Abstract: Inferior vena cava agenesis (IVCA) is a rare anomaly predisposing affected people to lower-limb venous thrombosis with low frequency of pulmonary embolism. Antenatal thrombosis and inherited thrombophilia have been suggested as causes of IVCA. However, there is little evidence on the clinical course and management of this condition. We designed a patient registry to assess the thrombotic risk and features of IVCA.

Abstract: Portal vein thrombosis (PVT) refers to the development of a non-malignant obstruction of the portal vein, its branches, its radicles, or a combination. This Review first provides a comprehensive overview of all aspects of PVT, namely the specifics of the portal venous system, the risk factors for PVT, the pathophysiology of portal hypertension in PVT, the interest in non-invasive tests, as well as therapeutic approaches including the effect of treating risk factors for PVT or cause of cirrhosis, anticoagulation, portal vein recanalisation by interventional radiology, and prevention and management of variceal bleeding in patients with PVT. Specific issues are also addressed including portal cholangiopathy, mesenteric ischaemia and intestinal necrosis, quality of life, fertility, contraception and pregnancy, and PVT in children. This Review will then present endpoints for future clinical studies in PVT, both in patients with and without cirrhosis, agreed by a large panel of experts through a Delphi consensus process. These endpoints include classification of portal vein thrombus extension, classification of PVT evolution, timing of assessment of PVT, and global endpoints for studies on PVT including clinical outcomes. These endpoints will help homogenise studies on PVT and thus facilitate reporting, comparison between studies, and validation of future studies and trials on PVT.

Abstract: ObjectiveTargeting bacterial translocation in cirrhosis is limited to antibiotics with risk of antimicrobial resistance. This study explored the therapeutic potential of a non-absorbable, gut-restricted, engineered carbon bead adsorbent, Yaq-001 in models of cirrhosis and acute-on-chronic liver failure (ACLF) and, its safety and tolerability in a clinical trial in cirrhosis.DesignPerformance of Yaq-001 was evaluated in vitro. Two-rat models of cirrhosis and ACLF, (4 weeks, bile duct ligation with or without lipopolysaccharide), receiving Yaq-001 for 2 weeks; and two-mouse models of cirrhosis (6-week and 12-week carbon tetrachloride (CCl4)) receiving Yaq-001 for 6 weeks were studied. Organ and immune function, gut permeability, transcriptomics, microbiome composition and metabolomics were analysed. The effect of faecal water on gut permeability from animal models was evaluated on intestinal organoids. A multicentre, double-blind, randomised, placebo-controlled clinical trial in 28 patients with cirrhosis, administered 4 gr/day Yaq-001 for 3 months was performed.ResultsYaq-001 exhibited rapid adsorption kinetics for endotoxin. In vivo, Yaq-001 reduced liver injury, progression of fibrosis, portal hypertension, renal dysfunction and mortality of ACLF animals significantly. Significant impact on severity of endotoxaemia, hyperammonaemia, liver cell death, systemic inflammation and organ transcriptomics with variable modulation of inflammation, cell death and senescence in the liver, kidneys, brain and colon was observed. Yaq-001 reduced gut permeability in the organoids and impacted positively on the microbiome composition and metabolism. Yaq-001 regulated as a device met its primary endpoint of safety and tolerability in the clinical trial.ConclusionsThis study provides strong preclinical rationale and safety in patients with cirrhosis to allow clinical translation.Trial registration numberNCT03202498.

Abstract: •Dadas las múltiples visitas en la atención primaria relacionadas con la incapacidad temporal (IT) y las múltiples incidencias que se generan, este artículo recoge un resumen de las diferentes situaciones con las que nos encontramos los médicos de familia para que nos pueda ser de utilidad y ayuda en la gestión de la IT.•Empezamos aclarando la gestión de la IT en los autónomos y en los pacientes con regímenes de MUFACE, MUGEJU e ISFAS ya que tienen un manejo diferente a los pacientes de régimen general.•A continuación se explican las diferentes casuísticas de los pacientes que se trasladan estando en situación de IT.•Por otro lado, se comenta el papel de las mutuas colaboradoras en los procesos de IT comentando el circuito de las propuestas de altas motivadas.•Seguidamente se describen otras situaciones que no son susceptibles de una IT y que conviene conocerlas para asesorar correctamente a los pacientes, como la prestación económica por riesgo durante el embarazo y/o durante la lactancia y la prestación económica por el cuidado de menores afectados por cáncer u otra enfermedad grave.•Posteriormente, se explica cómo se gestiona una IT cuando llega a 365 días.

