Abstract Studies reported the existence of instability catch (IC) during trunk flexion in patients with chronic low back pain (CLBP). However, different movement speeds can cause different neuromuscular demands resulting in altered kinematic patterns. In addition, kinematic characterization corresponding to clinical observation of IC is still limited. The associations between movement speed and kinematic parameters suggest that movement speed changes can alter kinematic patterns. Therefore, clinicians may challenge lumbopelvic neuromuscular control by modifying movement speed to elicit greater change in kinematic patterns. In addition, the NoLBP group used shared lumbar and pelvic contributions, while the CLBP group used less pelvic contribution. Finally, P2P and Area appeared to offer the greatest sensitivity to differentiate between the groups. Overall, these findings may enhance the understanding of the mechanism underlying IC in CLBP. This study aimed to explore the influence of leg posture control and individual flexibility on FRP in back and low limb muscles. Thirty-two male participants, evenly distributed into high- and low-flexibility groups, were recruited. Activities of the erector spinae, biceps femoris, and gastrocnemius muscles, alongside the lumbosacral angle (LSA), were recorded as participants executed trunk flexion from 0° to 90° in 15° increments, enabling an analysis of FRP and its correlation with the investigated variables. The findings highlighted significant effects of all examined factors on the measured responses. At a trunk flexion angle of 60°, the influence of leg posture and flexibility on erector spinae activities was particularly pronounced. Participants with limited flexibility exhibited the most prominent FRP under constrained leg posture, while those with greater flexibility and unconstrained leg posture displayed the least FRP, indicated by their relatively larger LSAs. Under constrained leg posture conditions, participants experienced an approximate 1/3 to 1/2 increase in gastrocnemius activity throughout trunk flexion from 30° to 90°, while biceps femoris activity remained relatively constant. Trunk strength plays a vital role in athletic performance, rehabilitation and general health, however, current assessment methods are expensive, non-portable or unreliable. This study aimed to investigate the within- and between-session reliability, variability, standard error of measurement and minimal detectable change (MDC) of trunk strength in the sagittal (flexion and extension) and frontal planes (left and right lateral flexion) using a fixed digital dynamometer. Eighteen participants (ten men and eight women) attended two sessions separated by 7 days. Participants were fitted with a trunk harness which was secured to an immovable base via a digital dynamometer. Three maximal voluntary isometric contractions were completed across four positions (prone, supine, left-side recumbent and right-side recumbent, respectively) on a glute-hamstring raise machine. All positions demonstrated excellent reliability and low variability within session (ICC: 0.95–0.98; CV: 5–7%) and between sessions (ICC: 0.98–0.99; CV: 4–6%), across all positions. The between-session MDC ranged from 8% (prone) to 13% (right-side recumbent), translating to absolute values between 2.9 and 3.2 kg across all positions. Maximal isometric force testing using a fixed digital dynamometer provides reliable measurements of multiplanar trunk strength, providing a practical method for use in clinical practice. Viscoelastic creep generated in the lumbar spine following sustained spine flexion may affect the relationship between tissue damage and perceived pain. Two processes supporting this altered relationship include altered neural feedback and inflammatory processes. Out purpose was to determine how low back mechanical pain sensitivity changes following seated lumbar spine flexion using pressure algometry in a repeated-measures. cross-sectional laboratory design. Thirty-eight participants underwent a 10-minute sustained seated maximal flexion exposure with a 40-minute standing recovery period. Pressure algometry assessed pressure pain thresholds and the perceived intensity and unpleasantness of fixed pressures. Accelerometers measured spine flexion angles, and electromyography measured muscular activity during flexion. The fiexion exposure produced 4.4° (2.7°) of creep that persisted throughout the entire recovery period. The perception of low back stimulus unpleasantness was elevated immediately following the exposure, 20 minutes before a delayed increase in lumbar erector spinae muscle activity. Women reported the fixed pressures to be more intense than men. Sustained flexion had immediate consequences to the quality of mechanical stimulus perceived but did not alter pressure pain thresholds. Neural feedback and inflammation seemed unlikely mechanisms for this given the time and direction of pain sensitivity changes, leaving a postulated cortical influence. [ABSTRACT FROM AUTHOR]

