Background. Marker-less systems based on digital video cameras and deep learning for gait analysis could have a deep impact in clinical routine. A recently developed system has shown promising results in terms of joint center position but has not been yet evaluated in terms of gait outcomes. Research question. How does this novel marker-less system compare to a marker-based reference system in terms of clinically relevant gait parameters? Methods. The deep learning method behind the developed marker-less system was trained on a dedicated dataset consisting of forty-one asymptomatic and pathological subjects each performing ten walking trials. The system could estimate the three-dimensional position of seventeen joint centers or keypoints (e.g., neck, shoulders, hip, knee, and ankles). We evaluated the marker-less system against a marker-based system in terms of differences in joint position (Euclidean distance), detection of gait events (e.g., heel strike and toe-off), spatiotemporal parameters (e.g., step length, time), kinematic parameters (e.g., hip and knee extension-flexion), and inter-trial reliability for kinematic parameters. Results. The marker-less system was able to estimate the three-dimensional position of joint centers with a mean difference of 13.1 mm (SD = 10.2 mm). 99% of the estimated gait events were estimated within 10 milliseconds of the corresponding reference values. Estimated spatiotemporal parameters showed zero bias. The mean and standard deviation of the differences of the estimated kinematic parameters varied by parameter (for example, the mean and standard deviation for knee extension flexion angle were -3.0° and 2.7°). Inter-trial reliability of the measured parameters was similar to that of the marker-based references. Significance. The developed marker-less system can measure the spatiotemporal parameters within the range of the minimum detectable changes obtained using the marker-based reference system. Aims This study addressed two questions: first, does surgical correction of an idiopathic scoliosis increase the volume of the rib cage, and second, is it possible to evaluate the change in lung function after corrective surgery for adolescent idiopathic scoliosis (AIS) using biplanar radiographs of the ribcage with 3D reconstruction? Methods A total of 45 patients with a thoracic AIS which needed surgical correction and fusion were included in a prospective study. All patients underwent pulmonary function testing (PFT) and low-dose biplanar radiographs both preoperatively and one year after surgery. The following measurements were recorded: forced vital capacity (FVC), slow vital capacity (SVC), and total lung capacity (TLC). Rib cage volume (RCV), maximum rib hump, main thoracic curve Cobb angle (MCCA), medial-lateral and anteroposterior diameter, and T4-T12 kyphosis were calculated from 3D reconstructions of the biplanar radiographs. Results All spinal and thoracic measurements improved significantly after surgery (p < 0.001). RCV increased from 4.9 l (SD 1) preoperatively to 5.3 l (SD 0.9) (p < 0.001) while TLC increased from 4.1 l (SD 0.9) preoperatively to 4.3 l (SD 0.8) (p < 0.001). RCV was correlated with all functional indexes before and after correction of the deformity. Improvement in RCV was weakly correlated with correction of the mean thoracic Cobb angle (p = 0.006). The difference in TLC was significantly correlated with changes in RCV (p = 0.041). It was possible to predict postoperative TLC from the postoperative RCV. Conclusion 3D rib cage assessment from biplanar radiographs could be a minimally invasive method of estimating pulmonary function before and after spinal fusion in patients with an AIS. Contexte Les troubles fonctionnels de la main sont généralement investigués, en premier lieu, à l’aide de l’imagerie radiographique conventionnelle. Cependant, les radiographies (deux dimensions (2D)) fournissent des renseignements limités, et l’information peut être réduite par le chevauchement des os et les biais de projection. Ce travail présente une méthode de reconstruction de la main en trois dimensions (3D) à partir de radiographies biplanes. Méthode Cette approche consiste en la déformation d’un modèle générique de main sur les radiographies biplanes par des processus manuels et automatiques. L’examen de référence étant la segmentation manuelle scanographique, la précision de la méthode a été évaluée par une comparaison entre les reconstructions à partir de radiographies biplanes et les reconstructions correspondantes à partir de scanner (0,3 mm d’épaisseur de coupe). Pour évaluer la reproductibilité de la méthode, 6 mains saines (6 sujets, 3 gauches, 3 hommes) ont été considérées. Deux opérateurs ont répété trois fois chaque reconstruction à partir de radiographies biplanes pour étudier la variabilité inter- et intra-opérateur. Trois paramètres anatomiques, qui pourraient être calculés automatiquement à partir des reconstructions, ont été considérés à partir des surfaces osseuses : la longueur du scaphoïde, la profondeur de l’extrémité distale du radius et la hauteur du trapèze. Résultats Le double de l’erreur quadratique moyenne (2 Root Mean Square, 2RMS), au niveau de la différence point/surface entre les radiographies biplanes et les reconstructions tomodensitométriques, variait de 0,46 mm pour les phalanges distales à 1,55 mm pour les os de la deuxième rangée. La variabilité inter-/intra-observateur a montré une précision à 95 % de l’intervalle de confiance inférieure à 1,32 mm, pour les paramètres anatomiques, et à 2,12 mm pour les barycentres osseux. Discussion La méthode actuelle permet d’obtenir une reconstruction 3D précise de la main et du poignet par rapport au scanner segmenté traditionnel. En améliorant l’automatisation de la méthode, cela nous permettrait d’obtenir rapidement des informations objectives sur la position des os dans l’espace. L’intérêt de cette méthode réside dans le diagnostic précoce de certaines pathologies ligamentaires (instabilité carpienne) et a également des implications dans la planification chirurgicale et la modélisation personnalisée par éléments finis.

