Abstract Efficient wastewater treatment monitoring is vital for addressing water scarcity. Microbial fuel cells (MFCs) have emerged as real-time biosensors for biochemical oxygen demand (BOD) in urban wastewater. Discrepancies in signal generation may arise due to changes in the composition and metabolism of mixed-culture electroactive biofilms stemming from different wastewater compositions. In this study, 3D-printed MFC-based biosensors were employed to assess the BOD of sterile complex artificial wastewater and untreated urban wastewater. Alterations in the microbial composition of the anode were evaluated using 16S rRNA sequencing and metagenomics analysis. Results show that MFC-based biosensors can be effectively recalibrated for diverse types of wastewater, maintaining consistent sensitivity (0.64 ± 0.10 mA L mg−1 m−2 with synthetic wastewater and 0.78 ± 0.13 mA L mg−1 m−2 with urban wastewater) and limit of detection (49 ± 8 mg L−1 for synthetic wastewater and 44 ± 7 mg L−1 for urban wastewater). Crucially, pre-sterilization, conductivity adjustments, and nitrogen purging of wastewater are not required before its introduction into the biosensor. However, the presence of native aerobic microorganisms in the wastewater might affect the current output. Metagenomics and taxonomic analyses revealed that the alterations in biofilm composition are predominantly in response to the varied chemical and microbiological compositions of different substrates. Despite variations in anodic biofilm composition, the MFC-based biosensor maintains a relative error comparable to the standard BOD test. This highlights the resilience and flexibility of the biosensor when directly used with a variety of wastewater types before full biofilm adjustment.

IntroducionFungal melanin is an underexplored natural biomaterial of great biotechnological interest in different areas. This study investigated the physical, chemical, electrochemical, and metal-binding properties of melanin extracted from the metallotolerant black fungus Exophiala mesophila strain IRTA-M2-F10.Materials and methodsSpecific inhibitory studies with tricyclazole and biochemical profiling of whole cells by synchrotron radiation-based Fourier-transform infrared spectral microscopy (SR-FTIRM) were performed. An optimized extraction protocol was implemented, and purified fungal melanin was characterized using an array of spectrophotometric techniques (UV-Vis, FTIR, and EPR) and by cyclic voltammetry (CV) experiments. The metal-binding capacity of melanin extracts was also assessed by using Cr(VI) as a model heavy metal.ResultsInhibitory studies indicated that 1,8-dihydroxynaphthalene may be the main precursor molecule of E. mesophila melanin (DHN-melanin). The biochemical characterization of fungal melanin extracts were benchmarked against those from two melanins comprising the precursor molecule L-3,4-dihydroxiphenylalanine (DOPA-melanin): extracts from the ink of the cephalopod Sepia officinalis and DOPA-melanin synthesized in the laboratory. The CV results of melanin extracts incubated with and without cell suspensions of the electroconductive bacterium Geobacter sulfurreducens were indicative of novel semiquinone/hydroquinone redox transformations specific for each melanin type. These interactions may play an important role in cation exchange for the adsorption of metals and in microbial interspecies electron transfer processes.DiscussionThe obtained results provided further evidence for the DHN-nature of E. mesophila melanin. The FTIR profiling of melanin extracts exposed to Cr(VI), compared to unexposed melanin, resulted in useful information on the distinct surface-binding properties of fungal melanin. The parameters of the Langmuir and Freundlicht isotherms for the adsorption of Cr(VI) were determined and compared to bibliographic data. Altogether, the inherent properties of fungal melanin suggest its promising potential as a biomaterial for environmental applications.

Bioelectrochemical processes are emerging as one of the most efficient and sustainable technologies for wastewater treatment. Their application for industrial wastewater treatment is still low due to the high toxicity and difficulty of biological treatment for industrial effluents. This is especially relevant in pharmaceutical industries, where different solvents, active pharma ingredients (APIs), extreme pH, and salinity usually form a lethal cocktail for the bacterial community in bioreactors. This work evaluates the impact of the anode architecture on the detoxification performance and analyzes, for the first time, the profile of some key bioremediation enzymes (catalase and esterase) and reactive oxygen species (ROS) during the operation of microbial electrochemical cells treating real pharmaceutical wastewater. Our results show the existence of oxidative stress and loss of cell viability in planktonic cells, while the electrogenic bacteria that form the biofilm maintain their biochemical machinery intact, as observed in the bioelectrochemical response. Monitorization of electrical current flowing in the bioelectrochemical system showed how electroactive biofilm, after a short adaptation period, started to degrade the pharma effluent. The electroactive biofilms are responsible for the detoxification of this type of industrial wastewater.

Abstract Microbial electrosynthesis (MES) constitutes a bioelectrochemical process where bacteria uptake electrons extracellularly from a polarized electrode to incorporate them into their anabolic metabolism. However, the efficiency of current MES reactor designs can be lower than expected due to limitations regarding electron transfer and mass transport. One of the most promising bioreactor configurations to overcome these bottlenecks is the Microbial Electrochemical Fluidized Bed Reactor (ME‐FBR). In this study, microbial CO2 fixation is investigated for the first time in a ME‐FBR operated as a 3‐phase reactor (solid–liquid–gas). An electroconductive carbon bed, acting as a working electrode, was fluidized with gas and polarized at different potentials (−0.6, −0.8 and −1 V vs. Ag/AgCl) so it could act as an electron donor (biocathode). Under these potentials, CO2 fixation and electron transfer were evaluated. Autotrophic electroactive microorganisms from anaerobic wastewater were enriched in a ME‐FBR in the presence of 2‐bromoethanosulfonic acid (BES) to inhibit the growth of methanogens. Cyclic voltammetry analysis revealed interaction between the microorganisms and the cathode. Furthermore, volatile fatty acids like propionate, formate and acetate were detected in the culture supernatant. Acetate production had a maximum rate of ca. 1 g L−1 day−1. Planktonic cell biomass was produced under continuous culture at values as high as ca. 0.7 g L−1 dry weight. Overall, this study demonstrates the feasibility of employing a fluidized electrode with gaseous substrates and electricity as the energy source for generating biomass and carboxylic acids.

Abstract The COVID-19 pandemic remains a significant global health threat, with uncertainties persisting regarding the factors determining whether individuals experience mild symptoms, severe conditions, or succumb to the disease. This study presents an NMR metabolomics-based approach, analysing 80 serum and urine samples from COVID-19 patients (34 intensive care patients and 46 hospitalized patients) and 32 from healthy controls. Our research identifies discriminant metabolites and clinical variables relevant to COVID-19 diagnosis and severity. These discriminant metabolites play a role in specific pathways, mainly “Phenylalanine, tyrosine and tryptophan biosynthesis”, “Phenylalanine metabolism”, “Glycerolipid metabolism” and “Arginine and proline metabolism”. We propose a three-metabolite diagnostic panel—comprising isoleucine, TMAO, and glucose—that effectively discriminates COVID-19 patients from healthy individuals, achieving high efficiency. Furthermore, we found an optimal biomarker panel capable of efficiently classify disease severity considering both clinical characteristics (obesity/overweight, dyslipidemia, and lymphocyte count) together with metabolites content (ethanol, TMAO, tyrosine and betaine).

