Les patients atteints de maladies chroniques, a fortiori de maladies auto-immunes et/ou systémiques rares associées à une incertitude diagnostique importante, ont une représentation de leur maladie et un vécu de leur hospitalisation parfois prolongé(e) qui peuvent être traumatiques et anxiogènes. Cette étude avait pour objectif d'évaluer l'impact d'une intervention non médicamenteuse d'hypnose médicale dans la réduction de l'état de stress et l'amélioration du vécu des patients hospitalisés dans un Service de médecine interne. Nous avons mené une étude prospective sur 24 patients hospitalisés dans le Service de médecine interne du CHU de Lille en 2023. Douze patients ont bénéficié d'une intervention non médicamenteuse d'hypnose médicale dite du « lieu de sécurité » (groupe cas) et ont été comparés à 12 patients n'en ayant pas bénéficié (groupe témoin). Le stress était évalué par le questionnaire STAI et le vécu de l'hospitalisation par un questionnaire de satisfaction. Les 24 patients dont 13 femmes avaient un âge moyen de 55 ± 17 ans à l'inclusion. À l'entrée en hospitalisation, la médiane de l'état de stress (STAI-ETAT) entre les deux groupes était de 43,5 (38,0 ; 56,6) dans le groupe cas versus 42,0 (37,0 ; 48,5) dans le groupe témoin (p = 0,45). Dans le groupe cas, le questionnaire STAI-ETAT réalisé immédiatement après la séance d'hypnose était significativement plus bas en médiane par rapport à celui du début d'hospitalisation (30,0 [25,5 ; 36,5] vs 43,5 [38,0 ; 56,5] p = 0,003) témoignant d'une réduction significative du stress. À la fin de l'hospitalisation, on observait également une persistance significative de la réduction significative en médiane entre les cas et les témoins (29,5 [26,5 ; 35,0] pour les cas vs 41,5 [33,5 ; 45,5] pour les témoins p = 0,002). Le vécu de l'hospitalisation était meilleur dans le groupe cas (médiane 5,0 [4,5 ; 5,0] vs 4,0 [4,0 ; 4,5], p = 0,016). Cette étude suggère que l'hypnose médicale est une intervention non médicamenteuse d'accompagnement prometteuse dans la réduction du stress ressenti et dans l'amélioration du vécu de cette dernière chez des patients hospitalisés dans un Service de médecine interne. The aim of this study was to evaluate the impact of a non-medicinal medical hypnosis intervention in reducing the stress state and improving the experience of patients hospitalized in an internal medicine department. We conducted a prospective study of 24 patients hospitalized in the Internal Medicine Department of Lille University Hospital in 2023. Twelve patients received a non-drug medical hypnosis intervention known as the "place of safety" (case group) and were compared with 12 patients who did not (control group). Stress was assessed by the STAI questionnaire and hospitalization experience by a satisfaction questionnaire. The 24 patients, 13 of whom were women, had a mean age of 55 ± 17 years at inclusion. On admission to hospital, the median STAI-ETAT between the two groups was 43.5 (38.0; 56.6) in the case group versus 42.0 (37.0; 48.5) in the control group (P = 0.45). In the case group, the median STAI-ETAT questionnaire taken immediately after the hypnosis session was significantly lower than at the start of hospitalization (30.0 [25.5; 36.5] vs. 43.5 [38.0; 56.5] P = 0.003), indicating a significant reduction in stress. At the end of hospitalization, there was also a significant persistence of the median significant reduction between cases and controls (29.5 [26.5; 35.0] for cases vs. 41.5 [33.5; 45.5] for controls P = 0.002). Experience of hospitalization was better in the case group (median 5.0 [4.5; 5.0] vs. 4.0 [4.0; 4.5], P = 0.016). • In vivo 3D imaging to estimate empty body and carcass chemical composition. • Body shape traits captured by 3D imaging of growing beef-on-dairy crossbred bulls. • Along the growth trajectory and at terminal slaughter BW, chemical component masses are well estimated. • Estimates of chemical component proportions are slightly less precise. • On-farm body composition phenotyping throughout the cattle's production cycle. The dynamics of cattle body chemical composition during growth and fattening periods determine animal performance and beef carcass quality. The aim of this study was to estimate the empty body (EB) and carcass chemical composition of growing beef-on-dairy crossbred bulls (Brown Swiss breed as dam with Angus, Limousin or Simmental as sire) using three-dimensional (3D) imaging. The 3D images of the cattle's external body shape were recorded in vivo on 48 bulls along growth trajectory (75–520 kg BW and 34–306 kg hot carcass weight [ HCW ]; set 1) and on 70 bulls at target market slaughter weight, including 18 animals from set 