Objective: To identify the different apex and transitional vertebra according to the shape of the pelvis of individuals despite their difference in sagittal alignment using our measurement system. Methods: Full spine X-rays using EOS in standard stand-position of 99 volunteers were selected (47 women, 52 men, mean age 31years old). Validated 3D reconstruction technique allowing extraction of spinopelvic parameters, and position and rotation of each vertebra and lumbar discs. Subjects were divided in three groups: low PI (low PI, n=37), moderate PI (mid PI, n=52), high PI (high PI, n=10), with respectively a PI below 45 °, between 45 °-60 ° and above 60 °. Occurrence of specific position and rotation values of apex and transitional vertebra were assessed in each groups. Results: Frequency curves tend to move cranially when the incidence increases except in cervicothoracic where T1 is a constant for all shape of spine with occurrence approaching 90%. Angulation value of relevant vertebra and lumbar lordosis are significantly positively correlated for the whole population. Conclusions: Our study allowed the assessment of the distribution of spine curvatures according to the pelvic incidence. It describes the occurrence of localization of the apex and transitional vertebrae according to pelvic incidence. These results should be taken into account during the analysis of the sagittal balance, especially when planning deformity surgery in adults.

International audience; Background: The deep learning-based human pose estimation methods, which can estimate joint centers position, have achieved promising results on the publicly available human pose datasets (e.g., Human3.6 M). However, these datasets may be less efficient for gait study, particularly for clinical applications, because of the limited number of subjects, their homogeneity (all asymptomatic adults), and the errors introduced by marker placement on subjects’ regular clothing. Research question: How a new human pose dataset, adapted for gait study, could contribute to the advancement and evaluation of marker-less motion capture systems? Methods: A marker-less system, based on deep learning-based pose estimation methods, was proposed. A new dataset (ENSAM dataset) was collected. Twenty-two asymptomatic adults, one adult with scoliosis, one adult with spondylolisthesis, and seven children with bone disease performed ten walking trials, while being recorded both by the proposed marker-less system and a reference system – combining a marker-based motion capture system and a medical imaging system (EOS). The dataset was split into training and test sets. The pose estimation method, already trained on the Human3.6 M dataset, was evaluated on the ENSAM test set, then reevaluated after further training on the ENSAM training set. The joints coordinates were evaluated, using Bland-Altman bias and 95 % confidence interval, and joint position error (the Euclidean distance between the estimated joint centers and the corresponding reference values). Results: The Bland-Altman 95 % confidence intervals were substantially improved after finetuning the pose estimation method on the ENSAM training set (e.g., from 106.9 mm to 17.4 mm for the hip joint). With the new dataset and approach, the mean joint position error varied from 6.2 mm for ankles to 21.1 mm for shoulders. Significance: The proposed marker-less system achieved promising results in terms of joint position errors. Future studies are necessary to assess the system in terms of gait parameters.