The aim of the study is to evaluate the survival benefit associated with G-CSF administration compared with standard medical therapy (SMT) in ACLF.

Abstract: BackgroundA pre-emptive transjugular intrahepatic portosystemic shunt (pTIPS) reduces mortality in high-risk patients with cirrhosis (Child-Pugh C/B+active bleeding) with acute variceal bleeding (AVB). Real-life studies point out that <15% of patients eligible for pTIPS ultimately undergo transjugular intrahepatic portosystemic shunt (TIPS) due to concerns about hepatic encephalopathy (HE). The outcome of patients undergoing pTIPS with HE is unknown. We aimed to (1) assess the prevalence of HE in patients with AVB; (2) evaluate the outcome of patients presenting HE at admission after pTIPS; and (3) determine if HE at admission is a risk factor for death and post-TIPS HE.Patients and methodsThis is an observational study including 2138 patients from 34 centres between October 2011 and May 2015. Placement of pTIPS was based on individual centre policy. Patients were followed up to 1 year, death or liver transplantation.Results671 of 2138 patients were considered at high risk, 66 received pTIPS and 605 endoscopic+drug treatment. At admission, HE was significantly more frequent in high-risk than in low-risk patients (39.2% vs 10.6%, p<0.001). In high-risk patients with HE at admission, pTIPS was associated with a lower 1-year mortality than endoscopic+drug (HR 0.374, 95% CI 0.166 to 0.845, p=0.0181). The incidence of HE was not different between patients treated with pTIPS and endoscopic+drug (38.2% vs 38.7%, p=0.9721), even in patients with HE at admission (56.4% vs 58.7%, p=0.4594). Age >56, shock, Model for End-Stage Liver Disease score >15, endoscopic+drug treatment and HE at admission were independent factors of death in high-risk patients.ConclusionpTIPS is associated with better survival than endoscopic treatment in high-risk patients with cirrhosis with variceal bleeding displaying HE at admission.

Abstract: Background and aimsCurrent prognostic scores of patients with acutely decompensated cirrhosis (AD), particularly those with acute-on-chronic liver failure (ACLF), underestimate the risk of mortality. This is probably because systemic inflammation (SI), the major driver of AD/ACLF, is not reflected in the scores. SI induces metabolic changes, which impair delivery of the necessary energy for the immune reaction. This investigation aimed to identify metabolites associated with short-term (28-day) death and to design metabolomic prognostic models.MethodsTwo prospective multicentre large cohorts from Europe for investigating ACLF and development of ACLF, CANONIC (discovery, n=831) and PREDICT (validation, n=851), were explored by untargeted serum metabolomics to identify and validate metabolites which could allow improved prognostic modelling.ResultsThree prognostic metabolites strongly associated with death were selected to build the models. 4-Hydroxy-3-methoxyphenylglycol sulfate is a norepinephrine derivative, which may be derived from the brainstem response to SI. Additionally, galacturonic acid and hexanoylcarnitine are associated with mitochondrial dysfunction. Model 1 included only these three prognostic metabolites and age. Model 2 was built around 4-hydroxy-3-methoxyphenylglycol sulfate, hexanoylcarnitine, bilirubin, international normalised ratio (INR) and age. In the discovery cohort, both models were more accurate in predicting death within 7, 14 and 28 days after admission compared with MELDNa score (C-index: 0.9267, 0.9002 and 0.8424, and 0.9369, 0.9206 and 0.8529, with model 1 and model 2, respectively). Similar results were found in the validation cohort (C-index: 0.940, 0.834 and 0.791, and 0.947, 0.857 and 0.810, with model 1 and model 2, respectively). Also, in ACLF, model 1 and model 2 outperformed MELDNa 7, 14 and 28 days after admission for prediction of mortality.ConclusionsModels including metabolites (CLIF-C MET) reflecting SI, mitochondrial dysfunction and sympathetic system activation are better predictors of short-term mortality than scores based only on organ dysfunction (eg, MELDNa), especially in patients with ACLF.