1 (average 517 ± 10 kg BW and 289 ± 10 kg HCW; set 2). The linear, circumference, curve, surface and volume measurements on the 3D body shape were determined. Those predictive variables were used in partial least square regressions, together with the effect of the sire breed whenever significant (P < 0.05), with leave-one-out cross-validation to estimate water, lipid, protein, mineral and energy mass or proportions in the EB and carcass. Mass and proportions were determined directly from postmortem grinding and chemical analyses (set 1) or indirectly using the 11th rib dissection method (set 2). In set 1, bulls' BW and HCW were estimated via 3D imaging, with root mean square error of prediction (RMSEP) of 12 kg and 6 kg, respectively. The EB and carcass chemical component proportions were estimated with RMSEP from 0.2% for EB minerals (observed mean 3.7 ± 0.2%) to 1.8% for EB lipid (11.6 ± 4.2%), close to the RMSEP found for the carcass. In set 2, the RMSEP for estimation via 3D imaging was 9 kg for BW and 6 kg for HCW. The EB energy and protein proportions were estimated, with RMSEP of 0.5 MJ/kg fresh matter (10.1 ± 0.8 MJ/DM) and 0.2% (18.7 ± 0.7%), respectively. Overall, the estimations of chemical component proportions from 3D imaging were slightly less precise for both sets than the mass estimations. The pivotal clinical trials have largely demonstrated the efficacy and safety of ustekinumab in Crohn's disease. Real-life cohorts published so far only include very few bio-naïve patients. This study assesses effectiveness and safety of ustekinumab in bio-naïve and bio-failure patients treated with ustekinumab in routine practice and look for predictors of response. We performed a retrospective monocentric study. Initial response was assessed by maintenance therapy beyond week 16. Sustained response was assessed by the continuation or cessation of therapy over time for another reason than stopping in sustained remission. Treatment persistence was assessed by Kaplan Meier curves and predictors of treatment persistence were studied by univariate and multivariate Cox model. Out of 156 recorded patients, three patients were still in their induction phase at time of analysis and 5 patients were lost to follow-up, leaving 148 patients for clinical effectiveness analyses, including 35 bio-naïve when starting ustekinumab. A maintenance therapy was initiated in 79.7%. At one year, the probability to be still treated with ustekinumab was 73.8%. Treatment cessation increased with smoking in multivariate analysis. Previous biologic failure (as a whole), CRP and fecal calprotectin baseline levels did not influence initial response and treatment persistence. A large proportion of CD patients initially respond to ustekinumab and continue this treatment beyond one year. Treatment persistence is as high in bio-failure as in bio-naïve patients. Résumé La sclérodermie systémique (ScS) concerne 9000 à 14 000 personnes en France. Le travail est souvent affecté. Notre étude vise à évaluer la fréquence des difficultés professionnelles (DP) des patients sclérodermiques, identifier ces DP, les symptômes en cause et les solutions exploitées. Un hétéroquestionnaire a été proposé aux patients sclérodermiques du CHRU de Lille. Les données étaient recoupées avec la base de données cliniques de l’observatoire national des patients atteints de sclérodermie. Nous avons exploité 104 questionnaires dont 84 % de femmes. Parmi les patients, 62,5 % (IC95 %[52,5–71,8]) avaient rencontré des DP en relation avec la ScS. Au total, 55 % rapportaient des symptômes aggravés au travail, 41 % une diminution d’efficacité et 19 % une perte d’emploi. Les symptômes responsables de leurs DP étaient principalement l’asthénie, le syndrome de Raynaud, les arthromyalgies et les ulcérations digitales. Seuls 40 % des patients en difficulté initiaient des démarches auprès de la maison départementale des personnes handicapées et 45 % en informaient leur médecin du travail (MT). L’absence d’information du MT était liée à une méconnaissance des aides possibles dans 45 % des cas ; 57 % des patients n’avaient pas trouvé de solution ; 41 % d’entre eux ont perdu leur emploi et 31 % des patients de la cohorte ont connu une baisse de revenu. L’impact de la ScS sur le travail est important. Les dispositifs d’aide au maintien en emploi sont sous-exploités du fait d’une méconnaissance des démarches à effectuer et du rôle du MT. La prise en compte des symptômes responsables de DP doit être améliorée pour participer à ce maintien en emploi. Between 9000 and 14000 people have systemic sclerosis (ScS) in France. The work is often affected. Our study aims to assess the frequency of professional