BackgroundLatin American youth have a high prevalence of mental health disorders and face major socioeconomic and public safety problems. This study assesses the association between stressful life events (SLEs) and depression, anxiety, and quality of life among adolescents and young adults from deprived neighbourhoods of Latin America.MethodsThe sample consisted of 2,402 participants, between adolescents (15–16 years) and young adults (20–24 years), from Bogotá (Colombia), Buenos Aires (Argentina), and Lima (Peru), assessed in 2021–2022 and recruited in education and community settings and social media. We evaluated the most frequent recent and distant SLEs (occurred in the previous year and more than a year ago, respectively), the relationship between SLEs and severity of depression (PHQ-8), anxiety (GAD-7), and quality of life (MANSA), and we tested for differences by gender and age group.ResultsThe most common recent and distant SLEs were related to public safety issues and financial distress. Accidents and school suspensions were more frequent among men, and sexual harassment and bullying among women. Every additional reported recent SLE increased the odds of mild, moderate, and severe depression (18, 17, and 25%, respectively) and anxiety (10, 17, and 21%, respectively) symptoms. Similar trends were found with distant SLEs and depression (8, 9, and 11% for mild, moderate, and severe symptoms, respectively) and anxiety (9, 11, and 12%, respectively). Abstract. Abdominoscrotal hydrocele (ASH) is a rare clinical finding comprising fluid collection between the layers of the tunica vaginalis, extending from the scrotum to the abdominal cavity. At present, there is no unique or recommended management for ASH, and different surgical treatments have been proposed. Despite an open surgical approach being the most common treatment, the use of laparoscopy has also previously been described. The most common intraoperative complication is devascularization of the testis due to damage to the spermatic cord, with consequent orchiectomy. We present a case of ASH treated with minimally invasive surgery, consisting of a right inguinotomy with puncture of the ASH by positioning a mono-J stent avoiding spermatic cord dissection and the risk of testis devascularization. Sclerotization of the hydrocelic sac with iodopovidone through a mono-J stent was performed with healing from ASH and preservation of testicular vascularization. Two months later, magnetic resonance imaging showed the presence of scar tissue replacing the previous ASH cavity.

Abstract: This study aimed to estimate the population-based Parkinson disease prevalence, and to explore potentially associated factors and conditions. A population-based survey was conducted in Northern Peru. Symptoms compatible with Parkinson’s were defined using a validated Spanish questionnaire (≥ 42 points suggest Parkinson’s). Potential factors (e.g., age, sex, etc.) and clinical conditions (e.g., depressive symptoms, perceived stress, etc.) associated with Parkinson’s were assessed. In total, 1,609 subjects were included, mean age of participants was 48.2 (SD: 10.6), and 810 (50.3%) were women. Parkinson’s prevalence was 1.6% (95%CI: 1.0; 2.4). Those aged ≥ 55 years, and those who reported using wood as fuel for household cooking had a Parkinson’s prevalence from 3.5 to 4 times greater than those who did not. The presence of depressive symptoms, anxiety symptoms, perceived stress, poor sleep quality, and cognitive impairment was more common among those with Parkinson’s, and quality of life in these participants was lower than those without Parkinson’s. In conclusion, 1.6% of the population shows symptoms compatible with Parkinson’s. Age and use of wood for household cooking were factors associated with Parkinson’s. Several mental health conditions and lower quality of life were more frequent among those with Parkinson’s. Appropriate strategies are required to detect, prevent, and manage Parkinson’s cases.

Background The prevalence of overweight and obesity has increased fastest in low- and middle-income countries in the last decades. Together with this rising prevalence, pharmacological and surgical interventions for obesity have emerged. How many people need these treatments is unknown. We quantified the prevalence of people in need of pharmacological and surgical treatment for obesity in Peru between 2014 and 2022. Methods Repeated cross-sectional analysis of national health surveys in Peru was conducted. Eligibility for pharmacological treatment for obesity was: body mass index (BMI) ≥30 kg/m2 or BMI ≥27 kg/m2 alongside type 2 diabetes or hypertension (self-reported). Eligibility for bariatric surgery were BMI ≥40 kg/m2 or BMI between 35 to 39.9 kg/m2 linked to weight-related health problems. We used Poisson regressions to identify associated factors with eligibility for obesity management. Results Across years, 260,131 people (mean age 44.0 and 54.5% were women) were studied, 66,629 (27.7%; 95% CI: 27.4% - 28.1%) were eligible for obesity medication, and 5,263 (2.5%; 95% CI: 2.4% - 2.6%) were eligible for bariatric surgery. Female sex, older age, higher socioeconomic level and study year were associated with higher probability of eligibility for both obesity medication and bariatric surgery. Conclusions Eligibility for obesity management has increased over time in Peru. There is a need to strengthen policies to tackle overweight and obesity in Peru, acknowledging that some individuals may benefit from pharmacological and surgical interventions.

Abstract Background Peru is facing a double burden of malnutrition (DBM), characterized by the co-existence of undernutrition and overnutrition. Double-duty actions that concurrently target common drivers of undernutrition and overnutrition, while ensuring no unintended side effects, are recommended to effectively address the DBM. To understand these complex common mechanisms and design context-specific double-duty actions, there is a need for participatory systems approaches. This paper provides a case study of applying a community-based system dynamics approach to capture stakeholder perspectives of food system drivers of the DBM in two regions in Peru. Methods We implemented a multi-stage community-based system dynamics approach, which included processes for research capacity building for systems approaches, and the designing, piloting, and implementation of stakeholder workshops. A total of 36 stakeholders, representing diverse perspectives, participated in five group model building workshops. Stakeholder views are presented in a causal loop diagram that showcases the feedback mechanisms between key food system drivers of overweight and stunting in Peru. Results The causal loop diagram highlights that prioritization of undernutrition over overnutrition in the policymaking process, due to Peru’s historically high levels of undernutrition, may undermine action against the DBM. It also describes potential mechanisms of unintended impacts of undernutrition policies on the DBM in Peru, including impacts related to within-family distribution and quality of food provided through food assistance programs, and unintended impacts due to regional dynamics. Conclusions This paper highlights the importance of a participatory approach to understand local needs and priorities when recommending double-duty actions in Peru and shares practical methodological guidance on applying participatory systems approaches in public health.

Abstract Background Point-of-care testing (POCT) devices are diagnostic tools that can provide quick and accurate results within minutes, making them suitable for diagnosing non-communicable diseases (NCDs). However, these devices are not widely implemented in healthcare systems and for this reason is relevant to understand the implementation process. Aim To describe the process and define a strategy to implement a multiparameter POCT device for diagnosing and managing NCDs in one region of Peru. Methods A descriptive and non-experimental study, using the participatory methodologies of co-creation process. It was conducted in one region of Peru (Tumbes) to design an intervention for implementing a multiparameter POCT device. Two co-creation sessions were conducted involving five groups: community members, primary healthcare workers, these groups in both rural and urban settings, and regional decision-makers. These sessions included activities to understand patient journeys in receiving care for NCDs, identify facilitators and barriers to POCT devices usage, and define an implementation strategy for POCT devices in both rural and urban settings of Tumbes. The research team analysed the data and summarized key topics for discussion after each session. Results A total of 78 participants were enrolled across the five groups. Among community members: 22.2% had only diabetes, 24.1% had only hypertension, and 18.5% had both diagnoses. In the patient journey, community members mentioned that it took at least three days to receive a diagnosis and treatment for an NCD. Most of the participants agreed that the POCT devices would be beneficial for their communities, but they also identified some concerns. The strategy for POCT devices implementation included healthcare workers training, POCT devices must be placed in the laboratory area and must be able to perform tests for glucose, glycated haemoglobin, cholesterol, and creatinine. Advertising about POCT devices should be displayed at the healthcare centres and the municipality using billboards and flyers. Conclusions The co-creation process was useful to develop strategies for the implementation of multiparameter POCT devices for NCDs, involving the participation of different groups of stakeholders guided by moderators in both, rural and urban, settings in Peru.

