Olecranon fractures are classified using the commonly accepted Mayo classification. Its reliability has been analyzed by means of radiographs. A CT scan is often obtained due to joint involvement. Purpose of this study was to evaluate the intra- and interobserver reliability of the Mayo classification based on CT examination.

Die primäre Implantation einer Ellenbogentotalendoprothese stellt bei komplexen distalen Humerusfrakturen des älteren Patienten eine valide Behandlungsoption dar. Die Wahl der Therapie sollte sich an den spezifischen Frakturmerkmalen, individuellen Patientenfaktoren und der Erfahrung des Operateurs orientieren. Meist werden zementierte, gekoppelte „semiconstrained“ Totalendoprothesen verwendet. Bei ungekoppelten Prothesendesigns und Hemiprothesen sind intakte oder gut rekonstruierbare Kondylen und muskuloligamentäre Strukturen sowie ein erhaltener oder ersetzter Radiuskopf notwendig. Die Indikationsstellung ist unter Betrachtung der lebenslangen Gewichtslimitierung bei Totalendoprothesen und potenzieller intra- und postoperativer Komplikationen sorgfältig. Dieser Beitrag liefert einen Überblick über die Operationstechnik der Ellenbogenendoprothetik und die entsprechenden Indikationen.

Die isolierte Ellenbogenluxation stellt im Kindesalter ein seltenes Verletzungsmuster dar. Das Behandlungsergebnis ist insbesondere vom Ausmaß der ligamentären Verletzungen und von der Anwesenheit knöcherner Begleitverletzungen abhängig, weshalb diesen bei der Diagnostik besondere Beachtung geschenkt werden muss. Ziel dieses Fallberichts ist die Darstellung der Behandlung und des Verlaufs einer isolierten traumatischen Ellenbogenluxation im Kindesalter. Bei einem 6-jährigen Mädchen bestand nach einem Sturz mit dem Tretroller eine komplette Ellenbogenluxation links. Die Diagnostik konnte eine knöcherne Begleitverletzung ausschließen. Nach geschlossener Reposition zeigte sich eine isolierte mediale Instabilität mit Subluxationsneigung, die sich im Magnetresonanztomogramm (MRT) als Abriss des medialen Kollateralbandes darstellte. Nach einer 6-tägigen Ruhigstellung im Oberarmgips konnte mit einer funktionellen Nachbehandlung begonnen werden. Sechs Wochen postoperativ zeigte sich eine schmerzfreie Bewegung mit freiem Bewegungsausmaß. Die isolierte kindliche Ellenbogenluxation ohne knöcherne Begleitverletzungen konnte in unserem Fall durch Gipsruhigstellung und anschließende funktionelle Therapie erfolgreich therapiert werden. Entscheidend bei der Behandlung dieser Verletzung ist die sorgfältige Diagnostik und Behandlung von Begleitverletzungen. Eine eingeschränkte Beweglichkeit oder persistierende Instabilität als Spätfolge ist sehr selten. Isolated elbow dislocations are uncommon injuries in children. Especially the extent of ligamentous lesions and additional bony injuries are predictive for an unfavorable functional outcome; thus, these lesions must be given special consideration during the diagnostic workup. The aim of this case report is the presentation of treatment and outcome of an isolated traumatic elbow dislocation in a child. We present a 6-year-old girl with a traumatic elbow dislocation after a fall on the outstretched arm. No additional bony lesions were seen in the diagnostic workup. After closed reduction, isolated medial instability was discovered with tendency for subluxation. The MRI showed a tear of the medial collateral ligament. After immobilization for 6 days in an upper-arm cast, early functional treatment was started. The follow-up examination 6 weeks after trauma showed no pain with a full range of motion. The treatment of isolated elbow dislocation without additional bony injuries in children with plaster immobilization and early functional treatment showed an excellent functional outcome in our case. Detection of additional bony injuries of the elbow joint is crucial for the treatment of these injuries. In the long-term follow-up, restriction of the range of motion or persistent joint instability are rare.

Background: Heterotopic ossification (HO) is defined as the formation of mature lamellar bone in non-skeletal tissues. Heterotopic ossification of the elbow is recognized as an uncommon but challenging complication following specific mechanisms of injury. However, the underlying pathophysiology as well as the optimal management and prophylaxis strategies continue to be debated. Methods: We reviewed the available literature searching Medline via the PubMed interface. The main objective was to highlight current concepts of the pathophysiology, risk factors, diagnostic modalities, treatment practices, and prophylaxis recommendations of elbow HO. Results: Heterotopic ossification of the elbow is commonly observed after trauma or surgery (3%–89%), CNS injuries (20%), and burns (0.1%–3%). The pathologic recruitment, activation, and expansion of chondro-osseous progenitors is affected by the local microenvironment resulting in formation of ectopic bone. Risk factors include specific mechanisms of injury, patient morbidity, and treatment modalities. Standard radiographs are usually sufficient for clinical assessment; however, ultrasound may be used as an early diagnostic modality and computed tomography (CT) scans can facilitate preoperative planning. Nonsteroidal anti-inflammatory drugs (NSAIDs) and radiation therapy (RT) are the most common prophylactic options for HO. In advanced stages, surgical resection has been found to improve patient outcomes regardless of the initial cause of injury. Conclusion: Elbow HO is a diverse pathologic process with specific risk factors related to the individual etiology. The prophylactic use of NSAIDs and RT needs to be further investigated in the context of elbow HO. Patients with persistent functional impairment due to the disorder may be treated effectively by open or arthroscopic excision.