Adolescent idiopathic scoliosis (AIS) is the most common spinal disorder in children. A severity index was recently proposed to identify the stable from the progressive scoliosis at the first standardized biplanar radiographic exam. The aim of this work was to extend the validation of the severity index and to determine if curve location influences its predictive capabilities. AIS patients with Cobb angle between 10° and 25°, Risser 0–2, and no previous treatment were included. They underwent standing biplanar radiography and 3D reconstruction of the spine and pelvis, which allowed to calculate their severity index. Patients were grouped by curve location (thoracic, thoracolumbar, lumbar). Patients were followed up until skeletal maturity (Risser ≥ 3) or brace prescription. Their outcome was compared to the prediction made by the severity index. In total, 205 AIS patients were included; 82% of them (155/189, 95% confidence interval [74–90%]) were correctly classified by the index, while 16 patients were unclassified. Positive predictive ratio was 78% and negative predictive ratio was 86%. Specificity (78%) was not significantly affected by curve location, while patients with thoracic and lumbar curves showed higher sensitivity (≥ 89%) than those with thoracolumbar curves (74%). In this multicentric cohort of 205 patients, the severity index was used to predict the risk of progression from mild to moderate scoliosis, with similar results of typical major curve types. This index represents a novel tool to aid the clinician and the patient in the modulation of the follow-up and, for progressive patients, their decision for brace treatment. • The severity index of adolescent idiopathic scoliosis has the potential to detect patients with progressive scoliosis as early as the first exam. • Out of 205 patients, 82% were correctly classified as either stable or progressive by the severity index. • The location of the main curve had small effect on the predictive capability of the index.

International audience; Subject-specific tensioning of ligaments is essential for the stability of the knee joint and represents a challenging aspect in the development of finite element models. We aimed to introduce and evaluate a new procedure for the quantification of ligament prestrains from biplanar X-ray and CT data. Subject-specific model evaluation was performed by comparing predicted femorotibial kinematics with the in vitro response of six cadaveric specimens. The differences obtained using personalized models were comparable to those reported in similar studies in the literature. This study is the first step toward the use of simplified, personalized knee FE models in clinical context such as ligament balancing.

International audience; Spine deformity during adolescent idiopathic scoliosis can induce a rib-cage deformity. This bone deformity can have direct consequences on the chest-wall muscles, including intercostal muscles, leading to respiratory impairments in individuals with severe cases. The aim of this study was to determine whether shear-wave elastography can be used to measure intercostal-muscle shear-wave speed (SWS) in healthy children and those with adolescent idiopathic scoliosis (AIS). Nineteen healthy participants and 16 with AIS took part. SWS measurements were taken by three operators, twice each. Average SWS was 2.3 ± 0.4 m/s, and inter-operator reproducibility was 0.2 m/s. SWS was significantly higher during apnea than in normal breathing (p < 0.01) in both groups. No significant difference was observed between groups in apnea or in normal breathing. Characterization of the intercostal muscles by ultrasound elastography is therefore feasible and reliable for children and adolescents with and without scoliosis.

Context Children with X-linked hypophosphatemic (XLH) rickets have muscle weakness that severely impairs their function. Intermuscular and intramuscular adipose tissue (IMAT and intraMAT, respectively) may contribute to this muscle weakness. Objective This work aimed to compare IMAT and intraMAT in XLH children vs typically developing (TD) children. Methods A prospective, monocentric cohort study was conducted of XLH (n = 11; aged 10.3 years [6-17]) and TD children (n = 22; aged 10.2 years [5-15.5]). All children underwent magnetic resonance imaging of the lower limbs; IMAT and intraMAT percentages were calculated after manual contouring of each muscle of the thigh and the deep fascia at mid-thigh level. Results XLH children were comparable in age but shorter and heavier than TD children (P = .001 and P = .03, respectively). They had smaller muscle length and volume than TD children (P < .001) but there was no statistically significant difference in muscle cross-sectional area between the groups (P = .833). The total percentage of IMAT was higher in XLH children (8.66% vs 3.60% in TD children; P < .0001). In addition, though the total percentage of intraMAT did not differ significantly (12.58% and 10.85% in XLH and TD children, respectively; P = .143) intraMAT was statistically significantly higher in XLH children than TD children in 4 of the 13 muscles studied. Conclusion Our results show that IMAT is higher in young children with XLH, independently of obesity and overweight. Further, these results will facilitate both the early prevention of functional and metabolic consequences of the increase in adipose tissue in XLH children.

