Peritoneal carcinomatosis is associated with poor prognosis in gastric cancer patients. Stage IV gastric cancer (GC) is diagnosed in 39.8 % of patients; local metastases without evidence of distant metastases occur only in 18–20 % of stage IV gastric cancer patients.The purpose of the study was to estimate the efficacy of personalized chemotherapy in the combined modality treatment of patients with stage IV GC with peritoneal carcinomatosis.Material and Methods. Cytoreductive surgery was performed in 70 patients with GC with peritoneal dissemination. The control group patients (n=35) received postoperative chemotherapy with the FOLFOX regimen. The study group patients (n=35) received personalized systemic and intraperitoneal chemotherapy based on the expression of chemosensitivity and resistance genes.Results. The median survival time (18.7 months) in the study group patients was higher than that in the control group and in studies described in the world literature (CRS + HIPEC). Personalized chemotherapy improved median progressionfree survival (PFS) by 4.6 months (29.1 %) and median overall survival (OS) by 6 months (32 %) compared to FOLFOX regimen chemotherapy. In the study group, the 1-, 2and 3-year survival rates were observed in 35 (100 %), 9 (27 %) and 1 (3 %) patients, respectively.Conclusion. Personalized chemotherapy in the combined modality treatment can improve long-term treatment outcomes (longer median PFS and OS) in GC patients with peritoneal dissemination.

Graft failure is a group of complications after allogeneic hematopoietic stem cell transplantation, which occurs according to different data up to 30%. The group of complications includes primary and secondary graft failure, primary, secondary and transient poor graft function and graft rejection. Diagnostic difficulties consist in the lack of unified diagnostic criteria accepted in the transplantation community and in the dual interpretation of these complications according to the foreign literature. The purpose of this literature review was to identify the most common criteria of different types of graft failure and determine the tactics of diagnosis and treatment. In this review we analyzed data from various literature sources, gave definitions of graft failure and poor graft function. We analyzed the literature data on the methods used to treat these conditions.

Relevance. The development of fermented herbal beverages will make it possible not only to process grain crops as efficiently as possible, but also to meet the needs of people with intolerance to milk components, as well as those who adhere to a vegetarian diet. As one of the directions, it is possible to offer production of fermented vegetable drinks with predetermined properties by enriching them with antioxidants of polyphenolic nature. These technologies will minimise the risks of non-communicable diseases in the Russian Federation and stabilise public health in the long term.Methods. Six samples of fermented herbal beverages were identified as objects of study, two of which were obtained by fermentation of wheat and barley grains using "Bifivit" starter, powdered flavonoid taxifolin was added to other two samples at the fermentation stage, while the last two used the hydroxypropyl-betacyclodextrin encapsulated form of taxifolin. Total flavonoid content using spectrophotometric method, antioxidant activity(AOA) using DPPH-method as well as bioavailability index and digestibility criterion were determined in all tested samples.Results. All the samples of fermented herbal beverages with taxifolin are characterized by sufficiently high flavonoid content. The samples containing encapsulated forms of the flavonoid have, on average, 7 to 8% lower values than the samples of herbal drinks containing the native form of the flavonoid. At the same time, the bioavailability index for the samples containing encapsulated forms of flavonoid was on average 40 to 42% higher, indicating the effectiveness of the encapsulation process. The highest values of antioxidant activity were characteristic of the plant drinks enriched with the initial form of taxifolin, which is probably due to the creation of a protective shell and a decrease in the AOA values by 10–12% on average in samples with encapsulated form. The process of fermentation of vegetable raw materials leads to an increase in their digestibility, as evidenced by the growth of the infusoria Tetrahymena pyriformis. Introduction of taxifolin into the formulation of drinks both in its original and encapsulated form increases the growth of protozoa by 187±4–315±3%.

