1. Assessment of the effectiveness of phentolamine in reducing the duration of dental local anaesthesia

Abstract

Effective local anaesthesia is required to ensure pain-free dental procedures, but existing anaesthetics produce persistent soft tissue anaesthesia well beyond the average time for the painful treatment. This may result in lip and tongue biting in young children and the handicapped. The study reports the clinical attempts to usefully reduce this level of unwanted soft tissue anaesthesia by injecting into the anaesthetised tissues a potent vasodilating agent, phentolamine. Lignocaine and adrenaline is the standard dental local anaesthetic combination, with the adrenaline included to prolong the duration and depth of anaesthesia in the highly vascular perioral tissues. The added catecholamine prevents the anaesthetic from being rapidly absorbed from the injection site by producing intense vasoconstriction. An alternative dental local anaesthetic is the combination of prilocaine and felypressin. This also produces excessively long periods of soft tissue anaesthesia despite the vasoconstriction not being mediated through ?-adrenoceptors. Investigation of the clinical effects of reversing residual soft tissue anaesthesia produced by prilocaine and felypressin were thought worthwhile to enable the clinical safety of a potentially inappropriate drug combination to be assessed. It also produced comparative data for interpreting the catecholamine reponses to the routine lignocaine and adrenaline anaesthetics. Phentolamine is a short-acting and potent a-adrenoceptor blocking agent. The investigations confirm that a 2mg/ml phentolamine reversal solution, when injected after a 30 minute delay into the perioral tissues in the vicinity of the local anaesthetic solution, quickly reverses the vasoconstrictor effects of adrenaline in vivo. Maxillary infiltration soft tissue anaesthesia was reduced from 138.1 to 16.9 minutes (95% CI) and pulp anaesthesia from 40.3 to 4.8 minutes (95% CI). Inferior dental nerve block reversals were reduced from 160.3 to 17.3 minutes (95% CI) and pulp anaesthesia from 59.5 to 11.1 minutes (95% CI). Multiple injections of 6ml 2% lignocaine + 1:80,000 adrenaline were reversed with 3ml 2mg/ml phentolamine, and venous plasma local anaesthetic concentrations assayed using High Performance Liquid Chromatography (HPLC). No statistically significant increase in plasma lignocaine concentration (95% CI) was observed. Venous plasma lignocaine and catecholamine concentrations were measured after multiple perioral injections of 5.4ml 2% lignocaine with three differing adrenaline concentrations; 1:50,000, 1:80,000 and 1:100,000. No statistically significant difference in plasma lignocaine concentrations was noted between the groups. Plasma adrenaline concentrations were statistically raised in the 1:50,000 group (95% CI), but the noradrenaline concentrations showed no significant variation. The perioral injection of 3 ml 2mg/ml phentolamine reversal agent after the 30 minute delay produced increases in plasma lignocaine concentrations, with the weakest 1:100,000 adrenaline anaesthetic the most statistically significant (95% CI). The intense local vasodilatation was demonstrated to be effective against the nonadrenaline alternative anaesthetic prilocaine and felypressin, with a statistically significant rise in the venous plasma prilocaine concentration (95% CI). Plasma local anaesthetic concentrations failed to reach accepted levels of toxicity, and no adverse cardiovascular events were recorded. Venous plasma adrenaline concentrations were measured before and after dental surgical extractions under lignocaine and adrenaline or prilocaine and felypressin local anaesthesia. Venous plasma adrenaline concentrations were significantly raised in only half the cases, but the prilocaine and felypressin group showed a marked reduction in data scatter. Additional experiments attempting to reverse the soft tissue anaesthesia following injection of another dental local anaesthetic combination, mepivacaine and adrenaline, are presented as appendices. Plasma mepivacaine concentrations were not statistically raised compared with placebo. Venous plasma catecholamine concentrations were measured following perioral injection of 6ml 2% mepivacaine + 1:100,000 adrenaline, and after a 30 minute delay, with 3ml 2mg/ml phentolamine or placebo. No statistical difference was noted between the groups for adrenaline or noradrenaline concentrations, but a trend for the noradrenaline concentration to rise over the 2 hour test period was observed. No adverse local reactions or systemic cardiovascular changes were observed in any of the studies, implying that the prolonged soft tissue effects of dental local anaesthesia may be safely and rapidly reversed by injection of small increments of phentolamine into the adjacent perioral tissues. Such a facility will be useful in clinical dental practice where needlessly prolonged anaesthesia may have significant consequences to patients, such as lip biting in the handicapped.