Background: Fractures of the coronoid process require adequate treatment, as otherwise they often lead to chronic instability and the development of rapidly progressive cubital osteoarthritis. Commonly, such patients are treated surgically; however, a defined fraction of patients might benefit from conservative treatment. Methods: We systematically reviewed the available literature searching Medline using the PubMed interface. The primary objective was to determine indications, treatment protocols, and associated functional outcomes and complications of entirely conservatively treated fractures of the coronoid. The PRISMA guidelines were applied. Results: We included five studies, all retrospective in design, with a total of 62 patients. For the indication of conservative treatment, joint congruency on imaging was a prerequisite in all studies. This was supplemented on most occasions by dynamic clinical examinations, with a focus on pain, range of motion, and adequate joint stability. Treatment protocols included close-meshed imaging, early functional mobilization, and some form of resting immobilization. Treatment success, defined as excellent or good results according to the Mayo Elbow Performance Score, was 95.1% at an average follow-up of 36 months. Six patients (9.7%) underwent surgical treatment during follow-up; 19 patients (30.6%) suffered complications that did not result in surgery. Conclusion: The data of this work indicate that most patients achieve satisfactory results following conservative treatment of coronoid fractures caused by rotational moments. Concentric reduction of the joint, clinical exclusion of even mild subluxations, systematic early functional aftercare, and close radiographic monitoring appear to be mandatory requirements. The evidence remains scarce, and conclusions drawn from this review should be viewed with caution, as prospective randomized controlled trials are not available to date. Durch den vorliegenden Artikel soll ein Überblick über den aktuellen Stellenwert der konservativen Therapie bei häufigen Verletzungen am Ellenbogen gegeben und Besonderheiten ausgewählter konservativer Verfahren dargestellt werden. Sowohl bei Akutverletzungen (bspw. Frakturen, ligamentäre Ellenbogenluxationen) als auch bei chronischen Pathologien (bspw. Epicondylopathia humeri radialis/ulnaris, Ellenbogensteife) sollte ein Behandlungspfad mit den Patienten herausgearbeitet werden, der sowohl die aktuellen konservativen als auch operativen Therapieoptionen berücksichtigt. Unter Betrachtung des Verletzungsmusters und den individuellen Ansprüchen der Patienten können durch die konservative Therapie häufig zufriedenstellende bis sehr gute Ergebnisse erreicht werden. Die Basis bietet bei akuten Verletzungen eine frühfunktionelle und durch Krankengymnastik unterstützte Therapie ohne längere Ruhigstellung. Bei chronischen Verletzungen setzt sich das konservative Therapiespektrum aus Modifikationen der Belastung, physiotherapeutischen Übungsbehandlungen, manueller Therapie, Stoßwellentherapie, Massagetherapie, antiphlogistischer oraler und topischer Therapie sowie auch aus lokalen Infiltrationen zusammen.

Lumbar spinal stenosis (LSS) and lumbar disc herniation (LDH) are often accompanied by frequently occurring leg cramps severely affecting patients' life and sleep quality. Recent evidence suggests that neuromuscular electric stimulation (NMES) of cramp‐prone muscles may prevent cramps in lumbar disorders. Thirty‐two men and women (63 ± 9 years) with LSS and/or LDH suffering from cramps were randomly allocated to four different groups. Unilateral stimulation of the gastrocnemius was applied twice a week over four weeks (3 × 6 × 5 sec stimulation trains at 30 Hz above the individual cramp threshold frequency [CTF]). Three groups received either 85%, 55%, or 25% of their maximum tolerated stimulation intensity, whereas one group only received pseudo‐stimulation. The number of reported leg cramps decreased in the 25% (25 ± 14 to 7 ± 4; p= 0.002), 55% (24 ± 10 to 10 ± 11; p= 0.014) and 85%NMES (23 ± 17 to 1 ± 1; p< 0.001) group, whereas it remained unchanged after pseudo‐stimulation (20 ± 32 to 19 ± 33; p> 0.999). In the 25% and 85%NMES group, this improvement was accompanied by an increased CTF (p< 0.001). Regularly applied NMES of the calf muscles reduces leg cramps in patients with LSS/LDH even at low stimulation intensity.

Background: Radial head fractures represent a common pathology that can cause permanent disability, especially if not treated correctly. Plain radiographs as well as computed tomography (CT) scans represent important diagnostic measures. The specific differences between these two imaging modalities with regard to diagnosis of radial head fractures have not been evaluated to date. Objective: This study aimed to compare estimations of fracture classification, percentage of articular fracture involvement, and fragment sizes through plain radiography with CT scan evaluations. Methods: A total of 52 consecutive cases of isolated radial head fractures with plain radiographs and CT scans were evaluated retrospectively. Two observers analyzed the fracture classification according to Mason, the percentage of articular fracture involvement, and the size of the largest fracture fragment by means of CT. Three trauma surgeons estimated these parameters through blinded plain radiographs. Intra- and inter-observer reliability were evaluated. Results: The CT scan evaluations showed high intra- and inter-observer reliability without significant differences between the two observers. X‑ray estimations of fracture classifications showed only fair agreements. Moreover, the estimations of articular fracture involvement and fragment sizes differed significantly from the CT scan evaluations. While the fragment size tended to be underrated, the articular involvement tended to be overrated. Conclusion: This study shows that plain radiographs often provide unreliable information regarding classification, articular involvement, and fragment sizes of radial head fractures. When in doubt, an additional CT scan should be carried out to assess the injury in greater detail.

