BACKGROUND: Montelukast is a selective leukotriene D-4 receptor antagonist, which specifically and reversibly inhibits cysteinyl leukotriene-1 receptor. The aim of this study was to investigate the protective effect of Montelukast on skeletal muscle reperfusion injury created as acute ischemia-reperfusion (IR) injury in Wistar-albino rats. METHODS: The study comprised 16 male Wistar-albino rats. The rats were randomly separated into two groups as control (IR) and treatment (IR+Montelukast). Ischemia was obtained using a femoral artery clamp. After reperfusion following a 2-hour ischemia, muscle samples were taken for biochemical and histopathological analyses. Metal üzeri metalkalça protezi uygulamalarında çok nadir ve sıra dışı bir komplikasyon olanprotez baş yuvasının ucunda oluşan yorgunluk kırığını sunmaktayız. Başın boyunyuvasının derin ucundaki katastrofik aşınması ve burada oluşan para benzeriserbest metal parça metallozis, osteoliz ve yetmezliğe neden olmuştur. Revizyonameliyatı sonuçları tatminkar olmuş ve hastalar sorunsuzca günlükaktivitelerine dönmüşlerdir. Metal üzeri metal eklemleşmelerde başın aşınmasıiyi bilinen bir sonuçtur ve instabilite, metallozis, ağrı ve mekaniksemptomlarla ortaya çıkabilir. The outcomes of the revision operations were satisfactory and thepatients returned to their daily activity without discomfort in their hip.Headwear is a well-known result of the metal on metal articulation and may presentwith instability, metallosis, pain and mechanical symptoms. These incidentsmight be important to investigate for tribological performance of hip implantsin biotribology science. We believe -3 sized metal head usage might be thereason of this catastrophic complication their usage should be avoided in metalon metal artroplasties.

Amaç: Manyetik rezonans görüntüleme dizde menisküs ve bağ yaralanmalarının tanısında sıklıkla kullanılmaktadır. Bu çalışmadaki amacımız dizdeki bu tip patolojilerin artroskopi sonuçları ile manyetik rezonans görüntüleme verilerinin retrospektif olarak karşılaştırılmasıdır. Yöntem: Travma sonrası klinik olarak menisküs ve/veya bağ yaralanması düşünülen ve manyetik rezonans görüntüleme tetkiki yapılmış 62 hasta çalışmaya dahil edildi. Hastaların ortalama yaşı 36.8 (dağılım 18–69 yaş) idi. Hastaların 37’si (%59.7) erkek iken 25’si (%40.3) kadın idi. 35 (%56.5) sol, 27 (%43.5) sağ dizde yaralanma mevcuttu. Klinik değerlendirmede ve manyetik rezonans görüntülemede patoloji düşünülüp diz artroskopisi yapılmış hastaların manyetik rezonans görüntüleme ve artroskopi sonuçları değerlendirildi. Bulgular: Artroskopi sonuçlarına göre 35 (%56.4) hastada izole medial menisküs yırtığı, 11 (%17.7) hastada izole lateral menisküs yırtığı, 5 (%8) hastada izole ön çapraz bağ yaralanması, 2 (%3.2) hastada medial menisküs yırtığı ve ön çapraz bağ yaralanması, 2 (%3.2) hastada lateral menisküs yırtığı ve ön çapraz bağ yaralanması, 5 (%8) hastada medial menisküs ve lateral menisküs yırtığı, 2 (%3.2) hastada medial menisküs, ön çapraz bağ ve arka çapraz bağ yırtığı tespit edildi. Yapılan değerlendirmede manyetik rezonans görüntülemede medial menisküs için sensitivite %95,5, spesifite %76,4, pozitif prediktif değeri (PPD) %91.4, negatif prediktif değeri (NPD) %86.6, doğruluk oranı %90,3; lateral menisküs için sensitivite %72.2, spesifite %93.1, PPD %81.2, NPD %89,1, doğruluk oranı %87; ön çapraz bağ için sensitivite %90.9, spesifite %88.2, PPD %62.5, NPD %97.8, doğruluk oranı %88.7; arka çapraz bağ için sensitivite %100, spesifite %98.3, PPD %66.6, NPD %100, doğruluk oranı %98.3 olarak tespit edilmiştir. Sonuç: Manyetik rezonans görüntüleme dizde menisküs ve bağ yaralanmalarının tanısında güvenle kullanılabilecek bir tetkikdir. Purpose: Magnetic resonance imaging is frequently used in the diagnosis of the meniscal and ligamentous injuries. The aim of this study is to compare the results of the knee arthroscopy and magnetic resonance imaging findings retrospectively. Methods: Of the sixty-two patients 37 (59.7%) were male and 25 (40.3%) were female. The average age was 36.8 years (range 18-69 years). Patients who had been clinically diagnosed with meniscal and/or ligamentous injuries and subsequently undergone further magnetic resonance imaging examination were included to the study. The left knee was involved in 35 (56.5%) and the right knee in 27 (43.5%) cases. Patients had gone to knee arthroscopy according to clinical examination and magnetic resonance imaging findings, and the results were evaluated. Results: 35 (56.4%) of the 62 patients had isolated medial meniscal tear, 11 (17.7%) isolated lateral meniscal tear and 5 (8%) had isolated complete anterior cruciate ligament injury. 2 (3.2%) had medial meniscal tear and anterior cruciate ligament complete injury, 2 (3.2%) had lateral meniscal tear and complete anterior cruciate ligament injury and 5 (8%) had medial and lateral meniscal tear. 2 (3.2%) had medial meniscal tear, anterior and posterior cruciate ligament complete injuries. Magnetic resonance imaging evaluation of the medial meniscus, lateral meniscus, anterior cruciate ligament and posterior cruciate ligament revealed sensitivity 95.5%, 72.2%, 90.9%, 100%, specificity 76.4%, 93.1%, 88.2%, 98.3%, positive predictive value 91.4%, 81.2%, 62.5%, 66.6%, negative predictive value 86.6%, 89.1%, 97.8%, 100%, and accuracy 90.3%, 87.0%, 88.7%, 98.3% respectively. Conclusion: Magnetic resonance imaging is a safe and appropriate evaluation in the diagnosis of the meniscal and ligament injuries of the knee.

