The article describes the case of a 74-year old woman diagnosed with granulomatosis with polyangiitis (GPA) and showed multiple cranial nerve manifestations and nasopharyngeal pseudotumor. The patient demonstrated left facial nerve palsy, hypoesthesia, inflammation and swelling at the left nasopharyngreal area, bilateral sensorineural hearing loss, and mass at the left lateral wall of nasopharynx. She was treated with methylprednisone and cyclosphosphamide. Other symptoms of GPA are indicated.

The confirmed cases with COVID-19 in children account for just 1% of the overall confirmed cases. Severe COVID-19 in children is rare. Case Presentation: Our patient was 16 years old with a severe case of COVID-19 and did not survive due to the presence of Granulomatosis with polyangiitis and being treated with immunosuppressive drugs. We used lopinavir, ritonavir, hydroxy chloroquine, intravenous immunoglobulin and continuous veno-venous hemodialysis for treatment. Conclusion: In this patient, an underlying disease and delayed admission to the hospital were two factors complicating his condition.

Objective: Granulomatosis with polyangiitis (GPA), formerly known as Wegner's granulomatosis, is a rare vasculitic syndrome classified under Anti-Neutrophilic Cytoplasmic Autoantibody (ANCA)-associated vasculitides, which is fatal if untreated. The mainstay of treatment consists of immunosuppression using a combination of corticosteroids with either rituximab (RTX) or cyclophosphamide (CYC). We aimed to compare the 4-year clinical outcomes between patients with GPA receiving CYC and RTX as remission induction. Methods: In this retrospective cohort, we used patient data from 92 patients with GPA at two large teaching hospitals and a private clinic in Isfahan, Iran. The patients were classified based on the medication they received for remission induction into RTX and CYC groups. The main outcomes were rate of death and relapse, disease activity assessed based on the Birmingham Vasculitis Activity Score (BVAS), disease-related complications, laboratory markers, and adverse-drug-reactions. Results: Fifty-three (57.6%) patients received CYC, whereas 39 (42.4%) received RTX. The mean duration of follow-up was 3.6 (62) years. Most of patients (70%) had a successful remission, while 20.7% experienced a relapse and 8.7% of patients died. The rate of death and relapse did not differ between the RTX and CYC groups. Disease-related complications involved an insignificantly higher proportion of patients in the CYC (12/53) group than the RTX (4/39) group. Patients in both groups showed a significant decrease in BVAS during follow-ups irrespective of the medication exposure. The rate of adverse events was similarly low (n =1) in both groups. Conclusion: RTX and CYC were similar in inducing remission and reducing adverse clinical outcomes among patients with GPA with acceptable side effect profiles. [ABSTRACT FROM AUTHOR]

What is the “fantasmatic deixis”? It is a very creative and productive cognitive–linguistic operation that allows a “transfer” to other real or fantastic times, places, and “worlds”. The underlying psychological question concerns the possibility of moving and being moved with respect to something or someone who is absent (Bühler, 1965). This “fiction game” is made possible by deictic indicators (Tenchini, 2008), terms that allow motion in time and space, always considering the here–now–I system of subjective orientation. When we refer to something that can be gathered by hearing or by sight, by terms such as here, there, I, and you, the receiver can easily use natural, prelinguistic aids (e.g., gestures, voice quality, facial expressions, and body orientation) to understand what the issuer intends to communicate to him (demonstratio ad oculos). But what happens when we move from the study of “immediate” behavior to that of “mediate” behavior, i.e., the field of memories (retrospection) or the constructive fantasy (prospecting)? The fantasmatic deixis implies enfranchisement from the physical position of the body and requires the assumption of the listener’s current tactile body image. Thus, the receiver assumes an inner attitude to correctly interpret the indications given by the speaker, seeing and hearing through the “inner” or “mental” eye and ear (Raynaud, 2006). In this way, the listener can bring something absent in his/her here, now, and I or feel moved to the point where the speaker leads him/her. My paper will focus on the features and types of fantasmatic deixis, providing some examples and showing how this operation involves language, motion, and cognitive processes.

Granulomatosis with polyangiitis is a systemic necrotizing granulomatous vasculitis that can predominantly affect systemic small- and medium-sized vessels. Isolated pituitary gland involvement at the onset of the disease is extremely rare in granulomatosis with polyangiitis and usually associated with other organ involvement, especially upper and lower respiratory tract and kidneys. 9. Correlation between clinical data and cytokines parameters in patients with Granulomatosis with Poilyangiitis (Wegener Granulomatosis)

Study Objective . To evaluate the correlation between cytokine synthesis and organ involvement in patients with granulomatosis with polyangiitis (GPA). Methods. The study involved 60 patients with GPA diagnosed following the ACR criteria and the CHCC-2012 definition. Cytokines genes expression was studied in blood samples from 57 patients with reverse transcription polymerase chain reaction. We evaluated mRNA for the following cytokines: interferon (IFN)-α, IFN-γ, interleukin (IL)-1β, IL-2, IL-4, IL-6, IL-8, IL-10, IL-12, IL-18, tumor necrosis factor (TNF)-α production. Also, we investigated the serum concentration of IFN-α, IL-1β, IL-8 IL-18 and TNF-α by ELISA in 40 patients with GPA. Results . Markers for granulomatous disease were detected in 45 (75,0%) GPA patients, markers for vasculitis - in 23 (38,3%) patients. GPA affected eyes in 33 patients (55,0%), ears in 29 (48,3%), nose and sinuses in 57 (95,0%), lungs in 35 (58,3%) and kidneys in 28 (46,7%). There was total positive correlation between eye disease and IL4 and IL12 genes expression ( r =1,00). Total negative correlations were between ear disease and IL-12 gene expression; glomerular filtration rate reduction more than 50% and serum IFN-α ( r =-1,00). Conclusion. Cytokines may upregulate the direct immune response in organ damage in GPA patients. Markers for granulomatous disease correlated with cytokines, that suppressed Th2 type immune response and promoted Th1 type immune response. Correlation between vasculitis markers and cytokines showed that IFN-α, TNF-α, IL-1β and IL-6 participated in vascular damage in GPA patients.

11. How the humanities shape medical culture: Knowing Wegener and other Nazi eponyms

While the medical humanities have experienced a renaissance, they are still largely a peripheral component of medical education. This is troublesome because the humanities include a number of disciplines that are foundational in understanding medicine and how it should be practiced. Nonetheless, current medical culture makes it difficult to fully incorporate the humanities into curriculum. We therefore propose an incremental approach to shaping the medical culture that can easily be incorporated into daily teaching as opposed to designing additional classes and resources that must be added to existing educational structures. An example of this approach is reviewed here through teaching historical and ethical lessons surrounding Nazi eponyms. The use of names like Wegener provide brief opportunities for sidebars during clinical lectures to remind learners that empirical data do not provide ethical direction and that our medical history has included atrocities that remind us to practice conscientiously. We provide other examples that can be included in daily learning. This approach eschews the burdens associated with large curricular changes, such as student resistance/apathy and logistical barriers, and can be easily implemented. It also enables change to be gradual and through structures that have already been established, allowing learners to see the benefits of insights from the humanities in small, digestible segments. Through this approach, medical culture can be shaped towards a greater appreciation toward the medical humanities.

12. Investigation of Anti-Myeloperoxidase Antibodies in a Dog with Bilateral Necrotizing Scleritis

Necrotizing scleritis is uncommon in dogs and presumed to be immune-mediated. Its clinical pattern and histopathology are similar to ocular lesions observed in humans suffering from granulomatosis with polyangiitis (GPA), formerly named Wegener’s granulomatosis, where the pathogenesis revolves around anti-neutrophil antibodies (e.g., anti-myeloperoxidase). These antibodies are used to diagnose and follow-up the disease in humans, but variants that only affect the eyes often test negative. Here, we present the first case of canine necrotizing scleritis where measurement of anti-myeloperoxidase antibodies was attempted. A 1.5 year-old female Scottish Terrier was presented with bilateral deep multifocal scleromalacia, severe inflammation of corneal/uveal/retrobulbar tissues, perilimbal corneal oedema and neovascularization, hypotony, and mild exophthalmos. Corticosteroids and antibiotics had been administrated (topically and orally) without success. Due to painful multifocal scleral perforation with vitreal haemorrhage, the left eye underwent enucleation, so did the right eye one week later. The histopathology of the left eye revealed a neutrophilic and histiocytic scleral infiltration with extension of pyogranulomatous inflammation to the cornea, choroid, ciliary body, and orbital fat. Levels of plasma anti-myeloperoxidase antibodies were not statistically significant to those of 13 healthy dogs. Further research is warranted to investigate the presence and role of anti-neutrophil antibodies in canine necrotizing scleritis.