Background. The accurate and early diagnosis of breast cancer can improve efficacy of the treatment. The standard diagnostic methods such as mammography, ultrasound, and magnetic resonance tomography have a pivotal role in the detection of breast tumors, however, in some cases, they have low diagnostic accuracy. Mammoscintigraphy (MSG) / molecular breast imaging (MBI) with tumor-specific radiopharmacy 99mTc-Technetril in patients with breast cancer can considerably increase the accuracy of diagnosis. However, the diagnostic performance of MSG / MBI in the detection of different biological subtypes of breast cancer is still under investigation.Aim. To evaluate the accuracy of MSG / MBI with 99mTc-Technetril in diagnosis of different biological subtypes of breast cancer.Materials and methods. The analysis included the results of MSG / MBI of 1080 patients (2154 mammary glands), who were examined for suspected breast cancer. MSG / MBI were performed 5–15 min after intravenous injection into the vein of one of the feet of 370–740 MBq of tumor-specific radiopharmacy 99mTc-Technetril. Examinations performed from 2007–2020 was carried out on the emission computed tomography Forte (Philips); since 2020 the molecular visualization has been providing on the special gamma-camera Discovery NM750b (General Electric). The obtained data were evaluated by 2 experienced radiologists. Verification of changes in breasts was provided by morphological examination (1060 cases) or dynamic observation.Results. The sensitivity, specificity and overall accuracy of MSG / MBI were 90 %, 98 %, 95 % correspondingly. When diagnosing tumors with a diameter of up to 10 mm, the sensitivity of MSG / MBI was decreased to 83 %. In patients with various biological subtypes, the sensitivity of MSG / MBI was as follows: luminal A – 88 %; luminal B– – 91 %; luminal B+ – 92 %; triple negative – 93 %; HER2-positive – 96 %. The intensity of tumor uptake depended on the biological subtype of breast cancer. The average values of the 99mTc-Technetril uptake coefficient were as follows: luminal A – 1.59; luminal B– – 1.71; luminal B+ – 1.95; triple negative – 1.93; HER2-positive – 2.22.Conclusion. Retrospective analysis indicate high diagnostic performance of MSG / MBI: sensitivity – 90 %, specificity – 98 %, accuracy – 95 %. There are significant differences in the intensity of 99mTc-Technetril accumulation in tumors in patients with different biological subtypes of breast cancer (p = 0.01–0.004). MSG / MBI characterized by significant differences in the sensitivity in the diagnosis of luminal A and HER2+ breast cancer subtypes: 88 % and 96 %, respectively (p = 0.02).

Relevance.Bread products are socially important foodstuffs, physically and economically accessible for the majority of population groups in the Russian Federation and many other countries. One of the directions could be the enrichment of bakery products with natural antioxidants of plant origin. Polyphenolic substances, including flavonoids, have priority among plant antioxidants. Today several approaches to enrichment of bakery products with polyphenols and flavonoids are known, each of them characterised by their own advantages and disadvantages. The basis of this study was to evaluate the possibility of using different bakery enrichment approaches to obtain products with given antioxidant properties.Methods. Six samples of bakery products, five of which were enriched with food ingredients— sources of antioxidants— were identified as objects of research. The total content of polyphenols and flavonoids was determined in all the examined samples using spectrophotometric method as well as the antioxidant activity using DPPH-method.Results. Studies have shown that the use of crushed green tea leaves, cedar flour, whole-milled flour from germinated wheat grain as enriching agents allows a significant increase in the total content of polyphenols in the finished samples of baked products, by 46–75% in relation to the control. All the samples of enriched products could be classified as functional foods, as the satisfaction of the daily requirement for flavonoids was more than 15% of the recommended level. At the same time, the use of individual enriching substances such as taxifolin allows to significantly increase the antioxidant activity of baked products (more than by4 times) and with a high probability to steadily regulate it. In general, in all investigated samples of enriched bakery products increase ofthe antioxidant activity by more than 2 times in relation to the controlwas registered. The results confirmed the possibility and feasibility of using the proposed approaches of bakery products enrichment to obtain products with antioxidant properties.

Background. Recovery of children with acute lymphoblastic leukemia (ALL) was one of the most significant achievements of clinical oncohematology of XX century. Success in the treatment of ALL became possible due to comprehensive (clinical, morphoimmunological, cytogenetic) diagnostics and the development of differentiated, risk-adapted treatment protocols. The German group BFM (Berlin–Frankfurt–Munster) became a pioneer in creating effective treatment programs for children with ALL. Its new principles and approaches in treatment allow the vast majority of patients to be cured.The aim of the study – presentation of the treatment results of children with ALL based on the ALL IC-BFM 2002 protocol as part of a multicenter long standing study.Materials and methods. There were 592 patients with a newly diagnosed ALL in the study . The average age of patients was 10.5 years (from 4 months to 21 years). Treatment was carried out according to the ALL IC-BFM 2002 protocol in 11 clinics from 01.11.2003 to 16.05.2022. The overall (OS), relapse-free (RFS) and event-free (EFS) survivals of patients were estimated on 01.07.2022.Results. Complete clinical and hematological remission was achieved by the 33 day of therapy in 582 (98.3 %) cases. 10-year OS was 90.4 ± 1.5 %, RFS – 83.9 ± 1.9 % EFS – 82.4 ± 1.9 %. The analysis of survival of ALL patients in prognostic risk groups showed that 10-year OS in the standard risk group was 92.8 ± 1.7 %, RFS – 86 ± 2.2 % and EFS – 84.3 ± 2.2 %. Among the patients of the intermediate risk group the OS, RFS and EFS were 94.6 ± 2.6 %, 82.2 ± 5 % and 81.7 ± 5 %, respectively. The prognosis of ALL patients from the high-risk group turned out to be the least favorable: OS was 69.5 ± 8.7 %, RFS – 62 ± 9.4 % and EFS – 60.3 ± 9.3 %.Conclusion. Based on the results of a multicenter study the ALL IC-BFM 2002 protocol showed high efficiency with the possibility of achieving a 10-year OS of 90.4 ± 1.5 %. This protocol turned out to be reproducible both in federal and regional clinics with high rates of long-term patients survival. This fact allows including it in the clinical recommendations of the Ministry of Health of Russia.