Abstract: Background: Combining endoscopic therapy and [beta]-blockers may improve outcomes in patients with cirrhosis and bleeding esophageal varices. Purpose: To assess whether a combination of endoscopic and drug therapy prevents overall and variceal rebleeding and improves survival better than either therapy alone. Data Sources: MEDLINE, EMBASE, the Cochrane Central Register of Controlled Trials, the Cochrane Database of Systematic Reviews, and conference proceedings through 30 December 2007. Study Selection: Randomized trials comparing endoscopic plus [beta]-blocker therapy with either therapy alone, without language restrictions. Data Extraction: Two reviewers independently extracted data on interventions and the primary study outcomes of overall rebleeding and mortality. Metaregression and stratified analysis were used to explore heterogeneity. Data Synthesis: 23 trials (1860 patients) met inclusion criteria. Combination therapy reduced overall rebleeding more than endoscopic therapy alone (pooled relative risk, 0.68 [95% CI, 0.52 to 0.89]; [/.sup.2] - 61%) or [beta]-blocker therapy alone (pooled relative risk, 0.71 [CI, 0.59 to 0.86]; [/.sup.2] = 0%). Combination therapy also reduced variceal rebleeding and variceal recurrence. Reduction in mortality from combination therapy did not statistically significantly differ from that from endoscopic (Peto odds ratio, 0.78 [CI, 0.58 to 1.07) or drug therapy (Peto odds ratio, 0.70 [CI, 0.46 to 1.06]). Effects were independent of the endoscopic procedure (injection sclerotherapy or banding). No trial-level variable associated with the effect was identified through metaregression or stratified analysis. Limitation: Statistically significant heterogeneity in trial quality and evidence for selective reporting and publication bias were found. Conclusion: A combination of endoscopic and drug therapy reduces overall and variceal rebleeding in cirrhosis more than either therapy alone.

Abstract: Abstract: Thermoplastic semi-crystalline polymers are excellent candidates for tissue engineering scaffolds thanks to facile processing and tunable properties, employed in melt-based additive manufacturing. Control of crystallization and ultimate crystallinity during processing affect properties like surface stiffness and roughness. These in turn influence cell attachment, proliferation and differentiation. Surface stiffness and roughness are intertwined via crystallinity, but never studied independently. The targeted stiffness range is besides difficult to realize for a single thermoplastic. Via correlation of thermal history, crystallization and ultimate crystallinity of vitamin E plasticized poly(lactide), surface stiffness and roughness are decoupled, disclosing a range of surface mechanics of biological interest. In osteogenic environment, human mesenchymal stromal cells were more responsive to surface roughness than to surface stiffness. Cells were particularly influenced by overall crystal size distribution, not by average roughness. Absence of mold-imposed boundary constrains makes additive manufacturing ideal to spatially control crystallization and henceforward surface roughness of semi-crystalline thermoplastics. 19. Enhanced Magnetism through Oxygenation of FePc/Ag(110) Monolayer Phases

Abstract: Iron phthalocyanines (FePc) adsorbed onto a Ag(110) substrate self-assemble into different monolayer phases going from rectangular to different oblique phases, with increasing molecular density. We have investigated the oxygen uptake capability of the different phases and their associated magneto-structural changes. Our study combines scanning tunneling microscopy and spectroscopy (STM/STS), X-ray magnetic circular dichroism (XMCD), and density functional theory (DFT) calculations. STM measurements reveal that the oxygenation reaction of the FePc/Ag(110) generally involves a displacement and a rotation of the molecules, which affects the electronic state of the Fe centers. The oxygen intercalation between FePc and the substrate is greatly obstructed by the steric hindrance in the high-density phases, to the point that a fraction of oblique phase molecules cannot change their position after oxidizing. Depending on the oxidation state and adsoption geometry, the STS spectra show clear differences in the Fe local density of states, which are mirrored in the XAS and XMCD experiments. Particularly, XMCD spectra of the oxidized phases reflect the distribution of FePc species (nonoxygenated, oxygenated-rotated, and oxygenated-unrotated) in the different cases. Sum rule analysis yields the effective spin (mseff) and orbital (mL) magnetic moments of Fe in the different FePc species. Upon oxygenation, the magnetic moment of FePc molecules increases about an order of magnitude, reaching mTOT∼ 2.2 μBper Fe atom.

