Lateral flow assays (LFAs) have been extensively used as point-of-care testing platforms because they are inexpensive, portable, simple, and rapid, which particularly improve their availability in resource-poor settings. However, the poor sensitivity of LFAs restricts their further applications. Herein, we proposed a novel and simple method to enhance the detection sensitivity of LFAs by embedding cotton thread-based barriers into paper and further integrating them into strips to decrease the flow rate of sample and extend the reaction time. The number and hydrophilicity of the embedded cotton threads were sequentially optimized. The flow rate of liquid in cotton thread-embedded LFAs was mathematically simulated using a circuit-like model and the simulation results are consistent with the experimental results. With using human immunodeficiency virus nucleic acid as a model target, the cotton thread-embedded LFAs presented a fourfold enhancement in detection sensitivity compared to that of the unmodified LFAs. The global HIV epidemic has undergone significant shifts over the last decade, transforming HIV into a chronic illness. A shift in focus is now sorely needed on how to provide treatment and care to individuals living with HIV that can improve their quality of life by addressing the psychosocial milieu of the disease. In the area of HIV prevention and testing, the benefits of working with couples, rather than individuals, have been delineated and empirically supported. This paper extends the work by proposing a model of a couple-based intervention that can help couples, regardless of HIV concordance status, move through the HIV care cascade, from testing, linkage to care, treatment, and retention using a cognitive-behavioral couples framework. This paper draws primarily on research with heterosexual couples in sub-Saharan Africa, though adapting this framework to meet the needs of diverse couples in other geographical regions is recommended. nal fissure is one of the most common and painful proctologic diseases. Its treatment has long been discussed and several different therapeutic options have been proposed. In the last decades, the understanding of its pathophysiology has led to a progressive reduction of invasive and potentially invalidating treatments in favor of conservative treatment based on anal sphincter muscle relaxation. Despite some systematic reviews and an American position statement, there is ongoing debate about the best treatment for anal fissure. This review is aimed at identifying the best treatment option drawing on evidence-based medicine and on the expert advice of 6 colorectal surgeons with extensive experience in this field in order to produce an Italian position statement for anal fissures. While there is little chance of a cure with conservative behavioral therapy, medical treatment with calcium channel blockers, diltiazem and nifepidine or glyceryl trinitrate, had a considerable success rate ranging from 50 to 90%. Use of 0.4% glyceryl trinitrate in standardized fashion seems to have the best results despite a higher percentage of headache, while the use of botulinum toxin had inconsistent results. Nonresponding patients should undergo lateral internal sphincterotomy. The risk of incontinence after this procedure seems to have been overemphasized in the past. CD4 cell count and viral load are the most widely used markers of Human Immunodeficiency Virus (HIV). Obtaining accurate measures of these laboratory markers can be challenging. The purpose of this study was to assess the reliability and validity of self-reported and abstracted paper medical record CD4 counts, viral load, and outpatient visits using electronic data as the gold standard. We sampled a subgroup of 114 HIV-infected individuals in Los Angeles from a larger multi-site study for whom we could confirm outpatient visits and CD4 count and viral load data through electronic data. Agreement between electronic and paper record abstractions was approximately 90% for CD4 count (kappa = 0.85; P < 0.001), 78% for viral load (kappa = 0.56; P < 0.001), and 62% for visits (kappa = 0.02; P = 0.36). Agreement between electronic and self-reported measures was 80% for CD4 count (kappa = 0.71; P < 0.001), 77% for viral load (kappa = 0.54; P < 0.001), and 62% for visits (kappa = 0.13; P < 0.05). Using electronic data as the gold standard, we found that the accuracy of self-report was as good as paper record abstraction regarding viral load levels and visits, and only slightly lower for CD4 count. Despite limited generalizability, these findings are important because they suggest that the accuracy of clinical information collected by self-report and paper medical records are similar and raise questions about whether conducting abstractions of paper medical records is worthwhile, especially if it increases the costs of data collection. Occupational exposures in the large industrial mining sector contributed significantly to South Africa's high excess death rate due to COVID-19. Historically poor work-protection oversight has perpetuated centuries of risky labor and living conditions within the industry, driving high levels of disease co-morbidities, and supporting enduring social vulnerabilities. In this paper, we offer a syndemic lens to consider the clustering of adversely interacting diseases among mineworkers in South Africa, drawing attention to the complex occupational health crisis and the need to move beyond simply reporting individual diseases or comorbidities among this population. The physically demanding and dangerous working conditions, the lack of adequate changes to crowded and unsanitary working and living situations, the failure to meet social and labor plan targets, the continued precarious nature of working contracts and mines, and the limited access to robust healthcare reflect the historically exploitative nature of industrial mining in South Africa that places miners at increased risk for various syndemics. This assessment of the adverse interactions of diseases and socioeconomic and political conditions highlights the need for focused research and more follow-through in comprehensive occupational reforms. Contact tracing (also known as partner notification) is a primary means of controlling infectious diseases such as tuberculosis (TB), human immunodeficiency virus (HIV), and sexually transmitted diseases (STDs). However, little work has been done to determine the optimal level of investment in contact tracing. In this paper, we present a methodology for evaluating the appropriate level of investment in contact tracing. We develop and apply a simulation model of contact tracing and the spread of an infectious disease among a network of individuals in order to evaluate the cost and effectiveness of different levels of contact tracing. We show that contact tracing is likely to have diminishing returns to scale in investment: incremental investments in contact tracing yield diminishing reductions in disease prevalence. In conjunction with a cost-effectiveness threshold, we then determine the optimal amount that should be invested in contact tracing. We first assume that the only incremental disease control is contact tracing. We