Forward body lean and greater horizontal ground reaction force have been associated with being able to accelerate during running. However, kinematic features which may predict acceleration during the run-to-sprint transition have not been determined. The purpose of this study was to determine which kinematic changes occur in recreationally active adults and which kinematic features may predict greater acceleration during the run-to-sprint transition. Forty-seven healthy adults completed straight line running along a 30 m track by running in at ∼4 m.s-1. A minimum of 20 trials were completed, with 25 % triggering a light to signal the participant to accelerate as fast as possible. Step characteristics (velocity, length, duration, cadence) and kinematics (neck, trunk, hip, knee and ankle angles and excursions) were determined using a radar gun and inertial measurement units, respectively. ANOVA was used to determine the step-to-step differences and a multiple linear regression was used to determine the relationship between kinematics and acceleration. There was an initial increase in trunk flexion angle during early acceleration (p < 0.001) with knee joint excursion significantly lower (p < 0.001) during loading and propulsion compared to the run-in steps. Greater acceleration was predicted using a stepwise linear regression by five variables including less neck flexion excursion and trunk flexion angle during swing of the 1st step, greater trunk flexion angle and extension excursion of the neck during propulsion of the 2nd step and greater hip flexion angle at foot strike of the 3rd step (r2 = 0.804, p < 0.001). Faster acceleration was observed when participants transitioned from an upright posture to greater forward trunk lean in the early phase of acceleration. Training the run-to-sprint transition, which was shown to have the greatest increase in velocity over the first 5 m, may be encouraged as a sports specific exercise. • Two phases of acceleration were observed during the run-to-sprint transition. • Greater trunk flexion was the most significant postural change during acceleration. • Postural changes begin at midswing of the 1st step after the signal to accelerate. • Transition from upright to greater forward trunk lean predicts faster acceleration. The purpose of this study was to compare interlimb asymmetries in trunk and knee kinematics during a single-leg drop landing between athletes 9 months post anterior cruciate ligament reconstruction (post-ACLR) and healthy athletes using two-dimensional analysis. Thirty-three recreational athletes (12 post-ACLR and 21 healthy) participated in the study. Participants post-ACLR showed significantly higher limb symmetry indices in peak trunk flexion (144.0%, SE drop landing kinematics: 22.7%) when compared to healthy participants (100.6%, SE: 10.5%; z = 2.17, p =.03) and lower limb symmetry indices in peak knee flexion (85.3%, SE: 3.6%) when compared to healthy participants (98.0%, SE: 3.3%; z = −2.43, p =.01). Two-dimensional analyses of a single-leg drop landing is a clinically applicable tool that can identify interlimb asymmetries in peak trunk flexion and peak knee flexion kinematics in athletes greater than 9 months post-ACLR when compared to healthy athletes. OBJECTIVE: Forward bending of the back is common in many jobs and a risk factor for sickness absence. However, this knowledge is based on self-reported forward bending that is generally imprecise. Thus, we aimed to investigate the dose–response relation between device-measured forward bending at work and prospective register-based risk of long-term sickness absence (LTSA). METHODS: At baseline, 944 workers (93% from blue-collar jobs) wore accelerometers on their upper back and thigh over 1–6 workdays to measure worktime with forward bending (>30˚ and >60˚) and body positions. The first event of LTSA (≥6 consecutive weeks) over a 4-year follow-up were retrieved from a national register. Compositional Cox proportional hazard analyses were used to model the association between worktime with forward bending of the back in an upright body position and LTSA adjusted for age, sex, body mass index (BMI), occupational lifting/carrying, type of work, and, in an additional step, for leisure time physical activity (PA) on workdays. RESULTS: During a mean worktime of 457 minutes/day, the workers on average spent 40 and 10 minutes on forward bending >30˚ and >60˚ in the upright position, respectively. Five more minutes forward bending >30˚ and >60˚ at work were associated with a 4% [95% confidence interval (CI) 1.01–1.07] and 8% (95% CI 1.01–1.16) higher LTSA risk, respectively. Adjustment for leisure-time PA did not influence the results. CONCLUSION: We found a dose–response association between device-measured forward bending of the back and prospective LTSA risk. This knowledge can be integrated into available feasible methods to measure forward bending of the back for improved workplace risk assessment and prevention.

