Treatment strategies vary depending on factors such as diagnosis, disease severity, and costs. Staffing resources and workload impact the duration and frequency of interventions. Understanding the workload of procedures may enhance patient care quality.To determine the frequency, duration, and workload of physiotherapy procedures applied to adult ICU patients. Secondarily, we analyzed the time spent per shift on teaching-related and non-procedure-related activities.Longitudinal panel study conducted in a 12-bed university hospital. Physiotherapy sessions applied to adult ICU patients were included. The list of procedures was derived using the Delphi method. The investigator followed physiotherapists to measure the duration of procedures and calculate a workload index for each (determined as duration x frequency). Duration of teaching-related and non-procedure-related tasks was also recorded.A total of 339 physiotherapy sessions were analyzed across 79 shifts, involving 181 patients and 19 physiotherapists. Procedures with the highest workload index were: out-of-bed mobilization, patient positioning, passive limb mobilization, active exercises, artificial airway suctioning, and equipment management. Time distribution across the shifts was as follows: direct patient care 40% (SD 12%), non-procedure-related activities 20% (SD 6%), and teaching-related activities 10% (range: 4%-19%).The procedures with the highest workload index were those directly related to rehabilitation, such as mobilization and exercises, and those not specific to patient condition, like positioning and equipment management. This paper, a mixed-methods study conducted in 2023, comprised a scoping review and a thematic analysis covering the period from 2000 to 2023 as the first phase of the study. In this regard, PubMed, Scopus, ProQuest, and the Cochrane Database of Systematic Reviews were searched. In the second phase, the authors engaged a group of experts to prioritize the most important practices suitable for Iranian healthcare centers to address climate change using the Grey MEREC-MABAC integrated approach. The scoping review resulted in 38 studies, and the thematic analysis revealed 11 primary categories of practices employed to address climate change. Subsequently, the Grey MEREC-MABAC approach indicated that the themes of 'Green Environment' (CC = 0.0929), 'Green Equipment, Waste, and Water Management' (CC = 0.0677), and 'Green Energy' (CC = 0.0653) were identified as having the highest priority among the practices deemed most important in the literature for addressing climate change within Iranian healthcare centers. The study proposed that practices or activities promoting biodiversity and environmental sustainability, such as organic food farming and tree planting, should be considered paramount approaches in Iranian healthcare centers to combat climate change. Background: The integration of artificial intelligence (AI) in the organization of nursing care has continually evolved, driven by the need for innovative solutions to ensure quality of care. The aim is to synthesize the evidence on the use of artificial intelligence in the organization of nursing care. Methods: A scoping review was carried out based on the Joanna Briggs Institute methodology, following the PRISMA-ScR guidelines, in the MEDLINE, CINAHL Complete, Business Source Ultimate and Scopus
® databases. We used ProQuest—Dissertations and Theses to search gray literature. Results: Ten studies were evaluated, identifying AI-mediated tools used in the organization of nursing care, and synthesized into three tool models, namely monitoring and prediction, decision support, and interaction and communication technologies. The contributions of using these tools in the organization of nursing care include improvements in operational efficiency, decision support and diagnostic accuracy, advanced interaction and efficient communication, logistical support, workload relief, and ongoing professional development. Conclusions: AI tools such as automated alert systems, predictive algorithms, and decision support transform nursing by increasing efficiency, accuracy, and patient-centered care, improving communication, reducing errors, and enabling earlier interventions with safer and more efficient quality care. Purpose Staff is essential to the university’s efficient administrative operations, which are critical for education, research, and service. Medical schools, often independent, need specialized administrative elements. This study explores how medical school staff perceives the organization using the Six-Box model and evaluates their perceived organizational support, job satisfaction, and organizational commitment based on the concept of job attitudes. Methods This study employs a mixed-methods approach, integrating quantitative and qualitative data via a convergent parallel design. It simultaneously collects and analyzes data from a survey and consensus workshop for medical school staff. The survey data were statistically analyzed (IBM SPSS ver. 25.0; IBM Corp., USA), and the workshop discussions were subjected to content analysis. The findings combined provide a comprehensive understanding of the medical school administrative system. Results Quantitative analysis revealed purpose (3.80) as the highest-rated organizational perception and rewards (2.72) as the lowest. Similarly, job satisfaction was highest (3.63) in job attitudes, while perceived organizational support (2.96) was the lowest. Objectives: Current policies for older patients do not adequately address the barriers to effective implementation of optimal care models in New Zealand, partly due to differences in patient definitions and the in-patient pathway they should follow through hospital. This research aims to: (a) synthesise a definition of a complex older patient; (b) identify and explore primary and secondary health measures; and (c) identify the primary components of a care model suitable for a tertiary hospital in the midland region of the North Island of New Zealand. Method: This mixed-methods study utilised a convergence model, in which qualitative and quantitative data were investigated separately and then combined for interpretation. Semi-structured interviews (n=11) were analysed using a general inductive method of enquiry to develop key codes, categories and themes. Univariate data analysis was employed using six years of routinely collected data of patients admitted to the emergency department and inpatient units (n=261,773) of the tertiary hospital. Results: A definition of a complex older patient was determined that incorporates chronic conditions, comorbidities and iatrogenic complications, functional decline, activities of daily living, case fatality, mortality, hospital length of stay, hospital costs, discharge destination, hospital readmission and emergency department revisit and age – not necessarily over 65 years old. Well-performing geriatric care models were found to include patient-centred care, frequent medical review, early rehabilitation, early discharge planning, a prepared environment and multidisciplinary teams. Conclusions: The findings of this New Zealand study increase understanding of acute geriatric care for complex older patients by filling a gap in policies and strategies, identifying potential components of an optimal care model and defining a complex geriatric patient. Implications for Public Health: The findings of this study present actionable opportunities for clinicians, managers, academics and policymakers to better understand a complex older patient in New Zealand, with significant relevance also for international geriatric care and to establish an effective acute geriatric care model that leads to beneficial health outcomes and provides safeguard mechanisms. 13. Designing a Smart human resources Architecture Model for Medical Sciences Universities

