The chapter overviews the recent developments in the remote sensing of cometary dust using visible, near-infrared, and thermal-infrared radiation, as well as interaction of the dust with electromagnetic radiation, which affects the dynamics of dust particles. It considers photometric, polarimetric, and spectral studies of cometary dust, focusing on those observables and correlations between them that allow revealing the composition, size, and structure of the dust particles. The analysis includes the observed brightness and polarization phase curves, color and polarimetric color of the cometary dust, and near- and thermal-infrared spectra. Special attention is paid to the role of gas contamination in the polarimetric and photometric data. A review of modeling efforts to interpret the observational results is also provided, describing the most popular (and some novel) techniques used in the computer modeling of light scattering by dust particles with a focus on modeling the most complex type of cometary particles: fluffy and porous agglomerates. The chapter also considers how properties of the dust particles affect their photoelectric emission and their response to the radiation pressure and radiative torque, including alignment and fragmentation of particles. Results of computer and some laboratory modeling are analyzed for their consistency with the observational and in situ data. In this paper, we consider whether parameter test ideals, conductors, $F$-ideals, and trace ideals are contained in an ideal whose quotient ring has finite phantom projective dimension (for example, ideals generated by a system of parameters or ideals with finite projective dimension). One of the main results asserts that such inclusions do not exist in quasi-Gorenstein complete local domains. We also provide examples of Cohen-Macaulay local rings with good properties where such inclusions occur, thus answering negatively a question of Huneke-Swanson., Comment: 13 pages

Background: This study aimed to investigate the risk factors for peritoneal recurrence (PR) in patients with stage II to III colorectal cancer who underwent colorectal surgery. Methods: A retrospective study was conducted on 232 patients who underwent colorectal surgery for stage II to III colorectal cancer. Univariate and multivariate analyses were performed to determine risk factors for PR. Results: The overall incidence of PR was 8.2 % (19/232). ABSTRACT Introduction Nutritional management is crucial in patients with chronic kidney disease. Therefore, it is important to assess nutritional status and detect malnutrition, especially in patients with diabetes. However, there is currently a lack of evidence regarding the relationship between nutritional indices and renal function in patients with type 2 diabetes. This study investigated whether the geriatric nutritional risk index (GNRI) is related to renal prognosis in type 2 diabetes patients. Materials and Methods The study included 946 type 2 diabetes patients enrolled in the Fukushima Cohort Study. The primary endpoint of this study was a renal event, defined as a combination of a 50% decline in eGFR from baseline and end‐stage kidney disease. All‐cause death and new cardiovascular events were also measured as secondary outcomes. The association between GNRI and these endpoints was assessed using Cox regression analysis. Results The median patient age was 66 years, 57% were men, the median eGFR was 67.9 mL/min/1.73 m2, and the median GNRI was 100.0. Compared to patients in the highest GNRI tertile, patients in the lowest tertile had a significantly increased risk of the renal event (HR 5.15, 95% CI 2.51–10.6) and all‐cause death (HR 2.30, 95% CI 1.20–4.42). A significant association was not observed between GNRI levels and cardiovascular events. Conclusions We observed an association between poor nutritional status, assessed by GNRI, and adverse outcomes in patients with type 2 diabetes. Nutritional status assessment has potential utility as a prognostic tool for individuals with type 2 diabetes.

ABSTRACT Background and Aims To prevent end‐stage renal disease caused by diabetic kidney disease, we created a predictive model for high‐risk patients using machine learning. Methods and Results The reference point was the time at which each patient's estimated glomerular filtration rate (eGFR) first fell below 60 mL/min/1.73 m2. The input period spanned the reference point to 1 year prior. The primary endpoint was a 50% decrease in eGFR from the mean of the input period over the 3 year evaluation period. We created predictive models for patients’ primary endpoints using time series data of various variables over the input period. Among 2,533 total patients, 1,409 had reference points, 31 had records for their input and evaluation periods and had reached their primary endpoints, and 317 patients had not. The area under the curve (AUC) of the predictive model peaked (0.81) when the minimum eGFR, the difference between maximum and minimum eGFR, and both maximum and minimum urinary protein values were included in the features. Conclusion The accuracy of prognosis prediction can be improved by considering the variable components of urinary protein and eGFR levels. This model will allow us to identify patients whose renal functions are relatively preserved with eGFR of more than 60 mL/min/1.73 m2 and are likely to benefit clinically from immediate treatment intensification.

