INTRODUCTION: Eculizumab is effective and well tolerated in patients with antiacetylcholine receptor antibody-positive refractory generalized myasthenia gravis (gMG; REGAIN; NCT01997229). We report an interim analysis of an open-label extension of REGAIN, evaluating eculizumab's long-term safety and efficacy. METHODS: Eculizumab (1,200 mg every 2 weeks for 22.7 months [median]) was administered to 117 patients. RESULTS: The safety profile of eculizumab was consistent with REGAIN; no cases of meningococcal infection were reported during the interim analysis period. Objective:The treatment of tobacco addiction in Brazil has expanded in recent years; however, we must increase knowledge about the characteristics of individuals who adhere to cessation programs in order to adjust treatments to specific characteristics of the target population that favor success. The aim of this study was to describe the characteristics of smokers who present to Brazilian public health units seeking help to quit smoking based on the experience of a primary health care unit that covers a poor community in the city of Rio de Janeiro.Methods:Data were collected at a Teaching Health Center from January 03 2012 to January 03 2014.Results:Mean patient age was 49.32±11.82 years, and 71% were women. About half of the participants successfully quit smoking (n=125, 51%). Higher levels of nicotine dependence were associated with lower levels of smoking cessation. There was a notable decrease in the probability of remaining smoking throughout the first month of treatment. After 3 weeks of treatment, only 19% had not quit smoking. The probability of quitting smoking decreases by 2% for every additional year of age.Conclusion:There is a need to revise and expand current strategies to make them more effective in preventing smoking since childhood.

The impact of water pollution caused by industrial wastes on our society prompted the urgent search for efficient remediation processes. Moved by the increasing interest in this research field, we report herein on the design and fabrication of an innovative water filtering device based on a nylon membrane coated with a synthetic melanin polymer inspired by fungal allomelanins and generated via the oxidative polymerization of 1,8-dihydroxynaphthalene. The melanin coating, obtained via the straightforward and low-cost ammonia induced solid state polymerization technique, proved to be highly stable with no evidence of debonding phenomena, did not alter the porous structure of the nylon filter and its flow rate and induced a slight but appreciable improvement of its tensile properties. The adsorbing performances of the melanin coated membrane were tested toward the azo dye methylene blue both in batch and in-flow conditions and compared with those of the synthetic melanin polymer as a bulk. The data revealed that the adsorption process: a) fits quite well with the Langmuir isotherm model, suggesting a monolayer adsorption on a surface with homogeneous sites, with a maximum adsorption capacity of 262 mg/g; b) is a favourable, exothermic and spontaneous process; c) is governed by a pseudo-second order kinetics. The in-flow experiments revealed the high efficiency of the filtering membrane in removing MB up to 99%; moreover, tests carried out to assess the regeneration and reuse of the membrane showed that the methylene blue removal efficiency was not affected up to seven adsorbing/desorbing cycles.

Herein is delineated a first systematic framework for the definition of structure-antioxidant property relationships in the dihydroxynaphthalene (DHN) series. The results obtained by a combined experimental and theoretical approach revealed that 1,8-DHN is the best performing antioxidant platform, with its unique hydrogen-bonded peri-hydroxylation pattern contributing to a fast H atom transfer process. Moreover, the comparative analysis of the antioxidant properties of DHNs carried out by performing DPPH and FRAP assays and laser flash photolysis experiments, revealed the higher antioxidant power associated with an α-substitution pattern (i. e. in 1,8- and 1,6-DHN) with respect to DHNs exhibiting a β-substitution pattern (i. e. in 2,6- and 2,7-DHN). DFT calculations and isolation and characterization of the main oligomer intermediates formed during the oxidative polymerization of DHNs supported this evidence by providing unprecedented insight into the generation and fate of the intermediate naphthoxyl radicals, which emerged as the main factor governing the antioxidant activity of DHNs