Background: This study was conducted with the aim of designing the architectural model of human resources of universities of medical sciences in southeastern provinces of Iran with the intelligent approach. Methods: This study was of a mixed method.The statistical population in the qualitative part was 20 experts of medical sciences managers in the southeastern provinces of Iran, namely Sistan and Baluchistan, Kerman, Hormozgan and South Khorasan. Also, 264 people were selected using G*Power for quantitative part. Sampling of this part was random cluster sampling. The data collection tool was a researcher-made questionnaire containing 77 items that included six dimensions of intelligent human resources architecture and six dimensions of intelligentization. The software used was Smart-PLS and SPSS-16. Results: The proposed model identified six main dimensions of human resource intelligentization, including intelligent systems and management (0.960), intelligent learning (0.955), intelligent architecture strategy (0.953), core technology (0.945), and knowledge management (0.451). The key components within these dimensions included intelligent performance evaluation, job task specialization, fostering creativity, and organizational efficiency. The model's validity was confirmed in terms of reliability (Cronbach's alpha and CR above 0.7), convergent validity (AVE above 0.5), and appropriate R² and t-values. The findings indicated that the model's dimensions and components align with the needs of medical universities. Conclusion: University managers should try to draw and compile the horizon of intelligent organizational architecture and in line with the implementation policies of intelligentization and plan to acquire new technologies in the field of artificial intelligence and use it.

Acute leukemia is a rapidly progressive cancer of the blood and bone marrow that requires a high degree of complex, specialized, resource-intensive clinical and supportive care. The aging Canadian population has introduced an unprecedented demand on the health care system for a variety of illnesses, including acute leukemia. The purpose of this work was to develop organizational requirements for service providers delivering care for patients aged 18 years and older with acute leukemia within a single-payer health care system in Ontario. This initiative was intended to support streamlining high-quality health care across Ontario. We worked collaboratively with an expert panel to conduct a review of the literature to synthesize the organizational requirements for delivering acute leukemia care. A total of 229 requirements were developed. The requirements were categorized into themes including (1) facility requirements, including infrastructure, data management, safety, policies and procedures; (2) availability of clinical services and service complexity; (3) personnel, including roles, responsibilities, and ongoing education; (4) patient care; (5) quality management; (6) clinical research; and (7) laboratory services. These requirements will act as a framework for the provision of service, complexity of care, safety, accessibility, and quality care across all levels from the patient, organization, and system perspectives. This framework will help support person-centred care, emphasizing providing care close to home, while optimizing the use of specialized resources. Moving forward, Ontario Health (Cancer Care Ontario) will continue to work with acute leukemia service providers in the province to determine compliance and focus improvement efforts in priority areas.

This study aimed to analyze the role of nurses in hospital management in the face of COVID-19. The study had a qualitative, descriptive, and exploratory approach. The setting was a hospital that was completely transformed to care for patients with COVID-19. At the time of data collection, ten nurses managed the services, and all participated in the semi-structured interview. After thematic analysis, the data were presented in three categories, representing the elements of Donabedian’s triad: structure, process, and result. Category 1 highlighted the hospital structure reconfiguration based on material and people management; category 2 addressed the work process restructuring to achieve goals with safety and quality; and category 3 focused on nurses’ experiences in describing the results achieved and expected. The analysis highlighted the importance of teamwork, involvement, and adaptation of managers in the face of the challenges of a new and life-threatening disease, scarce resources, and the complexity of human relationships in the crisis. In transformational leadership, these nurses encouraged behavior change, professional growth, and resilience. [ABSTRACT FROM AUTHOR]