13. FROM THE CONTRACTING EARTH TO CONTINENTAL DRIFT: WEGENER'S INFLUENCE ON PORTUGUESE AND SPANISH SCIENCE TEXTBOOKS THROUGH THE TWENTIETH CENTURY.

- Abstract
Alfred Wegener's work on continental drift profoundly influenced the development of geology during the twentieth century. The main objective of this historical research is to explore how Wegener's hypothesis about the origin of continents and oceans influenced Portuguese and Spanish science textbooks published in the twentieth century. For this purpose, a qualitative method based on the Chevallard's concept of didactic transposition was used. The didactic transposition of the fundamentals of continental drift, including displacements forces and geographical, geophysical, geological, paleontological, biological, and paleoclimatic arguments used by Wegener to support this hypothesis, was studied in a comprehensive sample of twenty science textbooks published after the publication of Die Entstehung der Kontinente und Ozeane (Wegener 1915). The analysis of textbooks shows that the didactic transposition of continental drift was commonly present in Portuguese and Spanish textbooks beginning in the 1930s, although, with different degrees of development. Science textbooks since the 1950s presented the displacement forces of sial continental blocks proposed by Wegener, namely the centrifugal force of Earth's rotation and the attractive gravitational force that results from the interaction of the earth with the moon and sun. The geographical, geological, paleontological and biological arguments supporting continental drift were commonly addressed in many science textbooks, followed by the paleoclimatic arguments. The geophysical arguments were briefly addressed in only two textbooks. Many Portuguese and Spanish textbooks also presented critiques of Wegener's work, mainly focused on the lack of explanation for the origin and nature of forces that could move continents at the Earth's surface. A lo largo de la actividad se suceden distintas organizaciones de aula y andamios didácticos, con el objetivo de promover dinámicas de creación de conocimiento científico y la sistematización del aprendizaje y la consecución del objetivo final: realizar un breve vídeo de divulgación científica sobre la historia geológica de este planeta imaginario.

Desde o surgimento da Teoria Conceptual da Metáfora, há mais de trinta anos, houve uma série de autores que apontaram a falta de contextualização historiográfica, uma vez que a maioria das hipóteses sobre a metáfora conceptual e até boa parte dos exemplos já são antecipadas desde o século dezessete em reflexões filosóficas, antropológicas, psicológicas e linguísticas. O presente trabalho pretende apresentar dois pensadores dos séculos dezoito e dezenove, a saber, o filósofo Johann Heinrich Lambert (1965) e o linguista Philipp Wegener (1991), que não apenas já desenvolvem uma teoria cognitiva da metáfora cotidiana, como também já conseguem integrar aspectos pragmáticos, que atualmente são redescobertos como fatores descuidados pela primeira geração da Linguística Cognitiva. Sendo assim, as duas concepções podem lançar uma ponte a discussões atuais, uma vez que ambas conseguem apontar caminhos para superar tanto o abismo entre o sujeito solipsista e a sociedade, como a dicotomia entre universalismo e relatividade cultural. Os dois autores entendem a metáfora como fenômeno cognitivo e, ao mesmo tempo, intersubjetivo, por ser negociado entre os participantes em situações comunicativas concretas.

Wegener's granulomatosis is a systemic necrotizing vasculitis that affects small and medium vessels, is associated with the expression of cytoplasmic ANCA antibodies. Characteristically disease causes airway injury and glomerulonephritis. Respiratory compromise is expressed as sinusitis, otic and nasal pathology, alveolar hemorrhage, being the most serious manifestation of lung involvement. Kidney damage is not indicative of severity, but it is of prognosis, so it is recommended to do a renal biopsy at the time of diagnosis of the disease. Treatment will depend on the severity of the clinical manifestation; it has a phase of remission induction and other of maintenance. In cases of massive alveolar hemorrhage or damage to any other target organ, plasmapheresis should be considered, as it has proven be highly effective in terms of remission. [ABSTRACT FROM AUTHOR]

18. Boris Choubert: The forgotten fit of the circum-Atlantic continents.

Boris Choubert was a strong supporter of Wegener's continental drift theory. In 1935, he published a very accurate fit of the circum-Atlantic continents, which was based on continental edges instead of coastlines; in the same paper, he interpreted the Palaeozoic belts as the result of horizontal movements of the Precambrian blocks; so, he greatly expanded the role of continental drift through time. This original and very prophetic work was almost completely ignored by his contemporaries. Thirty years later (1965), Bullard, Everett and Smith published in turn a similar but more sophisticated fit; they did not acknowledge Choubert's initial work. Bullard's fit was met with immediate and tremendous success. The present paper analyses the reasons why Boris Choubert was frustrated of his pioneering role. This lack of recognition is related to: (1) a great evolution in the geological concepts between 1935 and 1965, and (2) a poor choice of Choubert, regarding the title of his 1935 article. This work is a review of Boris Choubert’s paper (1935), which was published in French under the rather devalorizing title: “Research on the Genesis of Palaeozoic and Precambrian Belts.” Despite its innovative content, this article had no impact either at the time of its publication or even later. It begins with the construction of a remarkable fit of the circum-Atlantic continents. This was based on the −1.000 meters isobath instead of the shoreline. Thirty years before Bullard et al. (1965), it demonstrated in an indisputable way the reality of the continents motion on the surface of the Earth. Therefore, Choubert designated Wegener’s “continental drift” as the main cause of tectonics. Even going beyond Wegener’s theory, he argued that this mechanism was efficient well before the formation of the Triassic Pangæa, during the whole Palaeozoic to result in the building of the Caledonian and Hercynian mountains. Although he was still encumbered by the vocabulary of the time regarding geosynclines, Boris Choubert described tectonics based on the horizontal mobility of the Precambrian continental blocks. Oddly enough, he did not apply this model to the Precambrian structures, which he attributed to the effects of the Earth’s rotation on the continental crust during its solidification. At the time of its publication, this paper was a very important step towards understanding global tectonics. Unfortunately, Choubert’s contemporaries did not generally recognize its significance.

Granulomatosis with polyangiitis is a necrotising vasculitis with multisystem granulomatous inflammation which predominantly affects small blood vessels; it is associated with anti-neutrophil cytoplasmic antibodies (ANCAs). The purpose of this article is to describe the most common image findings in the areas of the head and the neck, in patients with granulomatosis with polyangiitis. To date no Mexican publications provide orientation on this point. The paranasal sinuses and the nasal septum are the best known areas, in head and neck, for this disease; however, disorders which may appear in eye sockets, ear, central nervous system, pituitary gland, and larynx, even without clinical manifestation at the time of image evaluation, should not be overlooked. It is important to opportunely report lesions due to granulomatosis with polyangiitis which suggest activity identified by imaging methods to the clinical physician and perform comparative studies to assess new lesions. In most cases active lesions identified by imaging are characterized by heightened signal intensity following administration of intravenous contrast medium, using T1 weighted sequences with fatty suppression. Image findings which suggest activity may be compared at different evolution times to determine possible remission or for histological confirmation. Long term, the presence of active lesions will help modify treatment, which may include starting corticoids or modifying or intensifying their use. [ABSTRACT FROM AUTHOR]

Significant eosinophilia is a prominent feature in Churg-Strauss syndrome but has not been described in granulomatosis with polyangiitis (GPA) in a pediatric patient. We present a biopsy case that confirmed granulomatosis with polyangiitis with significant eosinophilia > 30% on the initial presentation. Etiologies that could account for eosinophilia were excluded during workup. The patient's presentation of pulmonary alveolar hemorrhage, conjunctivitis arthritis, high-titer cytoplasmic antineutrophil, PR3-ANCA antibodies, and cytoplasmic antibodies (cANCA) was consistent with a clinical picture of atypical GPA in a pediatric patient. This case presents a rare opportunity not only due to GPA's low incidence in the pediatric population but due to the unusual nature of significant eosinophilia in GPA.

