Abstract

BackgroundIn the French Territories in the Americas (FTA), the risk of birth defects possibly associated with Zika virus (ZIKV) infection was 7.0% (95%CI: 5.0 to 9.5) among foetuses/infants of 546 women with symptomatic RT-PCR confirmed ZIKV infection during pregnancy. Many of these defects were isolated measurement-based microcephaly (i.e. without any detected brain or clinical abnormalities) or mild neurological conditions. We wanted to estimate the proportion of such minor findings among live births of women who were pregnant in the same region during the outbreak period but who were not infected with ZIKV.MethodsIn Guadeloupe, pregnant women were recruited at the time of delivery and tested for ZIKV infection. The outcomes of live born infants of ZIKV non-infected women were compared to those of ZIKV-exposed live born infants in Guadeloupe, extracted from the FTA prospective cohort.ResultsOf 490 live born infants without exposure to ZIKV, 42 infants (8.6%, 95%CI: 6.2-11.4) had mild abnormalities that have been described as 'potentially linked to ZIKV infection'; all but one of these was isolated measurement-based microcephaly. Among the 241 live born infants with ZIKV exposure, the proportion of such abnormalities, using the same definition, was similar (6.6%, 95%CI: 3.8-10.6).ConclusionsIsolated anthropometric abnormalities and mild neurological conditions were as prevalent among infants with and without in-utero ZIKV exposure. If such abnormalities had not been considered as 'potentially linked to ZIKV' in the original prospective cohort in Guadeloupe, the overall estimate of the risk of birth defects considered due to the virus would have been significantly lower, at approximately 1.6% (95% CI: 0.4-4.1).Trial registrationClinicalTrials.gov (NCT02916732).

During a 2014 outbreak, 450 patients with confirmed chikungunya virus infection were admitted to the University Hospital of Pointe-à-Pitre, Guadeloupe. Of these, 110 were nonpregnant adults; 42 had severe disease, and of those, 25 had severe sepsis or septic shock and 12 died. Severe sepsis may be a rare complication of chikungunya virus infection.

Abstract

Severe thrombocytopenia during or after the course of Zika virus infection has been rarely reported. We report 7 cases of severe thrombocytopenia and hemorrhagic signs and symptoms in Guadeloupe after infection with this virus. Clinical course and laboratory findings strongly suggest a causal link between Zika virus infection and immune-mediated thrombocytopenia.

Abstract

We report a case of pyogenic liver abscess caused by community-acquired Klebsiella quasipneumoniae subsp. quasipneumoniae. The infecting isolate had 2 prominent features of hypervirulent K. pneumoniae strains: the capsular polysaccharide synthesis region for K1 serotype and the integrative and conjugative element ICEKp1, which encodes the virulence factors yersiniabactin, salmochelin, and RmpA.

Abstract

Supplemental Digital Content is Available in the Text., Objectives: To investigate whether mycobacterial antigen–induced cytokine secretions are helpful in detecting Mycobacterium tuberculosis (Mtb) infection in a cohort of HIV-infected patients living in a country with a high burden of Mtb and HIV infections, and to determine their predictive value for the development of tuberculosis (TB)-associated immune reconstitution inflammatory syndrome. Design: A total of 352 HIV-infected patients (186 with active TB) were prospectively enrolled when initiating antiretroviral therapy (ART). Sequential blood samples were collected during the first 6 months of ART. Eighty-three HIV-uninfected subjects (39 with active TB) were enrolled as controls. Methods: The concentrations of 13 cytokines were measured in supernatants from blood mononuclear cells in vitro stimulated with purified protein derivative (PPD), heparin-binding hemagglutinin (HBHA) or early secreted antigen-6 (ESAT-6) and culture filtrate protein-10 (CFP-10), and results were compared with those of tuberculin skin tests (TST). Results: The best detection of Mtb infection was achieved by ESAT-6/CFP-10–induced interferon-γ concentrations, but results were often negative for patients with CD4+ T-cell counts