Introduction: We measured and analyzed the overall and industrial hygiene teaching loads among existing official master's programs that offer certification as upper level technician in occupational risk prevention for the 2023-2024 term in Spain. Methods: We consulted the Ministry of Universities QEDU and RUCT websites, as well as the official master's degrees program websites. We collected information on public or private ownership of the university, whether or not the teaching was in-person, the required credits for attaining one or all three specialties, the credits specific to industrial hygiene, the program location and tuition costs for the master's degree. Results: Out the 53 degree programs identified, 29 accredit all three specialties (hygiene, safety and ergonomics/psychosociology), requiring fewer than 80 credits. Another six programs can be completed without reaching this minimum; thus, 35 degrees (two-thirds) do not meet the duration requirements mandated by Royal Decree 39/1997 to certify the three specialties. For training in industrial hygiene, only 11 degrees (one-fifth) offer at least 17 credits between the common part and the specialty and comply with the minimum 80 credits for the three specialties. Conclusions: Most degrees do not provide the training hours required for certification as upper-level technicians in occupational risk prevention in industrial hygiene. We propose creating an enabling degree, updating the contents, and recuperating the authorization of the labor authority, which guarantees compliance with the duration and contents of the training plans described in Royal Decree 39/1997, together with those described by ANECA for the verification and reaccreditation of the quality of the degrees. Data collection involved 25 nurses during May and June 2022 through semi-structured interviews. The findings highlight two facilitators that impact management in the basic health unit: the collaborative team and effective communication. The barriers identified include the management of material and human resources. Nurses serving as team coordinators in the family health strategy play a crucial role in managing primary health care nursing. - Abstract
Objective: To investigate the current situation of awareness and practice testicular self - examination (TSE) among medical students in Wuhan, and to explore the relationship between TSE knowledge and practice. Methods: A total of 500 male medical students from the First Clinical College and the Second Clinical College of Wuhan University, the School of Health and Nursing, Wuchang Institute of Technology, the Department of Medicine, Wuhan City College and the Hubei University of Chinese Medicine from January 2023 to November 2023 were selected by convenient sampling. The present situation of TSE consciousness, practice and knowledge of male medical students, and the barriers and facilitators influencing TSE practice, were analyzed by using the Chinese version of TSE consciousness scale. Results: The average age of the 500 respondents was (21.93±1.72) years, only 45.4% of the students had heard of TSE. In terms of TSE awareness, 31.3% respondents believed that all men should understand TSE, 57.7% believed that TSE contributes to the early diagnosis of TC, and 43.6% believed that men should start TSE between 21 and 25 years of age, 52.9% of the students did not know the optimal frequency of TSE. In terms of TSE practice, only 37.4% of the students had ever done TSE, but only 22.5% practiced regularly, and 22.0% of the students knew the procedures and procedures of TSE. On the TSE knowledge, the accuracy of four items is above 50%, and the accuracy of the remaining five items is less than 50%. In terms of barriers and facilitators influencing TSE practice, for most respondents, the barrier to TSE was their fear of finding a lump and feeling embarrassed about TSE, and felt that the test should be done only in older men. Conclusions: It is necessary to emphasize the importance of TSE in the training of medical students, so that these students acquire the health knowledge and skills of TSE, and encourage male medical students to undergo TSE regularly, so as to better promote the social effect in public health education. Objective: The study aims to identify the effects of the University Reform during the 1978-1982 management of the University of São Paulo at Ribeirão Preto School of Nursing (EERP-USP). Methodology: Study in the field of History, in the area of History of Nursing, taking the Micro-History approach. To reach its objective, the study used Thematic Oral History. Results: The advances identified in management were related to human resources, physical infrastructure, consolidation of postgraduate courses, regulations and training of health professionals. As effects of the University Reform, it was observed the requirement, at the University of São Paulo, of the category of full professor for the exercise of direction, a fact that led to sixteen years of direction by non-nurse professors, adherence to the unification of the vestibular, the consolidation of graduate studies and curricular changes. Final Remarks: The study focused on the work of the two leaders at the time, but it is inferred that other people may have exercised leadership in the realization of the advances recorded in the minutes of the faculty board. Background. 27 percent of PHC providers in Poland employ the healthcare coordinator. Objectives. The aim was to determine the expected competencies of the coordinator in light of the needs of employers and the possibilities of an educational system. Material and methods. In the first stage, 24 study programs were analyzed in terms of qualifications of a new profession. In the second, a survey was conducted to assess the usefulness of knowledge, skills and social competencies among the representatives of 22 PHC providers. In the third, an expert panel developed the key competencies of the coordinator. Results. The analysis identified a number of new, unique qualifications. In the area of knowledge: understanding of work in PHC including new concepts (patient-centered care, value-based care, deinstitutionalization), population approach, key healthcare measures, interpersonal and health communication, basics of clinical work of all members of an interdisciplinary team, reporting of services. In the area of skills: communication using call-center systems, organization of teamwork, identification of patients for HC interventions in IT systems, effective recruitment, evaluation of PHC activities, health coaching and advocacy, providing health campaigns in social media. With regard to social competencies: the coordinator should build and maintain permanent relationships between the members of an interdisciplinary teams, be open to changes and new solutions, can be a guide for a patient and has a patient-centric attitude. Conclusions. We defined the professional profile of an iPHC coordinator as a guide oriented on patient’s needs, who is open to changes and innovations and who is effective in communication and interdisciplinary team-work. Abstract Objective: to analyze the availability (in terms of stock and composition) and accessibility (in terms of geographical distribution) of the nursing workforce in Brazil. Method: this is a descriptive, cross-sectional study with retrospective data collection, identified by combining databases available on institutional websites and structured according to indicators from the World Health Organization’s “National Health Workforce Accounts”. The study considered nursing professionals at senior level (nurses) and middle level (nursing auxiliaries and technicians). Indicators of stock, composition, distribution (by age group and gender) and the ratio of nurses to doctors were included. Results: there was an increase in the number of personnel between 2005 and 2010, mainly in middle and technical level professionals. There are more personnel aged between 36 and 55, with a predominance of women in all categories, despite the increase in men. There was an uneven distribution of personnel across the country’s regions, with the Southeast having the largest number of professionals. The ratio of nurses to doctors is less than one in the South and Southeast. Conclusion: despite the large number of nurses, their distribution is uneven. The growth of nursing technicians has significantly outstripped that of nurses, indicating more intensive technical training policies than those found in higher education.

