The 2022 global outbreak of human Mpox (formerly monkeypox) virus (MPXV) infection outside of the usual endemic zones in Africa challenged our understanding of the viruss natural history, transmission dynamics, and risk factors. This outbreak has highlighted the need for diagnostics, vaccines, therapeutics, and implementation research, all of which require more substantial investments in equitable collaborative partnerships. Global multidisciplinary networks need to tackle MPXV and other neglected emerging and reemerging zoonotic pathogens to address them locally and prevent or quickly control their worldwide spread. Political endorsement from individual countries and financial commitments to maintain control efforts will be essential for long-term sustainability.

BackgroundHyperglycemia and type 2 diabetes mellitus (T2DM) pose a significant risk for cardiovascular diseases and associated complications in individuals with hyperlipidemia. Statin therapy, effective in reducing cholesterol and cardiovascular risks, paradoxically increases incident T2DM risk due to its adverse impact on glucose homeostasis. Therefore, there is a pressing need for safe, and effective adjunctive or alternative therapies to manage hyperglycemia in hyperlipidemic individuals. There is growing body of pharmacological evidence suggesting that Amla and Olive extract supplementation can be beneficial in managing hyperglycemia in individuals with hyperlipidemia.ObjectiveThe present study aimed to assess for the first time the potential synergistic antihyperglycemic effects of a daily co-supplementation of 1,000 mg Amla fruit and 50 mg Olive fruit standardized extracts (Cholesfytol NG®) over a 2-months period in hyperlipidemic adults with T2DM or prediabetes.MethodsThis retrospective cross-sectional observational study analyzed treatment outcomes in 191 hyperlipidemic adults under the care of their physicians at 57 General Practitioner clinics in Belgium during real-life clinical practice between March 19, 2020, and January 31, 2022. These participants received Cholesfytol NG® as supplementary therapy to improve their metabolic health. Background As the use of sodium–glucose co-transporter 2 inhibitors (SGLT2is) has expanded beyond glucose-lowering therapy in type 2 diabetes mellitus (T2DM), including chronic kidney disease and heart failure, there has also been an increase in reported cases of diabetic ketoacidosis (DKA) associated with SGLT2i. Case summary A 77-year-old woman with T2DM presented to the emergency department with ST-segment elevation myocardial infarction (MI) complicated by atrial fibrillation. Her medications included empagliflozin, an SGLT2i, initiated for T2DM. Diabetic ketoacidosis was suspected on the basis of a large anion gap, despite a plasma glucose level below 200 mg/dL (11.1 mmol/L) and the absence of symptoms, including nausea and vomiting. Laboratory tests confirmed metabolic acidosis and high ketones. However, the diagnosis of euglycaemic DKA (eu-DKA) was delayed due to lack of symptoms and moderate hyperglycaemia. The patient was successfully treated according to DKA management guidelines. She was discharged on insulin, and SGLT2i was discontinued. Discussion This is a case of asymptomatic eu-DKA after acute MI (AMI). We discuss the use of SGLT2is in AMI and arrhythmias from a review of the literature and the prophylaxis of eu-DKA. Regular monitoring of blood glucose and ketones should be performed in hospitalized T2DM patients treated with SGLT2i. The SGLT2i should be stopped as soon as possible in the event of critical illness or suspected DKA in the setting of an acute illness such as AMI. Coronavirus disease 2019 (COVID-19) in pregnancy is associated with excess maternal and infant morbidity and mortality in both African and higher-resource settings. Furthermore, mounting evidence demonstrates the safety and efficacy of COVID-19 vaccination for pregnant women and infants. However, national guidelines in many African countries are equivocal or lack recommendations on COVID-19 vaccine in pregnancy. We summarize key data on COVID-19 epidemiology and vaccination among pregnant African women to highlight major barriers to vaccination and recommend 4 interventions. First, policymakers should prioritize pregnant women for COVID-19 vaccination, with a target of 100% coverage. Second, empirically supported public health campaigns should be sustainably implemented to inform and support pregnant women and their healthcare providers in overcoming vaccine hesitancy. Third, COVID-19 vaccination for pregnant women should be expanded to include antenatal care, obstetrics/gynecology, and targeted mass vaccination campaigns. Fourth, national monitoring and evaluation of COVID-19 vaccine uptake, safety, surveillance, and prospective outcomes assessment should be conducted. [ABSTRACT FROM AUTHOR]