then extend the analysis to consider the optimal allocation of a budget between contact tracing and screening for exogenous infection, and between contact tracing and screening for endogenous infection. We discuss how a simulation model of this type, appropriately tailored, could be used as a policy tool for determining the appropriate level of investment in contact tracing for a specific disease in a specific population. This special issue contains 10 invited review papers that highlighted and extended the presentations at the NIDA-sponsored workshop "Unraveling NeuroAIDS in the Presence of Substance Use Disorders" at the 25th Society on NeuroImmune Pharmacology conference in 2019. The topics covered by these papers focused on the interactive, additive or synergistic effects of substance use disorders (SUD) with HIV infection on the immune system and on neuropathogenesis. These papers reviewed four categories of substances of abuse (opioids, tobacco, stimulants, and cannabis) and how comorbid HIV infection (including models with HIV proteins, HIV transgenic rodent models and SIV) might further impact the dysregulated dopaminergic and immune systems, and the subsequent neuropathogenesis and behavioral disorders known as HIV-associated neurological disorders (HAND). These reviews provided detailed background knowledge regarding how each of these addictive substances and HIV individually or collectively affected the immune system at the cellular, molecular and system levels, and the subsequent clinical and behavioral outcomes. The article reports on the identification of tetherin, a protein factor that stops human immunodeficiency virus-1 (HIV) stuck to surface of human cells and unable to spread. Virologist Paul Bieniasz at the Aaron Diamond AIDS Research Center and Laboratory of Retrovirology at the Rockefeller University in New York investigated cell-surface proteins that might keep to HIV-1 in the absence of viral protein U (VPU). It is said that there is much to learn about tetherin for there are hints that it has broad antiviral activity. Moreover, there is a need for investigations whether genetic variation in tetherin might affect susceptibility to HIV infection.

Failure to meet international targets set for the human immunodeficiency virus HIV pandemic suggests that more effective public health strategies are needed. New strategies informed by behavioural economics are now increasingly being tested, with promising results. However, the evidence base is diverse and challenging for policymakers to interpret. This paper aims to synthesise existing evidence by reporting results from a systematic review of behavioural economics-based interventions for addressing HIV prevention, testing and treatment. The reported study was a systematic review of randomized controlled trials. The search was conducted in four electronic medical literature databases, six trial registries, four grey literature sources and was not restricted to any country or region. Bias was assessed using criteria outlined in the Cochrane Handbook for Systematic Reviews; quality of evidence was assessed using GRADE methodology. Fifteen full text articles were included in the final analysis. The synthesis of these studies revealed that strategies involving opt-out defaults, active-choice defaults, and lottery incentives can potentially increase uptake of HIV testing. Lottery incentives also showed signs of effectiveness in improving HIV prevention, ART adherence and initiation. Despite the promising findings, the overall evidence was judged to be of moderate to very low quality. Behavioural economics-based interventions are promising behavioural change strategies, although more well-designed studies are needed to strengthen the evidence base. [ABSTRACT FROM AUTHOR]

This scoping review maps recent research into peer navigation programs for people living with HIV. Four databases were systematically searched in June 2020. Results were screened according to defined criteria and were not restricted to any design, outcome or country. Six papers drew from randomised control trials, five from quasi-experimental or pragmatic trials, and four panel, eight qualitative, three mixed method and one cross-sectional designs were included for review. Programs incorporated health systems navigation and social support. Authors provided strong theoretical bases for peers to enhance program effects. Studies primarily reported program effects on continuum of care outcomes. Further research is required to capture the role HIV peer navigators play in preventing disease and promoting quality of life, mental health, and disease self-management in diverse settings and populations. Peer programs are complex, social interventions. Future work should evaluate detailed information about peer navigators, their activities, the quality of peer engagement as well as employee and community support structures to improve quality and impact. Due to stigma or legal issues, populations with higher HIV risk are often hard to reach, which impedes accurate population estimation of HIV burden. To better sample hard-to-reach populations (HTRPs) for HIV surveillance, various sampling methods have been designed and/or used since HIV epidemic following the first reported AIDS cases in 1981. This paper describes the development and the assessment (i.e., validity and reproducibility) of approximately eight sampling methods (e.g., convenience sampling, snowball sampling, time location sampling, and respondent-driven sampling) for HTRPs in HIV surveillance, with a focus on respondent-driven sampling (RDS). Compared to other methods, RDS has been greatly assessed. However, current evidence is still inadequate for RDS to be considered the best option for sampling HTRPs. This special issue contains 11 papers, including 3 Invited Reviews, one Brief Report and 6 Original Articles, which highlight the work by selected presenters at the third meeting of the biennial Conference Series to Promote Global Health held on August 15-17, 2017 in Los Angeles University City, California, USA. The Conference Series is sponsored by the National Institute on Drug Abuse, of the U.S. National Institutes of Health. A major goal of the conference is to promote international collaboration that will improve practices in the US and other countries, which is greatly needed to keep the scientific, treatment and service communities around the world abreast of the latest research findings and methods in substance abuse, HIV, and related areas, and to lead future directions in research. The papers in this special issue highlight current reseach in the areas of substance use disorders and HIV-infection, and related treatments to these disorders, as well as advancing the research of precision medicine in substance abuse, HIV, and care. Unlike prevailing research methodologies in the interdisciplinary field of global health, feminist methodologies allow researchers to unsettle the premises and assumptions of the field. This paper describes how we mobilize transnational feminist ethnography in our research on HIV in India and postabortion care in Senegal. Transnational feminist perspectives enable us to re-embed HIV prevention and post-abortion