RIVM rapport:De bacterie Mycobacterium avium ssp. paratuberculosis (Map) wordt beschouwd als een mogelijke oorzaak van de ziekte van Crohn (morbus Crohn, MC). In samenwerking met Gelre ziekenhuizen heeft het RIVM een onderzoek uitgevoerd naar het voorkomen van Map in darmbiopten van patienten met MC, patienten met Ulcerative Colitis (UC) en controlepersonen zonder darmontsteking. De aanwezigheid van Map in de darmbiopten werd onderzocht met behulp van kweekmethoden, een DNA amplificatiemethode (PCR) en een immunologische detectiemethode (immunoperoxidase kleuring, IP-kleuring). Met de PCR-methode werd Map aangetoond in 7% van de MC-patienten, 8% van de UC-patienten en 5% van de controlepersonen. Met de gecombineerde kweek- en PCR-methoden waren gemiddeld 25% van de MC-patienten, 7% van de UC patienten en 27% van de controlepersonen positief. Met behulp van de IP-kleuring werd Map aangetoond in 20% van de MC-patienten, 13% van de UC-patienten en 29% van de controlepersonen. De resultaten van de kweek- en PCR-methoden tonen geen significant verschil tussen de aanwezigheid van Map in MC-patienten vergeleken met controlepersonen. De resultaten van de IP-kleuring tonen zelfs een hoger percentage Map-positieve controlepersonen vergeleken bij MC-patienten. Onze resultaten tonen duidelijk aan dat Map zowel bij MC-patienten als bij UC-patienten en controlepersonen voorkomt. Naast de aanwezigheid van Map is ook gekeken naar de aanwezigheid van Chlamydia in de biopten. Surprisingly, these data show even a 50% higher presence of Map-antigens in control persons (29%) compared to CD patients (20%). Our data clearly show that Map is present both in IBD patients (CD and UC) and in control persons. The results of culturing and IP staining show a good correlation; with both techniques, an average of 23% of CD patients and 27% of control persons is positive for Map. Remarkably, the results of culturing and IP staining show an even higher prevalence of Map in control persons compared to CD patients. RIVM rapport:The general hypothesis exists that NH4 level in cigarettes and NO level in cigarette smoke do affect the nicotine availability in the lungs and subsequently play a role in the tobacco addiction. The objective of this report was to investigate whether correlations exist between the cigarette parameters: NH4 level, NO emission and the declared tar and nicotine values. These correlations can be used to support the postulated hypotheses. In this report the NH4 amount in cigarettes was determined by a wet-extraction method. The NO emission was determined by means of a smoking machine and an NO-analyser. The declared tar and nicotine values were read from the packages. he correlation was investigated by plotting the values of the cigarette parameters against each other and determining the respective correlation coefficient (r2). There was a positive correlation between the declared tar and the nicotine value (r2 = 0.95). Also, the NO emission and the tar value seems to be correlated with each other (r2 = 0.47). The NH4 amount did not correlated with any parameters. Furthermore, the 100 different cigarette brands were divided in several subsets. These subsets were:filter/non-filter cigarettes menthol/non-menthol filter cigarettes light/regular filter cigarettes Next, differences were examined within NH4 level, NO emission and ratio tar/nicotine between the subsets of the cigarette brands. The NO emission was significantly lower by menthol and by light filter cigarettes compared with NO emission by non-menthol and by regular cigarettes. The ratio tar/nicotine of regular cigarettes was significantly lower compared with that of light cigarettes. This study showed that the NH4 level in cigarettes is related to the cigarette brand. NO emission is weakly related to the declared tar value, probably due to the used tobacco raw material. Er werd een positieve correlatie gevonden tussen de teer en nicotine waarden (r2 = 0.95). Verder werd een zwakke correlatie tussen de gemeten NO concentratie en de gedeclareerde teer waarde gevonden (r2 = 0.47). Het NH4 gehalte was niet gecorreleerd met een van de parameters. RIVM rapport:Ozone in the atmosphere serves as a partially protective filter against the most harmful part of the solar UV-spectrum. Decreases in ozone lead to increases in ambient UV with a wide variety of adverse effects on human health, aquatic and terrestrial ecosystems and food chains. Human health effects include the incidence of skin cancer, cataracts and an impairment of the immune system. Ozone depletion has been observed over the past decades, and is most likely caused by man made emissions of halocarbons. The ozone depletion observed over the past decades has probably led to a 5-10% increase in harmful UV-radiation in large parts of Europe. Due to the long atmospheric life time of the ozone depleting substances the countermeasures agreed upon could at best be expected to lead to a slow recovery of the ozone layer in the next 50-60 years. However, in that best scenario it is assumed that no interaction occurs with climatological changes, and that a full global compliance with the strictest Amendments of the Montreal Protocol is obtained. Recent scientific evidence indicates that climate change might delay the recovery of the ozone layer by 10 to 20 years. This report summarizes the present knowledge on the climate-ozone interaction, the past and present UV-climate in Europe and dose-effect relationships for health and aquatic effects. Using this information a preliminary integrated risk analysis is provided for skin cancer risks and effects on the primary production of phytoplankton. Skin cancer risks due to ozone depletion peaks in the period 2050-2070. The excess risks in North western Europe due to ozone depletion is estimated at 50-60 additional cases per million per year if no climate-ozone interaction is included, and nearly 100 additional cases per million per year if the interaction is included. It should be noted that large uncertainties still exist in view of the gaps in the present knowledge on various aspects of the cause-effect chain.

