch can detect high‐quality EMG signals and provide cooling effects and self‐healing capabilities that enhance comfortability and durability. These features make the patch an advanced solution for developing next‐generation wearable patches that can meet the rigorous demands of durable body temperature control in various applications. Introduction: Ascending aorta or hemi‐arch replacement is a frequently used treatment for patients with acute type A thoracic aortic dissection, particularly those who are elderly or have multiple comorbidities. However, in cases where there are secondary entry tears in the aortic arch or descending aorta, this procedure may not fully resolve the issue. The true lumen may remain compressed due to perfusion of the false lumen and usually require reoperation. Methods: Between January 2019 and July 2022, 18 patients underwent endovascular total aortic arch repair and fenestration technique without requiring median re‐sternotomy. Aortic stent grafts were implanted via the femoral approach, utilizing prosthetic vessels as an appropriate proximal landing zone for aortic stent graft deployment. Based on the debranching conditions of the arch in previous surgery, single, double or triple in situ fenestrations (ISFs) were performed, respectively. Results: All 18 cases were technically successful, with a median follow‐up period of 20 months (range: 18–31 months). All patients had a favourable postoperative course, with no deaths within 30 days or during their hospital stay. There were no instances of disabling stroke, paraplegia, endo‐leak, stent graft migration or stent graft‐induced new entry. In addition, all patients exhibited complete thrombosis of the false lumen at the level of the aortic arch. Conclusion: Our preliminary experience suggests that endovascular total arch repair combined with ISF technique is a viable, effective and safe option for treatment. In this article, a 3-D printed mechanical wind sensor with reduced measurement errors is demonstrated. This sensor consists of a cylindrical sensing structure arranged vertically in the center of a cross-shape cantilever. Eight strain gauges are symmetrically designed on the four beams of the cross cantilever to measure the airflow-induced cantilever deformation. With these eight strain gauges, two sets of Wheatstone full-bridge circuits can be constructed, enabling the measurement errors of the mechanical wind sensor to reduce remarkably. The experimental results indicate that, compared with the four-strain-gauge mechanical wind sensor in the previous work, the eight-strain-gauge design presented in this article can achieve a 62.4% sensitivity improvement for wind speed measurement. This increased sensitivity allows the wind sensor to realize a maximum speed measurement error of 0.53 m/s in the range of 0–30 m/s, achieving a 39.1% reduction compared with 0.87 m/s of the previous four-strain-gauge sensor. In addition, the test results also show that the proposed full-bridge sensor can define the airflow direction over the full range of 360° with a maximum error of 6°, which is 2° less than the four-strain-gauge device. As the scientific research community along with healthcare professionals and decision makers around the world fight tirelessly against the coronavirus disease 2019 (COVID‐19) pandemic, the need for comparative effectiveness research (CER) on preventive and therapeutic interventions for COVID‐19 is immense. Randomized controlled trials markedly under‐represent the frail and complex patients seen in routine care, and they do not typically have data on long‐term treatment effects. The increasing availability of electronic health records (EHRs) for clinical research offers the opportunity to generate timely real‐world evidence reflective of routine care for optimal management of COVID‐19. However, there are many potential threats to the validity of CER based on EHR data that are not originally generated for research purposes. To ensure unbiased and robust results, we need high‐quality healthcare databases, rigorous study designs, and proper implementation of appropriate statistical methods. We aimed to describe opportunities and challenges in EHR‐based CER for COVID‐19‐related questions and to introduce best practices in pharmacoepidemiology to minimize potential biases. We structured our discussion into the following topics: (1) study population identification based on exposure status; (2) ascertainment of outcomes; (3) common biases and potential solutions; and (iv) data operational challenges specific to COVID‐19 CER using EHRs. We provide structured guidance for the proper conduct and appraisal of drug and vaccine effectiveness and safety research using EHR data for the pandemic. Purpose: Supplementing investigator‐specified variables with large numbers of empirically identified features that collectively serve as 'proxies' for unspecified or unmeasured factors can often improve confounding control in studies utilizing administrative healthcare databases. Consequently, there has been a recent focus on the development of data‐driven methods for high‐dimensional proxy confounder adjustment in pharmacoepidemiologic research. In this paper, we survey current approaches and recent advancements for high‐dimensional proxy confounder adjustment in healthcare database