There can be many reasons that cause MI, such as a sedentary lifestyle, a disordered diet, harmful habits such as smoking and alcoholism, concomitant congenital or acquired systemic pathologies. Patients who survive the acute event suffer a functional alteration of multiple body systems. The various cardiology associations recommend starting a rehabilitation process, pursuing the main objective of improving the patient’s health status. A negative consequence that can be linked to MI is the dysfunction of the main breathing muscle, the diaphragm. The diaphragm is essential not only for respiratory mechanisms but also for adequate production of cardiac pressures. Post-MI patients present a reduction in the performance of the diaphragm muscle, and this condition can become a risk factor for further relapses or for the onset of heart failure. The article reviews the rehabilitation path for post-MI patients, to highlight the absence given to the diaphragm in the recovery of the patient’s health status. The text reviews the post-MI diaphragmatic adaptation to highlight the importance of including targeted training for the diaphragm muscle in the rehabilitation process.Keywords: myocardial infarction, diaphragm, cardiovascular disease, breathing, physiotherapy, cardiac rehabilitation

Abstract

The self-energy of the photo-hole in 2H-TaSe2 is measured by angle-resolved photoemission spectroscopy (ARPES) as a function of binding energy and temperature. In the charge-density wave (CDW) state, a structure in the self- energy is detected at ~ 65 meV that can not be explained by electron-phonon scattering. A reduction in the scattering rates below this energy indicates the collapse of a major scattering channel with the formation of the CDW state, accompanying the appearance of a bosonic "mode" in the excitation spectrum of the system., Comment: 4 pages, 4 eps figures, minor changes, references added

Abstract

Background The worldwide use of prone position (PP) for invasively ventilated patients with COVID-19 is progressively increasing from the first pandemic wave in everyday clinical practice. Among the suggested treatments for the management of ARDS patients, PP was recommended in the Surviving Sepsis Campaign COVID-19 guidelines as an adjuvant therapy for improving ventilation. In patients with severe classical ARDS, some authors reported that early application of prolonged PP sessions significantly decreases 28-day and 90-day mortality. Methods and analysis Since January 2021, the COVID19 Veneto ICU Network research group has developed and implemented nationally and internationally the "PROVENT-C19 Registry", endorsed by the Italian Society of Anesthesia Analgesia Resuscitation and Intensive Care. . .'(SIAARTI). The PROVENT-C19 Registry wishes to describe 1. The real clinical practice on the use of PP in COVID-19 patients during the pandemic at a National and International level; and 2. Potential baseline and clinical characteristics that identify subpopulations of invasively ventilated patients with COVID-19 that may improve daily from PP therapy. This web-based registry will provide relevant information on how the database research tools may improve our daily clinical practice. Conclusions This multicenter, prospective registry is the first to identify and characterize the role of PP on clinical outcome in COVID-19 patients. In recent years, data emerging from large registries have been increasingly used to provide real-world evidence on the effectiveness, quality, and safety of a clinical intervention. Indeed observation-based registries could be effective tools aimed at identifying specific clusters of patients within a large study population with widely heterogeneous clinical characteristics. 7. Healthcare Students' and Educators' Views on the Integration of Gender-Based Violence Education into the Curriculum: a Qualitative Inquiry in Three Countries

Abstract

Purpose Health and social care professionals are ideally placed to identify and address gender-based violence (GBV), yet research continues to demonstrate that the subject is being poorly covered at undergraduate level. This qualitative study explored health and social care students’ and educators’ views on GBV education, with a view to identifying ‘best practice’. We aimed to capture students’ and educators’ experiences and perceptions of GBV education across participating countries; how participants thought GBV should be taught/learned within their curricula; and their views on how GBV education might be ‘optimized’. Methods We conducted nine focus group discussions and one semi-structured interview with 23 students and 21 academic staff across the UK, Australia and Chile. Results Thematic analysis yielded three themes: (1) GBV addressed in all but name, (2) Introduce sooner, explore later and (3) A qualitative approach to learning. Educators and students indicated that GBV is largely being overlooked or incompletely addressed within curricula. Many participants expressed a wish for the subject to be introduced early and revisited throughout their study, with content evolving as cohorts mature. Lastly, our findings indicate that GBV education could benefit from adopting a ‘qualitative’ approach, prioritizing survivor narratives and incorporating dialogue to facilitate student engagement. Conclusion Though time constraints and competing demands within undergraduate curricula are frequently cited as barriers to moving away from traditional didactic methods, our findings suggest that teacher-centered strategies are insufficient and, in some regards, inappropriate for GBV education. The need for a paradigm shift in GBV education is discussed.

