Amac: Akut alt solunum yolu enfeksiyonlari (ASE) 5 yas alti cocuklardaki olumlerinin en sik nedenini olusturur. Bu calismada akut ASE tanisi alan hastalarda serum 25 (OH) vitamin D duzeylerinin yas gruplarina gore ince lenmesi planlanmistir. Metod: Calismaya Saglik Bilimleri Universitesi Ankara Kecioren Egitim ve Arastirma Hastanesi’nde Temmuz 2018-Temmuz 2019 tarihleri arasinda ASE tanisi ile izlenen 1 ay-18 yas araliginda cocuk hastalar dahil edildi. Hastalarin hastaneye basvuru sikayeti, sosyodemografik ozellikleri, serum 25 (OH) vitamin D duzeyi, hastaneye yatis oykusu ve yatis suresi kaydedildi. Bulgular: Calismaya ortanca yasi 4 yas (1 ay-17 yas) olan 65’i (%61,9) erkek (E/K:1,6) toplam 105 hasta dahil edildi. Yirmi dokuz (%27,6) hasta yatirila rak, 76 (%72,6) hasta ayaktan takip edildi. Hastalarin 44’u viral akut alt solunum yolu enfeksiyonu, 61’i bakteriyel akut alt solunum yolu enfeksiyo nu tanisi ile izlenmistir. Serum 25 (OH) vitamin D duzeyi hastalarin 63’unde dusuk ( 20 ng/mL olarak saptandi. Calismaya dahil edilen 1 yas altindaki 32 hastanin kis mevsiminde dogan larinda tani aninda ortanca serum 25 (OH) vitamin D duzeyleri, diger mevsimlerde dogan 1 yas altindaki hastalara gore daha dusuk saptandi. Bir yasindan kucuk ve akut ASE nedeni ile yatirilarak tedavi edilen hastalarin serum ortanca 25 (OH) vitamin D duzeyi [25,9 ng/mL (1,7-87,7)], ayaktan tedavi edilen 1 yasindan kucuk hastalarinkine [42,1 ng/mL (16,6-62,6)] gore daha dusuk bulundu (p=0,035). Sonuc: Calismamizda akut ASE tanisi ile izlenen hastalarda serum vitamin D eksikligi/yetersizliginin sik olarak saptanmasi, vitamin D dusuklugunun cocuklarda akut ASE’nin gelisiminde ve ilerlemesinde onemli bir risk fakto ru oldugunu dusundurmustur.

ÖZET Amaç: Kawasaki hastalığı çocukluk çağının en yaygın vaskülitlerinden biridir. Bu çalışmada son 10 yılda pediatri kliniğinde Kawasaki hastalığı tanısı alan hastaların geriye dönük olarak değerlendirilmesi amaçlanmıştır. Materyal-Metod: Bu çalışmada, 2007-2017 yılları arasında tanı kriterlerine göre Kawasaki hastalığı tanısı almış olan 44 olgunun demografik ve klinik özellikleri, ekokardiyografi bulguları ve laboratuvar bulguları, tedavi ve izlem sonuçları dosya kayıtlarından geriye dönük olarak incelendi. Sonuçlar: Çalışmaya 24 (54%) erkek, 20 (%46) kız olmak üzere toplam 44 olgu dâhil edildi. Tanı esnasında hastaların ortalama yaşı 49,2±31,4 (7-160) aydı. Ortalama ateş süresi 8,1±2,1gün iken ortalama hastane yatış süresi 8,4gün ±2,9 gün olarak tespit edildi. Tanı alan olguların 36 (%81) komplet Kawasaki hastalığı, 8 (%19) hasta inkomplet Kawasaki hastalığı olarak değerlendirildi. Bazı olgularda serum sodyum, albümin ve/veya D vitamini düzeyi düşüklüğü ve/veya sterilpiyüri saptandı. Olguların hepsine yüksek doz intravenöz immünoglobulin (2 g/kg) ve antienflamatuvar dozda aspirin (80-100 mg/kg/gün) başlandı. İki olguya IVIG sonrası ateşin devam etmesi üzerine tekrarlayan intravenöz immünoglobulin infüzyonu verildi. Olguların tamamında klinik ve laboratuar bulguları tedavi ile düzelirken yalnızca bir olguda rekurrens gelişti. Tartışma: Kawasaki hastalığının erken dönemde tanı alması, bu hastalığa bağlı gelişebilecek ciddi komplikasyonları önleme açısından oldukça önemlidir. Hiponatremi, hipoalbüminemi, artmış serum transaminaz düzeyleri ve/veya steril piyüri varlığının Kawasaki hastalığıtanısını destekleyen laboratuvar bulguları olduğu düşünüldü.