RIVM rapport:Het doel van dit onderzoek was de evaluatie van de carcinogene potentie van cyclosporine A in XPA-/- muizen. XPA-/- muizen zijn deficient in DNA-repair, met name in nucleotide excision repair (NER), en hebben een grotere gevoeligheid voor genotoxische carcinogenen en ultraviolet licht dan wild-type muizen. Het onderzoek was een onderdeel van een wereldwijd evaluatie programma van alternatieve carcinogeniteitsstudies, waaronder studies met transgene muizen, gecoordineerd door het ILSI Health and Environmental Sciences Institute (USA). Cyclosporine A is een immunosuppressief geneesmiddel en een niet-genotoxisch humaan carcinogeen.Het gebruikte proefprotocol maakt tevens een reductie van het aantal proefdieren mogelijk, vergeleken met een conventionele carcinogeniteitsstudie. Groepen van 15 mannelijke en 15 vrouwelijke XPA-/- muizen en wild type muizen kregen een dieet met 0, 3, 10, 30 of 80 mg/kg lichaamsgewicht cyclosporine A voor een periode van 9 maanden. Groepen van 15 mannelijke en 15 vrouwelijke XPA-/- , P53+/- dubbeltransgene muizen kregen een dieet met 0, 30 of 80 mg/kg lichaamsgewicht cyclosporine A. Cyclosporine A induceerde lymfomen en lymfoide hyperplasie van milt en lymfeklieren. Conclusie is dat cyclosporine A negatief is voor carcinogeniteit in het kortdurende alternatieve carcinogeniteitsassay met mannelijke en vrouwelijke XPA-/- muizen en wild type muizen en in mannelijke dubbeltransgene muizen, bij doseringen tot de 'maximum tolerated dose' van 30 mg/kg lichaamsgewicht. Het is echter positief in vrouwelijke dubbeltransgene muizen. Bij een dosering van 80 mg/kg lichaamsgewicht, die duidelijk hoger is dan de 'maximum tolerated dose', is cyclosporine A overtuigend positief in XPA-/- muizen en in dubbeltransgene muizen van beide geslachten. De carcinogene respons is groter bij dubbeltransgene dan bij XPA-/- muizen. Bij deze dosering is cyclosporine A ook positief in wild type vrouwtjes, doch negatief in mannelijke wild type muizen.P, The objective of this study was to evaluate the carcinogenic response of cyclosporin A in XPA-/- mice having a C57BL/6 background. XPA-/- mice are deficient in nucleotide excision repair and have shown increased susceptibility to genotoxic carcinogens and uv-light. The study was part of a world-wide evaluation program of alternative carcinogenicity assays, including assays with transgenic mice, coordinated by the ILSI Health and Environmental Sciences Institute (USA). Cyclosporin A is an immunosuppressive drug and a non-genotoxic human carcinogen, inducing lymphomas and Kaposi sarcomas in transplantation patients. The study protocol also enables a reduction of the number of laboratory animals as compared with the conventional carcinogenicity study. Groups of 15 male and 15 female XPA-/- mice or wild type (WT) mice were fed diets containing 0, 3, 10, 30 or 80 mg/kg bw cyclosporin A for 9 months (ad lib). Groups of 15 male and 15 female XPA-/-,P53+/- double transgenic (DT) mice were fed 0, 30 or 80 mg/kg bw for 9 months. Cyclosporin A induced lymphomas and lymphoid hyperplasia of the spleen and lymph nodes. It is concluded that cyclosporin A is negative for carcinogenicity in the short-term alternative carcinogenicity assay in male and female XPA-/- and WT mice and in male DT mice when administered at the 'maximum tolerated dose' (30 mg/kg bw). It is positive in female DT mice at 30 mg/kg bw. At a dose of 80 mg/kg bw that distinctly exceeds the maximum tolerated dose, cyclosporin A is positive for carcinogenicity in XPA-/- mice and in DT mice of both sexes. The positive response is stronger in DT mice than in XPA-/- mice. At 80 mg/kg bw cyclosporin A is also considered to be positive in WT females (based on a positive Peto trend test) and it is negative in WT male mice.