studies. Methods: We discuss considerations underpinning three areas for high‐dimensional proxy confounder adjustment: (1) feature generation—transforming raw data into covariates (or features) to be used for proxy adjustment; (2) covariate prioritization, selection, and adjustment; and (3) diagnostic assessment. We discuss challenges and avenues of future development within each area. Results: There is a large literature on methods for high‐dimensional confounder prioritization/selection, but relatively little has been written on best practices for feature generation and diagnostic assessment. Consequently, these areas have particular limitations and challenges. Conclusions: There is a growing body of evidence showing that machine‐learning algorithms for high‐dimensional proxy‐confounder adjustment can supplement investigator‐specified variables to improve confounding control compared to adjustment based on investigator‐specified variables alone. However, more research is needed on best practices for feature generation and diagnostic assessment when applying methods for high‐dimensional proxy confounder adjustment in pharmacoepidemiologic studies. In this paper, the effect of wind-induced vibration on measurement range of microcantilever anemometer is investigated for the first time. The microcantilever anemometer is composed of a flexible substrate and a piezoresistor. The wind speed can be detected through the airflow-induced deformation in the flexible substrate. Previous work indicated that the flexible substrate vibrates violently once the wind speed exceeds a critical value, resulting in severe output jitter. This wind-induced vibration limits the measurement range of the anemometer, and the relationship between the anemometer measurement range and its structural parameters has not been explored systematically. Therefore, this paper aims to reveal this relationship theoretically and experimentally, demonstrating that a shorter and thicker cantilever with larger stiffness can effectively suppress the wind-induced vibration, leading to the critical speed rising. By eliminating the wind-induced vibration, the measurement range of the microcantilever anemometer can be increased by up to 697%. With the great progress made recently in next generation sequencing (NGS) technology, sequencing accuracy and throughput have increased, while the cost for data has decreased. Various human leukocyte antigen (HLA) typing algorithms and assays have been developed and have begun to be used in clinical practice. In this study, we compared the HLA typing performance of three HLA assays and seven NGS-based HLA algorithms and assessed the impact of sequencing depth and length on HLA typing accuracy based on 24 benchmarked samples. The algorithms HISAT-genotype and HLA-HD showed the highest accuracy at both the first field and the second field resolution, followed by HLAscan. Our internal capture-based HLA assay showed comparable performance with whole exome sequencing (WES). We found that the minimal depth was 100X for HISAT-genotype and HLA-HD to obtain more than 90% accuracy at the third field level. The top three algorithms were quite robust to the change of read length. Thus, we recommend using HISAT-genotype and HLA-HD for NGS-based HLA genotyping because of their higher accuracy and robustness to read length. We propose that a minimal sequence depth for obtaining more than 90% HLA typing accuracy at the third field level is 100X. Besides, targeting capture-based NGS HLA typing may be more suitable than WES in clinical practice due to its lower sequencing cost and higher HLA sequencing depth. [ABSTRACT FROM AUTHOR]

In this paper, the design, fabrication, and characterization of a robust MEMS thermal wind sensor utilizing ceramic film packaging are proposed. The ceramic film is fabricated by means of low temperature co-fired ceramic (LTCC) technology, achieving the excellent adhesion between silicon substrate and ceramic film without any thermal adhesive. The most important is that the adverse effects of the packaging on the performance of the thermal wind sensor can be minimized by adjusting the thickness and thermal conductivity of the ceramic film during the preparation of the ceramic slurry. The influences of thickness and thermal conductivity of ceramic film on the performance of sensor have been simulated by finite element method, and the results are in reasonable agreement with theoretical analysis. The proposed sensors were tested and calibrated in a wind tunnel. Experiments results demonstrate that the sensitivity of the proposed sensor is decreased only 4.7%, compared with the sensor without ceramic film. The maximum measurement error of the proposed sensor in wind speed and direction is 0.66 m/s and 5°, respectively. Compared with the sensor packaged with $250\mu \text{m}$ ceramic carrier, the heating power of the proposed sensor is reduced by nearly 50% and the sensitivity is increased by an order of magnitude at the same time. This paper presents a semi-empirical model for the temperature effect of a micromachined 2-D anemometer. The anemometer consists of a self-heated double Wheatstone bridge. Eight thermistors are symmetrically arranged in two orthogonal directions, and four of them in each direction make up a Wheatstone bridge. The eight thermistors