X-linked hypophosphatemic (XLH) rickets causes significant bone deformities in the lower limbs resulting from a bone mineralization defect. According to Frost's Mechanostat theory, compensatory modeling of the bones takes place during increased mechanical loads. In addition, mechanical stimuli modulate the differentiation of mesenchymal stem cells; common precursors to bone marrow adipocytes and osteoblasts.Bone deformities of the lower limbs lead to increased femoral bone mass and decreased fatty infiltration of the bone marrow (FIBM) in children with XLH rickets compared to a control group.Eleven children (10.3y [6-17]) with XLH rickets and 22 healthy children (10.2y [5-15.5]) underwent lower limb Magnetic Resonance Imaging. A calculation of FIBM was performed at the mid-femur, as well as a calculation of the total bone cross-sectional area (CSA), the cortical CSA, the anteroposterior and mediolateral axes of the femur, bone marrow and the thickness of the femoral cortices.Total bone CSA, total cortical CSA and bone marrow CSA were higher in the XLH group than in the control group (p0.05). The mid-lateral diameters of the femur and bone marrow were more elongated than those of the control group (p0.001). Only the anterior cortex was thinned in the XLH group (p=0.001), while there was no difference with the control group for the posterior, medial and lateral cortices. The total percentage of FIBM was 72.81% [±3.95] and 77.4% [±5.52] for the XLH and control groups respectively (p0.001).The increase in bone mass in the XLH population reflects an adaptation of bone tissue to the bone deformities present in this pathology. The decrease in FIBM indicates a lower risk of osteoporosis in the XLH population and may constitute a new monitoring parameter in this pathology.III; Case-control study.

Purpose: To assess dynamic postural alignment in ASD during walking using a subject-specific 3D approach. Methods: 69 ASD (51 ± 20 years, 77%F) and 62 controls (34 ± 13 years, 62%F) underwent gait analysis along with full-body biplanar Xrays and filled HRQoL questionnaires. Spinopelvic and postural parameters were computed from 3D skeletal reconstructions, including radiographic odontoid to hip axis angle (ODHA) that evaluates the head's position over the pelvis (rODHA), in addition to rSVA and rPT. The 3D bones were then registered on each gait frame to compute the dynamic ODHA (dODHA), dSVA, and dPT. Patients with high dODHA (> mean + 1SD in controls) were classified as ASD-DU (dynamically unbalanced), otherwise as ASD-DB (dynamically balanced). Between-group comparisons and relationship between parameters were investigated. Results: 26 patients were classified as ASD-DU having an average dODHA of 10.4° (ASD-DB: 1.2°, controls: 1.7°), dSVA of 112 mm (ASD-DB: 57 mm, controls: 43 mm), and dPT of 21° (ASD-DB: 18°, controls: 14°; all p < 0.001). On static radiographs, ASD-DU group showed more severe sagittal malalignment than ASD-DB, with more altered HRQoL outcomes. The ASD-DU group had an overall abnormal walking compared to ASD-DB & controls (gait deviation index: 81 versus 93 & 97 resp., p < 0.001) showing a reduced flexion/extension range of motion at the hips and knees with a slower gait speed and shorter step length. Dynamic ODHA was correlated to HRQoL scores. Conclusion: Dynamically unbalanced ASD had postural malalignment that persist during walking, associated with kinematic alterations in the trunk, pelvis, and lower limbs, making them more prone to falls. Dynamic-ODHA correlates better with HRQoL outcomes than dSVA and dPT.

Pathologies of the respiratory system can by accompanied by alterations of the biomechanical function of the rib cage, as well as of its morphology and movement. The assessment of such pathologies could benefit from rib cage kinematic analysis during breathing, but this analysis is challenging because of the difficulties in observing and quantifying bone movements in vivo. This work explored the feasibility of using biplanar x-rays to study rib cage modifications at different lung volumes and evaluated the potential of the method to characterize rib cage kinematic patterns in patients. Forty-seven asymptomatic adults and eleven obstructive sleep apnea syndrome (OSAS) patients underwent biplanar x-rays at three lung volumes: normal breathing, maximal and minimal volume. Rib cage and spinopelvic positional parameters were computed from 3D reconstruction of the skeleton. Results showed that inspiration mostly mobilized the ribs and costo-vertebral junction, while expiration was driven by the spine. OSAS patients had a different sagittal profile at rest than asymptomatic subjects, but these differences decreased at maximal and minimal volume. This suggests that patients employed different biomechanical strategies to attain a trunk configuration similar to asymptomatic subjects at minimal and maximal lung volume. This study confirmed that the proposed method could have an impact for the clinical assessment and understanding of pathologies involving breathing function, and which directly affect rib cage morphology.

