Introduction: Typically, the lateral femoral cutaneous nerve, also known as the lateral cutaneous nerve of the thigh (LCNT), runs under the inguinal ligament near the anterior superior iliac spine, runs anterior to the sartorius muscle and then divides into two terminal branches, i.e., anterior and posterior, which provide sensory innervation to the anterior and lateral thigh. This report describes an unusual anatomic variant in which terminal branches of the LCNT pierced the sartorius muscle. Case description: Dissection involved formalin-fixed right isolated lower limb. The procedure revealed anatomical variation of the LCNT. The nerve's division level into anterior and posterior branches was at the level of the inguinal ligament under the sartorius muscle. The posterior branch of the LCNT was directly adjacent to the anterior superior iliac spine. After a short course, this branch emerged from under the sartorius muscle lateral border 39 mm distally (in a straight line) to the inguinal ligament. The nerve's anterior branch pierced the sartorius muscle. It exited on the anterior surface of the muscle, 72 mm (in a straight line) from the inguinal ligament. The innervation territory of both branches was typical. We found no other anatomical variations of lumbar plexus branches. Conclusions: The LCNT terminal branches may show anatomic variations, which clinicians should consider during neurological assessments of nerve lesions. Deviations from the typical course of nerves should also be kept in mind when conducting surgical interventions.

Introduction: Anatomical variations of the nerves derived from the lumbar plexus are common and clinically valid. Therefore, this report aims to present the coexisting anatomical variations of the lateral femoral cutaneous and genitofemoral nerves. Case description: During routine dissection of an isolated male left lower limb (fixed in 10 % formalin solution), atypical arrangements of the lateral femoral cutaneous and genitofemoral nerves were found. The lateral femoral cutaneous nerve division level into anterior and posterior branches was high, above the inguinal ligament. Both branches passed beneath the inguinal ligament at the midpoint of the distance between the anterior superior iliac spine and the femoral artery. The anterior branch of the lateral femoral cutaneous nerve was better developed and had a greater diameter than the posterior branch. The femoral branch of the genitofemoral nerve was absent and partially replaced by the most medial branch derived from the anterior division of the lateral femoral cutaneous nerve. Conclusions: The lateral femoral cutaneous and genitofemoral nerves can show anatomic variability, which should be remembered during clinical assessments of nerve lesions and while performing surgical interventions.

Meralgia paresthetica is a common but probably underrecognized syndrome caused by dysfunction of the lateral femoral cutaneous nerve. The diagnosis is based on the patient's description of sensory disturbance, often painful, on the anterolateral aspect of the thigh, with normal strength and reflexes. Sensory nerve conduction studies and somatosensory evoked potentials may be used to support the diagnosis, but both have technical limitations, with low specificity and sensitivity. Risk factors for meralgia paresthetica include obesity, tight clothing, and diabetes mellitus. Some cases are complications of hip or lumbar spine surgery. Most cases are self-limited, but a small proportion of patients remain with refractory and disabling symptoms. Treatment options include medications for neuropathic pain, neurolysis, neurectomy, and radioablation, but controlled trials to compare efficacy are lacking., (Copyright © 2024 Elsevier B.V. Minimally invasive approaches have become standard for pediatric appendectomy. The laparoscopic assisted single port approach, also known as the “all-in-one” appendectomy, has gained recent popularity [1]. We describe a child who suffered meralgia paresthetica (a neuropathy in the distribution of the lateral femoral cutaneous nerve) after a laparoscopic assisted single port appendectomy, perhaps secondary to mobilization of the cecum.

The facsia iliaca block (FIB) is a relatively new regional technique where local anesthetic is delivered within the fascia iliaca region. Indications for a FIB include surgical anesthesia to the lower extremity after knee, femoral shaft, hip surgery, management of cancer pain or pain secondary to inflammatory conditions of the lumbar plexus, as well as treatment of acute pain in the setting of trauma, fracture, or burns. The FIB may be performed using either a loss of resistance technique or an ultrasound (US)-guided technique; however, the use of US has become commonplace and resulted in improved femoral nerve and obturator nerve motor blocks. The main targets of the FIB are the predominant nerves contained in the fascia iliaca compartment (FIC), namely the femoral nerve and the lateral femoral cutaneous nerve. The FIB US guided technique is beneficial to patients and the possibility to perform FIB should be discussed and coordinated with surgical staff appropriately, considering its superiority to general or epidural anesthesia., (Published by Elsevier Ltd.)

