Untersuchungen Im Blutzucker-Tagesprofil zeigten sich Hypoglykämien mit Werten Diagnose Symptomatische Hypoglykämie unter SGLT2(sodium-glucose linked transporter 2)-Inhibitor-Therapie bei einem Patienten mit HFrEF ohne Diabetes mellitus. Therapie und Verlauf Die Therapie mit Empagliflozin wurde beendet und ein erneutes Blutzucker-Tagesprofil durchgeführt – dieses zeigte keine hypoglykämischen Werte mehr. Folgerung Der hier vorgestellte Fall sensibilisiert für die unter SGLT2-Inhibitoren in Studien sehr selten auftretende Nebenwirkung der Hypoglykämie bei Patienten mit HFrEF ohne Vorliegen eines Diabetes mellitus.

Eine Coronavirus-Erkrankung mit SARS-CoV-2 (COVID-19) mit ihren unterschiedlichen Verläufen von mild bis kritisch ist häufig nicht nur eine akute Erkrankung, sondern führt darüber hinaus bei einem Teil der Betroffenen zu Organschäden und Körperfunktionsstörungen, die auch nach der Akutphase fortbestehen. Wenn die Gesundheitsfolgen die Selbstständigkeit im Alltag oder die beruflichen Fähigkeiten einschränken, ist eine spezifische rehabilitative Behandlung angezeigt.

ZusammenfassungDie S2k-Leitlinie SARS-CoV-2, COVID-19 und (Früh-) Rehabilitation macht auf der Basis einer Güterabwägung Handlungsanleitungen, die sowohl der Infektionsprävention, als auch den therapeutischen Zielen der Patient*innen Rechnung tragen. Die Leitlinie thematisiert in einem ersten Teil das Infektionsschutz-bezogene Vorgehen während der COVID-19-Pandemie generell bei Maßnahmen der Rehabilitation, einschließlich der Frührehabilitation. In einem zweiten Teil geht es um die Versorgung von COVID-19-Patient*innen mit rehabilitativen Therapieverfahren auf Intensivstationen und im Akutkrankenhaus, der Frührehabilitation, der Rehabilitation (Anschluss-Rehabilitation, Heilverfahren) und der ambulanten und Langzeit-Betreuung nach COVID-19, u. a. bei längerer Persistenz von Symptomen (Long- bzw. Post-COVID-19).Bei der aktuellen Version 2 der Leitlinie (Stand 1.11.2021) handelt es sich um eine konsensbasierte Leitlinie mit repräsentativem Gremium sowie einer strukturierten Konsensfindung (S2k-Leitlinie). Insgesamt nahmen 15 Fachgesellschaften an der Leitlinienerstellung und –konsentierung teil. In dieser Kurzfassung der Leitlinie werden alle Empfehlungen im Überblick wiedergegeben.

ZusammenfassungIn der COVID-19-Pandemie konnte der Dritte Abschnitt der Ärztlichen Prüfung an Simulationspersonen und Simulatoren durchgeführt werden. Auch im Fach Rechtsmedizin ist deren Einsatz sinnvoll, da eine höhere Standardisierung und Vergleichbarkeit von Prüfungsleistungen erreicht wird und die Verwendung von echten Leichnamen aus medikolegalen Gründen oft nicht vertretbar ist. Der vorliegende Fall berichtet über die Vor- und Nachteile einer Simulation im Staatsexamen, in der anhand einer Leichenschau am Simulator und einer Fremdanamnese eine Todesbescheinigung vollständig ausgefüllt werden sollte.