20. Protocolo de evaluación de la afectación hepática en las enfermedades inmunológicas

Abstract: En el siguiente protocolo trataremos la orientación diagnóstica de un paciente con enfermedad de origen inmunológico y afectación hepática. No es inusual que el hígado se vea afectado en el contexto de enfermedades sistémicas y autoinmunes. Si bien en la mayoría de las ocasiones se tratará de alteraciones de la bioquímica sin relevancia clínica, en algunos casos puede aparecer daño hepático que conduzca al desarrollo de cirrosis o fallo hepático agudo. En un paciente con una enfermedad autoinmune que se presente con enfermedad hepática debemos considerar siempre la posibilidad de que presente una enfermedad hepática primaria. Del mismo modo, debemos descartar la hepatotoxicidad por fármacos. Presentamos en este protocolo los principales patrones de daño asociados a las enfermedades inmunológicas que pueden ayudar a orientar el diagnóstico de estos pacientes.

Abstract: La hepatitis autoinmune es una enfermedad hepática crónica, generalmente progresiva y con actividad fluctuante, que puede afectar a ambos sexos en todas las edades de la vida, siendo especialmente frecuente en mujeres jóvenes. Su prevalencia en Europa es de unos 15-25 casos por cada 100.000 habitantes. En un 25% de los pacientes se encuentra asociada a otras enfermedades autoinmunes. Aunque su etiopatogenia es desconocida, en su desarrollo intervienen múltiples factores, tanto genéticos como ambientales, que desencadenan una alteración en la regulación del sistema inmune que produce daño hepático. Su espectro de presentación clínica es muy variable, desde pacientes asintomáticos con alteración de las transaminasas, hasta el fallo hepático fulminante. El diagnóstico es complejo y se establece en base a características clínicas, bioquímicas e histopatológicas, siendo obligatorio descartar otra posible etiología de la hepatopatía. El tratamiento de primera línea son los corticoides (prednisona) y el tratamiento de mantenimiento se realiza con azatioprina. En casos de fracaso del tratamiento convencional o de progresión de la enfermedad, podría ser necesario el uso de otros inmunosupresores o incluso el trasplante hepático.

Abstract: La enfermedad hepática alcohólica (EHA) es la causa más prevalente de enfermedad hepática avanzada y cirrosis hepática en Europa incluyendo a España. De acuerdo con la Organización Mundial de la Salud la fracción de cirrosis hepática atribuible al uso de alcohol en España es del 73,8% entre varones y del 56,3% entre mujeres. La EHA incluye diversos estadios como la esteatohepatitis, la cirrosis y el cáncer hepatocelular. Además, enfermos con EHA de base e ingesta abundante de alcohol pueden desarrollar hepatitis alcohólica, que cursa con una elevada mortalidad. Hasta la fecha, el único tratamiento efectivo para tratar la EHA es la abstinencia prolongada. No existen tratamientos específicos, y el único tratamiento que aumenta la esperanza de vida en la hepatitis alcohólica es la prednisolona. Para enfermos con hepatitis alcohólica que no responden al tratamiento, algunos centros ofrecen la posibilidad de un trasplante precoz. Estas guías de práctica clínica tienen como objetivo proponer recomendaciones sobre la EHA teniendo en cuenta su relevancia como causa de hepatopatía crónica avanzada y cirrosis hepática en nuestro medio. En el presente trabajo se propone como objetivo responder las preguntas claves para la práctica clínica de Gastroenterología, Hepatología, así como de Medicina Interna y centros de salud primaria, poniendo al servicio del profesional de la salud la información más actualizada respecto al manejo y tratamiento de la EHA. Estas guías proporcionan recomendaciones basadas en la evidencia para el manejo clínico de esta enfermedad.

Abstract: La Asociación Española para el Estudio del Hígado (AEEH) está convencida de que la eliminación de la hepatitisC en España es posible siempre y cuando seamos capaces de emplear los recursos y las herramientas necesarias para la misma. Este documento refleja la posición de la AEEH respecto a la eliminación del virus de la hepatitisC (VHC), estableciendo una amplia serie de recomendaciones que se pueden agrupar en cinco categorías: 1)cribado del VHC en función de la edad, de la existencia de factores de riesgo clásicos de adquisición de la infección, búsqueda activa de pacientes diagnosticados con anterioridad y desarrollo de estrategias de microeliminación en poblaciones vulnerables; 2)simplificación del diagnóstico del VHC (diagnóstico en un solo paso y diagnóstico en el punto de atención del paciente); 3)simplificación del tratamiento de los pacientes y mejora de los circuitos asistenciales; 4)medidas de política sanitaria, y, finalmente, 5)establecimiento de indicadores de eliminación del VHC.