Amaç: Migren çocuk ve ergen yaş grubunda değişen yaşam tarzları ve psikososyal faktörler ile birlikte görülme sıklığı giderek artan önemli bir klinik problemdir. Birincil baş ağrıları arasında yer alan migrenin biyokimyasal, genetik ve çevresel faktörlerle ilişkili nörovasküler bir hastalık olduğu bilinmektedir. Son yıllarda yapılan birçok çalışmada migren patogenezinde oksidatif stres sorumlu tutulmuştur. Oksidatif stres varlığında tiyol/disülfit homeostazisinin disülfit yönünde bozulması beklenmektedir. Tiyol/disülfit homeostazisinin çocuklarda migren ile olan ilişkisini araştıran geniş çaplı araştırmalar mevcut değildir. Çalışmamızda migren tanılı çocuklarda dinamik tiyol/disülfit homeostazisi ilişkisini belirlemeyi ve bu dengede disülfit yönünde bozulma görülürse migren proflaksisi ve/veya tedavisinde oksidatif stresten korunma ve/veya antioksidan tedavi kullanımı ile ilgili literatüre katkı sağlanmasını amaçladık.Gereç ve Yöntemler: Çalışmaya Sağlık Bilimleri Üniversitesi, Ankara Keçiören Eğitim ve Araştırma Hastanesi’nde Çocuk Nöroloji Polikliniği’nde değerlendirilerek migren tanısı alan 8-18 yaş arasındaki 71 hasta ve Genel Çocuk Polikliniği’ne başvuran 70 sağlıklı çocuk dahil edilmiştir. Toplanan örneklerde serum doğal tiyol, total tiyol, disülfit, disülfit/doğal tiyol yüzdesi, disülfit/total tiyol yüzdesi ve doğal tiyol/total tiyol yüzdesi ölçümleri yapılarak migren hastaları ve sağlam çocuklar karşılaştırılmıştır.Bulgular: Migren sıklığı yaşla birlikte artmış; 8-12 yaş %18.3, 13-15 yaş %36.6, 16-18 yaş %45.1 olarak tespit edilmiştir. Ağrı şekli %85.9 ile en sık zonklayıcı olup ağrıların %59.2’si bilateral bulunmuştur. Hastaların %11.3’ü atak ağrısının 30 dakikadan kısa sürdüğünü belirtmiştir. Gruplar karşılaştırıldığında doğal tiyol, doğal tiyol/total tiyol değerleri kontrol grubunda anlamlı olarak yüksek saptanmıştır (sırasıyla p=0.022, p=0.005). Disülfit, disülfit/doğal tiyol, disülfit/total tiyol değerleri ise hasta grubunda anlamlı olarak yüksek bulunmuştur (sırasıyla p=0.039, p=0.005, p=0.005). İstatistiksel olarak anlamlı olmasa da kontrol grubunda total tiyol değerleri hasta grubundan yüksek bulunmuştur (p=0.115).Sonuç: Migren hastalığı sık görülmesine rağmen çocuklarda bu konuda yapılmış prospektif çalışma oldukça az sayıdadır. Çalışmamızda migren tanılı çocuklarda migren hastalığı ile tiyol/disülfit homeostazisi arasındaki ilişki incelenmiş olup oksidatif strese yönelik parametreler yüksek bulunmuştur. Migren hastalarındaki yüksek oksidatif stres değerleri antioksidan etkili maddelerin hastalığın tedavisinde rol oynayabileceğini düşündürmektedir. Bu konuyla ilgili destekleyici çalışmalara ihtiyaç vardır., Objective: Migraine is a common disease in childhood. Oxidative stress has been implicated in the pathogenesis of migraine. Dynamic thiol/disulfide homeostasis is proven to be a marker of oxidative stress. We aimed to investigate the correlation between migraine and dynamic thiol/disulfide homeostasis. Material and Methods: A total of 141 children (71 migrain and 70 controls) were included. The serum total thiol, native thiol, and disulfide levels were measured and the ratios of disulfide/native thiol, disulfide/total thiol and native thiol/total thiol were compared between migraine patients and healthy children during attack-free period.Results: Native thiol levels and native thiol/total thiol ratio were significantly lower in the migraine group than the control group (p=0.022, p=0.005, respectively); whereas disulfide levels, disulfide/native thiol and disulfide/total thiol ratios were significantly higher in the migraine group than the control group (p=0.039, p=0.022, p=0.005 respectively).Conclusion: Our results demonstrate that there is an ongoing oxidative process in pediatric migraineurs even during attack-free period. This result may shed light on further studies analyzing dynamic changes in the oxidant-antioxidant balance during the attack and the aura phase to support the presence and importance of oxidative stress in pediatric migraine.