Abstract

Dopo un periodo in cui la pianificazione urbana e dei trasporti ha riconosciuto molta importanza alla velocità dello spostamento, al trasporto privato motorizzato e, più recentemente, a quello pubblico di massa, nel campo degli studi urbani e dei trasporti crescente attenzione è rivolta alla mobilità pedonale (walking) e alla pedonalità (walkability) degli ambienti urbani, ossia alla possibilità dei luoghi di essere raggiunti, fruiti, attraversati e visitati a piedi (Colleoni et al., 2017). In questo quadro, il contributo presenta i risultati di una ricerca sulle percezioni della pedonalità delle aree turistiche di un campione di residenti con 65 anni e più, residenti nel comune di Milano. L’obiettivo è di analizzare il rapporto tra tourist attractiveness e pedestrian friendliness dei territori e di individuare i gruppi di over 65 più vulnerabili sotto il profilo della pedonalità. Nella prima parte, verrà presentata una breve rassegna del rapporto tra popolazioni anziane, attrattività turistica e accessibilità pedonale e delle determinanti della pedonalità. In seguito, verranno presentati il metodo e i risultati per discutere, in sede conclusiva, il contributo dello studio per le politiche turistiche urbane che mirino a costruire città a misura d’anziano., After a period in which transport and urban planning attribute great importance to motorized private transport and, more recently, to public transport, nowadays increasing attention is paid to walking and the walkability of urban environments, namely the possibility of places and destinations to being reached, used, crossed and visited on foot. Walking is indeed crucial to be studied as it is an important mean for increasing the quality of life in cities, given its potential role in countering car-dependent urban development, improving the health status of citizens, and increasing the attractiveness and vitality of public spaces, accessibility to opportunities, social inclusion, and neighborhoods’ social relations. Research generally showed the relevance of environment’s characteristics such as land-use, urban design and furniture, landscape, etc. However, it is crucial to consider that the features enhancing the urban environments’ walkability may vary according to the needs, preferences, and perceptions of different urban populations, as in the case of tourists and the elders. The paper presents the results of a research on the perceptions about tourist areas’ pedestrian friendliness of a sample of over 65 years old residents living in Milan. The results show a general assonance between tourist attractiveness and the elderly people’ perceptions and a dissonance, namely in the case of women and oldest-old respondents. In this sense, we argue the need for urban policies and tourist policies to improve different walkability assessments in order to guarantee an increasing attractiveness and inclusiveness of the city of Milan.

11. Myopathy with DPP-4 inhibitors and statins in the real world: investigating the likelihood of drug–drug interactions through the FDA adverse event reporting system

Abstract

Aims: To investigate the occurrence of myopathy with oral glucose-lowering drugs (OGLDs) and statins, with a focus on dipeptidyl peptidase-4 inhibitors (DPP4-is). Methods: The FDA adverse event reporting system (FAERS) was queried (2004–2016) to compare the proportion of adverse events with OGLDs, alone and in combination with statins, using the reporting odds ratio (ROR) with relevant 95% confidence interval (95%Cl), adjusted for sex, age and concomitant presence of other OGLDs/lipid-lowering drugs. Drug–drug interaction is claimed whenever the frequency of an event is enhanced by combination treatment. Consistency/robustness of findings was tested by applying additive/multiplicative models and accounting for competition bias (i.e., adverse events previously known to be associated with OGLDs were removed). Results: Over a 13-year period, we retrieved 142,888 cases of myopathy. The use of DPP4-is alone was not associated with higher reporting of myopathy (no. of cases = 4898; adjusted ROR = 1.00; 95%CI = 0.96–1.04), with the notable exclusion of vildagliptin (262; 1.64; 1.42–1.88). No increased occurrence emerged when used in combination with statins, with consistent findings from additive/multiplicative models for all DPP4-is. Likewise, no increased reporting was found for other OGLDs (28,964; 0.64; 0.62–0.67); data on the interaction with statins were consistent/robust across analyses only for sulfonylureas (the interaction is likely and biologically plausible) and sodium glucose cotransporter-2 inhibitors. Conclusions: Real-world FAERS data do not raise concern for muscular toxicity with DPP4-is in combination with statins, making a drug interaction very unlikely.