Amaç: Bu çalışmada lökotrien reseptör antagonisti montelukastın kırık iyileşmesi üzerine olan etkileri incelendi ve sıçan tibia kırık modelinde montelukastla artmış kırık iyileşmesi görülebileceği hipotezi araştırıldı. Gereç ve yöntemler: Altmış erişkin (6 aylık) dişi Wistar albino cinsi sıçan (ortalama ağırlıkları 220 g, dağılım 210-270 g) rastgele şekilde iki gruba ayrıldı: montelukast grubu (n=30) ve kontrol grubu (n=30). Kapalı tibia kırığı oluşturuldu ve intramedüller Kirschner teliyle fikse edildi. Kırıktan üç ve altı hafta sonra sıçanlar sakrifiye edildi. Radyolojik ve histolojik değerlendirmeler yapıldı ve kemik mineral yoğunlukları ölçüldü. Bulgular: Çalışma sırasında montelukast grubunda üç, kontrol grubunda ise yalnızca bir sıçan öldü. Gruplar arasında hayvanların başlangıç, 3. hafta ve 6. haftadaki ağırlıkları açısından istatistiksel bir fark bulunmadı (p>0.05). Kemik mineral yoğunluğu kontrol ve çalışma grubunda sırasıyla 3. haftada 0.13±0.009 g/cm2 ve 0.13±0.01 g/cm2, 6. haftada ise 0.16±0.02 g/cm2 v e 0 .13±0.01 g /cm2 olarak bulundu. Histopatolojik skor kontrol ve çalışma gruplarında sırasıyla 3. haftada 3.42±0.6 ve 3.0±0.0, 6. haftada ise 3.5±0.5 ve 3.4±0.8 olarak bulundu. Radyolojik skor kontrol ve çalışma gruplarında sırasıyla 3. haftada 1.19±0.6 ve 1.0±0.6, 6. haftada ise 3.0±0.8 ve 2.9±0.9 olarak bulundu. Değerlendirilen hiçbir parametre açısından her iki zaman aralığında iki grup arasında anlamlı fark saptanmadı (p>0.05). Sonuç: Bizim çalışmamızda lökotrien reseptör antagonistinin 3. ve 6. haftalarda kırık iyileşmesine olumlu etkisi olmadığı görüldü. Objectives: In this study we evaluated the effects of montelukast, a leukotriene-receptor antagonist, on fracture healing, and investigated the hypothesis that enhanced fracture healing would be observed with montelukast in a rat tibia fracture model. Materials and methods: Sixty adult (6 months old) female Wistar albino rats (mean weight 220 g, range 210-270 g) were randomly divided into two groups: a montelukast group (n=30) and a control group (n=30). Closed tibia fractures were created and fixed by intramedullary Kirschner wire. The rats were sacrificed three and six weeks after the fractures. Radiological and histological evaluations were performed, and bone mineral density was measured. Results: Three rats died in the montelukast group, whereas only one died in the control group during the study. Initial weight and weight gain at the 3rd a nd 6 th weeks were not significantly different between the groups (p>0.05). Bone mineral densities in the control and study groups were 0.13±0.009 gr/cm2, and 0.13±0.01 gr/cm2 a t w eek t hree and 0.16±0.02 gr/cm2, and 0.13±0.01 gr/cm2 at week six, respectively. Histopathological scores in the control and study groups were 3.42±0.6, and 3.0±0.0 at week three and 3.5±0.5, and 3.4±0.8 at week six, respectively. Radiological scores in the control and study groups were 1.19±0.6, and 1.0±0.6 at week three and 3.0±0.8, and 2.9±0.9 at week six, respectively. There were no significant differences between the two groups in any parameters evaluated at either time interval (p>0.05). Conclusion: Our study failed to show a possible positive effect of leukotriene receptor inhibition on fracture healing at the 3rd and 6th postoperative weeks.