Objectives. To describe the clinical, laboratory, histopathological presentations and final diagnoses for children presenting to a tertiary paediatric rheumatology service with an inflammatory lesion of the orbit.Methods. This was a retrospective descriptive case series of children with an inflammatory lesion of the orbit presenting to a single paediatric rheumatology service between January 1999 and July 2010.Results. Ten patients, median age 11.5 (range 3.1–16.2) years at referral to the paediatric rheumatology department were identified; median duration of symptoms at referral was 9 (0.75–17) months. Imaging was performed in 9/10 cases: orbital MRI (n = 4), orbital CT scan (n = 1), both MRI and CT scan (n = 4). All 10 patients had an orbital biopsy; 2 patients had repeat biopsies. The final diagnoses were granulomatosis with polyangiitis (Wegener's) (n = 5; ANCA positive n = 4, ANCA negative n = 1), idiopathic orbital inflammation (n = 3), atypical mycobacterial infection (n = 1) and sarcoidosis (n = 1).Conclusion. Inflammatory mass lesion of the orbit is an unusual presentation in children. The differential diagnosis is wide and may evolve over time. Orbital biopsy and screening for systemic features is essential before treatment with CSs or other immunosuppressants to exclude malignancy, infection, vascular lesions, autoimmune conditions or other causes of orbital inflammation that can be associated with serious systemic manifestations. We report the case of a 36 years old man with multiple suppurations within the ENT domain, fever uninfluenced by antibiotics and antipyretics, two episodes of acute respiratory distress syndrome, requiring tracheostomy, serology suggestive of Wegener's granulomatosis (high titer of cANCA). 25. Recurrence of ANCA-associated vasculitis following renal transplantation in the modern era of immunosupression.

Progressive glomerulonephritis and attendant end-stage renal disease (ESRD) result from antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis. The optimum time of kidney transplantation in patients with ESRD due to ANCA-associated vasculitis (AAV) and the risk of renal or nonrenal recurrence of vasculitis after transplantation are unknown. To answer some of these questions, we followed 35 transplant recipients with diagnoses of microscopic polyangiitis (20 patients) and Wegener's granulomatosis (15 patients). The median time from diagnosis to transplantation was 25 months with all patients being in clinical remission. Fifteen patients were ANCA-positive at time of the transplant with 13 preemptive transplants. The most common immunosuppressive strategy included antibody induction, corticosteroid, mycophenolate mofetil, and tacrolimus with acute rejection occurring in eight cases. Overall and death-censored graft survivals were 94 and 100%, respectively, 5 years post-transplantation. Nonrenal relapse occurred in three patients with a satisfactory response to treatment. No clear risk factor to relapse emerged and no detrimental effect to renal function was found. We conclude that transplantation should be considered as the treatment of choice for ESRD due to AAV. Introduction and objective: Subglottic stenosis in Wegener’s disease (WD) represents a diagnostic challenge because it’s ability to be the unique manifestation. Description: A case of subglottic GW described in a woman 14 years. Discussion: Subglottic stenosis secondary to Wegener’s disease use to be associated with ENT symptoms, so these can give us the etiological suspicion. In the absence of these, we should include it in the differential diagnosis and try a histopathologic confirmation. Conclusions: In an isolated subglottic stenosis, rule Wegener's disease performing a biopsy of nasal mucosa and two serology tests.

The aim is to conduct a descriptive, population-based study in order to assess temporal and spatial changes in mortality due to granulomatosis with polyangiitis (GPA) in Spain from 1984 to 2016. Mortality data were obtained from the Spanish Annual Death Registry. Deaths in which GPA was the underlying cause were selected using the 446.4 and M31.3 codes from the International Classification of Diseases, 9th and 10th revision. Annual average age at death and age-adjusted mortality rates were calculated. Geographic analysis was performed at municipality and district level. Variations in mortality according to the type of municipality (urban, agro-urban or rural), district and geographic location (degrees of latitude) were assessed using standardized mortality ratios (SMRs) and smoothed-SMRs. Over the whole period, 620 deaths due to GPA were identified. Age at death increased at an average annual rate of 0.78% over the period 1987&ndash, 2016 (p <, 0.05). Age-adjusted mortality rates increased by an annual average of 20.58% from 1984 to 1992, after which they fell by 1.91% a year (p <, 0.05). The agro-urban category had the highest percentage (4.57%) of municipalities with a significantly higher GPA mortality rate than expected. Geographic analysis revealed four districts with a higher risk of death due to GPA, two in the North of Spain and two in the South. This population-based study revealed an increase in the age at death attributed to GPA. Age-adjusted mortality rates went up sharply until 1992, after which they started to decline until the end of the study period. Geographic differences in mortality risk were identified but further studies will be necessary to ascertain the reasons for the distribution of GPA disease.