Melanins are a wide class of natural pigments biosynthesized by different kinds of living organisms throughout all of the life domains, from bacteria to fungi, plants, and mammals. The biological functions played by these natural pigments are different (i.e., camouflage, radioprotection, thermoregulation) and ascribable to a peculiar set of physical-chemical properties making melanins a unique class of biopolymers. Among these, allomelanins from 1,8-dihydroxynaphthalene (1,8-DHNmel) produced by some Ascomycetes have recently attracted particular interest for their robustness and ability to protect fungi against both hostile (i.e., attack from fungicidal agents) and extreme (i.e., high energy radiations) environments. Starting from this background, in this mini-review we offer a panorama of the recent advances on the oxidative chemistry of 1,8-DHN leading to the formation of allomelanin mimics with tailored structural and functional properties for technological applications.

The fungal pathways of melanin synthesis have so far been little considered as a source of bio-inspiration in the field of functional materials, despite the interesting properties exhibited by Ascomycetes melanins from 1,8-dihydroxynaphthalene (1,8-DHN), including the ability to shield organisms from ionizing radiation. Herein, the processing techniques and characterizations of mycomelanin thin films obtained from the solid state polymerization of 1,8-DHN is reported for the first time. Overall, the results highlighted the role of synthetic mycomelanin thin films as a prototype of next generation bioinspired interfaces featuring high structural regularity and ultrasmooth morphology, high robustness against peroxidative bleaching and adhesion under water conditions, good biocompatibility and unprecedented effects in inducing the spontaneous differentiation of embryonic stem cells prevalently towards the endodermal lineages in the absence of added factors. These data open up new avenues towards the applications of this biomaterial in the fields of tissue engineering and regenerative medicine.

Disclosed herein is the first experimental evidence for a direct correlation between the broadband visible light absorption (“blackness”) and the coexistence of reduced and oxidized substructures in a set of model polymers from isomeric dihydroxynaphthalenes structurally related to fungal melanin (mycomelanin) from 1,8-dihydroxynaphthalene. Excellent linear plots (r2 = 0.97 and 0.94) were determined between the integrals of featureless absorbance curves over the 400–800 nm range, the electron spin density values in the EPR spectra and the width of selected oligomer peak clusters in the MALDI-MS spectra. Blackness, which is shown to be strongly interrelated with electron spin density, is thus proposed herein as a robust index of redox inhomogeneity and electron complexity reflecting the shift of oligomer populations toward highest oxidation states.