RIVM rapport:Humans are exposed via the environment and via food to Polycyclic Aromatic Hydrocarbons (PAH), mixtures considered carcinogenic by IARC. A quantitative cancer risk assessment for oral exposure is hampered by the absence of adequate data. The need for experimental data is substantiated by the fact that daily oral doses exceed inhaled doses for some potent carcinogenic PAH compounds, e.g. benzo[a]pyrene (B[a]P), by one order of magnitude, and the fact that epidemiological studies are not expected to provide useful data in this respect. For this reason we have performed a carcinogenicity study in rats; treatment (by gavage) for 2 years with the reference PAH B[a]P resulted in tumour-formation in a wide spectrum of organs and tissues, with liver and forestomach as major target-organs. Liver tumours in female rats were used for estimating a Virtually Safe Dose (VSD), i.e. the daily dose representing a one per million risk upon lifetime exposure, via methodology adopted by the Dutch Health Council (HCN, 1994-1996). Based on available data on occurrence and carcinogenic potency of PAH in Dutch diet it is suggested to apply a correction-factor of 10 for conversion to a VSD for B[a]P as indicator for all dietary PAH. With the resulting VSD of 0.5 ng B[a]P/kg bodyweight per day, cancer risks associated with PAH encountered in Dutch diet are estimated to be around acceptable risk levels. Parallel rat studies indicated that B[a]P-induced DNA adducts per se are not sufficient for tumour-development; induced local cell proliferation seems an additional critical factor. The possible implications of these findings are discussed., Polycyclische Aromatische Koolwaterstoffen (PAK) komen zowel wijdverbreid in het milieu als in voedsel voor, beide als gevolg van menselijk handelen. PAK worden beschouwd als kankerverwekkend voor de mens (IARC, 1983). Dit is gebaseerd op zowel dierexperimenteel werk als op epidemiologische studies. De mens staat continu bloot aan deze groep verbindingen via de inhalatoire alsook de orale route (via voedselconsumptie), en in sommige gevallen via de huid. Een kwantitatieve schatting van het risico op kanker als gevolg van de inhalatoire blootstelling aan PAK in het milieu laat zien dat deze de acceptabel geachte grens, 1 extra kankergeval per miljoen levenslang blootgestelden, ruimschoots overschrijdt. Schatting van het risico op kanker als gevolg van blootstelling aan PAK via het voedsel wordt belemmerd door gebrekkige dierexperimentele en epidemiologische gegevens. De noodzaak om dit risico te kwantificeren wordt ge6llustreerd door het feit dat de dagelijkse blootstelling via deze route in grootte een orde hoger geschat wordt dan die via inhalatoire blootstelling voor een aantal belangrijke carcinogene PAK, zoals benzo[a]pyreen (B[a]P). Omdat niet verwacht wordt dat epidemiologische studies hier op termijn uitkomst kunnen bieden, is grote behoefte aan goed uitgevoerd dierexperimenteel onderzoek. Om bovengenoemde reden is een carcinogeniteitsstudie uitgevoerd waarbij ratten levenslang oraal zijn blootgesteld aan B[a]P, algemeen beschouwd als een representatieve modelstof voor carcinogene PAK. De in het instituut gekweekte Wistar ratten (52 dieren per dosis en per sexe) zijn per maagsonde vijf dagen per week blootgesteld aan in soja-olie opgeloste B[a]P, in doseringen van 0 (kontrole), 3, 10 en 30 mg/kg lichaamsgewicht. Deze behandeling resulteerde in een dosis-gerelateerde toename in tumorincidentie in diverse organen en weefsels. RIVM rapport:Several epidemiological as well as experimental studies suggest that the composition of the maternal diet during pregnancy may have a programming effect on the fetus leading to epigenetic predisposition of chronic diseases in adulthood. Here, effects of various diets given during pregnancy and weaning on parameters related to chronic diseases during adulthood were studied in the rat. Seven groups of sixteen animals were put on an isocaloric semi-synthetic high-fat diet two weeks before pregnancy, and continuing throughout pregnancy and weaning. Diets differed in macronutrient - or fatty acid composition. Fetal development was studied on gestational day 21. After birth, litter size in parallel groups was reduced to eight pups. A series of physiological parameters were studied in the pups at age 12 weeks. The offspring of dams put on a low protein diet or a fish oil diet in utero and during weaning had a significantly reduced body weight gain from birth up to age 12 weeks in comparison to the control group. Male offspring aged 12 weeks fed the coconut oil diet in utero and during weaning appeared slightly insulin-resistant. No difference in the volume of the islets of Langerhans could be demonstrated in this group. Despite significantly smaller islets of Langerhans and a reduced number of large islets in the pancreas of 12-week-old male rats given the low protein diet in utero and during weaning, no differences in glucose or insulin concentration were found in comparison to the control group. Maternal diet during pregnancy and weaning did not influence immune function or blood pressure in the offspring at the age of 12 weeks. In conclusion, prenatal diet influences postnatal development, as determined by anatomical and physiological parameters., Uit zowel epidemiologische als dierexperimentele studies komen steeds meer aanwijzingen dat de samenstelling van de voeding tijdens de zwangerschap een belangrijke invloed heeft op de vatbaarheid van het nageslacht voor chronische ziekten op latere leeftijd. In deze dierexperimentele studie zijn de effecten van diervoeders met een verschillende samenstelling onderzocht op parameters voor chronische ziekten op volwassen leeftijd. Aan zeven groepen van elk zestien dieren werd een