difficulties (DP) of scleroderma patients, identify these DP, the symptoms involved and the solutions used. A heteroquestionnaire was offered to scleroderma patients the Lille University Hospital. Data were cross-checked with the clinical database of the National Observatory of scleroderma patients. We used 104 questionnaires of which 84% are women. A total of 62.5%, (95% CI [52.5 to 71.8]) patients had experienced DP related to SSc, 55% reported symptoms worsened at work, 41% lower efficiency and a 19% job loss. Symptoms responsible for their DP were mainly asthenia, Raynaud syndrome, arthralgias and finger ulcerations. Only 40% of patients in difficulty were initiating steps with the county home for disabled people and 45% in informing their doctor at work (MT). The MT information absence was related to a misunderstanding of possible aid in 45% of cases, 57% of patients had not found a solution, 41% of them have lost their jobs and 31% of patients in the cohort experienced a drop in income. The impact of SSc the work is important. The aid job retention devices are under-used due to lack of knowledge of steps to take and the role of MT. Les immunoglobulines intraveineuses (IgIV) sont le traitement de référence des formes les plus sévères du purpura thrombopénique immunologique (PTI) en raison de leur rapidité d'action. Il s'agit cependant d'un traitement coûteux dont l'efficacité est limitée à 2 à 3 semaines et il existe une tension majeure d'approvisionnement au niveau mondial qui a conduit récemment les autorités de santé à rappeler la nécessité pour les prescripteurs de suivre scrupuleusement les recommandations d'utilisation précisées dans le protocole national de diagnostic et de soins (PNDS) du PTI [1,2]. Les IgIV doivent être ainsi réservées (i) aux patients présentant un syndrome hémorragique important, (ii) chez ceux connus comme étant cortico-résistants en préparation à un acte invasif ou à l'accouchement et (iii) en cas de contre-indication avérée aux corticoïdes en l'absence de traitement alternatif. La sévérité du syndrome hémorragique est cotée en France par le score dit de « Khellaf » et il est recommandé de n'utiliser les IgIV que lorsque le score est > 8 [3]. Nous souhaitons décrire les pratiques de prescription des IgIV dans le cadre du PTI et évaluer leur adéquation aux recommandations du PNDS afin de proposer des mesures correctrices en cas de forte déviation par rapport aux recommandations. Nous prévoyons d'utiliser l'entrepôt de données de santé (EDS) de l'AP–HP où une première recherche a permis d'estimer à près de 700 le nombre de patients ayant reçu des IgIV pour un PTI entre 2016 et 2020. Avant d'accéder aux données de l'EDS, nous avons évalué la faisabilité du projet en réalisant une étude pilote monocentrique. Cette étude a été conduite au sein d'un seul service de Médecine Interne de l'AP-HP utilisant le logiciel de recueil de données et de prescription ORBIS sur lequel se base l'EDS de l'APHP. Les patients hospitalisés entre 2016 et 2020 avec un code PMSI D693 identifié en diagnostic principal ou associé ont été inclus. Une recherche multi-sources de la prescription d'IgIV a été réalisée, s'appuyant sur le logiciel de prescription ORBIS médicaments et l'analyse des comptes rendus d'hospitalisation (CRH). Le nombre de référence de patients ayant bénéficié d'une prescription d'IgIV a été établi grâce aux données issues de la pharmacie hospitalière. Cette analyse a permis d'identifier 135 patients ayant reçu des IgIV pour un PTI sur les 5 années évaluées (données pharmacie), parmi lesquels 118 (87 %) ont été repérés par évaluation automatique ORBIS-CRH. À l'inverse, 7 patients ont été repérés via ORBIS médicaments sans être répertoriés par la pharmacie: ces faux positifs correspondaient essentiellement à la mention d'IgIV dans les antécédents. Les 118 patients identifiés donnaient lieu à 264 séjours avec prescription d'IgIV. Parmi les 264 cures d'IgIV, le score de Khellaf était précisé dans le CRH ou calculable à partir des données d'ORBIS-CRH dans 100 % des cas. Le score de Khellaf était > 8 dans 79 cas (29.9 %) et la prescription d'IgIV était donc en accord avec les recommandations du PNDS. Parmi les 185 cures d'IgIV avec un score hémorragique ≤ 8, l'indication des IgIV était justifiée par l'existence d'une corticorésistance, corticodépendance ou avis d'expert chez des patients multiréfractaires dans 117 cas (44.3 %). L'encadrement d'un geste invasif a été retrouvé dans 23 cas (12.4 %) mais 4 de ceux-ci n'étaient pas associés à une corticorésistance ou à une contre-indication aux corticoïdes. Des contre-indications notées aux corticoïdes ont été retrouvées dans 11 séjours (5.9 %) mais 3 cas ne