Zusammenfassung Einleitung Die Klimakrise hat gravierende Folgen für viele Lebensbereiche. Ganz besonders gilt dies für die Gesundheit des Menschen – auch in Europa. Während kardiovaskuläre, pneumologische und dermatologische Erkrankungen mit Bezug auf die Klimakrise häufig diskutiert werden, sind gastroenterologische Erkrankungen als Folge der Krise ebenfalls von erheblicher Bedeutung. Methoden Anhand einer Literaturrecherche (Pubmed, Cochrane Library) wurden für die Gastroenterologie in (Mittel-)Europa relevante Arbeiten identifiziert. Diese Arbeiten wurden von einem interdisziplinären Team um weitere Arbeiten ergänzt und zusammengefasst. Ergebnisse Die Klimakrise hat in Europa durch vermehrte Hitzewellen, Hochwasser und Luftverschmutzung Einfluss auf die Häufigkeit und Schwere von Erkrankungen des Gastrointestinaltraktes. Dabei sind Patienten mit Darmerkrankungen besonders vulnerabel für akute Wetterereignisse. Hinsichtlich der Langzeitfolgen des Klimawandels haben gastrointestinale Karzinome und Lebererkrankungen eine herausragende Bedeutung. Neben Gastroenteritiden sind weitere durch die stetige Erwärmung, Hitzewellen und Überschwemmungen begünstigte Infektionskrankheiten wie vektorübertragene Erkrankungen und Parasiten von Bedeutung. Diskussion Für vulnerable Patientengruppen sind konsequent Anpassungsstrategien zu entwickeln und umzusetzen. Risikopersonen müssen hinsichtlich individuell umsetzbarer Maßnahmen wie Hitzevermeidung, entsprechendem Trinkverhalten und Hygiene beraten werden. Zur Prävention von Lebererkrankungen und Karzinomen sind in erster Linie Empfehlungen zur physischen Aktivität und gesunden nachhaltigen Ernährung sinnvoll. Maßnahmen zur Prävention und Resilienzförderung können von der Ärzteschaft auf verschiedenen Ebenen unterstützt werden. Neben Bemühungen zu Nachhaltigkeit im unmittelbaren eigenen Arbeitsumfeld ist ein übergeordnetes Engagement für Klimaschutz von Bedeutung.

For further treatment, inpatient cardiac rehabilitation war carried out.At the start of the cardiac rehabilitation, patient complained of shortness of breath with little exertion, corresponding to NYHA III. Drug therapy included empagliflozin 10 mg, sacubitril/valsartan 24/26 mg, edoxaban 60 mg, torasemide 5 mg, verapamil 80 mg and amiodarone 200 mg.The daily blood sugar profile showed hypoglycemia with values ​​ 3.7 mmol/l ( 67 mg/dl). The Coronavirus disease with SARS-CoV-2 viral infection (COVID-19) and its diverse courses of disease from mild to critical illness frequently is not only an acute disease, but will - in a proportion of those affected - lead to organ structure and body function deficits that still exist or become apparent after the acute stage of disease. When clinically relevant symptoms or functional deficits (impairments) are documented more than four weeks after COVID-19 onset, the syndrome is called "Long-COVID", from 12 weeks after onset onwards "Post-COVID".In such cases and when everyday life functioning and return to work are affected by persisting deficits specialized rehabilitation treatment is indicated. An individual medical, frequently multi-professional diagnostic evaluation is mandatory in that situation: For adequate treatment, it is important to identify and objectify the individually underlying health conditions based on knowledge about the diverse potential consequences of COVID-19, to assess type and severity of functional consequences (impairments, activity limitations, and restrictions of participation) of Long-/Post-COVID individually, and then to decide on the treatment necessities and plans. The multimodal program of inpatient rehabilitation can improve the symptoms and prognosis of these patients. At the present time, however, only few data are available on the effectiveness of rehabilitation for heart failure patients.Methods: After receiving study approval from the ethics committee of the Saxony–Anhalt Medical Association, 200 patients with a primary or secondary diagnosis of heart failure were prospectively included in the study at Paracelsus-Harz-Clinic Bad Suderode, Quedlinburg, Germany. Baseline parameters such as age, gender, and BMI were documented. Outcome variables included NYHA classifications, quality of life, and mortality. For follow-up, the patients were contacted again by mail or phone after three and 12 months and, data on symptoms and serious events were recorded.Results: The proportion of patients with a highly reduced ejection fraction (HFrEF) was 13.5%, with a midrange reduced ejection fraction (HFmrEF) 33%, and with preserved ejection fraction (HFpEF) 53.5%. The mean age was 64 ± 11.9 years, the proportion of women 24.1%. The effects of rehabilitation were documented by low overall mortality (no patient died during the stay, only 4% of the patients died in the 12-month follow-up) and an improvement in NYHA classification during and after the inpatient rehabilitation.Conclusion: This monocentric study showed effects both for symptoms (improvement in NYHA classifications) and prognosis (overall mortality) after rehabilitation. These data reflect the effectiveness of multimodal rehabilitation and underscore the need for rehabilitation in patients diagnosed with heart failure after an acute event and hospital stay or who present with chronic deterioration.Keywords: heart failure, cardiac rehabilitation, inpatient rehabilitation