isocalorisch hoog-vet voer gegeven met een verschillende macronutrient- en vetzuursamenstelling vanaf twee weken voor de dracht tot aan het einde van de speenperiode. Op dag 21 van de dracht werd de foetale ontwikkeling onderzocht. Op de leeftijd van 12 weken is een aantal fysiologische en morfologische parameters bepaald. Nageslacht dat in utero en tijdens de speenperiode was blootgesteld aan een laag eiwit voer of een voer rijk in visvetzuren had een significant lager lichaamsgewicht vanaf de geboorte tot aan de leeftijd van 12 weken, ten opzichte van de controlegroep. Het mannelijk nageslacht van dieren van de cocosnootgroep (rijk aan verzadigde vetten) lijkt licht insuline resistent te zijn. Er was echter geen verschil in volume van de eilandjes van Langerhans ten opzichte van de controlegroep. De aanwezigheid van antipolymeer antistoffen (APA) is in de literatuur beschreven bij vrouwen met ernstige ziekteverschijnselen en een siliconen-borstimplantaat (SBI). Het doel van het in dit rapport beschreven onderzoek is vast te stellen of er in Nederland een populatie vrouwen met een SBI bestaat met ernstige ziekteverschijnselen, en een hoge prevalentie van APA. Deelneemsters werden geselecteerd middels een uitgebreide vragenlijst met vragen over spier- en gewrichtspijn, vermoeidheid, en vragen over het lichamelijke en psychosociale gevoel van welbehagen. Bovendien werd van de huisarts informatie gevraagd over de klinische symptomen van de SBI vrouwen. In totaal namen 42 vrouwen deel aan het onderzoek. De leeftijd varieerde van 31-73 jaar. De gemiddelde blootstelling aan een SBI was 16 jaar. Bij de vrouwen werd klinisch onderzoek uitgevoerd en bloed afgenomen. De prevalentie van voedselallergie is ongeveer 2% van de bevolking; in kinderen 5-8%. The developing immune system is especially sensitive to immunomodulatory influences. The time at which food allergens are first encounterd may have an impact on the development of food allergy. Also factors that are in the food but are not allergenic themselves may influence the development of oral tolerance to food allergens, i.e. those moieties that influnece immune responses (immunotoxicants) or may influence entry of proteins into the mucosa.

RIVM rapport:A representation is given of an analysis of developments in the access to scientific literature, an overview of the possibilities of co-operation with other libraries and the result of an assessment of the informational needs of its RIVM-users. The Working Group gives recommendations concerning present and future tasks, costs and budgeting and the place of the library in the organisational structure of the RIVM. The most important recommendations are: continuation and acceleration of the policy of digital availability of information, reduction of the in-print collection, broadening the supply of information (other than official scientific literature), setting up an e-archive for RIVM-reports (in co-operation with external partners), institutional co-ordination of the RIVM-Internet and Intranet activities (together with the IT-department), support and facilitating of knowledge management within the institute and cost charging for a part of the library services., Naast een analyse van de ontwikkelingen in de toegang tot de wetenschappelijke literatuur en een inventarisatie van de samenwerkingsmogelijkheden met andere bibliotheken wordt het resultaat van een uitgebreide behoeftepeiling binnen het RIVM gepresenteerd. RIVM rapport:Several epidemiological as well as experimental studies suggest that the composition of the maternal diet during pregnancy may have a programming effect on the fetus leading to epigenetic predisposition of chronic diseases in adulthood. Here, effects of various diets given during pregnancy and weaning on parameters related to chronic diseases during adulthood were studied in the rat. Seven groups of sixteen animals were put on an isocaloric semi-synthetic high-fat diet two weeks before pregnancy, and continuing throughout pregnancy and weaning. Diets differed in macronutrient - or fatty acid composition. Fetal development was studied on gestational day 21. After birth, litter size in parallel groups was reduced to eight pups. A series of physiological parameters were studied in the pups at age 12 weeks. The offspring of dams put on a low protein diet or a fish oil diet in utero and during weaning had a significantly reduced body weight gain from birth up to age 12 weeks in comparison to the control group. Male offspring aged 12 weeks fed the coconut oil diet in utero and during weaning appeared slightly insulin-resistant. No difference in the volume of the islets of Langerhans could be demonstrated in this group. Despite significantly smaller islets of Langerhans and a reduced number of large islets in the pancreas of 12-week-old male rats given the low protein diet in utero and during weaning, no differences in glucose or insulin concentration were found in comparison to the control group. Maternal diet during pregnancy and weaning did not influence immune function or blood pressure in the offspring at the age of 12 weeks. In vervolgexperimenten zijn drie concentraties S. enteritidis gebruikt, waarna de respons gedurende 24 uur is gemeten. De gebruikte cellijn bleek na 24 uur nog intact, de Il-8 productie correleerde met de doses, en nam toe in de tijd. Hieruit kan geconcludeerd worden dat het door ons ontwikkelde model gebruikt kan worden voor het bestuderen van factoren die van invloed zijn op dosis-respons relaties. De respons betreft dan prikkeling van het immuunsysteem ten gevolge van adhesie en invasie van darmepitheelcellen door salmonellae. Tenslotte wordt een benaderingwijze voorgesteld om resultaten van in vitro dosis-respons experimenten te vertalen naar de mens.