Background: The spread of the coronavirus disease 2019 (COVID-19) in Peru has been reported at the regional level, few studies have evaluated its spread at the provincial level, in which the mechanisms could be different. Methods: We conducted an analytical, cross-sectional, multistage observational population study to assess the seroprevalence of SARS-COV-2 at the provincial and urban/rural levels in a high-altitude setting. The sampling unit was the household, including a randomly selected family member. Sampling was performed using a data collection sheet on clinical and epidemiological variables. Chemiluminescence tests were used to detect total anti-SARS-COV-2 antibodies (IgG and IgM simultaneously). The percentages were adjusted to the sampling design. Results: The overall prevalence in the region of Cusco was 25.9%, with considerably different prevalence between the 13 provinces (from 15.9% in Acomayo to 40.1% in Canchis) and between rural (21.1%) and urban (31.7%) areas. In multivariable model, living in a rural area was a protective factor (adjusted prevalence ratio [aPR], 0.68; 95% confidence interval [CI], 0.61–0.76). Conclusions: Geographic diversity and population density determine different prevalence rates, typically lower in rural areas, possibly due to natural social distancing or limited interaction with people at risk.

Over the last decade, various entrepreneurial school practices have proliferated, including the creation of mini-companies. In the Spanish context, the Empresa Joven Europea (EJE) program has been the most widely employed educative program in this respect. Empresa Joven Europea is an educational program that proposes that secondary education students create and manage their own mini-businesses, thereby taking the legal form of a cooperative society and facilitating the acquisition of all basic educational skills through the cultivation of entrepreneurship. Nevertheless, there is still a gap between the theory and the practice of entrepreneurship education which, together with the conceptual expansion experienced in recent years, increases the need to assess the effectiveness of this type of programs. By means of an explanatory and cross-sectional study, we evaluated the impact of the program on the configuration of the entrepreneurial potential of the participating adolescents ( N = 270). The analysis has been carried out utilizing PLS-SEM with SmartPLS software version 3.3.9 and SPSS version 25. None of the six hypotheses formulated have been confirmed, which means that the desired effects have not been achieved in any of the three variables under study: attitudes, knowledge, and entrepreneurial skills. Such results may be related to aspects of curricular organization, the type of experiences based on cooperatives, a certain structural and conceptual lack of definition of the pedagogical methodology used, and the development of skills linked to identity. Moreover, it is possible that, although it is not evident in the competencies acquired, the program has influenced the apperception of what it means to be an entrepreneur and of the creation of critical awareness, which is associated with entrepreneurial potential. Our study, in addition to opening suggestions for further research along the same lines, presents several implications related to administrative and organizational aspects, to the design of the different pedagogical elements of the programs and to the evaluative criterion adopted.

Background The co-occurrence of chronic diseases and acute infectious events exacerbates disability and diminishes quality of life, yet research in Low- and Middle-Income countries is scarce. We aimed to investigate the relationship between infectious events and multimorbidity in resource-constrained settings. Methods We conducted a sequential mixed-method study in Lima and Tumbes, Peru, with participants having multimorbidity from the CRONICAS Cohort Study. They completed a questionnaire on the occurrence, treatment, and health-seeking behaviour related to acute infectious events. Qualitative interviews explored the perceptions and links between multimorbidity and acute infectious events for a subgroup of participants. Findings Among individuals with multimorbidity, low awareness of chronic conditions and poor medication adherence. The cumulative incidence for respiratory and gastrointestinal infections, the most reported acute conditions, was 2.0 [95%CI: 1.8–2.2] and 1.6 [1.2–1.9] events per person per year, respectively. Individuals with cancer (6.4 [1.6–11.2] events per person per year) or gastrointestinal reflux (7.2 [4.4–10.1] events per person per year) reported higher cumulative incidence of infectious events than others, such as those with cardiovascular and metabolic conditions (5.2 [4.6–5.8] events per person per year). Those with three or more chronic conditions had a slightly higher cumulative incidence compared with individuals with two conditions (5.7 [4.4–7.0] vs 5.0 [4.4–5.6] events per person per year). Around 40% of individuals with multimorbidity sought healthcare assistance, while others chose drugstores or didn't seek help. Our qualitative analysis showed diverse perceptions among participants regarding the connections between chronic and acute conditions. Those who recognized a connection emphasized the challenges in managing these interactions. Interpretation Our study advances understanding of multimorbidity challenges in resource-limited settings, highlighting the impact of acute infections on patients' existing multimorbidity burden.

Using a random forest regression (RFR) machine learning technique, the critical temperature (Tc) of a superconductor was predicted in the context of Industry 4.0 in this study using features derived from the material's physico-chemical properties, containing atomic mass, electron affinity, atomic radius, valence, and thermal conductivity. The same experimental data were also fitted with multilayer perceptron (MLP) artificial neural networks (ANN), M5 model tree and multivariate linear regression (MLR) model for comparison. The current investigation's findings show that the proposed RFR–relied model can successfully forecast the critical temperature of a superconductor. Additionally, the Tc estimate was reached with a correlation coefficient of 0.9565 and a coefficient of determination 0.9146, when the observed dataset was used to test this unique technique. Additionally, the outcomes from the MLP, M5, and MLR models are obviously worse than those from the RFR–relied model. When it comes to fully comprehending the superconductivity, this investigation is noteworthy. Regarding forecasting effectiveness and feature reduction rate, the RFR approach has obvious advantages and generalizability, and it also demonstrates suitability for high-temperature superconductor Tc forecasting. In fact, it offers a practical and affordable approach to data-driven superconductor investigation.

There are few studies focused on the epidemiology of hypertensive crisis at the population level in resource-constrained settings. This study aimed to determine the prevalence and trends over time of hypertensive crisis, as well as the factors associated with this condition among adults. A secondary data analysis was carried out using the Peruvian Demographic and Family Health Survey (ENDES). Hypertensive crisis was defined based on the presence of systolic (≥ 180mmHg) or diastolic (≥ 110mmHg) blood pressure, regardless of previous diagnosis or medication use. The factors associated with our outcome were evaluated using multinomial logistic regression, and the trend of hypertensive crisis was evaluated using the Cochrane-Armitage test. Data from 260,167 participants were analyzed, with a mean age of 44.2 (SD: 16.9) years and 55.5% were women. Hypertension prevalence was 23% (95%CI: 22.7-23.4) and, among them, 5.7% (95%CI: 5.4-5.9) had hypertensive crisis, with an overall prevalence of 1.5% (95%CI: 1.4-1.6). From 2014 to 2022, a significant decrease in the prevalence of hypertensive crisis was observed, from 1.7% in 2014 to 1.4% in 2022 (p = 0.001). In the multivariable model, males, increasing age, living in urban areas, high body mass index, and self-reported type 2 diabetes were positively associated with hypertensive crisis, whereas higher educational level, socioeconomic status, and high altitude were inversely associated. There is a need to improve strategies for the diagnosis, treatment, and control of hypertension, especially hypertensive crisis.