care in the politics of gender and sexuality and in postcolonial histories of health governance. They consider the role of gender and sexuality not just in terms of gendered health inequalities, but also in terms of how global health problems are defined, managed, measured, and contested. We outline how our projects put this lens into practice by pushing the methodological boundaries of time, scale, and scope and through a historicized, multisited, and multiscalar approach that triangulates multiple sources of data. Feminist ethnography requires that we turn the critical gaze upon ourselves, reflecting on multiple and shifting personal and professional positionalities during and after fieldwork and our ethical commitments to our interlocutors. While acknowledging the significant personal, institutional, and professional challenges of using feminist methodologies, especially given the dominant modes of research in global health, we urge greater consideration, among both advanced and early scholars, of the possibilities they offer for studying global health problems. Purposeof Review: Young people face the highest number of new HIV infections globally. With today's increasing access to smartphones, serious games have been viewed as an effective means of improving knowledge and behavioral outcomes. This systematic review describes current HIV prevention serious games and their relationship with HIV-related knowledge and behavioral outcomes. Recent Findings: A search of HIV prevention serious games was conducted using PubMed, CINAL, IEEE, Web of Science, and Google Scholar. A total of 31 papers were identified, which consist of 20 studies and 11 protocols. Results for knowledge, attitudes, intentions, and behaviors were mixed. Two interventions reported improvement in PrEP usage and optimal dosing. Summary: Gaming appears to be a viable and engaging method to improve knowledge, attitudes, and behavioral outcomes to promote HIV prevention among diverse groups of adolescents and young adults globally. Some biological therapies for psoriasis can cause the reactivation of viral infections. Although recent studies suggest no increased rate of reactivation with biological therapies, some life-threatening cases have been reported. Therefore, this meta-analysis examined the rate of virus reactivation in patients with psoriasis with biological therapies and concurrent hepatitis B virus (HBV), hepatitis C virus (HCV), or human immunodeficiency virus (HIV) infection. PubMed, Embase, and the Cochrane Library were searched for available papers from inception to December 2021. The outcome was the number of patients with virus reactivation after using biological therapies. The random-effect model was used in all analyses. Fourteen reports (1033 patients) were included. The pooled overall rate of virus reactivation was 0.04 (95%CI 0.01–0.09; I2 = 67.7%, P < 0.001). The pooled rates of HBV, HCV, and HIV reactivation were 0.04 (95%CI 0.00–0.10; I2 = 79.9%, P < 0.001), 0.07 (95%CI 0.02–0.14; I2 = 23.7%, P = 0.24), and 0.12 (95%CI 0.00–0.40), respectively. The pooled rates of HBV and HCV reactivation were 0.10 (95%CI 0.03–0.19) and 0.08 (95%CI 0.03–0.15) in Asia, but 0.00 (95%CI 0.00–0.01) and 0.04 (95%CI 0.00–0.21) in Europe. The publication type also influenced the results. The use of biological therapy in patients with psoriasis and HBV, HCV, or HIV infection might be associated with the rate of viral reactivation, but this meta-analysis had limitations, and the evidence might be weak. Nevertheless, it might suggest that at least a consultation with an infection specialist might be warranted in patients with psoriasis in whom biological therapies are considered. Key occupational groups in sub-Saharan Africa (SSA) are at increased risk of HIV, and may be at increased risk of substance use. In January 2018, we systematically searched for studies reporting prevalence of, and risk factors for alcohol misuse or illicit drug use and their association with HIV incidence or prevalence among fisherfolk, uniformed personnel, truckers, miners, motorcycle taxi riders and sex workers in SSA. Seventy-one studies published between 1983 and 2017 were included: 35 reported on alcohol misuse (19 using AUDIT, 5 using CAGE) and 44 on illicit drug use (eight reported both). Median prevalence of alcohol misuse based on AUDIT/CAGE was 32.8% (IQR 20.8–48.5%). Prevalence of illicit drug use ranged from 0.1% (95% CI: 0.0–0.2%) for injection drug use to 97.1% (95% CI: 85.1–99.9%) for khat (among uniformed personnel). Among papers examining associations between substance use and HIV incidence (n = 3) or prevalence (n = 14), nine papers (53%) reported a significant positive association (2 with incidence, 7 with prevalence). This paper examines the effect of human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) on economic growth in sub-Saharan Africa (SSA) using data from 46 SSA countries spanning the period 2000–2015. Results based on the system-generalized method of moments estimation showed that a 1% increase in the HIV/AIDS prevalence rate in SSA decreased the growth in per capita income by 0.47%. The impact of HIV/AIDS on economic growth was felt most in Eastern Africa followed by West Africa, although the prevalence rate was highest in Southern Africa. In particular, a 1% increase in the prevalence rate of HIV/AIDS in Eastern Africa and West Africa retarded growth in per capita income by 0.64% and 0.47%, respectively, over the study period. The paper suggests that for SSA to minimize the impact of HIV/AIDS prevalence on growth, African governments should take a cue from the southern African countries by making anti-retroviral drugs more accessible and affordable to persons living with the disease. Dimension reduction is a common problem when analysing large data sets. The present paper proposes a method called reduced multidimensional scaling based on performing an initial standard multidimensional scaling on a reduced data set. This method faces the problem of finding a representative reduced sample. An algorithm is presented to perform this selection based on alternating sampling in outlier areas and observations in high density areas. A space is then constructed with the selected reduced sample by standard multidimentional scaling using pairwise distances. The observations not included in the reduced sample are then projected on the constructed space using Gower's formula in order to obtain a final representation of the whole data set. The only requirement is the ability to compute distances among observations. A simulation study showed that the proposed algorithm results performs well to detect outliers. Evaluation of running times suggests that the proposed method could run in a few hours with data sets that would take more than one year to analyse with standard multidimensional scaling. Since there is little evidence about gay community norms across the world, we reviewed published literature in this area and discuss implications for prevention and research. Eight databases were searched for articles and 16 were considered suitable for analysis. All used quantitative methodology and seven were based on a published theoretical framework. The most