Purpose: Patients with diabetes mellitus (DM) often exhibit refractory erectile dysfunction (ED). Red-light-controllable nitric oxide donor (NORD-1) and red-light irradiation have successfully enhanced erectile function in intact rats. In this study, we investigated whether the combination of NORD-1 and red-light irradiation effectively treated ED in streptozotocin (STZ)- treated rats with DM. Materials and Methods: Seven-week-old male Sprague-Dawley rats were used in this study. Rats in the DM and sham groups received intravenous STZ (50 mg/kg) and saline, respectively. One week after treatment, the blood glucose level of rats in the DM group was >250 mg/dL. Five weeks after the treatment, we performed a functional study by measuring intracavernous pressure (ICP) under cavernous nerve stimulation before and after NORD-1 treatment with and without light irradiation. Additionally, we performed an isometric tension study using the corpus cavernosum of rats treated with NORD-1 or the control compound, SiR650. Purpose: Twenty five years have passed since the first national survey on erectile dysfunction (ED) in Japan. The Japanese Society for Sexual Medicine conducted a nationwide survey on the actual status of sexual function targeting men over 20 years old in Japan using validated questionnaires commonly used in clinical practice. Materials and Methods: Japanese men aged 20 to 79 years participated in our online epidemiological study on sexual dysfunction. Erectile status was assessed by direct questioning and specific questionnaires. Risk factors and frequencies of sexual intercourse, masturbation, nocturnal erections, and feeling sexual desire were assessed. The prevalence of these risk factors was compared between men with and without ED. Prevalence and frequencies were calculated for each 5-year age group. Main outcomes were the prevalence and number of patients with ED and simultaneous evaluation of age-related variations. Results: Direct questioning of the men revealed that 13.0% felt troubled by ED. Although 81.0% of them had at least some ED symptoms based on a Sexual Health Inventory For Men score of ≤21, the prevalence of men with ED by Erection Hardness Score (EHS), the most appropriate questionnaire for Japanese with low sexual activity, was 30.9%, indicating that 14,012,596 men have ED. Most risk factors were related with ED, whereas frequencies of sexual intercourse, masturbation, nocturnal erections, and feeling sexual desire were affected by aging. However, the low frequency of these factors in the young generation was surprising. Conclusions: The EHS-based assessment revealed a prevalence of ED of 30.9%, which affected approximately 14 million men, and that the sexual desire, erection stiffness, orgasms, and satisfaction were lower than expected in young Japanese men, especially those aged 20 to 24 years, although those factors tended to worsen with aging. We believe that these findings actually reveal the current sexual status of men in Japan.

Abstract The purpose of this study was to analyze the efficacy of treatment and survival after administration of immune checkpoint inhibitor (ICI) in Japanese patients and had endocrine-related and/or other immune-related adverse events (irAEs), as well as irAEs in multiple organs. This is a single-center, retrospective, observational study of 571 Japanese patients treated with ICI at our hospital. We evaluated the occurrence of Grade 3 or higher irAEs and the life expectancy and treatment efficacy after ICI administration. Endocrine-related irAE (E-irAE), other irAE (O-irAE), endocrine-related and other irAE (EO-irAE), and multiple endocrine-related irAE (ME-irAE) were evaluated in groups. 80.8% of patients had an irAE, with the highest incidence of irAE with ipilimumab plus PD-1 inhibitor, followed by atezolizumab 59.0%, pembrolizumab 53.7%, avelumab 50.0%, and nivolumab 47.3%, Durvamumab 26.7% followed; Kaplan-Meier survival curves showed higher survival rates in patients with irAE compared to non-irAE, and higher survival rates in EO-irAE and ME-irAE compared to E-irAE and O-irAE (p