IntroductionSarcopenia and sarcopenic obesity (SO) have emerged as significant contributors to negative health outcomes in the past decade. We aimed to estimate the prevalence of probable sarcopenia, sarcopenia, and SO in a community-dwelling population of 1151 adults aged ≥55 years in Lima, Peru.MethodsThis cross-sectional study was conducted between 2018 and 2020. Sarcopenia was defined as the presence of low muscle strength (LMS) and low muscle mass (LMM) according to European (EWGSOP2), US (FNIH) and Asian (AWGS2) guidelines. We measured muscle strength by maximum handgrip strength and muscle mass using bioelectrical impedance analyzer. SO was defined as a body mass index ≥ 30 kg/m2 and sarcopenia.ResultsThe study participants had a mean age of 66.2 years (SD 7.1), age range between 60 to 92 years old, of which 621 (53.9%) were men. Among the sample, 41.7% were classified as obese (BMI ≥30.0 kg/m²). The prevalence of probable sarcopenia was estimated to be 22.7% (95%CI: 20.3-25.1) using the EWGSOP2 criteria and 27.8% (95%CI: 25.2-30.4) using the AWGS2 criteria. Sarcopenia prevalence, assessed using skeletal muscle index (SMI), was 5.7% (95%CI: 4.4-7.1) according to EWGSOP2 and 8.3% (95%CI: 6.7-9.9) using AWGS2 criteria. The prevalence of sarcopenia based on the FNIH criteria was 18.1% (95%CI: 15.8-20.3). The prevalence of SO, considering different sarcopenia definitions, ranged from 0.8% (95%CI: 0.3-1.3) to 5.0% (95%CI: 3.8-6.3).ConclusionOur findings reveal substantial variation in the prevalence of sarcopenia and SO, underscoring the necessity for context-specific cut-off values. Although the prevalence of SO was relatively low, this result may be underestimated. Furthermore, the consistently high proportion of probable sarcopenia and sarcopenia point to a substantial public health burden.

Background The long-term impact of elevated blood pressure on mortality outcomes has been recently revisited due to proposed changes in cut-offs for hypertension. This study aimed at assessing the association between high blood pressure levels and 10-year mortality using the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC-7) and the American College of Cardiology and the American Heart Association (ACC/AHA) 2017 blood pressure guidelines. Methods Data analysis of the PERU MIGRANT Study, a prospective ongoing cohort, was used. The outcome of interest was 10-year all-cause mortality, and exposures were blood pressure categories according to the JNC-7 and ACC/AHA 2017 guidelines. Log-rank test, Kaplan-Meier and Cox regression models were used to assess the associations of interest controlling for confounders. Hazard ratios (HR) and 95% confidence intervals (95% CI) were estimated. Results A total of 976 records, mean age of 60.4 (SD: 11.4), 513 (52.6%) women, were analyzed. Hypertension prevalence at baseline almost doubled from 16.0% (95% CI 13.7%–18.4%) to 31.3% (95% CI 28.4%–34.3%), using the JNC-7 and ACC/AHA 2017 definitions, respectively. Sixty-three (6.4%) participants died during the 10-year follow-up, equating to a mortality rate of 3.6 (95% CI 2.4–4.7) per 1000 person-years. Using JNC-7, and compared to those with normal blood pressure, those with pre-hypertension and hypertension had 2.1-fold and 5.1-fold increased risk of death, respectively. Similar mortality effect sizes were estimated using ACC/AHA 2017 for stage-1 and stage-2 hypertension. Conclusions Blood pressure levels under two different definitions increased the risk of 10-year all-cause mortality. Hypertension prevalence doubled using ACC/AHA 2017 compared to JNC-7. The choice of blood pressure cut-offs to classify hypertension categories need to be balanced against the patients benefit and the capacities of the health system to adequately handle a large proportion of new patients.

Abstract Background Point-of-care testing (POCT) devices may facilitate the delivery of rapid and timely results, providing a clinically important advantage in patient management. The challenges and constraints in the implementation process, considering different levels of actors have not been much explored. This scoping review aimed to assess literature pertaining to implementation facilitators and barriers of POCT devices for the diagnosis or monitoring of cardiometabolic diseases. Methods A scoping review of the literature was conducted. The inclusion criteria were studies on the inception, planning, or implementation of interventions with POCT devices for the diagnosis or monitoring of cardiometabolic diseases defined as dyslipidemia, cardiovascular diseases, type 2 diabetes, and chronic kidney disease. We searched MEDLINE, Embase, and Global Health databases using the OVID searching engine until May 2022. The Consolidated Framework of Implementation Research (CFIR) was used to classify implementation barriers and facilitators in five constructs. Also, patient, healthcare professional (HCP), and organization level was used. Results Twenty studies met the eligibility criteria for data extraction. All studies except two were conducted in high-income countries. Some findings are: 1) Intervention: the most widely recognized facilitator was the quick turnaround time with which results are obtained. 2) Outer setting: at the organizational level, the lack of clear regulatory and accreditation mechanisms has hindered the adoption and sustainability of the use of POCT. 3) Inner setting: for HCP, performing POCT during the consultation was both a facilitator and a barrier in terms of time, personnel, and service delivery. 4) Individuals: the implementation of POCT may generate stress and discomfort in some HCP in terms of training and new responsibilities. 5) Process: for patients, it is highly appreciated that obtaining the sample was simple and more comfortable if venipuncture was not used. Conclusion This scoping review has described the facilitators and barriers of implementing a POCT device for cardiometabolic conditions using the CFIR. The information can be used to design better strategies to implement these devices and benefit more populations that have low access to cardiometabolic tests.

Abstract Stroke is a recurrent and well-known cardiovascular event and a leading cause of death worldwide. We identified reliable epidemiological evidence of stroke in Latin America and the Caribbean (LAC) and estimated the prevalence and incidence of stroke, overall and by sex, in that region. A systematic search in OVID (Medline, Embase and Global Health) and in the Latin America and Caribbean Health Sciences Literature (LILACS) until the end of 2020 was made for all cross-sectional or longitudinal studies estimating (or allowing the estimation of) the prevalence or incidence of stroke among individuals of the general population ≥ 18 years from LAC countries. No language restriction was applied. Studies were assessed for methodological quality and risk of bias. Pooled estimates were calculated using random effect meta-analysis as high heterogeneity was expected. A total of 31 papers for prevalence and 11 papers for incidence were included in the review for analysis. The overall pooled stroke prevalence was 32 (95% CI 26–38) per 1000 subjects and were similar among men (21; 95% CI 17–25) and women (20; 95% CI 16–23) per 1000 subjects. The overall pooled stroke incidence was 255 (95% CI 217–293) per 100 000 person-years, being higher in men (261; 95% CI 221–301) compared to women (217; 95% CI 184–250) per 100 000 person-years. Our results highlight the relevance of the prevalence and incidence of stroke in the LAC region. The estimates were similar in stroke prevalence by sex, but with higher incidence rates among males than females. Subgroup analyses highlight the need for standardized methodologies to obtain appropriate prevalence and incidence estimates at the population level in a region with a great burden of cardiovascular events.

Objective To assess if the duration and quality of sleep vary due to the presence of multimorbidity.

Abstract: Evidence points to a direct relationship between nutritional quality and food expenditure. However, food expenditure is highly susceptible to changes, and nutritional quality of household food presents limited evidence. The aim of this study was to assess the relationship between nutritional quality available and total food expenditure in Peruvian households, and whether there were differences by area (urban and rural) and between years of the COVID-19 pandemic. For this, we used Peru’s National Household Survey (ENAHO) from 2019 and 2020. We assessed total food expenditure in US dollars per day, whereas household nutritional quality available was assessed based on dietary diversity and compliance with the household calorie requirements, percentage of food expenditure, and potential confounders. We used the Student’s t-test, analysis of variance (ANOVA), linear regression, and the Wald test to assess the interaction effect. Households with adequate total/partial nutritional quality available by area were found to spend, on average, USD 2.00 more in urban than in rural areas and, by year, they presented 7.1% more percentage of food expenditure in 2020 than in 2019. Despite associations existing between nutritional quality available and total food expenditure by year and study area, the effect modification was only present by study area. In multivariable model, households with adequate total/partial nutritional quality available consistently presented a lower total food expenditure by year, with a lower total food expenditure in urban areas. An inverse relationship was found between nutritional quality available and total food expenditure, in contrast to the direct relationship of studies assessing dietary cost and nutritional quality. Our results reflect the nutritional deficit in the food purchases of Peruvian households.

