Inherited disorders of fibrinogen are rare and can be quantitative (hypofibrinogenemia), qualitative (dysfibrinogenemia), or both (hypodysfibrinogenemia). Moreover, variants in fibrinogen have been described that associated with a bleeding, thrombotic, or combined phenotype. Although several genetic variations leading to hypodysfibrinogenemia have been identified, few clinically described cases have been molecularly characterized. In this report we describe a 16-year-old female who presented with an unprovoked portal vein thrombosis. At the time of presentation, she appeared to have hypofibrinogenemia based on quantitation using the Clauss method. Immunoreactive testing demonstrated in a discrepancy between fibrinogen antigen and activity level consistent with dysfibrinogenemia. Single gene sequencing of FGA, FGB, and FGG revealed a heterozygous mutation in FGB, the gene encoding the fibrinogen beta chain (p.Tyr356Cys;). This variant (fibrinogen Villeurbanne) was previously described in a family with a clinical diagnosis of hypofibrinogenemia and a bleeding phenotype. This variant results in an unpaired cysteine residue near the beta chain hole, potentially resulting in abnormal protein synthesis or folding leading to intracellular protein degradation, protein instability within circulation, and/or abnormal polymer formation. It is also conceivable that this mutation could lead to aggregate formation via this unpaired cysteine between individual fibrinogen molecules or between fibrinogen and other proteins. To better characterize this fibrinogen, we generated fibrin clots using purified fibrinogen from the patient and normal control. Clot structures were analyzed using scanning electron microscopy. The patient's fibrin clots, as compared to the control, were poorly formed, consisting of long, thin fibers with little bundling and irregular spaced branch points. These results indicate that the mutant fibrinogen is sufficiently stable in circulation to significantly impair polymer formation. Albumin is the most prevalent plasma protein and is known to have an unpaired cysteine residue, making it a reasonable candidate to aggregate with the mutant fibrinogen. Plasma from the patient and control samples underwent co-immunoprecipitation followed by Western blot analyses in order to determine if we could detect albumin aggregation with the mutant fibrinogen. We found evidence of interaction between the mutant fibrinogen and albumin that was not observed in normal controls, suggesting that this patient's fibrinogen forms aggregates with albumin. These findings contradict a previous conclusion that fibrinogen Villeurbanne results in hypofibrinogenemia, and indicate that it is a hypodysfibrinogenemia. Moreover, our laboratory findings together with the patient's presentation of an unprovoked portal vein thrombosis suggest the possibility that this mutant fibrinogen may, in some contexts, lead to pathological thrombosis. Для лечения диафизарных переломов используют методы фиксации, при которых образуются конструкции «отломки – фиксатор» с различными механическими свойствами и диапазоном перемещений отломков.Цель: обосновать концепцию влияния перемещений отломков на процесс формообразования регенерата при лечении диафизарных переломов ортезом или стержневым аппаратом (АВФ).Методы: перемещение отломков с помощью ультрасонографии оценено у 18 больных с диафизарными переломами плечевой (9), бедренной (5) и большеберцовой (5) костей. Среди них 8 пострадавших со свежими переломами плечевой кости лечили консервативно с использованием ортеза, у 5 со свежими переломами бедренной кости и костей голени использовали АВФ, 6 пациентов были с несросшимися переломами после металлоостеосинтеза.Результаты: установлено, что отломки имеют многовекторную траекторию перемещений в пространстве. У больных со свежими переломами плечевой кости, которым применяли ортез, линейные перемещения концов отломков были в пределах 1,1–6 мм (10–40 % от исходного расстояния), амплитуда угловых составила 2°–8°. При фиксации АВФ линейные перемещения отломков до 2 мм (13 %) выявлены только у больных с переломом бедренной кости. В случае несросшихся переломов под действием осевой нагрузки появлялась или увеличивалась (на вершине деформации на 37–61 %) угловая деформация сегмента.Выводы: механические свойства фибрин-клеточно-коллагеновой бластемы позволяют ей деформироваться на растяжение без негативных последствий в пределах 13–40 %. Подвижность отломков в первые три недели после диафизарного перелома вызывает деформацию и напряжение бластемы, что обуславливает в дальнейшем структуру полноценного периостального костного регенерата. Methods: dislocation of the bone fragments using ultrasonography studied in 18 patients with diaphyseal fractures of humerus (9), femur (5) and tibia (5). Eight patients out of them with acute humeral fractures were treated conservatively with orthosis, in 5 patients with acute femoral and tibial fractures ARD was used, 6 patients had nonunion after osteosynthesis. Results:it is found that bone fragments have a multidirectional path of movement in space. In patients with acute humeral fractures treated with orthosis, linear displacement of the fragments end ranged 1.1–6 mm (10–40 % of initial distance), angular amplitude was 2°–8°. In ARD fixation linear fragment displacement (2 mm — 13 %) found only in patients with femoral fractures. In a cases of nonunion angular skeletal deformation appeared and increased under the axial load (apical deformity angel 13–40 %). The mobility of bone fragments in the first three weeks after diaphyseal fracture causes deformation and strain blastema, which results in the future structure of the high-grade periosteal bone regenerate. Nonunion of diaphyseal fracture fragments mobility does not provide a stimulating and shaping effect.