Abstract Spermatogenesis is one of the most complex processes of cell differentiation and its failure is a major cause of male infertility. Therefore, a proper model that recapitulates spermatogenesis in vitro has been long sought out for basic and clinical research. Testis organ culture using the gas-liquid interphase method has been shown to support spermatogenesis in mice and rats. However, the conventional method using agarose gel has limitations including medium replacement efficiency and live imaging because agarose absorbs medium and is not transparent. To overcome this issue, we developed a new device using microporous membranes and oxygen-permeable materials. Mouse testes sandwiched between a microporous polyethylene terephthalate (PET) membrane on top and an oxygen-permeable 4-polymethyl-1-pentene polymer (PMP) membrane base maintained spermatogenesis over months. The chamber volume was minimized to 0.1% of the culture medium. Weekly time-lapse live imaging enabled us to observe transgenically fluorescent acrosome and nuclear shape formation throughout spermatogenesis. Finally, we obtained healthy fertile offspring from spermatozoa generated in our system. The device could be used not only for basic research to understand spermatogenesis but also for applied research, such as diagnosing and treating male infertility.

41. Influence of false lumen status on systemic inflammatory response triggered by acute aortic dissection

Abstract We investigated the influence of false lumen (FL) status on the systemic inflammatory response triggered by acute aortic dissection (AAD) using cytokine profiling. The study included 44 patients with AAD. Patients were divided between those with a thrombosed FL (Group T, n = 21) and those with a non-thrombosed FL (Group P, n = 23). On-admission serum concentrations of 29 cytokines were compared between unmatched and propensity-score matched (n = 10 pairs) FL groups and a control group (non-ruptured thoracic aortic aneurysm, Group C, n = 20). Unmatched analysis showed 12 cytokines differed between groups and fell into three categories: Category A (increased expression in both FL groups: IL-6, IL-10, IL-15, G-CSF); Category B (increased expression only in Group P: IL-1Ra, IL-1β, IL-8, IL-12p70, GM-CSF); and Category C (others: IP-10, VEGF-A, eotaxin). The increases in Category A and Category B cytokines in Group T were attenuated, but not significantly, compared to their increases in Group P. Propensity-score matching analysis revealed a similar expression pattern with respect to all four Category A cytokines, four Category B cytokines (IL-1β, IL-1Ra, IL-12p70, and GM-CSF), and two Category C cytokines (IP-10 and VEGF-A). A robust inflammatory response occurs in patients with AAD, but the response is attenuated when the FL is thrombosed.

Abstract Event-based surveillance is crucial for the early detection and rapid response to potential public health risks. In recent years, social networking services (SNS) have been recognized for their potential role in this domain. Previous studies have demonstrated the capacity of SNS posts for the early detection of health crises and affected individuals, including those related to infectious diseases. However, the reliability of such posts, being subjective and not clinically diagnosed, remains a challenge. In this study, we address this issue by assessing the classification performance of transformer-based pretrained language models to accurately classify Japanese tweets related to heat stroke, a significant health effect of climate change, as true or false. We also evaluated the efficacy of combining SNS and artificial intelligence for event-based public health surveillance by visualizing the data on correctly classified tweets and heat stroke emergency medical evacuees in time–space and animated video, respectively. The transformer-based pretrained language models exhibited good performance in classifying the tweets. Spatiotemporal and animated video visualizations revealed a reasonable correlation. This study demonstrates the potential of using Japanese tweets and deep learning algorithms based on transformer networks for event-based surveillance at high spatiotemporal levels to enable early detection of heat stroke risks.

Abstract Half-metallic ferromagnets exhibit a perfect spin-polarization at the Fermi energy. Among many candidates, Co2MnSi Heusler alloy is the most investigated material due to its half-metallic nature and high Curie temperature (T C). Magnetic junction devices using Co2MnSi show remarkable performance at low temperatures. However, the performance is significantly degraded at room temperature, which requires a detailed understanding of the temperature-dependent electronic structure of Co2MnSi films. Here, using surface-sensitive spin- and angle-resolved photoelectron spectroscopy combined with first-principles calculations, we verify the temperature- and momentum-dependent spin-polarization of Co2MnSi thin-film. The recorded spin-polarization reaches ~ 60-75% at 50 K, while it reduces ~ 30-50% at 300 K. The observed surface-specific spin-depolarization behavior can be described by the thermally excited magnon model even well below T C, and we conclude that the spin-fluctuation is markedly enhanced on its surface. Our findings provide insights into the temperature-dependent electronic structure of half-metallic Heusler films, which could have significant implications for future spintronic applications.