correspondaient pas à des contre-indications formelles (suspicion de pneumopathie Covid-19 et suspicion d'hémopathie maligne en attente de biopsie ganglionnaire). Enfin, 13 prescriptions d'IgIV (7 %) étaient en lien avec une grossesse mais seulement 7 étaient associées à une corticorésistance ou à une contre-indication aux corticoïdes. Au total, il existait donc une déviation de la prescription d'IgIV par rapport aux recommandations du PNDS dans 38 cas soit 14.4 % des prescriptions totales. Cette étude pilote monocentrique montre la faisabilité de cette étude à plus large échelle sur les données de l'EDS de l'AP-HP, dont l'accès nous a été accordé suite au succès de cette étude préliminaire. Nous démontrons la présence du score de Khellaf ou des éléments nécessaires à son calcul dans 100 % des CRH ainsi qu'une prescription des IgIV jugée en adéquation avec le PNDS dans 85.6 % des cas. L'étude future basée sur l'EDS de l'AP-HP permettra une évaluation plus globale des pratiques pour cet enjeu important de santé publique et d'économie de la santé et permettra de déterminer si des mesures correctrices doivent être proposées. Coronaritis is a rare but serious complication of giant-cell arteritis (GCA), with an estimated prevalence of less than 1%, however difficult to establish, and of early onset. We describe 2 cases of GCA presenting with coronaritis and present a review of the literature on this complication. The first patient presented with stable angina on common trunk coronaritis with ostial stenosis. Corticosteroid combined with tocilizumab from the outset resulted in improvement. Angioplasty was performed at 6 months with good outcome. The second patient presented with asymptomatic tritruncular ostial coronaritis. Corticosteroid allowed clinic-biological improvement of GCA. Two years later, he presented relapse with an acute coronary syndrome, with favorable evolution after angioplasty, increase of corticosteroids and addition of tocilizumab. Patients presented were successfully treated with corticosteroids combined with tocilizumab and angioplasty of their coronary stenoses. Efficacy of tocilizumab in GCA has not been evaluated especially on coronaritis due to the rarity of this complication. Our experience and the cases reported in the literature suggest good results of angioplasty in this indication. Studies with long-term follow-up will be necessary to evaluate the risk of restenosis. Abstract: The hard breakdown behaviors of boron–phosphorus compensated p-type and n-type Czochralski silicon solar cells were studied as functions of the type of conductivity, the net doping concentration and the temperature. We showed as expected from previous studies that the hard breakdown was governed by avalanche mechanisms (impact ionization) and that for a given net doping concentration the hard breakdown voltage was almost the same for both p-type and n-type solar cells, despite the different reported values for the electron and hole impact ionization coefficients. The studied compensated materials offered the opportunity to determine the hard breakdown voltages for samples with the same net doping concentration but different total amount of doping species. However no major effects of the dopant compensation were observed, the experimental data being described by the empirical expressions used for uncompensated silicon samples. [Copyright &y& Elsevier]

Abstract: Purpose: Systemic sclerosis (ScS) is very heterogeneous in its clinical presentation and its therapeutic care is not codified. A better knowledge of the patients’ needs and complaints could improve the patient educational strategies and their global care. Methods: A self-administered questionnaire aimed to the ScS patient was developed by subspecialty physicians and nurses involved in patient education. It was a cross-sectional study that also included several validated scales: the health control locus scale, the Mactar, HAD and sHAQ scales. Results: One hundred and eight patients (91 women; 18 limited ScS, 71 limited cutaneous ScS, 19 diffuse ScS) filled in the questionnaires. Fatigue was the main complaint in all types of ScS, independently of the ScS type. The aesthetic discomfort mentioned by the patients suffering from cutaneous sclerosis or from telangectasia was important and reached 52±33mm on a 100-mm visual scale. It was more common in the patients presenting a diffuse form of the illness but the difference did not reach a statistical significance (P =0.06). Twenty-seven percent of the patients said they were very or extremely worried because of the degradation of their physical appearance. The functional discomfort linked to the cutaneous sclerosis was rated 50±32mm