Coronoidfrakturen sind seltene Frakturen am Ellenbogengelenk, die eine akute oder chronische posteromediale Rotationsinstabilität bedingen können. Die Klassifikation nach O’Driscoll hat sich in diesem Zusammenhang bewährt. Frakturen der anteromedialen Facette gehen in aller Regel mit relevanten Verletzungen des lateralen Kollateralbandes (LCL) und ggf. auch des medialen Kollateralbands (MCL) einher. Im Rahmen der klinischen und radiologischen Diagnostik steht v. a. die Beurteilung der Gelenkstabilität im Blickpunkt des Untersuchers. Aufgrund ihrer Größe und Position können Coronoidfrakturen in der konventionellen Röntgendiagnostik leicht übersehen bzw. nur unzureichend beurteilt werden, sodass schnittbildgebende Verfahren zu empfehlen sind. In seltenen Fällen mit fehlender oder geringer Frakturdislokation kann bei zentriertem Gelenk die konservative Therapie erwogen werden. In aller Regel ist jedoch aufgrund der massiven Gelenkinstabilität mit Dezentrierung des Gelenks die operative Therapie mit knöcherner Rekonstruktion des Coronoids und LCL- sowie ggf. MCL-Rekonstruktion indiziert.

Use of a relatively larger glenosphere with some larger overhang helps to minimize posterior impingement and some degree of glenoid lateralization is also beneficial in reverse shoulder arthroplasty (RSA). The optimal amounts of inferior overhang and lateralization are not agreed upon. The purpose of this in-vitro biomechanical study is to analyze the effect of glenosphere size and glenoid lateralization on deltoid load, focusing on differences in its three distinct heads.

Lateralization of the metaglene reduces scapular notching or impingement. However, the effect on joint mobility remains unclear. With increased attention to reverse shoulder arthroplasty over the past years, the aim of this biomechanical study is to analyze the effect of metaglene lateralization on range of motion in reverse shoulder arthroplasty.

Hintergrund: Die parenterale Antibiotikagabe im Rahmen der Therapie von periprothetischen Infektionen erfordert in der Regel eine stationäre Behandlung und geht mit hohen Kosten einher. Fragestellung: Es wurden tatsächliche stationäre Behandlungskosten („inpatient parenteral antibiotic therapy“ [IPAT]) mit simulierten Kosten einer ambulanten Behandlung („outpatient parenteral antibiotic therapy“ [OPAT]) von Patienten mit periprothetischen Gelenkinfektionen verglichen. Die Auswertung erfolgte aus Perspektive der Kostenträger (gesetzliche Krankenversicherung [GKV]) und Leistungserbringer (Krankenhäuser). Material und Methoden: Die Analyse und Simulation erfolgten auf Grundlage einer ICD-10 (Internationale statistische Klassifikation der Krankheiten und verwandter Gesundheitsprobleme, 10. Revision) für das Behandlungsjahr 2015 mit der Diagnose T84. Ergebnisse: Die simulierte Reduktion von 159 Bettentagen bei den in die Studie eingeschlossenen 12 Patienten erbrachte aus Sicht der Kostenträger eine Reduktion der Gesamtkosten um >18.000?€. Aus Perspektive der Leistungserbringer verbesserte sich der Reinerlös um >22.000?€. Die Gesamtkosten der OPAT für den Kostenträger beliefen sich auf >57.000?€. Für den Leistungserbringer zeigte sich in der Differenz von Poliklinikerlös und -kosten der OPAT ein Verlust von >1500?€. Diskussion: Die OPAT ist für Leistungserbringer insgesamt finanziell vorteilhaft. Weitere Vorteile durch Opportunitätskosten erscheinen interessant. Für den Kostenträger ist die OPAT insbesondere durch die ambulanten Medikamentenkosten mit einem finanziellen Mehraufwand verbunden. Der niedergelassene Sektor sollte durch die anzunehmende Mehrbelastung ebenso wie der anzunehmende Patientenkomfort bedacht werden.

Bandplastiken am Ellenbogen werden zur Stabilisierung des Ellenbogens bei medialer oder lateraler Instabilität durchgeführt. Rezidivinstabilitäten bei medialer Bandplastik treten bei ca. 2 %, nach lateraler Bandplastik bei ca. 8 % auf. Andere revisionspflichtigen Komplikationen sind tiefe oder oberflächliche Infektionen, störende Osteophyten am Olecranon (4,4–11,7 %), Frakturen am Bohrkanal (0–2,2 %) oder eine Neuropathie des N. ulnaris (0–24 %). Ziel des Artikels ist die Zusammenfassung der Ursachen und therapeutischen Strategien für eine Revision nach Bandplastiken am Ellenbogen.

Die Behandlung der ligamentären Ellenbogenluxation erfolgt in den meisten Fällen konservativ. Dabei stellt die frühfunktionelle Rehabilitation den evidenzbasierten Behandlungsstandard dar. Eine Ruhigstellung sollte allenfalls für wenige Tage erfolgen. Die Indikationsstellung zur operativen Behandlung wird weiterhin diskutiert. Während die operative Therapie dem konservativen Vorgehen nicht grundsätzlich überlegen ist, hat sie bei höhergradiger Instabilität durchaus ihre Daseinsberechtigung. Die Grenzen sind dabei fließend und in der aktuellen Literatur nur unvollständig abgebildet, was die Behandlung der ligamentären Ellenbogenluxation im klinischen Alltag erschwert. Ligamentous elbow dislocation is treated conservatively in the majority of cases. Early functional rehabilitation represents the evidence-based standard of care. Immobilization should only be performed for a few days. The indications for surgical treatment remain the subject of discussion. While surgical treatment does not necessarily provide superior results when compared to conservative treatment, it is warranted in cases of pronounced instability. The transitions from one to another are fluent and only incompletely depicted in the current literature, making it difficult to treat ligamentous elbow dislocations in everyday clinical practice.

