Background: Day hospitals in child and adolescent mental health services are held to be helpful entities in the psychiatric care of young people. However, limited research has been done to look into how day hospitals and similar settings affect referrals, inpatient stays and in which cohort of patients do they show the most impact. This study's primary aims were to examine the impact of a new Child and Adolescent Mental Health Service (CAMHS) day hospital on referrals and admissions to the associated CAMHS in‐patient unit. A secondary aim of this study was to examine the demographics and clinical outcomes of the young people attending the day hospital. Methods: Information was gathered using retrospective reviews of clinical files and databases. Results: There was a reduction in admissions and in the total bed days from the study area following the opening of the day hospital. There was an improvement in clinical outcomes for young people attending the day hospital. Conclusion: Day hospital in child and adolescent mental health services can prove to be a useful modality to provide support to young people with a moderate to severe mental illness leading to reduced inpatient admissions and improving lengths of inpatient stay. Phytotherapeutic products in the field of psychiatry are rarely used. However, a significant number of patients take aromatherapy and phytotherapy treatments without informing their nursing staff, and sometimes with very little information from specialists. In this article, we'll look at how, within a mental health day hospital, we set out to explore and implement actions adapted to this particular context. A questionnaire was first distributed to 70 patients to assess their use of both plants and essential oils, and to identify their own needs and their own expectations. A clear demand for information emerged from this survey, which led to the writing of a "Guide to the use of essential oils and plants for relaxation and sleep". Then we set up aromatherapy workshops so that those who volunteered could prepare dilutions of essential oils for anxiolytic, hypnotic or anti-depressive purposes. Background: Schema therapy is effective for most outpatients with personality disorders (PDs). However, a subgroup does not sufficiently benefit from outpatient programmes. Despite its common clinical use, a thorough evaluation of day treatment group schema therapy (GST) is lacking. Aims: This study aimed to investigate the effectiveness of day treatment GST for patients with PDs. Methods and Procedures: Negative core beliefs were the primary outcome in a multiple baseline single‐case design, measured weekly before and during 30 weeks of day treatment GST. Secondary outcomes included severity of primary PD, early maladaptive schemas (EMS), schema modes and general psychopathology measured before and after day treatment GST. Intervention effects were evaluated through visual inspection and randomization test analysis, with a reliable change index calculated for the secondary outcome measures. Outcomes and Results: A total of 79% of treatment completers showed a significant positive effect of day treatment GST with large effect sizes (Cohen's d: 0.96–10.04). Secondary outcomes supported these findings: 56% had a significant decrease in the severity of primary PD and 53% in general psychopathology. In addition, 63% of EMS and 72% of schema modes (87.5% for functional schema modes) showed significant positive reliable changes. Conclusions and Implications: This is the first empirical study that demonstrated the effectiveness of day treatment GST in patients with severe PDs. Day treatment GST can serve as a stepped care treatment option for nonresponsive patients in outpatient programmes. Further randomized controlled (cost‐)effectiveness research is necessary to substantiate these findings and investigate the specific patient populations for which day treatment is essential. Methods: A descriptive exploratory qualitative study was carried out. Semi‐structured interviews were conducted with people attending a mental health day hospital. The data were analysed deductively by means of content analysis. Results: The participants described the recovery process as a process based on three pillars; the attitude towards recovery, hardship, and the effort required throughout the process. Regarding the stages of recovery, for the participants in the first stage of the process (Moratorium), the search for hope was the most important element. In the second stage (Awareness), the reestablishment of their identity, through the acceptance of the consequences derived from the mental health problem, together with being able to feel full and fulfilled, were the most outstanding elements. In the third stage (Preparation), participants highlighted the search for meaning in life, facing their fears and the process with an open mind. Finally, the last two stages (Rebuilding and Growth) were related to taking responsibility and empowerment for recovery. Conclusions: The results of this study provide insight into the perception of the recovery process and its stages in people attending a mental health day hospital. These findings may contribute to aligning the nurse–patient perspective, helping nurses to understand the key elements of patients according to their stage of recovery, and thus be able to subsequently individualise interventions. Background: In Germany, hospice and palliative care is well covered through inpatient, outpatient, and home-based care services. It is unknown if, and to what extent, there is a need for additional day care services to meet the specific needs of patients and caregivers. Methods: Two day hospices and two palliative day care clinics were selected. In the first step, two managers from each facility (n = 8) were interviewed by telephone, using a semi-structured interview guide. In the second step, four focus groups were conducted, each with three to seven representatives of hospice and palliative care from the facilities' hospice and palliative care networks. Interviews and focus groups were audio recorded, transcribed verbatim and analyzed using qualitative content analysis. Results: The interviewed experts perceived day care services as providing additional patient and caregiver benefits. Specifically, the services were perceived to meet patient needs for social interaction and bundled treatments, especially for patients who did not fit into inpatient settings (due to, e.g., their young age or a lack of desire for inpatient admission). The services were also perceived to meet caregiver needs for support, providing short-term relief for the home care situation. Conclusions: The results suggest that inpatient, outpatient, and home-based hospice and