Purpose: To evaluate the global alignment of non-operated subjects with adolescent idiopathic scoliosis. Method: A total of 254 subjects with AIS and 64 controls underwent low dose biplanar X-rays and had their spine, pelvis, and rib cage reconstructed in 3D. Global alignment was measured in the sagittal and frontal planes by calculating the OD-HA angle (between C2 dens to hip axis with the vertical). Subjects with AIS were classified as malaligned if the OD-HA was > 95th percentile relative to controls. Results: The sagittal OD-HA in AIS remained within the normal ranges. In the frontal plane, 182 AIS were normally aligned (Group 1, OD-HA = 0.9°) but 72 were malaligned (Group 2, OD-HA = 2.9°). Group 2 had a more severe spinal deformity in the frontal and horizontal planes compared to Group 1 (Cobb: 42 ± 16° vs. 30 ± 18°; apical vertebral rotation AVR: 19 ± 10° vs. 12 ± 7°, all p < 0.05). Group 2 subjects were mainly classified as Lenke 5 or 6. 19/72 malaligned subjects had a mild deformity (Cobb < 30°) but a progressive scoliosis (severity index ≥ 0.6). The frontal OD-HA angle was found to be mainly determined (adjusted-R2 = 0.22) by the apical vertebral rotation and secondarily by the Lenke type. Conclusions: This study showed that frontal malalignment is more common in distal major structural scoliosis and its main driver is the apical vertebral rotation. This highlights the importance of monitoring the axial plane deformity in order to avoid worsening of the frontal global alignment.

Vertebral burst fractures are usually due to traumatic high-energy distraction of the vertebra. They often require pedicle screw fixation to stabilize the spine during the healing process but once the fusion of the fractured vertebra has occurred, the posterior fixation can be removed or shortened to restore part of the spinal mobility. The importance of sparing fusion levels to ensure optimal functional results has been demonstrated. In surgical planning, the rule is therefore to ensure the osteosynthesis stability while limiting the number of levels included. Immobilization or fixation of a vertebral segment may be responsible for degeneration of the intervertebral disc and in case of surgical treatment the adjacent disc. Therefore, we can ask whether removal of a vertebral fixation could decrease the stress on adjacent segments. Ultrasound shearwave elastography is a new innovative and non-invasive technique allowing evaluation of soft tissue’s elastic modulus through the measurement of shear wave speed (SWS). This non-invasive imaging method has shown its added value in breast or in liver oncology and, more recently, in musculoskeletal pathologies assessment. In particular, Langlais et al. found that SWS measured in annulus fibrosus was significantly higher in progressive adolescent idiopathic scoliosis than in asymptomatic population, which is associated with a stiffer tissue. The same authors defined a corridor of normality (2.3–3.7 m/s) for the asymptomatic population and observed that annulus SWS tended to normalize 1 year after fusion surgery in severe adolescent idiopathic scoliosis. The aim of this case was to analyze the effect of removing the posterior fixation in a case of burst fracture on the mechanical properties of the bridged and adjacent intervertebral discs.