Abstract: El término de enfermedad vascular portosinusoidal (EVPS) hace referencia a una entidad clínico-patológica que engloba a aquellos pacientes con daño vascular intrahepático sin cirrosis en riesgo de desarrollar complicaciones graves de la hipertensión portal. Numerosas enfermedades sistémicas, alteraciones genéticas y agentes tóxicos se han relacionado con este trastorno, lo que hace que su diagnóstico sea un reto clínico importante. La reciente descripción de unos criterios diagnósticos uniformes y el mejor conocimiento de su fisiopatología permitirá una óptima identificación de los pacientes, incluso en fases precoces de la enfermedad. Aunque a día de hoy no se dispone de un tratamiento etiológico eficaz, el diagnóstico temprano permite establecer estrategias preventivas sobre algunas complicaciones graves de la hipertensión portal.

Representa la tercera causa de hospitalización por causa digestiva en el mundo occidental y continúa aumentando su incidencia debido a la elevada prevalencia de obesidad, colelitiasis, consumo de alcohol e hipertrigliceridemia en nuestro medio. A pesar de que en la mayoría de los casos el curso del cuadro clínico es leve, el fenómeno inflamatorio local puede complicarse con el desarrollo de una respuesta inflamatoria sistémica (SRIS) con capacidad de producir fracaso multiorgánico y muerte. El tratamiento de la pancreatitis aguda requiere monitorización estrecha y, en ocasiones, un abordaje multidisciplinar que incluya a gastroenterólogos, radiólogos, intensivistas y cirujanos.

Abstract: La pancreatitis aguda es una entidad de notable importancia debido a su elevada incidencia y a su no desdeñable morbimortalidad. Se conoce como pancreatitis aguda idiopática aquella en la que no se consigue determinar la causa del cuadro tras un estudio básico inicial. Conocer la etiología subyacente permite plantear un tratamiento dirigido para así disminuir el riesgo de recurrencia. La ecoendoscopia y la colangiografía por resonancia magnética son las pruebas de elección para profundizar en el estudio etiológico. La principal causa es la enfermedad litiásica no diagnosticada en el estudio inicial, cuyo tratamiento de elección es la colecistectomía. Por otra parte, la pancreatitis aguda recurrente se diagnostica tras la existencia de 2 o más episodios de pancreatitis aguda. El objetivo de esta revisión es proporcionar una aproximación actualizada de estas 2 entidades, repasando aspectos de su epidemiología, diagnóstico y alternativas terapéuticas disponibles.

Abstract: Background Missed oesophageal cancer (MEC) at upper gastrointestinal endoscopy (UGE) is poorly documented.Objective The objectives of this study were: (1) to assess the rate, predictors and survival of MEC; (2) to compare MEC and non-MEC tumours.Methods This was a retrospective cohort study conducted at four tertiary centres. Oesophageal cancers (ECs) diagnosed between 2008 and 2015 were included. Patients with a premalignant condition (Barrett, achalasia), prior diagnosis of EC or oesophagogastric junction tumour of gastric origin were excluded. MEC was defined as EC detected within 36 months after negative UGE.Results 123,395 UGEs were performed during the study period, with 502 ECs being diagnosed (0.4%). A total of 391 ECs were finally included. Overall MEC rate was 6.4% (95% confidence intervals (CI): 4.4–9.3%). The interval between negative and diagnostic UGE was less than 2 years in 84% of the cases. Multivariate analysis showed that a negative endoscopy was associated with proton pump inhibitor (PPI) therapy and less experienced endoscopists. MEC was smaller than non-MEC at diagnosis (25 versus 40 mm, p= 0.021), more often flat or depressed (p= 0.013) and less frequently diagnosed as metastatic disease (p= 0.013). Overall 2-year survival rate was similar for MEC (20%) and non-MEC (24.1%) (p= 0.95).Conclusions MEC accounted for 6.4% of all ECs and was associated with poor survival. High-quality UGE and awareness of MEC may help to reduce its incidence.