Most olecranon fractures are treated with open reduction and internal fixation (ORIF) leading largely to good clinical results. However, some studies have reported increased complication rates in the elderly. Therefore, nonoperative treatment of olecranon fractures in the elderly patient has been investigated by several authors in recent years. The aim of this study was to evaluate the clinical results of nonoperative treatment of olecranon fractures in elderly patients by means of a systematic review of literature. We systematically reviewed the literature on nonoperative treatment of olecranon fractures in patients with a minimum age of 70 years. Primary outcome parameters investigated were the Mayo Elbow Performance Index (MEPI) and the Disabilities of the Arm, Shoulder and Hand (DASH) score. Secondary outcome parameters were pain, range of motion (ROM), development of nonunion, and complications. Two retrospective case series, one prospective case study, and one randomized controlled trial were included (n= 70). The mean follow-up was 10.7 months. All studies reported good clinical results for nonoperative treatment of olecranon fractures. The mean MEPI was 95.1 and the mean DASH score was 12.3. Mean flexion of the elbow was 133° (106–140°) with a mean lag of extension of 15°. The operative cohort showed a significantly increased complication rate. Nonoperative treatment of olecranon fractures seems to provide acceptable clinical results in the elderly. However, the literature on the treatment of olecranon fractures in elderly patients is limited. While ORIF remains the gold standard, nonoperative treatment should be considered in the elderly patient in light of the recently published literature. Die meisten Olekranonfrakturen werden mittels offener Reposition und Fixateur interne (ORIF) behandelt, was im Wesentlichen zu guten Ergebnissen führt. Allerdings wurde in einigen Studien über erhöhte Komplikationsraten bei älteren Patienten berichtet. Daher wurde in den letzten Jahren die konservative Behandlung von Olekranonfrakturen bei älteren Patienten von verschiedenen Autoren untersucht. Ziel der vorliegenden Studie war es, die klinischen Ergebnisse der konservativen Behandlung von Olekranonfrakturen bei älteren Patienten anhand einer systematischen Übersicht über die Literatur zu ermitteln. Dazu durchsuchten die Autoren die Literatur zur konservativen Behandlung von Olekranonfrakturen bei älteren Patienten mit einem Mindestalter von 70 Jahren. Primäre Endpunkte waren der Mayo Elbow Performance Index (MEPI) und der Wert im Fragebogen Disabilities of the Arm, Shoulder and Hand (DASH). Sekundäre Endpunkte waren Schmerz, Bewegungsumfang („range of motion“, ROM), Entwicklung einer Pseudarthrose und Komplikationen. In die Auswertung wurden 2 retrospektive Fallserien, eine prospektive Fallstudie und eine randomisierte kontrollierte Studie eingeschlossen (n= 70). Die mittlere Nachbeobachtungsdauer betrug 10,7 Monate. In allen Studien wurde über gute klinische Ergebnisse für die nichtoperative Therapie von Ellbogenfrakturen berichtet. Durchschnittlich betrug der MEPI 95,1 und der DASH-Score 12,3. Die mittlere Flexion des Ellbogens lag bei 133° (106–140°) mit einem durchschnittlichen Rückstand bei der Extension von 15°. In der operativen Kohorte zeigte sich eine signifikant erhöhte Komplikationsrate. Die konservative Behandlung von Olekranonfrakturen scheint zu annehmbaren klinischen Ergebnissen bei älteren Patienten zu führen. Jedoch ist die Literatur zur Behandlung von Olekranonfrakturen bei älteren Patienten begrenzt. Die Behandlung mittels ORIF bleibt zwar der Goldstandard, aber angesichts der kürzlich publizierten Literatur sollte die konservative Behandlung bei älteren Patienten erwogen werden.

Die Kenntnis der Anatomie des Ellenbogengelenks ist entscheidend für Diagnostik und Behandlung der Instabilität am Ellenbogen. Verschiedene Strukturen stabilisieren das Ellenbogengelenk. Das humeroulnare Gelenk stabilisiert durch seine kongruente Form insbesondere in Extension das Ellenbogengelenk gegen Varus- und Valgusstress sowie Distraktion. Das Humeroradialgelenk und das proximale Radioulnargelenk dienen als sekundäre Stabilisatoren gegen Valgusstress. Der mediale Bandapparat, bestehend aus einem anterioren und einem posterioren Teil, stabilisiert primär gegen Valgusstress. Der laterale Bandapparat besteht aus einem radialen kollateralen Teil und einem lateralen ulnaren Kollateralband sowie dem Lig. anulare. In Kombination ist der laterale Kollateralbandapparat für eine Stabilisierung gegen Varusstress und die posterolaterale Instabilität besonders bedeutsam. Darüber hinaus stabilisiert die den Ellenbogen übergreifende Muskulatur dieses Gelenk dynamisch durch auf die Gelenkfläche ausgeübte Kompressionskräfte und statisch durch einen Faserverlauf der Muskulatur, der den Kollateralbändern ähnelt. Detailed knowledge of elbow anatomy is crucial for diagnosis and therapy of instabilities around the elbow joint. Several anatomical structures stabilize the elbow joint. Due to its high congruency, the ulnohumeral joint protects the joint especially against varus as well as valgus stress and distraction particularly in full extension. The radiohumeral joint and proximal radioulnar joint are secondary stabilizers against valgus stress. The primary stabilizer against valgus stress is the medial collateral ligament which can be divided into an anterior and a posterior bundle. The lateral collateral ligament consists of the radial collateral ligament, the lateral ulnar collateral ligament and the annular ligament. The lateral collateral ligament in its entirety stabilizes the elbow against varus forces and posterolateral rotatory instability. In addition, muscles spanning over the elbow joint are dynamic and static stabilizers via joint compression forces of the muscles and the orientation of muscle fibers that resemble those of the collateral ligaments.