Aim of the study: a systematic analysis of the modern literature data on the nivolumab monotherapy efficacy in patients with metastatic colorectal cancer (mCRC). Material and methods. The review summarizes the results of clinical studies of the nivolumab efficacy in patients with mCRC between 2012 and 2022. The current approaches to assessing the tumor response in patients treated with immune checkpoint inhibitors are considered, including response patterns and criteria. Results. Data analysis showed that the use of nivolumab in mCRC patients had significant clinical benefits. Nivolumab monotherapy was shown to improve survival in patients with high microsatellite instability (MSI) or deficiencies in mismatch repair (dMMR) that progressed during standard chemotherapy. Numerous clinical studies indicate the atypical response to nivolumab. Traditional response criteria, such as RECIST do not always adequately assess the therapeutic efficacy of nivolumab in patients with mCRC. Conclusion. To improve the efficacy of mCRC treatment, standardized approaches based on the proposed specific criteria for response to immunotherapy, including immune related RECIST, immune RECIST, and immune-modified RECIST must be developed.

Background. The neutrophil to lymphocyte ratio (NLR) is a prognostic marker in various carcinomas, including breast cancer. The NLR reflects the balance of systemic immunity and is associated with survival in patients with solid tumors.Aim. This study retrospectively evaluated the significance of the NLR and the absolute lymphocyte count in patients with breast cancer who received eribulin therapy.Materials and methods. The prognostic significance of the NLR and the absolute lymphocyte count in 33 patients with metastatic breast cancer treated with eribulin in 2020–2021 was analyzed.Results. The median follow-up time for patients was 9 [6; 15] months. Twenty-six of 33 (78.8 %) patients progressed. Seven of 33 (21.2 %) patients were censored due to therapy discontinuation, complete discontinuation after 1 course was required in 3 (9.1 %) patients and was associated with the development of hematological toxicity: 1 (3 %) case of a combination of grade 3 leucopenia and grade 3 thrombocytopenia, 2 (6 %) cases of grade 3 neutropenia. According to the analysis results, the NLR was a statistically significant risk factor for progression in patients on eribulin therapy, both without taking into account the influence of other factors (univariate model) and with the underlying influence of the absolute neutrophil count and the absolute lymphocyte count: hazard ratio (95 % confidence interval) was 1.21 [1.04; 1.41], p = 0.015 and 1.30 [1.07; 1.57], p = 0.008, respectively. There was no statistically significant effect of the absolute neutrophil count and the absolute lymphocyte count in both univariate and multivariate models. To illustrate the impact of SNL on the onset of progression, patients were divided into two subgroups based on median SNL: SNL˂ 2.46 and SNL ≥2.46. The median survival in patients with SNL ≥2.46 was 9 months, and in patients with SNL˂ 2.46 it was 15 months.Conclusion. The results of the pilot study state the NLR is a potential prognostic marker of long-term treatment with eribulin for patients with metastatic breast cancer, and can be used in clinical practice.

The objective: to determine risk factors of lethal outcomes (predictors) in tuberculosis patients treated with chemotherapy regimens I, II, III during confinement in the Russian penitentiary system.Subjects and Methods: data on 6,069 patients registered for treatment in 2019 and 2020 were studied. Univariate and multivariate analyses were performed.Results. The following factors were found to have effect on mortality: age (aOR = 1.05; 95% CI 1.03-1.06), multiple localizations of tuberculosis (aOR = 11.08; 95% CI 5.12-24.20), concurrent HIV infection (aOR = 9.13; 95% CI 6.90-12.24), and positive sputum microscopy at registration (aOR = 5.18; 95% CI 3.93-6.81). The patient gender was found to have no effect (OR = 0.9; 95% CI 0.4-1.9; p = 1).

An annotated list of mossy species of the museum-reserve “S. N. Khudekov’s Estate”, including 71 species, as well as data on the analysis of occurrence and ecological-substrate distribution, is given. The level of biodiversity and the proportion of rare species are estimated as quite high. The conclusion about the essential role of the museum-reserve in the preservation of the moss component of the natural ecosystems of the northern forest-steppe and landscape gardening is substantiated. In order to increase the environmental significance of the museum-reserve, it is recommended to combine the arboretum park and the Yerlin Forest into a single protected area.