common theory employed by four out of the seven papers was the Theory of Reasoned Action. All papers reviewed examined norms on condom use but norms on other risk reduction practices were not explored in these papers. Seven (44 %) studies found those men who perceived strong social support from their peers, were less likely to engage in unprotected anal intercourse. This finding suggests that social support from partners and community members plays a role in shaping safe sex norms. Better understanding of norms may help to redress HIV behaviour change programmes, particularly at the time when HIV diagnoses rates are increasing in most gay communities across the world. In the United States, HIV outbreaks are occurring in areas most affected by the opioid epidemic, including West Virginia (WV). Cultural Theory contends that multiple cultures co-exist within societies distinguished by their differing intensities of rules or norms of behavior ('grid') or degree of group allegiance/individual autonomy ('group'). Accordingly, we would expect that perceptions about HIV, including stigma, correspond with individuals' grid/group attributes. To explore this, we conducted qualitative interviews with people who inject drugs (PWID) recruited from a WV syringe service program. This paper focuses on our unexpected findings on stigma during a coinciding HIV outbreak. PWID living homeless identified as belonging to a 'street family'. Its members were mutually distrustful and constrained by poverty and drug dependence but despite their conflicts, reported openness between each other about HIV + status. Interviewees living with HIV perceived little enacted stigma from peers since the local outbreak. Contrasting stigmatizing attitudes were attributed to the town's mainstream society. The 'High Five' (Hi-V) Club, expressing defiance towards stigmatizing behavior outside the street family, epitomized the tensions between a desire for solidary and mutual support and a fatalistic tendency towards division and distrust. Fatalism may hinder cooperation, solidarity and HIV prevention but may explain perceived reductions in stigma. [ABSTRACT FROM AUTHOR]

Purpose of Review: There are growing expectations for the return of individual-level research results (RoR), which promotes autonomy and potential clinical and personal benefits. There are ethical and practical challenges, however, that may be exacerbated in research that assesses neurocognitive and psychological outcomes, including HIV-associated neurocognitive disorder (HAND). This paper reviews central concepts for RoR and recent empirical and conceptual articles from Alzheimer's disorder (AD) as a model for HIV. Recent Findings: Data from AD studies indicate high participant interest and low risk of harm from RoR, though additional research is needed. Investigators report a range of benefits, potential risks, and feasibility concerns. Standardized, evidence-based approaches are needed for RoR. Summary: For HIV research, we recommend a default position of offering RoR for cognitive and psychological outcomes. Investigators should justify decisions not to return results after assessing the potential value and feasibility of RoR. The Covid-19 pandemic has compounded the challenge of HIV/AIDS elimination, creating difficulties in accessing HIV care services such as early testing and treatment. This paper characterized the global online interest in HIV care services-related search terms before and during the pandemic. Global online search interest for HIV was measured using the Google Trends™ database. Spearman's rank-order correlation correlated country-specific characteristics and HIV prevalence data with the search volume index (SVI). We found a significant decrease in the global online search interest for HIV/AIDS care services-related search terms during the Covid-19 pandemic. The top countries with the highest online interest for "HIV/AIDS" search terms were Zambia, Eswatini, Malawi, Lesotho, and Zimbabwe. In addition, search volume indices for HIV correlated positively with HIV prevalence and negatively with GDP, GDP per capita, and the number of physicians. This result highlights that resource-poor countries with a high prevalence of HIV have a high online interest in HIV/AIDS. Therefore, there is a need to improve internet access, the quality of HIV-related health information, and online health literacy to improve health-seeking behavior, especially in areas with a high disease burden. Overall, our study shows that the infodemiologic approach through Google Trends™ can be used to assess the online interest of the public toward HIV infection and related healthcare services. This paper explores the perception of African, Caribbean, and Black (ACB) religious leaders on HIV vulnerability and their role in HIV prevention among ACB communities in Windsor, Ontario. We conducted one semi-structured focus group discussion with nine Black religious leaders, most of whose congregants are members of the ACB community. Most religious leaders in the focus group had a negative perception of the transmission of HIV, but they acknowledged their own important role in HIV prevention strategies. This role is collaborative in nature, from the stage of designing HIV prevention strategies to implementing prevention messages. The religious leaders noted, however, that some challenges, such as church doctrine and congregational culture, are likely to impede their HIV prevention efforts. [ABSTRACT FROM AUTHOR]

There are numerous contagious diseases caused by pathogenic microorganisms, including bacteria, viruses, fungi, and parasites, that have the propensity to culminate in fatal consequences. A communicable disease is an illness caused by a contagion agent or its toxins and spread directly or indirectly to a susceptible animal or human host by an infected person, animal, vector, or immaterial environment. Human immunodeficiency virus (HIV) infection, hepatitis A, B, and C, and measles are all examples of communicable diseases. Acquired immunodeficiency syndrome (AIDS) is a communicable disease caused by HIV infection that has become the most severe issue facing humanity. The research work in this paper is to numerically explore a mathematical model and demonstrate the dynamics of HIV/AIDS disease transmission using a continuous Galerkin–Petrov time discretization of a higher-order scheme, specifically the cGP(2)-scheme. Depict a graphical and tabular comparison between the outcomes of the mentioned scheme and those obtained through other classical schemes that exist in the literature. Further, a comparison is performed relative to the well-known fourth-order Ruge–Kutta (RK4) method with different step sizes. By contrast, the suggested approach provided more accurate results with a larger step size than RK4 with a smaller step size. After validation and confirmation of the suggested scheme and code, we implement the method to the extended model by introducing a treatment rate and show the impact of various non-linear source terms for the generation of new cells. We also determined the basic reproduction number and use the Routh-Hurwitz criterion to assess the stability of disease-free and unique endemic equilibrium