RIVM rapport:We investigated whether exposure to solar UVR would influence the occurrence of skin infections in a cohort of renal transplant recipients. In various experimental studies, exposure to UVR was demonstrated to possibly cause immunosuppression and impaired resistance to infections. We expected that such effects could be demonstrated more easily in the patients who were already immunocompromised. The lifetime cumulative exposure to solar UVR was estimated on the basis of self-reported data: the season in which the diagnosis was made was regarded as providing a rough estimate of the exposure just prior to or at the time of infection. In a Multivariate Poisson Regression Model for repeated measurements, we found the highest incidence of Herpes Simplex, Herpes Zoster and fungal/yeast infections to be associated with the summer season. There was no consistent association found between the lifetime cumulative estimate of exposure and the infection. The seasonal fluctuation may be due to the circannual rhythm in ambient levels of UVR., Dit rapport beschrijft een studie waarin werd nagegaan of blootstelling aan zonlicht van invloed is op het optreden van huidinfecties in een cohort niertransplantatie patienten. Dit deden wij, daar uit diverse met name experimentele studies bekend is dat blootstelling aan ultraviolette straling (UV) een immuunsuppressie en een verlaagde weerstand tegen diverse infecties kan veroorzaken. Wij verwachtten dat in deze groep patienten door de reeds aanwezige immuunsuppressie dergelijke effecten eerder aan het licht zullen treden. De 'life-time' cumulatieve blootstelling aan zonlicht werd retrospectief geschat op basis van zelf-gerapporteerde gegevens; seizoen van diagnose werd gezien als ruwe maat voor de blootstelling vlak voor of op het moment van het optreden van de infectie. In een multivariaat Poisson regressie model voor herhaalde waarnemingen werd gevonden dat Herpes Simplex-, Herpes Zoster- en schimmel/gist infecties het meest in het zonnige seizoen gevonden werden, terwijl er geen duidelijke samenhang bleek te zijn met de 'life-time' cumulatieve maat voor zonlicht blootstelling. De gevonden seizoenseffecten zouden samen kunnen hangen met het jaarritme in de UV belasting.

RIVM rapport:The effect of season on the antibody response after Hepatitis B (HB), Measles and Rubella vaccination in humans was investigated. In view of the immunosuppressive effects of ultraviolet radiation (UVR), especially the B-waveband (UVB), it was hypothesised that a lower antibody response after vaccination procedures that were started in summer might be detectable. IgG assessments and dates of vaccination were available 1. from a survey on the formation of anti-HB antibodies (anti-HBs) after vaccination to HB among paramedical students in Utrecht, 2. from an experimental study on the effect of artificial UVB on the immune response to HB vaccine, and 3. from a cross-sectional serosurveillance study on various vaccinations in a random sample of the Dutch population. In the surveys on HB the antibody formation in the course of a standard HB vaccination procedure (Engerix-B, SB; standard 0, 1, 6 months) was analysed by season of first vaccine injection. In the serosurveillance study the anti-Measles and anti-Rubella IgG titers in children who were aged 2-7 years at the time of the survey, and who had received their Mumps-, Measles-, Rubella-vaccination only once (MMR-1, at 14 months of age) were analysed by age and season. The data from the survey among paramedical students indicated a slightly retarded antibody response during vaccinations that were started in a sunny season. However, this finding was not consistently found after breaking down the data by calendar year and at the end of the procedure equal levels of anti-HBs were found. In the other surveys no seasonal influences on the formation of antibodies could be established. These data support the hypothesis of a reduced immunoprotection due to high ambient UVR during sunny season only to a limited extent. The study design may have been too crude with respect to both personal differences in exposure and susceptibility to UVR and the immune responses following immunisation for demonstrating the postulated effect of, Het effect van seizoen op de vorming van antistoffen na vaccinatie tegen hepatitis B, mazelen en rubella werd onderzocht. RIVM rapport:Recent studies on the immunosuppressive effects of UVR and the related resistance to infections in humans are presented. The waveband dependency of trans-to-cis isomerisation of urocanic acid in the stratum corneum and the role of DNA damage in UVR-induced erythema and immunosuppression were investigated to further elucidate the underlying mechanisms. Furthermore, human experimental studies on UVR-induced immunomodulation were performed. It appeared that the doses needed to suppress various immune parameters in humans (e.g. NK-activity, contact hypersensitivity) were higher than those needed in experiments in rodents. Still, extrapolation of experimental animal data to the human situation showed that UVR may impair the resistance to different systemic infections at relevant outdoor doses. In observational human studies we aimed to substantiate the relevance of UVR for infections in humans. It was shown that sunny season was associated with a slightly retarded but clinically non-relevant antibody response to hepatitis B vaccination. Furthermore, sunny season appeared to be associated with a small decline in the number of CD4+ T-helper cells in a cohort of HIV infected persons and a higher recurrence of herpes simplex and herpes zoster in a cohort of renal transplant recipients. However, in a study among young children a higher exposure to solar UVR was associated with a lower occurrence of upper respiratory tract symptoms. As disentangling the effects of UVR from other relevant factors is often impossible in observational studies, concise quantitative risk estimations for the human situation cannot be given at present.