simultaneously act as both heating and sensing elements. The terminal voltage across the bridge and the bridge output voltage, under the constant current supply, are utilized to measure the wind speed and direction, respectively. The semi-empirical model has taken the thermal physical properties of the air, sensor substrate, and thermistors into account. It shows that the measurement of wind direction has a weak temperature dependence due to the symmetric config uration of the self-heated double Wheatstone bridge, which suppresses the temperature drift. The temperature compensation has been performed by means of the semi-empirical model. The experimental measurements demonstrate that, when the air temperature changes from 270 to 310 K, the wind speed varies from 0 to 30 m/s with an error less than ±1.5 m/s while the wind direction spans from 0° to 360° with the weak temperature dependence. Modeling of the packaged wind sensor operating on constant power (CP) mode is proposed in this paper. The proposed model is based on the theory of heat conduction and forced convection. The presented model indicates that the total output dc voltage is proportional to the square root of the wind speed, and ${V}_{x}$ , as well as ${V}_{y}$ , varies with the wind direction based on cosine function. The wind sensor is fabricated by microelectromechanical systems (MEMS) technology and packaged with the glue. The test is performed in the wind tunnel and the measured results indicate an agreement with the presented model. By applying the constant current supply to the bridge, the terminal voltage across the bridge and the bridge output voltage are utilized to measure the wind speed and direction, respectively. The 2-D wind sensor has been simulated numerically and fabricated directly on the ceramic substrate. Experiments demonstrate that the measured speed varies from 0 to 40 m/s with an error less than ±2 m/s while the measured direction spans from 0° to 360° with an error less than ±3° after calibration. This paper introduces a novel sensing element scheme to improve the accuracy of 2-D micromachined thermal wind sensor. The sensing element consists of eight heaters and eight thermistors placed on ceramic substrate. The octagon-arranged sixteen resistors are divided into two wind sensing groups, cross-type group and saltire-type group. These two groups monitor wind independently, and the final measured wind speed and direction are extracted from the measurement results of the two groups. Based on this octagon-sensing design, the sensor achieves higher accuracy for wind measurement, which is verified by the experiment. The results show that for the wind speed up to 33 m/s, the sensor has a measurement error less than ±1%, and the airflow direction over the full range of 360° is determined with a maximum error of ±1.5°. Drug-induced QT interval prolongation may increase the risk of sudden cardiac death or ventricular arrhythmias (SCD/VA), and therefore affects the safety profile of medications. Administrative databases can be used to inform pharmacoepidemiologic drug safety studies for such rare events. In order to compare event rates between studies, validated operational definitions of these events are needed. We conducted a systematic literature review in PubMed to identify algorithms for SCD/VA. Twenty-two studies were included in the review. Fifteen (68%) studies evaluated International Classification of Diseases, 9th revision (ICD-9) based medical data, of which six utilized a common, validated operational definition. This algorithm was based on principal hospitalization discharge diagnosis or first-listed emergency department visit diagnosis, with an average positive predictive value (PPV) of 85%. Four studies evaluated ICD-9 based death data, of which three utilized a common algorithm with an average PPV of 88%. Further validation of ICD, 10th revision algorithms are needed. In conclusion, we identified a validated algorithm for SCD/VA in medical data, as well as in death data. As such, to ensure comparability between new research and the existing literature, pharmacoepidemiologic research in this area should utilize common, validated algorithms, such as the ones identified in our review, to operationally define these events. In this paper, the effect of insulation trenches on micromachined silicon thermal wind sensors is explored. The sensor is composed of a central heater and four thermistors. It measures wind speed and direction through detecting the airflow-induced temperature difference on the chip surface. Three different forms of insulation trenches are designed and fabricated to explore their effects on the performance of wind sensors. The experimental results show that when the sensors work in constant voltage mode, the center and arch trenches enhance the performance, while the arch trenches have the most significant effect on the sensitivity improvement. Moreover, all the three kinds of insulation trenches reduce the power consumption for the sensors operated in constant temperature difference mode. This paper presents the design, fabrication, and performance of a micromachined silicon thermal wind sensor with improved sensitivity. Deep reactive ion etching (DRIE) trenches are fabricated between the heater and the thermistors to suppress the lateral