Abstract: We report a case of hidradenocarcinoma, which showed only slight 18 F-FDG uptake. However, the rare sweat gland tumor demonstrated intense tracer uptake on 68 Ga-FAPI PET/CT. Abstract: Dermatofibrosarcoma protuberans is a rare soft tissue sarcoma with a high recurrence rate. Herein, we present 68Ga-FAPI and 18F-FDG PET/CT findings of dermatofibrosarcoma protuberans in a 45-year-old man. Dermatofibrosarcoma protuberans only shows limited FDG uptakes on 18F-FDG PET/CT, but demonstrated intense tracer uptakes on 68Ga-FAPI PET/CT. Abstract: 68Ga-FAPI is a newly developed tumor imaging agent with promising clinical applications. However, benign lesions may also show increased FAPI activity. We accidentally discovered that Schmorl node expressed FAPI activity in a patient with sweat gland cancer. Abstract: A 45-year-old woman recently complained of abdominal pain, poor appetite, and weight loss. Her serum ferritin, CA199, and CA242 levels were elevated. 68Ga-FAPI PET/CT revealed a subcapsular mass in the liver, which showed a significant FAPI uptake without other abnormal activity. Abstract: Mediastinal teratoma is a relatively rare disease that usually contains tissue from the endoderm, mesoderm, and ectoderm. We report a finding that mediastinal benign teratoma showed intense FAPI activity on 68Ga-FAPI PET/CT, which may lead to misdiagnosis of this benign lesion. Abstract: We present findings of FDG PET/CT and FAPI PET/CT in a 55-year-old woman with a history of black stools and low-back pain. Pelvic CT detected a giant presacral mass that was suspected to be malignant. 18F-FDG and 68Ga-FAPI PET/CT showed that the mass had intense activity. Abstract: Dermatomyositis is an idiopathic inflammatory myopathy often associated with malignancies. 68Ga-FAPI PET/CT was performed on a 58-year-old man with newly diagnosed dermatomyositis. 68Ga-FAPI PET/CT showed multiple increased FAPI activity in whole-body muscles and nasopharyngeal lesion. Abstract: A 67-year-old man presented with generalized muscle pain for 2 months. A potential malignancy was suspected. The patient was subsequently enrolled in the clinical trial of 68Ga-FAPI. Increased activity of FAPI and FDG was observed in muscle lesions. Moreover, 68Ga-FAPI PET/CT showed intense FAPI uptake in the pancreas. Abstract: 68Ga-labeled quinoline-based fibroblast activation protein inhibitors (68Ga-FAPIs) are promising agents for tumor imaging. However, some nonneoplastic lesions can also cause increased FAPI uptake. Abstract: A 48-year-old man presented with a painless enlarging mass on his right thigh for 2 months. He had a history of neurofibromatosis. MRI showed a mass in his right thigh. Malignant tumor was suspected. He was enrolled in the clinical trial of 68Ga-FAPI in solid tumors. Multiple increased FAPI uptake was observed in the right thigh mass, lung, and mediastinal lymph nodes, which were confirmed being pleomorphic rhabdomyosarcoma by biopsy. In addition, widespread metabolic activity was also observed in his cutaneous fibroma nodules. Abstract: We describe the case of a 48-year-old man suspected to have recurrent colon carcinoma who underwent 68Ga-DOTA-FAPI-04 PET/CT, revealing the presence of an incidentaloma with intense FAPI uptake in the left thyroid lobe. Based on PET/CT findings, malignancy was suspected. Biopsy confirmed the diagnosis of follicular thyroid adenoma with fibrosis and calcification. Abstract: Subcutaneous Ewing sarcoma is very rare. This report presents the case of a 23-year-old woman with a history of primary subcutaneous Ewing sarcoma who recently found a gradually increasing mass. Recurrent subcutaneous Ewing sarcoma was thus suspected. 18F-FDG PET/CT was performed and showed an FDG-avid mass in her buttocks. Abstract: A 46-year-old man presented with a 3-month history of epistaxis. CT and MRI scan showed a soft tissue mass in the left nasal cavity. An excisional biopsy of the mass revealed extramedullary plasmacytoma. 18F-FDG PET/CT showed intense activity in the region of the left nasal cavity but no other FDG-avid lesions. Additional workups did not reveal any evidence of systematic disease. Based on these findings, the patient was diagnosed as having solitary nasal extramedullary plasmacytoma. He received the local radiation therapy. Abstract: A left pulmonary nodule was identified by CT scan in a 53-year-old woman who had a car accident 10 days earlier. 18F-FDG PET/CT showed multiple FDG-avid lesions located at the left lung nodule, mediastinal lymph nodules, and L4 vertebral body. 68Ga-FAPI PET/CT was performed for further evaluation. However, 68Ga-FAPI demonstrated intense FAPI uptake in the accident-related fracture of the L4 vertebral body. A 5-year-old boy presented with right testicular swelling for 1 week. Ultrasonography revealed a right testicular hypoechoic mass with increased blood flow. A malignant tumor was suspected, and further investigation with F-FDG PET/CT showed a soft tissue mass with increased FDG uptake in the right testis. Biopsy of the mass confirmed a diagnosis of myeloid sarcoma.