states of the HIV model. Background: The use of patient-reported outcome measures (PROM) and patient-reported experience measures (PREM) provide health providers with valuable feedback on how to improve clinical care and patient outcomes. This paper describes a qualitative study that was conducted to learn about factors influencing the well-being of people living with HIV (PLHIV) in Finland. The findings will be used to develop themes for HIV-specific PROM and PREM questions. Methods: PROMs and PREMs were developed by the Finnish Institute for Health (THL) as a part of a project to develop a national quality-of-care registry for HIV. The study aimed to identify issues and concerns among people living with HIV (PLHIV) that influence their well-being (PROMs) and their experiences in the healthcare system (PREMs). The data were collected through face-to-face in-depth interviews and focus group discussions based on open-ended and semi-structured questions. The data were analyzed using thematic analysis. Results: The assessment identified the following PROMs of concern: psychological well-being, concerns about stigma, physical health, social well-being, sexual well-being, medication uptake, managing other medications with antiretrovirals (ARVs), and growing old. The assessment identified the following PREMs: helping patients understand their own health status, proving an opportunity for patients to discuss physical health, psychological and sexual well-being, supporting the uptake of ARVs, assisting patients with medication use, showing compassion towards patients, and empowering patients against stigma. This paper examines how antiretroviral therapy (ART) use and fears towards the onward transmission of HIV have changed among people living with HIV (PLHIV) in Australia between 1997 and 2018. Participants were recruited as part of the HIV Futures study, a large cross-sectional survey of PLHIV in Australia, in 1997, 2003, 2012 and 2018 (total n = 3889). ART use, viral load detectability, and fear of onward HIV transmission were compared between years. Predictors of onward transmission fear were assessed among the 2018 subsample. While ART use within our sample decreased between 1997 and 2003, it subsequently increased to 97% in 2018. Self-reported viral load undetectability steadily increased over time, up to 88% in 2018. Notably, fewer PLHIV reported being fearful of transmitting HIV in 2018 compared to all other years. Being unfamiliar with the undetectable = untransmissible health movement, and having a detectable or uncertain viral load at last test, were significant predictors of being fearful of onward HIV transmission. Beyond the immediate medical considerations of HIV treatment, these results suggest that the undetectable = untransmissible movement may play a critical role in attenuating burdens experienced by PLHIV in Australia and that such messaging, in tandem with early and consistent ART use, should remain a salient feature of heath messaging among this population. One of the most important steps in HIV prevention is the provision of education to raise awareness to ensure individuals have a sufficient understanding of HIV and AIDS and encourage them to apply that knowledge. The study is based on an earlier descriptive cross-sectional study that sought a link between fishers' mobility and their risk of HIV exposure in the Elmina fishing community in Ghana. The current paper focuses on initiatives taken within the study area to inform fishers about HIV and AIDS. Additionally, it analyzes the successes and challenges of these programs and offers suggestions for improvement. Public and community durbars, town hall meetings, radio and television transmission, film creation, and the distribution of IE&C materials are the main strategies utilized to carry out HIV and AIDS education. The results suggest that few fishers in the study area take part in programs, whiles projects on HIV and AIDS cannot be implemented consistently and effectively due to a lack of funding and delays in the delivery of government funds. To enable organizations whose mandates, relate to HIV and AIDS interventions to carry out the programs timely and fully, it is advised that government enhances its financing and ensures the timely and full release of program funds. Relationship agreements are important for HIV prevention among gay and bisexual men (GBM) in relationships, with research earlier in the HIV epidemic often finding that agreements specified monogamy or condom use with casual partners. There is evidence that HIV pre-exposure prophylaxis (PrEP) has shifted sexual practices among some men in relationships, such as allowing condomless sex with casual partners, but there has been little attention paid to relationship agreements among GBM who use PrEP. In this paper, we analyzed national, Australian, cross-sectional data from an online survey completed by non-HIV-positive GBM in 2021 (N = 1,185). Using logistic regression, we identified demographic characteristics, sexual practices and the types of relationship agreement that were associated with PrEP use among GBM in relationships. Using Pearson's chi-squared tests, we explored whether PrEP users in relationships reported similar sexual practices to PrEP users not in relationships. PrEP use among GBM in relationships was independently associated with older age, identifying as gay, being in a non-monogamous relationship, having a spoken (explicit) relationship agreement, having a primary HIV-negative partner taking PrEP or a primary partner living with HIV, reporting recent condomless casual sex, reporting an STI diagnosis in the past year, and knowing at least one other PrEP user. We found that PrEP users in relationships had similar sexual practices to PrEP users not in relationships. GBM in relationships who have casual sex and who meet PrEP suitability criteria may be good candidates for PrEP. Adherence to and sustainment of HIV pre-exposure prophylaxis (PrEP) are critical to its effectiveness. Adherence self-efficacy, i.e., confidence in one's ability to adhere to a particular medication, is a key psychological determinant of health behavior that strongly predicts HIV treatment adherence but has been understudied in PrEP research. This paper describes the psychometric evaluation and validation of the PrEP Adherence Self-Efficacy Scale (PrEP-ASES), adapted from the previously validated HIV Treatment Adherence Self-Efficacy Scale (HIV-ASES). Data are drawn from two studies conducted at a community health center, one focused on gay and bisexual cisgender men and the other on transgender women. Factor analyses support a one-factor score (eigenvalue = 6.78) that explained 75.3% of the variance, with good test–retest reliability (rs > 0.40). In both studies, higher PrEP-ASES scores were associated with PrEP uptake, adherence, and sustainment. Mass incarceration has shaped the environment of HIV transmission, yet there is limited research on the conditions that enhance or mitigate access to health and social resources for people living with HIV (PLWH) with criminal justice (CJ) involvement. This paper aims to explore structural vulnerability, social safety net services, and criminal justice involvement among PLWH. Among a community-based sample of 336 PLWH in Baltimore, Maryland, we compared measures of structural vulnerability and access to safety net services on the outcome of either recent involvement or no involvement in the CJ system. CJ involved PLWH were more likely to be structurally vulnerable and more likely to be enrolled in temporary social services and apply for permanent income. Enrollment in longer-term social services did not differ by CJ involvement, and neither did application for housing assistance. The results shed light on the socio-economic context of CJ involvement among PLWH. As the HIV epidemic persists, focusing research on the impediments to structural wellbeing, such as the drivers of criminal justice involvement, are important. [ABSTRACT FROM AUTHOR]