on a 100-mm visual scale. The intensity of the pain, the importance of the functional discomfort linked to the sclerosis and the intensity of the dyspnea were correlated to the sHAQ (P <0.001). Patients having more frequent recurrent digital ulcers had higher sHAQ scores (P =0.04). The repercussions on the professional life were linked to fatigue first, to the Raynaud''s syndrome and to arthralgia. The repercussions on the personal life were mainly linked to the fatigue, the pain and the dyspnea. The patients’ compliance was good. Conclusion: Fatigue, pain, dyspnea and discomfort linked to sclerosis are major chronic symptoms of the patients with ScS. Identifying the needs and complaints of the patients with ScS should help to improve their care by implementation of an educational program. [Copyright &y& Elsevier]

Determining energy utilization in growing animals enables to adjust the nutritional constraints to nutrient requirements while maximizing the ratio between lean retention and fat retention to improve feed efficiency. In turkey production, the important sexual dimorphism and differences between strains may contribute to differences in basal energy metabolism and the partitioning of energy retention between protein and lipid. The objective of this study was to determine the dynamics of energy utilization in males and females of a heavy strain of turkeys fed ad libitum from 1 to 23 weeks of age. Heat production (HP) was determined by indirect calorimetry and retained energy (RE) was calculated as the difference between metabolizable energy (ME) intake and HP. The RE as protein was determined by a nitrogen balance, while the remaining RE was assumed to be lipid. A modeling procedure allowed partitioning HP between fasting HP (FHP), activity-related HP and thermic effect of feeding. A multiple regression analysis was used to estimate the maintenance energy expenditure (MEm) and the energy efficiencies of protein and lipid retention (kp and kf, respectively). Results were expressed either per day or per kg BW0.75 per day. In comparison with females, males consumed more feed (440 v. 368 g/day), grew faster (163 v. 147 g/day) and retained more protein (38 v. 28 g/day) during the experimental period. Expressed per kg BW0.75 per day, ME intake decreased linearly with increasing age and was not affected by gender. Similarly, RE as protein decreased with increasing age and tended to be greater in males than in females, whereas RE as lipid increased with increasing age and was lower in males than in females. In addition, HP decreased with increasing age and was greater in males than in females, because of greater activity-related HP and FHP (47% and 9% greater in males compared with females). The FHP averaged 417 kJ/(kg BW)0.75 per day during the first 3 weeks of age and decreased to 317 and 277 kJ/(kg BW)0.75 per day in males and females, respectively, from 20 weeks of age onwards. Similar to FHP, MEm was lower in females than in males ((586 to 12 × BW) and (586 to 5 × BW) kJ/(kg BW)0.75 per day, respectively) and the kp and kf were estimated at 0.63 and 0.87, respectively. This study shows that the partitioning of RE and HP differs between genders in growing turkeys, which likely results in differences in nutrient requirements. [ABSTRACT FROM AUTHOR]

Abstract: This letter focuses on the variation of the Hall majority carrier mobility with the dopant compensation level in purely Boron-doped Czochralski grown silicon single crystals. Compensation was varied continuously at the sample scale via a step by step activation of the oxygen-based thermal donors. At room temperature, we show a strong drop in mobility for high compensation levels in both p- and n-type Si. Mobility models taking into account carrier scattering on ionized impurities and phonons could not reproduce this drop. We conclude that a specific effect of compensation must be taken into account to explain the observed behaviour. We qualitatively discuss physical mechanisms susceptible to reduce mobility in highly compensated Si. [Copyright &y& Elsevier]