Objective: Lower respiratory tract infections (LRTI) are the most common reason of mortality for under 5 years old. We aimed to examine serum 25 (OH) vitamin D level of children with LRTI according to age, time of admission and gender. Methods: Children between 1 months-18 years of age with a diagnosis of LRTI admitted to the Health Science University Keciören Research and Training Hospital between July 2018 and July 2019 were included. Sociodemographic characteristics, serum 25 (OH) vitamin D levels, history of recurrent LRTI, and hospitalisation time were recorded. Results: A total of 105 patients, 65 (61.9%) male (M/F:1.6) with a median age of 4 years (1 month-17 years) were included. Twenty nine patients (27.6%) were hospitalised and 76 (72.6%) patients were outpatients. Of them, 44 patients were diagnosed with viral acute lower respiratory tract infection and the remaining 61 with bacterial acute lower respiratory tract infection. Serum 25 (OH) vitamin D level was low in 63 patients (20 ng/mL. In 32 patients under 1 year old of age, median serum 25 (OH) vitamin D levels were lower in patients born in winter compared to patients born in other seasons. The serum median 25 (OH) vitamin D level of hospitalised children under 1 year old of age was statistically significantly lower than in outpatients of the same age group, [25.9 ng/mL (1.7-87.7) versus 42.1 ng/mL (16.6- 62.6), p=0.035]. Amaç: Günümüzde çocukluk çağında obezitenin yaygınlaşmasıyla birlikte hipertansiyon görülme sıklığı artmaktadır. Ayaktan (ambulatuvar) kan basıncı (KB) monitörizasyonu (AKBM) hedef organ hasarı riskini ofis ölçümünden daha iyi öngörmektedir. Bu çalışmada KB yüksekliği nedeni ile izlenen olguların ofis KB parametreleri ile AKBM sonuçlarının karşılaştırılması ve Amerikan Kalp Birliği’nin 2014 yılında güncellediği çocuklarda AKBM raporuna göre olguların değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Ofis KB değerleri yüksek bulunan (>90 persentil) ve AKBM uygulanan olguların kayıtları retrospektif olarak incelendi. İkincil hipertansiyonu olan veya AKBM sırasında antihipertansif ilaç kullanmakta olan hastalar çalışma dışında tutuldu. Olguların ofis KB ve AKBM sonuçları Amerikan Kalp Birliği’nin 2014’de güncellediği çocuklarda AKBM raporuna göre evrelendirildi. Olguların demografik bilgileri, vücut kitle indeksi (VKİ), VKİ-standart deviasyon skoru (SDS) ve AKBM sonuçları istatistiksel olarak karşılaştırıldı. Bulgular: Çalışmaya 31 erkek, 26 kız olgu alınmış olup, olguların yaş ortalaması 14.42±2.14 yıldı. Ofis KB ve AKBM sonuçlarına göre, olguların %63’ünde (n=36) beyaz önlük hipertansiyonu, %15.7’inde (n=9) ambulatuvar hipertansiyon, % 12.2’sinde (n=7) ciddi ambulatuvar hipertansiyon, %5.3’ünde (n=3) prehipertansiyon ve %3,5’inde (n=2) maskelenmiş hipertansiyon saptandı. Prehipertansiyonu olan olguların 2’sinde (n=2/3, %66.7) ve ambulatuvar hipertansiyonu olan olguların 10’unda (n=10/18, %55) obezite mevcuttu. Yirmi dört saatlik ortalama sistolik ve diyastolik KB arasında pozitif korelasyon belirlendi. Olgularda VKİ ile 24 saatlik ortalama sistolik KB, gece sistolik KB ve 24 saatlik ortalama arter basıncı değerleri arasında da pozitif korelasyon saptandı. Sonuç: Ofis KB yüksek olan ve hipertansiyon için risk faktörü taşıyan çocukların izleminde AKBM uygulaması önerilir.

Turk J Pediatr 2019; 61: 292-296. Bismuth intoxication is a rare cause of acute kidney injury (AKI) and is usually reversible by appropriate therapeutic measures. We present here a case of an adolescent pregnant girl who developed AKI due to an overdose of colloidal bismuth subcitrate (CBS, total amount of 6 g). She received parenteral chelating agent dimercaprol for 14 days. Continuous venovenous hemodiafiltration (CVVHD) with high-flux membrane was carried out in the first 3 days of chelating therapy and intermittent hemodialysis for 11 days, thereafter. The patient recovered clinically and was discharged after 21 days. She gave birth to a healthy term boy. At the last visit, the baby was 6 months old with normal growth and development as well as normal kidney functions. Neither deterioration in renal functions nor emergence of proteinuria was recorded in the patient during follow-up care after hospital discharge. In cases of AKI due to an overdose of CBS, treatment with dimercaprol combined with high flux hemodiafiltration and subsequently hemodialysis appears to be both useful and safe for bismuth elimination.