Buddleja cordata cell suspension cultures could be used as a tool for investigating the capabilities of this species to tolerate heavy metals (HMs) and for assessing the effects of HMs on the accumulation of phenolic compounds in this species. It grows in a wide range of habitats in Mexico, including ultramafic soils, and mobilizes some HMs in the soil. The mobilization of these HMs has been associated with phenolic substances. In addition, this species is used in Mexican traditional medicine. In the present study, a B. cordata cell suspension culture was grown for 18 days in a culture medium enriched with Cu (0.03–0.25 mM), Fe (0.25–1.5 mM), Mn (0.5–3.0 mM), or Zn (0.5–2.0 mM) to determine the effects of these HMs on growth and HM accumulation. We also assessed the effects of the HMs on phenolic compound accumulation after 1 and 18 days of HM exposure. Cells were able to grow at almost all tested HM concentrations and accumulated significant amounts of each HM. The highest accumulation levels were as follows: 1160 mg Cu kg−1, 6845 mg Fe kg−1, 3770 mg Mn kg−1, and 6581 mg Zn kg−1. Phenolic compound accumulation was affected by the HM exposure time and corresponded to each HM and its concentration. Future research should analyze whole plants to determine the capabilities of Buddleja cordata to accumulate abnormally high amounts of HM and to evaluate the physiological impact of changes in the accumulation of phenolic compounds.

Tumour markers have no established role in the monitoring of the course of metastatic breast cancer during antineoplastic therapy, yet cancer antigen 15.3 (CA15.3) and carcinoembryonic antigen (CEA) are commonly used in clinical practice to aid in the early detection of progression of disease (PD). In our multicentre, prospective, real-life study, we enrolled 142 consecutive patients with advanced breast cancer receiving endocrine therapy in combination with a CDK4/6 inhibitor from January 2017 to October 2020; 75 patients had PD at the time of database closure. We measured serum marker concentrations at regular 4-month intervals together with radiological tumour response assessments and in cases of clinical suspicion of PD. Appropriate descriptive and inferential statistical methods were used to analyse serum marker level trends amongst prespecified subgroups and at specific time points (baseline, best radiologically documented tumour response and first detection of PD) in the subpopulation of patients with PD at the time of database closure. Notably, the median time from treatment initiation to best tumour response was 4.4 months. We evaluated the presence of an association between baseline CA15.3 and CEA levels and prespecified clinical characteristics but found no clinically meaningful correlation. We assessed marker level variations at the time of best radiologically documented disease response and PD: in the subgroup of patients who responded to treatment before progressing, we detected a statistically significant correlation with tumour marker variation between the time of best response and progression; this finding was not confirmed in the subgroup of patients that did not benefit from treatment. In conclusion, serum tumour marker flares can be useful in the early diagnosis of PD but should not be used as the sole factor prompting a change in treatment strategy without radiological confirmation. In February 2016 an elementary school kid from the vicinity of Ferrara (Italy) submitted, jointly with his teacher, an application to the “Accademia della Crusca”, the oldest linguistics academy in the world, to propose including the new word “petaloso”, which literally means “petal-y”, in the Italian language. The official reply from the academy, explains that «a new word does not enter the vocabulary when someone invents it, even if it is a “beautiful” and useful word. For it to enter a vocabulary, in fact, the new word must not be known and used only by those who invented it, but it’s necessary that many people use it and many people understand it.» And continues: «That's how it works: it's not the scholars, the ones who make dictionaries, who decide which new words are beautiful or ugly, useful or useless. When a new word is on everyone's (or many's) lips, then the scholar understands that that word has become a word like the others and puts it in the vocabulary.» Such news had all the perfect ingredients to not pass unobserved in online media and, after being bounced around in a few newspaper websites, it quickly spiralled up, when the Italian prime minister retweeted the hashtag #petaloso.

Objetivamos descrever as experiências da dodiscência durante o Ensino remoto emergencial (2020-2021), sendo ele a estratégia de funcionamento adotada pelas escolas devido ao isolamento social decorrente da pandemia de COVID-19. Metodologicamente, recorremos à descrição fenomenológica, que busca romper a atitude natural perante os fenômenos, voltando-nos rigorosamente para a experiência fáctica. Primeiro apresentamos as descrições da docência e depois as experiências discentes, para então, trazermos o cruzamento delas por meio da dodiscência. Concluímos que as precariedades da pandemia solaparam docentes e discentes, sendo a dodiscência uma possibilidade de abordar a complexidade desses impactos, que ainda precisam ser documentados e melhor explorados.

ObjectiveThis study aimed to conduct a process evaluation of a salt substitute trial conducted in Peru.MethodsThrough semi-structured interviews of intervention participants, we documented and analyzed process evaluation variables as defined by the Medical Research Council Framework. This study was a stepped wedge trial conducted in Tumbes, Peru in 2014. The intervention was a community-wide replacement of regular salt (100% sodium) with “Salt Liz” (75% sodium and 25% potassium) using social marketing strategies to promote the adoption and continued use of the salt substitute in daily life. The components of the social marketing campaign included entertainment educational activities and local product promoters (“Amigas de Liz”). Another component of the intervention was the Salt Liz spoon to help guide the amount of salt that families should consume. The process evaluation variables measured were the context, mechanism of action, and implementation outcomes (acceptability, fidelity and adoption, perceptions, and feedback).ResultsIn total, 60 women were interviewed, 20 with hypertension and 40 without hypertension. Regarding context, common characteristics across the four villages included residents who primarily ate their meals at home and women who were responsible for household food preparation. As the mechanism of action, most participants did not notice a difference in the flavor between regular salt and Salt Liz; those that did notice a difference took around 2 weeks to become accustomed to the taste of the salt substitute. In terms of implementation outcomes, the Salt Liz was accepted by villagers and factors explaining this acceptability included that it was perceived as a “high quality” salt and as having a positive effect on one's health. Participants recognized that the Salt Liz is healthier than regular salt and that it can help prevent or control hypertension. However, most participants could not accurately recall how the compositions of the Salt Liz and regular salt differed and the role they play in hypertension. Although the use of the Salt Liz was far-reaching at the community level, the use of the Salt Liz spoon was poor. Educational entertainment activities were well-received, and most participants enjoyed them despite not always being active participants but rather sideline observers.ConclusionThis process evaluation identifies key intervention components that enabled a successful trial. Seeking and incorporating feedback from the target population helps deepen the understanding of contextual factors that influence an intervention's success. Furthermore, feedback received can aid the development of the intervention product. Some factors that can be improved for future interventions are acknowledged.Clinical trial registrationNCT01960972.