The initial question of this thesis is concerned with the legitimation, motivation and organization of the anti-vaccination movement in the area of today’s Hesse, starting with the coverage and description of the protagonists on an individual level, thus trying to reconstruct network structures. As a part of the current state of research concerning the history of anti-vaccination movements, this thesis renders a contribution to the appraisal of the regional and historical anti-vaccination- tendencies in the area of Hesse by applying network-theories, which have already been successfully implemented in various other historical studies. The core part of this thesis analyzed the motivation and legitimation of individual protagonists as well as their involvement in anti-vaccination networks. As part of the wider analysis, this thesis took a closer look at how different individual actors with different individual viewpoints united in their common goal and against vaccinations. As in recent years, a “revival” of vaccination skeptic ideology was observed across some western cultures, this thesis investigates potential parallels between the anti-vaccination movement after the enactment of the “Reichsimpfgesetz” 1874, and the vaccination skeptic movement of the 21st century. In comparing the historic and contemporary vaccination critics, this thesis particularly looked at differences in network structures and considered the influences of modern technical innovations like the internet and other new communication media. A coverage of the anti-vaccination movement in Hesse has not been realized on an individual level so far, hence this approach promised to gain an insight into the individual motivation and legitimation of individual protagonists, who then in turn constructed their own network mainly by the use of the periodical “Der Impfgegner”. Contrary to analyzing the anti-vaccination movement as a whole (within which the varying different motivations of a broader range of individual members might have been lost) the approach in this thesis enabled a detailed analysis of the structure of the movement, based on individual points of view. Anti-vaccinations tendencies had already been present before the enactment of the “Reichsimpfgesetz”. However, the anti-vaccination movement as a formally organized network has only been established through this very act. From thereon, “Der Impfgegner”, a monthly periodical founded in 1881, built a platform for public exchange for anti-vaccination enthusiasts. Its ideological character was mainly influenced by the Spohr-family from Gießen, consisting of the naturopath and former serviceman Peter Spohr, together with his two sons Curt Spohr (lawyer in Gießen) and Roderich Spohr (Physician in Frankfurt). During the period of investigation from 1908 to 1914, Peter Spohr was already over 80 years old and had claimed fame for his doctrines in certain social circles. His studies proposed a life in accordance with nature, based on the usage of air and later on also water-based therapies. A proper ventilation seemed to be as important to him, as well as the treatment with water at certain temperatures in order to heal various sufferings. His theories were essentially influenced by J.H.Rausses book „Wasser tuts freilich“. Spohr effaced himself in the „Impfgegner“, which seemed to be in accordance with his personality. The magazine however used him as a figurehead and prominent pioneer of the anti-vaccination movement, even though Spohr never actively claimed this position. His sons Curt and Roderich Spohr adopted the naturopathic position of their father and opposed compulsory vaccination in accordance with their professions – Roderich Spohr was a general practitioner in Frankfurt and prevented the vaccination of his two sons through medical attest. In the year of 1912 his vaccination-skepticism would lead to court proceedings, after he overlooked smallpox on a patient, which then caused an endemic with several causalities. Due to Roderich Spohr’s conviction, his brother Curt Spohr increasingly made public appearances.Having contributed only few articles to the “Impfgegner” so far, Curt Spohr started writing longer discourses about the question of vaccination. He also became a chairman of the “Reichsverband der Impfgegner”. Apart from the Spohr family, there were several other authors, whose articles influenced the character of the “Impfgegner” between 1908 and 1914. Hugo Wegener, a graduate engineer from Frankfurt, shaped the „Impfgegner “and the vaccination-skeptical movement with his unprecedented actionism and clarity. Between 1910 and 1914 he published several vaccination-critical books, of which one contained more than 36.000 cases of vaccination failures according to his claims. Some of them were also illustrated with pictures or drawings. Wegener’s agenda was hard to grasp due to the multiplicity of his publications. Because he published his books throughout the publishing company of his wife, and simultaneously advertised them in the “Impfgegner”, one could allege monetary interests. However, it was possible to find his true motives being situated in a possibly querulous personality. Wegener can be regarded as one of the most active and colorful personalities of the anti-vaccination movement in Hesse and beyond. Another important vaccination critic was Dr. Bilfinger, who temporarily lived in Kassel, and founded the “Verein impfgegnerischer Ärzte” ibidem. This was recognized with great enthusiasm within certain social circles and constituted an important step towards the organized resistance against vaccinations. Even though Bilfinger was ranked as an important personality in the anti-vaccination network from then on, he remained in the background from a journalistic point of view. His role as medical head of the “Sanatorium Gossmann” in Kassel opened the door to an investigation of the Kassel-Wilhelmshöhe. Local naturopathic orientation made a vaccination-skeptic movement likely, which however couldn’t be verified after extensive inspection. Bilfinger's motive seemed to be of personal belief and he used the founding of the antivaccination association as a step towards grouping single efforts. However, his endeavors seemed to be crowned by little success, since the “Verein impfgegnerischer Ärzte” didn’t seem to play a greater role until the end of the investigation period 1914. Further members of the anti-vaccination-movement were physician von Niessen from Wiesbaden and the physicians von Hohenhausen and Voigt from Frankfurt. Even though they contributed to the subject though critical contributions and presentations, they had no trendsetting meaning. Their motivation was mainly based on personal beliefs. Investigating the connections between the protagonists among themselves enabled an inclusion of Hesse’s anti-vaccination movement into a network of regional and national ties. The primary focus of this thesis was a qualitative analysis, which then allowed further conclusions in a geographical consideration. Based on the analysis of the number of contributions in the „Impfgegner“ and their geographical evaluation according to their activity, it was then possible to gain further knowledge of the authors. While the area of Frankfurt seemed to have been the epicenter of the anti-vaccination movement in the “Geographiekarte”, when considering the publicized literature of the time,,the city Gießen might need to be considered of equal importance. Thus it was possible to acquire information, which couldn’t be displayed in a topographical point of view. After that, nationwide connections between important members of the network were analyzed. Correspondence between Peter Spohr and the Swiss Adolf Vogt allowed an insight into the personality of Oberst Spohr, as well as the exchange of letters with racial theorist Ludwig Schemann. This again highlights the importance of a appraisal on an individual level and examining personal opinions of key figures, instead of focusing on the public viewpoints. The same can be applied to the correspondence between Wilhelm Schwaner and Hugo Wegener, in which the latter explained the delayed production of a vaccination skeptic leaflet by being short on money. To deduce his actions from monetary motives seemed to be unlikely. Thus it was possible to draw conclusions about his alleged motivation, which was rather intangible so far. Therefore, the network approach of this thesis did not only allow a gain of information when investigating the main protagonists, but also a gain of knowledge through graphical and quantitative analysis of the connections among the protagonists. Contrary to the existing studies of the history of the anti-vaccination movement, the regional approach of this thesis constitutes a novel approach. It critically enabled all of the abovementioned detailed analyses of important personalities in the network and hence contributed to the body of knowledge surrounding the anti-vaccination movement. As a result, it was possible to obtain a very detailed conclusion for the region of Hesse of today and the time period discussed in this thesis. The central theme of this thesis was to investigate the detailed motivations and standpoints of key actors in the anti-vaccination movement of Hesse between 1904 and 1914. Further studies must show, to which extent the results presented here can be transferred to other regions of Germany or beyond. The scientific conclusions of this thesis are hence representative only for a certain period of time and place, because further studies for other regions have not yet been implemented. Nevertheless, relevant parallels appeared in comparison with the anti-vaccination movement of the 21st century, since completely new ways of communication like the Internet or social media have emerged in the meantime. On one hand,the latter underwent a substantial change due to technical improvements like the internet. On the other hand, similarities could be found at the very exchange of ideas on social platforms like Facebook or internet forums. By distributing an opinion or question to thousands of group members in real time, the impact seems much higher than the effect of a monthly periodical like the “Impfgegner”. Still, the basic principle of organizing through a publicly available medium stayed the same, and a level of continuity in the spread of information can be observed. Further parallels emerged between aforementioned Hugo Wegener and Hans Tolzin (born in 1958). A textual analysis of the Hans Tolzin’s webpages revealed a strong overlap regarding rhetoric, public relations activity and also lack of medical knowledge. While Wegener used books and magazines for his anti-vaccination activity, Tolzin, from a journalistic point of view, was highly active on the internet. One of the greatest parallels could be found in the form of the “Impf-report”, a periodical which has been published every two months in the Hans-Tolzin- Verlag since 2004. Recent events have shown that the fundamental question regarding vaccinations and the fundamental criticism associated has survived for more than 140 years past the issuing of the “Reichsimpfgesetz”. This thesis showed a conclusive discourse about the current discussion regarding the tightening of the vaccination law by comparing the legal situation in Germany to the one in other European countries and by considering current epidemical infection rates. Here, the focus lay on measles, since, according to the WHO, smallpox had been declared to be extinct globally for more than 40 years., Die Ausgangsfrage der vorliegenden Arbeit beschäftigte sich mit der Erforschung von Legitimation, Motivation und Organisation der Impfgegnerschaft im Raum des heutigen Hessens, ausgehend von der Erfassung und Darstellung der Protagonisten auf individueller Ebene und eines sich daran anschließenden Versuchs der Rekonstruktion von Netzwerkstrukturen. Im Rahmen der Forschungslage zur Geschichte der Impfgegnerschaft leistet diese Studie einen Beitrag zur regionalgeschichtlichen Aufarbeitung impfkritischer Strömungen im Raum Hessen und erlaubt eine Verwendung des Netzwerk-Theorems, welches bereits für mehrere historische Studien erkenntnisfördernd angewandt wurde. Die Fragestellung zielte insbesondere auf die Analyse von Motivation und Legitimation einzelner Protagonisten ab und verfolgte diese hin zu einer Einordnung in ein dem Widerstand in der Impffrage verschriebenen Netzwerk. Ausgehend von einer individuellen Aufarbeitung sollte geklärt werden, wie sich einzelne Impfgegner mit teilweise unterschiedlicher Motivation zusammenfanden, um für ein gemeinsames Ziel einzutreten. Ferner galt es herauszuarbeiten, ob sich Parallelen zwischen der Impfgegnerschaft nach Erlass des Reichsimpfgesetzes 1874 und der impfkritischen Bewegung des 21. Jahrhunderts finden lassen. Auch für diesen Ansatz sollte ausgehend von der Netzwerkidee ein Vergleich von „damaligen“ und „heutigen“ Netzwerkstrukturen dienen, welcher unter Einbeziehung technischer Neuerungen wie dem Internet und anderer „neuer“ Publikationsmedien erfolgte. Da eine Analyse auf personenbezogener Ebene für die Impfgegnerschaft in Hessen bisher noch nicht erfolgt war, versprach dieser Zugang insbesondere einen Erkenntnisgewinn über die individuelle Motivation und Legitimation einzelner Protagonisten, welche sich dann mit Hilfe des Periodikums „Der Impfgegner“ zu einem Netzwerk zusammenfanden. Gegenüber einer Betrachtung der Impfgegnerschaft als gesamtheitliche Bewegung, in der individuelle Motivationen tendenziell unterschlagen werden würden, konnte durch den in dieser Arbeit gewählten Zugang ausgehend von einer persönlichen Betrachtungsebene eine detaillierte Analyse ihrer Zusammensetzung erreicht werden. So ließ sich erarbeiten, dass der Widerstand gegen die Impfung zwar bereits vor Erlass des Reichsimpfgesetzes 1874 bestand, jedoch erst durch dieses einen organisierten Charakter erfuhr. Vor allem das 1881 gegründete monatliche Periodikum „Der Impfgegner“ diente fortan als öffentliches Forum und Organisationsplattform der Impfgegner. Sie erfuhr maßgeblich durch die Gießener Familie Spohr, bestehend aus dem Naturheilkundler und Militäroberst Peter Spohr, sowie seine beiden Söhne, Curt Spohr (Jurist in Gießen) und Roderich Spohr (Arzt in Frankfurt), ihre ideologische Prägung. Peter Spohr war im Untersuchungszeitraum des „Impfgegners“ zwischen 1908 und 1914 bereits über 80 Jahre alt, und hatte sich durch seine naturheilkundlichen Lehren in entsprechenden Kreisen bereits einen Namen gemacht. Seine Werke propagierten ein Leben im Einklang mit der Natur, dessen Grundlage die Anwendung von Luft- und später auch Wassertherapien bildete. Eine gute Durchlüftung der Räume schien ihm ebenso wichtig wie die Applikation von Wasser unterschiedlicher Temperatur zur Heilung diverser Leiden, dessen Grundlage nach eigenen Angaben das Werk „Wasser tuts freilich“ von J.H. Rausse bildete. Spohr hielt sich im „Impfgegner“ im Hintergrund, was grundsätzlich seiner Persönlichkeit zu entsprechen schien. Die Zeitschrift verwendete ihn indes als Art Gallionsfigur und prominenten Vorreiter der Impfgegnerschaft, wenngleich Spohr diese Position nie aktiv eingefordert hatte. Seine Söhne Curt und Roderich Spohr übernahmen die naturheilkundliche Einstellung ihres Vaters und widersetzten sich dem Impfzwang im Rahmen ihres jeweiligen Berufes – Roderich Spohr praktizierte in Frankfurt als Allgemeinarzt und verhinderte beispielsweise die Impfung seiner Söhne mittels ärztlichem Attest. Im Jahr 1912 sollte seine Impfskepsis zu einem Gerichtsprozess führen, als er bei einer Patientin eine Pockenerkrankung übersah und sich in der Folge eine Pockenendemie entwickelte, die mehrere Todesopfer forderte. Mit seiner Verurteilung zu einer hohen Geldstrafe trat nun auch dessen Bruder Curt Spohr vermehrt in Erscheinung. Hatte dieser bisher nur wenige Artikel im „Impfgegner“, hauptsächlich zu rechtlichen Fragen verfasst, folgten nun mehrseitige Abhandlungen über die Impffrage und eine Aufnahme in den Vorstand des „Reichsverbandes der Impfgegner“. Neben der Familie Spohr sind diverse weitere Autoren zu nennen, deren Artikel im „Impfgegner“ insbesondere im Zeitraum zwischen 1908 und 1914 zu dessen Charakter beitrugen. Der Frankfurter Diplomingenieur Hugo Wegener prägte sowohl den „Impfgegner“, als auch die impfgegnerische Strömung mit beispiellosem Aktionismus und Schärfe. Zwischen 1910 und 1914 publizierte er mehrere impfkritische Bücher, eines davon laut eigenen Angaben mit mehr als 36.000 Fällen angeblichen Impfversagens, welche zum Teil auch mit Abbildungen versehen waren. Wegeners Agenda ließ sich gerade auf Grund der Fülle des publizierten Materials nur schwer bewerten. Da die Werke im Verlag seiner Frau erschienen und er im „Impfgegner“ Werbung für diesen abdrucken ließ, könnte man ihm monetäre Interessen unterstellen. Es konnte jedoch auch eine querulatorische Komponente herausgearbeitet werden, die möglicherweise den wahrscheinlicheren Beweggrund bildete. Wegener kann auch über Hessen hinaus als einer der aktivsten aber auch schillerndsten Persönlichkeiten der Impfgegnerschaft gelten. Ein weiterer bedeutender Impfgegner war der zeitweise in Kassel lebende Dr. Bilfinger, der 1908 den „Verein impfgegnerischer Ärzte“ ebendort gründete. Dies wurde in einschlägigen Kreisen mit Begeisterung aufgenommen und stellte einen weiteren Schritt im organisierten Widerstand gegen die Impfung dar. Wenngleich Bilfinger dadurch als wichtige Persönlichkeit im Impfgegnernetzwerk gelten konnte, hielt er sich aus publizistischer Sicht eher im Hintergrund. Seine Funktion als kurzzeitiger Leiter des Sanatoriums Gossmann zu Kassel eröffnete den Zugang zur Untersuchung des Kurortes Kassel-Wilhelmshöhe. Die dortige naturheilkundliche Ausrichtung ließ auch impfkritische Strömungen vermuten, welche nach eingehender Betrachtung jedoch nicht verifiziert werden konnten. Bilfinger schien auf Grund persönlicher Überzeugung gegen die Impfung einzustehen und bemühte sich durch Vereinsgründung und Organisation impfgegnerischer Ärzte um eine Bündelung einzelner Kräfte. Sein Bestreben war offenbar nur von mäßigem Erfolg gekrönt, da der „Verein impfgegnerischer Ärzte“ zumindest bis zum Ende des Untersuchungszeitraumes (1914) augenscheinlich keine allzu große Rolle spielte. Weitere im Netzwerk aktive Impfgegner waren der Wiesbadener Arzt von Niessen, sowie die Frankfurter Impfärzte von Hohenhausen und Voigt. Sie trugen zwar ebenfalls durch kritische Bei- und Vorträge zum Impfthema bei, hatten aber keine richtungsweisende Bedeutung. Ihre Motivation lag mehrheitlich in ihren persönlichen Motiven in der Impffrage begründet. Die Untersuchung der Verbindungen der Protagonisten untereinander ermöglichte in der Folge eine Eingliederung der Impfgegnerschaft in Hessen in ein Netzwerk aus regionalen und überregionalen Beziehungen. Im Fokus stand dabei zunächst die Aufarbeitung entsprechend eines qualitativen Zugangs, welcher dann mittels einer geographischen Betrachtung weitere Rückschlüsse zuließ. Eine Analyse nach Zahl der Beiträge in der Zeitschrift „der Impfgegner“ erlaubte einen zusätzlichen Erkenntnisgewinn durch eine Gewichtung und graphische Hervorhebung einzelner Autoren entsprechend ihrer Aktivität. So imponierte in der „Geographiekarte“ beispielsweise die Region Frankfurt als Epizentrum des impfgegnerischen Widerstandes, während nach Einbeziehung der schriftstellerischen Tätigkeit auch die Stadt Gießen an Bedeutung gewann. Informationen, welche also in der topographischen Ansicht nicht darstellbar waren, konnten durch eine unterschiedlich verortete Betrachtungsweise herausgearbeitet werden. In einem weiteren Schritt wurden zudem auch überregionale Beziehungen besonders relevanter Vertreter im Netzwerk untersucht. So gaben Briefwechsel zwischen Peter Spohr und dem Schweizer Adolf Vogt ebenso Einblick in die Persönlichkeit des Oberst, wie auch dessen Korrespondenz mit dem Rassentheoretiker Ludwig Schemann. Hier zeigte sich zudem der Nutzen einer Aufarbeitung auf persönlicher Ebene, welche nicht nur einen Einblick in die öffentliche, sondern auch in die persönliche Meinung einer Schlüsselfigur zuließ. Selbiges galt für den Briefwechsel zwischen Wilhelm Schwaner und Hugo Wegener, in welchem Letzterer unter anderem die verspätete Anfertigung eines impfkritischen Flugblattes mit Geldmangel erklärte. Sein Handeln auf monetäre Beweggründe zurückzuführen schien daher unwahrscheinlich. So konnten Rückschlüsse auf seine mutmaßliche Motivation gezogen werden, die bis dahin nur schwer greifbar war. Der Netzwerkzugang ermöglichte daher nicht nur einen Informationsgewinn bei der Untersuchung der Protagonisten, sondern auch einen Erkenntnisgewinn durch graphische Darstellung und qualitative Analyse der Beziehungen untereinander. Gegenüber den bisher vorliegenden Studien zur Geschichte der Impfgegnerschaft stellt der regionale Zugang ein Novum dar, da eine regionale Betrachtung die oben genannte detaillierte Analyse wichtiger Persönlichkeiten im Netzwerk mit entsprechendem Erkenntnisgewinn ermöglicht. So konnte über die Region des heutigen Hessens für den in dieser Arbeit behandelten Zeitraum eine sehr detaillierte Aussage getroffen werden. Diese Herangehensweise versprach einen Informationsgewinn über die damalige impfgegnerische Strömung in Hessen, weitere Studien müssten zeigen, inwieweit die Ergebnisse repräsentativ für andere Regionen Deutschlands oder auch darüber hinaus sind. Die Ergebnisse sind daher für einen begrenzten (Zeit-)Raum repräsentativ, da methodisch ähnlich angelegte Untersuchungen zu anderen Regionen bislang nicht vorliegen. Allerdings zeigten sich durch einen Vergleich mit der Impfgegnerschaft des 21. Jahrhunderts angesichts der völlig veränderten technischen Möglichkeiten der Kommunikation durch Internet und social media interessante Gemeinsamkeiten. Einerseits unterlag Letztere durch technische Neuerungen wie dem Internet einem klaren Wandel. Andererseits fanden sich gerade an dieser Stelle Gemeinsamkeiten, wie beispielsweise der Meinungsaustausch über soziale Plattformen wie Facebook, oder Internetforen. Durch die Verbreitung einer Meinung oder Frage in Echtzeit an tausende Gruppenmitglieder gleichzeitig übertreffen diese die Wirkkraft eines monatlichen Periodikums wie den „Impfgegner“ bei weitem. Trotzdem ist das Grundprinzip der Organisation in einem öffentlich zugänglichen Medium gleich geblieben, weswegen diesbezüglich eine gewisse Kontinuität festgestellt werden konnte. Weitere Parallelen ließen sich zwischen dem eingangs erwähnten Hugo Wegener und dem 1958 geborenen Hans Tolzin erstellen. Ein inhaltlicher Vergleich diverser Internetseiten des Letzteren ergab darüber hinaus starke Überschneidungen in Rhetorik, Öffentlichkeitsarbeit und schlussendlich auch der laienmedizinischen Perspektive. Während Wegener für seine impfkritischen Aktivitäten das Medium Buch und Periodikum nutzt, konnte für Tolzin eine hohe publizistische Aktivität im Internet festgestellt werden. Eine der größten Parallelen fand sich in Form des „Impf-reportes“, ein alle zwei Monate erscheinendes Periodikum, welches seit 2004 im Hans-Tolzin-Verlag unter seiner Leitung erscheint. Dass das Impfthema auch über 140 Jahre nach Erlass des Reichsimpfgesetzes nicht an Aktualität eingebüßt hat, konnte durch einen abschließenden Diskurs über die derzeitige Diskussion zur Verschärfung des Impfrechtes gezeigt werden. Hierzu wurde die Gesetzeslage in Deutschland mit der des europäischen Auslandes verglichen und vor dem Hintergrund aktueller Infektionsraten durch Seuchenkrankheiten behandelt; hierbei standen allerdings die Masern im Fokus - da die Pocken laut WHO seit 40 Jahren weltweit als ausgerottet gelten.