Objective Congenital nephrogenic diabetes insipidus (CNDI) is a rare inherited disorder characterized by a renal insensitivity to arginine vasopressin (AVP). In the majority of the cases, CNDI is caused by mutations in the arginine vasopressin receptor 2 (AVPR2) gene. Our objective is to report a novel mutation in the AVPR2 gene causing CNDI in a 6-year-old boy, presenting with growth failure and dull normal cognitive functions. Methods The proband was the third off-spring of non-consanguineous parents and had polyuria (4.3 L/day), polydipsia (5 L/day). The diagnosis of CNDI was established by a water-deprivation test and a desmopressin challenge test. Genetic studies were also carried out in the mother, siblings and affected family members, since excessive fluid intake and diuresis were also reported in these individuals. All exons of the AVPR2 gene for all participants were amplified and sequenced. Bioinformatics analysis for wild-type and mutant AVPR2 were obtained with Swiss-Model and UCSF Chimera 1.10.2. Results A novel, hemizygous, missense mutation was identified at the position 80th in exon 2 (p.H80Y) of AVPR2 in the proband. The proband’s mother, maternal aunt and grandmother were heterozygous and his maternal uncle was hemizygous for this mutation. Bioinformatic analysis indicates this mutation would cause significant conformational changes in protein structure. Conclusion p.H80Y mutation will cause inappropriate folding of the protein compromising water homeostasis via AVPR2 and AVP and leading to diabetes insipidus. We suggest that future functional investigations of the H80Y mutation may provide a basis for understanding the pathophysiology of the NDI in patients with this variant.

Giriş: Ebeveyn anketleri, çocukların gelişimini değerlendirmek amacıyla çocuk sağlığı ve hastalıkları polikliniklerinde sıkça kullanılmaktadır. Bu çalışmada annenin, yaşı ve eğitim durumunagöre çocuğunun gelişimi hakkında bilgi düzeyi ile standart gelişimsel tarama testi sonuçları karşılaştırılmış ve toplumumuzda çocukların gelişiminin değerlendirilmesinde ebeveyn anketlerinin doğruluğu değerlendirilmiştir.Materyal ve Metod: Annelere Türk çocuklarına uyarlanan ve standardize edilen Denver II gelişimsel tarama testindeki çocuğun kronolojik yaşına uygun maddeler soruldu ve cevapları kaydedildi. Daha sonra çocuklara Denver II gelişimsel tarama testi, sertifikalı çocuk gelişim uzmanı veya çocuk sağlığı ve hastalıkları uzmanı tarafından uygulandı. Annelerin ankete verdiği cevaplar ile Denver II gelişimsel tarama testi sonuçları karşılaştırıldı, birbiriyle uyumlu ve uyumsuz maddeler kaydedildi.Bulgular: Çalışmaya 178 anne ve çocuk dahil edildi. Anne raporu ve test sonuçları olguların %55’inde uyumluydu. Anne yaşı ve eğitim düzeyine göre belirlenmiş gruplarda bu uyum benzer oranlardaydı. Uyumsuz maddelerin dağılımı her gelişim alanında benzer orandaydı.Tartışma:Bu çalışmada anne raporu ile standart gelişimsel tarama testi sonucu arasındaki uyum yetersiz bulunmuştur. Diğer gelişim testleri ve okul performansı ile korelasyon gösterdiği saptanan Denver II gelişimsel tarama testinin anne raporlarına göre daha güvenilir olduğu düşünülmüştür. Gelişimselgecikmelerin belirlenmesi için anne raporları ve Denver II testi sonuçlarının, Türk toplumuna uyarlanmış ve standardize edilmiş diğer testlerle doğrulanması gerekmektedir., Background: Parental questionnaires are extensively used in pediatric outpatient clinics for developmental evaluation. Our purpose was to examine the accuracy of questionnaires in our population by comparing developmental screening test results with mother's report in relation with her age and education.Methods: Mothers were interviewed using the items corresponding to their child's chronological age on the Denver II developmental screening test adapted for Turkish children. Then the Denver II test was administered to the child by a certified child development specialist or pediatrician. Mothers' answers to the questionnaire and the child's Denver II test results were compared and discrepancies were recorded.Results: The sample consisted of 178 mother-child dyads. Mothers' reports and test scores were concordant in 55% of cases, with similar rates in all maternal age and educational groups. Items subject to disagreement were distributed equally over all developmental domains.Conclusion: The rate of agreement between mother's reports and screening test results was unsatisfactory. While the Denver II, previously shown to correlate with other tests and school performance, is more likely to be accurate than mother's report, both data need to be confirmed by other tests standardized and adapted to the population in order to detect developmental delays.