Background: There is some evidence to suggest that the human immunodeficiency virus (HIV) prevalence may be lower in Muslim-majority countries and among Muslims in general. The low prevalence is usually accredited to Islam’s influence on the behaviours that affect transmission of HIV. Reports of HIV epidemics, predominately among injection drug users (IDUs), are emerging in Muslim-majority countries, forcing the Muslim world to respond to them. Objective: This study is a scoping review of the literature that addresses (a) the messages communicated regarding HIV from an Islamic perspective, (b) the approaches Muslims adopt to comprehend and treat people living with HIV, and (c) roles of Islam and religious leaders. This review scopes the available literature and identifies the nature and extent of research conducted to date. Methods: A comprehensive search was performed using PubMed, Medline, Web of Science, ScienceDirect, Scopus, and Academic Search Ultimate. Primary research that focused on the Islamic perspective and discourse of HIV and AIDS, were considered. In total 22 studies dating from 2002 to 2017 studies were included. Findings. Five themes emerged (1) Western Import, (2) God’s Wrath, (3) God’s Test, (4) Tolerance and Mercy, and (5) Religious Silence. Conclusion: HIV has indiscriminately affected communities globally and the Muslim community has not been an exception. More diverse research is required as well as Muslim- majority countries and communities must develop effective HIV awareness and prevention campaigns that are rooted in Islamic teachings and involve religious leaders. Heterogeneity in susceptibility and infectivity is a central issue in epidemiology. Although the latter has received some attention recently, the former is often neglected in modeling of epidemic systems. Moreover, very few studies consider both of these heterogeneities. This paper is concerned with the characterization of epidemic models with differential susceptibility and differential infectivity under a general setup. Specifically, we investigate the global asymptotic behavior of equilibria of these systems when the network configuration of the Susceptible-Infectious interactions is strongly connected. These results prove two conjectures by Bonzi et al. (J Math Biol 62:39–64, 2011) and Hyman and Li (Math Biosci Eng 3:89–100, 2006). Moreover, we consider the scenario in which the strong connectivity hypothesis is dropped. In this case, the model exhibits a wider range of dynamical behavior, including the rise of boundary and interior equilibria, all based on the topology of network connectivity. Photovoice, a community-based participatory research method, employs images and words to convey participants' needs, concerns, and desires. It proves particularly valuable in researching marginalized communities who face elevated health risks, disease transmission, and social and health disparities. This paper seeks to investigate the ethical considerations inherent in photovoice research projects. We conducted an extensive literature review spanning four databases to identify pertinent photovoice studies. Ethical issues from the selected articles were identified, categorized, and summarized. Our analysis of twenty-five photovoice studies uncovered various ethical concerns, which had been grouped into informed consent, participant safety and disclosure, privacy and confidentiality, misrepresentation, power dynamics, and compensation. In essence, our findings underscore the importance of addressing these ethical concerns to uphold the rights and autonomy of participants, even as photovoice research strives for authenticity, inclusivity, and empowerment. In recent years, the machine learning research community has benefited tremendously from the availability of openly accessible benchmark datasets. Clinical data are usually not openly available due to their confidential nature. This has hampered the development of reproducible and generalisable machine learning applications in health care. Here we introduce the Health Gym - a growing collection of highly realistic synthetic medical datasets that can be freely accessed to prototype, evaluate, and compare machine learning algorithms, with a specific focus on reinforcement learning. The three synthetic datasets described in this paper present patient cohorts with acute hypotension and sepsis in the intensive care unit, and people with human immunodeficiency virus (HIV) receiving antiretroviral therapy. The datasets were created using a novel generative adversarial network (GAN). Acquired immunodeficiency syndrome (AIDS) is still one of the main health problems worldwide. It is therefore essential to keep making progress in improving the prognosis and quality of life of affected patients. One way to advance along this pathway is to uncover connections between other disorders associated with HIV/AIDS—so that they can be anticipated and possibly mitigated. We propose to achieve this by using Association Rules (ARs). They allow us to represent the dependencies between a number of diseases and other specific diseases. However, classical techniques systematically generate every AR meeting some minimal conditions on data frequency, hence generating a vast amount of uninteresting ARs, which need to be filtered out. The lack of manually annotated ARs has favored unsupervised filtering, even though they produce limited results. In this paper, we propose a semi-supervised system, able to identify relevant ARs among HIV-related diseases with a minimal amount of annotated training data. Our system has been able to extract a good number of relationships between HIV-related diseases that have been previously detected in the literature but are scattered and are often little known. This study compared the effectiveness of different intervention sequences in reducing tension among alcohol-consuming men living with HIV in India.This secondary data analysis paper utilized data from a randomized trial study titled “Alcohol and Antiretroviral Therapy (ART) Adherence: Assessment, Intervention, and Modeling in India.” The multilevel interventions, including individual counseling (IC), group intervention (GI), and collective advocacy (CA), were conducted sequentially over three cycles at three ART centers. Additionally, another ART center, serving as a pilot site, received all three interventions simultaneously in the final cycle. Participants