palliative care services do not meet the palliative care needs of all patients. Although the population that is most likely to benefit from day care services is assumed to be relatively small, such services may meet the needs of certain patient groups more effectively than other forms of care. Research has shown so far the efficacy of day hospital treatment programs for patients with a personality disorder experiencing a crisis episode. However, relatively few studies have relied on a dimensional definition of personality pathology, despite the rise of dimensional models of personality disorders in recent years. The objective of this study is therefore to delimit profiles of patients with a personality disorder admitted in a day hospital treatment program using dimensional models of personality and attachment, and psychological distress. A total of 287 patients with a personality disorder completed a series of self-report measures assessing the Big Five personality traits, the Dark Triad components, insecure attachment dimensions, and psychological distress. Cluster analysis, successively using hierarchical and TwoStep algorithms, yielded a three-cluster solution. The three clusters were labeled: Externalizing-Antagonistic, Internalizing-Detached, and Relational and Emotional Difficulties. Each cluster showed a specific pattern of scores on Big Five personality traits, Dark Triad components, and insecure attachment dimensions, and all clusters were associated with clinically significant levels of psychological distress. Results highlight that features from the externalizing, internalizing, and/or detachment spectra of psychopathology distinguish clusters of PD patients admitted in a day hospital treatment program. These clusters are aligned with descriptions found in the recently proposed Hierarchical Taxonomy of Psychopathology. A continuum of severity in personality impairments across the three clusters was also discovered. The clinical and conceptual implications of the three-cluster solution are discussed. Background: Premedication of cancer therapy against nausea and vomiting (NV) and hypersensitivity reaction (HS) is essential for good patient management. However, this prescription is not always optimal. Today, as a large part of cancer therapies are administered in day hospitals (DH), premedication taken on the day of the cancer treatment is injected as a 30-min infusion. Objective: To assess compliance with recommendations for premedication prescription and intake; to analyse patient attitude about switching to exclusively oral forms taken at home. Method: The study is conducted in the medical oncology DH of a French Hospital from 17 January to 25 February 2022. The data collection is carried out as an individual interview, associated with the distribution of two questionnaires. Data are coupled with the premedication set up on our software and the last medical report. Intakes are considered optimal when recommendations, tolerance, background, and adherence of the patient are taken into account. Results: Seventy patients were included for interviews. Regarding software prescriptions, our configuration was consistent with recommendations in 100% of cases for HS and 37% for NV. Intakes were compliant in 51.4% of cases, non-compliant in 17.1% and debatable in 31.5%. Disparities between the practices of different physicians were identified. Regarding the feasibility of oral substitution, it could concern 63.5% of patients. • Measurement-based care is a promising adjunct to any psychological treatment. • Measurement-based care usually relies on standardized outcome measures. • Individualized outcome measures can capture complex change in high acuity settings. • The Youth Top Problems captures unique clinical information in acute settings. • Top problems were heterogeneous and sensitive to clinical change. Measurement-based care (MBC), or the regular use of progress measures to inform clinical decision-making, improves quality of care and clinical outcomes. MBC typically focuses on standardized rather than individualized outcome measurement. In this pilot study, we examined the clinical utility of integrating individualized measurement with existing standardized outcome monitoring in a children's partial hospitalization program. Participants were 48 youth (M age 10.13 ± 1.39; 54.2% male, 41.7% female, 4.2% transgender or nonbinary). Comorbidity was common; 83.4% of youth had more than one diagnosis at intake. Using the Youth Top Problems for individualized outcome measurement, we examined Top Problem content and clinical improvement over time. Finally, we examined completion rates and describe implementation issues. Top Problems were heterogeneous and sensitive to change. Of the 144 problems, 107 (74%) had a focus consistent with measures used in program, while 37 (26%) were not captured by standardized measures used in program. Effect sizes from admission to final measurement ranged from Cohen's d =.75 – 1.00. Initial adoption of the MBC was strong, but sustained use of the system over the treatment course was challenging. Individualized outcome measurement in children's partial hospitalization programs is feasible to administer and sensitive to clinical change that is unique from change captured in standardized measures. Parents were able to self-identify clinically meaningful, highly individualized Top Problems. Primary health care in modern socio-economic conditions is given special attention. This type of medical care makes up a large part of the volume of the entire healthcare system in our country, but the least expensive compared to inpatient care. Practically up to 80 % of all available domestic healthcare resources are annually distributed to provide medical care to citizens in stationary conditions. While in modern conditions it is possible to use the available resources more economically and more efficiently. The introduction of hospital-replacing technologies into the practice of medical organizations, both outpatient and inpatient types, is an undoubted advantage of the development of domestic healthcare. During the COVID-19 pandemic, when most of the hospitals are being converted into infectious diseases hospitals, day hospital treatment has become of paramount importance as the main form of hospital-replacing technologies. At the same time, the day hospital is an intermediate link between the outpatient and inpatient networks. Day hospitals in our country appeared relatively recently, but they have received significant development from the standpoint of the greatest medical, social and economic efficiency of treating patients compared to round-the-clock hospitals under the same conditions. During the pandemic, this circumstance was undeniably proven.