Abstract

The Simons Observatory (SO) is a new cosmic microwave background experiment being built on Cerro Toco in Chile, due to begin observations in the early 2020s. We describe the scientific goals of the experiment, motivate the design, and forecast its performance. SO will measure the temperature and polarization anisotropy of the cosmic microwave background in six frequency bands centered at: 27, 39, 93, 145, 225 and 280 GHz. The initial con figuration of SO will have three small-aperture 0.5-m telescopes and one large-aperture 6-m telescope, with a total of 60,000 cryogenic bolometers. Our key science goals are to characterize the primordial perturbations, measure the number of relativistic species and the mass of neutrinos, test for deviations from a cosmological constant, improve our understanding of galaxy evolution, and constrain the duration of reionization. The small aperture telescopes will target the largest angular scales observable from Chile, mapping approximate to 10% of the sky to a white noise level of 2 mu K-arcmin in combined 93 and 145 GHz bands, to measure the primordial tensor-to-scalar ratio, r, at a target level of sigma(r) = 0.003. The large aperture telescope will map approximate to 40% of the sky at arcminute angular resolution to an expected white noise level of 6 mu K-arcmin in combined 93 and 145 GHz bands, overlapping with the majority of the Large Synoptic Survey Telescope sky region and partially with the Dark Energy Spectroscopic Instrument. With up to an order of magnitude lower polarization noise than maps from the Planck satellite, the high-resolution sky maps will constrain cosmological parameters derived from the damping tail, gravitational lensing of the microwave background, the primordial bispectrum, and the thermal and kinematic Sunyaev-Zel'dovich effects, and will aid in delensing the large-angle polarization signal to measure the tensor-to-scalar ratio. Abstract

Rationale & objectiveCardiovascular disease (CVD) is common and overall graft survival is suboptimal among kidney transplant recipients. Although albuminuria is a known risk factor for adverse outcomes among persons with native chronic kidney disease, the relationship of albuminuria with cardiovascular and kidney outcomes in transplant recipients is uncertain.Study designPost hoc longitudinal cohort analysis of the Folic Acid for Vascular Outcomes Reduction in Transplantation (FAVORIT) Trial.Setting & participantsStable kidney transplant recipients with elevated homocysteine levels from 30 sites in the United States, Canada, and Brazil.PredictorUrine albumin-creatinine ratio (ACR) at randomization.OutcomesAllograft failure, CVD, and all-cause death.Analytical approachMultivariable Cox models adjusted for age; sex; race; randomized treatment allocation; country; systolic and diastolic blood pressure; history of CVD, diabetes, and hypertension; smoking; cholesterol; body mass index; estimated glomerular filtration rate (eGFR); donor type; transplant vintage; medications; and immunosuppression.ResultsAmong 3,511 participants with complete data, median ACR was 24 (Q1-Q3, 9-98) mg/g, mean eGFR was 49±18 (standard deviation) mL/min/1.73m2, mean age was 52±9 years, and median graft vintage was 4.1 (Q1-Q3, 1.7-7.4) years. There were 1,017 (29%) with ACR < 10mg/g, 912 (26%) with ACR of 10 to 29mg/g, 1,134 (32%) with ACR of 30 to 299mg/g, and 448 (13%) with ACR ≥ 300mg/g. During approximately 4 years, 282 allograft failure events, 497 CVD events, and 407 deaths occurred. Event rates were higher at both lower eGFRs and higher ACR. ACR of 30 to 299 and ≥300mg/g relative to ACR < 10mg/g were independently associated with graft failure (HRs of 3.40 [95% CI, 2.19-5.30] and 9.96 [95% CI, 6.35-15.62], respectively), CVD events (HRs of 1.25 [95% CI, 0.96-1.61] and 1.55 [95% CI, 1.13-2.11], respectively), and all-cause death (HRs of 1.6