heat conduction in the chip. In addition, eight thermistors symmetrically arranged in four directions around the heater form two Wheatstone full-bridges, resulting in about 50% increase of the sensitivity with respect to four thermistors. Based on these two methods, the sensitivity of the micromachined silicon thermal wind sensor is improved remarkably, which is verified by the experiment. The results show that the measurement wind speed range is up to 33 m/s in constant voltage (CV) mode with the initial heating power of 256 mW. The sensitivity is measured to be 29.37 mV/ms−1 at the wind speed 3.3 m/s, achieving improvement of about 226%, compared with that of the traditional wind sensor. Researches show that androgens have important effects on migration of endothelial cells and endothelial protection in coronary heart disease. Endothelial progenitor cells (EPCs) as a progenitor cell type that can differentiate into endothelial cells, have a critical role in angiogenesis and endothelial protection. The relationship between androgen and the functions of EPCs has animated much interest and controversy. In this study, we investigated the angiogenic and migratory functions of EPCs after treatment by dihydrotestosterone (DHT) and the molecular mechanisms as well. We found that DHT treatment enhanced the incorporation of EPCs into tubular structures formed by HUVECs and the migratory activity of EPCs in the transwell assay dose dependently. Moreover, microarray analysis was performed to explore how DHT changes the gene expression profiles of EPCs. We found 346 differentially expressed genes in androgen-treated EPCs. Angiogenesis-related genes like Egr-1, Vcan, Efnb2, and Cdk2ap1 were identified to be regulated upon DHT treatment. Furthermore, the enhanced angiogenic and migratory abilities of EPCs after DHT treatment were inhibited by Egr1-siRNA transfection. In conclusion, our findings suggest that DHT markedly enhances the vessel forming ability and migration capacity of EPCs. Egr1 signaling may be a possible pathway in this process. Summary Motor behaviors recruit task-specific neuronal ensembles in motor cortices, which are consolidated over subsequent learning. However, little is known about the molecules that can identify the participating neurons and predict the outcomes of the consolidation process. Using a mouse rotarod-learning task, we showed that lesion or inactivation of the secondary motor (M2) cortex disrupts learning of skilled movements. We tracked the endogenous promoter activity of the neuronal activity-regulated gene Arc in individual M2 neurons during rotarod learning by in vivo two-photon imaging of a knockin reporter. We found that task training initially recruits Arc-promoter-activated neurons and then consolidates them into a specific ensemble exhibiting persistent reactivation of Arc-promoter. The intensity of a neuron’s initial Arc-promoter activation predicts its reactivation probability and neurons with weak initial Arc-promoter activation are dismissed from the ensemble during subsequent training. Although gastroparesis carries a considerable health care and patient burden, associated epidemiological data are limited. To provide new real-world evidence for gastroparesis, we estimated disease prevalence, and investigated patient demographics and disease etiology in a large US claims database. This retrospective, cross-sectional analysis used de-identified, longitudinal patient-level enrollment and billing data for adults from the Optum Clinformatics Data Mart database, a large US national administrative health insurance claims database. Prevalence was age-, sex-, and geographical region–standardized using the 2018 US census. Descriptive analyses of demographic and clinical variables and underlying disease etiologies were performed. The overall standardized prevalence of gastroparesis was 267.7 (95% confidence interval [CI] 264.8–270.7) per 100,000 US adults, whereas prevalence of "definite" gastroparesis (individuals diagnosed within 3 months of gastric emptying scintigraphy testing with persistent symptoms for more than 3 months) was 21.5 (95% CI 20.6–22.4) per 100,000 persons. Patients with gastroparesis had an overall Charlson Comorbidity Index score of 4.2, indicating substantial comorbidity burden. The most frequently documented comorbidities were chronic pulmonary disease (46.4%), diabetes with chronic complication (37.3%), and peripheral vascular disease (30.4%). Patients most commonly had a diabetic etiology (57.4%; type 1, 5.7% and type 2, 51.7%), followed by postsurgical (15.0%), drug-induced (11.8%), and idiopathic (11.3%) etiologies. New evidence is provided regarding the prevalence, patient demographics, and etiology of gastroparesis in the US general population. Wider availability of reliable objective gastric emptying measures and further education of medical professionals in recognizing and diagnosing gastroparesis would benefit future studies and improve understanding of disease epidemiology. Background: Antioxidant vitamin (vitamin E, beta-carotene, and vitamin C) are widely used for preventing major cardiovascular outcomes. However, the effect of antioxidant vitamin on cardiovascular events remains unclear. Methodology and Principal Findings: We searched PubMed, EmBase, the Cochrane Central Register of Controlled