Die Herzinsuffizienz führt jährlich in Deutschland zu Behandlungskosten in Höhe von >5 Mrd. Euro. Die weltweite Prävalenz soll in den nächsten 15 Jahren um 25% zunehmen, und es ist mit einem Anstieg der Krankheitskosten um über 200% zu rechnen. Gründe sind u.a. die Zunahme kostenintensiver Prozeduren und die Abnahme der Krankenhausmortalität. Durch das multimodale Programm der Rehabilitation kann dieser Kreislauf unterbrochen werden.

Diese Leitlinie ( http://leitlinien.net ) fokussiert ausschlieslich auf den infarktbedingten kardiogenen Schock (IKS). Beide Strategien, sowohl die kardiologisch/herzchirurgische als auch die intensivmedizinische, sind zur erfolgreichen Behandlung und zum Uberleben der Patienten mit IKS essenziell. Dennoch beschaftigen sich sowohl die europaischen als auch die amerikanischen Leitlinien zu Herzinfarkt und Herzinsuffizienz und auch die Positionspapiere zum kardiogenen Schock nahezu ausschlieslich mit den kardiologischen Aspekten. In einem nominalen Gruppenprozess der Delegierten der Deutschen Gesellschaft fur Kardiologie – Herz- und Kreislaufforschung (DGK), der Deutschen Gesellschaft fur Internistische Intensiv- und Notfallmedizin (DGIIN), der Deutschen Gesellschaft fur Thorax‑, Herz- und Gefaschirurgie (DGTHG), der Deutschen Gesellschaft fur Anasthesiologie und Intensivmedizin (DGAI), der Osterreichischen Gesellschaft fur Internistische und Allgemeine Intensivmedizin (OGIAIN), der Osterreichischen Kardiologischen Gesellschaft (OKG), der Deutschen Gesellschaft fur Pravention und Rehabilitation von Herz-Kreislauferkrankungen (DGPR) und der Deutschen Interdisziplinaren Vereinigung fur Intensivmedizin (DIVI) wurden unter Leitung der Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften (AWMF) die Evidenzen zur Diagnose, zum Monitoring und zur Therapie des IKS systematisch gesammelt und – darauf aufbauend – Empfehlungen ausgearbeitet. Lag jeweils nur geringe Evidenz zum IKS vor, wurden auch Studienergebnisse generell zu Intensivpatienten gesichtet und dargestellt, um Analogieschlusse zu erlauben. Es wurden 95 Empfehlungen – inklusive zweier Statements – erarbeitet und darauf basierend 7 Algorithmen mit konkreten Anweisungen zum Handlungsablauf.