completed surveys assessing demographic characteristics, tension, and other variables including HIV stigma, a 4-day ART adherence, HIV symptoms, and depressive symptoms at four time points: baseline, 9 months, 18 months, and 24 months. General linear mixed models were employed to assess the intervention effects on tension.Out of 940 participants, 666 reported experiencing tension, including 54% reporting high tension. At site 1, the GI-CA-IC sequence resulted in a slope of −0.06, indicating that this sequence reduced tension from T1 to T4 compared to the control group (p < 0.01). Additionally, the pilot site where the intervention package was conducted simultaneously had a slope of −0.06, indicating that the intervention package also reduced tension compared to the control group (p < 0.01). The threshold regression model is an effective alternative to the Cox proportional hazards regression model when the proportional hazards assumption is not met. This paper considers variable selection for threshold regression. This model has separate regression functions for the initial health status and the speed of degradation in health. This flexibility is an important advantage when considering relevant risk factors for a complex time-to-event model where one needs to decide which variables should be included in the regression function for the initial health status, in the function for the speed of degradation in health, or in both functions. In this paper, we extend the broken adaptive ridge (BAR) method, originally designed for variable selection for one regression function, to simultaneous variable selection for both regression functions needed in the threshold regression model. We establish variable selection consistency of the proposed method and asymptotic normality of the estimator of non-zero regression coefficients. Simulation results show that our method outperformed threshold regression without variable selection and variable selection based on the Akaike information criterion. We apply the proposed method to data from an HIV drug adherence study in which electronic monitoring of drug intake is used to identify risk factors for non-adherence. [ABSTRACT FROM AUTHOR]

Purpose of Review: Cognitive impairment leading to disability is increasingly seen in people living with human immunodeficiency virus (PLWH). Rehabilitation can alleviate the effects of cognitive impairment upon function. The aim of this paper is to discuss the strategies that have been used in cognitive and neurologic rehabilitation in PLWH. Recent Findings: Studies examining pharmacological and non-pharmacological strategies were analysed. Medical management of HIV and co-morbidities should be optimised. Non-pharmacological strategies, including nerve stimulation techniques, exercise-based interventions, and paper and computer-based cognitive rehabilitation, have some evidence supporting their use in PLWH either as stand-alone interventions or as part of a multidisciplinary approach. Summary: Both pharmacological and non-pharmacological rehabilitation strategies have been used with PLWH. Comprehensive sexuality education is seen as a key intervention to address the high HIV-incidence rate and low knowledge among young people in South Africa, yet this lacks inclusion of learners with disabilities. At the same time, educators of learners with disabilities experience cultural barriers and lack skills and knowledge on how to deliver sexuality education in accessible formats. In response, the Breaking the Silence approach, a curriculum implementation and disability-accommodating approach, was developed and tested. This paper presents findings from a formative evaluation using in-depth interviews with educators from eight “schools for learners with special educational needs”. The paper focuses on their experience with implementing the approach. The analysis used conventional content analysis and NVIVO software. Educators conceptualized sexuality education within a risk-protection discourse. They revealed that the training and tools enabled them to provide sexuality education in accessible formats, tackle difficult topics such as sexual orientation and masturbation, and improved awareness and assertiveness within their learners. Educators identified the need for further development including a whole school approach, adjustments to differentiating developmental stages and addressing gender-specific topics. Through its application within the South African mainstream Life Orientation curriculum, this approach lends itself for further development and testing through implementing the approach within a whole school setting and ongoing support that will not only drive change in educators’ skills and knowledge but also drive the needed shift in cultural norms and values. This paper advances research on racism and health by presenting a conceptual model that delineates pathways linking policing practices to HIV vulnerability among Black men who have sex with men in the urban USA. Pathways include perceived discrimination based on race, sexuality and gender performance, mental health, and condom-carrying behaviors. The model, intended to stimulate future empirical work, is based on a review of the literature and on ethnographic data collected in 2014 in New York City. This mini review focuses on the diagnosis and treatment of virus diseases using Crisper-Cas technology. The present paper describes various strategies involved in diagnosing diseases using Crispr-Cas-based assays. Additionally, CRISPR-Cas systems offer great potential as new therapeutic tools for treating viral infections including HIV, Influenza, and SARS-CoV-2. There are several major challenges to be overcome before this technology can be applied routinely in clinical settings, such as finding a suitable delivery tool, toxicity, and immunogenicity, as well as off-target effects. Purpose of Review: Patients diagnosed with HIV can now survive well into their old age. Aging with HIV is not only associated with comorbid medical illnesses but also with neuropsychiatric conditions that can range from cognitive changes to severe behavioral manifestations. This paper reviews mood, anxiety, and cognitive changes in older patients with HIV, as well as some of the treatment challenges in this population. Recent Findings: Most recent findings show that untreated HIV illness over a long period of time may further worsen both preexisting neuropsychiatric illness and may cause new onset behavioral and cognitive symptoms. HIV induces immune phenotypic changes that have been compared to accelerated aging Low CD 4 counts and high viral counts are indicative of poor prognosis. Summary: Evaluation for potential HIV infections may be overlooked in older adults and require screening. Older adults experience accelerated CD4 cell loss. Older adults endorsing new onset mood or cognitive changes must be screened for HIV infection. The US South is disproportionately impacted by HIV. Social, cultural, economic, and political characteristics of the South shape access to mental health services leaving adverse impacts on health and wellness outcomes among People Living with HIV. The aim of this paper was to: (a) identify meso