Background Complement is likely to have a role in refractory generalised myasthenia gravis, but no approved therapies specifically target this system. Results from a phase 2 study suggested that eculizumab, a terminal complement inhibitor, produced clinically meaningful improvements in patients with anti-acetylcholine receptor antibody-positive refractory generalised myasthenia gravis. We further assessed the efficacy and safety of eculizumab in this patient population in a phase 3 trial. Methods We did a phase 3, randomised, double-blind, placebo-controlled, multicentre study (REGAIN) in 76 hospitals and specialised clinics in 17 countries across North America, Latin America, Europe, and Asia. Eligible patients were aged at least 18 years, with a Myasthenia Gravis-Activities of Daily Living (MG-ADL) score of 6 or more, Myasthenia Gravis Foundation of America (MGFA) class II-IV disease, vaccination against Neisseria meningitides, and previous treatment with at least two immunosuppressive therapies or one immunosuppressive therapy and chronic intravenous immunoglobulin or plasma exchange for 12 months without symptom control. Patients with a history of thymoma or thymic neoplasms, thymectomy within 12 months before screening, or use of intravenous immunoglobulin or plasma exchange within 4 weeks before randomisation, or rituximab within 6 months before screening, were excluded. We randomly assigned participants (1:1) to either intravenous eculizumab or intravenous matched placebo for 26 weeks. Dosing for eculizumab was 900 mg on day 1 and at weeks 1, 2, and 3; 1200 mg at week 4; and 1200 mg given every second week thereafter as maintenance dosing. Randomisation was done centrally with an interactive voice or web-response system with patients stratified to one of four groups based on MGFA disease classification. Where possible, patients were maintained on existing myasthenia gravis therapies and rescue medication was allowed at the study physician's discretion. Patients, investigators, staff, and outcome assessors were masked to treatment assignment. The primary efficacy endpoint was the change from baseline to week 26 in MG-ADL total score measured by worst-rank ANCOVA. The efficacy population set was defined as all patients randomly assigned to treatment groups who received at least one dose of study drug, had a valid baseline MG-ADL assessment, and at least one post-baseline MG-ADL assessment. The safety analyses included all randomly assigned patients who received eculizumab or placebo. This trial is registered with ClinicalTrials.gov, number NCT01997229. Findings Between April 30, 2014, and Feb 19, 2016, we randomly assigned and treated 125 patients, 62 with eculizumab and 63 with placebo. The primary analysis showed no significant difference between eculizumab and placebo (least-squares mean rank 56·6 [SEM 4·5] vs 68·3 [4·5]; rank-based treatment difference -11·7, 95% CI -24·3 to 0·96; p=0·0698). No deaths or cases of meningococcal infection occurred during the study. The most common adverse events in both groups were headache and upper respiratory tract infection (ten [16%] for both events in the eculizumab group and 12 [19%] for both in the placebo group). Myasthenia gravis exacerbations were reported by six (10%) patients in the eculizumab group and 15 (24%) in the placebo group. Six (10%) patients in the eculizumab group and 12 (19%) in the placebo group required rescue therapy. Interpretation The change in the MG-ADL score was not statistically significant between eculizumab and placebo, as measured by the worst-rank analysis. Eculizumab was well tolerated. The use of a worst-rank analytical approach proved to be an important limitation of this study since the secondary and sensitivity analyses results were inconsistent with the primary endpoint result; further research into the role of complement is needed. Developing effective strategies to increase the chemical stability and to fine-tune the physico-chemical properties of melanin biopolymers by rational control of π-electron conjugation is an important goal in materials science for biomedical and technological applications. Herein we report that poly-1,8-dihydroxynaphthalene (pDHN), a non-nitrogenous, catechol-free fungal melanin mimic, displays a high degree of structural integrity (from MALDI-MS and CP/MAS 13 C NMR analysis), a strong radical scavenging capacity (DPPH and FRAP assays), and an unusually intense EPR signal (g=2.0030). Morphological and spectral characterization of pDHN, along with deassembly experiments in ionic liquids, indicated amorphous aggregates of small globular structures with an estimated stacking distance of 3.9 A and broadband absorption throughout the visible range. These results indicate that DHN-based melanins exhibit a high structural integrity and enhanced antioxidant and free-radical properties of potentially greater biomedical and technological relevance than for typical indole-based eumelanins.

