Nonalcoholic fatty liver disease (NAFLD) is the most common form of chronic liver disease, affecting almost one-third of the general population and 75% of obese patients with type 2 diabetes. The aim of this article is to review the current evidence concerning the role of quercetin, a natural compound and flavonoid, and its possible therapeutic effects on this modern-day disease. Despite the fact that the exact pathophysiological mechanisms through which quercetin has a hepatoprotective effect on NAFLD are still not fully elucidated, this review clearly demonstrates that this flavonoid has potent antioxidative stress action and inhibitory effects on hepatocyte apoptosis, inflammation, and generation of reactive oxygen species, factors which are linked to the development of the disease. NAFLD is closely associated with increased dietary fat consumption, especially in Western countries. The hepatoprotective effect of quercetin against NAFLD merits serious consideration and further validation by future studies.

Σκοπός: Η δυσθρεψία συνιστά συχνό φαινόμενο στους ασθενείς με καρκίνους γαστρεντερικού συστήματος. Πολλές μέθοδοι έχουν χρησιμοποιηθεί για την προεγχειρητική αξιολόγηση των ασθενών, χωρίς καμία ωστόσο να θεωρείται επαρκής. Σκοπός της παρούσας μελέτης ήταν η σύγκριση των κλασσικών μεθόδων διατροφικής αξιολόγησης με τα αποτελέσματα των αξονικών τομογραφιών άνω κοιλίας σε ασθενείς με καρκίνο οισοφάγου, στομάχου, ήπατος και παγκρέατος. Επιπλέον, διερευνήθηκε η συσχέτιση της κατάστασης θρέψης με την ποιότητα ζωής των ασθενών και με τη μετεγχειρητική νοσηρότητα. Τέλος, επαναξιολογήθηκε η κατάσταση θρέψης των ασθενών με καρκίνο ανώτερου πεπτικού στους έξι μήνες μετεγχειρητικά. Μεθοδολογία: Το υλικό της μελέτης αποτελείται από 212 ασθενείς με καρκίνο οισοφάγου (n=13), καρδιοοισοφαγικής συμβολής (n=79), στομάχου (n=37), παγκρέατος (n=57) και ήπατος (n=26), οι οποίοι προσήλθαν για χειρουργική επέμβαση στην Α’ Χειρουργική Κλινική στο Λαϊκό Νοσοκομείο, από το Σεπτέμβριο του 2015 έως και τον Ιούνιο του 2019. Η αξιολόγηση της δυσθρεψίας έγινε βάσει του ερωτηματολογίου PG-SGA (Patient Generated Subjective Global Assessment), ενώ η ανίχνευση διατροφικού κινδύνου βασίσθηκε στον δείκτη GNRI (Geriatric Nutrition Risk Index). Η μυϊκή μάζα υπολογίσθηκε μέσω του Skeletal Muscle Index (SMI) βάσει της ανάλυσης εικόνων αξονικών τομογραφιών άνω κοιλίας, ενώ η δύναμη λαβής και η ταχύτητα βάδισης χρησιμοποιήθηκαν ως δείκτες της λειτουργικότητας των ασθενών. Η αξιολόγηση της ποιότητας ζωής έγινε μέσω του ερωτηματολογίου EORTC QLQ-C30. Τέλος, η καταγραφή των μετεγχειρητικών επιπλοκών έγινε βάσει της ταξινόμησης κατά Clavien-Dindo. Αποτελέσματα: Το 29,7% των ασθενών είχε καλή προεγχειρητική κατάσταση θρέψης, ενώ το 34% και 36,3% βρισκόταν σε κίνδυνο υποσιτισμού ή ήταν υποσιτισμένο αντιστοίχως, σύμφωνα με το ερωτηματολόγιο PG-SGA. Επιπλέον, το 15,6% των ασθενών είχε χαμηλή σωματική απόδοση και το 15,6% των ασθενών είχε χαμηλή μυϊκή ισχύ προεγχειρητικά, ενώ το 54,2% των ασθενών της μελέτης ήταν σαρκοπενικοί. Ο δείκτης SMI συσχετίσθηκε με τις κλινικές, ανθρωπομετρικές και λειτουργικές παραμέτρους των ασθενών, ενώ οι καλά σιτισμένοι ασθενείς παρουσίασαν υψηλότερες τιμές σε όλες τις παραμέτρους της ποιότητας ζωής, εκτός από τη Συναισθηματική Λειτουργικότητα, συγκριτικά με τους μετρίως και τους σοβαρά υποσιτισμένους ασθενείς. Ακόμη, οι σαρκοπενικοί ασθενείς παρουσίασαν μειωμένα επίπεδα Συνολικής Κατάστασης Υγείας (59,94 ± 19,21 vs 65,68 ± 20,08, p=0,050) και Σωματικής Λειτουργικότητας (77,63 ± 23,08 vs 86,58 ± 15,26, p=0,003) και αυξημένα επίπεδα κόπωσης (35,80 ± 25,19 vs 25,73 ± 22,31, p=0,008), συγκριτικά με τους μη σαρκοπενικούς ασθενείς. Επιπλέον, ο δείκτης GNRI (OR: 7,47, 95% CI:1,06-52,86, p=0,044) και η σαρκοπενία (OR: 5,68, 95% CI: 1,02-31,73, p=0,048) ήταν ανεξάρτητοι προγνωστικοί παράγοντες για την εμφάνιση μετεγχειρητικών επιπλοκών και την ανάπτυξη αναστομωτικής διαφυγής αντιστοίχως. Aim To investigate the impact of malnutrition on postoperative complications in esophageal cancer patients. Background and Methods Malnutrition is common in esophageal cancer patients due to the debilitating nature of their disease. Several methods of nutritional assessment have emerged as significant prognostic factors for short-and long-term outcomes in patients operated for esophageal cancer. The study sample consisted of 85 patients with esophageal (n=11) and gastroesophageal junction (n=74) cancer who were admitted for surgery in the First Department of Surgery, Laikon General Hospital, Athens, Greece, between September 2015 and March 2019. Out of them, 65 patients underwent esophagectomy, while 20 patients underwent total gastrectomy. The assessment of nutritional status included the Geriatric Nutritional Risk Index (GNRI), the Patient Generated Subjective Global Assessment (PG-SGA) and sarcopenia. GNRI was based on preoperative values of patients’ serum albumin and body weight. The preoperative assessment of sarcopenia was based on Skeletal Muscle Index (SMI) derived from analysis of CT scans using SliceOmatic® Software version 4.3 (Tomovision, Montreal, Canada). Postoperative complications were graded according to Clavien-Dindo classification. Minor complications included categories I-II, whereas major complications included categories III-V. Results Thirty nine patients (47.6%) developed postoperative complications. More specifically, 21 patients (24.7%) developed minor complications and 18 patients (21.2%) developed major complications, while anastomotic leakage occurred in 10 patients (11.8%). Eighty patients (94.1%) had a high-risk GNRI ( Conclusion Higher-risk scores on the GNRI are associated with an increased risk for developing postoperative complications, while sarcopenia is associated with higher risk for anastomotic leakage among esophageal cancer patients. Preoperative assessment of GNRI and sarcopenia should be performed in all patients in order to detect patients who are at greater risk of postoperative morbidity.

