The transition to oral therapies in patients with multiple myeloma (MM) offers potential benefits to patients; however, they must self-manage their medication and adherence plays an important role in patient care. It has been shown that patient satisfaction with their medication has a strong positive correlation with adherence in chronic diseases. The aim of this study was to estimate adherence rate of oral antimyeloma therapies and to identify risk factors for medication non-adherence. This observational, prospective, and multicentre survey based on a self-report questionnaire enrolled MM patients with at least 3 months of oral therapy. The 6-item Girerd scale and the medication possession ratio (MPR) were used for measuring medication adherence and the SATMED-Q® questionnaire was used for measuring satisfaction. An analysis of risk factors for non-adherence to oral therapy was performed using univariate analysis. A total of 101 patients participated in the survey, yielding a response rate of 87%. The prevalence of adherence to oral antimyeloma therapy was estimated at 51.5% using the Girerd questionnaire. According to the MPR, adherence was evaluated at 96% (i.e. MPR ≥ 0.80). Both methods combined, adherence was estimated at 50.5%. One risk factor for medication non-adherence was identified: Eastern Cooperative Oncology Group Performance Status > 2 (p = 0.007). One predictive factor for high medication adherence was identified: high satisfaction with treatment (p = 0.01). Identifying patients at higher risk for non-adherence allows clinical pharmacists to personalise therapeutic information and education and to improve the quality of healthcare overall. Le choix du concentré plaquettaire (CP) par le personnel de délivrance/distribution s'effectue sur la quantité de principe actif (QPA) puis sur la durée de conservation du CP. Cette stratégie n'a pas été ré-évaluée avec les derniers procédés de sécurisation et de préparation (Intercept® Amotosalen et Dual Storage) ni avec l'augmentation de la durée de conservation à 7 jours. Entre juin 2020 et février 2021, quatre types de CP prophylactiques ont été administrés à dix-neuf patients en cours d'induction ou de greffe de cellules souches hématopoïétiques : (1) QPA 0,5 × 1011 à 0,7 × 1011/kg et durée de conservation < j6 ; (2) QPA 0,5 × 1011 à 0,7 × 1011/kg et durée de conservation j6–7 ; (3) QPA > 0,7 × 1011/kg et durée de conservation < j6 ; (4) QPA > 0,7 × 1011/kg et durée de conservation j6–7. La variation des incréments plaquettaires corrigés (CCI), des intervalles transfusionnelles (IT) et la survenue d'hémorragie ont été observés. Les CCI à 24 h sont d'autant plus élevés que les CP sont frais (j2–j3–j4), et présentent une QPA supérieure à 0,7 × 1011/kg. Seule la stratégie (2) QPA 0,5 × 1011 à 0,7 × 1011/kg et durée de conservation j6–7 semble être associée à un IT plus court (−1 jour) dans 2/3 des cas. Quel que soit le type de CP administré, aucun évènement hémorragique n'a été constaté. Chez les patients présentant une durée d'aplasie prolongée, la variation de la durée de conservation et de la QPA ne semble pas impacter la survenue d'hémorragie ni l'IT. À l'occasion de la rédaction d'un livret d'information sur la transfusion à destination des patients, l'idée de l'illustrer par des témoignages d'acteurs de la transfusion s'est imposée. Créer une dynamique de communication autour de la transfusion par ses acteurs (donneurs, receveurs, professionnels de santé, bénévoles...) pourrait permettre de : – coté donneurs : favoriser l'adhésion et l'engagement au don de sang ; – coté receveurs : humaniser l'acte transfusionnel ; – créer un lien social, virtuel et anonyme, entre ces acteurs. Un groupe de travail constitué de quatre infirmiers dont deux cadres et trois médecins en hématologie ; un médecin responsable de hémovigilance ; un pharmacien responsable de la distribution/délivrance et deux chargés de communication au sein de l'Établissement français du sang (EFS) ont rédigé le livret du 25/10/2019 au 17/10/2020. En parallèle, à compter du 21/09/2020, des entretiens semi-directifs ont été réalisés chez des receveurs en oncologie et en hématologie. Le livret et les neufs premiers témoignages de receveurs sont disponibles en format écrit et audio. À l'avenir, les acteurs de la transfusion pourront les consulter lors de collectes de sang, de transfusion à l'hôpital ou sur le site de l'EFS. Ils seront invités à