In the absence of an effective vaccine or treatment for the disease, the efforts have to be focused on prevention. Health policies adopted by the Brazilian government resulted in stabilization of the disease in the country on younger age groups, although this trend hasn`t been happening in other age groups. Check the incidence of AIDS in the elderly, in Niterói, RJ, according to sex, age, period and birth cohort of 1982-2011, in addition to analyzing the spatial dynamics of the AIDS epidemic in the elderly (individuals aged 60 or more) in the state of Rio de Janeiro in the period 1997-2011, are the goals of this study. Population data by age, sex and group were obtained from population censuses, population count (1996), inter-census projections and data from the Information System for Notifiable Diseases, Mortality and Control Laboratory Tests. The incidence rates per 100,000 were calculated for geographical units by counting the number of new AIDS cases in individuals aged 60 years or older and size of the local population in the same age group. To evaluate the spatial dependence of the global rate, Moran index was calculated. Moran Maps were used to show the spatial correlation schemes potentially different in different sub-regions. Probability distributions and empirical Bayesian analysis were also used. There were 575 cases of AIDS among residents of Niteroi with ≥ 50 years of age. Increasing trend of incidence rates over time were found in both sexes. Also, the study of the spatial dynamics of the incidence of the syndrome(AIDS) in the elderly, in Rio de Janeiro, in the period 1997-2011 immunodeficiency syndrome rates between men and women, remained buoyant throughout the period. In this study, it wasn`t possible to detect overall significant correlation using the global Moran index. On the southeast coast of the state, where there are the major metropolitan areas (Rio de Janeiro and Niterói), we observe groups of cities with rates up to 20 cases per 100 000 population. This concentration becomes more pronounced in subsequent periods when it seems gradual spread of the epidemic occur south coast to the north of Rio de Janeiro. A síndrome de imunodeficiência adquirida (AIDS) é um problema de saúde pública que alcançou grandes proporções. Na ausência de uma vacina eficaz ou tratamento efetivo para a doença, esforços têm ser concentrados na prevenção. As políticas de saúde adotadas pelo governo brasileiro têm resultado em estabilização da enfermidade no país na faixa etária mais jovem, muito embora essa tendência não venha acontecendo nos outros grupos etários mais velhos. Verificar a incidência da AIDS em indivíduos idosos, no município de Niterói, RJ, de acordo com sexo, idade, período e coorte de nascimento de 1982-2011, além de analisar a dinâmica espacial da epidemia de AIDS em idosos (indivíduos com 60 anos ou mais) no estado do Rio de Janeiro no período de 1997-2011, são os objetivos deste estudo. Os dados da população por idade, sexo e grupo, foram obtidos a partir de: censos populacionais, contagem da população (1996), projeções intercensitárias, informações do Sistema de Informações de Agravos de Notificação, de Mortalidade e de Controle de Exames Laboratoriais. As taxas de incidência por 100 000 foram calculadas para as unidades geográficas através da contagem do número de novos casos de AIDS em indivíduos com 60 anos ou mais e tamanho da população do município no mesmo grupo etário. Para avaliar a dependência espacial das taxas foi calculado o índice de Moran global. Moran Mapas foram construídos para mostrar regimes de correlação espacial potencialmente distintos em diferentes subregiões. Distribuições de probabilidade e método Bayes empírico foram aplicados para a correção das taxas de incidência da AIDS. Ocorreram 575 casos de AIDS em residentes de Niterói com ≥50 anos de idade. Tendência crescente de taxas de incidência ao longo do tempo foi detectada em ambos os sexos. No estudo da dinâmica espacial da incidência da AIDS em idosos, Rio de Janeiro, no período de 1997 a 2011, as taxas entre homens e mulheres permaneceram flutuantes ao longo de todo o período. Não foi possível detectar correlação significativa global, usando o índice global de Moran. Na costa sudeste do Estado, onde se localizam as grandes áreas metropolitanas (Rio de Janeiro e Niterói), observaram-se grupos de cidades com taxas de até 20 casos por 100 000 hab. Esta concentração se torna mais pronunciada em períodos subsequentes, quando parece ocorrer propagação gradual da epidemia da costa sul até o norte do Rio de Janeiro.

- Abstract
Formation and structural modification of oxygenated polycyclic aromatic hydrocarbons (oxyPAHs) by UV irradiation on minerals have recently been proposed as a possible channel of PAH transformation in astrochemical and prebiotic scenarios of possible relevance for the origin of life. Herein, it is demonstrated that high-energy proton-beam irradiation in the presence of various meteorites, including stony iron, achondrite, and chondrite types, promotes the conversion of two representative oxyPAH compounds, 1-naphthol and 1,8-dihydroxynaphthalene, to complex mixtures of oxygenated and oligomeric derivatives. The main identified products include polyhydroxy derivatives, isomeric dimers encompassing benzofuran and benzopyran scaffolds, and, notably, a range of quinones and perylene derivatives. Addition of urea, a prebiotically relevant chemical precursor, expanded the range of identified species to include, among others, quinone diimines. Proton-beam irradiation of oxyPAH modulated by nitrogen-containing compounds such as urea is proposed as a possible contributory mechanism for the formation and processing of insoluble organic matter in meteorites and in prebiotic processes., (© 2020 Wiley-VCH GmbH.)