Abstract

Chile ha logrado una reducción importante en los índices de salud perinatal, con un aumento de la importancia relativa de las malformaciones congénitas en la mortalidad perinatal e infantil. Para mantener esa tendencia es necesario desarrollar estrategias para reducir la mortalidad asociada a malformaciones congénitas. En respuesta a esta nueva situación, se creó el Centro de Referencia Perinatal Oriente (CERPO), constituido por un equipo multidisciplinario de profesionales. El objetivo principal es el trabajo coordinado para mejorar la calidad de atención y los resultados perinatales dependientes de malformaciones congénitas del área oriente y regiones. Para lograr esos objetivos se establece un modelo de atención integral, multidisciplinario, orientado a satisfacer sus necesidades. Se describe en detalle el modelo de atención CERPO y las actividades realizadas en el primer añoChile has achieved an important improvement in the indices of perinatal health, which has increased the impact of congenital malformations in perinatal and infant mortality. In order to maintain this reduction it is necessary to develop strategies to improve the mortality associated to congenital malformations. In answer to this new situation, it was created the "Centro de Referencia Perinatal Oriente" (CERPO). A multidisciplinary team of professionals. Our objective is to work coordinated in order to improve the quality of attention and perinatal results of pregnant women of Eastern Santiago and regions, whose fetuses present a congenital malformation that will require of neonatal attention in high complexity pediatrics centers. In order to achieve the objectives, a model of integral, multidisciplinary attention was settled, with the purpose of satisfy their necessities. Abstract

Background The worldwide use of prone position (PP) for invasively ventilated patients with COVID-19 is progressively increasing from the first pandemic wave in everyday clinical practice. Among the suggested treatments for the management of ARDS patients, PP was recommended in the Surviving Sepsis Campaign COVID-19 guidelines as an adjuvant therapy for improving ventilation. In patients with severe classical ARDS, some authors reported that early application of prolonged PP sessions significantly decreases 28-day and 90-day mortality. Methods and analysis Since January 2021, the COVID19 Veneto ICU Network research group has developed and implemented nationally and internationally the “PROVENT-C19 Registry”, endorsed by the Italian Society of Anesthesia Analgesia Resuscitation and Intensive Care…’(SIAARTI). The PROVENT-C19 Registry wishes to describe 1. The real clinical practice on the use of PP in COVID-19 patients during the pandemic at a National and International level; and 2. Potential baseline and clinical characteristics that identify subpopulations of invasively ventilated patients with COVID-19 that may improve daily from PP therapy. This web-based registry will provide relevant information on how the database research tools may improve our daily clinical practice. Conclusions This multicenter, prospective registry is the first to identify and characterize the role of PP on clinical outcome in COVID-19 patients. In recent years, data emerging from large registries have been increasingly used to provide real-world evidence on the effectiveness, quality, and safety of a clinical intervention. Indeed observation-based registries could be effective tools aimed at identifying specific clusters of patients within a large study population with widely heterogeneous clinical characteristics. Trial registration The registry was registered (ClinicalTrial.Gov Trials Register NCT04905875) on May 28,2021.

16. Hepatitis B vaccination and the putative risk of central demyelinating diseases – A systematic review and meta-analysis