Amaç: Bu çalışmada önkol ve eldeki geniş ve kompleks yumuşak doku eksikliklerinin serbest anterolateral uyluk (ALU) flebi ile onarım sonuçları değerlendirildi. Hastalar ve yöntemler: Ön kolda ve elde kompleks ve geniş doku kaybı olan 13 hastaya (11 erkek, 2 kadın; ort. yaş 32.5 yıl; dağılım 18-55 yıl) serbest ALU flep ile onarım yapıldı. Hastaların dokuzunda doku kaybının nedeni ezilme yaralanması, ikisinde tümör çıkarılması, birinde elektrik yanığı ve birinde unstable skar dokusu çıkarılmasıydı. Beş hastada arter defektini onarmak için flow through ALU flep, üç hastada tendon defektlerini onarmak için vaskülarize fasya ALU flebi beş hastada ise nörosensöriyal ALU flebi kullanıldı. Hastalar 44±14.6 ay boyunca takip edildi. Fonksiyonel sonuçlar ameliyat sonrası birinci yılda Chen sınıflaması kullanılarak değerlendirildi. Bulgular: On bir hastada flepler tam olarak yaşadı. Kalan iki hastada ise flep kenarlarında yer yer minimal nekroz oluştu. Nekroze olan bu alanlar debride edildikten sonra pansumanla iyileşti. On bir hastada fonksiyonel sonuçlar tatminkarken (6 hasta Chen I ve 5 hasta Chen II), iki hastada kötüydü (Chen III). Sonuç: Anterolateral uyluk flebi, çeşitli kompleks ön kol ve el defektlerinin kapatılmasında mükemmel bir seçenektir. Tendon, sinir ve damarsal yapıların üzerinin kaliteli bir dokuyla kapatılmasının yanında bu dokuların eksikliklerinin onarımını da sağlar. Objectives: The aim of this study was to evaluate the results of free anterolateral thigh (ALT) flap reconstruction for large and complex soft tissue defects of forearm and hand. Patients and methods: Thirteen patients (11 males, 2 females; mean age 32.5 years; range 18 to 55 years) underwent free ALT flap reconstruction for forearm and hand defects. The etiology of defects was crush injury in nine patients, tumor excision in two patients, electric burn in one patient, and unstable scar excision in one patient. In five patients a flow through ALT flap was used to reconstruct vascular defects. In tree patients a vascularised fascial ALT flap was used to reconstruct the tendon defects and in five patients neurosensory ALT flap was used. Patients were followed for 44±14.6 months. Chen classification was used to evaluate functional outcomes at the end of the postoperative first year. Results: All flaps survived completely in 11 patients. In the other two patients marginal necrosis was observed. These areas with necrosis healed after debridement and daily dressings. The functional results were satisfactory in 11 patients according to Chen classification (6 patients Chen I and 5 patients Chen II) and poor in two patients (Chen III). Conclusion: The anterolateral thigh flap represents an excellent option for covering various complex defects in the forearm and hand. The flap provides a thin, pliable, vascularized tissue for covering the exposed tendons, nerves, bones, and reconstruct deficiencies.

Amaç: Bu çalışmadaki amacımız konservatif tedaviye yanıt vermeyen hastalarda açık anterior akromioplasti cerrahisi sonuçlarının retrospektif olarak değerlendirilmesidir. Gereç ve Yöntemler: Subakromial sıkışma sendromu tanısı konan 40 hastaya açık anterior akromioplasti yapıldı. Hastaların ortalama yaşı 50 (dağılım 35-75 yaş) idi. Hastaların 25'inde (%62,5) dominant omuzda şikayet mevcuttu. Hastaların değerlendirilmesinde omuz hareket açıklığı, American Shoulder and Elbow Surgeons (ASES) skoru ve Vizüel Analog Skala'nın (VAS) yer aldığı kişisel değerlendirme formu kullanıldı. Ortalama takip süresi 20,3 ay (dağılım 12-42 ay) idi. Bulgular: Hastaların ameliyat öncesi değerlendirilmesinde ortalama ASES 43,7+/-14 olarak tespit edildi. However additional radiation exposure during reduction of the fracture, application of the plate to the bone and screw fixation is one of the disadvantages of this technique. We describe a technical trick for screw fixation in MIPPO with locking compression plates which decreases the duration of fluoroscopy use during the operation.