Background: Cognitive behavioral therapy (CBT) and pharmacotherapy with antidepressants are both a highly effective treatment for agoraphobia and/or panic disorder; however, a combination of CBT and antidepressants is under debate due to potentially unfavorable interference effects. The associations of existing antidepressant medication with panic and agoraphobia symptom burden and their change in the context of a structured 5‑week day hospital and exposure-focused treatment in a naturalistic setting were investigated. Methods: Out of a total of n = 488 patients medication use during treatment was retrospectively determined for n = 380: n = 100 (26.3%) were taking antidepressants of different drug classes. Calculations were performed using multiple linear regression analysis, t‑tests, response analyses, and χ2-tests. Results: Patients with existing antidepressant medication more often met the criteria for comorbid depressive disorder (p < 0.001). The measure of symptom change and treatment response rates did not differ between patients with and without antidepressants with respect to anxiety symptoms. Discussion: In the context studied, patients with and without existing antidepressant medication benefited equally from CBT with respect to anxiety symptoms. [ABSTRACT FROM AUTHOR]

Schizophrenia is associated with cognitive deficits that may compromise functional capacity. Although there are few studies with validated instruments in Brazil, the literature indicates changes in memory, attention, visuoconstruction and executive functions. Thus, this study aims to characterize these functions in this group, besides verifying the association between cognitive performance and sociodemographic, diagnostic and therapeutic characteristics. This is a cross-sectional study with 62 individuals, who were submitted to the Rey Verbal Auditory Learning Test (RAVLT), Rey-Osterrieth Complex Figures (FCRO), WAIS-III Digits and Cubes subtests, and the Colored Trails Test (TTC). The association between the presence of deficit and the variables was verified by chi-square or fisher's exact test. It was found that 50% of the subjects presented below average performance in Cubes and 19.4% in Digits. Therefore, 85.48% had a lower than expected z-score in the RAVLT learning index, 30.7% low recognition rate, 51.6% suffered a strong influence of retroactive interference, 32.3% had a broad impact of proactive interference and a high rate of forgetfulness. In the TTC, most participants were in the lower range in form 1 (54.8%) and 43.6% had below average yields in form 2. In the case of correlations with the variables, a borderline or statistically significant influence was observed between schooling, the Digits test and the 5 factors analyzed by the RAVLT, associations between the time of diagnosis and learning capacity, correlations between the age of diagnosis and the retroactive interference index, as well as the influence of the type of medication on the proactive interference index. The results indicate a notorious occurrence of mnemonic, attentive and visuoconstructive difficulties in this population. Additionally, shorter schooling, longer disease time and earlier age of diagnosis seem to cause worse performances in working memory, attention and processes adjacent to learning. ObjectiveEpidemiological data support that sexual minorities (SM) report higher levels of eating pathology. Theories suggest these disparities exist due to stressors specific to belonging to a minority group; however, few studies have specifically explored differences between SM and heterosexual individuals in clinical eating disorder samples. Thus, the present study compared SM and heterosexual patients with eating disorders on demographic characteristics and eating disorder and psychological outcomes during day hospital treatment.MethodPatients (N = 389) completed surveys of eating pathology, mood, anxiety, and skills use at treatment admission, 1-month post-admission, discharge, and 6-month follow-up. Overall, 19.8% of patients (n = 79) identified as SM, while 8.0% (n = 32) reported not identifying with any sexual orientation. SM were more likely to present across genders (17.7% of females, 24.2% of males, 33.3% of transgender patients, and 87.5% of nonbinary patients).ResultsSM patients were significantly more likely to endorse major depressive disorder, panic disorder, and self-harm at admission than their heterosexual counterparts. Multilevel models demonstrated that across time, SM patients demonstrated greater eating pathology, emotion dysregulation, depressive symptoms, and anxiety symptoms. Significant interactions between sexual orientation and time were found for eating pathology and emotion dysregulation, such that although SM patients started treatment with higher scores, they improved at a faster rate compared to heterosexual patients.DiscussionConsistent with minority stress theory, SM patients report greater overall eating disorder and comorbid symptoms. Importantly, results do not support that there appear to be significant disparities in treatment outcome for SM patients in this sample of day hospital patients.

Background: In recent years, a new recovery model has gained ground in which recovery is understood as a process of change where individuals are able to improve their health and wellbeing, lead self-sufficient lives and strive to achieve their maximum potential (personal recovery). Despite the existence of data regarding the effectiveness of mental health day hospitals (MHDHs) in reducing relapses in terms of hospital admissions and emergencies, no studies have to date assessed how this change affected the new personal recovery model. Objectives: To verify the effectiveness of MHDHs in improving personal recovery processes among people with mental disorders (MDs). Methods: A prospective cohort study. A group of patients receiving follow-up at MHDHs was compared with another group of patients receiving follow-up in other therapeutic units over a period of three months. Results: Patient recovery at the MHDHs, assessed using the Individual Recovery Outcomes Counter (I.ROC), was found to be significantly better than that of patients attended in other units. Conclusions: MHDHs can contribute to the recovery of people with MDs. Abstract Background Previous studies about Quality of Life (QoL) in autistic children (ASD) have put forward the negative impact of factors such as Autism Spectrum Disorder (ASD) severity, psychiatric comorbidities and adaptive behaviour impairment. However, little is known about the relation of these factors to school adjustment, measured with the International Classification of Functions disability and health (ICF) framework (World Health Organization, 2001), and QoL evolutions. Thus, this study aimed at investigating the determinants of behaviours, school adjustment and QoL changes in 32 children in an ASD inclusion program over one academic year. Methods Using Bayesian methods, we studied the impact of ASD severity, psychiatric comorbidities, adaptive behaviour level and a diagnosis of Pathological Demand Avoidance (PDA) on evolutions of behaviour, school adjustment (measured with the ICF) and QoL. Results As predicted, adequate adaptive behaviour levels were associated with better progress of behaviours and school adjustment whereas psychiatric comorbidities were related to worse outcome of school adjustment. Contrary to our hypotheses, severe ASD was associated to better evolution of adjustment at school. PDA was not discriminant. We did not find any association between the studied factors and the evolution of QoL over the academic year. Conclusion Our results show that the assessment of adaptive behaviour levels, psychiatric comorbidities and ASD severity level may be useful predictors to discriminate of school adjustment evolution (assessed by teachers within the ICF model) over a one-year period in autistic children. The assessment of this time course of school adjustment was sensitive to change and adapted to differentiate evolutions in an inclusive education framework. The investigation of quality of school life of autistic children as well as its determinants may therefore be relevant to improving academic adaptation. However, further research in larger groups, over longer periods and in different personalized school settings for autistic children is needed.