The article is devoted to the study of the procedure for the formation of standard costs for the remuneration of the coaching staff in relation to the applied systems of remuneration of their labor. The authors, having studied the issue of the systems of remuneration used, come to the conclusion about the enormous popularity of the “group” system, but the partial use of the “hourly”, “capitation”, “rating” system of remuneration. The study of the procedure for the formation of standard costs for the wages fund indicates a devaluation of the standard costs for wages of trainers to 72.5%, which is explained by insufficient consideration of the conditions of the external and (or) internal environment of the organization. In the context of the harmonization of legislation carried out in the country, the authors proposed to restructure the procedure for determining the normative costs, taking into account the identified contradictions, taking into account: 1) revision of the allocation of independent work as a mandatory form of organizing the training process, 2) unification of the duration of the lesson regardless of the type of economic activity of the program being implemented, 3) special the nature of the work of coaches, 4) the peculiarities of recruiting groups of sports training, 5) the need to revise the classification of sports training services.

Purpose of the study. The purpose of the study was to analyze parameters of molecular markers of structural and cellular renal damage in localized renal cell carcinoma (RCC) with determining the nature of the initial abnormalities in the kidney functional state before the treatment.Patients and methods. The study included 46 patients receiving elective surgical treatment for localized renal cancer in the Department of Oncourology, National Medical Research Centre for Oncology. The comparison group included the clinical and laboratory data of 13 healthy people comparable with the RCC patients in terms of age and gender. Cystatin C, IL‑18, KIM‑1, L-FABP, NGAL were determined in blood and urine in all patients.Results. Evaluation of the kidney functional state of RCC patients showed that the initial values of serum creatinine and the glomerular filtration rate were similar to the reference levels in healthy people, but statistically significant differences were found in the ratios of cystatin C concentrations in the blood and urine in all patients, compared with normal values. Determination of L-FABP indices in RCC patients showed that their levels were 2.5 times higher than normal values, and the urine concentration of IL‑18 was 1.7 times higher than normal values (p < 0.05). Blood and urine levels of NGAL and KIM‑1 did not differ significantly from the comparison group.Conclusions. The development of localized RCC is accompanied by the formation of tubulointerstitial dysfunction with impaired renal filtration capacity. All RCC patients showed elevated endogenous markers of structural and cellular renal damage – cystatin C, L-FABP, and IL‑18.

The research made it possible to develop a technology for the production of soft beverages using natural vegetable raw materials. To prepare a non-alcoholic natural drink of direct brewing, the following vegetable raw materials were selected: chamomile, orange, lemongrass, valerian, cinnamon, ginger, rose, karkade, rosehip, honeybush. As a result of the study of the composition of the ingredients, it was found that this raw material is rich in vitamins, macro- and microelements, organic acids, tannins, dietary fibers, flavonols and other functional substances. At the same time, they contain a small amount of calories. Due to the presence of benzoic acid in the composition of rosehip fruits, sorbic acid in the composition of orange, the drink will be stabilized, since they are natural preservatives. Also, orange peel, karkade give the drink a taste and a pleasant aroma. Thus, the considered raw materials are a favorable object to create a soft drink based on them. Beverages based on these natural herbal ingredients will be in demand among people who take care of their health. During the experiment, three experimental blends were made for two beverages, with different contents of chamomile, orange, lemongrass, valerian, cinnamon, ginger and rose, karkade, rosehip, honeybush, lemongrass. According to the organoleptic parameters, one option was selected for each drink. The organoleptic and physico-chemical parameters of the finished product - a natural soft drink of direct brewing - have been studied. Based on the conducted research, a technological scheme for obtaining a soft drink based on the direct brewing method was modeled. Technological documentation for beverages has been developed.