Background: Meralgia paresthetica, a pain syndrome that is caused by injury to the lateral femoral cutaneous nerve, is a well-documented complication after anterior hip arthroplasty (THA). Traditional treatment of this peripheral nerve entrapment syndrome can be complicated in patients who have had THA via an anterior approach owing to the presence of scar in the postoperative bed., Case Description: In a 70-year-old man, we performed a novel laparoscopic-assisted intra-abdominal approach to treat meralgia paresthetica in the setting of previous anterior THA., Conclusions: Minimally invasive intra-abdominal treatment of meralgia paresthetica following anterior THA results in durable pain relief. Objective: To trace anatomic variations of the lateral femoral cutaneous nerve (LFCN) in its intrapelvic course., Methods: Forty cadavers (80 sides) fixed in 10% formalin solution were dissected. The following parameters were recorded: LFCN diameter and variations in its origin and number. The dissection comprised exposure and excision of the lumbar plexus, together with the roots of LFCN, followed by retrograde intraneural fascicular dissection using microsurgical instruments., Results: Several types of LFCN origin from the lumbar plexus were observed. Typically, the LFCN appears as a single trunk arising from dorsal divisions of the ventral rami of the lumbar plexus. The most prevalent origin of the nerve was from the L2 and L3 roots (47 cases; 58.75%). The LFCN took an origin from the L1-L2 level in 12 cases (15%) and from the L2 nerve in 9 cases (11.25%). The main observed variations were the presence of the accessory LFCN (2 cases; 2.5%) and branching of the LFCN from the femoral nerve (6 cases; 7.5%). Communications between the LFCN and the femoral or genitofemoral nerves also were observed occasionally. An atypical course of the LFCN with respect to the anterior psoas was observed in our material in 3 of the 80 sides (3.75% of the examined LFCN specimens)., Conclusions: Considerable variability in the origin and the course of the LFCN was observed, which should be taken into account during clinical assessment of nerve lesions and during surgery via transpsoas approaches to the lumbar spine., (Copyright © 2018 Elsevier Inc. We present an unusual case of sartorius muscle tear presenting as acute meralgia paresthetica. A healthy 67-year old male was referred to our department with a one-week history of pain, numbness and bruising over his hip and anterolateral thigh. Extended ultrasound assessment revealed an intramuscular tear of sartorius, with acute hematoma surrounding the adjacent lateral femoral cutaneous nerve. Meralgia paresthetica from acute trauma is rare, with only three published cases relating to fractures of the anterior superior iliac spine. To our knowledge, this is the first case caused by muscular tear in the literature. Background: We report a surgical case of entrapment neuropathy of lateral femoral cutaneous nerve (LFCN) with anatomical variation., Case Description: This 53-year-old man had a 10-year history of paresthesia and pain in the right anterolateral thigh exacerbated by prolonged standing and walking. His symptoms improved completely but transiently by LFCN block. The diagnosis was LFCN entrapment. Because additional treatment with drugs and repeat LFCN block was ineffective, we performed surgical decompression under local anesthesia. A nerve stimulator located the LFCN 4.5 cm medial to the anterior superior iliac spine. It formed a sharp curve and was embedded in connective tissue. Proximal dissection showed it to run parallel to the femoral nerve at the level of the inguinal ligament. The inguinal ligament was partially released to complete dissection/release. Postoperatively, his symptoms improved and the numeric rating scale fell from 8 to 1., Conclusion: We report a rare anatomical variation in the course of the LFCN., (Copyright © 2018 Elsevier Inc. Introduction: Anterior pelvic internal fixator (INFIX) is used to treat unstable pelvic ring injuries. Nerve injury complications with this procedure have been reported., Objectives: This anatomic study attempted to identify structures at risk after application of INFIX., Materials and Methods: INFIX was applied in fifteen fresh, frozen, anatomical specimens using polyaxial pedicular screws and subcutaneous rods. Surgical dissection was done to identify the structures at risk including the femoral nerve (FN), femoral artery (FA), femoral vein (FV) and the lateral femoral cutaneous nerve (LFCN) related to which are potentially affected by the implant., Results: All structures at risk were closer to the rod than to the pedicular screw. Measurements were made between the rod and the structures at risk. The LFCN was an average of 13.49±1.65mm (95% CI 12.871-14.103) from the lateral end of the rod. The FN was an average of 12.43±3.42mm (95% CI 11.151-13.709), the FA was an average of 12.80±3.67 (95% CI 11.430-14.173) and the FV was an average of 13.48±3.73 (95% CI 12.082-14.871) below the rod. No direct compression of the rod to the structure at risk was observed., Conclusions: The femoral nerve is the structure most at risk of compression by the INFIX rod. Careful surgical technique is required in every step of this surgery. We suggest using polyaxial screws and recommend that during screw insertion the surgeon should leave some space between the screw and rectus fascia. Meralgia paresthetica is a chronic pain syndrome that is extremely rare in the pediatric population. It is manifested by hypesthesia or pain in the distribution of the lateral femoral cutaneous nerve (LFCN) and is typically caused by entrapment as the nerve passes deep to the inguinal ligament. This sensory mononeuropathy is rare in children and diagnosis is typically delayed, often leading to prolonged functional impairment and unnecessary medical testing. A 9-year-old girl presented to the pain clinic with a 6-week history of right anterolateral thigh pain first noticed after a nontraumatic cheerleading practice. Comprehensive laboratory and radiographic evaluation by multiple prior specialists revealed no clear nociceptive source of pain. History and examination were consistent with a diagnosis of idiopathic, compressive meralgia paresthetica. Conservative management including physical therapy was followed for 2 weeks with only mild improvement noted. To facilitate physical therapy, an ultrasound-guided LFCN block was performed which confirmed the diagnosis by providing complete analgesia. The patient reported overall 25% improvement from multimodal therapy at another 2 weeks. A second LFCN block was performed with complete resolution of symptoms and restoration of function. The patient remains pain-free and has returned to walking, running, and competitive sports. The primary goal of pediatric chronic pain management, regardless of pain etiology, is early restoration of function to avoid prolonged absence from school, sports, or other productive activities and limit the psychological burden of chronic disease., (Copyright © 2016 Elsevier Inc.