Necrotizing scleritis is uncommon in dogs and presumed to be immune-mediated. Its clinical pattern and histopathology are similar to ocular lesions observed in humans suffering from granulomatosis with polyangiitis (GPA), formerly named Wegener’s granulomatosis, where the pathogenesis revolves around anti-neutrophil antibodies (e.g., anti-myeloperoxidase). These antibodies are used to diagnose and follow-up the disease in humans, but variants that only affect the eyes often test negative. Here, we present the first case of canine necrotizing scleritis where measurement of anti-myeloperoxidase antibodies was attempted. A 1.5 year-old female Scottish Terrier was presented with bilateral deep multifocal scleromalacia, severe inflammation of corneal/uveal/retrobulbar tissues, perilimbal corneal oedema and neovascularization, hypotony, and mild exophthalmos. Corticosteroids and antibiotics had been administrated (topically and orally) without success. Due to painful multifocal scleral perforation with vitreal haemorrhage, the left eye underwent enucleation, so did the right eye one week later. The histopathology of the left eye revealed a neutrophilic and histiocytic scleral infiltration with extension of pyogranulomatous inflammation to the cornea, choroid, ciliary body, and orbital fat. Levels of plasma anti-myeloperoxidase antibodies were not statistically significant to those of 13 healthy dogs. Further research is warranted to investigate the presence and role of anti-neutrophil antibodies in canine necrotizing scleritis.