Background. The quality and availability of medical care to the population directly depends on the organization of the treatment and diagnostic process, which is directly related to the material and technical resources of medical organizations. Aims. Assess how the level of performance indicators of medical organizations of the dermatovenereological profile has changed after the restructuring of their material and technical resources for the period 20102020 in the Russian Federation as a whole. Materials and methods. The study is based on a retrospective analysis of the main intensive and extensive indicators that evaluate the work of medical organizations of the dermatovenereological profile. Results. Structural transformations of material and technical resources, which are under the jurisdiction of the dermatovenereological service, gave the following results in the whole of the Russian Federation. The activities of medical organizations intensified, which naturally led to a positive economic effect. The reduction of the excess number of resource-intensive beds in round-the-clock hospitals and the expansion of day hospitals due to the redistribution of part of the round-the-clock beds into beds and beds in day hospitals, increasing their capacity, did not negatively affect the volume of specialized dermatovenereological medical care for the population, they not only did not decrease, but even slightly increased. Conclusion. Thus, the study showed that the restructuring of the material and technical resources of the dermatovenereological service in the Russian Federation as a whole led to a positive effect of the use of material and technical resources. However, it should be noted that their further reduction is inappropriate, as it can have the opposite effect. For example, a further reduction in the ATC may adversely affect the provision of the population with human resources, which will affect the availability and quality of medical specialized dermatovenereological care.

Este artículo tiene como objetivo aportar evidencia sobre la efectividad de un Hospital de Día de Adultos en el tratamiento de personas con trastorno mental. Se trata de un estudio intragrupo pre-post de todas las personas (n = 235) ingresadas desde el 2008 al 2018 incluido. Se recogieron datos sobre variables demográficas e indicadores de resultado. Los resultados muestran una reducción del número y duración de ingresos psiquiátricos que se mantiene a lo largo del tiempo, una disminución de las urgencias en los primeros meses postalta y la recuperación de la capacidad laboral. Se aporta evidencia que apoya un dispositivo de tratamiento útil y eficaz, intensivo y prolongado en el tiempo para grupos de pacientes en situaciones vitales distintas, con diagnósticos y perfiles demográficos diversos, organizado en un ambiente terapéutico e implementado por un equipo multiprofesional que se adapta a las demandas de atención a la salud mental de la población.

Abstract Aim In Japan, day hospitals are a method of rehabilitation for psychiatric disorders, especially for recovering social functioning. Currently, 60% of day hospital users have schizophrenia, the majority of whom are long‐term users (for over 1 year). However, they show no progress in community participation. This study aimed to investigate whether people with schizophrenia who use day hospitals can improve their social functioning and recovery levels, and alleviate psychiatric symptoms, when they engage in self‐determination regularly. Methods This study employed a multicenter randomized controlled design with a 3‐month intervention. Participants were divided into two groups. The intervention group (n = 24) determined their daily goals in life and attended the day hospitals' programs, while the control group (n = 25) only participated in the day hospitals' programs. Participants' social functioning was assessed using the Global Assessment of Functioning (GAF) scale, psychiatric symptoms using the Positive and Negative Syndrome Scale (PANSS), and recovery using the Recovery Assessment Scale (RAS), before and after the intervention. Results In the intergroup factors, there were no significant differences in GAF, PANSS, or RAS scores. Conclusion The results suggest that participants' regular self‐determination of their daily goals did not lead to significantly positive behavior in improving their social functioning. However, the social functioning and psychiatric symptoms of participants in day hospitals improved after 3 months.

Sensorineural hearing loss (SNHL) is a form of hearing loss, which today has medical, social and economic significance. According to the World Health Organization (WHO), today there is a rapid increase in the number of patients suffering from SNHL. As it is known, the success in the treatment of such patients depends on the beginning of the treatment time. Accordingly, it is very important to organize the medical care efficiently and in time, but it is not always possible to ensure this. For this, it is necessary to improve the otorhinolaryngological service and the health care system. Purpose. Based on the analysis of literature sources, consider the issues of the incidence of SNHL and present the main stages and problems of organizing medical care for patients with sensorineural hearing loss. Materials and methods. A search and analysis of scientific data on the research topic was carried out. The collection was carried out in Russian and foreign electronic data systems: eLIBRARY.ru, PubMed, cyberleninka.ru, Google Scholar, glav-otolar.ru, who.int/ru, rosstat.gov.ru. Conclusion. The data we found in the course of studying the scientific literature indicate the growing interest in this problem. Sensorineural hearing loss is a complex disease, due to the variety of possible etiological factors and the absence of a single etiopathogenesis. The procedure for organizing medical care for patients with SNHL at each stage of its provision is regulated by regulatory and legal acts. The study of morbidity, assessment of the quality of medical care makes it possible to determine the ways to improve the provision of medical care to patients suffering from SNHL.