témoigner à leur tour via une plateforme téléphonique ou un formulaire en ligne afin d'entretenir un « réseau social » dynamique. L'impact sur le don sera évalué par des indicateurs de performance et la satisfaction des patients par un questionnaire. Nucleophosmin-1 (NPM1) mutations in acute myeloid leukemia (AML) confer a survival advantage in the absence of FLT3-internal tandem duplication (FLT3-ITD). Here, we investigated the main predictors of outcome after allogeneic hematopoietic stem cell transplantation (allo-HCT). We identified 1572 adult (age ≥ 18 year) patients with NPM1-mutated AML in first complete remission (CR1:78%) or second complete remission (CR2:22%) who were transplanted from matched sibling donors (30.8%) or unrelated donors (57.4%) between 2007 and 2019 at EBMT participating centers. Median follow-up for survivors was 23.7 months. FLT3-ITD was present in 69.3% of patients and 39.2% had detectable minimal/measurable residual disease (MRD) at transplant. In multivariate analysis, relapse incidence (RI) and leukemia-free survival (LFS) were negatively affected by concomitant FLT3-ITD mutation (HR 1.66 p = 0.0001, and HR 1.53, p < 0.0001, respectively), MRD positivity at transplant (HR 2.18, p < 10-5 and HR 1.71, p < 10-5, respectively), and transplant in CR2 (HR 1.36, p = 0.026, and HR 1.26, p = 0.033, respectively), but positively affected by Karnofsky score ≥90 (HR 0.74, p = 0.012, and HR 0.7, p = 0.0002, respectively). Overall survival (OS) was also negatively influenced by concomitant FLT3-ITD (HR 1.6, p = 0.0001), MRD positivity at transplant (HR 1.61, p < 10-5), and older age (HR 1.22 per 10 years, p < 0.0001), but positively affected by matched sibling donor (unrelated donor: HR 1.35, p = 0.012; haploidentical donor: HR 1.45, p = 0.037) and Karnofsky score ≥90 (HR 0.73, p = 0.004). These results highlight the independent and significant role of FLT3-ITD, MRD status, and disease status on posttransplant outcomes in patients with NPM1-mutated AML allowing physicians to identify patients at risk of relapse who may benefit from posttransplant prophylactic interventions. The use of peripheral blood (PB) or bone marrow (BM) stem cells graft in haploidentical hematopoietic stem cell transplantation with post-transplant cyclophosphamide (PTCy) for graft-versus-host disease (GVHD) prophylaxis remains controversial. Moreover, the value of adding anti-thymoglobulin (ATG) to PTCy is unknown. A total of 1344 adult patients received an unmanipulated haploidentical transplant at 37 centers from 2012 to 2019 for hematologic malignancy. We compared the outcomes of patients according to the type of graft, using a propensity score analysis. In total population, grade II–IV and III–IV acute GVHD (aGVHD) were lower with BM than with PB. Grade III–IV aGVHD was lower with BM than with PB + ATG. All outcomes were similar in PB and PB + ATG groups. Then, in total population, adding ATG does not benefit the procedure. In acute leukemia, myelodysplastic syndrome and myeloproliferative syndrome (AL-MDS-MPS) subgroup receiving non-myeloablative conditioning, risk of relapse was twice greater with BM than with PB (51 vs. 22%, respectively). Conversely, risk of aGVHD was greater with PB (38% for aGVHD II–IV; 16% for aGVHD III–IV) than with BM (28% for aGVHD II–IV; 8% for aGVHD III–IV). In this subgroup with intensified conditioning regimen, risk of relapse became similar with PB and BM but risk of aGVHD III–IV remained higher with PB than with BM graft (HR = 2.0; range [1.17–3.43], p = 0.012). Introduction: Allogeneic hematopoietic stem-cell transplantation (allo-HSCT) remains the best curative option for high-risk myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML). Unfortunately, it is still associated with a significant risk of relapse due to mechanisms of escape from the control of alloreactive T cells. Repetitive adjuvant donor lymphocyte infusion (DLI), termed prophylactic DLI (proDLI), as an effective strategy in preventing relapse is still debated. Methods: We performed a retrospective multicenter study to evaluate the efficacy of proDLI in allografted AML and MDS. We identified 56 patients treated with proDLI (DLI planned in full chimeras without any sign of disease relapse) and matched them to 167 patients in control group, (DLI performed for mixed chimerism or positive minimal residual disease) based on similar age, initial disease, cytogenetic