Abstract

International audience; BACKGROUND:The risk of congenital neurologic defects related to Zika virus (ZIKV) infection has ranged from 6 to 42% in various reports. The aim of this study was to estimate this risk among pregnant women with symptomatic ZIKV infection in French territories in the Americas.METHODS:From March 2016 through November 2016, we enrolled in this prospective cohort study pregnant women with symptomatic ZIKV infection that was confirmed by polymerase-chain-reaction (PCR) assay. The analysis included all data collected up to April 27, 2017, the date of the last delivery in the cohort.RESULTS:Among the 555 fetuses and infants in the 546 pregnancies included in the analysis, 28 (5.0%) were not carried to term or were stillborn, and 527 were born alive. Neurologic and ocular defects possibly associated with ZIKV infection were seen in 39 fetuses and infants (7.0%; 95% confidence interval, 5.0 to 9.5); of these, 10 were not carried to term because of termination of pregnancy for medical reasons, 1 was stillborn, and 28 were live-born. Microcephaly (defined as head circumference more than 2 SD below the mean for sex and gestational age) was detected in 32 fetuses and infants (5.8%), of whom 9 (1.6%) had severe microcephaly (more than 3 SD below the mean). Neurologic and ocular defects were more common when ZIKV infection occurred during the first trimester (24 of 189 fetuses and infants [12.7%]) than when it occurred during the second trimester (9 of 252 [3.6%]) or third trimester (6 of 114 [5.3%]) (P=0.001).CONCLUSIONS:Among pregnant women with symptomatic, PCR-confirmed ZIKV infection, birth defects possibly associated with ZIKV infection were present in 7% of fetuses and infants. Defects occurred more frequently in fetuses and infants whose mothers had been infected early in pregnancy. Abstract

A total of 120 bronchoalveolar lavage specimens from HIV and non-HIV immunocompromised patients, positive for Pneumocystis jirovecii by an "in house" real-time polymerase chain reaction (PCR), were evaluated by the Bio-Evolution Pneumocystis real-time PCR, a commercial quantitative assay. Patients were classified in 2 categories based on clinical and radiological findings: definite and unlikely Pneumocystis pneumonia (PCP). For the "in house" PCR, cycle threshold 34 was established as cut-off value to discriminate definite PCP from unlikely PCP with 65% and 85% of sensitivity and specificity, respectively. For the Bio-Evolution quantitative PCR, a cut-off value of 2.8×10(5)copies/mL was defined with 72% and 82% of sensitivity and specificity, respectively. Overlapped zones of results for definite and unlikely PCP were observed. Quantitative PCR is probably a useful tool for PCP diagnosis. However, for optimal management of PCP in non-HIV immunocompromised patients, operational thresholds should be assessed according to underlying diseases and other clinical and radiological parameters.

Abstract

The search for novel vaccines against tuberculosis (TB) would benefit from in-depths knowledge of the human immune responses to Mycobacterium tuberculosis (Mtb) infection. Here, we characterised in a low TB incidence country, the immune responses to a new candidate vaccine antigen against TB, the heparin-binding haemagglutinin (HBHA), in young children in contact with an active TB case (aTB). Children with no history of BCG vaccination were compared to those vaccinated at birth to compare the initial immune responses to HBHA with secondary immune responses. Fifty-eight children with aTB and 76 with latent TB infection (LTBI) were included and they were compared to 90 non-infected children. Whereas Mtb-infected children globally secreted more interferon-gamma (IFN-γ) in response to HBHA compared to the non-infected children, these IFN-γ concentrations were higher in previously BCG-vaccinated compared to non-vaccinated children. The IFN-γ concentrations were similar in LTBI and aTB children, but appeared to differ qualitatively. Whereas the IFN-γ secretion induced by native methylated and recombinant non-methylated HBHA were well correlated for aTB, this was not the case for LTBI children. Thus, Mtb-infected young children develop IFN-γ responses to HBHA that are enhanced by prior BCG vaccination, indicating BCG-induced priming, thereby supporting a prime-boost strategy for HBHA-based vaccines. The qualitative differences between aTB and LTBI in their HBHA-induced IFN-γ responses may perhaps be exploited for diagnostic purposes.