Abstract

Background: The anti-hepatitis B immunization campaigns launched in the early 1990s were a major public health breakthrough and targeted various populations (at-risk adults, newborns, adolescents). However, debate is still active about a possible link between this vaccine and central demyelination. This study provides a pooled estimate of this risk based on a comprehensive review and meta-analysis of all available epidemiologic studies. Methods: A systematic review was conducted in Medline, Embase, ISI Web of Science and the Cochrane Library from database inception to 10 May 2017. Grey literature was searched and snowballing was also undertaken. Only observational studies including a control group were retained. Primary outcome was multiple sclerosis diagnosed by recognized criteria. Study selection was performed by two independent reviewers with disagreements solved through discussion. This meta-analysis based on crude, adjusted estimates, or risks limited to the 3 months following immunization was performed using a generic inverse variance random-effect model. Heterogeneity was investigated; sensitivity and subgroup analyses were performed when necessary. This study followed the PRISMA statement and the MOOSE reporting guideline (Study protocol registered in PROSPERO: CRD42015020808). Findings: Of the 2804 references reviewed, 13 studies with a control group were analysed. None of the pooled risk estimates for either multiple sclerosis or central demyelination following HB immunization reached statistical significance. When considering adjusted risk ratios, the following non-significant figures were obtained: 1.19 (95%CI: 0.93 – 1.52) and 1.25 (95%CI: 0.97 – 1.62), for multiple sclerosis and central demyelination, respectively. Conclusions: No evidence of an association between hepatitis B vaccination and central demyelination was found.

Abstract

Approved in 2006, human papillomavirus (HPV) vaccines were initially targeted for girls aged 9–14 years. Although the safety of these vaccines has been monitored through post-licensure surveillance programmes, cases of neurological events have been reported worldwide. The present study aimed to assess the risk of developing demyelination after HPV immunization by meta-analysing risk estimates from pharmacoepidemiologic studies. A systematic review was conducted in Medline, Embase, ISI Web of Science and the Cochrane Library from inception to 10 May 2017, without language restriction. Only observational studies including a control group were retained. Study selection was performed by two independent reviewers with disagreements solved through discussion. This meta-analysis was performed using a generic inverse variance random-effect model. Outcomes of interest included a broad category of central demyelination, multiple sclerosis (MS), optic neuritis (ON), and Guillain-Barré syndrome (GBS), each being considered independently. Heterogeneity was investigated; sensitivity and subgroup analyses were performed when necessary. In parallel, post-licensure safety studies were considered for a qualitative review. This study followed the PRISMA statement and the MOOSE reporting guideline. Of the 2,863 references identified, 11 articles were selected for meta-analysis. No significant association emerged between HPV vaccination and central demyelination, the pooled odds ratio being 0.96 [95% CI 0.77–1.20], with a moderate but non-significant heterogeneity (I2 = 29%). Similar results were found for MS and ON. Sensitivity analyses did not alter our conclusions. Findings from qualitative review of 14 safety studies concluded in an absence of a relevant signal. Owing to limited data on GBS, no meta-analysis was performed for this outcome. This study strongly supports the absence of association between HPV vaccines and central demyelination.

Abstract

Background Duchenne Muscular Dystrophy (DMD) is a rare disorder caused by mutations in the dystrophin gene. A recent systematic review and meta-analysis of global DMD epidemiology is not available. This study aimed to estimate the global overall and birth prevalence of DMD through an updated systematic review of the literature. Methods MEDLINE and EMBASE databases were searched for original research articles on the epidemiology of DMD from inception until 1st October 2019. Studies were included if they were original observational research articles written in English, reporting DMD prevalence and/or incidence along with the number of individuals of the underlying population. The quality of the studies was assessed using a STrengthening the Reporting of OBservational studies in Epidemiology (STROBE) checklist adapted for observational studies on rare diseases. To derive the pooled epidemiological prevalence estimates, a meta-analysis was performed using random-effects logistic models for overall and birth prevalence and within two different underlying populations (i.e. all individuals and in males only), separately. Heterogeneity was assessed using Cochran’s Q-test along with its derived measure of inconsistency I2. Results A total of 44 studies reporting the global epidemiology of DMD were included in the systematic review and only 40 were included in the meta-analysis. The pooled global DMD prevalence was 7.1 cases (95% CI: 5.0–10.1) per 100,000 males and 2.8 cases (95% CI: 1.6–4.6) per 100,000 in the general population, while the pooled global DMD birth prevalence was 19.8 (95% CI:16.6–23.6) per 100,000 live male births. A very high between-study heterogeneity was found for each epidemiological outcome and for all underlying populations (I2 > 90%). The test for funnel plot asymmetry suggested the absence of publication bias. Of the 44 studies included in this systematic review, 36 (81.8%) were assessed as being of medium and 8 (18.2%) of low quality, while no study was assessed as being of high quality. Conclusions Generating epidemiological evidence on DMD is fundamental to support public health decision-making. The high heterogeneity and the lack of high quality studies highlights the need to conduct better quality studies on rare diseases.