Background: International experience has shown that deterioration of healthcare services is a common consequence of socio-economic crises. Exact mechanism of this deterioration varies with respect to particularities of each healthcare system, government and administrative policies and local epidemiological conditions. The aim of the present study was to evaluate the effect of Greek economic crisis on the profile and the satisfaction rates of patients seeking surgical services in public and private hospitals. Methods: A questionnaire-based survey concerning healthcare quality and patients' satisfaction was conducted at a private and a public (university) hospital. Patient demographics alongside with patient satisfaction before and after treatment were quantified and compared. Results: Significant differences between private and public sector patients were found concerning nationality, socio-economic status and medical conditions. Private sector patients are younger, of a higher socio-economic status and admitted for elective rather than urgent medical conditions. Patient expectations before treatment arc lower for public sector concerning a variety of markers but patient satisfaction is similar. Conclusions: Even in the years of financial crisis, Greek patients seem to be satisfied by the quality of the healthcare services in both public and private hospitals. Despite the limitations of our study regarding the selection of the population, we believe that the findings might generate more meticulous research on the field hoping that juxtaposed discussions will sensitize policy makers.

ObjectiveTo identify among a wide range of perinatal indices, as well as certain family sociodemographic and parental characteristics, those independently associated with insulin resistance (IR) in late childhood. MethodsA representative sample of 2195 Greek schoolchildren, aged 9-13yr, was examined, and based on the biochemical indices collected IR was estimated using the homeostasis model assessment (HOMA-IR30000 Euro) as the only significant correlates of IR after also controlling for children’s body mass index (BMI) and Tanner stage. ConclusionsThe current study highlighted small birth weight and female sex as the only perinatal factors independently associated with the occurrence of IR in late childhood, when examined at a multivariable level with a wide range of perinatal indices as well as certain family sociodemographic and parental characteristics.