Trials, and the proceedings of major conferences for relevant literature. Eligible studies were randomized controlled trials that reported on the effects of antioxidant vitamin on cardiovascular outcomes as compared to placebo. Outcomes analyzed were major cardiovascular events, myocardial infarction, stroke, cardiac death, total death, and any possible adverse events. We used the I2 statistic to measure heterogeneity between trials and calculated risk estimates for cardiovascular outcomes with random-effect meta-analysis. Independent extraction was performed by two reviewers and consensus was reached. Of 293 identified studies, we included 15 trials reporting data on 188209 participants. These studies reported 12749 major cardiovascular events, 6699 myocardial infarction, 3749 strokes, 14122 total death, and 5980 cardiac deaths. Overall, antioxidant vitamin supplementation as compared to placebo had no effect on major cardiovascular events (RR, 1.00; 95%CI, 0.96–1.03), myocardial infarction (RR, 0.98; 95%CI, 0.92–1.04), stroke (RR, 0.99; 95%CI, 0.93–1.05), total death (RR, 1.03; 95%CI, 0.98–1.07), cardiac death (RR, 1.02; 95%CI, 0.97–1.07), revascularization (RR, 1.00; 95%CI, 0.95–1.05), total CHD (RR, 0.96; 95%CI, 0.87–1.05), angina (RR, 0.98; 95%CI, 0.90–1.07), and congestive heart failure (RR, 1.07; 95%CI, 0.96 to 1.19). Background: Nosocomial infection (NI) prolongs hospital stay and heightens mortality among patients who underwent cardiac surgery. We constructed a retrospective study to explore the prevalence of Staphylococcus aureus and methicillin-resistant Staphylococcus aureus (SA/MRSA) nasal colonization, as well as the effects of SA/MRSA decolonization bundle measures on SA/MRSA-related infection among Chinese cardiac patients., Methods: After reviewing the medical records, we divided cardiovascular surgery patients treated at our central campus into two groups: the baseline group (treated between January 2012 and December 2013) and the intervention group (treated between May 2014 and December 2020). Intervention measures consisted of preoperative nasal screening and targeted decolonization bundle therapy. The medical records of patients at our southern campus (treated between January 2017 and December 2020) were collected as an additional control group, since we did not implement SA intervention measures at this location. The incidences of SA/MRSA-related NI were then compared between the groups., Results: There were 794 patients in the baseline group and 2,826 in the intervention group. A total of 131 (4.6%) patients had SA nasal colonization, and among them, 33 patients (1.2%) were MRSA colonized. SA/MRSA was cleared in approximately 95% of the carriers. The total level of SA-related infection was significantly lower in the intervention group compared to the baseline group [0.354% vs. 1.133%, respectively; P=0.021; risk ratio (RR): 0.312; 95% confidence interval (CI): 0.127-0.766]. The incidence of MRSA-related infection followed the same trend (0.212% vs. 0.756%, respectively; P=0.030; RR: 0.281; 95% CI: 0.091-0.860). When compared to the southern campus, SA intervention measures at the central campus resulted in a significant reduction in total SA-related infection (1.132% vs. 0.284%, respectively; P=0.035; RR: 0.251; 95% CI: 0.077-0.820)., Conclusions: The prevalence of SA/MRSA colonization is relatively low among Chinese patients who received cardiovascular surgery. Urokinase is widely used in the dissolution of an acute pulmonary embolism due to its high biocatalytic effect. An important step of somatic variant calling algorithms for deep sequencing data is quantifying the errors. For targeted sequencing in which hotspot mutations are of interest, site-specific error estimation allows more accurate calling. The site-specific error rates are often estimated from a panel of normal samples, which has limited size and is subject to sampling bias and variance. We propose a novel statistical validation method for single-nucleotide variation (SNV) calling based on historical data. The validation method extracts the high-quality reads from the Binary Alignment/Map (BAM) files, finds the negative samples in the data, and builds a statistical model to call individual samples. It is particularly useful in detecting low-frequency variants that may be missed by traditional panel of normal-based SNV methods. Abstract: In precision oncology, immune check point blockade therapy has quickly emerged as novel strategy by its efficacy, where programmed death ligand 1 (PD-L1) expression is used as a clinically validated predictive biomarker of response for the therapy. Automating pathological image analysis and accelerating pathology evaluation is becoming an unmet need. Artificial Intelligence and deep learning tools in digital pathology have been studied in order to evaluate PD-L1 expression in PD-L1 immunohistochemistry image. We proposed a Dual-scale Categorization (DSC)-based deep learning method that employed 2 VGG16 neural networks, 1 network for 1 scale, to critically evaluate PD-L1 expression. The DSC-based deep learning method was tested in a cohort of 110 patients diagnosed as non-small cell lung cancer. This method showed a concordance of 88% with pathologist, which was higher than concordance of 83% of 1-scale categorization-based method.