Die Mitral- und Trikuspidalklappeninsuffizienz sind haufige Herzklappenerkrankungen. Sie gehen als unabhangige Risikofaktoren mit einer erhohten Sterblichkeit bei Herzinsuffizienz einher. Die kathetergestutzte Rekonstruktion der Klappen stellt eine mittlerweile relevante Alternative zur rein medikamentosen bzw. chirurgischen Behandlung dar. Wahrend die kathetergestutzte Therapie der Trikuspidalklappeninsuffizienz in ersten Registern Effektivitat und Sicherheit demonstrierte, liegen fur die kathetergestutzte Therapie der Mitralklappeninsuffizienz mittlerweile mehrere randomisierte Studien vor. Dieses Papier stellt die Ergebnisse aktueller Studien dar und gibt Empfehlungen zur Indikation interventioneller Behandlungsverfahren bei Mitral- und Trikuspidalklappeninsuffizienz. Zur Sicherung der Qualitat dieser Verfahren werden von der Deutschen Gesellschaft fur Kardiologie die Kriterien fur die Zertifizierung von Zentren zur kathetergestutzten Therapie der Mitralklappeninsuffizienz aktualisiert.

ZusammenfassungDie neue Leitlinie der European Society of Cardiology (ESC) von 2019 liefert neue Empfehlungen zum Umgang mit dem chronischen Koronarsyndrom (CCS, früher stabile koronare Herzerkrankung = KHK). In diesem Beitrag werden Methoden zur Bestimmung der Prätestwahrscheinlichkeit eines CCS und die relevanten diagnostischen Mittel vorgestellt, wie etwa die neuen Empfehlungen für die Stellung des Angio-CT und die Verwendung der Koronarangiografie. Ein besonderer Schwerpunkt dieses Beitrags liegt auf denjenigen Patientengruppen, die Ärzte und Pflegekräfte im klinischen Alltag vor besondere Herausforderungen stellen. Hierzu gehören Patienten mit atypischer oder fehlender Klinik, Patienten höheren Alters und Patientinnen. Für diese Gruppen werden gesonderte Empfehlungen formuliert.

With increasing age valvular heart disease is among the most frequent diseases of the heart. Relevant valvular disease impairs not only the long-term prognosis but also physical resilience, activities of daily living and the quality of life. In cases of middle to high-grade symptomatic cardiac defects, valve replacement or valve reconstruction is still the surgical procedure of choice; however, in recent years the transcatheter percutaneous aortic valve replacement (TAVI) procedure has become more prominent for the most frequent defect, aortic valve stenosis. This article provides an overview of the aftercare and rehabilitation of patients following a TAVI intervention.

During the COVID-19 pandemic the third section of the medical examination could be performed on simulation patients and simulators. Their use is also beneficial in forensic medicine, as a higher level of standardization and comparability of examination performance is achieved, and the use of real corpses is often not justifiable for medicolegal reasons. This case reports on the advantages and disadvantages of a simulation in the state examination in which a death certificate was to be completely filled out on the basis of an external postmortem examination on the simulator and an external anamnesis.

In Germany, the COVID-19 pandemic led to substantial changes in outpatient and acute clinical cardiac care and rehabilitation. To estimate the influence of the COVID-19 pandemic on cardiac rehabilitation in 2020, institutional performance was compared to the pre-pandemic year 2019.The performance numbers from rehabilitation institutions were compared. These data were provided in 2019 and 2020 as part of an online survey that the German Society for the Prevention and Rehabilitation of Cardiovascular Diseases (DGPR) performs annually.Complete data for 2019 and 2020 were available from 60 institutions. The overall number of patients treated was reduced by 14.3%, with substantial differences between institutions. Women were more affected (-16.4%) than men (-13.4%) and retirees (-14.5%) significantly more than people still in the workforce (-7.5%). In 25 institutions (42.4%) there was a COVID-19 outbreak resulting in a partial or complete shutdown in 15.7%. In total, 34.5% of the institutions treated patients with COVID-19 infection, 30.5% after such an infection. A large majority of these institutions (72.1%) had to bear the costs for testing suspected cases of SARS-CoV-2-infection alone.The economic and logistic burden of the COVID-19 pandemic has posed a threat to cardiac rehabilitation in Germany.