International audience; Boris Choubert was a strong supporter of Wegener's continental drift theory. In 1935, he published a very accurate fit of the circum-Atlantic continents, which was based on continental edges instead of coastlines; in the same paper, he interpreted the Palaeozoic belts as the result of horizontal movements of the Precambrian blocks; so, he greatly expanded the role of continental drift through time. This original and very prophetic work was almost completely ignored by his contemporaries. Thirty years later (1965), Bullard, Everett and Smith published in turn a similar but more sophisticated fit; they did not acknowledge Choubert's initial work. Bullard's fit was met with immediate and tremendous success. The present paper analyses the reasons why Boris Choubert was frustrated of his pioneering role. This lack of recognition is related to: (1) a great evolution in the geological concepts between 1935 and 1965, and (2) a poor choice of Choubert, regarding the title of his 1935 article.

Introduction and objective: Subglottic stenosis in Wegener’s disease (WD) represents a diagnostic challenge because it’s ability to be the unique manifestation. Description: A case of subglottic GW described in a woman 14 years. Discussion: Subglottic stenosis secondary to Wegener’s disease use to be associated with ENT symptoms, so these can give us the etiological suspicion. In the absence of these, we should include it in the differential diagnosis and try a histopathologic confirmation. Conclusions: In an isolated subglottic stenosis, rule Wegener's disease performing a biopsy of nasal mucosa and two serology tests., Introducción y objetivo: La estenosis subglótica en la granulomatosis de Wegener (GW) representa un desafío diagnóstico ante la posibilidad de manifestarse de forma aislada. Descripción: Se describe un caso de GW subglótica en una mujer de 14 años. Discusión: La estenosis subglótica secundaria a GW puede asociarse a otros síntomas otorrinolaringológicos que nos pueden orientar a dicha etiología. En ausencia de estos debemos incluirla en el diagnóstico diferencial e intentar una confirmación histopatológica. Conclusiones: Ante una estenosis subglótica aislada, descartar la GW realizando una biopsia de mucosa nasal y dos serologías separadas en el tiempo.

Significant eosinophilia is a prominent feature in Churg-Strauss syndrome but has not been described in granulomatosis with polyangiitis (GPA) in a pediatric patient. We present a biopsy case that confirmed granulomatosis with polyangiitis with significant eosinophilia > 30% on the initial presentation. Etiologies that could account for eosinophilia were excluded during workup. The patient's presentation of pulmonary alveolar hemorrhage, conjunctivitis arthritis, high-titer cytoplasmic antineutrophil, PR3-ANCA antibodies, and cytoplasmic antibodies (cANCA) was consistent with a clinical picture of atypical GPA in a pediatric patient. Se describe una actividad de indagación sobre la Tectónica de placas. En ella, el alumnado debe reconstruir la historia geológica de un planeta imaginario integrando distintos tipos de datos (distribución geográfica de fósiles, antigüedad de orógenos, regiones sísmicas) para construir un modelo de la ubicación de los límites de placa tectónica y los cambios que han sufrido los distintos continentes. A lo largo de la actividad se suceden distintas organizaciones de aula y andamios didácticos, con el objetivo de promover dinámicas de creación de conocimiento científico y la sistematización del aprendizaje y la consecución del objetivo final: realizar un breve vídeo de divulgación científica sobre la historia geológica de este planeta imaginario., The article describes an Inquiry didactic sequence on Plate Tectonics. Students are asked to reconstruct the geologic evolution of an imaginary planet by correlating different kinds of data (geographic distribution of fossils, orogens age, seismic regions...) discovering the positions of the plates and the changes suffered by the continents. Though the activity, several classroom dynamics and didactic scaffolds are set to promote dynamics of creation of scientific knowledge and contents sistematization to reach the final goal: to perform a science dissemination video describing the geological history of this imaginary planet.

Fik Vening Meinesz (1887-1966) was a Dutch civil engineer, employed by the Dutch Geodetic Committee. He devised an apparatus, containing a system of multiple pendulums, with which he could measure gravity in areas where such measurements would otherwise be impossible. By chance, he was made aware that a submerged submarine, when powered by electromotors, would provide an ideal, sufficiently stable, platform for such measurements at sea. The Dutch Navy, in pursuit of good publicity in the interwar period, gave Vening Meinesz its full support, enabling him to carry out a number of submarine expeditions. His findings stimulated postwar geophysical and geological research, both in the USA and elsewhere. Initially Vening Meinesz used his results to validate the International Gravity Formula, being the closest mathematical representation of the equipotential surface of the Earth at sea level - the geoid. Subsequently he interpreted these results from a tectonophysical perspective, using them to explain the occurrence of the elongated 'Minus' zone of negative gravity anomalies, discovered by him to surround the Indonesian archipelago and which he postulated as being caused by a system of convection currents in the Earth's mantle. Whilst he supported the idea of fixed continents, becoming increasingly opposed to Wegener's idea of drifting continents, his work on mantle convection was to become fundamental to, and incorporated in, the hypothesis of seafloor spreading. This led to the current paradigm of plate tectonics, which interprets the 'Minus' zone as the expression of the boundary between two convergent plates.