Research on veal calf production has focused on maximizing lean tissue growth. Nevertheless, limited attention has been paid to the evolution of digestive and metabolic utilization of N and energy as calves get older, whereas age at slaughter increases. The objective of this study was to determine the effects of 4 concentrations of dietary crude protein (CP) content on protein and fat deposition and energy utilization in milk-fed calves at 3 stages of fattening using the balance technique combined with heat production measurements in a respiration chamber. At each stage, 16 Prim'Holstein male calves (mean body weight at each stage: 72, 136, and 212 kg) received 4 isocaloric diets with CP contents of 76, 88, 100, and 112% of a reference CP content fixed at 20% during the first stage and 19% during the 2 later stages. After 2 wk of adaptation to their respective diets and housing conditions, the calves were placed for 1 wk in an open-circuit respiration chamber for N and energy balance measurements (first 6 d) and measurement of the fasting heat production (last day). Measurements for a stage were performed over 2 periods of 4 successive weeks. There was no effect of dietary CP on digestibility during the 2 later stages, but the low-protein diet resulted in lower digestibility coefficients for dry matter, organic matter, gross energy, CP, and crude fat during the first stage. Endogenous fecal N was estimated as 2.5 g/kg of dry matter intake irrespective of stage, and metabolic urinary N was estimated at 0.07 g/kg of body weight0.85 per day. Maximum N retention was 32.8, 40.5, and 44.0 g/d at stages 1, 2, and 3, respectively. The effect of protein intake on protein deposition was dependent on age of the calves, because the marginal efficiency of digestible protein utilization decreased from 64 to 18% as animals got older. Fat deposition decreased with increasing dietary CP content irrespective of stage. Total energy retention was not modified by dietary CP content. The composition of body weight gain was affected differently for each stage, because the protein content of body weight gain increased with increasing dietary CP content during the first stage, whereas it remained constant during the other 2 stages. Fat and energy content in body weight gain decreased with increasing dietary CP irrespective of stage. Abstract: Introduction: Almost 50% of patients with Wegener''s granulomatosis (WG) develop ocular features, leading to visual loss in 8%. However, central retinal artery occlusion (CRAO) has exceptionally been reported. Exegesis: We report 2 patients with CRAO and WG according to ACR classification criteria. A literature search indicates that CRAO with WG has only been reported in 15 patients. We analyse the 17 cases to discuss systemic manifestations, ocular prognosis and treatment. Conclusion: CRAO is a rare manifestation of WG. Nevertheless, CRAO seems to be associated with vasculitic type of WG. So, a prompt diagnosis could lead to early aggressive regimen to improve visual and general prognosis. [Copyright &y& Elsevier]

We show that the global solutions to the Navier–Stokes equations in R3 with data in VMO-1 which belong to the space defined by Koch and Tataru are stable, in the sense that they vanish at infinity (in time), that they depend analytically on their data, and that the set of Cauchy data giving rise to such a solution is open in the BMO-1 topology. Mathematical models are increasingly used to predict the response of an animal to a changing nutrient supply. The objective of this experiment was to provide data that can be used in model development or evaluation and concerns the energetic efficiency with which nutrients are used for protein and lipid deposition. A basal diet (D1), limiting in lysine supply, was fed at 1.7 MJ metabolizable energy (ME)/(kg BW(0.60) x d1) to growing pigs that weighed approximately 60 kg. Four additional diets were formulated: the basal diet and a dietary supplement that consisted of starch (D2), starch and corn gluten meal (D3), starch and casein (D4) or starch and lipid (D5). The latter four diets were fed at 2.55 MJ ME/(kg BW(0.60) x d1) and ensured the same intake of the basal diet across treatments; the difference was supplied by the supplement. Metabolic utilization of the basal diet and supplements was determined using nitrogen and energy balances (indirect calorimetry). The N retention was similar in pigs fed diets D1, D2, D3 and D5 but considerably higher in those fed D4. A data analysis model was developed to account for differences in ME utilization between nutrients. The ME not deposited as protein entered a common pool of energy, which was used for adenosine triphosphate synthesis or lipid deposition. The energetic efficiencies of ME utilization were 0.842, 0.520 and 0.883 for starch, protein and lipid, respectively. Determines the accuracy of telephone-administered dietary recalls for 159 elderly subjects by comparing the telephone recalls of single midday meals consumed at congregate meal sites with data on actual intake of the meals. Accuracy was determined for kinds of food eaten, size of portions eaten, and content of 15 nutrients. Concludes that telephone contact is an acceptable way to obtain short-term dietary recall data from elderly subjects; Methods and results.