Objectives Treatment with lipid-lowering therapy (LLT) such as statins, cholesterol absorption inhibitors, or PCSK9 inhibitors is of major importance for the survival of patients with atherosclerotic diseases, and adherence to LLT is essential for treatment success. The intention of this study was to investigate adherence to LLT in patients with coronary heart disease (CHD) in a 12-month follow-up period in Saxony-Anhalt, the state with the highest incidence and mortality for CHD in Germany. Patients and methods Data were taken from 542 hospitalized patients with angiographically documented CHD who were prospectively included in this study conducted in the Department of Medicine III of the University Clinics (Halle). We collected data concerning medication at discharge and after 3 and 12 months. Results A total of 542 patients were included in this study. Mean age was 69.2 ± 11.8 years. In all, 68.8% were males, 165 (30.4%) were smokers, 39.7% suffered from diabetes, and 86.9% had arterial hypertension. The follow-up time of this study was 12 months. At discharge, 463 patients (85.4%) were being treated with a statin. After 3 months 409 (75.5%) and after 12 months, 395 patients (72.9%) were still on statin therapy, respectively. In total treatment, adherence for the statin medication decreased by 15.7% in 12 months. Kaplan-Meier analyses showed that survival, taken as freedom of death from any cause, decreased significantly if statin treatment was stopped (p=0.001). This was confirmed by multivariate Cox regression (HR 1.78, p=0.012). Ezetemibe was prescribed for 56 patients at discharge (10.3%). After 3 months, 40 patients (7.4%) were still taking ezetemibe. After 12 months, adherence to ezetemibe treatment decreased to 4.1% (22 patients). Conclusion During follow-up for 3 and 12 months, adherence for statin therapy decreased by 15.7% and for ezetemibe by 46.6%. Here, low adherence to statin therapy was associated with fatal outcome.

Background: Cardiac rehabilitation (CR) in patients with coronary heart disease (CHD) increases adherence to a healthy lifestyle and to secondary preventive medication. A notable example of such medication is lipid-lowering therapy (LLT). LLT during CR improves quality of life and prognosis, and thus is particularly relevant for patients with diabetes mellitus, which is a major risk factor for CHD. Design: A prospective, multicenter registry study with patients from six rehabilitation centers in Germany. Methods: During CR, 1100 patients with a minimum age of 18 years and CHD documented by coronary angiography were included in a LLT registry. Results: In 369 patients (33.9%), diabetes mellitus was diagnosed. Diabetic patients were older (65.5 ± 9.0 vs. 62.2 ± 10.9 years, p &lt, 0.001) than nondiabetic patients and were more likely to be obese (BMI: 30.2 ± 5.2 kg/m2 vs. 27.8 ± 4.2 kg/m2, p &lt, 0.001). Analysis indicated that diabetic patients were more likely to show LDL cholesterol levels below 55 mg/dL than patients without diabetes at the start of CR (Odds Ratio (OR) 1.9, 95% CI 1.3 to 2.9) until 3 months of follow-up (OR 1.9, 95% CI 1.2 to 2.9). During 12 months of follow-up, overall and LDL cholesterol levels decreased within the first 3 months and remained at the lower level thereafter (p &lt, 0.001), irrespective of prevalent diabetes. At the end of the follow-up period, LDL cholesterol did not differ significantly between patients with or without diabetes mellitus (p = 0.413). Conclusion: Within 3 months after CR, total and LDL cholesterol were significantly reduced, irrespective of prevalent diabetes mellitus. In addition, CHD patients with diabetes responded faster to LTT than nondiabetic patients, suggesting that diabetic patients benefit more from LLT treatment during CR.