Daarnaast geeft het rapport een overzicht van de verschillende determinanten van atopische ziektebeelden, zoals genetische, omgevings- en leefstijl factoren. Tot slot wordt een aantal biomarkers gepresenteerd, die gebruikt kunnen worden de incidentie van reeds ontwikkelde atopische ziektes verder te onderzoeken in epidemiologische studies. De afgelopen 20 tot 30 jaar is de prevalentie van astma met ongeveer 50% per tien jaar toegenomen. In dezelfde periode nam de prevalentie van hooikoorts eveneens snel toe. De genetische achtergrond speelt een belangrijke rol bij atopie, aangezien het risico om atopisch te zijn 50% is als een van de ouders atopisch is, en 60% indien beide ouders atopisch zijn. Omgevings- en leefstijlfactoren, zoals infecties op jonge leeftijd en blootstelling aan luchtverontreiniging kunnen gedeeltelijk verantwoordelijk zijn voor de waargenomen stijging in de prevalentie van allergische aandoeningen. In Nederland is de prevalentie en de incidentie voor astma in kinderen en volwassenen ongeveer gelijk met andere westerse Europese landen, waarbij de prevalentie van allergie voor luchtwegallergenen in kinderen tussen 7 en 12 jaar inmiddels is gestegen naar 30%. Op omgevings- en leefstijlfactoren zijn in principe interventies te plegen. Het is echter van belang om de aanleg van een kind om overgevoelig te worden in de vroege fase van zijn of haar leven te voorspellen, omdat preventieve maatregelen minder effectief zijn nadat de sensibilisatie reeds is opgetreden. Dit illustreert duidelijk de hoge behoefte aan 'markers', die het hoge risico op toekomstige sensibilisatie in jonge kinderen op betrouwbare wijze kunnen voorspellen. Onlangs kwamen we naar aanleid, The present report reviews the actual prevalence of allergic diseases and outlines that allergy is an important risk factor for asthma. Hypersensitivity to allergens, in particular house dust mite, is associated with an increased risk for later development of asthma. In addition, the report gives an overview of the various determinants of atopic diseases, such as genetic, environmental and lifestyle factors. Finally, a number of biomarkers is presented which can be applied to further investigate in epidemiological studies the incidence of established atopic diseases. Over the past 20 to 30 years the prevalence of asthma has increased by some 50% every ten years. In the same period the prevalence of hay fever has increased rapidly. Genetics play an important role in atopy, considering that the risk to be atopic is 50% if one parent is atopic and 60% when both parents are atopic. Environmental and lifestyle factors such as childhood infections and exposure to air pollution, may be partially responsible for the observed increase in the prevalence of allergic disease. In the Netherlands, the increase in the prevalence of asthma in children and adults seems to be similar compared to other Western European countries, by which the prevalence of allergy to aeroallergens in children aged 7 to 12 years has recently been shown to be increased to 30%. Environmental and lifestyle factors are in principle subject to intervention, but it is important to predict in the early stage of its life that a child is prone to become hypersensitive, because preventive measures are less efficient after sensitisation has become apparent. This clearly illustrates the urgent need for markers that reliably predict a high risk for future sensitisation in young children. Recently, we concluded from a cross-sectional epidemiological study that a relatively high level of exhaled NO at young age might be a suitable predictor of an increased risk to develop allergy later in life. RIVM rapport:Van 10-12 Mei 2000 werd in Bilthoven een wetenschappelijke bijeenkomst gehouden, getiteld "Exploratory Meeting Epidemiology on Occupational and Environmental Factors Associated with Autoimmunity". Het doel was te bepalen wat de optimale methodologie zou zijn voor het vaststellen van autoimmuniteit die is geassocieerd met blootstelling aan agentia op de werkplek of aan omgevingsfactoren. Bovendien werd de mogelijkheid besproken om in interdisciplinaire samenwerkingsverbanden dergelijke associaties te onderzoeken, gericht op silica, hexachlorobenzeen, en ultraviolette straling. Deze agentia werden geselecteerd omdat er aanwijzingen zijn dat zij een invloed hebben op autoimmuniteit in de bevolking. Er werd een aantal aanbevelingen gedaan: 1) systematische epidemiologische gegevens betreffende autoimmuniteit en autoimmuunziekten dienen te worden vergaard; 2) het opzetten van data bestanden betreffende autoimmuunaandoeningen dient te worden bevorderd; 3) er dient een internationaal geaccepteerde standaard voor diagnostische criteria voor autoimmuunziekten te worden ontwikkeld; 4) sociale en economische repercussies van autoimmuunziekten dienen in kaart te worden; 5) methoden om blootstelling vast te stellen en de gegevens te gebruiken in epidemiologische studies dienen te worden gestandaardiseerd; 6) laboratoriummethoden om biologische responsen te meten dienen te worden gestandaardiseerd; en 7) het opnemen van indicatoren van autoimmuneit en autoimmuunziekten alsmede relevante blootstellingen in lopende epidemiologische studies dient te worden bevorderd., An "Exploratory Meeting Epidemiology on Occupational and Environmental Factors Associated with Autoimmunity" was organized in Bilthoven the Netherlands, from May 10-12, 2000. The aim was to determine the optimal methodology for assessment of autoimmunity associated with occupational or environmental exposures in the human population, and to set up interdisciplinary and collaborative epidemiological studies to investigate the association of exposure to silica, hexachlorobenzene, ultraviolet radiation, and other agents with autoimmunity and autoimmune diseases in the human population. These agents were selected as carrying particular suspicion at present. Several recommendations were made: 1) systematic descriptive epidemiological data on autoimmunity and autoimmune disorders are required; 2) the establishment of disease-reporting registries should be encouraged; 3) the development of internationally accepted standard diagnostic criteria for all autoimmune diseases should be encouraged; 4) the social impact of these disorders should be evaluated and estimations of direct and indirect economic costs should also be made; 5) the methods of exposure assessment used in epidemiological studies should be standardized; 6) laboratory methods for measurement of biological responses should be standardized; and 7) the inclusion of indicators of autoimmunity and autoimmune diseases and of relevant environmental exposures in ongoing epidemiological studies should be encouraged.