Introducción: Se vienen dando cambios en la alimentación en los países en vías de desarrollo como parte de la transición nutricional. Estos podrían afectar el consumo de ciertos grupos de alimentos y por lo tanto la salud mental, especialmente en los adultos mayores. El objetivo fue evaluar si existe asociación entre el consumo de ciertos grupos de alimentos (lácteos, huevos y menestras, carne, aves de corral o pescado, y frutas y/o verduras) y la presencia de síntomas depresivos en adultos mayores de pobreza y pobreza extrema en el Perú. Metodología: Análisis secundario de un estudio de base poblacional (Encuesta de Salud y Bienestar del Adulto Mayor, en Perú) realizada en el 2012. La variable desenlace fue la presencia de síntomas depresivos, mientras que la exposición fue la frecuencia de consumo de ciertos grupos de alimentos: lácteos; huevos y menestras; carne, aves de corral o pescado; y frutas y/o verduras. Se usó modelos de regresión de Poisson, reportándose razones de prevalencia (RP) e intervalos de confianza al 95% (IC95%). Resultados: Un total de 4214 registros fueron analizados, edad promedio de 71,2 (DE: 4,4) años, 54,2% de varones, y 61,4% de zona rural. Un total de 1621 (38,6%; IC95%: 37,1%-40,1%) adultos mayores presentaron síntomas depresivos. En modelo multivariable, la frecuencia de síntomas depresivos fue un 41% superior (IC95%: 30%-54%) entre los que no consumían huevos y menestras al menos una vez a la semana. Similarmente, se encontró asociación en aquellos que reportaron no consumir carne, aves de corral o pescado al menos tres veces por semana (18%; IC95%: 9%-28%) y los que no consumieron frutas y/o verduras al menos dos veces al día (15%; IC95%: 6%-24%). Conclusión: Este estudio evidenció que el consumo de frutas y/o verduras, aves de corral o pescado, huevos y menestras están relacionados de manera inversa con la presencia de síntomas depresivos en el adulto mayor

«La Isla de las Tentaciones» ha supuesto una revolución entre los jóvenes con edades comprendidas entre los 6 y 24 años, quienes han conformado un alto porcentaje de los espectadores. El objetivo de este trabajo es investigar la repercusión e impacto mediático que han tenido en Instagram los participantes del programa, tanto desde la progresión de sus perfiles como categorizando los contenidos más presentes en el programa. Del mismo modo, se busca identificar las motivaciones que posee este colectivo hacia el dating talk show. Para ello se empleó un enfoque metodológico mixto, aplicando un análisis cuantitativo para establecer la progresión de 11 perfiles de Instagram de los participantes. Los resultados del estudio revelan que el grupo de 18 a 24 años es el colectivo de mayor representación entre los seguidores en Instagram del programa. Se observa que el amor romántico se encuentra relacionado con valores como el respeto y el ideal de la «media naranja». De ahí que estos espectadores muestren sentimientos de rechazo y vergüenza por las acciones contrarias a este concepto. Sin embargo, otro sector del público muestra su aprobación e incluso normaliza la infidelidad. Este tipo de programas muestran una cultura mediática basada en la hipersexualización de los personajes por lo que es necesario una educación que analice críticamente las representaciones que realizan estos programas sobre la realidad. /// «Temptation Island» has been a revolution among young people between the ages of 6 and 24, who have made up a high percentage of the audience. Therefore, the aim of this work is to investigate the repercussion and media impact that the show's participants have had on Instagram, both from the progression of their profiles and by categorising the contents that are most present in the show. Likewise, it seeks to identify the motivations of this group towards the dating talk show. A mixed methodological approach was used, applying a quantitative analysis to establish the progression of 11 Instagram profiles of the participants. The results of the study reveal that the 18-24 year old group is the most represented group of Instagram’s followers. It was observed that romantic love is related to values such as respect and the ideal of the «better half». Hence, these viewers show feelings of rejection and shame for actions contrary to this concept. However, another sector of the public shows its approval and even normalises infidelity. This kind of programmes show a media culture based on the hypersexualization of characters, so it is necessary an education that critically analyzes the representations that these programmes make about reality.

Objetivo. El objetivo del presente estudio fue evaluar las tendencias ocurridas en diferentes indicadores de tabaquismo en adolescentes peruanos. Asimismo, se evaluó las tendencias en dichos indicadores de acuerdo a sexo y en aquellos sin antecedente de tabaquismo previo. Materiales y métodos. El presente estudio usa la Encuesta Mundial sobre Tabaquismo en Jóvenes (Global Youth Tobacco Survey 2007, 2014, 2019). Tabaquismo activo se definió según el consumo de tabaco en los últimos 30 días. Tabaquismo pasivo se definió en función a exposición a tabaco dentro y fuera del hogar, tanto global como en forma diaria. Los análisis consideraron el diseño muestral. Resultados. Un total de 17 047 registros (9869 en el 2007, 3424 en el 2014, y 3754 en el 2019) fueron analizados; edad media 14 años y 49,9% mujeres. El 26,6% reportó antecedente de tabaquismo previo; dicha prevalencia cayó de 45,2% (2007), a 25,3% (2014), y a 19,4% (2019, p de tendencias < 0,001), mientras que el tabaquismo activo cayó de 17,1% (2007) a 8,7% (2014) y a 5,7% (2019). La prevalencia global de tabaquismo pasivo en el hogar cayó de 24,7%, a 12,9% y a 10,4% (p de tendencias

The development of entrepreneurial potential in the training of school-age pupils is relevant for carrying out projects of an innovative and transformative nature. Entrepreneurial training is enacted through Spanish educational regulations, in relation to the development of the key entrepreneurial competence. Thus, a training programme in entrepreneurial potential for school-age students, who are considered as agents of change, was evaluated under the approach of “enterprise education pedagogy”. For this purpose, an experimental research study, pre-test and post-test, with a control and experimental group, was designed. The sample consisted of 1036 participants from eight autonomous communities in Spain. The data analysis was carried out by means of a t-test to compare the mean before and after the application of the programme on the total number of participants, as well as on the subgroups with and without entrepreneurial intentions. The results show that the PEIEO programme had a positive effect on entrepreneurial potential. The experimental group, compared to the control group, significantly increased their total score in the t-test, as well as for each dimension of entrepreneurial potential. Similarly, the participants considered to have entrepreneurial intentions further increased their entrepreneurial potential compared to the group considered to have no entrepreneurial intentions. Consequently, the results indicate that entrepreneurship education, in a holistic sense, has a relevant impact on entrepreneurial potential in a programme aimed at training agents of change.

A cardiac lesion detected at ultrasonography might turn out to be a normal structure, a benign tumor or rarely a malignancy, and lesion characterization is very important to appropriately manage the lesion itself. The exact relationship of the mass with coronary arteries and the knowledge of possible concomitant coronary artery disease are necessary preoperative information. Moreover, the increasingly performed coronary CT angiography to evaluate non-invasively coronary artery disease leads to a rising number of incidental findings. Therefore, CT and MRI are frequently performed imaging modalities when echocardiography is deemed insufficient to evaluate a lesion. A brief comprehensive overview about diagnostic radiological imaging and the clinical background of cardiac masses and pseudomasses is reported.