Osteo kondrom en sık görülen benign kemik tümörüdür(%20 –50). Elde nadir görülmektedir. Gebelik döneminde hormonal değişikliklere bağlı vücutta ödem görülebilmektedir. Tenosinovit ve tuzak nöropatilerin görülme oranı artmaktadır. Bizim sunduğumuz olgumuzda, 24 yaşında bayan hastanın sağ el 3. parmağında osteokondrom mevcuttu. Hastanın şikayeti yoktu. Gebelik ile şikayetleri başladı. Geb elik sonrası 6 ay medikal tedavi almasına rağmen şikayetleri geçmedi. Hastaya kitle eksizyonu önerildi. Kitle eksizyonu sonrasında hastanın şikayetleri geçti. Literatürde benzer olguya rastlanmamıştır. Osteochondroma is the most common benign bone tumor (% 20-50). Osteochondroma is rarely seen on hand. Edema in the body due to hormonal changes during pregnancy can be seen. Tenosynovitis and entrapment neuropathies are increasing the rate. In our case we report; 24 years old female patient's osteochondroma on the right-hand third finger was existing. There was no complaint of the patient for 4 years. Patient’s complaints were started with pregna ncy. The complaints were not finished with medical tr eatment until six month after pregnancy. Mass excision was sugges ted to the patient. The patient’s complaint was finished after mass excision. No similar case was met in the literature.

Amaç: Lateral kondil kırıkları çocuklarda humerus suprako ndil kırıklarından sonra 2. sırada, humerus distal epifiz kırıkları içerisinde ise en sık görülen kırıklardır. Yanlış kaynam a ve kaynamama gibi önemli komplikasyonları olması nedeniyle özellikle ayrışması fazla olan kırıklarda açık cerrahi ve redüksiyon sonrası fiksasyon ile tedavi edilmesi bu kırıklarda ön plandadır. Çalışmamızda lateral kondil kırığı ile gelen çocuklarda uyguladığımız cerrahi ve konservatif tedavi sonuçlarımızı sunmayı amaçladık. Yöntem: : 2009 Temmuz -2012 Eylül döneminde lateral kondil kırığı tanısı ile tedavi edilen 36 hasta çalışmaya alındı. Bulgular : Hastaların 25’i (%69.4) erkek, 11’i (%30.6) kız çocuğu olup, yaş ortalamaları 5.7 (3,4 -8,1) yıl idi. Hastalar ortalama 25(6 -24ay) ay izlendi. Hiçbir hastada fonksiyon kaybı olmadı. Fakat 14 hastada lateralde spur oluştu. Açık cerrahi yapılanların tümünde spur mevcuttu. Kapalı yapıla nlardan tip II kırıklardan 5’sinde görüldü. Tip III kırık olup kapalı yapılanlarda Tip III kırık olup açık cerrahi yapılanlara göre daha küçük boyutta spur saptandı. Tip I kırıklarda spur yoktu. Hiçbir hastaya lateralde oluşan spur nedeni ile cerrahi eksizyon uygulanmadı. Cerrahi uygulanan hastalardan 1'inde(%2.7) yüzeyel pin dibi enfeksiyonu görüldü. Sonuç : Çocuk humerus lateral kondil kırıklarında 2mm’nin altındaki kırıklar hariç cerrahi tespit gerekmektedir. Çalışm aya dahil ettiğimiz olgularda lateral spurun görülmesi literatürle ben zerlik göstererek başlangıç deplasmanı ile ilişkili olduğu gösterilmiştir. Yine literatürle benzer olarak şupurun oluşması eklem hareketlerinde kısıtlılığa neden olmamıştır. Objective : In children humerus lateral condyle fractures are second most commo n fractures after supracondyler hume rus fractures and it is the most commen type inside distal humeral epiphyseal fractures. Because of the important complications such as non – union, mal – union, open redu ction and internally fixation is favorable, especially in di splaced fractures. In our study, we aimed to compare the results of surgery or conservative treatments of children with humerus lateral condyle fractures. Method : For our study, we chose 36 patients with humerus lateral condyle fracture that we treated between July 2009 – September 2012. Results : 25 patients (69.4 %) were male, 11(30.6 %) patients were female. Their average age was 5.7 (3.4 – 8.1). Patients were followed up for an average of 25 (6 – 24) months. None of them had function loss. But in 14 patients spur formation was seen on lateral humerus. Spur formation was seen in all patients who were treated by open surgery. Spur formation was seen in 5 patients with type II fractures that treated by close surgery. There was more spur formation in patients with type III fractures that treated by open surgery than patients with type III fractures that trea ted by close surgery. We didn’t re -operate any patient for spur excision. There was a superficial pin – tract infection in 1 patient (2.7 %). Conclusion : In children humerus lateral condyle fra c tures that has displacement 2mm or more, needs surgery. In our cases we find similar results about spur formation co m pared with literature, it was related with the displacement at the beginning as literature says. Similar with literature spur formation didn’t cause any limitation on range of m otion.