27. Development and Psychometric Evaluation of the Career Growth Scale for Nurses

Summary: Purpose: The purpose of this study was to develop the career growth scale for nurses (CGSN) and evaluate its psychometric properties. Methods: This study was conducted in four phases: (1) creating a pool of potential items through a qualitative design; (2) developing a preliminary scale using a modified two-round Delphi process; (3) refining the preliminary scale to finalize the scale using a cross-sectional survey; and (4) evaluating the psychometric properties of the final scale using another cross-sectional survey. A sample of 858 registered nurses from 12 general hospitals was recruited for this study. Results: The final scale consisted of 17 items categorized into three factors: career goal progress, professional ability and attribute improvement, and career promotion and prestige increase. The three factors accounted for 75.4% of the observed variance in career growth. The overall Cronbach's α was .96, and the intraclass correlation coefficient was .92. The content validity index was .97. Confirmatory factor analysis showed acceptable model fitness. Conclusions: These results showed that the CGSN has good psychometric properties and can be used to evaluate specific career growth among nurses. This new instrument can further help nurse managers and clinical nurses themselves assess career growth and identify unsatisfactory aspects of growth, thereby designing tailored training programs and evaluating the effectiveness of such interventions.

BACKGROUND: The University of South Australia's Bachelor of Physiotherapy course, 'Work Health and Safety (WHS) Practice', provides students with introductory practical WHS experience. Students undertake 80-hour effort WHS projects for industry, culminating in a report for hosts. OBJECTIVE: To establish the types of industry that host WHS undergraduate physiotherapy placements and the nature of activities that students conduct. METHODS: Reports were retrospectively, systematically classified using a descriptive, document content analysis approach. Demographic data were collected: report and student numbers, placement locations and industry types. Selected reports were used as a subset for further analysis with definitions created to classify the nature of placement activities. RESULTS: Most reports were written by a pair of students (88%, n = 269), with placements based in metropolitan Adelaide (91%, n = 284). Various industries hosted students, including healthcare and social assistance (40%, n = 117) and manufacturing (30%, n = 89). Reports primarily included risk management activities as required by WHS legislation (97%, n = 229). CONCLUSION: Physiotherapy student WHS activities maintain close links with industry, involving stakeholders (workforce, students, University of South Australia and academics). At an entry-level standard, the student WHS activities align well with some criteria set as key competencies for Australian Occupational Health physiotherapy practitioners. [ABSTRACT FROM AUTHOR]