Die arthroskopisch assistierte Osteosynthese von ausgewählten Frakturen des Ellenbogengelenks stellt eine potentielle Alternative zum offenen Vorgehen dar. Die Arthroskopie bietet die Vorteile einer minimal-invasiven Operation mit geringerer Zugangsmorbidität bei gleichzeitig detaillierterer Beurteilbarkeit der Frakturreposition und der bestehenden Begleitpathologien. Dislozierte Two-part-Frakturen ohne Trümmerzone eignen sich für eine arthroskopisch gestützte Versorgung. Hierzu zählen Mason-2-Frakturen des Radiuskopfes, transverse Koronoidfrakturen und koronare Abscherfrakturen des distalen Humerus Typ 1/2 A nach Dubberley. Nach temporärer Reposition mit Kirschner-Drähten erfolgt die definitive Osteosynthese mit kopflosen und kanülierten Kompressionsschrauben. Koronoidspitzenfrakturen können alternativ indirekt durch Anschlingen der am Fragment anhaftenden ventralen Kapsel in Form einer Suture-lasso-Technik versorgt werden. Relevante Begleitverletzungen müssen im Rahmen einer systematischen diagnostischen Arthroskopie erfasst und sollten, sofern nötig, mitversorgt werden. Die bisher publizierten Fallserien und Einzelfallberichte zur arthroskopisch gestützten Frakturversorgung am Ellenbogen schildern vielversprechende Ergebnisse, die verfügbaren klinischen Daten sind jedoch noch begrenzt. Arthroscopically assisted reduction and internal fixation of select elbow fractures represents a potential alternative to an open approach. Arthroscopy offers the advantages of a minimally invasive procedure with less approach-related morbidity while at the same time providing better evaluation of anatomic fracture reduction and detailed detection of concomitant injuries. Noncomminuted two-part fractures are suitable for arthroscopic fixation. These include Mason type 2 fractures of the radial head, transverse coronoid fractures and Dubberley type 1/2 A fractures of the capitulum and the trochlea, respectively. After temporary reduction with Kirschner wires, internal fixation with headless and cannulated compression screws can be performed. Coronoid tip fractures can alternatively be addressed indirectly by tying the anterior capsule, which attaches to the coronoid, down to the fracture bed using a suture lasso technique. Relevant concomitant injuries have to be identified through a systematic diagnostic arthroscopy and should be addressed if necessary. Currently published case reports and case series concerning arthroscopically assisted fixation of elbow fractures report promising results; clinical data, however, are limited thus far.

Die Anzahl der durchgeführten Implantationen von Ellenbogentotalendoprothesen ist zwischen 1993 und 2007 um mehr als das 2,5-Fache angestiegen. Das Outcome nach Endoprothetik des Ellenbogens ist nicht nur vom Prothesendesign, Zugangsweg und der Erfahrung des Operateurs abhängig, sondern auch von der postoperativen Rehabilitation. Ziel der vorliegenden Arbeit ist es, einen Überblick und eine Zusammenfassung der gegenwärtig in der Literatur verfügbaren Nachbehandlungskonzepte nach Ellenbogenendoprothetik zu geben. Des Weiteren sollen eigene Erfahrungen einbezogen werden. Die vorhandenen Publikationen zur Thematik beschreiben Nachbehandlungskonzepte, ohne diese wissenschaftlich zu untersuchen, die Konzepte zeigen jedoch vergleichbare Ansätze. Unterschiede in der Nachbehandlung ergeben sich durch das Prothesendesign sowie den Zugangsweg und die daraus resultierende Integrität des M. triceps brachii. Im Allgemeinen wird nach initialer Ruhigstellung zur Reduktion der Weichteilschwellung und Sicherstellung der primären Wundheilung unter befundabhängiger Limitation des Bewegungsumfangs eine physiotherapeutische Behandlung mit Steigerung der Beweglichkeit über 6 Wochen durchgeführt. Die maximale Belastung sollte erst nach 12 Wochen begonnen werden. Es besteht eine lebenslange Limitation der Gewichtsbelastung von 5 kg und repetitiver Belastung von 1 kg. Unterschiede in der Nachbehandlung ergeben sich durch das Prothesendesign sowie den Zugangsweg. Zugänge unter Schonung des Streckapparats sind zu bevorzugen, da sie eine Nachbehandlung mit rascher Wiederherstellung der aktiven Beweglichkeit ermöglichen und das Risiko einer Trizepsinsuffizienz minimieren. The number of arthroplasties of the elbow increased by 2.5-fold between 1993 and 2007. The postoperative outcome following total arthroplasty of the elbow depends not only on the design of the prosthesis, the surgical approach and the surgeon’s experience but also on the postoperative rehabilitation. The aim of the current article is to review and summarize the literature regarding rehabilitation concepts following total arthroplasty of the elbow including the expertise of our own clinical practice. The available literature on this topic describe follow-up treatment concepts without undertaking any scientific investigations but the mentioned rehabilitation concepts are similar. Postoperative rehabilitation differs due to the design of the prosthesis as well as the surgical approach and the resulting integrity of the triceps muscle. Following initial immobilization for reduction of swelling and tissue repair, a rehabilitation program with a limited range of motion increasing over 6 weeks is started. Load bearing should be avoided for 12 weeks. The patient has to be educated on the lifetime restriction of load bearing exceeding 5 kg and repetitive loads exceeding 1 kg. Differences in rehabilitation following total arthroplasty of the elbow result from the design of the prosthesis as well as the surgical approach. Surgical approaches preserving the triceps muscle are preferable due to the possibility of a faster postoperative recovery and the reduced risk of postoperative triceps insufficiency.