factors (at individual, organizational and community-level manifestations) which impact mental health services among People living with HIV in the South of those factors and (b) pose community-articulated recommendation and strategies. Through qualitative interviews with People Living with HIV and service providers, this study found that the meso factors of restricted funding and compounding stigma shaped mental health services in the South. Given the disproportionate rate of HIV, lack of mental health care, and landscape of socio-political factors unique to the region, attention to intervenable meso factors and community-based strategies are needed to enhance mental health services and respond to the HIV epidemic in the US South. The population in the eastern Democratic Republic of Congo (DRC) has been terrorized by widespread sexual violence for decades. Local armed groups, including the government army, continue to systematically rape civilians for strategic reasons. This paper argues that the discussed genocidal functions of rape as a weapon of war in the DRC can be neutralized or 'de-functionalized' by providing victims and targeted communities with safe abortion, anti-retroviral medication, reconstructive surgery and awareness programmes, thereby both responding to short-term humanitarian needs of victims and the need for long-term solutions to preventing further sexual violence. Mathematical models have shed light on the dynamics of HIV- 1 infection in vivo. In this paper, we generalize continuous mathematical models of drug therapy for HIV-1 by Perelson et al. (Science 271:1582–1586, 1996) and Perelson and Nelson (SIAM Rev 41:3–44, 1999) on time scales, i.e., a nonempty closed subset of real numbers in order to derive new discrete models that predict the total concentration of plasma virus as a function of time. One of our main goals is to compare discrete mathematical models with the continuous model in Perelson et al. (1996) where HIV infected patients were given protease inhibitors and sampled frequently thereafter. For the comparison, we use experimental data collected in Perelson et al. (1996) and estimate the parameters such as the virion clearance rate and the rate of loss of infected cells by fitting the total concentration of plasma virus to this data set. Our results show that discrete systems describe the best fit. In the previous models of this study, the efficacy of protease inhibitor is assumed to be perfect. Motivated by Perelson and Nelson (1999), we end the paper with a mathematical model of imperfect protease inhibitor and reverse transcriptase (RT) inhibitor combination therapy of HIV-1 infection on time scales with its stability analysis. This paper examines the recently formed Movement for Change and Social Justice (MCSJ) in the township of Gugulethu, in Cape Town, South Africa. MCSJ is a health and social justice movement that has an unusual trajectory—guided by the political principles and strategies of South Africa AIDS activism, inspired but also frustrated by the contemporary progressive public health activism, and catalysed by a university research partnership at the University of Cape Town (UCT) that focused on innovative ways of using health information to spur community and health system action to respond to issues of men, masculinity and HIV. This paper synthesises findings from participant observation, qualitative process evaluations of MCSJ's work, and reflections from the UCT-based research team to develop a number of lessons about the challenges and opportunities in efforts to foster a healthy public for men and HIV. MCSJ's approach builds on current modes and models of public health thinking while also pushing for more responsive, more inclusive, more sustained, and more locally rooted forms of practice. The paper identifies a range of enabling conditions for MCSJ's work, describes the often delicate balancing act MCSJ has to manage in its work, and outlines some of the key strategies of this community–university partnerships that guided efforts to develop a healthy public for men and HIV. Lessons learned from this case study will be helpful for other efforts to promote and sustain engaged and impactful university–community collaborations to support the emergence of healthy publics. Purpose of Review: The purpose of this scoping review was to summarize literature regarding the use of user-generated digital data collected for non-epidemiological purposes in human immunodeficiency virus (HIV) research. Recent Findings: Thirty-nine papers were included in the final review. Four types of digital data were used: social media data, web search queries, mobile phone data, and data from global positioning system (GPS) devices. With these data, four HIV epidemiological objectives were pursued, including disease surveillance, behavioral surveillance, assessment of public attention to HIV, and characterization of risk contexts. Approximately one-third used machine learning for classification, prediction, or topic modeling. Less than a quarter discussed the ethics of using user-generated data for epidemiological purposes. Summary: User-generated digital data can be used to monitor, predict, and contextualize HIV risk and can help disrupt trajectories of risk closer to onset. However, more attention needs to be paid to digital ethics and the direction of the field in a post-Application Programming Interface (API) world. [ABSTRACT FROM AUTHOR]

Aim: Suicide is one of the leading causes of death globally, inherently linked with mental health conditions. Existing studies have also found a significant association between suicide and chronic health conditions such as human immunodeficiency virus (HIV). Although there have been enormous studies on these global health issues, knowledge about their co-existence is elusive. This paper examines the link between HIV and suicide in 186 countries using geospatial techniques and spatial statistics. Subject and methods: We used country-level suicide and HIV data obtained from the United Nations Development Program database. The local bivariate spatial association technique was used to determine the covariation of HIV and suicide, while autoregressive models were used to estimate the unexplained country-level variables in the model. Due to evidence of spatial dependencies, we applied multiscale geographically weighted regression (MGWR) to examine the spatial variation in the predictor variables. Results: Analysis indicated that HIV significantly covaried with suicide in regions of Africa and Europe. Our result suggests that HIV is a significant driver of suicide among females in sub-Saharan Africa while it drives suicide among males in many regions including the country of South Africa, Lesotho, Swaziland, Oceania, and Southeast Asia. MGWR indicated that HIV was negatively associated with suicide in the Americas and European countries, but positively associated with suicide in Africa, the Middle East, and Oceania. Conclusion: There is a double burden of HIV and suicide in sub-Saharan Africa. Therefore, investment in both mental healthcare and access to antiretroviral drugs is essential in reducing suicide at both global and local levels. [ABSTRACT FROM AUTHOR]