Abstract In vertebrates, skeletal muscle comprises fast and slow fibers. Slow and fast muscle cells in fish are spatially segregated; slow muscle cells are located only in a superficial region, and comprise a small fraction of the total muscle cell mass. Slow muscles support low-speed, low-force movements, while fast muscles are responsible for high-speed, high-force movements. However, speed and strength of movement are not binary states, but rather fall on a continuum. This raises the question of whether any recruitment patterns exist within fast muscles, which constitute the majority of muscle cell mass. In the present study, we investigated activation patterns of trunk fast muscles during movements of varying speeds and strengths using larval zebrafish. We employed two complementary methods: calcium imaging and electrophysiology. The results obtained from both methods supported the conclusion that there are spatially-ordered recruitment patterns in fast muscle cells. During weaker/slower movements, only the lateral portion of fast muscle cells is recruited. As the speed or strength of the movements increases, more fast muscle cells are recruited in a spatially-ordered manner, progressively from lateral to medial. We also conducted anatomical studies to examine muscle fiber size. The results of those experiments indicated that muscle fiber size increases systematically from lateral to medial. Therefore, the spatially ordered recruitment of fast muscle fibers, progressing from lateral to medial, correlates with an increase in fiber size. These findings provide significant insights into the organization and function of fast muscles in larval zebrafish, illustrating how spatial recruitment and fiber size interact to optimize movement performance.

To tackle disorder in crystals and short- and intermediate-range order in amorphous materials, such as glass, we developed a carry-in diffractometer to utilise X-ray fluorescence holography (XFH) and anomalous X-ray scattering (AXS), facilitating element-specific analyses with atomic resolution using the wavelength tunability of a synchrotron X-ray source. Our diffractometer unifies XFH and AXS configurations to determine the crystal orientation via diffractometry. In particular, XFH was realised even for a crystal with blurred emission lines by a standing wave in a hologram, and high-throughput AXS with sufficient count statistics and energy resolution was achieved using three multi-array detectors with crystal analysers. These features increase tractable targets by XFH and AXS, which have novel functionalities.

X-ray spectromicroscopy is extensively utilized for nondestructive mapping of chemical states in materials. However, understanding and analyzing the geometric and topological aspects of such data pose challenges due to their representation in 4D space, encompassing (x, y, z) coordinates along with the energy (E) axis and often extending to 5D space with the inclusion of time (t) or reaction degree. In this study, we addressed this challenge by developing a new approach and introducing a device named `4D-XASView', specifically designed for visualizing X-ray absorption fine structures (XAFS) data in 4D (comprising 3D space and energy), through a multi-projection system, within the virtual reality (VR) environment. As a test case for the new system, X-ray spectromicroscopy measurements were conducted on a specimen prepared from serpentinized harzburgite sourced from the upper mantle section of the Oman ophiolite. Our 4D-XASView facilitates the visualization and analysis of the geometric and topological aspects of the data using VR goggles, enabling detailed exploration of microstructures via rotation and zooming functionalities. This capability allows us to extract XAFS spectral data by selecting specific positions and regions, thereby aiding in the identification of `trigger sites' (magnetite in serpentine), which are characteristic locations within materials that substantially influence the macroscopic propagation of reactions. Our methodology establishes a new platform for analyzing 4D or 5D XAFS data that has applicability potential in various other multidimensional datasets, including microstructures coupled with spectroscopy and diffraction data.