The aim is to conduct a descriptive, population-based study in order to assess temporal and spatial changes in mortality due to granulomatosis with polyangiitis (GPA) in Spain from 1984 to 2016. Mortality data were obtained from the Spanish Annual Death Registry. Deaths in which GPA was the underlying cause were selected using the 446.4 and M31.3 codes from the International Classification of Diseases, 9th and 10th revision. Annual average age at death and age-adjusted mortality rates were calculated. Geographic analysis was performed at municipality and district level. Variations in mortality according to the type of municipality (urban, agro-urban or rural), district and geographic location (degrees of latitude) were assessed using standardized mortality ratios (SMRs) and smoothed-SMRs. Over the whole period, 620 deaths due to GPA were identified. Age at death increased at an average annual rate of 0.78% over the period 1987-2016 ( p < 0.05). Age-adjusted mortality rates increased by an annual average of 20.58% from 1984 to 1992, after which they fell by 1.91% a year ( p < 0.05). The agro-urban category had the highest percentage (4.57%) of municipalities with a significantly higher GPA mortality rate than expected. Geographic analysis revealed four districts with a higher risk of death due to GPA, two in the North of Spain and two in the South. This population-based study revealed an increase in the age at death attributed to GPA. Age-adjusted mortality rates went up sharply until 1992, after which they started to decline until the end of the study period. Geographic differences in mortality risk were identified but further studies will be necessary to ascertain the reasons for the distribution of GPA disease., Competing Interests: The authors declare no conflict of interest.

Using the baculovirus/insect cells system, we have expressed a recombinant proteinase 3 (PR3) -- the neutrophil-derived serine protease autoantigen in Wegener's granulomatosis -- as a glycosylated intracellular and membrane-associated protein. Oligosaccharides accounted for the difference in molecular weights between recombinant (34 kDa) and neutrophil-PR3 (29 kDa). Whereas rabbit-anti-PR3 IgG recognized both recombinant and neutrophil-derived PR3, autoantibodies from Wegener patient sera recognized only neutrophil-derived PR3. Although oligosaccharides were not involved in PR3 epitope recognition, autoantibodies did not recognize the amino acid primary structure of recombinant PR3. Improper disulfide bond formation and/or lack of post-translational events in insect cells, may affect the conformation and/or lack of post-translational events in insect cells, may affect the conformation of PR3, precluding its reactivity with sera from WG patients.

El punto de partida del presente trabajo fue la novela Der Golem de Gustav Meyrink del año 1915. La figura del Golem entretejida con elementos de la cábala y el tarot crea una imagen fantástico-mística que el protagonista de la novela interioriza y asimila. Casi simultáneamente las reflexiones se asociaron con literatura secundaria, por el vínculo de la novela con la película Der Golem wie er in die Welt kam de Paul Wegener. Estas reflexiones iníciales fueron completadas con bibliografía secundaria, con el fin de comprobarse si se había establecido un vínculo entre la novela Der Golem de Gustav Meyrink y la película Der Golem wie er in die Welt kam de Paul Wegener. Esta película, manifestación del cine mudo de los años 20 del siglo pasado, incluye igualmente elementos fantásticos y ha sido estudiada a menudo junto con la novela. El nexo entre ambas obras no es sólo lo fantástico sino también el tema central y el mensaje que contienen. En este trabajo partimos de la siguiente hipótesis: entre la novela de Meyrink y la película de Wegener se establecen estrechos lazos semánticos. Para poder verificar esta hipótesis se debían analizar en un primer paso los elementos fantásticos y muy puntualmente definir la identidad del fenómeno fantástico. En el desarrollo de la investigación se puso de manifiesto que además de los elementos fantásticos debían tenerse en cuenta aspectos relacionados con disciplinas como el psicoanálisis, el ocultismo y esoterismo. Gracias a estas aportaciones culturales, que inciden en la interpretación, ambas figuras del Golem pueden ser estudiadas desde una nueva perspectiva, además de que se posibilita la creación de un vínculo entre la novela y la película, prácticamente inexistentes hasta ahora en la bibliografía secundaria. Se trata de un resultado que obedece a distintas razones: primer lugar, tanto la publicación de la novela de Meyrink como el primer acercamiento al tema del Golem por parte de Wegener se produce en el mismo año. En segundo lugar, el tema central de ambos textos (el film también es un texto) atrajo a ambos autores en un tipo de grandes cambios como fue la primera guerra mundial. La influencia de lo fantástico, que ha podido ser constatada a lo largo de este trabajo, posee una variable histórica, sobre todo porque en épocas de cambios sociales y de inestabilidad se advierte una tendencia hacia la expresión fantástica (aparte de Gustav Meyrink el capítulo “Fragwürdigstes” de la novela Der Zauberberg de Thomas Mann aporta otro ejemplo). Con este tras-fondo se explica la estructura de nuestro estudio: en el primer capítulo se tematiza la cuestión de lo fantástico en la novela de Meyrink, para posteriormente explorar este concepto en el marco de la literatura fantástica. La argumentación se amplía con reflexiones sobre lo fantástico en la estructura de películas, y sobre la constitución del lenguaje cinematográfico en sí. El segundo capítulo está dedicado al análisis de la novela de Meyrink; -y el tercero-, a la de la película de Wegener. En el cuarto capítulo se analiza la conexión entre ambas fuentes, no solamente con el fin de hacer referencia a la relación en el aspecto formal sino también para encontrar respuestas a la pregunta sobre la existencia de un mensaje común a ambas obras.

Die Granulomatose mit Polyangiitis (GPA) ist eine multifaktorielle Autoimmunerkrankung. Literaturhinweise deuten auf eine gestörte sinunasale Barriere. Die Untersuchung der Einflussfaktoren einer solchen Störung war das Ziel dieser Arbeit. In der nasalen Mukosa wurden 10 differentiell exprimierte Transkripte identifiziert sowie S. aureus intraepithelial nachgewiesen. Im Überstand nasaler Epithelzellen waren die Konzentrationen von G-CSF erhöht und von IL-8 erniedrigt. Nasale Epithelzellen sekretieren nach S. aureus-Stimulation weniger IL-8 und hBD3. Ergo zeigt sich eine vielschichtige nasale Barrierestörung bei der GPA: Reduzierte IL-8-, hBD3- und Lysozym-Konzentrationen sowie geringere Expression von Mustererkennungs-Rezeptoren ermöglichen eine Veränderung der mikrobiellen Flora der nasalen Mukosa und begünstigen eine Besiedlung mit S. aureus. Die Resultate eröffnen ein besseres Verständnis der komplexen Pathomechanismen der GPA sowie Perspektiven für neue Therapie-Optionen. The granulomatosis with polyangiitis (GPA) is a multifactorial autoimmune disease. Evidence from literature suggests an impaired sinonasal barrier. The study of the factors that influence such interference was the goal of this work. In the nasal mucosa, 10 differentially expressed transcripts were identified and S. aureus was demonstrated intraepithelial. In the supernatant of nasal epithelial cells the concentration of G-CSF was increased while IL-8 was decreased. Nasal epithelial cells secrete after S. aureus stimulation less IL-8 and hBD3. A complex nasal barrier dysfunction in GPA is shown: reduced IL-8, hBD3 and lysozyme concentrations and lower expression of pattern recognition receptors allow a change in the microbial flora of the nasal mucosa and promote colonization with S. aureus. These results allow a better understanding of the complex pathophysiology of the GPA and opens up perspectives for novel therapeutic options.

Alfred Wegener formalizou o conceito de deriva continental (1912-1915) com base na mobilidade da superfície sólida da Terra; ele teve precursores mas, como meteorologista, conhecendo os movimentos rápidos dos fluidos na atmosfera, tinha as bases e a motivação necessárias para extrapolar esta mobilidade para o fluxo lento da superfície sólida terrestre, um salto fenomenológico maior nas Ciências da Terra. A tectónica de placas (1962-1969) progrediu a partir do conhecimento das bacias oceânicas para modelo cinemático completo da litosfera à escala global; através do conceito do Ciclo de Wilson a teoria pôde alargar a história da Terra para a evolução dos oceanos, continentes e cinturas móveis ao longo das placas tectónicas. Tornou-se um paradigma para a teoria da Terra como um sistema aberto e dinâmico.

Pulmonary-renal syndrome is defined as the combination of diffuse alveolar haemorrhage (DAH) and glomerulonephritis. The majority of cases are initiated by small vessel vasculitides presenting antineutrophil cytoplasmic autoantibodies (ANCA): Wegener's granulomatosis (WG), microscopic polyangiitis (MPA) and Churg-Strauss-syndrome (CSS). Up to 25% of these ANCA-associated vasculitides (AAV) have also an increased venous thromboembolic risk due to circulating anti-plasminogen antibodies. Pulmonary-renal syndrome in AAV is often fatal without treatment, which still relies on immunosuppression by cytotoxic agents. As such therapy may be lethal itself, accurate assessment of disease severity is mandatory. In this scenario, plasma exchange (PLEX) has proven a fast-acting therapeutic adjunct during severe disease as it limits the use of immunosuppressives and their life-threatening side effects. Novel biomarkers of AAV activity, as well as data from randomized trials investigating new agents, will be essential to improve treatment of severe and refractory forms in the future.