OBJECTIVE: Henoch-Schönlein purpura HSP is the most common childhood vasculitis, characterized by purpuric rash, arthritis, gastrointestinal system and renal involvement. In this study, our aim is to analyze the epidemiological, clinical and laboratory data of the children with HSP being followed-up at the Department of Pediatrics, Keçiören Training and Research Hospital.METHODS: HSP patients who were hospitalized between November 2006 and November 2011 and followed-up for at least six months were enrolled in the study. The patient records were analyzed retrospectively. RESULTS: Ninety-one cases aged 2-17 years 48 girls, 43 boys were enrolled in the study. Purpuric rash was present in all of the patients. Gastrointestinal involvement was detected in 69.2% n=63 of the cases as the second common finding. However, invagination developed in only two cases 2.2% . Joint and renal involvement was present in 64.8% n=59 and 16.4% n=15 of the cases, respectively. Only 2.2% of the patients with renal involvement had nephrotic level of proteinuria n=2 . Rarer findings including testicular involvement and peripheric neuropathy were detected in 8.8% n=8 and %1.1 n=1 of the cases. Disease recurrence rate was low in our patient group 6.6%, n=6 .CONCLUSION: It is remarkable that gastrointestinal involvement was the second most common finding in our study. Serious complications emerged in the early term, mainly due to gastrointestinal involvement. Meanwhile disease recurrence and surgical complication rates were relatively low and the course of HSP was benign in most of the cases. Nevertheless, it should be remembered that long-term follow-up patients especially with renal involvement is of great importance., AMAÇ: Henoch-Schönlein Purpurası HSP purpurik döküntü, artrit, gastrointestinal ve renal tutulum ile karakterize, çocukluk çağının en sık görülen vaskülitidir. Bu çalışmada beş yıllık bir süreç içerisinde Keçiören Eğitim ve Araştırma Hastanesi Çocuk Sağlığı ve Hastalıkları Kliniği’nde HSP tanısı ile izleme alınan çocukların epidemiyolojik, klinik ve laboratuvar verilerinin değerlendirilmesi amaçlandı.YÖNTEMLER: Kasım 2006-Kasım 2011 tarihleri arasında HSP tanısı ile en az altı ay izlenmiş olan hastalar çalışmaya alındı. Hastaların dosya kayıtları retrospektif olarak değerlendirildi.BULGULAR: Yaş aralığı 2-17 yıl olan toplam 91 olgu 48 kız, 43 erkek değerlendirildi. Purpurik döküntü tüm olgularda ortak görülen tek bulguydu. Hastalarda ikinci sıklıkta gastrointestinal tutulumun ortaya çıktığı %69,2, n=63 , bununla beraber sadece iki olguda invajinasyon geliştiği görüldü %2,2 . Eklem tutulumu sıklığı %64,8 n=59 iken, olguların %16,4’ünde n=15 renal tutulum ve bu olguların yalnızca %2,2’sinde n=2 nefrotik düzeyde proteinüri saptandı. Olguların %8,8’inde n=8 testiküler tutulum ve %1,1’inde n=1 periferik sinir tutulumu gibi daha nadir bulgular geliştiği gözlendi. Çalışmamızda hastalığa bağlı nüks oranları düşük bulundu %6,6, n=6 .SONUÇ: Çalışmamızda purpuradan sonraki en sık bulgunun gastrointestinal sistem tutulumu olması dikkat çekiciydi. Ciddi komplikasyonlar erken dönemde yine gastrointestinal tutuluma bağlı olarak görüldü. Bununla beraber hastalığa bağlı cerrahi komplikasyon ve nüks oranları düşüktü. Ortalama on aylık izlem sonunda prognozun iyi olduğu ve tüm olguların herhangi bir sekel bırakmadan iyileştiği gözlendi. Yine de HSP’li hastalarda özellikle renal tutulum varlığında uzun süreli izlemin önem taşıdığı sonucuna varıldı.