Amaç: Çalışmamızın amacı radius alt uç kırığı ile gelen, konservatif ve perkütan pinlemeyle tedavi edilen çocuk hastaların retrospektif değerlendirilmesiydi. Gereç ve yöntem: Radius alt uç kırığı tedavisi uygulanan 5-15 yaş arası 117 hasta değerlendirilmeye alındı. Tüm hastalara ilk olarak kapalı redüksiyon ve alçılama uygulandı. Redüksiyonun sağlanamadığı veya kontrollerde redüksiyonu bozulduğu görülen hastalara anestezi altında skopi kontrollü kapalı redüksiyon perkütan Kirschner(K) teli ile fiksasyon yapıldı. Bulgular: Toplam 117 hastanın 104 tanesi kapalı redüksiyon ve alçılama ile tedavi edildi. 13(6 geç-7 erken) hastaya redüksiyonun yetersiz olması veya korunamaması nedeni ile anestezi altında skopi kontrollü kapalı redüksiyon ve K-teli ile fiksasyon yapıldı. Geç dönemde redüksiyonu bozulan hastaların büyük oranda tam yer değiştirmiş kırıklar olduğu gözlendi. Erken dönemde de tam yer değiştirmiş kırıklar (5 hasta) kısmi yer değiştirmiş kırıklardan (2 hasta) fazlaydı. Sonuç: Çalışmamızda distal radius kırıklı çocuk ilk geldiği zaman kapalı redüksiyon ve alçıyla immobilizasyon yapıldı. Sonuç olarak iyileşme ve remodelizasyonun yüksek olduğu çocuk radius alt uç kırıklarında deplasman miktarına bakılmaksızın kapalı redüksiyon denenmesi yeterli redüksiyon sağlanamadığı veya kontrollerde redüksiyon bozulduğu durumlarda cerrahi yapılması uygun olacaktır. Objectives: This study designed to evaluate the results of treatment, closed reduction and percutaneous wires, of the distal radius fractures in children. Materials and methods: A retrospective analysis was carried out in children aged between 5-15 years who presented with a displaced fracture of the distal radius to our hospital. They were initially treated with closed reduction and cast immobilization. If the fractures redisplaced treated by percutaneous Kirschner (K-) wire with scope under a general anaesthesia. Results: Totally 104 patients, who have distal radius fractures were treated by closed reduction and immobilization in a plaster cast. 13 patient who have distal radius fractures were treated by closed reduction under general anaesthesia and fixed by percutaneous Kirschner (K-) wire. Patients with impaired the alignment of the fracture in late period were usually completely displaced fractures. (n=5, 4,3%), in early period, completely displaced fractures (n=5, 4,3%) are superior to partial displaced fractures (n=2, 1,7%). Conclusion: In our study, when children with distal radius fracture first come, they were treated by closed reduction and immobilization in a plaster cast. We thought that in redisplaced fractures patients were suitable for the closed reduction with percutaneous wire treatment.

Total diz protezi cerrahisi sonrası femur boynu stres kırıkları nadir görülür. Deplase olmayan ilk 2–3 hafta direkt radyografik değerlendirmede bulgu vermeyebilir, manyetik rezonans görüntüleme (MRG) ve kemik sintigrafisi önem taşır. Total diz artroplastisi uygulanan 80 yaşında bayan hasta 3 ay sonra sol inguinal bölgede şiddetli ağrı ve topallama şikayeti ile başvurdu. Radyografide kırık lehine bulgu yoktu, manyetik rezonans görüntülemede sol femur boynu inferiorunda lineer düşük sinyal dansitesi şeklinde kırık hattı belirgin olarak görülmekteydi. Stres kırığı olan hastamızda tanı sonrası 6 hafta süreyle sol alt ekstremitesine yük vermesi engellendi. Onikinci aylardaki kontrollerde hastanın klinik şikayeti olmadığı görüldü. Sonuç olarak total diz artroplastisi sonrası, inguinal bölgede ağrısı bulunan yaşlı hastalarda şüpheci olunmalı; stres kırıkları açısından erken tanının morbidite ve mortaliteyi en aza indireceği unutulmamalıdır. Femoral neck stress fractures are rarely seen following the total knee arthroplasty. Non-displaced fractures may not be seen on direct radiographies, in which case magnetic resonance imaging and bone scintigraphy are two important diagnostic tools. An eighty-year-old female patient admitted to the hospital with a severe inguinal pain and a marked limp. Radiographies were negative, but magnetic resonance imaging revealed a fracture line at the inferior femur neck as a linear signal density. The patient was allowed to walk with non weight bearing to the left lower extremity for 6 weeks. She was symptomless at the first year of follow-up. As a result, inguinal pain after total knee arthroplasty should raise a suspicion of a stress fracture of the neck of femur and early diagnosis of stress fractures could lower the morbidity and mortality.