Abstract

The Simons Observatory (SO) is a new cosmic microwave background experiment being built on Cerro Toco in Chile, due to begin observations in the early 2020s. We describe the scientific goals of the experiment, motivate the design, and forecast its performance. SO will measure the temperature and polarization anisotropy of the cosmic microwave background in six frequency bands centered at: 27, 39, 93, 145, 225 and 280 GHz. The initial con figuration of SO will have three small-aperture 0.5-m telescopes and one large-aperture 6-m telescope, with a total of 60,000 cryogenic bolometers. Our key science goals are to characterize the primordial perturbations, measure the number of relativistic species and the mass of neutrinos, test for deviations from a cosmological constant, improve our understanding of galaxy evolution, and constrain the duration of reionization. The small aperture telescopes will target the largest angular scales observable from Chile, mapping approximate to 10% of the sky to a white noise level of 2 mu K-arcmin in combined 93 and 145 GHz bands, to measure the primordial tensor-to-scalar ratio, r, at a target level of sigma(r) = 0.003. The large aperture telescope will map approximate to 40% of the sky at arcminute angular resolution to an expected white noise level of 6 mu K-arcmin in combined 93 and 145 GHz bands, overlapping with the majority of the Large Synoptic Survey Telescope sky region and partially with the Dark Energy Spectroscopic Instrument. With up to an order of magnitude lower polarization noise than maps from the Planck satellite, the high-resolution sky maps will constrain cosmological parameters derived from the damping tail, gravitational lensing of the microwave background, the primordial bispectrum, and the thermal and kinematic Sunyaev-Zel'dovich effects, and will aid in delensing the large-angle polarization signal to measure the tensor-to-scalar ratio. Abstract

The next generation of cryogenic CMB and submillimeter cameras under development require densely instrumented sensor arrays to meet their science goals. The readout of large numbers ($\sim$10,000--100,000 per camera) of sub-Kelvin sensors, for instance as proposed for the CMB-S4 experiment, will require substantial improvements in cold and warm readout techniques. To reduce the readout cost per sensor and integration complexity, efforts are presently focused on achieving higher multiplexing density while maintaining readout noise subdominant to intrinsic detector noise. Highly-multiplexed cold readout technologies in active development include Microwave Kinetic Inductance Sensors (MKIDs) and microwave rf-SQUIDs. Both exploit the high quality factors of superconducting microwave resonators to densely channelize sub-Kelvin sensors into the bandwidth of a microwave transmission line. We present advancements in the development of a new warm readout system for microwave SQUID multiplexing, the SLAC Superconducting Microresonator RF electronics, or SMuRF. The SMuRF system is unique in its ability to track each tone, minimizing the total RF power required to read out each resonator, thereby significantly reducing the linearity requirements on the cold and warm readout. Here, we present measurements of the readout noise and linearity of the first full SMuRF system, including a demonstration of closed-loop tone tracking on a 528 channel cryogenic microwave SQUID multiplexer. SMuRF is being explored as a potential readout solution for several future CMB projects including Simons Observatory, BICEP Array, CCAT-prime, Ali-CPT, and CMB-S4. Abstract

The Simons Observatory (SO) is a new cosmic microwave background experiment being built on Cerro Toco in Chile, due to begin observations in the early 2020s. We describe the scientific goals of the experiment, motivate the design, and forecast its performance. SO will measure the temperature and polarization anisotropy of the cosmic microwave background in six frequency bands: 27, 39, 93, 145, 225 and 280 GHz. The initial configuration of SO will have three small-aperture 0.5-m telescopes (SATs) and one large-aperture 6-m telescope (LAT), with a total of 60,000 cryogenic bolometers. Our key science goals are to characterize the primordial perturbations, measure the number of relativistic species and the mass of neutrinos, test for deviations from a cosmological constant, improve our understanding of galaxy evolution, and constrain the duration of reionization. The SATs will target the largest angular scales observable from Chile, mapping ~10% of the sky to a white noise level of 2 $\mu$K-arcmin in combined 93 and 145 GHz bands, to measure the primordial tensor-to-scalar ratio, $r$, at a target level of $\sigma(r)=0.003$. The LAT will map ~40% of the sky at arcminute angular resolution to an expected white noise level of 6 $\mu$K-arcmin in combined 93 and 145 GHz bands, overlapping with the majority of the LSST sky region and partially with DESI. With up to an order of magnitude lower polarization noise than maps from the Planck satellite, the high-resolution sky maps will constrain cosmological parameters derived from the damping tail, gravitational lensing of the microwave background, the primordial bispectrum, and the thermal and kinematic Sunyaev-Zel'dovich effects, and will aid in delensing the large-angle polarization signal to measure the tensor-to-scalar ratio. Abstract