prognosis, and conditioning intensity. Results: In univariate analysis, the incidence of severe aGVHD at 100 days and incidence of all grades of chronic GVHD 1 year after allo-HSCT were similar in the two groups. We also observed a trend of higher 3-year RI (52.61% [95% confidence interval 25.99–79.23]) in the proDLI group versus the control group (29.31% [20.28–38.34], p = 0.067). However, 3-year overall survival (p = 0.892), progression-free survival (p = 0.239), and nonrelapse mortality (p = 0.343) were similar between the two groups. In multivariate analysis, the only factor influencing overall and progression-free survival was anti-thymocyte globulin administration during the conditioning regimen. Conclusion: The proDLI strategy had an acceptable toxicity profile but did not improve patient outcomes compared to the pre-emptive strategy. [ABSTRACT FROM AUTHOR]

Simple Summary: Until recently, there were no approved therapies for the aggressive blood cancer blastic plasmacytoid dendritic cell neoplasm (BPDCN). Survival for patients diagnosed with BPDCN is under two years, and improved treatments are needed. In 2018, tagraxofusp became the first approved drug for BPDCN. Tagraxofusp is an interleukin 3-dipththeria toxin recombinant fusion protein that targets CD123, a component of the interleukin 3 receptor, on the surface of BPDCN cells. Here, we discuss the development of tagraxofusp and other newer agents that also target CD123. We also present rationale for several other cell surface proteins, expressed on BPDCN, that are targets for therapies already in development for other cancers and that might be also considered for evaluation in BPDCN. Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare and aggressive leukemia derived from plasmacytoid dendritic cells (pDCs). It is associated with a remarkably poor prognosis and unmet need for better therapies. Recently, the first-in-class CD123-targeting therapy, tagraxofusp, was approved for treatment of BPDCN. Other CD123-targeting strategies are in development, including bispecific antibodies and combination approaches with tagraxofusp and other novel agents. In other blood cancers, adoptive T-cell therapy using chimeric antigen receptor (CAR)-modified T cells represents a promising new avenue in immunotherapy, showing durable remissions in some relapsed hematologic malignancies. Here, we report on novel and innovative therapies in development to target surface molecules in BPDCN currently in clinical trials or in preclinical stages. Background: Three different scoring systems have been developed to assess pre‐transplant comorbidity in allogeneic hematopoietic stem cell transplantation (Allo‐HSCT): the Hematopoietic Cell Transplantation‐Specific Comorbidity Index, the Comorbidity/Age index, and the Augmented Comorbidity/Age index. All were devised to predict overall survival (OS) and disease‐free survival (DFS) survivals and non‐relapse mortality (NRM) in patients receiving HLA‐matched Allo‐HSCT, but their performance has scarcely been studied in the haploidentical Allo‐HSCT setting with post‐transplant cyclophosphamide, a procedure in constant expansion worldwide. Methods: To address this issue, their impact on survivals and NRM was examined in a cohort of 223 patients treated with haploidentical Allo‐HSCT in four different centers. Results: With a median follow‐up of 35.6 months, 3‐year OS, DFS, and NRM were 48.1% ± 4%, 46.3% ± 4%, and 30.0% ± 3%, respectively. No impact was found for any of the three comorbidity scores in univariate analysis. In multivariate analyses, the only three factors associated with lower OS were DRI (p < 0.001), an older age of recipients (≥55 years old, p = 0.02) and of donors (≥40 years old, p = 0.005). Older donor age was also associated with lower DFS and higher NRM. Conclusion: The comorbidity scores do not predict survivals nor NRM in haploidentical Allo‐HSCT with PTCY, suggesting that pre‐transplant comorbidities should not be a contra‐indication to this procedure. The article describes the case of a 23 year-old immunocompromised patient whose acute respiratory syndrome coronavirus 2 (SARS-CoV-2) replication for over a year was successfully treated by neutralizing monoclonal antibody (NmAb). Topics covered include the medical history of the patient, NmAB as a curative therapeutic option in immunocompromised patients with prolonged COVID-19, and patient outcome following treatment.