Zusammenfassung Hintergrund In Deutschland hat die COVID-19-Pandemie die kardiologische Versorgung sowohl im ambulanten als auch im akutstationaren Bereich unter anderem durch die Absage elektiver Eingriffe erheblich verandert. Um Hinweise darauf zu bekommen, inwieweit dies auch fur Einrichtungen der kardiologischen Rehabilitation gilt, ist diese Untersuchung durchgefuhrt worden. Methode Im August 2020 wurden alle 107 Mitgliedseinrichtungen der DGPR kontaktiert und gebeten, an einer Online-Erhebung teilzunehmen, bei der 12 Fragenkomplexe zum Thema beantwortet werden sollten. Ergebnisse Ende August lagen von 45 Einrichtungen Daten vor. 31,1% der befragten Einrichtungen rehabilitierten Patienten mit kardialen Komplikationen/Manifestationen einer COVID-19 Erkrankung, uberwiegend nach akutem Koronarsyndrom (29,6%) und Lungenarterienembolie (25,9%). Mehr als 40% der Einrichtungen mussten teilweise oder komplett schliesen und 14% befurchteten eine teilweise oder komplette Schliesung zum Ende des Jahres 2020. Kosten fur die Testung bei Verdacht auf SARS-CoV-2-Infektion hatten die Einrichtungen uberwiegend (72,1%) zu tragen. Schlussfolgerung Die vorliegende Untersuchung gibt trotz der Limitationen einer sehr kurzfristigen Datenerhebung und einer Rucklaufquote von ca. 45% Hinweise auf die herausfordernde Situation der COVID-19-Pandemie fur kardiologische Rehabilitationseinrichtungen in Deutschland.

Background: Although cardiovascular rehabilitation (CR) is well accepted in general, CR-attendance and delivery still considerably vary between the European countries. Moreover, clinical and prognostic effects of CR are not well established for a variety of cardiovascular diseases. Methods: The guidelines address all aspects of CR including indications, contents and delivery. By processing the guidelines, every step was externally supervised and moderated by independent members of the “Association of the Scientific Medical Societies in Germany” (AWMF). Four meta-analyses were performed to evaluate the prognostic effect of CR after acute coronary syndrome (ACS), after coronary bypass grafting (CABG), in patients with severe chronic systolic heart failure (HFrEF), and to define the effect of psychological interventions during CR. All other indications for CR-delivery were based on a predefined semi-structured literature search and recommendations were established by a formal consenting process including all medical societies involved in guideline generation. Background In the current guideline of the ESC, in patients with very high cardiovascular risk such as coronary heart disease (CHD) a treatment target for LDL-C Methods The primary objective of this prospective, multi-centre register study was the question of the guidance-based adaptation and adherence to lipid-lowering therapy during and after a cardiac rehabilitation in 1,100 patients with CHD up to 12 months after discharge from the six rehabilitation clinics involved. Patients were included from 2016 to 2018. Results The median age of the 1,100 patients was 63.4±10.4 years, the mean BMI was 28.5±4.7kg/m2, and 24.1% of patients were female. 12.2% were active smokers, 91.6% reported dyslipoproteinemia, 33.9% suffered from diabetes mellitus and 86.5% from hypertension. The majority of patients were included with the main indications NSTEMI (31.6%), STEMI (29.6%) and after CABG surgery (26.4%). The proportion of patients treated with statins was more than 94% when admitted and discharged from the rehabilitation clinic, as well as in 3- and 12-months follow-ups. Approximately 9% of patients were treated with ezetemibe at baseline. On discharge from the rehabilitation clinic 23% of patients were treated with ezetemibe, which remains stable at 3 and 12 months. Objectives: Patient education and compliance play an important role in the success of rehabilitation in cardiovascular diseases. The aim of this study is to analyze whether interactive learning methods, in this study, the audience response system with a “clicker,” can improve the learning success of patients during and after their rehabilitation process. Methods: In a randomized, prospective cohort study, a total of 260 patients were randomized to either an interactive training group using Athens audience response system or to a control group without the use of audience response system during the educational sessions. Patients were taught and tested on four different topics concerning their primary disease: heart failure, arterial hypertension, prevention of cardiovascular diseases, and coronary heart disease. After each session, the patients had to answer questions on the previously taught topics via questionnaires. These questions were asked again at the day of discharge, as well as 3 and 12 months after discharge. Additional information on the patients’ health, plus their mental status, was gathered with the help of further questionnaires (HADS and SF-12). Results: A total of 260 patients (201 men and 59 women) were recruited. The patients were on average 61.1 ± 11 years old. A significant short-term effect on the patients’ knowledge about their disease was found immediately after the educational sessions in the intervention group. However, there was no long-term effect in either the intervention or control group. Although there was no statistical significance found in any of the observations, a positive short-term effect on learning capacity as well as positive trends in mental and physical health after discharge could be found in patients after the use of audience response system during their rehabilitation. Conclusion: This study provides interesting and new data on the use of an interactive learning method for patients to gain knowledge about their primary disease and eventually improve their physical and mental health status in a long-term perspective. By implementing different and new ways of teaching and interaction during the hospitalization, not only patients, but also medical staff and caregivers could benefit.