Abstract: El incremento acelerado de la esperanza de vida en nuestro medio está conduciendo a un aumento importante del uso de endoscopia digestiva en pacientes de edad avanzada. Teniendo en cuenta estos cambios demográficos, el abordaje de la endoscopia gastrointestinal en pacientes de edad avanzada cobra gran importancia. Si bien la endoscopia gastrointestinal es en general segura y bien tolerada incluso en pacientes muy ancianos, la aparición de cambios fisiológicos asociados al envejecimiento y la mayor prevalencia de comorbilidades cardiovasculares y pulmonares aumentan el riesgo de la sedación en estos pacientes. La edad por sí misma no supone una contraindicación para la realización de ningún procedimiento endoscópico. Sin embargo, los ancianos representan una población con particularidades propias que obligan a revisar las características, los riesgos y los beneficios de los distintos procedimientos endoscópicos digestivos en este grupo de edad.

Abstract: La enfermedad hepática grasa no alcohólica (EHGNA) es la causa más frecuente de hepatopatía crónica en nuestro medio y se prevé un incremento de su incidencia en los próximos años asociada al incremento de la obesidad y el síndrome metabólico. Esta guía de práctica clínica propone recomendaciones sobre el diagnóstico y en especial marcadores no invasivos, así como en el manejo y seguimiento de esta enfermedad. La intervención dietética basada en la dieta mediterránea y el cambio del estilo de vida constituyen el pilar del tratamiento de la EHGNA, pero aún falta por elucidar si la composición de la dieta puede influir en la mejoría de la enfermedad más allá de la pérdida de peso. El tratamiento con fármacos debe restringirse a los pacientes con esteatohepatitis y fibrosis significativa que no consiguen resolución de la esteatohepatitis después de una intervención con dieta y ejercicio físico durante un año. Nuevos fármacos aún en fases iniciales de desarrollo han demostrado ser superiores a placebo. Por último, el impacto de la EHGNA en la indicación de trasplante hepático, la viabilidad del injerto y la recidiva de EHGNA de novo tras el trasplante, así como el incrementado riesgo cardiovascular determinan todo el proceso peritrasplante hepático. Esta guía de práctica clínica se ha elaborado tras la I Reunión de Consenso sobre EHGNA con un panel de experto nacionales e internaciones en Sevilla y tienen como objetivo proponer recomendaciones basadas en la evidencia científica disponible para el manejo de estos pacientes.

44. Enfermedad hepática crónica asociada con cirugía de Fontan

Abstract: La enfermedad hepática relacionada con la cirugía de Fontan hace referencia a los cambios hepáticos secundarios a las alteraciones hemodinámicas y la congestión venosa sistémica que se producen tras ese procedimiento. Aunque su historia natural aún no está establecida, los pacientes con daño hepático más avanzado pueden presentar complicaciones de la hipertensión portal, como la ascitis, la hemorragia por rotura de varices o encefalopatía. En los últimos años se ha demostrado que el riesgo de hepatocarcinoma está incrementado. El seguimiento hepático periódico es fundamental para prevenir y tratar precozmente las complicaciones hepáticas.

Una característica común a muchas de estas enfermedades es que pueden causar hipertensión portal, con la elevada morbimortalidad que ello conlleva. Con frecuencia estas enfermedades se diagnostican en pacientes jóvenes y el retraso en su diagnóstico y/o un tratamiento inadecuado pueden reducir de forma importante la esperanza de vida.

47. Infecciones en la enfermedad inflamatoria intestinal

Abstract: Los pacientes con enfermedad inflamatoria intestinal son una población con especial predisposición a presentar infecciones bacterianas, víricas y fúngicas. La inmunosupresión iatrogénica, el contacto frecuente con el medio hospitalario y las intervenciones quirúrgicas son algunos de los factores de riesgo que explican el que las infecciones sean una de las principales causas de morbimortalidad en esta enfermedad. Algunas de estas infecciones cursan de forma larvada y paucisintomática en muchas de las fases de su historia natural; su diagnóstico y tratamiento suponen un verdadero reto si no se sistematiza su detección o no se tienen presentes en el diagnóstico diferencial.