Trunk flexion contracture is an abnormal posture in elderly individuals with lumbar kyphosis. It is unclear whether this posture affects locomotor stability (margin of stability [MoS]) during obstacle crossing, which is a common trigger for falls in elderly people. Does trunk flexion contracture negatively affect MoS during obstacle crossing in elderly people? Ten healthy elderly individuals performed five trials of obstacle crossing using a comfortable speed under two experimental conditions, namely, with (FLEX) or without (NORMAL) a hard lumbar brace to simulate trunk flexion contracture. The obstacle-crossing motion was captured using an optical motion analysis system in order to calculate the MoS in the anteroposterior direction. The MoS at initial contact (IC) and that when the swing foot was above the obstacle (Obs) was compared between FLEX and NORMAL. A greater MoS suggests greater risk of a forward fall. The trunk and lower limb joint angles were measured at Obs. FLEX significantly increased the MoS at IC, whereas the MoS at Obs did not differ between the two conditions. FLEX demonstrated a crouch posture characterized by an increased flexion angle of stance-side hip and knee joints at the Obs instant. Forward fall chance might be increased at IC in obstacle crossing with trunk flexion contracture. Meanwhile, the MoS at Obs might be controlled by increasing the crouch posture to offset a forward shift in the CoM position due to the trunk flexion. Because the risk of a stumble on an obstacle and of forward falls should be higher at Obs than at IC, the crouch posture seems to be an effective adaptation that enables elderly people with trunk flexion contracture to safely cross obstacles. • We investigated trunk postures during obstacle crossing in elderly people. • Trunk flexion negatively affected the margin of stability at initial contact. • Trunk flexion caused an anterior shift of CoM relative to the base of support. • A crouch posture is needed to offset this shift of CoM. The flexion-relaxation phenomenon (FRP) refers to the deactivation of back muscles during deep forward trunk bending. This study examined the effects of back muscle strength, individual flexibility, and trunk angle on FRP in the back muscles. Forty male participants were classified into four groups according to toe-touch flexibility and back muscle strength. Lumbar erector spinae (LES) activity and the lumbosacral angle (LSA) were measured at incremental trunk flexion angles (0°–90°, with increments of 15°) to analyze FRP. Results indicated significant effects of back muscle strength, flexibility, and trunk angle on LES activity (all p < 0.001). Flexibility (p < 0.05) and trunk angle (p < 0.001) also influenced LSA. Additionally, an interaction between flexibility and trunk angle impacted LES activation (p < 0.001). Flexibility mainly determined FRP onset, while back muscle strength influenced efforts during moderate trunk flexion (30°–60°). These findings indicate that differences in lower back load among individuals with varying back muscle strengths become apparent even at relatively small trunk flexion angles (approximately 30°). When prolonged static trunk flexion in workplace settings places considerable strain on the lower back, we recommend utilizing toe-touch flexibility and back muscle strength assessments as practical screening tools for identifying early and subtle indicators of the FRP in workers. Background: Low-back pain (LBP) is one of the most common musculoskeletal problems; it is the leading cause of disability worldwide. Physiotherapy exercise is a method that enables health care professionals to pass on their knowledge and experience to patients so that they can participate consciously and actively in their recovery. Aim: The present study aims to examine the effect of physiotherapy on LBP. Materials and Methods: This retrospective study was carried out in the Department of Physiotherapy in a tertiary care hospital, India. A total of 50 patients of age range 18 to 60 years with chronic low back pain were enrolled in the present study. Pre and post physiotherapy effect on low back pain and trunk flexion was evaluated and compared. Results: The majority of the patients 44% were 46-60 years age group, mean age ± SD was 48.6 ± 9.17 years. Slight male predominance (52%) was found. The mean ± SD, BMI was 29.4 ± 5.0 and was mean ± SD duration of LBP (months) was 17.4 ± 2.2 was patients. Significant improvement of VAS score (pain) and trunk flexion movement was seen in post treatment group. Conclusion: Multidisciplinary approaches including physical therapy should be implemented to provide long-term improvement in pain and functional status in the treatment of chronic LBP. Nowadays, the better assessment of low back pain (LBP) is an important challenge, as it is the leading musculoskeletal condition worldwide in