Materials and methods: Sixty adult (6 months old) female Wistar albino rats (mean weight 220 g, range 210-270 g) were randomly divided into two groups: a montelukast group (n=30) and a control group (n=30). Closed tibia fractures were created and fixed by intramedullary Kirschner wire. The rats were sacrificed three and six weeks after the fractures. Radiological and histological evaluations were performed, and bone mineral density was measured. Results: Three rats died in the montelukast group, whereas only one died in the control group during the study. Initial weight and weight gain at the 3rd a nd 6 th weeks were not significantly different between the groups (p>0.05). Bone mineral densities in the control and study groups were 0.13±0.009 gr/cm2, and 0.13±0.01 gr/cm2 a t w eek t hree and 0.16±0.02 gr/cm2, and 0.13±0.01 gr/cm2 at week six, respectively. Histopathological scores in the control and study groups were 3.42±0.6, and 3.0±0.0 at week three and 3.5±0.5, and 3.4±0.8 at week six, respectively. Radiological scores in the control and study groups were 1.19±0.6, and 1.0±0.6 at week three and 3.0±0.8, and 2.9±0.9 at week six, respectively. There were no significant differences between the two groups in any parameters evaluated at either time interval (p>0.05). WOS:000545984600012 PubMed: 32584721 Objectives: This study aims to biomechanically and histopathologically investigate the effects of methylene blue (MB) on preventing postoperative adhesion in chickens undergoing full - thickness flexor tendon incision. Materials and methods: This study was performed between June 2017 and June 2018 on Hubbard JA 57 type chickens (age, 6 months; weight, 2.2?0.3 kg). Sixty-four tendons were studied in 32 chickens, including 16 in the control group and 16 in the experimental group. The second and third finger flexor digitorum profundus tendons of the left foot of each chicken were repaired primarily after a full -thickness incision approximately 1 cm proximal to the distal adhesion area. In the control (n=32) and experimental groups (n=32), 0.25 mL of normal saline and 0.25 mL of 1% MB solutions were applied locally to the surgical site, respectively. The operated limb was immobilized using a circular cast. 16 chickens were randomly selected in each group and examined at the fourth week, and the remaining 16 chickens were examined at the sixth week. Thirty-two of these tendons were evaluated using the Tang histopathological adhesion classification system, and the other 32 were evaluated with a biomechanical pull system. Results: Adhesion was found to be less in the experimental group compared to the control group at the end of the fourth and sixth weeks both histopathologically and biomechanically. Furthermore, adhesion was significantly less in the experimental group at the end of the sixth week compared to the fourth week both histopathologically and biomechanically. Conclusion: Histopathological and biomechanical results show that MB, which has anti-inflammatory, antiseptic, antimicrobial and antioxidant effects, reduces adhesion during the healing process after tendon repair. We think that local MB application, particularly in surgeries performed after this type of injury, will be beneficial on early rehabilitation and functional results.

Objective. Sympathetic blocks are used as an adjunct for pain management in the treatment of orthopedic and traumatic conditions. Stellate ganglion (ganglion stellatum) provides sympathetic innervation of the head, neck and cervicothoracic regions, and upper extremities. No study was found in the literature investigating the effects of stellate ganglion block performed in the upper extremity, on blood supply to bone, density, vascularization, and bone metabolism. Therefore, the objective of this study was to investigate the effects of stellate ganglion block on healing of closed forearm fractures that were induced in rats.Material and Methods. A total of 42 Wistar albino rats weighing between 398 and 510 g were used in this study. The rats were randomly divided into 2 groups with one group treated with stellate ganglion and the other included as the control group. In each 2 groups, a closed forearm fracture was created, confirmed with X-ray, and then stabilized by splint application. The forearm bones were examined with X-ray views on the same day and were then decalcified.Results. When histological findings of the fracture region were examined, predominantly cartilage and less woven bone were found in 7 rats, equally distributed cartilage and immature bone in 14 rats, and predominantly imitation bone and less cartilage formation in 21 rats. In the control group, the agreement between the 1st and 2nd orthopedists for the radiological evaluation of bone formation was moderate.Conclusion. The group administered stellate ganglion block showed a more significant fracture healing.