Çocukluktan erişkinliğe geçiş dönemi olarak bilinen ergenlikdönemi; fiziksel, psikolojik ve sosyal olgunluğa erişmenin tamamlandığı birdönemdir. Ayrıca iskelet gelişmesinde yeterli düzeyde D vitaminialımı da gerekmektedir. Kan hacminin artmasında ve iskelet gelişiminde önemligörevi olan demir ihtiyacı artar. İlaveten C vitamininden yüksek gıdalar dademirin vücut tarafından kullanımını arttırır. Amaç: Günümüzde çocukluk çağında obezitenin yaygınlaşmasıyla birlikte hipertansiyon görülme sıklığı artmaktadır. Ayaktan (ambulatuvar) kan basıncı (KB) monitörizasyonu (AKBM) hedef organ hasarı riskini ofis ölçümünden daha iyi öngörmektedir. Bu çalışmada KB yüksekliği nedeni ile izlenen olguların ofis KB parametreleri ile AKBM sonuçlarının karşılaştırılması ve Amerikan Kalp Birliği’nin 2014 yılında güncellediği çocuklarda AKBM raporuna göre olguların değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Ofis KB değerleri yüksek bulunan (>90 persentil) ve AKBM uygulanan olguların kayıtları retrospektif olarak incelendi. İkincil hipertansiyonu olan veya AKBM sırasında antihipertansif ilaç kullanmakta olan hastalar çalışma dışında tutuldu. Olguların ofis KB ve AKBM sonuçları Amerikan Kalp Birliği’nin 2014’de güncellediği çocuklarda AKBM raporuna göre evrelendirildi. Olguların demografik bilgileri, vücut kitle indeksi (VKİ), VKİ-standart deviasyon skoru (SDS) ve AKBM sonuçları istatistiksel olarak karşılaştırıldı. Bulgular: Çalışmaya 31 erkek, 26 kız olgu alınmış olup, olguların yaş ortalaması 14.42±2.14 yıldı. Ofis KB ve AKBM sonuçlarına göre, olguların %63’ünde (n=36) beyaz önlük hipertansiyonu, %15.7’inde (n=9) ambulatuvar hipertansiyon, % 12.2’sinde (n=7) ciddi ambulatuvar hipertansiyon, %5.3’ünde (n=3) prehipertansiyon ve %3,5’inde (n=2) maskelenmiş hipertansiyon saptandı. Prehipertansiyonu olan olguların 2’sinde (n=2/3, %66.7) ve ambulatuvar hipertansiyonu olan olguların 10’unda (n=10/18, %55) obezite mevcuttu. Yirmi dört saatlik ortalama sistolik ve diyastolik KB arasında pozitif korelasyon belirlendi. Olgularda VKİ ile 24 saatlik ortalama sistolik KB, gece sistolik KB ve 24 saatlik ortalama arter basıncı değerleri arasında da pozitif korelasyon saptandı. Sonuç: Ofis KB yüksek olan ve hipertansiyon için risk faktörü taşıyan çocukların izleminde AKBM uygulaması önerilir., Aim: Prevalence of hypertension in youths is rising due to childhood obesity epidemic. Ambulatory blood pressure monitoring (ABPM) has superiority over office blood pressure (BP) measurement for distinguishing patients at higher risk for target-organ damage. We aim to compare the office BP levels with ambulatory levels in patients with prediagnosis of primary hypertension and interpret the data according to the recommendations of American Heart Society on the use of ABPM in the pediatric population reported in 2014.Results: The study included 31 boys and 26 girls. Mean age was 14.42±2.14 years. BP was categorized based on office and ABPM results into pre- (5.3%, n=3), white-coat (63%, n=36), masked (3.2%, n=2), ambulatory (15.7%, n=9) and severe ambulatory (12.2%, n=7) hypertension. Two of 3 patients with prehypertension and 10 of 18 patients with ambulatory/severe ambulatory hypertension were obese. Positive correlation was found between 24-hour mean systolic and diastolic BP. BMI positively correlated with 24-hour mean systolic BP, night systolic BP, 24 hour mean arterial BP, respectively. Conclusion: We suggest that children with high office BP measurements and risk for hypertension should be evaluated with ABPM.

Objective: Headache is a common complaint in both adults and children. Although the majority of headaches in children are benign, they are still frightening for the parents. The aim of this study was to determine headache types, etiology, triggering factors, associated symptoms, and the necessity of brain imaging techniques in this patient group. Material and Methods: This study was conducted on children with headaches who presented to the pediatric neurology clinic of Kecioren Training Hospital from November 2014 to January 2015. The criteria defined by the International Headache Society were used to classify the headache types. results: One hundred patients consisting of 59 girls and 41 boys, with an age range of 3-17 years, were enrolled in this prospective study. The mean age on admission was 12.8±3.2 years. The duration of headache was more than 2 years in 31% of the patients. The leading cause of headache was migraine in 58 patients (58%), followed by tension type headache in 26 patients (26%). A relevant family history was present in 37 patients (44%) with primary headache. Six patients had cerebral magnetic resonance abnormalities. conclusion: The evaluation of headache should include not only a detailed history of the child but also detailed general and neurological examinations. Neuroimaging should not be part of a routine initial examiation of children with headache.