terms of years of disability. The objective of this study was to evaluate the relevance of various machine learning (ML) algorithms and Sample Entropy (SampEn), which assesses the complexity of motion variability in identifying the condition of low back pain. Twenty chronic low-back pain (CLBP) patients and 20 healthy non-LBP participants performed 1-min repetitive bending (flexion) and return (extension) trunk movements. Analysis was performed using the time series recorded by three inertial sensors attached to the participants. It was found that SampEn was significantly lower in CLBP patients, indicating a loss of movement complexity due to LBP. Gaussian Naive Bayes ML proved to be the best of the various tested algorithms, achieving 79% accuracy in identifying CLBP patients. Angular velocity of flexion movement was the most discriminative feature in the ML analysis. This study demonstrated that: supervised ML and a complexity assessment of trunk movement variability are useful in the identification of CLBP condition, and that simple kinematic indicators are sensitive to this condition. Purpose: The flexion relaxation phenomenon (FRP) is characterized by the reduction of paraspinal muscle activity at maximum trunk flexion. The FRP is reported to be altered (persistence of spinal muscle activity) in nonspecific chronic low back pain (NSCLBP) and is considered a promising biomarker. The aim of this systematic review was to synthetize current knowledge on FRP in the NSCLBP population regarding prevalence, the reliability of FRP measurement using surface electromyography (sEMG), the average value, and variation of the relaxation ratios (RR). Methods: Five databases were searched (PubMed, EMBASE, Web of Sciences, Cochrane Library, Pedro). A qualitative analysis was done for all included studies and meta-analysis studying prevalence, mean value of flexion relaxation ratio (FRR) and extension relaxation ratio (ERR), and difference between asymptomatic and NSCLBP FRR. Results: Twenty-seven studies were included for qualitative analysis and 21 studies for meta-analysis. The prevalence of the altered FRP was 55% (95%CI [32–79%]) in the NSCLBP population. The studies on reliability reported good within-session and between-session reproducibility. In the NSLBP population, the mean FRR was 2.96 (95%CI [2.02; 3.90]) and the mean ERR was 4.07 (95%CI [2.08; 6.07]). The difference between asymptomatic and NSCLBP FRR was − 1.19 (95%CI [− 1.92, − 0.45]). In all meta-analysis, the I2 index was > 80%. Conclusion: An altered FRP is frequently found in NSCLBP population using sEMG and the test has a good reproducibility. The difference between asymptomatic and NSCLBP FRR was significant. Background: Stand-to-sit (StandTS) movement is an important functional activity that can be challenging for older adults due to age-related changes in neuromotor control. Although trunk flexion, eccentric contraction of the rectus femoris (RF), and coordination of RF and biceps femoris (BF) muscles are important to the StandTS task, the effects of aging on these and related outcomes are not well studied.Research Question: What are the age-related differences in trunk flexion, lower extremity muscle activation patterns, and postural stability during a StandTS task and what is the relationship between these variables?Methods: Ten younger and ten older healthy adults performed three StandTS trials at self-selected speeds. Outcomes included peak amplitude, peak timing, burst duration, and onset latency of electromyography (EMG) activity of the RF and BF muscles, trunk flexion angle and angular velocity, whole body center of mass (CoM) displacement, center of pressure (CoP) velocity, and ground reaction force (GRF).Results: There were no age-related differences in weight-bearing symmetry, StandTS and trunk flexion angular velocity, or BF activity. In both groups, EMG peak timing of RF was preceded by BF. Compared to younger adults, older adults demonstrated shorter RF EMG burst duration, reduced trunk flexion, and reduced stability as indicated by the longer duration in which CoM was maintained beyond the posterior limit of base of support (BoS), greater mean anterior-posterior CoP velocity and larger standard deviation of CoM vertical acceleration during StandTS with smaller vertical GRF immediately prior to StandTS termination. Trunk flexion angle and RF EMG burst duration correlated with stability as measured by the duration in which the CoM stayed within the BoS.Significance: Decreased trunk flexion and impaired eccentric control of the RF are associated with StandTS instability in aging and suggest the importance of including StandTS training as a part of a comprehensive balance intervention. Background: The head, arms and trunk segment constitute a large proportion of the body's mass. Therefore, small alterations in trunk inclination may affect