Abstract

To estimate the risk of sudden cardiac death (SCD) or sudden unexpected death (SUD) related to individual antipsychotics, a meta-analysis of observational studies was performed. Adjusted odds ratio (OR) of SCD/SUD with 95% confidence intervals (CI) were extracted and pooled; heterogeneity was studied using Q statistic and I(2) index, and its potential causes (e.g., hERG blockade potency) explored using meta-regression. Two cohort (740,306 person-years) and four case-control (2,557 cases; 17,670 controls) studies, investigating nine antipsychotics, were included. Compared with nonusers, the risk was increased for quetiapine (OR = 1.72, 95% CI: 1.33-2.23), olanzapine (OR = 2.04, 1.52-2.74), risperidone (OR = 3.04, 2.39-3.86), haloperidol (OR = 2.97, 1.59-5.54), clozapine (OR = 3.67, 1.94-6.94), and thioridazine (OR = 4.58, 2.09-10.05). Heterogeneity was found (Q = 20.0, P = 0.01; I(2) = 60.0%), and the increasing mean hERG blockade potency (P = 0.01) accounted for 43% of this. The SCD/SUD risk differed between individual antipsychotics, and mean hERG blockade potency could be an explanatory factor. This should be considered when initiating antipsychotic treatment.

Abstract

Appraisals are based on regression models. Its major application limits are all in situations in which the real estate market doesn’t offer a sufficient number of comparables to implement statistical tools. In low-transaction real estate markets, the most effective appraisal method is the market comparison approach (MCA). The application limits of MCA depend on the degree of similarity between the properties’ characteristics and the reliability of the prices of comparables. In this paper, we introduce some rational measurements in order to guarantee a scientific approach to MCA is associated with the correct prices. Our method is able to restore appraisals of the market value of any type of property, even in situations in which the comparables are limited or when the reliability of the buying and selling prices could be better.

Prospective randomized study of early versus late enteral iron supplementation in children who weighed < 1,301 grams at birth Objective: Determine whether early iron supplementation would decrease the need, the number and volume of transfused red blood cells in relation to late iron supplementation in children with birth weight less than 1,301 g. Patients and Methods: Very low birth weight (VLBW) infants were randomly assigned to receive early iron supplementation of 3 mg/kg/day as soon as they could tolerate enteral feeding of 100 ml/kg/day, or at 61 days of life as late supplementation. Hemoglobin levels were measured at the beginning of early iron supplementation and at 2 months of age. The red blood cell transfusion was performed according to transfu-sion guidelines and erythropoietin was not administered. Results: No differences were observed regarding the number of red cell transfusions between the groups. Morbidities associated with prematurity presented no significant differences. Abstract

The appraisal value is an appraiser's opinion (not determination) of the current worth of a property on the real estate market. The assessed value is the value placed on real estate property by government assessors for determining ad valorem taxes. Assessed value is used rarely as basis for appraisal value. The Italian appraisal practice is characterized by valuations developed in subjective opinions formulated by the valuers, according to the experience and the competence rather than on the survey of the market data of comparable properties. These opinions are indicated by the term expertise, borrowed from the estimates made in the art market. In Italy, the valuation of the properties applies a form of expertise based on subjective opinion rather than on the market prices. The basis of the expertise is one synthetic estimate of the value based on a single parameter, often derived from generic list of interval values quotations.