For life style modifications primary and secondary prevention of acute coronary syndrome (ACS) are approximately similar, even though in the postinfarction situation functional diagnostic programs have to be performed in a rehabilitative manner. All three life style pillars of fitness, nutrition and relaxation implicate prognostic significance and the efficacy is higher for secondary prevention than for primary. The pharmacotherapeutic indications for thrombocyte aggregation inhibition are connected to the presence of atherosclerosis and statin medication is already connected to cardiovascular risk factor stratification, for which scores are used. Depending on the postinfarction myocardial destruction after ACS, additional pharmacotherapies, such as angiotensin-converting enzyme (ACE) inhibitors, angiotensin-II receptor antagonists, beta blockers and also mineral corticoid receptor antagonists are evident. New potential for prevention is ascribed to the new oral anticoagulants (NOAC) in the context of coincidental atrial fibrillation.

Heute differenziert man zwischen morphologischen und funktionellen Phanotypen, wobei jeder Phanotyp in eine familiare und eine nicht-familiare Form unterteilt wird. Je nachdem, welche Form vorliegt, differiert auch die Therapie.

Zusammenfassung Einleitung Körperliche Aktivität gilt als wichtiger, modifizierbarer Risikofaktor der Koronaren Herzkrankheit. In Ergänzung zum aeroben Ausdauertraining wird moderates Krafttraining bei Patienten mit Koronarer Herzkrankheit empfohlen. Unklar ist jedoch, ob und wann mit einem Kraftausdauertraining nach aortokoronarer Bypassoperation begonnen werden kann, um postoperativen Kraftdefiziten entgegenzuwirken. Methodik Der Einschluss der aortokoronar bypassoperierten Patienten in die prospektive Beobachtungsstudie zum Kraftausdauertraining erfolgte von Mai 2017 bis November 2018 in der Paracelsus-Harz-Klinik Bad Suderode. Während der Anschlussheilbehandlung absolvierten die Patienten 3-mal pro Woche eine Bewegungstherapie mit Kraftübungen, welche Dehn- und Scherbewegungen am Sternum sowie eine Thoraxkompression vermieden. Drei Monate nach dem Aufenthalt wurden die Patienten zu Wundheilungsstörungen, anderen Komplikationen und erneuten Klinikaufenthalten befragt. Ergebnisse 92% der Patienten waren männlich, das Durchschnittsalter lag bei 61,7±12,3 Jahren und der mittlere BMI betrug 27,5±3,8 kg/m2. Die Patienten erreichten nach einem Median von 16 (13/19) Tagen postoperativ die Anschlussheilbehandlung. Während der stationären Rehabilitation traten bei 1,5% der Patienten Wundheilungsstörungen auf, die konservativ behandelt wurden. Im Follow-up gaben 8% Wundheilungsstörungen an, darunter 5 Patienten mit Narbeninfektionen und 3 weitere Patienten mit verzögerten Heilprozessen. In 2 Fällen war das Sternum betroffen. Weitere 8% gaben Schmerzen im Operationsgebiet an. Weiterhin waren bei 7,8% der Patienten ein erneuter stationärer Aufenthalt in einer Akutklinik nötig, bei einem Patienten im Rahmen einer Wundheilungsstörung (Erysipel im Bereich der Saphenektomie-Narbe). Schlussfolgerung Diese Beobachtungsstudie zeigt die kurz- und mittelfristige Sicherheit eines selektiven Kraftausdauertrainings in der stationären Rehabilitation, welches früh nach ACB-Operation durchgeführt wird.

