The gut microbiota is ascribed a crucial role in human health, particularly in regulating immune and inflammatory responses, which is why it is being associated with a wide range of diseases, including obesity, diabetes, and cancer. Nonetheless, fundamental ecological questions of microbiome establishment, stability and resilience, as well as its transmission across hosts and generations remain incompletely understood, partly due to the lack of methods for high-resolution microbiome profiling. New insights in this field can therefore directly contribute to the development of bacterial and microbiota-based therapies. This work introduces SameStr, a novel bioinformatic program for strain-resolved metagenomics that allows for the specific tracking of microbes across samples, enabling the detection and quantification of microbial transmission and persistence, as well as the observation of direct strain competition. Deployed across cohorts to process over 4200 metagenomes, SameStr enabled analysis of the microbiome with unprecedented phylogenetic resolution. The data included both publicly available metagenomes and sequence data generated in collaboration with our research partners, and was examined using multivariate statistics and machine learning frameworks. First, the establishment and development of the neonatal microbiota was studied, revealing a birth mode-dependent vertical transmission of the maternal microbiota. The microbiota of neonates born by cesarean section was characterized by increased relative abundance of oxygen-tolerant and atypical organisms and showed signs of a delayed establishment of a strictly anaerobic gut environment in these children. Such birth mode-dependent differences diminished over time, yet were measurable within the first two years of life. Furthermore, strain analysis verified the transmission and colonization of parental microbes, which indicated a possible lifelong colonization by microbes from selected species. Hiermit können der Nachweis und die Quantifizierung der Übertragung und Persistenz, sowie die Beobachtung der direkten Konkurrenz mikrobieller Stämme erfolgen. SameStr wurde kohortenübergreifend für die Analyse von über 4200 Metagenomen eingesetzt und ermöglichte die Profilierung des Mikrobioms mit einer beispiellosen phylogenetischen Auflösung. Dissertation von Benjamin Seethaler zum Thema "Einfluss der mediterranen Ernährung auf das Fettsäuremuster von Erythrozytenmembranen sowie auf Darmmikrobiota- und Darmbarriere-assoziierte Biomarker - Mechanismen und klinische Anwendungen". Zusammengefasst bieten die Ergebnisse des Promotionsprojekts neue Erkenntnisse zu den Wirkmechanismen und gesundheitlichen Effekten der mediterranen Ernährung. Von besonderer Bedeutung ist der von uns gezeigte Zusammenhang zwischen Ballaststoffen aus der mediterranen Ernährung, deren Verstoffwechselung zu kurzkettigen Fettsäuren und deren günstiger Einfluss auf eine gestörte Darmbarriere-Funktion. Unsere Untersuchungen können zukünftig die Grundlage für eine personalisierte Ernährungstherapie zur Verbesserung der gestörten Darmbarriere-Funktion bei Brustkrebs-Hochrisikopatientinnen darstellen. Weiterhin konnten wir LBP und Zonulin als Biomarker zur Erfassung der Darmbarriere-Funktion sowie zur Bestimmung und Beurteilung von Darmbarriere-Störungen etablieren. In summary, the results of the PhD project offer new insights into the biomedical mechanisms of action and health effects of the Mediterranean diet. Of particular importance is the relationship we have shown between dietary fiber from the Mediterranean diet, its fermentation to short-chain fatty acids, and its beneficial influence on impaired intestinal barrier function. In the future, our studies may provide the basis for personalized nutritional therapy to improve impaired gut barrier function in high-risk breast cancer patients. Furthermore, we were able to establish LBP and zonulin as biomarkers to detect gut barrier function and to determine and assess gut barrier disorders. This method validation simplifies or enables the assessment of intestinal barrier function in clinical practice or clinical trials.

Industrialization has impacted the human gut ecosystem, resulting in altered microbiome composition and diversity. Whether bacterial genomes may also adapt to the industrialization of their host populations remains largely unexplored. Here, we investigate the extent to which the rates and targets of horizontal gene transfer (HGT) vary across thousands of bacterial strains from 15 human populations spanning a range of industrialization. We show that HGTs have accumulated in the microbiome over recent host generations and that HGT occurs at high frequency within individuals. Comparison across human populations reveals that industrialized lifestyles are associated with higher HGT rates and that the functions of HGTs are related to the level of host industrialization. Our results suggest that gut bacteria continuously acquire new functionality based on host lifestyle and that high rates of HGT may be a recent development in human history linked to industrialization.