In today's complex healthcare landscape, exacerbated by resource constraints at various levels, optimization of health professionals' roles is becoming increasingly paramount. Interprofessional collaboration, underpinned by role recognition and teamwork, leads to improved patient and organizational outcomes. Hospital pharmacists play a pivotal role in multidisciplinary teams, and it is imperative to understand multidisciplinary viewpoints on hospital pharmacists' roles to guide role prioritization and organizational efficiency. However, no study extensively investigated multidisciplinary views on values of diverse pharmacist roles in tertiary settings. This study aims to address this gap by examining non-pharmacist health professionals' views on hospital pharmacists' roles, recognizing their specialized niches as a crucial step towards optimizing their roles and services in Australia and internationally. Multiple focus group discussions and interviews were held via a virtual conferencing platform. Study participants were recruited using the study investigators' professional networks who were non-pharmacist health professionals with experience working with pharmacists in hospital settings. Data were collected from transcripts of the focus group recordings, which were later summarized using descriptive statistics and thematic analysis. Overarching themes were categorized and mapped against work system models to conceptualize organizational implications of multidisciplinary feedback, linking them to patient and organizational outcomes. Twenty-seven health professionals participated across focus groups and interviews, with the majority of professions being doctors and nurses. Three major themes were identified as follows: (i) overarching perceptions regarding hospital pharmacists; (ii) professional niches of hospital pharmacists; and (iii) future opportunities to optimize hospital pharmacy services. Valued professional niches included patient and health professional educators, transition-of-care facilitators, and quality use of medicines analysts. The study highlights critical insights into hospital pharmacists' roles in Australia, identifying their niche expertise as vital to healthcare efficiency and success. Background: The speed of advance in medical education, creativity in technology, limit time for new work has created new vision in medical education. Considering the importance of developing Iran's global position in the scientific and technological in Southwest Asia and the importance of improving the quality of learning and education, the present study identifies and examines the current status of mobile learning indicators in medical sciences universities. Methods: This study was applied in terms of purpose, descriptive-correlation in nature and survey method. The statistical population of the study consists of specialists from different medical groups. Based on Morgan's table, the sample size was estimated to be 200 people who were selected by simple random. Mobile learning components were extracted using text analysis and interviews with experts. In order to comply with the principle of validity in the questionnaire, in addition to the opinions of supervisors and advisors, the validity of factor analysis has been used. Cronbach's alpha coefficient was estimated above 0.7, so the reliability of the questionnaire was confirmed. For data analysis, exploratory factor analysis and univariate analysis were used in Spss23 software. Results: Four factors (infrastructure, organizational planning, tools and equipment, human resources) and 16 indicators explain about 79.9% of mobile learning variance. Background: The quality of services is significant factors in evaluating the performance of organizations. The purpose of this research was to examine how emotional intelligence mediated the performance of Tehran's Imam Khomeini Medical personnel. Methods: A questionnaire was employed in this practical cross-sectional investigation. Its constituent parts were ascertained by a review of relevant literature in the form of the first suggested model and verification by professionals in the fields of management and healthcare. The Ale-Omran Hospital service quality indicators, Salovey & Mayer's emotional intelligence emotions, and questionnaires evaluating customer expectations and perceptions were among the study's instruments. In this regard, 360 questionnaires were sent online using random stratified sampling. Exploratory and confirmatory factor analysis and validity were analyzed. Results: At 0.6, the coefficient of determination, or R2, suggests that the predictor variables account for 60% of the variation in the dependent variable. When it comes to increasing quality-oriented services, "setting strategic goals" has a greater impact than other predictor factors, according to the F2 value of the effect size of the predictor variables. The association between quality-oriented services and quality-oriented structure is therefore verified, with emotional intelligence acting as a mediating factor. Conclusion: Hospital organizational structures must take into consideration the elements that are effective in providing high-quality medical services from the perspective of their patients, given the mediating role that emotional intelligence plays in the relationship between quality-oriented services and quality-oriented structures.

Purpose Research pharmacy effort required to safely and compliantly manage investigational products (IP) varies between studies. No validated tool exists in the United States to evaluate these differences in effort. The Vizient Pharmacy Research Committee Investigational Drug Services (IDS) Subcommittee previously developed a systematic complexity scoring tool (CST) through expert consensus to assign a complexity score for pharmacy effort. This project seeks to develop and validate complexity categories based on CST scores. Methods Vizient member institutions in IDS assigned a CST complexity score and a perceived complexity category (low, medium, or high) for study initiation and maintenance. Receiver operating characteristic (ROC) curve analysis defined the best CST score cutoff points for each complexity category. Comparing the CST-assigned to the user-perceived complexity category determined whether the CST-assigned complexity category aligned with practitioner assignment. Results A total of 322 responses were used to determine complexity score categories. The AUC values for study initiation and maintenance were 0.79 (P < 0.001) for the low/medium boundary and 0.80 (P < 0.001) for the medium/high boundary, suggesting the performance of the CST is good. The agreement between CST-assigned and user-perceived complexity categories was 60% for study initiation and 58% for maintenance. The Kendall rank correlation coefficient between the raters and ROC categories was strong, with a value of 0.48 for study initiation and 0.47 for maintenance. Conclusion Development of the CST allows IDS pharmacies to objectively measure the complexity of clinical trials, which is a significant step towards assessing workload and guiding resource allocation. 35. National Collegiate Athletic Association athletic trainers' response to the Arrington settlement: management, compliance, and practice patterns.