Elbow fractures and dislocations cause injury to important stabilizing structures and require specific treatment in order to regain functionality of the elbow joint. Regardless of whether surgical or non-surgical treatment is performed, correct rehabilitation following diagnosis of the injury pattern is crucial to achieve the best possible outcome in each individual case. After osteosynthesis, as well as after total elbow arthroplasty of distal humerus fractures, the postoperative protocol depends on whether a triceps-on or triceps-off approach was used. While triceps-on approaches allow active extension immediately postoperatively, triceps-off approaches require a more restrictive protocol. Radial head fractures are frequently accompanied by additional osteoligamentous injuries. Rehabilitation following radial head fractures usually involves active-assisted mobilization over the full range of motion; however, concomitant injuries need to be considered when formulating a treatment plan. Rehabilitation of surgically treated olecranon fractures commonly includes active flexion and gravity-assisted extension. Coronoid fractures are often the result of posterior elbow dislocation, which needs to be kept in mind during rehabilitation of these injuries. Complex proximal ulna fractures (e. g. Monteggia-like injuries) are frequently associated with persisting disability. Simple elbow dislocations are usually treated non-surgically. Early functional treatment should be preferred over prolonged immobilization as it decreases the risk of post-traumatic elbow stiffness without increasing the possibility of persistent instability. Ellenbogenfrakturen und -luxationen führen zu Verletzungen wichtiger Gelenkstabilisatoren und bedürfen einer spezifischen Therapie, um die Funktion des Ellenbogengelenks wiederherzustellen. Unabhängig davon, ob eine operative oder konservative Behandlung erfolgt, ist die korrekte Nachbehandlung nach Diagnose des Verletzungsmusters essenziell, um in jedem einzelnen Fall das bestmögliche klinische Ergebnis zu erreichen. Sowohl nach Osteosynthese als auch nach Ellenbogenprothese hängt das postoperative Nachbehandlungsregime nach distaler Humerusfraktur vom verwendeten Zugang ab. Während bei Trizeps-on-Zugängen unmittelbar postoperativ eine aktive Extension möglich ist, muss nach Trizeps-off Zugängen restriktiver nachbehandelt werden. Sie gehen häufig mit osteoligamentären Begleitverletzungen einher. Die Nachbehandlung umfasst in der Regel die aktiv assistierte Mobilisation über den vollen Bewegungsumfang. Die Begleitverletzungen müssen jedoch im Rehabilitationsplan berücksichtigt werden. Die Rehabilitation von operativ versorgten Olekranonfrakturen beinhaltet in der Regel die aktive Flexion und die Streckung entlang der Schwerkraft. Koronoidfrakturen resultieren häufig aus dorsalen Ellenbogenluxationen. Dies muss man sich bei der Nachbehandlung dieser Verletzungen in Erinnerung rufen. Komplexe proximale Ulnafrakturen (z. B. der Monteggia-Fraktur ähnliche Verletzungen) verursachen häufig dauerhafte Einschränkungen. Sie werden für gewöhnlich konservativ behandelt. Die frühfunktionelle Therapie ist gegenüber der Ruhigstellung zu bevorzugen, da sie das Risiko einer posttraumatischen Ellenbogensteife verringert, ohne mit einem relevant erhöhten Risiko für dauerhafte Instabilität einherzugehen.

Die Ellenbogensteife tritt meist als Folge einer Ellenbogenverletzung auf und kann aufgrund individueller Ansprüche für Patienten schon bei geringen Bewegungseinschränkungen negative Einflüsse auf die Lebensqualität bedeuten. Bei einem konservativen Therapieversuch ohne zufriedenstellendes Ergebnis können als operative Therapieverfahren eine arthroskopische oder offene Arthrolyse (ggf. mit Fixateur externe) bzw. eine Distraktionsarthrolyse durchgeführt werden. Dieser Übersichtsartikel beschreibt die Ätiologie und Therapiemöglichkeiten der Ellenbogensteife, sowie die unterschiedlichen Techniken der Arthrolyse, deren Indikationen und Ergebnisse. Elbow stiffness usually occurs after elbow trauma. Depending on the patient’s demands, even a small loss of motion can lead to severe functional limitations of the elbow joint which are likely to substantially reduce the quality of life. If conservative treatment does not provide a satisfying result, arthrolysis of the stiff elbow can be performed either arthroscopically or using an open approach (with external fixation) or distraction arthroplasty. This article discusses the etiology and treatment options for elbow stiffness and the different arthrolysis techniques with their indications and results.