Objetivo. Determinar si existe asociación entre la presencia de diabetes mellitus tipo 2 (DM2) y la duración, dificultad para dormir, y calidad de sueño, en sujetos de 30 a 69 años en Tumbes. Materiales y métodos. Estudio transversal analítico. Las variables dependientes fueron dificultad para dormir (a veces/casi nunca y frecuentemente), duración del sueño (normal, corto y prolongado) y calidad del sueño (buena y mala). La variable independiente fue la presencia de DM2 evaluada usando la prueba de tolerancia oral a la glucosa (no DM2, con DM2 y sin diagnóstico previo, y con DM2 y diagnóstico previo). Se usó modelos de regresión de Poisson para reportar razones de prevalencia (RP) e intervalos de confianza al 95% (IC 95%). Resultados. Se analizaron 1604 individuos, con una edad media de 48,2 años; 50,3% fueron mujeres; 71 (4,4%) tenían DM2 sin diagnóstico previo y 105 (6,5%) tenían DM2 con diagnóstico previo. Según las características del sueño, 12,0% presentó sueño corto y 8,2% presentó sueño prolongado; 3,7% reportó dificultad para dormir, y 19,5% tuvo mala calidad de sueño. En el modelo multivariable, tener DM2 con diagnóstico previo estuvo asociado con dificultad para dormir (RP = 2,20; IC 95%: 1,13–4,27) y mala calidad de sueño (RP = 1,40; IC 95%: 1,05–1,92) comparado con aquellos sin DM2. Conclusiones. Los individuos con DM2 que tenían diagnóstico previo tuvieron mayor probabilidad de presentar dificultad para dormir y mala calidad de sueño. Nuestros resultados sugieren la necesidad de evaluar en forma periódica las características del sueño en pacientes con DM2.

Summary: Background: Second-hand smoke exposure is prevalent amongst adolescents, despite of being a preventable risk factor associated with unfavourable outcomes. The distribution of this risk factor varies by underlying determinants and public health officers need contemporary evidence to update policies. Using the most recent data available from adolescents in Latin America and the Caribbean (LAC), we described the prevalence of second-hand smoking. Methods: Pooled analysis of Global School-based Student Health (GSHS) surveys conducted from 2010 to 2018 was conducted. Summary: Background: The hypertension care cascade has been characterized worldwide, yet it has not been quantified how far above the blood pressure control threshold people with uncontrolled treated hypertension are. We summarized the mean systolic blood pressure (SBP; mmHg) in people treated for hypertension but SBP not 130/80 mmHg were included. We quantified the mean SBP overall and by socio-demographic (sex, age, urban/rural location, education) and cardiometabolic (current smoking, self-reported diabetes) risk factors. Findings: The lowest SBP was observed in Kuwait (146.6; 95% CI: 143.8–149.4 mmHg) and the highest in Libya (171.9; 95% CI: 167.8–176.0 mmHg). In 29 countries, the SBP was higher in men, and SBP tended to be higher in older groups except in six countries. In 17 countries, the SBP was higher in rural than in urban sites, for example in Turkmenistan the SBP was 162.3 (95% CI: 158.4–166.2) mmHg in rural versus 151.6 (95% CI: 148.7–154.4) mmHg in urban areas. In 25 countries, the SBP was higher in adults with no education, for example in Benin the SBP in people without formal education was 175.3 (95% CI: 168.8–181.9) mmHg versus 156.4 (95% CI: 148.8–164.0) mmHg in people with higher education. Interpretation: Stronger interventions to improve and secure access to effective management are needed in most countries and specific groups, to reach hypertension control in people with hypertension already receiving antihypertensive medication. Neoadjuvant chemotherapy was originally designed to convert inoperable cancer. Nowadays, this concept has expanded since it can also offer the possibility to evaluate markers of response such as pathological complete response (pCR) with possible implications in long-term prognostic outcomes. A substantial body of literature tried to evaluate the ability of pCR to fulfill the conditions required to establish a preliminary endpoint, such as pCR, as a surrogate for the final endpoint, the overall survival (OS) but no systematic reviews have been performed yet. In this review we systematically analyzed the prognostic role of pCR in various cancers (breast, gastro-oesophageal, rectal, ovarian, bladder, lung) in which neoadjuvant treatment is a standard of care, evaluating articles published in the English language of phase III or phase II randomized controlled trials and meta-analyses. Abstract Clinical trials evaluating cardiac progenitor cells (CPC) demonstrated feasibility and safety, but no clear functional benefits. Therefore a deeper understanding of CPC biology is warranted to inform strategies capable to enhance their therapeutic potential. Here we have defined, using a label-free proteomic approach, the differential cytoplasmic and nuclear compartments of human CPC (hCPC). Global analysis of cytoplasmic repertoire in hCPC suggested an important hypoxia response capacity and active collagen metabolism. In addition, comparative analysis of the nuclear protein compartment identified a significant regulation of a small number of proteins in hCPC versus human mesenchymal stem cells (hMSC). Two proteins significantly upregulated in the hCPC nuclear compartment, IL1A and IMP3, showed also a parallel increase in mRNA expression in hCPC versus hMSC, and were studied further. IL1A, subjected to an important post-transcriptional regulation, was demonstrated to act as a dual-function cytokine with a plausible role in apoptosis regulation. The knockdown of the mRNA binding protein (IMP3) did not negatively impact hCPC viability, but reduced their proliferation and migration capacity. Analysis of a panel of putative candidate genes identified HMGA2 and PTPRF as IMP3 targets in hCPC. Therefore, they are potentially involved in hCPC proliferation/migration regulation.

Objective To determine the agreement between the cardiovascular disease (CVD) risk predictions computed with the WHO non-laboratory-based model and laboratory-based model in a nationally representative sample of Peruvian adults.Design Cross-sectional analysis of a national health survey.Methods Absolute CVD risk was computed with the 2019 WHO laboratory-based and non-laboratory-based models. The risk predictions from both models were compared with Bland-Altman plots, Lin’s concordance coefficient correlation (LCCC), and kappa statistics, stratified by sex, age, body mass index categories, smoking and diabetes status.Results 663 people aged 30–59 years were included in the analysis. Overall, there were no substantial differences between the mean CVD risk computed with the laboratory-based model 2.0% (95% CI 1.8% to 2.2%) and the non-laboratory-based model 2.0% (95% CI 1.8% to 2.1%). In the Bland-Altman plots, the limits of agreement were the widest among people with diabetes (−0.21; 4.37) compared with people without diabetes (−1.17; 0.95). The lowest agreement as per the LCCC was also seen in people with diabetes (0.74 (95% CI 0.63 to 0.82)), the same was observed with the kappa statistic (kappa=0.36). In general, agreement between the scores was appropriate in terms of clinical significance.Conclusions The absolute cardiovascular predicted risk was similar between the laboratory-based and non-laboratory-based 2019 WHO cardiovascular risk models. Pending validation from longitudinal studies, the non-laboratory-based model (instead of the laboratory-based) could be used when assessing CVD risk in Peruvian population.

ABSTRACT OBJECTIVE To assess the longitudinal effect of double burden of malnutrition (DBM) on the risk of developing child and adolescent overweight or obesity. METHODS Analysis of the Peruvian Young Lives Study, younger cohort: baseline (2002) and 4 follow-ups (2006–2007, 2009–2010, 2013–2014, and 2016–2017). Outcomes were the incidence of overweight and obesity as defined by the World Health Organization standards. The exposure comprised a variable with 4 categories: non-stunted child with a non-overweight mother (reference group), non-stunted child with an overweight mother, stunted child with a non-overweight mother, and stunted child with an overweight mother (i.e., DBM). Poisson regression models were built to assess the association of interest, and relative risks (RR) and 95%CI were reported. RESULTS Data from 2,034 children; 50.0% were girls and the mean age was 12.0 (3.6) months at baseline. Non-stunted children with an overweight mother had greater risk (RR = 1.64; 95%CI: 1.35–1.99) of developing overweight, compared with the risk for stunted children with a non-overweight mother (RR = 1.38; 95%CI: 1.10–1.72), and for those with DBM (RR = 1.28; 95%CI: 1.02–1.61). When compared with the reference group, obesity risk was greater among non-stunted children with an overweight mother (RR = 2.33; 95%CI: 1.68–3.22), greater among stunted children with a non-overweight mother (RR = 2.59; 95%CI: 1.75–3.84), and greater among those with DBM (RR = 2.14; 95%CI: 1.39–3.28). CONCLUSIONS DBM is a risk factor for childhood overweight and obesity in Peru. Dual-duty policies tackling both undernutrition in children and overweight in mothers are needed to reduce DBM and its future effects in Peru.