Abstract

International audience; The Guillain–Barré syndrome (GBS) has been reported as a possible complication of acute chikungunya infection. The chikungunya epidemics, which occurred in Martinique and Guadeloupe in 2014, affected 308,000 people in these two islands. GBS occurred during or immediately after acute chikungunya infection in 13 patients (10 men, three women; mean age: 61 years). Median time from acute chikungunya to GBS onset was 9 days. Twelve patients were treated with intravenous polyvalent immunoglobulins, nine of whom improved within 7 days. Five of 13 patients required mechanical ventilation. Two patients with severe GBS died. At 6 months of follow-up, 7/13 achieved a good functional recovery with no or minor residual symptoms. A 2-fold increase in incidence was observed during the year of chikungunya outbreak. This study supports prior reports suggesting that GBS may be a complication of chikungunya.

Abstract

International audience; The French National Reference Center for Invasive Mycoses and Antifungals leads an active and sustained nationwide surveillance program on probable and proven invasive fungal diseases (IFDs) to determine their epidemiology in France. Between 2012 and 2018, a total of 10,886 IFDs were recorded. The incidence increased slightly over time (2.16 to 2.36/10,000 hospitalization days, P = 0.0562) in relation with an increase of fungemia incidence (1.03 to 1.19/10,000, P = 0.0023), while that of other IFDs remained stable. The proportion of ≥65-year-old patients increased from 38.4% to 45.3% (P < 0.0001). Yeast fungemia (n = 5,444) was due mainly to Candida albicans (55.6%) with stable proportions of species over time. Echinocandins became the main drug prescribed (46.7% to 61.8%), but global mortality rate remained unchanged (36.3% at 1 month). Pneumocystis jirovecii pneumonia (n = 2,106) was diagnosed mostly in HIV-negative patients (80.7%) with a significantly higher mortality than in HIV-positive patients (21.9% versus 5.4% at 1 month, P < 0.0001). Invasive aspergillosis (n = 1,661) and mucormycosis (n = 314) were diagnosed mostly in hematology (>60% of the cases) with a global mortality rate of 42.5% and 59.3%, respectively, at 3 months and significant changes in diagnosis procedure over time. More concurrent infections were also diagnosed over time (from 5.4% to 9.4% for mold IFDs, P = 0.0115). In conclusion, we observed an aging of patients with IFD with a significant increase in incidence only for yeast fungemia, a trend toward more concurrent infections, which raises diagnostic and therapeutic issues. Overall, global survival associated with IFDs has not improved despite updated guidelines and new diagnostic tools.IMPORTANCE The epidemiology of invasive fungal diseases (IFDs) is hard to delineate given the difficulties in ascertaining the diagnosis that is often based on the confrontation of clinical and microbiological criteria. The present report underlines the interest of active surveillance involving mycologists and clinicians to describe the global incidence and that of the main IFDs. Globally, although the incidence of Pneumocystis pneumonia, invasive aspergillosis, and mucormycosis remained stable over the study period (2012 to 2018), that of yeast fungemia increased slightly. We also show here that IFDs seem to affect older people more frequently. The most worrisome observation is the lack of improvement in the global survival rate associated with IFDs despite the increasing use of more sensitive diagnostic tools, the availability of new antifungal drugs very active in clinical trials, and a still low/marginal rate of acquired in vitro resistance in France. Therefore, other tracks of improvement should be investigated actively.