Die arthroskopische Arthrolyse ist ein minimal-invasives, jedoch anspruchsvolles Therapieverfahren bei Ellenbogensteife. Die prospektive klinische Datenlage hierzu ist begrenzt. Ziel war es, klinische Kurzzeitergebnisse nach arthroskopischer Ellenbogenarthrolyse prospektiv zu analysieren und Einflussfaktoren auf das Outcome zu evaluieren. Es wurden 28 Patienten nach arthroskopischer Arthrolyse eingeschlossen. Präoperativ, 6 Wochen und 12 Monate postoperativ wurden der VAS-Score (visuelle Analogskala), der Bewegungsumfang (ROM) und der DASH-Score erhoben. Das Patientenalter betrug 50 ± 14 Jahre. 16 Patienten waren männlich, 12 waren weiblich. Die Ellenbogensteife war in 15 Fällen posttraumatischer Genese, Folge einer primären Arthrose in 10 Fällen und bei 3 Patienten Folge sonstiger Pathologien. Der Arthrosegrad nach Broberg/Morrey betrug 0 in 1 Fall, 1 in 7 Fällen, 2 in 9 Fällen und 3 in 11 Fällen. Der VAS-Score betrug präoperativ 5,3 ± 2,4 Punkte und verringerte sich auf 2,8 ± 2,7 Punkte nach 6 Wochen und 1,6 ± 1,8 Punkte nach 12 Monaten (p≤ 0,001). Der ROM verbesserte sich von 86°± 30° auf 106°± 28° nach 6 Wochen und 121°± 18° nach 12 Monaten (p≤ 0,01). Der DASH-Score verbesserte sich von 39,3 ± 17,8 Punkten auf 23,8 ± 15,7 Punkte nach 6 Wochen und 11,3 ± 8,7 Punkte nach 12 Monaten (p≤ 0,001). Geschlecht, Alter, Arthrosegrad und Ursache der Ellenbogensteife stellten keine prädiktiven Faktoren für das Outcome dar. 2 Komplikationen (7 %) wurden beobachtet. Die arthroskopische Arthrolyse stellt eine effektive Behandlungsmethode bei Ellenbogensteife dar, die unabhängig von Geschlecht, Alter, Arthrosegrad und Genese den Bewegungsumfang erhöht, Schmerzen lindert und so die Funktionalität der betroffenen Extremität verbessert. Arthroscopic arthrolysis is a minimally invasive but technically demanding treatment option for elbow stiffness. Prospective clinical data are limited thus far. The aim was to analyze the clinical short-term results of arthroscopic elbow arthrolysis and to evaluate factors predictive of the outcome. 28 patients (16 men, 12 women) following arthroscopic arthrolysis were enrolled in this study. The VAS (visual analogue scale) score, range of motion (ROM), and DASH score were obtained preoperatively as well as 6 weeks and 12 months after surgery. Posttraumatic elbow stiffness was present in 15 cases, elbow stiffness was due to primary osteoarthritis in 10 cases and due to other reasons in 3 cases. Osteoarthritis was absent in 1 case, grade 1 osteoarthritis was present in 7 cases, grade 2 in 9 cases and grade 3 in 11 cases according to the Broberg and Morrey classification. The VAS score was 5.3 ± 2.4 points preoperatively and decreased to 2.8 ± 2.7 points after 6 weeks and 1.6 ± 1.8 points after 12 months (p≤ 0.001). The ROM increased from 86± 30° to 106± 28° after 6 weeks and to 121± 18° 12 months postoperatively (p≤ 0.01). The DASH score improved from 39.3 ± 17.8 points to 23.8 ± 15.7 points after 6 weeks and to 11.3 ± 8.7 points 12 months postoperatively (p≤ 0.001). Sex, age, grade of osteoarthritis and cause of elbow stiffness were not predictive of the clinical outcome. Complications were seen in 2 patients (7 %). Arthroscopic arthrolysis represents an effective treatment method for elbow stiffness, which increases the range of motion, decreases pain levels and thus improves the functionality of the affected extremity regardless of sex, age, severity of osteoarthritis and etiology of elbow stiffness.

Verfügbare Radiuskopfprothesen reproduzieren die individuell variable Anatomie des proximalen Radius nur bedingt. Dies könnte einen Einfluss auf die Anatomie und Kinematik des proximalen Radioulnargelenks (PRUG) haben, welcher bislang noch nicht abschließend geklärt ist. Es sollte die native Gelenkkongruenz im PRUG in Abhängigkeit der Unterarmrotation analysiert und mit der nach Implantation einer monopolaren Radiuskopfprothese verglichen werden. Für die vorliegende Studie standen 20 formalinfixierte Ellenbogengelenke zur Verfügung. Es wurden Optosil®-Abdrücke des proximalen radioulnaren Gelenkspalts im Nativzustand sowie nach endoprothetischem Radiuskopfersatz (MoPyC, Tornier, Frankreich) in Neutralrotation, in 80° Pronation und in 80° Supination erstellt. Nach Anfertigung transversaler Schnitte dieser Abdrücke an der Ober- und Unterkante sowie der Mitte der Incisura radialis wurde die jeweilige Weite des Gelenkspalts an der anterioren Kante, im Zentrum und an der dorsalen Kante des PRUG digital vermessen. Der native Radiuskopf liegt in Neutralstellung zentral in der Incisura radialis und „shiftet“ in Pronation nach ventral und in Supination nach dorsal. Nach Radiuskopfersatz zeigt sich ein ähnliches Phänomen, jedoch ist die Dezentrierung des Gelenkspalts in Pro- und Supination signifikant deutlicher ausgeprägt mit einem zu einer Seite klaffenden Gelenkspalt von bis zu 3 mm. Radiuskopfprothesen weisen eine verminderte Kontaktfläche zur Incisura radialis auf. Dies könnte zu einer ungleichmäßigen Druckverteilung im proximalen Radioulnargelenk und somit zu frühzeitiger Arthrose mit Schmerzen und Bewegungseinschränkungen führen. Currently available radial head prostheses reproduce the variable anatomy of the proximal radius only to a limited extent. This could influence the anatomy and kinematics of the proximal radioulnar joint (PRUJ), which has not yet been fully investigated. The aim was to analyze the native joint congruity of the PRUJ depending on forearm rotation and to compare the results to the joint congruity after monopolar radial head replacement. A total of 20 formalin-fixed elbow joint specimens were available for this study. Optosil putty impressions of the joint space of the PRUJs were obtained from the native specimens and following radial head replacement (MoPyC, Tornier, France) in neutral rotation, in 80° pronation and in 80° supination. Axial sections of these impressions were performed at the upper and lower borders as well as in the middle of the lesser sigmoid notch of the ulna. The respective joint spaces at the anterior edge, at the center and at the posterior edge of the PRUJ were then measured digitally. The native radial head is centered within the lesser sigmoid notch in neutral rotation. It shifts anteriorly in pronation and posteriorly in supination, which also applies to the radial head implant; however, the anterior and posterior shift of the radial head replacement is significantly more pronounced which leads to a misalignment of the PRUJ with gaping of the joint space of up to 3 mm at either the anterior or posterior edge of the joint. Radial head prostheses reduce the contact area to the lesser sigmoid notch. This could lead to an imbalance of contact pressure in the PRUJ with subsequent development of osteoarthritis, which might cause pain and a decrease in range of motion.