Assim como qualquer outra área da Ciência, a Epistemologia da Educação Geográfica está em constante renovação. Hora e outra surge alguma nova tendência educacional e a indicação de renovação das bases filosóficas que sustentam a Geografia Escolar. Nesse artigo, pensamos a Epistemologia da Educação Geográfica a partir do adoecimento dos professores de Geografia como impeditivo das realizações existenciais e, como consequência, dos sentidos que os professores atribuem a sua prática. Chegamos à essa consideração por meio da interpretação fenomenológica-hermenêutica de asserções de diferentes autores sobre a Educação Geográfica que, nas generalidades de seus sentidos, corroboram com as metas de formar cidadãos ativos, educar a partir de e para a realidade dos educandos e educar para a transformação. Entretanto, as descrições das condições e organizações do trabalho docente demonstraram que, na prática cotidiana da sala de aula os professores experienciam o adoecimento como impeditivo das realizações pessoais, e da própria Educação Geográfica.

Background Peru faces challenges to provide adequate care to stroke patients. Length of hospitalization and in-hospital mortality are two well-known indicators of stroke care. We aimed to describe the length of stay (LOS) of stroke in Peru, and to assess in-hospital mortality risk due to stroke, and subtypes. Methods This retrospective cohort study used hospitalization registries coding with ICD-10 from 2002 to 2017 (N = 98,605) provided by the Ministry of Health; in-hospital mortality was available for 2016–2017 (N = 6,566). Stroke cases aged ≥35 years were divided into subarachnoid hemorrhage (I60), intracerebral hemorrhage (I61), cerebral infarction (I63), and stroke not specified as hemorrhage or infarction (I64). Data included stroke LOS and in-hospital mortality; socio-demographic and clinical variables. We fitted a region- and hospital level-stratified Weibull proportional hazard model to assess the in-hospital mortality. Results The median LOS was 7 days (IQR: 4–13). Ageratina pichinchensis (Kunth) R.M. King & H. Rob. belongs to the Asteraceae family and is a plant native to Mexico to which several biological properties are attributed. In this study, the cytotoxic effect of four extracts from the wild plants and two extracts from A. pichinchensis callus culture were evaluated against carcinogenic cell lines including prostate carcinoma, cervical cancer, hepatocellular carcinoma, hepatoma human, lung cancer, and cellular keratinocytes. The extracts were obtained with ethyl acetate and methanol using both leaves and stems or the callus. Only the ethyl acetate extract of the callus culture influenced the cervical cancer cell line (HeLa) with an IC50 of 94.79 ± 2.0 µg/mL. From the ethyl acetate callus extract, 2,3-dihydrobenzofuran was isolated and purified and also evaluated against cancer cells. The cytotoxic evaluation of this compound showed a significant effect against the HeLa cell line with an IC50 of 23.86 ± 2.5 µg/mL. Our results contribute to the development of biotechnological alternatives and extraction processes to produce compounds with possible potential against certain types of human cancer.

Oxidative stress-induced myocardial apoptosis and necrosis are critically involved in ischemic infarction, and several sources of extracellular vesicles appear to be enriched in therapeutic activities. The central objective was to identify and validate the differential exosome miRNA repertoire in human cardiac progenitor cells (CPC). CPC exosomes were first analyzed by LC-MS/MS and compared by RNAseq with exomes of human mesenchymal stromal cells and human fibroblasts to define their differential exosome miRNA repertoire (exo-miRSEL). Proteomics demonstrated a highly significant representation of cardiovascular development functions and angiogenesis in CPC exosomes, and RNAseq analysis yielded about 350 different miRNAs; among the exo-miRSEL population, miR-935 was confirmed as the miRNA most significantly up-regulated; interestingly, miR-935 was also found to be preferentially expressed in mouse primary cardiac Bmi1+high CPC, a population highly enriched in progenitors. Furthermore, it was found that transfection of an miR-935 antagomiR combined with oxidative stress treatment provoked a significant increment both in apoptotic and necrotic populations, whereas transfection of a miR-935 mimic did not modify the response. Conclusion. miR-935 is a highly differentially expressed miRNA in exo-miRSEL, and its expression reduction promotes oxidative stress-associated apoptosis. MiR-935, together with other exosomal miRNA members, could counteract oxidative stress-related apoptosis, at least in CPC surroundings.

ABSTRACT OBJECTIVE To determine the association between socioeconomic level and the presence of obesity, hypertension and type 2 diabetes mellitus in the Peruvian population. METHODS Secondary analysis of data from the National Demographic and Family Health Survey ( Encuesta Nacional Demográfica y de Salud Familiar , Endes) from 2018 to 2020. The outcomes were obesity, hypertension, and type 2 diabetes mellitus. The exposure variables were two indicators of socioeconomic status: educational level (< 7 years, 7–11 years, and 12+ years) and wealth index (in tertiles). Models were created using Poisson regression, reporting prevalence ratios (PR) and 95% confidence intervals (95%CI). RESULTS Data from 98,846 subjects were analyzed. Mean age: 45.3 (SD: 16.0) years, and 55.5% were women. The prevalence of obesity was 26.0% (95%CI: 25.4–26.6); of hypertension, 24.9% (95%CI: 24.3–25.5); and of type 2 diabetes mellitus, 4.8% (95%CI: 4.5–5.1). In multivariate model, and compared with those with a low wealth index, those with a high wealth index had a higher prevalence of obesity (PR = 1.49; 95%CI: 1.38–1.62), hypertension (PR = 1.09; 95%CI: 1.02–1.17) and type 2 diabetes mellitus (PR = 1.72; 95%CI: 1.29–2.29). On the other hand, higher educational level was only associated with a reduction in the prevalence of obesity (PR = 0.89; 95%CI: 0.84–0.95). CONCLUSIONS There is a differential association between the wealth index, educational level and markers of noncommunicable diseases. There is evidence of a positive association between wealth index and obesity, hypertension and type 2 diabetes mellitus, whereas educational level was only negatively associated with obesity.

As Ciências emergiram e se consolidaram ao longo dos últimos séculos como um conhecimento que operacionalizou a vida cotidiana, mudando o nosso modo de existência no mundo. Isto não ocorreu de forma tranquila para os cientistas, por que as Ciências se desenvolveram como conhecimento hegemônico na Modernidade por meio de conflitos e controvérsias. Um dos pontos nodais deste desenvolvimento ocorreu a partir da década de 1960, em que as Ciências ganharam novas feições pela multiplicidade de perspectivas, propostas e teorias. Isto ocorreu, dentre muitos motivos, devido à inclusão, mesmo que marginal, de alguns sujeitos e de certos segmentos sociais que até então eram somente pesquisados pelos cientistas, mas não tinham sua própria voz dentro da Academia. Somado a isto, as redes sociais virtuais possibilitaram aos sujeitos novas formas de acesso as informações, dentre estas, as propostas científicas. Paradoxalmente, o mesmo fenômeno que possibilitou o maior acesso às informações e às propostas científicas, amparou as contestações quanto a legitimidade das Ciências por parte de alguns sujeitos e segmentos sociais organizados. Desse modo, nosso principal objetivo desse manuscrito é enveredar-se por estas controvérsias para interpretar como elas constituem as Ciências na contemporaneidade.