Stellat gangliyonun blokajı veya çıkarılması bu bölgelerde vazodilatasyon ile sonuçlandığı bilinmektedir. Yapılan çalışmalardan elde edilen aşırı sempatik aktivitenin kemik oluşumunu baskıladığı ve osteoklastik aktiviteyi artırdığı, ayrıca refleks sempatik distrofili hastaların kemik mineral yoğunluğu ve içeriğinin düşük olduğu yönündeki bulgular, stellat gangliyon blokajının sempatik aktivite azalması ve üst ekstremiteleri besleyen kan damarlarında dilatasyon sonucu kemik kırıklarının iyileşmesini hızlandırabileceği ve kemik yoğunluğu ve mineral içeriğini de artırabileceğini düşündürmüştür. Çalışmamızda, stellat ganglion blokajının sıçanlarda kırık iyileşmesi üzerine olası etkilerinin araştırılması amaçlanmıştır.Deneysel tipteki çalışmamızda, toplam 42 adet Winstar Albino cinsi dişi sıçan kullanılmıştır. Sıçanlar rastgele 2 gruba ayrılarak bir gruba Stellat ganglion blokajı yapılmış diğer grup kontrol grubu olmuştur. Her 2 gruba da kapalı ön kol kırığı oluşturularak X ray de görüntülendi ve ardından atel uygulanarak stabilize edilmiştir. Bu çalışmanın amacı humerus'un cerrahi boyun uzunluğunun rotator kas yırtıkları üzerine olan olası etkisinin değerlendirilmesidir. Çalışma 37 erkek bireyin sol omuz görüntüleri üzerinden yapılmıştır. Parsiyel yırtığı olan 10 olgu, tam kat yırtığı olan 7 olgu olmak üzere toplam 17 rotator kas yırtığı olan olgunun sol omuz manyetik rezonans (MR) görüntüleri, herhangi bir patolojisi olmayan 20 adet normal omuz MR görüntüleri ile karşılaştırılmıştır. Filmlere digimizer programında lineer ölçümler yapılmıştır. Elde edilen bilgiler SPSS 13.0 yazılımı ile değerlendirilmiştir. Epifiz kırıkları erişkin kemik kırıklarından hem oluş mekanizması hem de kırık iyileşmesi süreci ve komplikasyonları açısından önemli farklar içermektedir. Epifiz bölgesi kırıklarında kısmi veya total büyüme durmasına bağlı olarak ekstremite eşitsizlikleri ve açısal deformiteler meydana gelebilir. Üst ve alt ekstremitede ileri düzeyde fonksiyonel kısıtlılık yaratabilir. Salter Harris Tip 4 Epifiz kırıklarının (epifizden başlayıp metafize doğru uzanan kırık) tedavisinde; Kirschner (K) teli ile perkütan tespit veya açık redüksiyon + vida ile tespit sıkça kullanılan yöntemlerdir. Objective: The purpose of this experimental study was to analyze the effects of local autologous platelet-rich plasma (PRP) injection on tendon-to-bone healing in a rotator cuff repair model in rats.Methods: Rotator cuff injury was created in 68 left shoulders of rats. Klasik literatür bilgilerinde; vasküler, mekanik, anatomik, dejeneratif ve travmatik nedenlerin ekstrinsik ve intrinsik yollarla Rotator manşet yırtığına yol açtığı belirtilmektedir.Çalışmamızda, RM yırtıkları ile glenohumeral eklem uyumu arasındaki ilişkiyi sorguladık. RM yırtığına sahip olan hastalarda, glenohumeral instabiliteye yol açacak bir glenohumeral eklem uyumsuzluğu var ise bu durumu iki farklı ölçüm yöntemi ile ortaya koymayı amaçladık.Glenohumeral eklemin statik stabilizatörleri ile dinamik stabilizatörleri arasındaki ilişkiyi araştıran yeterli çalışma yoktur.Çalışmamızda, 01.01.2010 ile 31.12.2011 tarihleri arasında AİBÜ Tıp Fakültesi Ortopedi ve Travmatoloji polikliniğine omuz ağrısı ve hareket kısıtlılığı şikayeti nedeniyle başvuran bütün hastaların tanısal amaçlı olarak yaptırdığı omuz MR ları değerlendirildi. İkinci ölçüm olarak, humerusun konveks caput humeri kısımlarının sonlandığı en üst ve en alt nokta arasındaki uzunluk collum anatomicum mesafesi olarak planlandı ve ölçüldü (H değeri). Bu sayede G/H oranı hesaplandı.Geometrik morfometrik ölçümde ve lineer ölçümde aynı referans noktaları kullanıldı.Geometrik morfometrik ölçüm sonuçlarının manova analizi sonucunda, üç grup arasında glenohumeral eklem şekilleri açısından anlamlı bir fark bulunmadı (p>0,05). At dually comparision test we found difference between group 1 and group 2.Statistical analiysis of linear measurement results showed that there was no difference between 3 groups about G value and H value. At the evaluation for G/H ratio (glenohumeral joint compatibility), there was no significant difference between group 1 and group 3 (p>0,05), group 2 and group 3 , but there was difference between group 1 and group 2 (P average diameter of group 1 ). According to these results we commentated that there wasn?t any instability at group 2 patients, cause classical litterateur knowledge says decrease of glenohumeral index results in glenohumeral instability.In our study we evaluated glenohumeral compatibility by two different measurement methods. Our results showed that 3 group?s G/H results are similar to normal values of litterateur knowledge. So our study shows that there is no difference about glenohumeral joint compatibility between patients with rotator cuff tears and healty population (intact RC).We think that researching of relation between rotator cuff tears and static stabilizers; like labrum and capsuloligamentous structures other than shoulder joint anatomy; should be important to find out the relation between instabile shoulders and rotator cuff tears. 116

Introduction Patellar tendon ossification is a rare pathology that may be seen as a complication after sleeve fractures of the tibial tuberosity, total patellectomy during arthroplasty, intramedullary nailing of tibial fractures, anterior cruciate ligament reconstruction with patellar tendon autograft and knee injury without fracture. However, its occurrence after partial patellectomy surgery has never been reported in the literature. Case presentation We present the case of a 35-year-old Turkish man with a comminuted inferior patellar pole fracture that was treated with partial patellectomy. During the follow-up period, his patellar tendon healed with ossification and then ruptured from the inferior attachment to the tibial tubercle. The ossification was excised and the tendon was subsequently repaired. Conclusion To the best of our knowledge, this is the first report of patellar tendon ossification occurring after partial patellectomy. Orthopaedic surgeons are thus cautioned to be conscious of this rare complication after partial patellectomy.