There are no studies of the safety and effectiveness of telehealth psychiatric treatment of partial hospital level of care, in general, and for borderline personality disorder (BPD) in particular. In the present report from the Rhode Island Methods to Improve Diagnostic Assessment and Services (MIDAS) project, the authors compared the effectiveness of their partial hospital treatment program in treating patients with BPD. For both the in-person and telehealth partial hospital level of care, patients with BPD were highly satisfied with treatment and reported a significant reduction in symptoms from admission to discharge. Both groups reported a significant improvement in functioning, coping ability, positive mental health, and general well-being. A large effect size of treatment was found in both treatment groups. No patients attempted suicide. Telehealth partial hospital treatment was as effective as in-person treatment in terms of patient satisfaction, symptom reduction, and improved functioning and well-being for patients with BPD. Background: Previous studies about Quality of Life (QoL) in autistic children (ASD) have put forward the negative impact of factors such as Autism Spectrum Disorder (ASD) severity, psychiatric comorbidities and adaptive behaviour impairment. However, little is known about the relation of these factors to school adjustment, measured with the International Classification of Functions disability and health (ICF) framework (World Health Organization, 2001), and QoL evolutions. Thus, this study aimed at investigating the determinants of behaviours, school adjustment and QoL changes in 32 children in an ASD inclusion program over one academic year. Methods: Using Bayesian methods, we studied the impact of ASD severity, psychiatric comorbidities, adaptive behaviour level and a diagnosis of Pathological Demand Avoidance (PDA) on evolutions of behaviour, school adjustment (measured with the ICF) and QoL. Results: As predicted, adequate adaptive behaviour levels were associated with better progress of behaviours and school adjustment whereas psychiatric comorbidities were related to worse outcome of school adjustment. Contrary to our hypotheses, severe ASD was associated to better evolution of adjustment at school. PDA was not discriminant. We did not find any association between the studied factors and the evolution of QoL over the academic year. Conclusion: Our results show that the assessment of adaptive behaviour levels, psychiatric comorbidities and ASD severity level may be useful predictors to discriminate of school adjustment evolution (assessed by teachers within the ICF model) over a one-year period in autistic children. The assessment of this time course of school adjustment was sensitive to change and adapted to differentiate evolutions in an inclusive education framework. The investigation of quality of school life of autistic children as well as its determinants may therefore be relevant to improving academic adaptation. DSM-5 dimensional-hybrid model and PID-5 Personality Inventory might be particularly useful in a day hospital treatment program for personality disorders, while the need for treatment for mixed personality disorder and unspecified personality disorder is such as frequent as for borderline personality disorder. Effective treatment programs in the public health system are required. The study shows the results from the day hospital treatment program for personality disorders implemented at San Juan Hospital in Alicante. The treatment involves an intensive and interdisciplinary program that applies the DSM-5 dimensional model and the PID-5 personality inventory to identify therapeutic needs, with both individual and group therapies with a cognitive-behavioural therapeutic approach. Results support the relevance of therapeutic components regarding this program. Moreover, its effectiveness has been demonstrated in achieving significant changes in symptoms, traits, comorbidity, and psychosocial functioning. It also appears to be efficient not only in reducing the number of visits to the emergency and hospital admissions, but also in ensuring continuity and therapeutic compliance, thus reducing public health costs.

In the last three decades surgeons have begun to perform outpatient thyroid surgery (OTS). Important outcome measures of a day-hospital procedure are the patient's quality of life (QoL) and satisfaction, but information on these issues in the OTS setting is scanty. The aim of this pilot study was to explore how early discharge after thyroidectomy affects patients' QoL and satisfaction. Postoperative QoL and satisfaction were assessed retrospectively by giving each patient a self-report questionnaire specifically created in our center for OTS and derived from the post-discharge surgical recovery (PSR) scale to assess physical and mental well-being. Twenty-three of 24 patients (96%), 16 women and 7 men with a median age of 48 years (range 16–72), completed the questionnaire, answering 92% of the questions. QoL based on this scale gave a median score of 81.8% (range 62–98.8%). No major or minor complications occurred in the study group. Regarding QoL eight patients (35%) reported feeling "tired all the time" and six patients (26%) reported mild pain, which in two cases resolved spontaneously. Regarding patient satisfaction two-thirds of patients judged OTS positively while the remaining one-third would not recommend it. Our study showed very good uptake by patients of a new questionnaire dedicated to OTS as a possible aid in the identification of areas for improvement of OTS management. However, to be considered a safe procedure with maximum patient compliance and satisfaction, OTS was found to require considerable effort by hospital staff and patients' caregivers compared to inpatient thyroid surgery. Objective: To assess outcomes in adult patients with eating disorders administered enhanced cognitive behavioral therapy (CBT‐E) in a day‐hospital setting. Method: Forty‐three consecutive patients with eating disorders (86% females; mean age 28.8 [SD = 11.8]) were admitted to a 13‐week day‐hospital CBT‐E program. Twenty‐five (58.1%) patients were