Obiettivo dello studio: La ventilazione a pressione positiva continua (Continuous Positive Airways Pressure - CPAP) è il trattamento migliore per la Sindrome delle Apnee Ostruttive del Sonno (OSAS), ma l'aderenza limita la sua efficacia. Strategie educative e multidimensionali sembrano essere gli interventi più efficaci per migliorare l'aderenza. In questo studio sono stati analizzati i dati di una popolazione di pazienti affetti da OSAS di grado severo al fine di valutare l'effetto di un intervento educativo e di altri principali fattori sull'aderenza alla CPAP. Metodi: Un programma educativo strutturato è stato somministrato ai pazienti OSAS che avevano indicazione all'utilizzo della CPAP. I dati sono stati raccolti dal colloquio clinico, dall'analisi dei report dei ventilatori CPAP e dalla registrazione poligrafica notturna. Sono stati presi in considerazione i seguenti fattori: età, indice di massa corporea, fumo, severità del disturbo, tipo di maschera, tipo di ventilazione, comorbilità cardiologica, comorbilità psichiatrica o neurologica, scolarità, stato maritale, numero di giorni e numero di accessi al Centro del Sonno (CS) necessari per raggiungere un adeguato trattamento e misure di efficacia terapeutica. Sono state considerate come misure di outcome l'aderenza e la media delle ore di utilizzo CPAP. Risutati: E' stato osservato che l'aderenza a lungo termine non è direttamente correlata al tempo dedicato all'intervento educativo, ma la percentuale cumulativa di pazienti che presentano una buona aderenza è correlata al numero di accessi al CS per colloqui di rinforzo con una relazione quadratica. Una buona aderenza è fortemente correlata alla scomparsa della sonnolenza diurna e al miglioramento della sintomatologia soggettiva ed è stata osservata anche in pazienti con maschera oronasale o comorbilità cognitiva o psichiatrica. Conclusioni: Le strategie educative sono tecniche efficaci per migliorare l'adrenza alla CPAP anche in popolazioni che abitualmente presentano bassa compliance come i pazienti senza beneficio soggettivo dalla terapia con CPAP o pazienti con comorbilità psichiatrica o cognitiva. Alexithymia is a personality trait that consists of difficulty in identifying and acknowledging one's own and others' feelings. Recent studies reported that alexithymia is present in both anorexia (AN) and bulimia nervosa (BN). Brain morphological studies on healthy subjects showed that alexithymia correlates with several brain regions involved in emotions processing. The aim of this study was to investigate the anatomical correlates of alexithymia in AN and BN.We performed a voxel-based morphometry study on 21 patients with AN and 18 with BN. Seventeen healthy subjects were used as a control group. Alexithymia, depression and anxiety were assessed with self-administered questionnaires and correlated to gray matter (GM) density in each group.In BN, alexithymia was correlated with the GM of the parietal lobe, in particular of the right angular gyrus. The correlation was predominantly linked with Difficulty Describing Feelings. In AN, we did not find correlations between GM and alexithymia.In BN, our results support the hypothesis that this trait may represent a relevant pathogenic or maintenance factor that contributes to relational difficulties, present in this pathology. In AN, the lack of correlation between GM volume and alexithymia may be influenced by atrophy in several brain regions that in turn can be, as previously reported, a consequence of caloric restriction. Also, the nature of alexithymia may be different from that of BN and controls and this trait could be secondary to a psychopathologic process specific to AN.

The objective of this study was to investigate the possible associations between the Distress Thermometer (DT) scores and the brain metabolism of structures involved in stress response. Twenty-one cancer patients were assessed using the DT, Problem Checklist and Hospital Anxiety and Depression Scale (HADS). The psychological measures were correlated with [18 F]PET-FDG brain glucose metabolism. Multiple and linear regression and binary logistic regression were run to analyse data. The DT and HADS scores illustrated that 48% of patients were distressed, 19% were depressed and 48% were anxious. Results showed that some subcortical areas activity, such as part of midbrain and of hypothalamus, was correlated with the DT scores. The Problem Checklist scores correlated with the activity of the same areas and included more regions in the limbic forebrain and brainstem. Compared with the DT and Problem Checklist, HADS-Depression scores showed a more extensive pattern of correlation with brain activity, including limbic and cortical areas. The results highlighted that the DT scores correlated with the activity of brain areas typically involved in stress response. Indeed, hypothalamus metabolism was found to be the best predictor of distressed patients.