Patellar tendinopathy is characterized by activity-related anterior knee pain. It is most commonly related to sports activity, but has also been reported in the non-athletic population. Most injuries are caused by microtrauma, resulting in tendinitis or tendinosis. Extraskeletal paraarticular osteochondromas, which occur in the soft tissues near the joint, are rare. The infrapatellar fat pad and joint capsule are the most common sites of these tumors. Here, a case of patellar tendinitis caused by an extraskeletal paraarticular osteochondroma is reported. The symptoms included intensifying pain upon flexion and a palpable click that was located at the medial side of the mass. The patient was pain-free within 3 weeks after excision of the tumor and the clicking disappeared. To our best knowledge, no other case of patellar tendinitis caused by an extraskeletal paraarticular osteochondroma has been reported in the English literature. © 2009 Ozturan et al; licensee BioMed Central Ltd.

We present a new method for measuring tibial torsion using conventional radiographs. We compared the method with several clinical methods and with CT measurement in 44 lower extremities of 25 subjects. The radiographic method agreed well with all of the clinical methods, and this agreement was better than agreement between CT and clinical examination. The best agreement was between thigh-foot angle and the radiographic method. The proposed radiographic measurement is a practical method for evaluation of tibial torsion in outpatient clinics without the need for specialized equipment. WOS:000261043200006 PubMed: 24692820 BACKGROUND: A variety of methods to improve skin flap survival, including the use of pharmacologic agents, have been intensively investigated. Decreasing neutrophil-mediated inflammation and tissue injury has been reported to be effective in improving flap survival. Montelukast is a selective reversible cysteinyl leukotriene receptor-1 antagonist that has been found to have protective effects against renal ischemia reperfusion injury and burn-induced oxidative injury of the skin in rats. However, its effects on skin flap survival have not been previously reported. OBJECTIVE: The aim of this study was to investigate the effects of montelukast on neutrophil-mediated random pattern skin flap survival. METHODS: Mate Sprague-Dawley rats weighing 230 to 250 g were randomly divided into 2 groups-the montelukast-treated group and the control group. Caudally based rectangular random pattern skin flaps 3 x 9 cm were elevated on the backs of the rats. The flaps were sutured into their original places. In the montelukast group, 1 mL of solution containing 10 mg/kg montelukast was administered intraperitoneally (IP) 30 minutes before surgery and then daily for 6 days. In the control group, I mL of saline was administered IP 30 minutes before surgery and then daily for 6 days. To observe the effects of montelukast, myeloperoxidase (MPO) activity, an index of tissue neutrophil infiltration, was measured from extracted skin tissue 12 hours after flap elevation. Flap viability was evaluated 7 days after surgery by measuring necrotic flap area and total flap area. RESULTS: Sixteen rats (mean [SD] weight, 240.6 [6.6] g) were equally divided between the 2 groups. All rats survived throughout the study period. Mean (SD) MPO activity in flap tissue was significantly lower in the montelukast group than in the control group (14.57 [2.33] vs 21.28 [4.86] U/g protein; P = 0.005). The percentage of necrotic flap area was significantly lower in the montelukast group than in the control group (17.17 [7-95] vs 37.51 [10.72]; P = 0.001). CONCLUSION: This small, experimental, in vivo animal study found that montelukast was associated with both lower MPO activity and a lower percentage of necrotic random pattern skin flap area. Lipoma arborescens (diffuse articular lipomatosis) is a rare intra-articular lesion consisting of subsynovial villous proliferation of mature fat cells. The usual clinical presentation is painless swelling. The aim of this series was to emphasize the importance of gray scale and color Doppler sonography through the investigation of chronic joint swelling. Methods. Lipoma arborescens of the knee and extensor digitorum longus tendon sheath at the ankle region are described. The latter is extremely rare. We examined 2 patients with sonography before using cross-sectional modalities. Results Gray scale sonography revealed hyperechoic irregular villous and frondlike projections within prominent joint effusion. A wavelike motion of the projections was shown by dynamic compression and manipulation of the effusion. We observed moderate vascularity on color Doppler sonography. Spectral analysis revealed a low-resistance arterial waveform. Conclusions. Gray scale sonography is a useful diagnostic modality that can be performed to evaluate chronic joint swelling before the use of cross-sectional and more expensive modalities.