Jedná se o práci teoreticko-empirickou. Teoretická část se věnuje nejčastěji diagnostikovaným onemocněním GIT a komplexní ošetřovatelské péči při endoskopických výkonech. Data ukazují, že stoupá počet odběrů biopsie, a tím i výskyt onkologických a neonkologických nálezů., The thesis focuses on the increasing occurrence of both oncological and non-oncological GIT diseases in the catchment area of the gastroenterological outpatients´ department in Kutná Hora within the period of four years. The thesis consists both of a theoretical and a research part. The theoretical part is devoted to the description of the complex nursing care during endoscopic procedures, as well as of the most frequent GIT diseases. Background The human gut microbiome represents a diverse microbial community that varies across individuals and populations, and is influenced by factors such as host genetics and lifestyle. Diet is a major force shaping the gut microbiome, and the effects of dietary choices on microbiome composition are well documented. However, it remains poorly known how natural temporal variation in diet can affect the microbiome. The traditional Inuit diet is primarily based on animal products, which are thought to vary seasonally according to prey availability. We previously investigated the Inuit gut microbiome sampled at a single time point, and found no detectable differences in overall microbiome community composition attributable to the traditional Inuit diet. Results To determine whether seasonal changes in the Inuit diet might induce more pronounced changes in the microbiome, we collected stool and toilet paper samples, and dietary information from Inuit volunteers living in Resolute Bay (Nunavut, Canada), and compared them to individuals of European descent living in Montréal (Québec, Canada) consuming a typical Western diet. We sequenced the V4 region of the 16S rRNA gene to characterize microbiome diversity and composition, and compared samples collected with toilet paper or from stool. Our results show that these sampling methods provide similar, but non-identical portraits of the microbiome. Based on toilet paper samples, we found that much of the variation in microbiome community composition could be explained by individual identity (45–61% of variation explained, depending on the beta diversity metric used), with small but significant variation (3–5%) explained by sex or geography (Nunavut or Montréal). In contrast with our previous study at one time point, sampling over the course of a year revealed that diet explains 11% of variation in community composition across all participants, and 17% of variation specifically among Nunavut participants. Diet is a major determinant of community composition in the human gut microbiome, and “traditional” diets have been associated with distinct and highly diverse communities, compared to Western diets. However, most traditional diets studied have been those of agrarians and hunter-gatherers consuming fiber-rich diets. In contrast, the Inuit of the Canadian Arctic have been consuming a traditional diet low in carbohydrates and rich in animal fats and protein for thousands of years. We hypothesized that the Inuit diet and lifestyle would be associated with a distinct microbiome. We used deep sequencing of the 16S rRNA gene to compare the gut microbiomes of Montrealers with a Western diet to those of the Inuit consuming a range of traditional and Western diets. At the overall microbial community level, the gut microbiomes of Montrealers and Inuit were indistinguishable and contained similar levels of microbial diversity. However, we observed significant differences in the relative abundances of certain microbial taxa down to the subgenus level using oligotyping. For example, Prevotella spp., which have been previously associated with high-fiber diets, were enriched in Montrealers and among the Inuit consuming a Western diet. The gut microbiomes of Inuit consuming a traditional diet also had significantly less genetic diversity within the Prevotella genus, suggesting that a low-fiber diet might not only select against Prevotella but also reduce its diversity. Other microbes, such as Akkermansia, were associated with geography as well as diet, suggesting limited dispersal to the Arctic. Our report provides a snapshot of the Inuit microbiome as Western-like in overall community structure but distinct in the relative abundances and diversity of certain genera and strains. Ce mémoire s'insère dans le cadre d'un programme de recherche visant à identifier des loci génétiques jouant un rôle dans le développement des maladies inflammatoires de l'intestin. Le principal objectif de notre étude est de mieux comprendre la composante génétique dans l'étiologie des maladies inflammatoires de l'intestin. Ceci sera fait à l'aide de la reconstitution et de l'analyse des généalogies ascendantes d'individus atteints et non atteints, recrutés dans la population du Saguenay-Lac-Saint-Jean. Notre cohorte est composée de 49 personnes, dont 23 ont la colite ulcéreuse et 26 la maladie de Crohn. Le groupe témoin, est composé de 49 personnes. Nos analyses montrent des coefficients d'apparentement plus élevés chez les proposants non-fumeurs et chez ceux ayant la colite ulcéreuse. Les coefficients de consanguinité sont aussi plus élevés chez les proposants ayant une histoire familiale de maladies inflammatoires de l'intestin et chez ceux qui présentent des manifestations extra-intestinales. Finalement, cette étude nous permet de conclure qu'il y a probablement un facteur génétique dans les maladies inflammatoires de l'intestin et que ce facteur pourrait avoir des composantes récessives et dominantes.

Bij afsnoering van een gedeelte van de dunne darm geraakt de patiënt in een levensgevaarlijke situatie, waarbij chirurgisch ingrijpen altijd noodzakelijk is. Soms lijkt het, dat de circulatie in het aangedane gedeelte van de darm zich kan herstellen. De chirurg moet uiteindelijk echter beslissen, of resectie van het ischaemische gebied al dan niet dient plaats te vinden. Is slechts een kort gedeelte van de darm aangedaan, dan zal bij de minste twijfel over de vitaliteit van dat gedeelte tot resectie worden over· gegaan, waarbij het verwijderen van 10 of 20 cm darm meer of minder van weinig belang is. Als echter een groot gedeelte van de darm moet worden gereseceerd, kunnen soms afhankelijk van de lengte van het dunne darmgedeelte - resorptiestoornissen worden verwacht. De beslissing over de plaats van de transsectie van de darm kan in ~mike gevallen zeer moeilijk zijn: resectie van 50 cm dunne darm of minder kan dan voor de morbiditeit en eventueel mortaliteit van de patiënt doorslaggevend zijn. Het tijdens de operatie beoordelen van de vitaliteit van het darmweefsel geschiedt, conventioneel uitgevoerd, op grond van zorgvuldige observatie en palpatie. Deze wijzen van beoordelen voldoen in de praktijk, zeker bij het beoordelen van korte afgesnoerde segmenten, redelijk goed. Maar toch ontstaan in een niet onaanzienlijk aantal gevallen na dunne darm afsnoering complicaties, als gevolg van perforatie of stenose. Er is maar weinig onderzoek gedaan naar de mate van zekerheid die pcr-operatoire observatie en palpatie biedt. Het is heel wel denkbaar, dat met deze werkwijzen vaak teveel of onnodig wordt gereseceerd. Terwijl het ook mogelijk is dat, ten onrechte, niets of te weinig darmweefsel wordt weggenomen. Bij het beoordelen van de vitaliteit van een lang afgesnoerd gedeelte van de dunne darm, kunnen deze werkwijzen mogelijk aanleiding geven tot te uitgebreide resecties resulterend in resorptiestoornissel).