RIVM rapport:Pneumolysine is het belangrijkste (cyto-) toxine van Streptococcus pneumoniae. Het is toxisch voor celmembranen, activeert het complement-systeem en bepaalt mede de virulentie van pneumococcen. Pneumolysine wordt daarom als kandidaatvaccin gezien. De toxiciteit van pneumolysine zelf beperkt echter de toepasbaarheid. Daarom werden er enkele mutanten en een dubbele mutant (o.a. respectievelijk PdB en PdBD) van pneumolysine gemaakt om de toxiciteit te verminderen met behoud van beschermende immuniteit tegen het wildtype. In mannelijke ratten bleek de LD50 waarde na intraveneuze toediening (IV) voor beide mutant producten 7-10 x hoger te zijn dan voor pneumolysine (ong. 300 ipv 30-45ug/kg), maar veel lager dan verwacht op grond van in vitro vooronderzoek naar haemolytische activiteit. Er werden geen verschillen waargenomen tussen pneumolysine, PdB en PdBD wat betreft de primaire doelorganen van toxiciteit (bloedcellen en -vaten). In een sub-acute toxiciteitonderzoek had pneumolysine toediening (5 weken, 1 maal/week 5 ug of 15 ug pneumolysine /kg intraveneus of subcutaan) geen effect op orgaangewichten, bloed- en beenmergparameters, klinische chemie en immunologische testen. Pneumolysine toediening leidde tot ernstige nierschade vanwege de membraantoxische (dus ook haemolytische) activiteit van pneumolysine. Vrouwelijke ratten bleken gevoeliger voor deze nefrotoxiciteit. Tevens werd een minimale ontstekingsreactie op de SC injectieplaats waargenomen en leidde toediening van pneumolysine tot geringe immunologische veranderingen (verhoging van IgM serumspiegels en veranderingen in absolute of relatieve aantallen B en T-cellen in de milt). Daarom moet bij gebruik van een rDNA geproduceerde pneumolysine mutant in een vaccin gestreefd worden naar een product met minimale haemolytische activiteit waarbij de beschermende immuniteit gehandhaafd blijft., Pneumolysin is the major cytotoxin of Streptococcus pneumoniae . It has membrane toxicity, activates the complement systeme and determines pneumoccal virulence. Pneumolysin is considered as candidate vaccine. However, the pneumolysin toxicity may limit its application. Therefore, single and double mutants of pneumolysin (e.g. PdB and PdBD, respectively) were produced to reduce toxicity with conservation of protective immunity against the wild type pneumolysin. In male rats LD50 values after intravenous administration (IV) for both mutant products were 7 -10 x higher than for pneumolysin (appr. 300 ug/kg and 30-45 ug/kg, respectively), but much lower than expected, based of in vitro studies(haemolytic activity). With regard to the suspected primary targets of toxicity (i.e. blood cells and endothelium), no differences were observed between pneumolysin, PdB and PdBD. In a sub-acute toxicity study pneumolysin administration (5 weeks, 1 administration/ week, 5 or 15 ug/kg, IV or SC) had no effect on organ weights, haematology, bone marrow cell counts, clinical chemistry and immunological assays. Pneumolysin induced major kidney damage as result of membrane toxicity (e.g. for erythrocytes and bloodvessels). Female rats were found to be more susceptible to this nephrotoxicity. Minimal inflammatory reactions at the SC injection sites were observed too. Pneumolysin induced minor immunological changes (increase of serum IgM levels, changes in relative and absolute splenic B- and in T cell counts). Therefore, when using rDNA-derived pneumolysin toxoids in a vaccine, it seems to be warranted to use a product with a minimum of haemolytic activity with conservation of protective immunity.