Introduction: The final events of the activated clotting cascade include the conversion of fibrinogen to fibrin and stabilization of the growing fibrin clot by factor XIII (FXIII). A missense variant in the alpha chain of fibrinogen, α-fibrinogen (FGA) Thr312Ala (rs6050, NM_000508.3:c.991A>G, NP_000499.1:p.[Thr331Ala]), has been previously shown to interfere with FXIII-dependent cross-linking, leading to stiffer blood clots. It is believed this pathophysiology underlies the increased risk of venous thromboembolism (VTE) associated with the variant. The Thr312Ala variant has been shown to be significantly associated with VTE among White, Non-Hispanic populations. The only investigation of the variant in a Black population (Rasmussen-Torvik et al 2007) found no significant relationship between Thr312Ala and VTE, but the authors cite the relatively small number of participants included in the analysis as a possible reason for a null finding. We investigated the association between FGA Thr312Ala and VTE in a case-control study enrolling a relatively large racially diverse study population. Methods: Conditional logistic regression was used to assess the association between FGA Thr312Ala and VTE among enrollees in the Genetic Attributes and Thrombosis Epidemiology (GATE) study, a case-control study enrolling 1,042 VTE cases and 1,213 controls with no history of VTE, frequency-matched on age, sex, and race. Crude models conditioned on the matching factors, and adjusted models conditioned on the matching factors and controlled for patient characteristics found to be associated with both case/control status and FGA Thr312Ala genotype. Among all patient characteristics evaluated, the β-fibrinogen (FGB) promoter region variant -455 G>A (rs1800790, NM_001184741.1:c.-463G>A) was the only patient characteristic associated with both case/control status and FGA Thr312Ala genotype. Potential effect modification was assessed using likelihood ratio tests. Results: Among 660 White controls and 528 White cases in the GATE study, carrying at least one FGA rs6050 G (Ala) allele was associated with increased odds of VTE compared to not carrying a G allele (odds ratio (OR) and 95% confidence interval (CI) 1.2 [1.0-1.5]) in a crude model. This association was strongest when restricting to n=205 idiopathic (unprovoked) cases, n=125 recurrent cases, or n=112 cases exhibiting both deep vein thrombosis (DVT) and pulmonary embolism (PE) (Table 1). This association was modified by obesity status, with the effect among obese enrollees being multiplicative (Table 2). In contrast, there was no association between carrying at least one FGA rs6050 G (Ala) allele and VTE among 553 Black controls and 514 Black cases in the GATE study (OR and 95% CI 1.0 [0.8-1.2]) when assessing the association among all cases (Table 1). However, the odds of VTE was higher among Black participants carrying the G allele compared to those not carrying the allele when restricting to n=103 PE only cases (OR and 95% CI 1.5 [1.0-2.4]). The association between FGA Thr312Ala and VTE did not appear to be modified by obesity status among Black participants (Table 2). Conclusions: These results indicate that the FGA Thr312Ala (rs6050, NM_000508.3:c.991A>G, NP_000499.1:p.[Thr331Ala]) polymorphism is associated with VTE among White participants in the GATE study, with the strongest effect seen in participants exhibiting both DVT and PE. However, the association among Black participants appears limited to PE, indicating further exploration of genetic risk factors for VTE in Black populations is warranted in order to better understand genetic risk factors for all types of VTE in this population. Chloride intracellular channel 1 (CLIC1) is a member of a family of six highly homologous membrane proteins (CLIC1-6), which have been shown to be co-regulated with integrins suggesting their involvement in cell adhesion, migration and proliferation. CLIC1 is a metamorphic protein that functions as an oxidoreductase in the cytoplasm as well as an ion channel in the cell membrane. CLIC1 is upregulated in angiogenic endothelial and metastatic tumor cells. In addition, studies in CLIC1 knockout mice have shown that CLIC1 promotes platelet function. Here, we hypothesize that CLIC1 supports cell adhesive functions in platelets as well as endothelial cells and in doing so mediates critical vascular biological processes such as thrombus formation, vascular repair and angiogenesis. To assess the role of CLIC1 in endothelial cells, we monitored human umbilical venous endothelial cells (HUVEC) after transfection with CLIC1 Smartpool® or non-targeted siRNA by phase contrast microscopy. This experiment revealed a significant anti-proliferative effect in CLIC1 knockdown cells in conventional 2-dimensional culture that coincided with reduced cell spreading. The deficit in spreading was reiterated after embedding the CLIC1-depleted HUVEC in a 3-dimensional matrix of fibrin clot and this loss in anchorage led to a dramatic increase in cell