underweight (i.e., body mass index [BMI] <18.5 kg/m2; BMI 15.4 [SD = 2.2]) and 18 not (mean BMI 23.1 [SD = 6.3]) at baseline. All patients responded poorly to prior outpatient treatment. Body mass index and Eating Disorder Examination Questionnaire (EDE‐Q), Brief Symptom Inventory (BSI), and Clinical Impairment Assessment (CIA) scores were recorded at baseline, end of treatment, and 20‐week follow‐up. Results: In 37 patients (86.0%) who completed the treatment, there were substantial improvements in eating‐disorder features, general psychopathology, clinical impairment, and body weight (only in underweight patients), which were well maintained at follow‐up. L'allergie oculaire est une pathologie fréquente pouvant engendrer des complications, notamment cornéennes, sévères, et parfois pourvoyeuse de cécité. Une activité multidisciplinaire d'allergo-dermato-ophtalmologie a été initiée en novembre 2016 au CHU de Saint-Étienne, sous forme d'un hôpital de jour dans le service de dermatologie. Celui-ci est assuré par trois médecins spécialistes allergologue, ophtalmologue et dermatologue, assistés par une infirmière. L'analyse rétrospective de cette activité a été réalisée à 2 ans et demi. Un total de 296 hospitalisations de jour ont été réalisées chez 196 patients. La majorité des patients (158, soit 80,6 %) présentaient une pathologie oculaire allergique. 35 patients présentaient une conjonctivite allergique saisonnière (22,1 %), 50 patients étaient atteints d'une conjonctivite perannuelle (31,6 %), 36 patients avaient une kératoconjonctivite vernale (22,8 %) et 9 patients souffraient d'une kératoconjonctivite atopique (5,7 %). Un total de 114 patients (58,1 %) ont eu un bilan allergologique positif. Les principaux allergènes retrouvés étaient les graminées, les acariens, les phanères d'animaux, les herbacées et les arbres. Le traitement proposé allait du simple collyre antihistaminique aux immunosuppresseurs systémiques, en passant par les corticoïdes et les immununosuppresseurs locaux. Vingt-cinq patients (12,8 %) ont pu débuter un traitement de désensibilisation. La collaboration de trois spécialités, que sont l'allergologie, la dermatologie et l'ophtalmologie, au sein d'un hôpital de jour est inédite en France et permet une prise en charge globale des patients allergiques. Cette activité permet de contribuer à l'amélioration de la qualité de vie de ces patients et à une meilleure organisation de cette filière de soins. Ocular allergy is a common pathology that can lead to severe complications, including corneal, and sometimes blindness. A multidisciplinary activity of allergo-dermato-ophthalmology has been initiated in November 2016 at the University Hospital of Saint-Etienne, in the form of a day hospital in the department of dermatology and external ophthalmology consultations. The day hospital is provided by three specialist doctors: an allergist, an ophthalmologist and a dermatologist, assisted by a nurse. The retrospective analysis of this activity was carried out after 2.5 years. A total of 296 day hospitalizations were performed for 196 patients. The majority of patients (158, or 80.6 %) involved an allergic ocular pathology. 35 patients had seasonal allergic conjunctivitis (22.1 %), 50 patients had perennial conjunctivitis (31.6 %), 36 patients had vernal keratoconjunctivitis (22.8 %) and 9 patients had atopic keratoconjunctivitis (5.7 %). Background: In recent years, a new recovery model has gained ground in which recovery is understood as a process of change where individuals are able to improve their health and wellbeing, lead self-sufficient lives and strive to achieve their maximum potential (personal recovery). Despite the existence of data regarding the effectiveness of mental health day hospitals (MHDHs) in reducing relapses in terms of hospital admissions and emergencies, no studies have to date assessed how this change affected the new personal recovery model. Objectives: To verify the effectiveness of MHDHs in improving personal recovery processes among people with mental disorders (MDs). Methods: A prospective cohort study. A group of patients receiving follow-up at MHDHs was compared with another group of patients receiving follow-up in other therapeutic units over a period of three months. Results: Patient recovery at the MHDHs, assessed using the Individual Recovery Outcomes Counter (I.ROC), was found to be significantly better than that of patients attended in other units. Conclusions: MHDHs can contribute to the recovery of people with MDs. This is particularly important at a time when some patients may have experienced impediments to their recovery processes due to the pandemic.

Background. In the context of the ongoing COVID-19 pandemic, the vector of healthcare development is aimed at minimizing contacts among citizens, which is especially important for cancer patients, given the immunosuppression caused by cytostatics.The objective of the review is to present the development prospects of hospital-substituting technologies for drug antitumor treatment. Two main components are considered that allow effective and safe infusion of anticancer drugs in a day hospital: central venous access devices and infusion pumps, various types of these devices, their safety and experience of use today are described.Materials and methods. We searched for available literature published in PubMed, Medline, eLIBRARY, Cochrane Library, CyberLeninka, Global.health, etc. 42 sources were found and analyzed, published from 2002 to 2021.Results. Outpatient anticancer drug treatment is a real alternative to hospital stay, based on many years of world experience. The qualifications level of medical personnel involved in the treatment process should allow early monitoring of various adverse events development. With the ongoing COVID-19 pandemic, it is important to ensure the continuity of the treatment process while minimizing risks to the patient.Conclusion. With a careful approach and adequate infrastructure availability, drug treatment in a day hospital can reduce the burden on round-the-clock stay hospital and positively affect the patients’ quality of life and their compliance with treatment.

