Abstract

A 78-year-old woman with an aortic pseudoaneurysm suffered cardiac arrest due to aspiration and underwent cardiopulmonary resuscitation (CPR), but was found to then be hypotensive with a new chest hematoma. Imaging revealed aortic rupture at the site of the pseudoaneurysm. Aortic rupture is a rare but catastrophic complication of CPR. Abstract

Type A intramural hematoma (IMH) constitutes a variant of acute aortic syndrome. Western guidelines support an aggressive surgical approach, whereas Asian centers propose initial conservative treatment. Further expanding on this notion, we present a case of conservative subacute type A IMH management, resulting in radical hematoma resorption within 4 weeks. We report a 65-year-old man who underwent aortic valve replacement because of severe aortic stenosis associated with de novo iatrogenic variant transthyretin amyloidosis derived from a liver graft extracted from a patient with hereditary transthyretin amyloidosis 9 years after the domino liver transplantation.