Background: Superior capsular reconstruction (SCR) is a surgical option for massive irreparable rotator cuff tears, particularly involving the supraspinatus and infraspinatus. In this procedure, the torn infraspinatus is not repaired or reconstructed. However, an improvement in postoperative external rotation (ER) angle and strength is observed. There is a lack of studies explaining how ER is restored after SCR. The aim of this study is to identify the ER compensator by assessing the muscle volume of the posterior deltoid and teres minor. Methods: Sixty-eight patients with massive rotator cuff tears underwent SCR during 2016-2021. Of these patients, 28 who met the following inclusion criteria were retrospectively reviewed: (1) massive rotator cuff tears, including the supraspinatus and infraspinatus, (2) severe muscle atrophy and fatty change, (3) intact or reparable subscapularis tendon, and (4) Hamada of grade 3 or lower. Posterior deltoid and teres minor volume were measured using open-source medical image processing software preoperatively and 1-year postoperatively. The percentage of the posterior deltoid and teres minor muscle volume change was compared between patients with 1-year postoperative ER manual muscle testing (MMT) of grade 5 and of grade < 5. The relationship between grade of fatty change, percentage of the muscle volume change, and ER angle and strength were evaluated. Abstract Background Point-of-care ultrasound (POCUS) is a valuable skill for generalist physicians, nurse practitioners (NPs), and nurses; however, its utilization remains limited. This study was performed to investigate the current status, barriers, and facilitators of POCUS implementation among physicians, NPs, and nurses in family and hospital medicine in Japan and to identify differences in influencing factors between physicians and NPs/nurses. Results A web-based survey was distributed via the mailing lists of four major academic societies in general medicine in Japan—the Japanese Society of Hospital General Medicine, the Japan Primary Care Association, the Japanese Association for Home Care Medicine, and the Japan Society of Nurse Practitioner—from April to June 2024. The respondents included physicians, NPs, and nurses affiliated with these societies. Responses from other professions, duplicate entries, and incomplete surveys were excluded from the analysis, resulting in 913 valid responses (692 physicians and 221 NPs/nurses). Physicians reported a higher POCUS implementation rate than NPs/nurses (73.0 vs. 63.4%, p = 0.006). The top two barriers were consistent across both groups: lack of training opportunities (p = 0.385) and lack of image acquisition skills (p = 0.369). Abstract We analyse the relationship between population influx and the effective reproduction number in the 23 wards of Tokyo during the COVID-19 pandemic to estimate hotspots of infection. We identify some patterns of population influx via factor analysis and estimate specific areas as infection-related hotspots by focusing on influx patterns that are highly correlated with the effective reproduction number. As a result, several influx patterns are assumed to be directly related to the subsequent spread of the infection. This analytical method has the potential to detect unknown hotspots related to pandemics in the future.

Abstract Halorhodospira (Hlr.) halophila strain BN9622 is an extremely halophilic and alkaliphilic purple phototrophic bacterium and has been widely used as a model for exploring the osmoadaptive and photosynthetic strategies employed by phototrophic extreme halophiles that enable them to thrive in hypersaline environments. Here we present the cryo-EM structures of (1) a unique native Hlr. halophila triple-complex formed from light-harvesting (LH1), the reaction center (RC), and high-potential iron–sulfur protein (HiPIP) at 2.44 Å resolution, and (2) a HiPIP-free LH1–RC complex at 2.64 Å resolution. Differing from the LH1 in the Hlr. halophila LH1–LH2 co-complex where LH1 encircles LH2, the RC-associated LH1 complex consists of 16 (rather than 18) αβ-subunits circularly surrounding the RC. These distinct forms of LH1 indicate that the number of subunits in a Hlr. halophila LH1 complex is flexible and its size is a function of the photocomplex it encircles. Like LH1 in the LH1–LH2 co-complex, the RC-associated LH1 complex also contained two forms of αβ-polypeptides and both dimeric and monomeric molecules of bacteriochlorophyll a. The majority of the isolated Hlr. halophila LH1–RC complexes contained the electron donor HiPIP bound to the surface of the RC cytochrome subunit near the heme-1 group. The bound HiPIP consisted of an N-terminal functional domain and a long C-terminal extension firmly attached to the cytochrome subunit. Despite overall highly negative surface-charge distributions for both the cytochrome subunit and HiPIP, the interface between the two proteins was relatively uncharged and neutral, forming a pathway for electron tunneling. The structure of the Hlr. halophila LH1–RC–HiPIP complex provides insights into the mechanism of light energy acquisition coupled with a long-distance electron donating process toward the charge separation site in a multi-extremophilic phototroph.