The primary purpose of this study was to assess Athletic Trainers' (ATs) report of NCAA member institution compliance with the Arrington settlement, the concussion lawsuit vs. the NCAA, and to elucidate compliance predictors. A secondary purpose was to provide a contemporary concussion management clinical practice pattern description among NCAA collegiate athletic trainers. Head Athletic Trainers from NCAA Division I, II, and III completed an electronic questionnaire in August 2020 regarding their institution's response to the Arrington Settlement and their current concussion management clinical practice patterns. The 37-item questionnaire included AT and institution demographics, current concussion management policies, and response to the Arrington settlement with a specific focus on the five settlement requirements. An overall compliance score on the five requirements, compliance on the individual requirements, and concussion management practices are reported with descriptives. Regression was used to identify specific predictors of both overall and individual settlement requirements. An ANOVA compared compliance by NCAA division level. Being pressured to be non-compliant was assessed between sexes by a chi-square. There were 223 respondents (21.8%), and overall compliance was high (4.1 ± 0.7) with the five required Arrington Settlement components. Settlement requirement 1, pre-season baseline testing, and requirement 5, presence of trained personnel at all contact sport practices, had the lowest compliance rates at 44.8% and 73.3%, respectively. The number of sports the institution offered was the only significant predictor of each requirement. There was no difference in compliance between NCAA divisions. Although the overall rate of being non-compliant pressure was low (13.8%), females were 3.28x more likely report being pressured than males. NCAA institutions are generally compliant with the Arrington settlement; however, lack of clarity in the requirements, particularly requirement 1, raises potential concerns. Background Although it is known that resilience is negatively associated with burnout, and that certain interventions can effectively increase resilience, little is known about online resilience-enhancing interventions during the COVID-19 crisis. The aim of this study was to identify the association between an online resilience-enhancing intervention and workplace social support, and burnout among nurses working in the mainland and remote islands of Japan during the COVID-19 crisis. Methods Pretest-posttest was conducted between April 2020 and February 2021, and the questionnaire survey included the bidimensional resilience scale, the Japanese version of Pine's Burnout Measure, and the workplace social support scale. Changes in burnout, resilience, and social support and the associations with nursing discussions as intervention were analyzed. Participants were 98 Nurses with 1 to 10 years of experience from Japan's mainland and remote island hospitals of Kagoshima Prefecture participated in a baseline survey in April 2020. Of these, 76 participated in a secondary survey in September 2020, and 69 participated in the intervention program and a third survey in February 2021. The online intervention over Zoom consisted of small-group nursing discussions based on the broaden-and-build theory. Results Changes in burnout showed a significant negative association with change in workplace social support (Coef. = -0.019, 95% CI -0.035--0.003), as did the interaction between change in acquired resilience and intervention (Coef. = -0.088, 95% CI -0.164--0.011). Conclusion Change in workplace social support was significantly negatively associated with changes in burnout, as was the interaction of intervention and acquired resilience. Promotion of this intervention and making workplace social support more accessible may contribute to reduce burnout in nurses. [ABSTRACT FROM AUTHOR]

Aim:Family health centers (FHCs) are reimbursed for their current expenditures based on a classification of four clusters in Turkey. This study compared the coordination, comprehensiveness, continuity, accessibility, and the first contact of care among different reimbursement FHC groups.Methods:The data were obtained from the Turkish data of the Quality and Costs of Primary Care in Europe study. Data was collected in provinces from six geographical regions. Physicians and patients from Classes A and B FHCs were called the first group, and others were called the second group.Results:A total of 296 physicians and 2623 patients were enrolled. According to the reimbursement groups, 593 (22.6%) patients received services from the first group and 2012 (77.4%) patients from the second group. The first contact with care and the admission frequency of 3 or more in the last six months were higher in the first group (respectively, 99.2% vs. 97.7%, p=0.027; 55.4% vs. 49.6%, p=0.015).Conclusion:The reimbursement classification did not make a difference in coordination, comprehensive care, continuity, accessibility, and being the first contact of care. Therefore, the current classification does not contribute to improving the quality of primary care in terms of service provision.

Abstract Background Artificial intelligence (AI) is increasingly used to support general practice in the early detection of disease and treatment recommendations. However, AI systems aimed at alleviating time-consuming administrative tasks currently appear limited. This scoping review thus aims to summarize the research that has been carried out in methods of machine learning applied to the support and automation of administrative tasks in general practice. Methods Databases covering the fields of health care and engineering sciences (PubMed, Embase, CINAHL with full text, Cochrane Library, Scopus, and IEEE Xplore) were searched. Screening for eligible studies was completed using Covidence, and data was extracted along nine research-based attributes concerning general practice, administrative tasks, and machine learning. The search and screening processes were completed during the period of April to June 2022. Results 1439 records were identified and 1158 were screened for eligibility criteria. A total of 12 studies were included. The extracted attributes indicate that most studies concern various scheduling tasks using supervised machine learning methods with relatively low general practitioner (GP) involvement. Importantly, four studies employed the latest available machine learning methods and the data used frequently varied in terms of setting, type, and availability. Conclusion The limited field of research developing in the application of machine learning to administrative tasks in general practice indicates that there is a great need and high potential for such methods. However, there is currently a lack of research likely due to the unavailability of open-source data and a prioritization of diagnostic-based tasks. Future research would benefit from open-source data, cutting-edge methods of machine learning, and clearly stated GP involvement, so that improved and replicable scientific research can be done.