Objetivos: realizar una recopilación biográfíca de los primeros descriptores de la granulomatosis de Wegener (GW) y establecer de una manera detallada los principales hechos que marcaron la evolución de la GW hasta nuestros días. Material y métodos: revisión sistemática de la literatura desde 1866 a 2007 a través de Pubmed, Embase, Ovid, Sciencedirect, SpringerLink, Blackwell Synergy, Highwire y búsqueda en la biblioteca de la Universidad Nacional de Colombia Resultados: se encontraron 4.588 artículos con relación a la GW, de los cuales seleccionamos en idiomas inglés y alemán, con énfasis en revisiones históricas médicas, ensayos clínicos relevantes, estudios controlados, aleatorizados y revisiones en general, con interés para las principales asociaciones médicas, quedando al final un conglomerado de 89 fuentes bibliográficas. Los datos biográficos de los descriptores iniciales están sujetos principalmente a la literatura alemana, muchos datos se perdieron en los escrutinios alemanes y polacos. Conclusiones: las descripciones de la literatura en cuanto al reconocimiento de la GW se centra en el norte de Europa, su descripción se asoció a la de otras vasculitis y permitió reforzar los conocimientos inmunológicos del presente. Observamos cómo la clínica, el tratamiento y posteriormente los estudios paraclínicos fueron ampliándose cada día, logrando así que muchos conceptos originados hace más de 40 años hagan parte de los regímenes actuales. Objectives: make a biographical compilation of the first ones who described the WG and establish in a detailed way the main facts that marked the evolution of the WG to the present time. Material and methods: a systematic review of literature from 1866 to 2007 through Pubmed, Embase, Ovid, Sciencedirect, SpringerLink, Blackwell Synergy, Highwire and searches in the library of the National University of Colombia Results: There were 4588 articles found related to WG, from which we selected in English and German language, with emphasis in medical historical revisions, relevant clinical tests, controlled randomizeds studies and revisions in general, with interest for the main medical associations and also to the consideration of the authors of this review. Being left in the end a conglomerate of 89. The bibliographic data of the first describers are attached mainly to german literature, many of this information was lost in the German and Polish scrutinies. Conclusions: the descriptions of literature as far as the recognition of WG are focused in the northern Europe, its description was associated to those of other vaculitis and allowed to reinforce immunological knowledge which is basic in the present. We observed how the clinic, the treatment and later de paraclinic studies were extended every day, and how many concepts created more than 40 years ago are still part of actual regimes.

Three attempts of understanding the mechanisms that could account for the Earth structure have been developed before the current Global Tectonics exists. First attempt, on the eve of modern geology, Léonce Élie de Beaumont hypothesizes the occurrence of "uplifted craters" (cratères de soulèvement) and "uplifted systems" related to the age of the successive mountain belts. This rigid system, crowned with the "pentagonal network" theory, eventually collapsed. Second attempt, at the end of the 19th century, Eduard Suess replaces the "uplift" hypothesis by that of the "downwarps", at the origin of the oceans. Tangential forces explain the polarity of the fold belts that moved as giant "solid waves" towards their foreland, during three major periods of foldings. Third - 76 - attempt, in 1912, Alfred Wegener rejects the dogma of the cooling and shrinking globe, and launches his hypothesis of "continental drift" following the break of a post-Carboniferous "Pangea". This theory was fiercely combated, but Émile Argand took it into account for the Alpine belts. It appears as a premature avatar of Global Tectonics., Trois essais de compréhension de mécanismes essentiels expliquant la structure du globe terrestre ont été faits avant l'actuelle " tectonique des plaques ". 1) Aux aurores de la géologie moderne, Léonce Élie de Beaumont développe les hypothèses des " cratères de soulèvement " et des " systèmes de soulèvements " donnant un âge aux montagnes successives. Ce système rigide, couronné par la théorie du " réseau pentagonal ", s'effondra avant la mort de son auteur. 2) À la fin du X I Xe siècle, Eduard Suess remplace l'hypothèse des " soulèvements " par celle des " affaissements ", à l'origine des océans. L'action des forces tangentielles explique la polarité des chaînes plissées, avec déplacement de grandes " vagues solides " vers un avant-pays, lors de trois grands plissements successifs. 3) Niant le dogme de la contraction par refroidissement du Globe, Alfred Wegener lance en 1912 son hypothèse de " dérive des continents ", déplacés vers le sud et vers l'ouest après rupture d'une Pangée post-Carbonifère. Cette théorie, vigoureusement combattue, fut cependant prise en compte par Émile Argand dans les chaînes alpines. On y voit un avatar prématuré de l'actuelle tectonique globale.

La maladie de Wegener est une vascularite systémique rare, touchant les vaisseaux de petit et moyen calibre. Elle atteint principalement les voies aériennes supérieures, les poumons et les reins. Les premières manifestations peuvent cependant être buccales et se traduire par des ulcérations, une hyperplasie gingivale et/ou une inflammation des glandes salivaires. Il s'agit de manifestations mineures qui représentent toutefois un élément diagnostique souvent méconnu. Trois cas de maladie de Wegener avec des manifestations buccales traitées dans la Division de stomatologie, chirurgie orale et radiologie dentaire et maxillo-faciale de L'Ecole de médecine dentaire de Genève sont présentés. Dans deux cas, les lésions buccales ont révélé la maladie. Ce travail souligne, à la lumière de la littérature, l'importance d'un examen clinique complet et rigoureux pour le diagnostic de cette maladie rare, dont les complications sont potentiellement graves en absence d'une prise en charge précoce.

- Abstract
Wegener's granulomatosis (WG) is a unique clinicopathological disease characterized by necrotizing granulomatous vasculitis of the respiratory tract, pauci-immune necrotizing glomerulonephritis and small-vessel vasculitis. Owing to its wide range of clinical manifestations, WG has a broad spectrum of severity that includes the potential for alveolar hemorrhage or rapidly progressive glomerulonephritis, which are immediately life threatening. WG is associated with the presence of circulating antineutrophil cytoplasm antibodies (c-ANCAs). The most widely accepted pathogenetic model suggests that c-ANCA-activated cytokine-primed neutrophils induce microvascular damage and a rapid escalation of inflammation with recruitment of mononuclear cells. The diagnosis of WG is made on the basis of typical clinical and radiologic findings, by biopsy of involved organ, the presence of c-ANCA and exclusion of all other small-vessel vasculitis. Currently, a regimen consisting of daily cyclophosphamide and corticosteroids is considered standard therapy. A number of trials have evaluated the efficacy of less-toxic immunosuppressants and antibacterials for treating patients with WG, resulting in the identification of effective alternative regimens to induce or maintain remission in certain subpopulations of patients. Recent investigation has focused on other immunomodulatory agents (e.g., TNF-alpha inhibitors and anti-CD20 antibodies), intravenous immunoglobulins and antithymocyte globulins for treating patients with resistant WG.

Il volume è articolato in due parti. Nella prima vengono presentati, sistematizzati per aree tematiche e collocati nel dibattito linguistico coevo gli aspetti funzionali e pragmatici delle riflessioni linguistiche di Wegener che lo collocano fra i precursori della moderna teoria pragmatica (per es. la concezione del linguaggio come azione e interrelazione, la dialettica tra forma e funzione, l’articolazione testuale in soggetto logico e predicato logico, la relazione tra enunciazione e situazione o contesto). Nella seconda trova ampio spazio la traduzione di alcuni capitoli delle Untersuchungen relativi alle tematiche presentate nel saggio introduttivo., The volume is divided into two parts. The first,entitled Aspetti funzionali e pragmatici nel pensiero linguistico di Philipp Wegener, discusses those aspects of Wegener’s linguistic thought which placed him among the forerunners of the modern pragmatic theory (e.g. the view of the language as action and interaction, the dialectics between form and function, the text informational organization in psychological subject and psychological predicate, the relationship between utterance and situation or context). Wegener’s main concepts are systematized according to thematic areas and discussed with reference to the coeval linguistic debate and to the following developments in linguistic theory. The second part of the work consists in the translation of some chapters of Wegener’s Untersuchungen über die Grundfragen des Sprachlebens related to the thematic areas focused on in the introduction essay.