Today, the relevance of studying medicinal plants and creating medicines based on them occupies a special place in medicine and pharmacy. Interest in the study of already known and new medicinal plants of the domestic flora is growing. On their basis, new herbal medicines and dietary supplements are created, which due to their unique compositionhave a mild therapeutic and complex effect on the organs and systems of the human body. They are virtually devoid of side effects, and they do not develop addiction. Aim. To study the qualitative composition and the content of fatty acids in the medicinal plant raw material from sea buckthorn using the chromatographic method; to determine the specific feature of the fatty acid composition for identification of the sea buckthorn raw material. Materials and methods. The study objects were leaves, fruit pulp, seeds and bark of sea buckthorn harvested at the pharmacopoeial garden of the National University of Pharmacy (2018). The analysis was performed using gas chromatography-mass spectrometry (GC/MS). Results and discussion. The results of the analysis indicate that the medicinal plant raw material from sea buckthorn has a rich diverse composition of saturated and unsaturated fatty acids, namely: in the leaves of sea buckthorn there are significant amounts of fatty acids. Among saturated fatty acids there are palmitic (55.33 mg/g), behenic (1.07 mg/g),stearic (1.03 mg/g), arachinic (0.91 mg/g), lignocerinic (0.78 mg/g), margaric (0.32 mg/g), myristic (0.28 mg/g) acids; among unsaturated fatty acids – oleic (7.79 mg/g), linoleic (2.42 mg/g). Fatty acids of the sea buckthorn bark are representedby saturated arachidonic (20.85 mg/g), palmitic (2.14 mg/g), erucic (2.09 mg/g), heneukocylic (1.87 mg/g),behenic (1.38 mg/g) acids and unsaturated oleic (5.75 mg/g) and linoleic (4.86 m/g) fatty acids. In the pulp of sea buckthorn fruits 9 fatty acids have been identified. Palmitic acid (23.55 mg/g) is contained in large quantities, there are also stearic acid (2.68 mg/g), myristic acid (1.36 mg/g), arachinic acid (0.87 mg/g). Among unsaturated acids, oleic acid (44.42 mg/g), linoleic acid (12.49 mg/g), linolenic acid (5.96 mg/g), palmitoleic acid (5.16 mg/g) and vaccenic acid (3.79 mg/g) have been identified. The seeds of sea buckthorn contain 8 fatty acids. Three of them are saturated: palmitic (15.89 mg/g), stearic (2.51 mg/g), myristic (0.71 mg/g), and there are five unsaturated acids: oleic (31.41 mg/g), linoleic (27.03 mg/g),linolenic (17.00 mg/g), vaccenic (2.86 mg/g) and palmitoleic (2.56 mg/g). Conclusions. The results of the analysis show a rich fatty acid composition of the medicinal plant raw material from sea buckthorn. It has been determined that the specific feature is the presence and the ratio of palmitoleic and vaccenic acid in the pulp and seeds; this feature is characteristic when identifying the sea buckthorn raw material. Thus, this raw material can be promising for the creation of medicines and dietary supplements based on it, for the treatment and prevention of various diseases.

Background. The study of anemia causes in children born prematurely remains the urgent problem due to the continuous search for effective ways to prevent and treat it.Objective. The aim of the study is to study the anemia severity in premature infants, peripheral blood test results (MCV, MCH, MCHС), serum iron, and ferritin at different gestational periods.Materials and methods. 82 medical records of preterm infants were analyzed. 12 children were extremely premature (EР), 36 were very premature (VP), 27 were moderately premature (MP), and 7 were late premature (LP). The frequency of anemia, its severity, and laboratory indicators of iron metabolism were studied.Results. In EР infants, anemia developed in 100% (severe and moderate form in 16.7%, mild in 66.6%), in VP — in 94.6% (of which severe form in 5.6%, moderate — 22%, mild — 67%). Severe forms were not detected in children over 32 weeks of gestation. In infants with ЕР, the iron level at 2 weeks Me (median) 10.6 μmol/l [8.8–12.1], which is lower than in the comparison groups: VP — 15.9 [12.3–18.9] (p = 0.007), MP — 18,7 [15.8–20.5] (p = 0.0002), LP — 14,0 [16.2–23.8] (p = 0.011). Serum ferritin in the EР group is 58.4 μg/l [52.8–71.7], lower than the level in children with OH — 165.5 μg/l [77.3–195] (p = 0.03). The indicators MCV, MCH did not reflect the etiology of anemia, MCHC is reduced in all groups. With oral subsidization of 3-valent iron, its level increase in EР: before treatment — 10.65 μmol/l [8.2–12.1], after 2 weeks — 21.95 μmol/l [17.8–23,9] (p = 0.011).Conclusion. Severe and moderate degrees of early anemia of prematurity are diagnosed in the EР and VP groups. Significant deficiency of serum iron was detected in EР at the age of 2 weeks. MCHC less than 33.1% was registered in all gestational periods, it can be the marker of early anemia of prematurity. Iron supplement is required for EР.