RIVM rapport:Het is nog onduidelijk of zonlicht, en in het bijzonder ultraviolet B, schadelijk is voor cellulaire immuniteit in personen die met HIV zijn geinfecteerd, leidend tot een versnelde progressie van AIDS. Wij hebben bij HIV+ homosexuele deelnemers in de "Amsterdam Cohort Study" de mate van UVB blootstelling vastgesteld gedurende de periode van 1995 tot 1997, door middel van een retrospectieve questionnaire. Een tweetal maten werd daarbij gehanteerd: een cumulatieve maat (het totaal aan UVB opgelopen gedurende de gehele periode) en een korte termijn maat (de blootstelling gedurende het aangegeven tijdvak). Gebruikmakend van de non-parametrische Mann-Whitney test en regressie analyse werd de associatie tussen de totale UVB blootstelling die werd opgelopen gedurende twee jaar en het verloop van de volgende immunologische parameters bestudeerd: CD4+ T cel aantallen, CD4/CD8 ratio, en T cel reactiviteit na stimulatie met anti-CD3 antilichamen. Voorts werd de korte termijn associatie tussen UVB blootstelling per periode en de immunologische parameters bestudeerd, gebruikmakend van een lineair mixed-effect model (LME). Met geen van beide benaderingen was het mogelijk een significant effect van blootstelling op deze parameters aan te tonen. In de benadering met de korte termijn blootstelling was een trendmatig effect te zien. Het is niet aannemelijk dat dit geringe effect klinische relevantie voor de individuen in het onderzoekscohort heeft, maar het geeft wel aan dat er mogelijk een effect van UV in een grotere onderzoeksgroep vastgesteld zou kunnen worden., Whether sunlight (especially ultraviolet B radiation: UVB) exposure is harmful to the cellular immunity of HIV infected individuals and whether it is associated with an accelerated progression toward AIDS is still under debate. Among HIV+ homosexual participants of the Amsterdam Cohort Study, the degree of exposure to UVB was assessed over a period from 1995 until 1997 by means of a retrospective questionnaire. Two UVB measures were calculated: a cumulative measure (the total amount of UVB exposure received over two years) and a short-term measure (the amount of UVB exposure per stated period). Using both the nonparametric Mann-Whitney test and robust regression, the association between the total amount of UVB received in the two-year interval and the slopes of the following immunological parameters were studied: CD4+ T cell count, CD4/CD8 ratio and T cell reactivity after stimulation with CD3 antibodies. Secondly, the short-term association between UVB exposure per period and the immunological parameters were studied with linear mixed-effect models (LME). In the study group with a median CD4+ T cell count of 480 x 106/l, no association could be established between UVB exposure, and the immunological parameters. Among HIV infected individuals, no indications could be found between the two year cumulative UVB exposure and markers of cellular immunity. Using the linear mixed effects model for longitudinal data, a slight trend in lower values of markers of cellular immunity after higher short term UVB-exposure was noted. It is quite unlikely that this small effect has clinical significance for the individuals of the study cohort, yet it may indicate that an effect of UV might have been established in a larger cohort.

RIVM rapport:De beste manier om in het proefdier effecten op het immuunsysteem vast te stellen, is de bestudering van effecten op antigeen specifieke immuunresponsen, bijvoorbeeld na sensibilisatie met T-cel afhankelijke antigenen. Waarschijnlijk gaat dit ook op voor het testen van effecten in de bevolking. Om die reden is gesuggereerd antilichaam responsen na vaccinatie als uitleessysteem te gebruiken. Naast omgevingsfactoren worden vaccinatieresponsen beinvloed door andere factoren. Een factor die van grote invloed is, is het vaccin zelf en de procedure om te vaccineren. Daarnaast is de intrinsieke capaciteit van de recipient om te reageren van belang; dit wordt bepaald door genetische factoren en de leeftijd. Daarnaast zijn psychologische stress, voeding en ziekte (waaronder infectieziekten) van belang. In het rapport wordt een overzicht gegeven van de literatuur over invloeden op vaccinatieresponsen. Het blijkt dat voor wat betreft exogene factoren er duidelijk aanwijzingen zijn dat roken, voeding, psychologische stress en bepaalde infectieziekten een effect op vaccinatieresponsen hebben, maar dat het moeilijk is vast te stellen wat het relatieve belang ervan is. Bekend is dat genetische factoren sommige individuen voor sommige vaccins ongevoelig maken. Een algemene conclusie is dat in epidemiologische studies, waarbij nadelige effecten van blootstelling aan omgevingsfactoren op het immuunsysteem worden bestudeerd, en waarbij vaccinatietiters worden gebruikt, die additionele factoren in aamerking genomen dienen te worden. Indien voor deze additionele factoren wordt gecorrigeerd, kan een studie waarbij associatie wordt gevonden van een verminderde vaccinatierespons met blootstelling aan een omgevingsfactor indiceren dat het immuunsysteem suboptimaal functioneert. Het is niet waarschijnlijk dat een dergelijk effect zal inhouden dat bescherming waarvoor de vaccinatie was bedoeld wordt aangetast. Provided that these factors are corrected for, a study that shows an association of exposure to a given agent with diminished vaccination responses may indicate a suboptimal function of the immune system, and clinically relevant diminished immune response. It is quite unlikely that environmental exposures that affect responses to vaccination, may in fact abrogate protection to the specific pathogen for which vaccination was meant. Only in those cases, where individuals have a poor response to the vaccine, exogenous factors may perhaps have a clinically significant influence on resistance to the specific pathogen