Objective: In light of the health promotion approaches and strategies proposed in the Ottawa Charter, endorsed by the World Health Organization, it was analyzed how the national health authorities of Colombia, Brazil and Mexico, which have health systems structured differently, conceptualize and guide health promotion. Methodology: Documentary analysis of official information from the agencies in charge of health promotion in each country. Subsequently, a coherence analysis around this concept was carried out. Results: There are different levels of coherence regarding health promotion: in Colombia the concept is transmuted into individual risk management through the documents that implement the Ten-Year Public Health Plan; in Brazil, the concept of health promotion is applied coherently although operational priorities are focused on individual aspects, and each state and municipality implements it according to their needs and capabilities; in Mexico, health promotion is institutionalized, but the access of citizens to the service depends on insurance and individual agency. Conclusions: Health promotion is done within a framework of health service provision focused on individual risk control. Officially, the three countries showed coherence with the health promotion postulates set out in the Ottawa Charter, without adequate coherence in its implementation. [ABSTRACT FROM AUTHOR]

This mixed-methods study aims to understand what the perceptions of leaders and healthcare professionals are regarding causes of disparities, cultural competence, and motivation before launching a disparity reduction project in hypertension care, contrasting perceptions in Federally Qualified Health Centers (FQHCs), and in a non-FQHC system. We interviewed leaders of six participating primary care systems and surveyed providers and staff. FQHC respondents reported more positive cultural competence attitudes and behavior, higher motivation to implement the project, and less concern about barriers to caring for disadvantaged patients than those in the non-FQHC practices; however, egalitarian beliefs were similar among all. Qualitative analysis suggested that the organizational missions of the FQHCs reflect their critical role in serving vulnerable populations. All system leaders were aware of the challenges of provider care to underserved groups, but comprehensive initiatives to address social determinants of health and improve cultural competence were still needed in both system types. The study provides insights into the perceptions and motivations of primary care organizational leaders and providers who are interested in improving chronic care. It also offers an example for care disparity programs to understand commitment and values of the participants for tailoring interventions and setting baseline for progress. [ABSTRACT FROM AUTHOR]

Aim: This paper aims to propose Park's sweet spot theory‐driven implementation strategy, which makes optimal safe staffing policy really work in nursing practice. Background: For the last 40 years, mainstream nursing workforce research has emphasized that having more registered nurses leads to better patient outcomes, and yet staffing policies have failed to implement this crucial concept. Meanwhile, global nursing shortages have become rampant, a problem that only dilutes the skill‐mix ratios in the nursing workforce. Low fertility and an aging population worldwide are then accelerating these shortages. These dire circumstances may be persisting because of unclear, unsubstantiated cost‐efficiency in the nursing workforce. For this reason, there remains a dearth of well‐researched evidence for a clear threshold on optimal safe staffing levels that could maximize quality of care relative to cost given limited healthcare financial budgets and which could also be fitted into each care setting. Along with that, an implementation strategy for optimal safe staffing levels is nonexistent. Sources of evidence: An implementation strategy has been developed through interdisciplinary consilience‐based theory synthesis of both prospective theory and regulatory focus theory combined with Park's optimized nursing staffing (sweet spot) estimation theory. Discussion/conclusions: A theory‐driven novel implementation strategy is proposed, which functions as a nudge strategy that uses the synchronous balance of quality of care, nurse staffing, and cost. It illustrates (1) how to create shared value among patients, nurses, and stakeholders through robust evidence‐based, informed shared decision‐making rationales on the optimal safe nurse staffing levels and (2) how to induce stakeholders to overcome resistance to innovation and improve their nursing workforce through value chain in management science. Implications for nursing workforce policy: This novel implementation strategy may be a viable solution to mitigate the nursing shortage by leading stakeholders (1) to compete with each other (on the basis of nursing sufficiency) and (2) to competitively demonstrate the patient‐centered value (patient‐perceived care quality relative to cost) of their institutions. ABSTRACT Objective: To develop a theoretical model for management of research groups in nursing graduate programs. Method: This is a Grounded Theory developed in a stricto sensu nursing graduate program of a public university. Data collection was conducted from April to October 2018 and the theoretical sample consisted of 21 participants arranged in three sample groups. Results: Research activities are linked to research groups, which require human, material and financial resources to develop their studies. These conditions call for management strategies and national and international interactions that result in theoretical, scientific and technological development of the profession. Final considerations: The theoretical model for management of research groups can serve as a guide for graduate programs in nursing and health for planning the work process and so that they can contribute with research of great impact for society.