Aim: Thelaboratory and imaging methods are not sufficiently sensitive to determine precisely the neurotoxic effects of bilirubin in neonates. The neuron-specific enolase and calcium binding protein B, which are sensitive biomarkers of cellular damage in the central nerve system, were used in the present study to demonstrate possible neurotoxic effects of bilirubin below 20 mg/dL. We hypothesized that neuron-specific enolase and calcium binding protein B might be helpful for our purposes. Material and Methods: The present study included 33 full-term infants hospitalized for phototherapy treatment (patient group) along with 29 healthy full-term infants (control group). The serum bilirubin levels of the patient group were all below 20 mg/dl. Two serum samples were obtained from all 62 infants at an interval of at least 48hrs which were used for the measurement of bilirubin, calcium binding protein B, and neuron-specific enolase levels. Results: There was no significant difference in terms of the serum levels of calcium binding protein B between the patient and control groups but there was a significant difference of the serum levels of neuron-specific enolase between the groups. In addition, there were no significant changes in the levels of calcium binding protein B and neuron-specific enolase among the patient group before and after the phototherapy. Here we report two newborn cases in whom a chemical burn injury occurred in the genital region due to local application of benzalkonium chloride (zefiran™) prepared in an inappropriate concentration for the purpose of antisepsis before placing a collecting urinary bag.

Oculocerebrorenal syndrome of Lowe (OCRL) is a rare, X-linked disorder characterized by congenital cataracts, neonatal or infantile hypotonia, seizures, cognitive impairment, and renal tubular dysfunction. In this article, we report two maternal cousins with OCRL with a hemizygous p.Ala788Asp mutation in exon 22 of the OCRL gene. They presented with diverse features of selective proximal renal tubular defect and high serum levels of total cholesterol, low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C).

Bu çalışmada renal parankimal skarı olan ve olmayan primer vezikoüreteral reflüsü (VUR) olan hastalarda idrar beta 2 mikroglobulin (ß2M) ve nötrofil jelatinaz ilişkili lipokalin (NGAL) düzeyi ile birlikte, idrar konnektif doku büyüme faktörü (CTGF) düzeyinin renal zedelenmenin belirlenmesinde ve ilerlemesindeki önemi araştırılmıştır. Çalışmaya proteinürisi olmayan, son 3 ay içinde idrar yolu enfeksiyonu geçirmemiş ve Schwartz formülü ile hesaplanmış tahmini glomerüler filtrasyon hızı ? 90 ml/dk/1,73 m2 olan, 1-16 yaş aralığında 94 VUR'u olan hasta ve 42 sağlıklı çocuk alınmıştır. Çalışma grubunu, I.-II. derece VUR'u olan 32 hasta, III. derece VUR'u olan 41 hasta ve IV.-V. derece VUR'u olan 21 hasta oluşturmuştur. Teknesyum (Tc)-99m dimerkaptosüksinik asit (DMSA) sintigrafilerinde renal skar olup olmamasına göre hastalar kendi içlerinde 2 alt gruba ayrılmıştır (0: skar yok, 1: skar var). VUR'u olan hastalarda renal parankimal skar oranı %27,6 olarak bulunmuştur. Renal parankimal skarı olan hastaların VUR tanısı aldıkları yaş ve çalışmaya alındıkları yaş, renal parankimal skarı olmayan hastalara göre yüksek bulunmuştur. VUR derecelerine göre belirlenmiş hasta gruplarında, VUR derecesi arttıkça, renal parankimal skar bulunma oranının arttığı gözlenmiştir. Hastaların idrar örneklerinde protein, ß2M, NGAL ve CTGF düzeyleri ölçülmüş ve kontrol değerleri ile karşılaştırılmıştır. VUR'u olan hastalar ile kontrol grubu arasında idrar protein/kreatinin, idrar NGAL/kreatinin ve idrar CTGF/kreatinin oranları açısından istatistiksel olarak anlamlı fark saptanmıştır. Hasta grupları kendi aralarında karşılaştırıldığında bu parametreler açısından aralarında istatistiksel olarak anlamlı fark bulunmamakla birlikte yüksek dereceli reflüsü olan hastaların idrar ß2M/kreatinin oranı, I.-II. derece ve III. derece reflüsü olan hastalara göre yüksek, idrar NGAL/kreatinin ve CTGF/kreatinin oranı düşük bulunmuştur. VUR'u olan hastaların idrar protein/kreatinin oranı ile idrar NGAL/kreatinin, idrar ? 