death. Immunocytochemistry and fluorescence microscopy of HUVEC 10 minutes after initial cell attachment showed strong CLIC1 expression in nascent lamellipodia, where it colocalized with F-actin. Recruitment of CLIC1 to the cell membrane was associated with lamellipodia formation and was lost at later stages of adhesion when HUVEC began to generate stress fibers. CLIC1 cell membrane expression was inhibited after knocking down integrin β3 or treating HUVEC with the small molecule CLIC1 inhibitor IAA94. Notably, inhibiting CLIC1 or integrin β3 caused a significant reduction of F-actin assembly in lamellipodia suggesting that both molecules cooperate during the initial phase of cell spreading. Paralleling our experiments with HUVEC, we observed that CLIC1 co-localizes with F-actin in the leading edges as well as in the cortical ring of newly attached platelets. Moreover, using flow cytometry, we detected robust CLIC1 cell surface expression in platelets after treatment with ADP, which was enhanced after addition of the β3 integrin ligand fibrinogen and reduced when we blocked integrins with peptides representing the integrin recognition motif RGD. These data suggest a close coordination of CLIC1 membrane recruitment with the adhesive machinery in activated platelets. Moreover, inhibition of CLIC1 relocation to the platelet membrane with IAA94 hindered activation of integrin αIIbβ3, which resulted in impaired platelet aggregation. Lastly, we tested whether CLIC1 inhibition has antithrombotic effects in vivo. To this end, we assessed vasoocclusion in a murine dorsal skinfold chamber model where blood flow was monitored by intravital fluorescence microscopy. Intraperitoneal injections of mice with the CLIC1 inhibitor IAA94 (20mg/kg) 19 hours and 1 hour before the photochemical induction of thrombus formation caused a significant delay in vaso-occlusion in response to CLIC1 inhibition compared to vehicle-treated controls indicating that CLIC1 inhibition has a significant anti-thrombotic effects in vivo. Together, our data show that CLIC1 cooperates with integrins during cell adhesion and as such mediates functions related to platelet homeostasis, thrombus formation, and angiogenesis. Ділянку перелому досліджували за допомогою методів гістології через 5 год, 1, 4 і 15 діб. Вивчено навколовідламковий простір у 43 постраждалих із закритими переломами довгих кісток кінцівок, яким виконували відкриту репозицію відламків. Ретроспективно оцінено клінічний матеріал 546 постраждалих з діафізарними переломами кісток кінцівок, накопичений за останні 30 років.Результати: доведено, що фібрин-кров’яний згусток є закономірно необхідною структурою для проліферації та диференціації клітин і утворення нової кісткової тканини. У результаті проведення УЗД встановлено, що форма та локалізація кісткового регенерату відповідає формі й локалізації фібрин-кров’яного згустку. На підставі дослідження кутової рухомості кісткових відламків та утворення кісткового регенерату встановили терміни загоєння переломів плечової кістки в період від 4 до 8 тижнів, стегнової — від 8 до 12, кісток гомілки — від 8 до 17.Висновок: як першу важливу стадію регенерації кістки запропоновано виділити формування фібрин-кров’яного згустку, що заповнює повністю або частково навколовідламковий простір. The process is divided into stages between which it is impossible to draw a clear line. Objective: To identify key clinical and morphological phenomena that define the staging process of fragments fusion after the bone fracture, to clarify the essence of the first stage, to set the dates of each stage in humans. Methods: Experiments were performed in 8 adult Karakul breed sheep weighing 35–40 kg underwent a closed fracture of the right femur. Plot fracture examined by histological methods after 5 hours, 1, 4 and 15 days. Near fragmental space studied in 43 patients with closed fractures of long bones which was performed open reduction of bone fragments in. Retrospectively we evaluated clinical data in 546 patients with diaphyseal fractures of the extremities accumulated over the past 30 years. Results: it is proved that the fibrin blood clot is naturally necessary structure for proliferation and differentiation of cells and for the formation of new bone tissue. After ultrasound examination it was found that the shape and location of bone regenerate conforms to the shape and location of the fibrin blood clot. Based on the study of angular mobility of bone fragments and bone regenerate forma­tion we established terms of healing of fractures in humerus in the period from 4 to 8 weeks, in femur — from 8 to 12, and in shin bones — from 8 to 17 weeks. Conclusion: There were proposed to highlight formation of a fibrin blood clot as the first important step of bone regeneration which completely or partially fills near fragmental space. Approximate lifetime formation and stabiliza­tion of its structure is from 12 to 24 hours. There were established terms of the stages of healing of diaphyseal fractures in human long bones.