Abstract

Seismic networks often record signals characterized by similar shapes that provide important information according to their geographic positions. We propose an approach to identify homogeneous clusters of seismic waves, combining analysis of waveforms with metadata and spectrogram information. In waveforms clustering, cross-correlation measures between signals may presents some limitations, so we refer to more recent contributes relating data-depth based clustering analysis. The mechanism for alignment is also an important topic of the analysis: warping (or aligning) procedures identify nuisance effects in phase variation, that, if ignored, may result in a possible loss of information and the immediate consequence is that the underlying pattern could not be retained. The effectiveness of the approach is investigated by mean of real data. The data consist of a set of recordings of 21 earthquakes in the Centre of Italy with magnitude more than 5.5 mw, provided by the seismic network RAN (Rete Accelerometrica Nazionale) managed by the Italian Department of Civil Protection, are obtained from ESM/ITACA database (esm.mi.ing.it; itaca.mi.ingv.it).The signals were recorded by stations, whose distances from the epicenter are in the range from 50 to 100 km. The goal is dividing the spatial domain into homogeneous clusters and extracting information from the shapes of the underlying curves. Abstract

The knowledge of real estate prices trend over time is requisite to understand and expect dynamics of real estate market, and is useful to construct index numbers of real estate prices, numerical indicators exploitable to understand real estate profitability and land phenomena in different market segments. This work aims at the construction of prices index numbers relevant to building area site in the town of Paternò, in Catania province, and at the corresponding hedonic prices index numbers linked to the main real estate characteristics in the space of a period that goes from 2004 to 2012. The proposed methodology allows to build the index numbers of the building area’s unit prices in the complex Italian real estate market, in order to explain appreciation/depreciation of the relative rates and hedonic prices index numbers of real estate characteristics. Index numbers building is done with the method known as Sistema Generale di Stima (SGS), in order to simulate the technique of repeated sales.

Abstract

The appraisal smoothing is the phenomenon in which the valuation of a property, based on the judgments of appraisers (expertise) rather than on the collection of market data, is influenced by previous valuations performed by the same expert. The most obvious consequence of the exercise of expertise is the tendency to generate in the time a smoothing in the estimated value. This is the first experimental application of the appraisal smoothing theory in Italy. The general spread of real estate valuations, based on prices and average prices refer to a single commercial parameter (usually the surface), generates the natural suspicion that the smoothing effect could be widely disseminated and the practical certainty of his presence. The study aims to measure this effect through an experimental case using a sample of urban real estate sales for the period of five years. The research applies deterministic and econometric models to counteract the appraisal smoothing based on the results of the statistical analysis of the data, on the basis of literature and direct observation of the phenomenon.

Abstract

Introduction: Spontaneous reporting systems (SRSs) are pivotal for signal detection, especially for rare events with a high drug-attributable component, such as torsade de pointes (TdP). Use of different national SRSs is rarely attempted because of inherent difficulties, but should be considered on the assumption that rare events are diluted in international databases. Objective: The aim was to describe TdP-related events associated with antipsychotics, H1-antihistamines and anti-infectives in three national SRSs (in Italy, Germany and France) and highlight potential signals of torsadogenicity through a combined literature evaluation. Methods: A common search strategy was applied to extract TdP-related events: (1) TdP, (2) QT interval abnormalities, (3) ventricular fibrillation/tachycardia, and (4) sudden cardiac death. Signals of disproportionate reporting (SDRs) were calculated for TdP + QT interval abnormalities and defined by a lower limit of the 95 % confidence interval of the reporting odds ratio (ROR) >1. Among SDRs with at least three cases without concomitant pro-arrhythmic drugs, we defined potential new signal of torsadogenicity as drugs with no published evidence from (a) the crediblemeds® website (http://www.crediblemeds.com, as of November 1st, 2014); (b) studies on the FDA Adverse Event Reporting System (FAERS); and (c) safety trials or pharmaco-epidemiological studies (as of December 16th, 2014). Results: Overall, 3505 cases were retrieved (1372, 1468, and 801 for France, Germany and Italy, respectively). Antipsychotics were mainly recorded in Germany (792 cases), whereas antibiotics peaked at 515 and 491 (France and Italy, respectively). Forty-one drugs met criteria for SDRs in at least one single source, of which 31 were detected only from one single SRS: 18, ten and three (French, German and Italian SRS, respectively). Abstract