2M/kreatinin ve idrar CTGF/kreatinin oranları arasındavpozitif korelasyon saptanmıştır. Renal parankimal skarı olan ve olmayan olan hastalar arasında sadece idrar ß2M/kreatinin oranı açısından istatistiksel olarak anlamlı fark bulunmakla birlikte, renal parankimal skarı olan hastaların idrar NGAL/kreatinin ve idrar CTGF/kreatinin oranları, renal parankimal skarı olmayan hastalara göre yüksek bulunmuştur. En iyi kesim noktasında idrar ß2M/kreatinin oranının VUR'u olan hastalarda renal parakimal skarı predikte edebilme gücü orta düzeyde olmakla birlikte idrar NGAL/kreatinin ve idrar CTGF/kreatinin oranına göre yüksek bulunmuştur. VUR'u olan hastalarda tübüler hasarın olabileceği ve idrar ß2M düzeyinin reflü nefropatili hastalarda önemli düzeyde proteinüri gelişmeden önce renal hasarı belirleyebileceği düşünülmüştür. İdrar NGAL/kreatinin ve idrar CTGF/kreatinin oranlarının VUR'u olan hastalarda renal hasarın belirlenmesinde idrar ß2M/kreatinin oranına üstünlüğü gösterilememiştir. VUR'u ve renal parankimal skarı olan hastaların izleminde belli aralıklarla değerlendirilen idrar CTGF/kreatinin oranındaki değişikliklerin belirlenmesi ve deneysel çalışmalarda böbrek dokusunda CTGF ekspresyonu ile renal skar arasındaki ilişkinin gösterilmesi bu konuya açıklık getirecektir. The purpose of our study was to investigate the diagnostic and prognostic significance of urinary connective tissue growth factor (CTGF) level, together with urinary beta 2 microglobulin (ß2M) and neutrophil gelatinase associated lipocalin (NGAL) levels in renal injury in patients who had primary vesicoureteral reflux with or without renal parenchymal scarring. Ninety four children aged between 1-16 years with primary VUR who were free of UTI within the previous 3 months, who had an estimated glomerular filtration rate calculated by using Schwartz formula of ?90 ml/dk/1.73 m2 and no proteinuria and 42 healthy children aged between 1-16 years were enrolled in this study. The study group consisted 32 patients with Ist-IInd grades of VUR, 41 patients with IIIrd grade of VUR and 21 patients with IVth-Vth grades of VUR. The patients in VUR groups were also divided into subgroups according to the presence or absence of renal scars on technetium (Tc)-99m dimercaptosuccinic acid (DMSA) scans (0: patients without renal parechymal scarring, 1: patients with parenchymal scarring). Renal parenchymal scarring was present in 27.6% of patients with primary VUR. The ages of the patients with renal parenchymal scarring at diagnosis and at enrollment in the study were higher than of the patients without renal parenchymal scarring. Higher rates of renal parenchymal scarring were observed in patient groups with higher grades of VUR. The urinary protein, ß2M, NGAL and CTGF levels of the patients were measured and compared with those of the control group. A statistically significant difference was found for the ratios of urinary protein/creatinine, NGAL/creatinine and CTGF/creatinine between patients with VUR and healthy controls, respectively. Although there was no statistically significant difference in these parameters among VUR groups, the patients with high reflux grade had higher ratio of urinary ß2M/creatinine and lower ratios of NGAL/creatinine and CTGF/creatinine than the patients with Ist-IInd grades and the patients with IIIrd gradeviiof VUR. The ratio of urinary protein/creatinine positively correlated with the ratios of urinary ? 2M/creatinine, NGAL/creatinine and CTGF/creatinine in patients with VUR. Although a statistically significant difference was found only for the ratio of ß2M/creatinine between patients with and without renal parenchymal scarring, the patients with renal parenchymal scarring had higher ratios of the urinary NGAL/creatinine and CTGF/creatinine than the patients without renal parenchymal scarring. At the best cut-off value, the ratio of urinary ß2M/creatinine showed a moderately good diagnostic profile in identifying renal parenchymal scarring in patients with VUR and it was better than the diagnostic profiles of the ratios of urinary NGAL/creatinine and CTGF/creatinine. We suggest that tubular injury may be present in patients with VUR and the urinary ß2M level may detect renal injury before overt proteinuria evolves in the patients with VUR. We suggest that the ratios of urinary NGAL/creatinine and CTGF/creatinine do not show predominancy over the ratio of urinary ß2M/creatinine in detecting renal parenchymal scarring in patients with VUR. We also suggest that the detection of alterations of urinary CTGF/creatinine ratio regularly evaluated during follow-up of the patients with VUR and renal parenchymal scarring and the demonstration of the relationship between CTGF expression in renal tissue and renal scarring may clarify this subject. 115