lower limb joint moments and muscle activation patterns. Although previous research has investigated the effect of changing frontal plane inclination of the trunk, it is not clear how increasing trunk flexion will impact on the activation of the lower limb muscles.Research Question: What is the effect of independently manipulating trunk flexion angle on lower limb kinematics, moments and muscle function?Methods: Gait analysis was carried out on 20 healthy people under four trunk flexion conditions: normal walking (NW), NW-5°, NW+5° and NW+10°. For the latter three conditions, a biofeedback approach was used to tightly control trunk flexion angle. A linear mixed model was used to investigate the effect of changing trunk flexion on joint angles, moments, and knee muscle activation.Results: There were clear increases in hip and ankle moments as trunk flexion was increased, but no change in knee moments. The results also showed a linear increase in knee flexor muscle activity and a corresponding increase in co-contraction as trunk flexion increased. Interestingly, there was a dramatic change in the profile of hamstring activity. In the medial hamstrings, this change led to a 100% increase in activation during early stance as flexion was increased by 5° from NW.Significance: This is the first study to demonstrate a strong dependence of knee flexor muscle activity on trunk flexion. This is important as people with knee osteoarthritis have been observed to walk with elevated muscle activation and this has been linked to increased joint loads. Introduction: Postural abnormalities are common in patients with Parkinson’s disease (PD) and lead to gait abnormalities. Relationships between changes in the trunk posture of PD patients and gait profile score (GPS) and gait spatiotemporal parameters are poorly investigated. The aim of the current study was to investigate the relationships between trunk posture, GPS, and gait spatiotemporal parameters, in patients with PD. Materials and Methods: Twenty-three people with PD and nineteen age-matched healthy people participated in this study. A 3D gait kinematical analysis was applied to all participants using the Plug-In Gait Full BodyTM tool. Trunk and limb kinematics patterns and gait spatio-temporal parameters of patients with PD and the control group were compared. Additionally, correlations between trunk kinematics patterns, gait spatio-temporal parameters, and GPS of the PD group were tested. Results: Cadence, opposite foot off, step time, single support, double support, foot off, gait speed, trunk kinematics, and GPS showed significant differences between the two groups (p ≤ 0.05). Posture of the trunk during gait was not related to the spatio-temporal parameters and gait profile score in the PD group. The trunk flexor pattern influenced GPS domains, mainly of the ankle and the knee. Discussion and Conclusions: Flexed posture of the trunk in patients with PD seems to influence both ankle and knee movement patterns during the gait. The GPS analysis provided direct and simplified kinematic information for the PD group. These results may have implications for understanding the importance of considering the positioning of the trunk during gait.

Muscular imbalances of the trunk muscles are held responsible for changes in body posture. At the same time, whole-body electromyostimulation (WB-EMS) has been established as a new training method that enables simultaneous stimulation of many muscle groups. This study was aiming to analyze if a 10 weeks WB-EMS training changes posture-relevant parameters and/or improves isometric strength of the trunk extensors and flexors, and if there are differences based on stimulation at 20 Hz and 85 Hz. Fifty eight untrained adult test persons were divided into three groups (control, CON; training with 20 Hz stimulation, TR20; training with 85 Hz, TR85). Anthropometric parameters, trunk extension and flexion forces and torques, and posture parameters were determined before (n = 58) and after (n = 53: CON: n = 15, TR20: n = 19, TR85: n = 19) a 10 weeks WB-EMS training program (15 applications, 9 exercises). Differences between the groups were calculated for pre- and post-tests using univariate ANOVA and between the test times using repeated (2 × 3) ANOVA. Comparisons of pairs were calculated post hoc based on Fisher (LSD). No differences between the groups were found for the posture parameters. The post hoc analysis of both trunk flexion and trunk extension forces and torques showed a significant difference between the groups TR85 and CON but no difference between the other group pairs. A 10 weeks whole-body electrostimulation training with a stimulation frequency of 85 Hz in contrast to training with a stimulation frequency of 20 Hz improves the trunk muscle strength of an untrained group but does not significantly change posture parameters.