Over the past ten years, the creation of "smart cities" has become a priority area for the development of the digital economy in the world, in our country this direction is just beginning to develop both in legal and regulatory documents, and in economic, social aspects. The achievement of reference points for the spatial development of the economy is due to the implementation of the Smart City project. In our country, the Smart City project is aimed at creating an effective urban management system, creating security and comfortable living conditions for citizens and increasing the competitiveness of Russian cities. The implementation of the national project "Housing and Urban Environment" and the national program "Digital Economy" is being implemented as part of the national project "Smart City", which was developed by the Ministry of Construction of Russia and started in 2018. The implementation of the "Smart City" project is gradually being implemented in various regions of the Russian Federation using digital technologies in urban infrastructure development. The implementation of this project involves taking into account the economic potential of the region, financial resources, human capital (level of education and age of the population), innovation potential, which is determined by the availability and quality of the relevant business infrastructure, scientific, technical and educational base. When implementing each project, risks are not excluded: social, economic, political, technological, which must be taken into account when implementing the Smart City project. The work considered the problems of introducing a national project in the regions, as well as during the analysis highlighted the risks in creating "smart cities." The main directions of trends in the development of "smart cities" are considered: social, economic, technological. The IQ index of the largest "smart cities" for the period 2019-2020 was analyzed. Dynamics of growth of "intellectual" index of digitalization of Russian cities revealed.

Gastric cancer (gc) is one of the most common cancers worldwide. The majority of newly diagnosed gastric cancer cases present with distant metastases. Peritoneal carcinomatosis (pc) is the most unfavorable type of progression of primary gc, which occurs in 14–43 % of patients. The purpose of the study was to highlight modern approaches to the treatment of gc with pc. Material and methods. We analyzed 136 publications available from pubmed, medline, cochrane library, and elibrary databases. The final analysis included 46 studies that met the specified parameters. Results. The modern approaches to the treatment of gc with peritoneal carcinomatosis were reviewed, namely: cytoreductive surgery (crs), combination of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (crs/hipec); neoadjuvant intraperitoneal/systemic chemotherapy (nips) and pressurized intraperitoneal aerosol chemotherapy (pipac). The results of large randomized trials and meta-analyses were analyzed. Benefits and limitations of these trials were assessed. Conclusion. The peritoneal cancer index (pci) and the level of cytoreduction are two key prognostic factors for increasing the median overall survival. By reducing tumor volume through cytoreductive surgery, it is possible to allow tumor cells to re-enter the proliferative phase of the cell cycle and make them more sensitive to antitumor agents. The hematoperitoneal barrier is the main reason that prevents the effective delivery of drugs from the systemic bloodstream to the abdominal cavity, which is why the effect of systemic chemotherapy on peritoneal metastases is extremely limited. Intraperitoneal chemotherapy offers a more effective and intensive regional therapy, creating a so-called «depot» of a chemotherapy drug, thereby prolonging the effect of the administered drugs. Cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (crs + hipec) using the combination of surgical resection, cytotoxic chemotherapy, hyperthermic ablation of the tumor and hydrodynamic flushing, is a promising approach in the treatment of gc with peritoneal carcinomatosis.

Background. Triple negative breast cancer has no specific treatment sites for chemotherapy and is unfavorable in terms of prognosis. One of the drugs widely used in this cohort of patients is eribulin, which in addition to its antimitotic effect has an effect on the tumor microenvironment. The search for biological criteria that will allow predicting the effectiveness of the drug is assumed relevant since it will help to select patients who may receive the most benefit from certain therapy regimens.Objective: identification of immunological predictors of the therapeutic effectiveness of eribulin in patients with locally advanced or metastatic triple-negative breast cancer.Materials and methods. The study included 20 patients with locally advanced and metastatic triple negative breast cancer. 50 % had a short-term response (progression-free survival 6 months). Seven-color immunofluorescence was used to determine the subpopulation composition of tumor-infiltrating lymphocytes and their PD1 expression. Image acquisition and analysis were performed using the Vectra® 3.0 system and InForm® software (Akoya Biosciences, USA).Results. It has been shown that the ratio of the number of PD1-negative to PD1-positive CD20+ B-lymphocytes less than 5.5 associated with the long-term effectiveness of eribulin in patients with locally advanced or metastatic triple negative breast cancer.Conclusion. The results showed that the ratio of the number of PD1-negative to PD1-positive CD20+ B-lymphocytes can be considered as a possible marker to predict the effectiveness of eribulin in patients with breast cancer.