However, no prospective data on PE after inpatient rehabilitation have been published so far. Patients and methods: For this monocentric study, 70 patients with PE were prospectively recruited between November 2013 and November 2014 after giving written informed consent. This study was approved by the ethics committee of the Medical Association of Saxony-Anhalt. Inclusion criteria were as follows: age ≥18years and a stay at the Paracelsus-Harz Clinic in Bad Suderode, Germany, with the main indication of PE. During the hospital stay, history-relevant medical data and diagnostic findings were collected and documented. Furthermore, we recorded whether patients were rehospitalized or died during the treatment period in the rehabilitation clinic or during the 12-month follow-up. Results: The mean age was 64.5 ± 13.0years, the mean body mass index (BMI) was 30.4 ± 6.0kg/m2, and 54.3% were women. During rehabilitation, two patients (3.9%) were transferred to a primary care hospital; no patient died. However, four patients died (5.7%) in the 12-month follow-up period. A total of 20 patients were hospitalized in the 12-month follow-up period (hospitalization rate during the 12-month follow-up period: 28.6%). Of these 20 patients, one patient was rehospitalized with a newly diagnosed PE (1.4%) and two patients were rehospitalized for bleeding events (2.8%). Conclusion: PE is a life-threatening disease, and therefore it seems reasonable to recommend rehabilitation at least in patients with an intermediate- or high-risk PE. In this study, death and other serious event rates were low during the in-hospital rehabilitation and in the 12-month follow-up period, which underlined the safety and importance of a standardized rehabilitation program after survived PE. Background Combination of oral anticoagulation (OAC) and antiplatelets is used in atrial fibrillation (AF) patients undergoing percutaneous coronary intervention and stent (PCI-S) procedure but is associated with increased bleeding when triple antithrombotic therapy (TAT) is used. Our aim was to analyze the impact of time in therapeutic range (TTR) on outcomes, in patients prescribed with TAT. Methods Ancillary analysis from the AFCAS registry in patients assigned to TAT. TTR was calculated with Rosendaal method. Outcomes were analyzed according to TTR tertiles (T1 [≤56.8%] vs. T2 [56.9–93.8%] vs. T3 [≥93.9%]). Major bleeding was the primary outcome. Results Of 963 patients enrolled, 470(48.8%) were prescribed with TAT at discharge and qualified for this analysis. Median [IQR] TTR was 80.0% [45.3–100%]. After 359 [341–370] days, major bleeding rates were progressively lower with increasing TTR tertiles (T1 vs. T2 vs. T3: 10.3% vs. 4.7% vs. 2.3%, P = .006). Kaplan–Meier analysis demonstrated a progressively lower risk for major bleeding across tertiles (P = .006). Patients in the highest TTR tertile had a non-significant lower risk for major adverse coronary and cerebrovascular events (MACCE) (log-rank: 4.905, P = .086). Cox regression analysis showed that T2 and T3 were inversely associated with major bleeding (hazard ratio [HR]:0.39, P = .050 and HR: 0.21, P = .005). Heart failure patients (HF-rEF, HF-pEF or in combination) represent a relevant proportion of patients in cardiological rehabilitation facilities. They require a multimodal therapy concept due to the impaired systolic and/or diastolic ventricular function, the risk of malignant arrhythmias and the cardiac implantable electrical devices (CIED). In addition to guideline-based pharmacotherapy, individualized training therapy and psychosocial support play a key role in the rehabilitation of heart failure patients. Professional reintegration is determined by hemodynamic stability, cognitive performance, job requirements and job security, especially for CIED carriers. The suitability for driving is mainly determined in individual decisions and depends, among other things, on the left ventricular ejection fraction, the hemodynamic stability and the arrhythmia-free interval.