Aim: Diabetic foot ulcers, particularly those with high Wagner scores, are difficult to heal. They are, per definition, infected and patients in this category typically also suffer from serious comorbidities. TDA* is an innovative debriding agent that works via topical desiccation of necrosis and biofilm through a hygroscopic action. This results in the denaturing of their proteins and, typically, a rapid sloughing off. Efficacy of TDA was evaluated in patients with Wagner III and IV diabetic foot ulcers with culture-confirmed colonization of multidrug-resistant Pseudomonas Aeruginosa. Method: TDA was applied for 60 second and rinsed of, followed by the application of standard dressings. Study participation was limited to 40 days. Results / Discussion: The average age of patients was 74+12.2 years. Average ulcer (N=20) size was 9.1+7.6 cm2. 16 of 20 patients (75%) were revascularized prior to the initiation of TDA. Five patients (25%) were on hemodialysis, and all were on anticoagulant or antiplatelet therapy. Within the maximum observation period of 40 days, improvement of the wound occurred in 17 of 20 (85%) cases. Size reduction was noted in 16 of 20 patients (80%) and six of the 20 ulcers (30%) healed completely. Two ulcers (10%) were unchanged at 40 days and one ulcer (5%) worsened, probably due to poor compliance. Conclusion: Wagner III and IV diabetic ulcers were treated with TDA as the primary debridement agent. Aim: An acid-based debriding gel (TDA*) works through a strong hygroscopic action. When applied onto biofilm and/or necrosis, rapid desiccation and denaturation of microorganisms and tissues occurs. Typically, quick sloughing follows. In a previous study, TDA was shown to assist in the rapid development of granulation tissue, a necessary step in healing by secondary intention. Negative pressure wound therapy (NPWT) also supports the development of granulation tissue. The aim of our study was to assess the combination of TDA and NPWT on efficacy of the development of granulation tissue. Method: TDA was applied for 60 seconds and rinsed off, after which NPWT (at -125 mmHg) was applied for up to one week. The percentage of granulation was assessed on a 3-daily basis. Results / Discussion: 12 ulcers (in 12 patients) of the lower leg participated. Ulcers were venous (N=5), arterial (N=2), and mixed venous/arterial (N=1). One lesion was post-trauma (etiology unknown: N=3). Average size was 308 cm2 and average wound-age was 13 months. Each patient had at least one serious comorbidity. Average time to complete granulation was 13,2 days (range: 7-21). There were no side effects to NPWT or TDA treatment. Conclusion: In this study, combining TDA therapy with NPWT was shown to assist in the rapid development of granulation tissue. This indicates that healing by secondary intention is supported by this treatment routine. Background and Aims: Most data on liver assessment in type 2 diabetes mellitus (T2DM) patients are from retrospective cohorts with selection bias. We aimed at appraising the feasibility, results, and benefits of an outpatient systematic noninvasive screening for metabolic dysfunction-associated fatty liver disease (MAFLD) severity and determinants in T2DM patients., Methods: We conducted a 50-week cross-sectional study enrolling adult T2DM outpatients from a diabetes clinic. An algorithm based on guidelines was applied using simple bioclinical scores and, if applicable, ultrasound and/or elastometry., Results: Two hundred and thirteen patients were included. Mean age and body mass index were 62 years and 31 kg/m 2 and 29% of patients had abnormal transaminase levels. The acceptance rate of additional liver examinations was 92%. The prevalence of MAFLD, advanced fibrosis and cirrhosis was 87%, 11%, and 4%, respectively. More than half of the cases of advanced fibrosis had not been suspected and were detected by this screening. MAFLD was associated with poor glycemic control, elevated transaminases, low HDL-C and the absence of peripheral arterial disease. Advanced fibrosis was linked to high waist circumference and excessive alcohol consumption, which should be interpreted with caution owing to the small number of patients reporting excessive consumption., Conclusions: Simple bioclinical tools allowed routine triage of T2DM patients for MAFLD severity, with high adherence of high-risk patients to subsequent noninvasive exams., Competing Interests: The authors have no conflict of interests related to this publication., (© 2023 Authors.)

Background: Few data are available on COVID-19 outcomes among pregnant women in sub-Saharan Africa (SSA), where high-risk comorbidities are prevalent. We investigated the impact of pregnancy on SARS-CoV-2 infection and of SARS-CoV-2 infection on pregnancy to generate evidence for health policy and clinical practice., Methods: We conducted a 6-country retrospective cohort study among hospitalized women of childbearing age between 1 March 2020 and 31 March 2021. Exposures were (1) pregnancy and (2) a positive SARS-CoV-2 RT-PCR test. The primary outcome for both analyses was intensive care unit (ICU) admission. Secondary outcomes included supplemental oxygen requirement, mechanical ventilation, adverse birth outcomes, and in-hospital mortality. We used log-binomial regression to estimate the effect between pregnancy and SARS-CoV-2 infection. Factors associated with mortality were evaluated using competing-risk proportional subdistribution hazards models., Results: Our analyses included 1315 hospitalized women: 510 pregnant women with SARS-CoV-2, 403 nonpregnant women with SARS-CoV-2, and 402 pregnant women without SARS-CoV-2 infection. Among women with SARS-CoV-2 infection, pregnancy was associated with increased risk for ICU admission (adjusted risk ratio [aRR]: 2.38; 95% CI: 1.42-4.01), oxygen supplementation (aRR: 1.86; 95% CI: 1.44-2.42), and hazard of in-hospital death (adjusted sub-hazard ratio [aSHR]: 2.00; 95% CI: 1.08-3.70). Among pregnant women, SARS-CoV-2 infection increased the risk of ICU admission (aRR: 2.0; 95% CI: 1.20-3.35), oxygen supplementation (aRR: 1.57; 95% CI: 1.17-2.11), and hazard of in-hospital death (aSHR: 5.03; 95% CI: 1.79-14.13)., Conclusions: Among hospitalized women in SSA, both SARS-CoV-2 infection and pregnancy independently increased risks of ICU admission, oxygen supplementation, and death.