Background: The circumstances of employment are determined by a variety of elements in today's world. The present study investigated the mediating role of organizational partnership and hospital staff management in improving hospital performance and staff satisfaction. Methods: This study was applied, descriptive-correlation. The statistical population included all employees of hospitals in Bushehr province. Using stratified random sampling, 384 participants were chosen as the sample size based on Morgan's table. Organizational development was assessed using the 36-question questionnaire developed by Avolio, Gardner, and Valium, while organizational participation was assessed using the Hosseininoveh questionnaire. 30 elite individuals' opinions were sought after in order to verify and examine the face and content validity of the surveys. Over 0.70 indicates acceptable reliability, according to Cronbach's alpha coefficient. Data analysis tools included SPSS 23 and the Sobel test. Results: According to the findings, there is a link between organizational development in public organizations and internalized ethical perspective, idealized influence, and intellectual stimulation through organizational collaboration. Conclusion: Labor productivity is one of the most significant factors in economic policymaking. According to the studies and the trust of experts, one can expect productivity in other production factors when the factors affecting organizational development are first known.

ABSTRACT Objective To reduce surgical center idleness by analyzing the competitive structure of the surgical center in a hospital, and thereby generate value in operations and increase hospital revenue. Methods The study used qualitative and quantitative methods and an action research approach involving the surgical center leadership of a small private specialized hospital in southeastern Brazil. We used the Strengths, Weaknesses, Opportunities, and Threats or SWOT tool to analyze the competitive structure of the surgical center and then implemented interventions as proposed by the science of improvement method proposed by the Institute of Healthcare Improvement. Results By applying the SWOT tool, we identified a concentration of surgeries in the specialty of Otolaryngology and the need to establish a health management system to reduce the idleness of the operating rooms. Based on subsequent intervention, procedures from other specialties were inserted that increased surgical production by 2.62X, reduced idleness by 67.84%, and increased revenue by over US$ 276,609.87 in 2018 compared to the previous year 2017. Conclusion Investing in quality, surgical schedule management, and inducting new surgeons to the clinical staff resulted in decreased surgical idleness, increased production, better uniformity in scheduling, and increased revenue, while costs remained below the linear trend, allowing for increased profits.

Objective To analyze the research hotspots and theme in the field of infection prevention and control(infection control) management in operating room in recent six years in China, so as to provide reference for further research on infection control management in operating room. Methods The articles related to infection control management in operating room published in China National Knowledge Infrastructure(CNKI), Wanfang and VIP databases from 2015 to 2021 were selected as the research objects, and Nvivo software query, coding and cluster analysis were adopted as the main methods to visually present the research hotspots and theme groups in the field of infection management in operating rooms. Results A total of 128 literatures with 30 high-frequency keywords were included. After free coding, 8 parent nodes, 28 child nodes, 60 secondary nodes and 1 736 coding reference points were formed. Six theme groups had been formed, which were the guidelines for infection control of COVID-19 infection, construction and management of clean operating rooms, management of relevant personnel in operating rooms, management of surgical supplies, operating room environment control, theoretical support and occupational exposure control. Conclusion The research content related to infection control management in operating rooms is becoming increasingly mature, and is constantly improving in line with the development of the times. The research direction and theory are also showing a trend of diversification. In response to the huge challenge of COVID-19 situation, it is supposed to preparing for surgery protection in general hospitals actively, ensure the safety of patients and medical staff, and prevent hospital infection events. Introduction: The article presents the professional biography of a great leader in Brazilian nursing, narrated from photographs and documents from the period. Objective: to describe the biography of nurse Edith de Magalhães Fraenkel and analyze the content of her photographic iconography, covering the period from 1942 to 1955. Methodology: This is a qualitative, historical=social and biographical research, guided by the methodology of analysis and interpretation of photographic sources, by Boris Kossoy. Results: Edith de Magalhães Fraenkel was a leading figure in Brazilian and São Paulo nursing. She had a notable performance in care, teaching and management of nursing schools and fought tirelessly to promote the development of the profession in Brazil and South America. The organization and curricular innovations implemented in the nursing course during her administration as the first director of the School of Nursing of the University of São Paulo are remarkable. She promoted the enrollment of men and women, white and black, from different social classes and of Brazilian and foreign nationality. Conclusion: Through Edith's direction, the School of Nursing of the University of São Paulo became a pioneer in promoting a culture of gender equality and valuing sociocultural diversity among students.