Background: Over 50% of the body's mass is concentrated within the head, arms and trunk. Thus, small deviations in the orientation of the trunk, during normal walking, could influence the position of the centre of mass relative to the lower limb joint centres and impact on lower limb biomechanics. However, there are minimal data available on sagittal kinematics of the trunk in people with knee osteoarthritis (OA) during walking.Research Question: Do people with knee OA have altered kinematic patterns of the trunk, pelvis or hip compared with healthy control participants during walking?Methods: Statistical parametric mapping was used to compare sagittal and frontal plane kinematic patterns, during walking, between a healthy group and cohort of people with knee OA.Results: Individuals with knee OA walked with a mean increase in trunk flexion of 2.6°. Although this difference was more pronounced during early stance, it was maintained across the whole of stance phase. There were no differences, between the groups, in sagittal plane pelvic or hip kinematics. There were also no differences in trunk, pelvic or hip kinematics in the frontal plane.Significance: Most previous gait research investigating trunk motion in people with knee OA has focused on the frontal plane. However, our data suggest that an increase in sagittal trunk flexion may be a clinical hallmark of people with this disease. Abstract Background Obesity alters whole body kinematics during activities of daily living such as sit-to-stand (STS), but the relative contributions of excess body mass and decreased relative strength are unknown. Methods Three-dimensional motion analysis data was collected on 18 obese subjects performing sit-to-stand (chair height: 52 cm). Isometric knee extensor strength was measured at 900 knee flexion. Forward stepwise linear regression was used to determine the association between the independent variables BMI and the knee extensor torque with the dependent variables: foot position and trunk kinematics. Findings BMI, but not knee extensor torque, was inversely related to shank angle and positively related to stance width. Relative knee extensor torque, but not BMI, was inversely associated with initial trunk angle, peak trunk flexion angle, and peak trunk extension velocity (r2 = 0.470–0.495). BMI was positively associated with peak trunk flexion velocity, but no other parameters of trunk kinematics. In the final regression model, BMI was the primary predictor (r2 = 0.423) and relative knee extensor strength served as a secondary predictor (r2 = 0.118) of peak trunk flexion velocity. Interpretation BMI and knee extensor strength differently contribute to sit-to-stand performance strategies in obese subjects. Muscle strength may be an important determinant of whole-body kinematics during activities of daily living such as STS. OBJECTIVES: The aim of this paper was to investigate if objectively measured daily duration of forward bending of the trunk increases the risk of the development or aggravation of low-back pain (LBP) over one year in a working blue-collar population by examining (i) the incidence rate of LBP among workers reporting no LBP at baseline, and (ii) the aggravation of LBP among workers reporting LBP at baseline. METHODS: Using data from the Danish Physical Activity Cohort with Objective Measurements (DPhacto), the study measured forward bending of the trunk (>60˚) at work (FBW) and during leisure time (FBL), diurnally with accelerometers, and LBP with one-year monthly self-reports among 682 blue-collar workers from 15 workplaces. The development of LBP was investigated with Cox’s proportional hazards model (N=200), and the aggravation of LBP was investigated with mixed model for repeated measurements (N=482). RESULTS: Workers with no LBP at baseline had a FBW median of 7.9 minutes/day. Workers with LBP at baseline had a FBW median of 7.3 minutes/day. No significant associations were found between daily duration of forward bending of the trunk and development or aggravation of LBP. Similar results were found in the secondary analyses, in which FBL, different degrees of forward bending (>30˚ and >90˚), and varying follow-up time since measurement were considered. CONCLUSION: Using objective measurements of forward bending and monthly follow-up of LBP over one year, this study did not confirm the hypothesis of a positive association between daily duration of forward bending and LBP.