The use of tractor tugs throughout the United States for shiphandling and escort work has been a growing factor over the past several years. The West Coast has been on the leading edge in the use of these boats for tanker escort and ship assist work since the early l980s. Across the country the construction rate is now approaching ten new buildings a year and there are few signs of slowing. On the East Coast, however, tug companies have generally failed to follow in the same direction as Pacific Coast tug owners, such as Foss Maritime, Crowley Marine, and SeaSpan. Many East Coast towing companies believe that the equipment currently in use is satisfactory for the work required. It is also a general consensus that the cost of purchasing a new advanced tug is unwarranted by current demand. The Port of Charleston, South Carolina shares this same common problem with other East Coast ports such as Norfolk, Savannah, and New York. The intent of this study is to determine a possible solution for the Port of Charleston of how best to handle ships of increasing size safely, in a manner which is least expensive for the operation of the port as a whole. Currently, larger container vessels calling on the port's North Charleston Terminal are restricted in their transit times to the periods of flood tide only. Additionally, an increased number of private terminals will be increasing overall vessel calls. A suggestion will be made for the best action to take considering the proper application of tractor tugs and their specialized operating modes. This suggestion will be based on increasing the safety and efficiency of vessel transits in Charleston Harbor.

Abstract

Television cameras utilized by aircraft carrier Landing Signal Officers (LSO) need to provide good imagery, day and night, under conditions where the aircraft is illuminated by ambient light but the aircraft's bright landing lights are on, resulting in intra-scene dynamic range of -10E6. These TV cameras provide the LSO with glide-slope and lineup information, and also view deck operations, approaches, and launches. This application, vital to aircraft carrier operations, needs a television image sensor and camera system to replace the Image Isocon based cameras which are no longer manufactured. The television image sensor affording the necessary dynamic range is the Charge Coupled Device(CCD). Their high quantum efficiency and low readout noise make CCDs competitive at low illumination levels with intensified image sensors and provide superior performance at moderate and high light levels. The conclusion of this Phase I study is that a camera based on back illuminated CCD technology can match and in some cases exceed the benchmark Image Isocon camera performance in the demanding ILARTS application.

12. Optimising the process of anaerobic digestion through improved understanding of fundamental operational parameters

Abstract

The anaerobic digestion (AD) process is an established treatment for sewage sludge produced at municipal wastewater treatment plants. However, water utilities lack guidelines on the effect of upstream processes and AD operational conditions on the overall digestion performance. This is the first research that investigates the interactive effects of different process conditions to determine their overall impact on sludge AD performance. The research aims at determining the impact of physico-chemical properties on the biodegradability of feed sludge; quantifying the impact of multiple AD operational conditions on process performance and digestate quality and integrating the findings into a steady-state model to design, monitor and optimise the AD process of sewage sludge. The Box Behnken method was selected to design the experiment on multiple operational parameters in order to improve the estimation of the regression coefficients. Novel automatic pilot scale digesters were used to conduct uninterrupted digestion experiments for approximately 1,000 days. Together with these an array of lab scale and batch digestion systems were used to provide reference and monitor feed sludge biodegradability throughout the experimental programme. The results showed that measured sludge physico-chemical characteristics did not control bio-methane yield of primary sludge (PS), and PS produced similar biogas yield irrespective of the sludge source. By contrast, physico-chemical and operational parameters impacted the bio-methane yield of surplus activated sludge (SAS). These included sludge physico-chemical conditions (volatile solids content and elemental content) and upstream process conditions (solids retention time in the activated sludge plant). The operational parameters tested were PS:SAS ratio, digestion temperature and feed dry solids concentration (DS%). The results indicated a positive and significant interaction of high temperature and high DS% on specific biogas yield and pathogen removal, which could have significant implications in terms of process optimisation. In terms of reactor stability, the concentration of alkalinity and ammoniacal nitrogen were found primarily increasing with increasing DS% and SAS content, suggesting that operating the digester at relatively high load produces a more stable bio-chemical environment. It was concluded that AD of sewage sludge should be operated between 6-8% DS% at a temperature of 45oC in order to simultaneously produce a stable bio-chemical environment in the AD reactor, maximize biogas production and produce a conventionally treated sludge within a 16 day retention time. The produced steady-state model was used to determine that investments to upgrade an existing large scale sludge AD plant in the UK operating at sub-optimal conditions, could have a payback time of six years due to the benefits arising from displacement of imported electricity and fossil fuels.