Du fait de la rareté des pathologies propres à notre spécialité, les étudiants en médecine bénéficient souvent d'un enseignement purement théorique de la médecine interne. Cette modalité d'apprentissage n'est cependant pas la plus adaptée pour l'acquisition de compétences d'examen physique d'une part, et pour la confrontation clinique aux pathologies de médecine interne d'autre part. Nous rapportons ici les résultats d'une expérimentation pilote utilisant une formation originale basée sur le jeu de rôle pour l'enseignement de la démarche diagnostique face à un phénomène de Raynaud. L'atelier se composait de 2 stations au format ECOS (examen clinique objectif et structuré) durant chacune 30 minutes. Les étudiants de DFASM-1 inscrits à l'enseignement optionnel "Maladies Systémiques" étaient répartis par groupes de 4–5 personnes. Tous avaient bénéficié au préalable d'un cours théorique sur le phénomène de Raynaud et la sclérodermie systémique. Sur chaque station, 2 étudiants étaient acteurs (l'un jouant le rôle de l'interne, l'autre celui de l'externe) et 2 étaient observateurs. Après un briefing succinct, les étudiants jouaient le scénario pendant 15 minutes, puis bénéficiait d'un débriefing de 15 minutes. Lors de la première station, les étudiants recevaient en consultation une femme de 26 ans, adressée par son médecin traitant pour suspicion de phénomène de Raynaud. La consigne donnée aux étudiants était de mener l'interrogatoire et l'examen physique de la patiente, de formuler une hypothèse diagnostique et de prescrire les éventuels examens complémentaires nécessaires. Les étudiants devaient aboutir au diagnostic de probable phénomène de Raynaud idiopathique. Le rôle de la patiente était joué par une facilitatrice médecin bénéficiant d'une expertise sur le phénomène de Raynaud et préalablement formée au rôle proposé. Lors de la deuxième station, les étudiants recevaient en consultation une femme de 56 ans, adressée par son médecin traitant pour un phénomène de Raynaud compliqué d'ulcération digitale. La consigne donnée aux étudiants était identique à la première station. Les étudiants devaient aboutir au diagnostic de probable sclérodermie systémique. Le rôle de la patiente était tenu par une patiente présentant une sclérodermie systémique, suivie par l'un des enseignants animant l'atelier, ayant bénéficié d'une formation préalable et ayant donné son accord pour participer à cet enseignement. Chaque station était animée par des enseignants formés au jeu de rôle et au débriefing. Au décours de l'atelier, les étudiants devaient remplir un questionnaire de satisfaction. Au total, 21 étudiants ont participé à l'atelier et 17 ont rempli le questionnaire de satisfaction. Les étudiants se montraient « très satisfaits » (Likert 4/4) de cette formation dans 94 % des cas. Ils considéraient cet atelier « peu stressant » (Likert 2/4) et « très formateur » (Likert 4/4) dans 71 % des cas, mais « un peu trop court » (Likert 2/4) dans 88 % des cas. A l'issue de cette formation, l'ensemble des étudiants s'estimaient « un peu » (Likert 3/4, 24 %) ou « beaucoup plus à l'aise » (Likert 4/4, 76 %) pour prendre en charge un patient atteint de phénomène de Raynaud idiopathique ; et « un peu » (Likert 3/4, 65 %) ou « beaucoup plus à l'aise » (Likert 4/4, 35 %) pour prendre en charge un patient atteint de sclérodermie systémique. Tous la recommanderaient aux étudiants ne l'ayant pas suivi. Interrogés en texte libre sur les principaux points forts de cette formation, les étudiants soulignaient surtout les avantages procurés par : – la mise en situation réelle en petits groupes, permettant une meilleure acquisition des compétences pratiques (notamment d'examen physique) et un échange plus interactif avec l'enseignant ; – la confrontation avec un vrai patient, permettant une meilleure rétention de la sémiologie et l'intégration d'une dimension relationnelle et humaine à l'apprentissage de la pathologie. Les principaux points faibles rapportés étaient une durée trop courte, l'illustration pratique de seulement 2 causes de phénomène de Raynaud (au détriment des autres) et le caractère potentiellement biaisant et stressant d'être observé lors de la mise en situation. Cette expérimentation originale suggère l'intérêt et la faisabilité sur un petit groupe d'étudiants d'ateliers de jeu de rôle portant sur