Introduction. The development of a malignant tumor naturally affects renal function. During tumor formation, the renal tissue is destructed either by direct invasion into the parenchyma, or by mechanical change in the renal architecture caused by compression of the renal parenchyma, collecting ducts, tubules, and nephrons. In addition, a tumor can secrete biologically active substances, which have an indirect negative influence the functional state of the organ. Currently, it has been established that kallikrein-kinin and renin-angiotensin-aldosterone systems play an important role in the development of nephropathy of various genesis. At the same time, these systems' role in the development of renal function disorders in the setting of tumor damage has not yet been studied.Purpose of the study. To study changes in the components of the kallikrein-kinin and renin-angiotensin-aldosterone systems in the case of localized kidney cancer.Materials and methods. Forty-five patients diagnosed with T1N0M0 kidney cancer and 13 relatively healthy patients without cancer were examined. The determination of the components of the systems under study was carried out by the kinetic method after chromatography of blood plasma and urine using DEAE-Sephadex A-50 (Amersham Biosciences Corp., Sweden). The indices of angiotensin-1, renin, aldosterone, and cortisol were studied by an indirect method of radioimmunoassay. Statistical processing was carried out using Statistica 8.0 software (StatSoft Inc., IBM Corp., USA) by means of the Student-Fisher test (p < 0.05).Results. The development of kidney cancer is accompanied by a 2.3-fold increase in the activity of kallikrein and other trypsin proteases with a significant deficiency of their inhibitors (p < 0,05). Against this background, there is a 1.3-fold decrease in the cortisol/renin ratio from a 2.9-fold and 2.3-fold increase in the values of the renin/angiotensin-I and cortisol/angiotensin-I interaction ratios, respectively, compared with the normal values of these indicators (p < 0,05).Conclusions. Renal cell carcinoma is accompanied by trespassing of local metabolism with the formation of tubulointerstitial dysfunction and a shift of the proteinase-inhibitory balance towards proteolytic activation.

Introduction. The standard of 1st line treatment of HER2+ metastatic breast cancer (mBC) is double blockade with trastuzumab and pertuzumab + taxane, 2nd line – Trastuzumab-emtazine. There are no standards for further treatment, as well as the optimal drug sequence. Expansion of the arsenal of therapeutic possibilities and the use of new combinations will certainly improve the results of treatment of this category of patients and increase their life expectancy.Aim. We sought to describe treatment patterns of eribulin and clinical outcomes of metastatic HER2-positive breast cancer treated with eribulin plus trastuzumab combination in academic institutions and community oncology practices across the Russian Federation.Materials and methods. Patients treated with eribulin anytime between Jan, 2014 and Sep, 2019 with a diagnosis of MBC were identified by 23 providers from Russia. Providers retrospectively reviewed the health records and abstracted selected data points into an electronic case report form for each eligible patient.Results. 100 HER2-positive pts received eribulin in combination with trastuzumab. Median age was 55 (31–80) yrs and ECOG status 0–3. 67% pts had visceral metastases. Eribulin was administered as 1st and 2nd line to 23 (23%) pts, 3rd line to 31 (31%) pts, 4th line and later to 46 (46%). Median number of cycles was 5 (2–27). ORR was 12%, SD – 72%, SD > 6 months – 23%, PD – 16%. Clinical efficacy rate achieved in 35%. Median PFS was 5.07 months (95% CI 4.021–6.119). According to the ER-status the response to eribulin and trastuzumab was different. ORR was 18.8%, SD 72.9% in pts with ER-positive MBC (n = 48) and 5.8% and 71.2% respectively in ER-negative MBC (n = 52). Median PFS was 6.97 months (95% CI 3.924–10.016) in pts with ER-positive MBC and 4.67 months (95% CI 3.841–5.499) in ER-negative MBC (р = 0.3). The combination was well tolerated: dose reductions were required in 12% pts, withdrawal due to toxicity in 4% pts. The most common type of toxicity was hematological with neutropenia Gr III-IV in 14 (14%) pts. Peripheral neuropathy Gr III was observed in 5 (5%) pts. No cardiotoxicity was detected.Conclusions. This is the real-life data of clinical outcomes for patients receiving eribulin plus trastuzumab for HER2-positive MBC throughout the Russian Federation. Our experience with eribulin plus trastuzumab demonstrates that this combination may be a potential effective treatment option for HER-2 positive MBC patients.

This clinical observation demonstrates a method of a motivated use of a transdermal therapeutic system (TTS) based on fentanyl for chemical pleurodesis in a patient with prolonged air leakage after lung resection for cancer. The most common complication after elective lung resections is an alveolar-pleural fistula or prolonged air leakage. This clinical phenomenon occurs as a result of communication between the alveoli of the lung parenchyma distal to the segmental bronchus and the pleural cavity. In most cases, air leakage through the drains is eliminated spontaneously, but the frequency of prolonged pneumostasis absence in the postoperative period can reach 25 %, which has a negative effect on the outcomes of surgical interventions due to the development of pneumonia and empyema. Long-term drainage of the pleural cavity does not always end with aerostasis and requires repeated invasive interventions. One of the ways to achieve the tightness of the lung tissue involves various methods of chemical pleurodesis, which is a surgical manipulation – the introduction of a sclerosing chemical substance into the pleural cavity by spraying medical talc through a trocar or a injecting tetracycline solution into the pleural drains. The chemical causes aseptic inflammation and adhesions between the visceral and parietal pleura, followed by obliteration of the pleural cavity. The sclerosant introduction is accompanied by severe pain that can provoke respiratory and/or hemodynamic deficits, up to apnea and life-threatening heart rhythm disturbances. Pain relief during chemical pleurodesis is obviously an important factor in the prevention of a number of complications in patients undergoing surgery for lung cancer. Bolus intravenous injections of narcotic analgesics lead to an analgesic effect, but a short-term one due to the absence of a depot in the body and a sharp drop in the drug concentration in the blood serum. Unfortunately, this method of introducing narcotic drugs can cause various complications in weakened and elderly cancer patients, such as respiratory depression and cardiac arrest. The TTS action is characterized with continuous dosing and the creation of a constant concentration of the narcotic drug over a certain period of time. This method provides a multilevel and systematic approach to pain relief, reduces toxicity and minimizes the inhibition of the central mechanisms of external respiration regulation without causing respiratory and cardiac disorders in patients who underwent lung resection.