Die elastisch stabile intramedulläre Nagelung (ESIN) mit „titanium elastic nail“ (TEN) bei Klavikulaschaftfrakturen ist als operatives Alternativverfahren zur Plattenosteosynthese etabliert. Typische Probleme des medial überstehende Endes, der lateralen Perforation sowie des Telescopings werden in der vorliegenden Arbeit analysiert. Bei 233 Patienten im Zeitraum seit 1996 fanden sich bei 31 % Schmerzen über dem medialen Ende, wobei bei lediglich 1 % eine Nachkürzung erforderlich wurde. Bei 5 % konnte ein Telescoping beobachtet werden, in 2 % eine (nicht intraoperativ aufgetretene) laterale Perforation. Ursächlich ist in den meisten Fällen eine initiale Fehleinschätzung des Frakturtyps mit unzureichender Abstützung der Hauptfragmente. Bei Komplikationen wie eingetretener Verkürzung oder schmerzhafter Pseudarthrose erfolgte die Revision und Reosteosynthese mit winkelstabiler Platte. Zusammenfassend ist die ESIN bei richtiger Indikation eine sichere operative Verfahrensalternative zur Plattenosteosynthese bei Klavikulaschaftfrakturen des Typ A und B nach OTA. The titanium elastic nail (TEN) in midshaft-clavicular fractures is an established alternative procedure to plate osteosynthesis. Typical complications are the overextending medial end, lateral perforation, and telescoping. Since 1996, 233 patients have been evaluated: 31 % patients developed pain at the medial end of the TEN, but only 1 % of them required prompt shortening of the overextending TEN. In 5 % of the cases, telescoping was detected, whereby lateral perforation was found postoperatively in 2 % of patients. These complications were caused by incorrect classification of the fracture with inadequate stabilization of the two main fragments. In cases of shortening of the clavicle, painful pseudarthrosis or malunion, an operative procedure with locking contact plates was performed. In summary, the TEN is a safe and useful procedure compared to plate osteosynthesis in midshaft clavicular fractures of type A or B according to the OTA classification.

Die Doppelplattenosteosynthese stellt den Goldstandard der Versorgung komplexer, distaler Humerusfrakturen im Erwachsenenalter dar. Ein Impingement zwischen der posterolateralen Humerusplatte und Olekranonspitze oder dem Caput radii – insbesondere nach Olekranonosteotomie – bei Extension des Ellenbogengelenks stellt dabei eine mögliche Komplikation dar. In der vorliegenden Studie sollten die anatomischen Lageverhältnisse der Olekranonspitze zur posterolateralen Humerusplatte analysiert werden, um Orientierungswerte für die intraoperative Applikati­on zu definieren. An 71 Leichenpräparaten wurden die Lageverhältnisse einer anatomisch präformierten, posterolateralen Humerusplatte zur Olekranonspitze in Streckung des Ellenbogens untersucht. In der Folge wurden makroskopisch sowie mithilfe eines Bildwandlers (distale Olekranon-Impingement-Linie, OIL) Landmarken bestimmt, die intraoperativ reproduzierbar sind. Der Abstand des distalen Plattenrands zur Knorpel-Knochen-Grenze des Capitulum humeri betrug im Mittel 4,8 mm [Standardabweichung (SD)  ± 1,2 mm]. Die Distanz vom medialen Plattenrand zum lateralen Rand der Fossa olecrani bzw. dem Epicondylus lateralis betrug im Durchschnitt 2,5 mm (SD ± 0,9 mm) bzw. 22,7 mm (SD ± 2,4 mm). Die röntgenologisch bestimmte distale OIL weist linksseitig einen Mittelwert von 5,2 mm (SD ± 1,5 mm), sowie rechtsseitig einen Mittelwert von 7,2 mm (SD ± 1,4 mm) auf. Die bestimmten Messwerte am Präparat dienen als Orientierungshilfe zur korrekten Positionierung der posterolateralen Humerusplatte. Zur Vermeidung eines Im­pingement sollten v. a. der Abstand zwischen lateralem Fossarand und medialer Plattenbegrenzung sowie das proximale Ende der di­stalen OIL berücksichtigt werden. Die poste­rolaterale Humerusplatte sollte nach Möglichkeit mindestens 4,3 mm lateral des radialen Fossarands platziert werden, um ein Impingement sicher zu vermeiden. Double-plate osteosynthesis has to be considered the gold standard for treating complex distal humeral fractures in adults. Impingement of the posterolateral plate with the olecranon or the radial head in extension of the elbow joint is a possible complication after double-plate osteosynthesis with a 90° configuration, especially after olecranon osteotomy. The goal of this study was to analyze the positioning of the posterolateral humeral plate in order to define guidance values for intraoperative application. The positioning of the posterolateral humeral plate in relation to the olecranon in full extension of the elbow joint was analyzed in 71 dissected upper extremities of cadavers. Values on the cadavers were measured under image-intensifier control (distal olecranon impingement line) that can be reproduced intraoperatively. The average distance between the distal edge of the plate and junction of cartilage and bone at the capitulum humeri was 4.8 mm (SD ± 1.2 mm). The average distances from the medial border of the plate to the lateral border of the olecranon fossa and to the lateral epicondyle were 2.5 mm (SD ± 0.9 mm) and 22.7 mm (SD ± 2.4 mm), respectively. The distal olecranon impingement line averaged 5.2 mm (SD ± 1.5 mm) on the left and 7.2 mm (SD ± 1.4 mm) on the right side of cadavers. The measured values offer landmarks for correct positioning of the posterolateral humeral plate. The distance between the lateral border of the olecranon fossa and the medial edge of the posterolateral plate as well as the proximal end of the distal olecranon impingement line should be respected in order to avoid impingement. If possible, the posterolateral plate should be placed at least 4.3 mm lateral to the radial edge of the olecranon fossa so that impingement can be successfully avoided.

Confronted with a failed rotator cuff (RC) repair or a massive tear with compromised tissue quality patch augmentation becomes a logical adjunct from a mechanical and biological point of view. We hypothesize that augmentation of the repair with a novel bioabsorbable scaffold might improve healing rates with few complications and reasonable added costs.