Little is known of cancer rehabilitation needs in Europe. EUROCHIP-3 organised a group of experts to propose a list of population-based indicators used for describing cancer rehabilitation across Europe. The aim of this study is to present and discuss these indicators. A EUROCHIP-3 expert panel reached agreement on two types of indicators. (a) Cancer prevalence indicators. These were proposed as a means of characterising the burden of cancer rehabilitation needs by time from diagnosis and patient health status. These indicators can be estimated from cancer registry data or by collecting data on follow-up and treatments for samples of cases archived in cancer registries. (b) Indicators of rehabilitation success. These include: return to work, quality of life, and satisfaction of specific rehabilitation needs. Studies can be performed to estimate these indicators in individual countries, but to obtain comparable data across European countries it will be necessary to administer a questionnaire to randomly selected samples of patients from population-based cancer registry databases. However, three factors complicate questionnaire studies: patients may not be aware that they have cancer; incomplete participation in surveys could lead to bias; and national confidentiality laws in some cases prohibit cancer registries from approaching patients. Although these studies are expensive and difficult to perform, but as the number of cancer survivors increases, it is important to document their needs in order to provide information on cancer control. 48. Transplantation for acute liver failure in patients exposed to NSAIDs or paracetamol (Acetaminophen): The multinational case-population SALT study

Abstract

Background: Most NSAIDs are thought to be able to cause hepatic injury and acute liver failure (ALF), but the event rates of those leading to transplantation (ALFT) remain uncertain. Objectives: The aim of the study was to estimate population event rates for NSAID-associated ALFT Methods: This was a case-population study of ALFT in 57 eligible liver transplant centres in seven countries (France, Greece, Ireland, Italy, The Netherlands, Portugal and the UK). Cases were all adults registered from 2005 to 2007 for a liver transplant following ALFT without identified clinical aetiology, exposed to an NSAID or paracetamol (acetaminophen) within 30 days before the onset of clinical symptoms. NSAID and paracetamol population exposures were assessed using national sales data from Intercontinental Marketing Services (IMS). Risk was estimated as the rate of ALFT per million treatment-years (MTY). Results: In the 52 participating centres, 9479 patients were registered for transplantation, with 600 for ALFT, 301 of whom, without clinical aetiology, had been exposed to a drug within 30 days. Of these 301 patients, 40 had been exposed to an NSAID and 192 to paracetamol (81 of whom were without overdose). Event rates per MTY were 1.59 (95 % CI 1.1-2.2) for all NSAIDs pooled, 2.3 (95 % CI 1.2-3.9) for ibuprofen, 1.9 (95 % CI 0.8-3.7) for nimesulide, 1.6 (95 % CI 0.6-3.4) for diclofenac and 1.6 (95 % CI 0.3-4.5) for ketoprofen. For paracetamol, the event rate was 3.3 per MTY (95 % CI 2.6-4.1) without overdoses and 7.8 (95 % CI 6.8-9.0) including overdoses. Conclusions: ALF leading to registration for transplantation after exposure to an NSAID was rare, with no major difference between NSAID. Non-overdose paracetamol-exposed liver failure was twice more common than NSAID-exposed liver failure. © 2013 The Author(s).

Abstract

The aim of this paper was to report clinical, functional and radiological results of 80 patients surgically treated with a combined mini-open and percutaneous surgical repair as proposed by Kakiuchi.All patients were evaluated with a physical examination, evaluation scales, a functional test (Ergo-jump Bosco System), and an ultrasonographic exam along with Power Doppler Ultrasonography (PDU) (S/S).At a mean follow-up of 58 months no cases of rerupture were detected. VISA-A evaluation scale showed an excellent score in 63 patients (78.75%), a good score in 14 patients (17.5%), a fair score in two patients (2.5%), and a poor score in one patient (1.25%). Hannover scale showed an excellent score in 63 patients (78.75%), and a good score in 17 patients (21.25%). Ergo-Jump evaluation showed a 2.07% mean deficit of the affected limb at the Squatting Jump test, a 3.26% mean deficit at the Counter Movement Jump test, and a 0.0062% mean improvement at the Repetitive Jump test. Ultrasonographic exam showed in all cases a satisfactory recovery of the integrity of the operated tendon. The mean AP and LL widths showed a significant increase of 7.13±2.97 mm (+56.1%) and of 4.01±2.36 mm (+43.81%) respectively. According to the modified Öhberg score scale, PDU exam showed a grade +1 in 16 patients (20%) and a grade +2 in seven cases (8.7%).The absence of rerupture cases, the satisfactory functional and ultrasonographic results of the patients included in this study cause us to consider this technique as reliable and effective even in young high-demand patients.