Psychiatric disorders are common and can cause psychological disabilities. While the creation of day hospitals (DHs) was intended to direct psychiatric care towards community settings, they may have paradoxically contributed to a form of chronicity. Furthermore, the heterogeneity and lack of evaluation of care within DHs prevent the availability needed to collect objective data on users outcomes. In this article, we aim to describe and measure the effects of a transformation of practice within a sector-based DH initially focused on traditional institutional psychiatry towards a rehabilitation model of care which offers different therapeutic tools, structured in three stages, and whose main objective is professional integration. This retrospective mirror study compares, before and after the transformation of this DH, several indicators including the rate of professional integration and its maintenance after two years. We found that this psychosocial rehabilitation model for care allowed a very clear increase in the professional integration rate and its maintenance at two years while reducing the length of stay to around 18 months. Objectives: The aim of this study was to describe the associations between sarcopenia and variables derived from wrist accelerometry in community-dwelling older adults attending a day hospital service in Ireland. Methods: An observational cross-sectional study was carried out using a consecutive series of older adults attending a day hospital service. Sarcopenia was diagnosed using the latest European Working Group of Sarcopenia in Older People guidelines. Accelerometers were worn by each participant for a 7-day period on their non-dominant wrist. Results: Thirty-eight out of forty-one participants (93%) met the accelerometer wear time criterion and were included in statistical analyses. Included participants had a mean age of 81.1 years (standard deviation 6.2). Both sarcopenia (Grip) and sarcopenia (Lower limb) were associated with increased time spent in low physical activity and reduced average of Kcals per hour. Only sarcopenia (Lower limb) was associated with increased time in sedentary behaviour as well as reduced number of steps taken in a week. Conclusions: Accelerometer data can be used in an older day hospital population to track physical activity levels and sedentary behaviours. The assessment tool used to assess muscle strength and the cut-off criteria for physical activity behaviour influences the association with sarcopenia. [ABSTRACT FROM AUTHOR]

Objective: Aim of the pilot study was to evaluate the effectiveness of a day hospital program for eating disorders (EDs) after implementation of mentalization‐based treatment (MBT), including a comparison to a historical matched control group. Method: All consecutively admitted patients with an ED were included in a prospective, observational study over a period of 2 years. Main outcome criteria were eating and overall psychopathology. Furthermore, changes in the capacity to mentalize (reflective functioning [RF]), difficulties with affect regulation and interpersonal problems were assessed at admission, discharge and a three months follow‐up. Results: Thirty‐eight out of fourty patients could be included. The program led to significant reductions in eating and general pathology. There was significant improvement in RF, difficulties in emotion regulation and interpersonal functioning. However, there was no advantage in comparison to a matched control group (main outcomes). ED symptoms showed a slight renewed increase after discharge, while body mass index further increased. RF change was predictive of change in overall psychopathology, but not ED symptoms. Drop‐out rate was 13.2%. Conclusion: The program showed no advantage in comparison to a historical control group concerning a reduction in eating and overall psychopathology. However, it was associated with low drop‐out rates, and changes which MBT targets: An increase in RF and a reduction of interpersonal problems as well as difficulties in emotion regulation. The mean age was 81.7 (SD ± 7.1). Sixty-one percent (N = 82) were female. The prevalence of sarcopenia ranged from 27 to 37% depending on the assessment tool used to assess muscle strength. Sarcopenia was associated with older age, frailty, reduced nutritional state, poor physical performance and reduced anthropometric measures, irrespective of how muscle strength was measured. Independently associated factors differed depending on muscle strength test, except for older age. Conclusion: The prevalence of sarcopenia in the day hospital ranged from 27 to 37%. In this article we address the admission and the effects that the topology read from psychoanalysis has had in a sector of the province of Buenos Aires, particularly in the Day Hospital of the Hospital Zonal General de Agudos "General Manuel Belgrano" and later "Fundación Brizna ", between 1985 and 1999, institutions dedicated to the treatment of patients with psychotic subjectivity. Thus we work on aspects of surface topology and nodal topology. There we locate the passage of some ideas and theoretical articulations, of greater geographical and epochal extension, but we also trace the versions that these ideas have acquired in these latitudes. We find, in this way, nosographic constructions, structural and structuring reflections and clinical intervention modalities, sustained in an ethic against the consideration of a subject. Introduction The improvement perceived by the patients is a subjective measure of the psychic state, while the clinical evaluation corresponds to an objective evaluation of the psychopathological improvement performed by a psychiatrist. It is therefore relevant to evaluate whether these parameters evolve in a common way after patients have undergone an intervention in Day Hospital focusing on first psychotic episodes. Objectives Study the relationship between subjective improvement and clinical evaluation. Methods This is a prospective study, which includes consecutive patients admitted to the Day Hospital during 2018. Their objective clinical improvement was assessed by means of the PANSS and GAF scales at admission and discharge. Subjective clinical improvement was assessed using an anonymous Likert scale with a score between 1 and 7. Sociodemographic data and other satisfaction parameters were also collected. A statistical analysis was performed using Pearson’s correlation. Results