Roentgenographic and scintiphotographic studies of the arthritis of Reiter's syndrome demonstrate concurrent involvement of various small joints of the lower extremities often associated with sacroiliitis. The most frequently affected sites are the heels, toes, sacroiliac joints, and ankles and tarsals. Simultaneous involvement of these sites is characteristic of this syndrome.

SummaryThe causes of sp. a. are not known and it is not known whether it is a disease sui generis. It is possible that it would be more correct to speak of a syndrome which may have different causes. In the first place, the diagnosis will have to be based on x-ray evidence, particularly in the form of sacroiliitis with erosions and sclerosis which must not be confused with degenerative changes. The other radiological and laboratory findings and the clinical symptoms are also discussed.

Summary and ConclusionsFollow-up information is presented for a series of 47 cases of Reiter's disease. During the acute episodes observed, pain consistent with sacroiliitis or spondylitis was present in 40 cases (85 %). Of 35 patients with radiographs 2 or more years after onset, 20 (57%) had definite bilateral sacro-iliac disease. Ankylosing spondylitis has been diagnosed in 12 patients, several of whom have followed a progressive course typical for that of severe ankylosing spondylitis. Rheumatoid arthritis has not been documented in our series. Our data and other accumulating evidence appear to link Reiter's disease closely with ankylosing spondylitis.