Low Back Pain (LBP) is one of the most common incidences all over the world. For the assessment of LBP, descriptive medical scores are widely used. Nevertheless, there is a need for the quantitative assessment of LBP by appropriate physiological and kinematic measurements. Quantitative assessment methods are of interest since they might provide reliable and repeatable measures related to low back pain in both everyday clinical practice and at home or work. In this paper, we proposed simple measurements of the trunk angle and the activity of back muscles during simple flexion/relaxation task for an improved assessment of LBP (Low Back Pain). The application of the proposed instrumentation and signal processing is evaluated in three healthy subjects and in two individuals with LBP.The presented data analysis indicates that angle velocity might be a promising parameter in a combination with electromyography profiles for differentiation between healthy subjects and in individuals with LBP for improved medical diagnostics and assessment. / Bol u leđima je jedan od najčešćih zdravstvenih problema ljudi u celom svetu. Za ocenu oporavka pacijenata sa bolom u leđima i dijagnostiku u upotrebi su deskriptivne medicinske skale. Međutim, kako bi se kvantitativno ocenio oporavak pacijenata neophodno je definisati odgovarajuće fiziološke i kinematičke mere. One su značajne kako bi se obezbedila neophodna ponovljivost i pouzdanost ocene za njihovu primenu u kliničkoj praksi i u kućnim uslovima. U ovom radu predloženo je jednostavno merenje elektromiografskih (EMG) signala mišića leđa i merenje ugla trupa tokom fleksije/ekstenzije trupa. Primena predložene instrumentacije i rutina za procesiranje signala testirani su merenjima na tri zdrava ispitanika i na dva ispitanika sa bolom u leđima. Predložena analiza izmerenih signala pokazala je da bi se za kvantitativnu ocenu pacijenata i zdravih ispitanika mogla koristiti brzina fleksije/esktenzije trupa u kombinaciji sa profilima aktivacije mišića trupa. Rezultati testiranja pokazali su da je analizom trajektorija ugla u trupu, brzine trupa i mišićnih profila moguće razlikovati zdrave i bolne profile fleksije trupa, što bi značajno unapredilo današnju medicinsku dijagnostiku. / Боли в спине одна из самых распространенных заболеваний во всем мире. Для установления диагноза и прогноза реабилитации пациентов применяется описательные шкалы. Но для достоверной количественной оценки реабилитации пациентов необходимо определить подходящие физиологические и кинематические меры. Такие меры важны для обеспечения точности оценки, и могут применяться как в клинических, так и в домашних условиях. В статье рассматривается обыкновенное электромиографическое исследование (ЭМГ) сигналов мышц спины и измерение угла туловища при сгибании/разгибании позвоночника. Был проведен эксперимент по предложенным процедурам измерения сигнала на трех здоровых людях и 2-х пациентах с болями в спине. Проведенный эксперимент измерения сигнала показал, что на точность количественной оценки больных и здоровых обследуемых влияет скорость сгибания/разгибания позвоночника, и свойства активированных мышц туловища. Результаты исследования показали, что при анализе траектории угла туловища, скорости сгибания/разгибания, и профиля мышц, возможно различить здоровые профили сгибания туловища от больных. Применение данных результатов могло бы значительно улучшить эффективность современной медицинской диагностики.

Objective: To examine the lumbar facet joint kinematics in vivo during dynamic lifting and the effects of the load lifted.Background: Although extensive efforts have been dedicated to investigating the risk factors of low back pain (LBP) associated with load handling in the workplace, the biomechanics of lumbar facet joints during such activities is not well understood.Method: Fourteen healthy participants performed a load-lifting task while a dynamic stereo-radiography system captured their lumbar motion continuously. Data from 11 participants were included for subsequent analysis. A randomized block design was employed to study the load effect (4.5 kg, 9.0 kg, and 13.5 kg) on bilateral facet joint motions at approximately 60°, 40°, 20°, and 0° trunk-flexion postures. The facet orientations were also examined.Results: Significant load effects were found for the flexion and lateral bending and superior-inferior translation of the facet joints. The L5-S1 displayed greater lateral bending and twisting, which was due to its more posterolateral orientation than the L2-L3, L3-L4, and L4-L5 facet joints. The left-right asymmetry in facet orientation was observed, most prominently at L3-L4 and L5-S1 facet joints.Conclusion: The lumbar facet joint kinematics are affected by the magnitude of the lifted load and are dependent on the orientations of articulating adjacent facets.Application: This study provided new insights into the role of lumbar facet joints in vivo during lifting. This study aimed to determine if trunk posture during walking is related to increases in rectal temperature (Tre). 