A total of 73 patients were included. The perception of improvement on the part of the patients is very high presenting average values close to the maximum in almost all the evaluated items. The correlation between subjective improvement and PANSS variation presented a Pearson value 0.008; p = .957 and with the GAF variation presented a Pearson correlation of -0.066; p = .578 which indicates that there is no significant correlation between the variables. Introduction Day care programs have been extensively used to treat people with acute psychiatric disorders. Day hospitals (DH) can act as an alternative to admission in patients with acute symptoms, shorten the duration of admission, be useful for rehabilitation and maintenance care or enhance treatment in patients with poor adherence to outpatient care. Few research has been conducted in delusional disorder (DD). Objectives To investigate whether DH care increases adherence with psychiatric appointments in patients with DD. To describe functions of partial hospitalization in DD. Methods Comparative study including DD patients who attended a DH (Group 1:n=12) versus patients who did not receive DH care (Group 2;n=7). Patients attending DH were classified into 3 groups according to the program function at referral. Adherence with outpatient follow-up appointments (primary outcome) and pharmacy refill data (secondary outcome) were assessed after discharge over a 6-month period (DH) and compared with group 2. For statistical analyses, non-parametric tests were performed. Results Program function (DH): alternative to admission (n=4); shortening of admission (n=5) and enhancing outpatient treatment (n=3). Patients receiving DH care were more frequently referred from the inpatient unit or emergency department compared to those who did not attend DH (commonly referred from primary care services). No statistically significant differences were found between both groups in adherence to psychiatric appointments. Introduction: Impulsivity is one of the main symptoms among adolescents with severe mental disorder, which is associated with worse emotional regulation, and deficits in social skills, decisionmaking and problem solving, as well as poorer stress management. A specific intervention is necessary due to its serious personal and social implications. Psychosocial interventions may be an appropriate treatment approach for this population. Objective: to carry out a pilot study aimed to design, implement and evaluate the feasibility and shortterm impact of a brief sport-based intervention designed to reduce impulsivity and related factors in a group of adolescents with severe mental disorder admitted at the Child and Adolescent Day Hospital of Castellón. Methodology: the study involved 6 adolescents with an age range between 13 and 17 years, 4 females and 2 males, mainly diagnosed with externalizing disorders (83.3%). Results: the results show a clinical improvement after treatment in scores of problem solving, in addition to total, general and, above all, unplanned impulsivity. In addition, the scores in the social skills scales evaluated increased, obtaining a statistically significant improvement in communication skills and total social skills. Conclusions: the use of sport in psychosocial interventions with children and adolescents can be beneficial to learn coping skills and is feasible in resources such as day hospitals. The article describes the experience of Algerian specialists of organization of support to children with autism spectrum disorders to in Algeria. Algerian children began to receive comprehensive support at the state level two decades ago. The second part of the article describes a case and the organization of treatment for a autistic patient with comorbid mental retardation in a day hospital. The role of the family and parental organizations in the process of psychological, medical and social support of children and adolescents with ASD is considered, as well as the prospects for the development of a comprehensive care system in Algeria in the coming years. Résumé: L'éducation thérapeutique (ETP) permet d'évaluer les compétences d'autosoins et d'adaptation du patient à sa maladie. La chute est une maladie chronique entraînant perte d'autonomie, peur de chuter et entrées en institution. Ce travail a pour objectif d'évaluer à 3 et 6 mois, l'impact de l'ETP sur la survenue de nouvelles chutes, la qualité de vie perçue et la peur de chuter dans une population âgée de plus de 75 ans vivant à domicile. Matériels et méthodes : Nous avons suivi un groupe participant à l'ETP en hôpital de jour (groupe ETP) et un groupe témoin (groupe Hospitalisé) recruté dans des courts séjours gériatriques suite à une chute à domicile, ne bénéficiant pas d'ETP. Résultats : 28 patients dans le groupe ETP et 20 patients dans le groupe Hospitalisé ont été inclus. À 3 et 6 mois, la qualité de vie et la peur de chuter du groupe ETP était significativement améliorée par rapport au groupe Hospitalisé. Conclusion : Le groupe des sujets chuteurs bénéficiant d'une éducation thérapeutique vivant à domicile ont eu une qualité de vie à 3 et 6 mois améliorée associée à une diminution de la peur de chuter à 6 mois. Therapeutic patient education (TPE) allows elderly as well as young adults to evaluate patient's self-care and adaptation skills to their own clinical condition. Falling is a symptom present in various chronic diseases, which leads to loss of autonomy, fear of recidivism and frequent admissions into institutions. Study objective was to evaluate at 3 and 6 months the impact of TPE on fall recurrence, perceived quality of life and fear of falling, in elderly over 75 living at home. Methods : We performed a prospective study comparing two groups: a group participating in day hospital (group "TPE") and a control group (group "Hospitalized") of elderly patient not receiving TPE recruited in short geriatric stays following a fall at home. Results : 28 patients in the "TPE" group and 20 patients in the "Hospitalized" group were included. The "TPE" group reported significantly better perceived quality of life at 3 months and 6 months. At 6 months, fear of falling was twice as high in the "Hospitalized" group than in the "TPE" group. Conclusions : The group of subjects who was able to benefit from therapeutic education at home had an improved quality of life at 3 and 6 months and a decrease in the fear of falling at 6 months.