Clear translation remains a major challenge to better communication and understanding of the international academic literature, despite advances in Machine Translation (MT). Automatic translation systems which captured the detail and the sense of any manuscript in any language for a reader from any other linguistic background would find global applications.In this article, we discuss the current opportunities and constraints to the wider use of machine translation and computer-assisted human translation (CAT). At the present stage of technology development, these instruments offer a number of advantages to specialists working with scientific texts. These include the facility to skim and scan large amounts of information in foreign languages, and to act as digital dictionaries, thesauri and encyclopedias. Word-to-word and phrase-to-phrase translation between many languages and scripts is now well advanced.The availability of modern machine translation has therefore changed the work of specialist scientific translators, placing greater emphasis on more advanced text and sense editing skills. However, machine translation is still challenged by the nuances of language and culture from one society to another, particularly in the freestyle literature of the arts and humanities. Scientific papers are generally much more structured, but the quality of machine translation still largely depends on the quality of the source text. This varies considerably between different scientific disciplines and from one author to another.The most advanced translation systems are making steady progress. It is timely to revisit traditional training programmes in the field of written translation to focus on the development of higher-level research competencies, such as terminology search, and so to make best use of evolving machine translation technologies.More widely, we consider that there is a challenge across the higher education systems in all countries to develop a simple, clear and consistent “international” writing style to assist fast, reliable and low-cost machine translation and hence to advance mutual understanding across the global scientific literature.

The graft-versus-host disease (GVHD) is among the most common complications after hematopoietic stem cell transplantation (allo-HSCT). The main tools for GVHD prevention remain calcineurin inhibitors (cyclosporin A, tacrolimus), methotrexate, mycophenolate mofetil. Upon implementation of reduced-intensity conditioning regimens, antithymocyte globulin was widely introduced. However, negative effects upon reconstitution of T-cell immunity have been noted, thus increasing risk of severe infectious complications and disease relapse. With extended practice of HSCT from alternative (partially matched or haploidentical) donors, cyclophosphamide was increasingly used. Our aim was to study reconstitution of immune cell subpopulations in the patients undergoing bone marrow transplantation (BMT), when using different GVHD prophylaxis regimens, including the schedules with post-transplant CP usage. The study concerned 44 cases classified into 2 groups. The first one included patients with standard immunosuppressive therapy, antithymocyte therapy, cyclosporine A, methotrexate, mycophenolate mofetil. The second group included the patients who received CP as immunosuppressive drug combined with other treatments (cyclosporine A, methotrexate, mycophenolate mofetil). At specified control terms, (D+14, +30, +60, +90) the blood leukocyte subpopulations were assayed by means of multicolor flow cytometry. Absolute counts of CD4+ cells in HSCT recipients treated with CP post-BMT proved to be sufficiently lower at D+14 and +30, than in those treated with classical immunosuppressive therapy. However, at later terms, (D+60, +90), these differences were not observed. Moreover, in CP-treated bone marrow recipients, absolute numbers of CD8+cells was significantly higher, compared to the patients who received conventional GVHD prophylaxis. Reconstitution of the studied lymphocyte populations in hematopoietic cell recipients did not depend on the GVHD prophylaxis regimen. Usage of CP combined with bone marrow as a source of stem cells, brings about sufficient decrease of some cell populations (CD4+; CD8+; NK cells) at early terms post-transplant. Administration of CP combined with hematopoietic stem cells as the source of hematopoietic graft seems to be more reasonable.

Retinopathy of prematurity (RP) is a complex pathology among prematurely born children, which requires a comprehensive approach, including both prevention of the development of more severe stages of the disease, leading to poor vision and irreversible loss of visual functions, and the organization of dispensary observation and rehabilitation. Modern organization of dispensary observation, which is based on a detailed study of the etiology, pathogenesis, as well as the interaction of local and systemic factors on visual functions, can significantly reduce the visual disability of children as a result of this disease.