24 males treadmill walked in one of four conditions (1): 30 min at 3.0 mph and 0% grade, 20 °C and 50% relative humidity (RH), wearing healthcare worker (HCW) PPE; (2): 30 min at 3.0 mph and 0% grade, 27.5 °C and 60% RH, HCW PPE; (3): 30 min at 3.0 mph and 0% grade, 32.5 °C and 70% RH, HCW PPE; and (4): 40 min at 40% VO2max, 30 °C and 70% RH, wearing firefighter PPE. Trunk posture (Zephyr BioHarness 3) and Tre were measured continuously. Tre was positively related to trunk posture, controlling for covariates (B = 3.49, p < .001). BMI and age moderated this relationship (Tre×age, B = 0.76, p < .001; Tre*BMI, B = −1.85, p < .001). Trunk posture measurement may be useful in monitoring fall potential and magnitude of heat stress of workers in hot environments. Practitioner Summary: Occupational hyperthermia increases worker risk for heat illness and injury but is difficult to monitor in the field. This investigation shows that trunk posture is independently and positively related to core temperature. Background It has been documented that pregnancy-related increased connective tissue laxity may persist postpartum; however, it is still unclear for how long. This longitudinal study aimed to compare total trunk forward flexion mobility in women between their first trimester of pregnancy and at 2- and 6-month postpartum follow-ups. We also searched for a correlation between women's trunk flexibility and their postural stability in the sagittal plane. Methods Seventeen healthy women participated in the study. Data were collected at their 7–12 weeks gestation appointments and at 6–10 and 25–28 weeks postpartum. At each session, the women performed a finger floor distance test, and data were collected on their waist circumference and BMI. The women's center of foot pressure mean velocity in the anterior-posterior direction was computed from 30-s long quiet-standing trials on a stationary force plate. Findings Total trunk forward flexion mobility was significantly higher at 2 and 6 months postpartum compared to that in early pregnancy ( P  < 0.05). At 6 months postpartum, a moderate negative correlation between finger floor distance test values and their anterior-posterior center of foot pressure mean velocity was observed ( r  = −0.6, P  < 0.05). Interpretation Increased total trunk flexibility may be present in women 6 months postpartum. Lower back pain (LBP) is a condition with high prevalence and high cost both in the United States and around the world. The magnitude of mechanical loading on spine is strongly associated with the occurrence of LBP. Previously, to assess spinal loading, biologically assisted biomechanical models were developed to estimate trunk muscle contraction forces. Loadings on lumbar passive tissues are estimated using anatomical models. However, despite the substantial individual variability in lumbar ligament geometry and viscoelastic properties, the existing anatomical models do not account for these differences. As such, the accuracy of model prediction is compromised especially when mid to full range of trunk motions are involved. This paper describes a new modeling approach to assess lumbar passive tissue loading with the consideration of individual differences in lumbar passive tissue properties. A data set that has trunk bending data from 13 human participants was analyzed; on average, lumbar passive tissue contributes to ∼89% of the total spinal compression force at fully flexed trunk postures; the estimated spinal tissue loadings were in feasible ranges as reported from previous cadaver studies; the estimated spinal loadings were also mostly in agreement with results from previous in vivo studies. Barbell back squats are a popular exercise for developing lower extremity strength and power. However, this exercise has potential injury risks, particularly to the lumbar spine, pelvis, and hip joint. Previous literature suggests heel wedges as a means of favorably adjusting trunk and pelvis kinematics with the intention of reducing such injury risks. Yet no direct biomechanical research exists to support these recommendations. Therefore, the purpose of this study was to examine the effects of heel wedges compared with barefoot on minimally loaded barbell back squats. Fourteen trained male participants performed a barbell back squat in bare feet or with their feet raised bilaterally with a 2.5-cm wooden block while 3-dimensional kinematics, kinetics, and electromyograms were collected. The heel wedge condition elicited significantly less forward trunk flexion angles at peak knee flexion, and peak external hip joint moments (p # 0.05) compared with barefoot conditions. However, no significant differences were observed between conditions for trunk and pelvis angle differences at peak knee flexion (p > 0.05). Lastly, no peak or root mean square differences in muscle activity were elicited between conditions (p > 0.05). Our results lend support for the suggestions provided in literature aimed at using heel wedges as a means of reducing excessive forward trunk flexion. However, the maintenance of a neutral spine, another important safety factor, is not affected by the use of heel wedges. Therefore, heel wedges may be a viable modification for reduction of excessive forward trunk flexion but not for reduction in relative trunk-pelvis flexion during barbell back squats.