Abstract

Introduction: In 2015 some estimations evaluated that 30% to 50% of antibiotic prescriptions in France were not conformed to the latest guidelines for good practice of antibiotic use. The resistance to antibiotics is a worldwide threat to public health, with a negative impact in patient outcomes. Our aim was to identify the determining factors in the prescription of fluoroquinolones (FQ) by the general practitioners (GP) in Guadeloupe (indications, choice of the molecule, dosage, duration and adaptation) and make an inventory of this prescription before and after corrective intervention. Methods: We realized a qualitative survey of the determinants of the prescription of FQ by the general practitioners of the quality groups in Guadeloupe in 2016. We then conducted a descriptive case-control study before and after corrective intervention, focused on the average number of prescribed boxes of FQ by these practitioners,during the first semester of 2016 and 2017. Results: Of the 94 physicians to include, 51 participated in the study. Of the 30 analyzable records, 90% (n = 27) of FQ prescriptions were for urinary tract disease with major use of Ofloxacin. Only 4 cards out of the 30 were fully compliant with FQ's policy of good practice, with 18 questionable indications and 15 nonconformities for the duration of prescription. For the descriptive study before and after corrective intervention, the FQ prescription of 86 doctors of the Guadeloupe quality groups could be compared to that of a control group with 199 GPs from Guadeloupe. Thus,we observed that between 2016 and 2017, the quality groups decreased their FQ prescription with an average of 19 to 17boxes per GP per semester whereas an upward trend could be observed in the control group increasing from 27 to 28. The corrective intervention was associated with an improvement in the prescription of fluoroquinolones by the physicians of the quality groups (p = 0.03). Conclusion: Our study showed that GPs of the quality groups used FQ mainly to treat diseases related to the urinary system, which is an approved indication of use in first intention only in case of acute nephritis. Dosages were respected but the duration was too long in half of the cases (n=15). On the basis of this observation, a corrective intervention was carried out and significantly improved the prescription of FQ by the physicians of the quality groups compared to that of the control group, but repeated intervention would probably have a greater impact., Introduction : En 2015, des estimations montraient que 30 à 50 % des antibiothérapies prescrites en France, n’étaient pas conformes aux dernières recommandations du bon usage des antibiotiques. La résistance bactérienne qui en résulte est un problème majeur de santé publique, avec un impact défavorable sur la morbi-mortalité des patients. L’objectif de cette étude était d’identifier les éléments déterminants dans la prescription des fluoroquinolones (FQ) par les médecins généralistes de Guadeloupe(indications, choix de la molécule, posologie, durée et adaptation) et faire un état des lieux de cette prescription avant et après intervention corrective. Méthodes : Il s’agit d’une enquête qualitative des éléments déterminants la prescription de FQ de janvier 2016 à juin 2017 par les médecins généralistes des groupes qualité de Guadeloupe,suivie d’une étude descriptive cas-témoins avant/après intervention corrective,centrée sur le nombre moyen semestriel de boites prescrites de FQ chez ces médecins au premier semestre 2016 et 2017. Résultats : Sur les 94 médecins à inclure, 54% (n=51) ont participé à l’étude. Sur les 30 fiches analysables, 90% (n=27) des prescriptions de FQ avaient pour principale indication une pathologie urinaire avec une utilisation prédominante d’Ofloxacine. Seules 4 fiches sur les 30 étaient entièrement conformes à la politique de bon usage des FQ, avec 18 indications discutables et 15 non-conformités pour la durée de prescription. Pour l’étude descriptive avant/après intervention corrective, on a pu observer qu’entre 2016 et 2017, les groupes qualité (86 médecins des groupes qualité) ont diminué leur prescription de FQ avec une moyenne de prescription semestrielle passant de 19 à 17, alors qu’une tendance à la hausse a pu être observée dans le groupe témoin (199 médecins généralistes) avec une moyenne semestrielle passant de 27 à 28. L’intervention corrective a été associée à une amélioration significative de la prescription des fluoroquinolones (p=0.03). Conclusion : Notre étude a montré que les médecins généralistes des groupes qualité utilisaient les FQ principalement pour traiter des pathologies en rapport avec l’appareil urinaire, ce qui représente une bonne indication en première intention uniquement pour les pyélonéphrites simples. Les posologies ont été respectées, mais la durée, elle, a été trop longue dans la moitié des cas (n=15). En partant de ce constat, une intervention corrective a été réalisée et a permis, une amélioration significative de la prescription des FQ par les médecins des groupes qualité par rapport à celle du groupe témoin qui devra surement être répétée pour obtenir des résultats pérennes.