l'enseignement des maladies rares à partir de situations fréquentes et la promotion de la "culture du doute". L'évaluation de son efficacité pédagogique et sa généralisation sur de larges promotions d'étudiants, sur la base de l'Axe 9 du Plan Maladies Rares 3, sont en cours de réflexion au sein de la commission de réforme de la formation initiale de la Conférence des Doyens. La granulomatose éosinophilique avec polyangéite (GEPA) est une vascularite affectant les vaisseaux de petit calibre et appartient aux vascularites associées aux ANCA (anticorps anti-cytoplasme des polynucléaires neutrophiles) [1]. Des manifestations digestives (douleurs abdominales, diarrhées, méléna) surviennent chez 20 à 30 % des patients atteints de GEPA [2]. L'atteinte digestive grave est un facteur prédictif de mortalité pris en compte dans le score pronostic Five Factor Score (FFS) et se révèle être un enjeu diagnostic. Nous rapportons un cas d'atteinte digestive diagnostiquée par une vidéocapsule. Cet homme âgé de 47 ans a été adressé pour altération de l'état général associée à une dyspnée et douleurs abdominales post prandiales responsables d'un amaigrissement de 10 kg en 1 mois. Ses antécédents étaient un diabète de type 1, une hypothyroïdie sur thyroïdite auto-immune et un asthme depuis l'âge de 10 ans. Il existait une hyperéosinophilie jusqu'à 11 000/mm3 (Fig. 1). Il y avait un syndrome inflammatoire biologique et une dénutrition sévère. La fonction rénale était normale, sans anomalies du sédiment urinaire. Le bilan thyroïdien était normal. La sérologie VHB témoignait d'une immunité vaccinale. Les sérologies parasitaires et examens parasitologiques des selles étaient négatifs. La recherche d'ANCA était positive de spécificité anti-MPO à un titre élevé 175,8 IA (< 6). Le scanner thoracique montrait un infiltrat micro-nodulaire diffus en arbre à bourgeons, il n'y avait pas d'hémorragie intra-alvéolaire sur la fibroscopie bronchique. Les biopsies bronchiques étaient en faveur d'une infiltration à éosinophiles. L'EMG montrait une mononeuropathie multiple. L'endoscopie digestive, le scanner abdominal et l'entéro-IRM étaient normaux. Les biopsies montraient une gastrite interstitielle et érosive modérée sans infiltration à éosinophiles. La vidéo-capsule montrait de multiples érosions et ulcérations superficielles du grêle proximal (flèches) sans ulcère creusant, sans atrophie villositaire ni sang. L'évolution a été favorable après induction par bolus de SOLUMEDROL et CYCLOPHOSPHAMIDE. L'atteinte digestive dans les GEPA est rarement isolée et peut menacer le pronostic vital [3]. La GEPA évolue selon une phase prodromique caractérisée par des manifestations allergiques (asthme, rhinite) puis s'installe une hyperéosinophilie sanguine et tissulaire contribuant à l'infiltration viscérale. Enfin, la phase de vascularite nécrosante explique les manifestations systémiques survenant quelques années après la phase prodromique [4]. L'inflammation vasculaire localisée sur les artères mésentériques est principalement responsable d'infarctus ischémique et perforations de l'intestin grêle. L'infiltration des muqueuses digestives par les polynucléaires éosinophiles peut se compliquer d'hémorragies digestives et d'ulcérations ischémiques gastriques, grêliques ou recto-coliques [5]. L'utilisation de la vidéo-capsule a permis dans trois cas rapportés [6–8] , de confirmer la présence d'anomalies digestives suspectées sur scanner et endoscopie: œdème sous muqueux, lymphangiectasies, ulcérations jéjuno-iléales. Devant la persistance des douleurs abdominales de notre patient et la normalité du bilan morphologique classique (endoscopie, scanner injecté et entéro-IRM), le recours à la vidéocapsule a permis d'apprécier la sévérité du diagnostic et d'instaurer un traitement plus agressif. Récemment, une étude rétrospective a montré l'importance de la vidéocapsule pour étudier l'intégralité du tractus gastro-intestinal afin de ne pas sous-estimer l'atteinte digestive des GEPA [9]. Ces lésions digestives peuvent aussi mimer celles observées au cours des maladies inflammatoires chroniques de l'intestin (MICI) et exceptionnellement, y être associées [10]. La vidéo-capsule pourrait être davantage utilisée pour diagnostiquer précocement les atteintes digestives graves de GEPA cotées dans le score pronostic FFS. [ABSTRACT FROM AUTHOR]