Introduction X-linked hypophosphatemia (XLH) rickets mainly causes leg deformities in children that can get worse as they grow. We hypothesized that quantifying the bone parameters will help to document and monitor these deformities in children with XLH. Methods Thirty-five growing children affected by XLH were included in this cross-sectional study. Biplanar radiographs were taken with an EOS system allowing 3D reconstructions of the pelvis and legs. Sixteen geometric parameters were calculated for the legs and pelvis. A control group of 40 age-matched patients was used to define the reference values for these geometric parameters. Results For the legs, significant differences (p < 0.05) appeared between the XLH patients and the control group in the neck-shaft angle, femur/tibia length ratio and HKS. Among the 70 legs in the XLH group, 23 were in genu varum, 25 were in genu valgum and 22 were straight. There were significant differences between the genu varum and genu valgum subgroups in the femoral mechanical angle and the HKS. A strong correlation was found between the femoral mechanical angle and tibiofemoral angle (r² = 0.73) and between the femoral mechanical angle and HKS (r²=0.69) The sacral slope and acetabular anteversion were significant different from the reference values. Discussion Quantitative radiological parameters derived from 3D reconstructions show that the deformities in XLH patients are 1) mainly in but not limited to the femoral shaft; 2) highly variable from one person to another. Background: Physiologic aging is associated with loss of mobility, sarcopenia, skin atrophy and loss of elasticity. These factors contribute, in the elderly, to the occurrence of a pressure ulcer (PU). Brightness mode ultrasound (US) and shear wave elastography (SWE) have been proposed as a patient-specific, bedside, and predictive tool for PU. However, reliability and clinical feasibility in application to the sacral region have not been clearly established. Method: The current study aimed to propose a simple bedside protocol combining US and SWE. The protocol was first tested on a group of 19 healthy young subjects by two operators. The measurements were repeated three times. Eight parameters were evaluated at the medial sacral crest. Intraclass Correlation Coefficient (ICC) was used for reliability assessment and the modified Bland Altman plot analysis for agreement assessment. The protocol was then evaluated for clinical feasibility on a healthy older group of 11 subjects with a mean age of 65 ± 2.4 yrs. Findings: ICC showed poor to good reliability except for skin SWE and hypodermis thickness with an ICC (reported as: mean(95%CI)) of 0.78(0.50-0.91) and 0.98(0.95-0.99) respectively. No significant differences were observed between the young and older group except for the muscle Shear Wave Speed (SWS) (respectively 2.11 ± 0.27 m/s vs 1.70 ± 0.17 m/s). Interpretation: This is the first protocol combining US and SWE that can be proposed on a large scale in nursing homes. Reliability, however, was unsatisfactory for most parameters despite efforts to standardize the protocol and measurement definitions. Further studies are needed to improve reliability. Adolescent idiopathic scoliosis (AIS) is a three-dimensional deformity of the spine. Spine slenderness, which represents its potential instability to buckling under compressive loads, was shown to be higher in AIS patients than non-scoliotic subjects, but it is not clear at what stage of the progression this difference appeared, nor if slenderness could be used as an early sign of progression. In this study, we hypothesized that slenderness could be an early sign of progression. One-hundred thirty-eight patients and 93 non-scoliotic subjects were included. They underwent standing biplanar radiography and 3D reconstruction of the spine, which allowed computing vertebra and disc slenderness ratio. Then, patients were followed until progression of the deformity or skeletal maturity (stable patients). Vertebral slenderness ratio in AIS patients varied between 2.9 [2.7; 3.0] (T9) and 3.4 [3.2; 3.6] (T1), while disc slenderness ranged from 0.6 [0.6; 0.7] at T6-T7 to 1.2 [1.1; 1.3] at L4-L5. Slenderness ratio increased with age, while disc slenderness tended to decrease with age and Cobb angle. Slenderness was similar between progressive and stable patients, and also between patients and non-scoliotic subjects. In conclusion, spinal slenderness does not appear to be an early sign of progression. Further studies should analyse the development of slenderness during growth, and how it could be affected by non-operative treatment.

Background As a leading cause of disability with a high societal and economic cost, it is crucial to better understand risk factors of neck pain and surgical complications. Getting subject-specific external loading is essential for quantifying muscle forces and joint loads but it requires exertion trials and load cells which are uncommon in clinical settings. Methods This paper presents a method to compute the gravitational loading at four levels of the cervical spine (C3C4, C4C5, C5C6, C6C7) in neutral standing position from biplanar radiographs exclusively. The resulting load was decomposed in local disc frames and its components were used to compare different populations: 118 asymptomatic subjects and 46 patients before and after surgery (anterior cervical discectomy and fusion or total disc replacement). Comparisons were performed at C6C7 and the upper level adjacent to surgery. Findings Significant changes in gravitational loading were observed with age in healthy subjects as well as in patients after surgery and have been associated with changes in posture. Interpretation This approach quantifies the influence of postural changes on gravitational loading on the cervical spine. It represents a simple way to obtain necessary input for muscle force quantification models in clinical routine and to use them for patient evaluation. The study of the subsequent subject-specific spinal loading could help further the understanding of cervical spine biomechanics, degeneration mechanisms and complications following surgery. EOS biplane radiographs of 117 subjects between 20 and 83 years were analyzed to compute the upper body lever arm over the L1 vertebra and its impact on vertebral strength. Postural sagittal alignment alteration was observed with age and resulted in a greater lever arm causing vertebral strength to decrease. The purpose of this study was to analyze the impact of postural alignment changes with age on vertebral strength using finite element analysis and barycentremetry. A total of 117 subjects from 20 to 83 years were divided in three age groups: young (20 to 40 years, 62 subjects), intermediate (40 to 60 years, 26 subjects), and elderly (60 years and over, 29 subjects). EOS biplane radiographs were acquired, allowing 3D reconstruction of the spine and body envelope as well as spinal, pelvic, and sagittal alignment parameter measurements. A barycentremetry method allowed the estimation of the mass and center of mass (CoM) position of the upper body above L1, relatively to the center of the L1 vertebra (lever arm). To investigate the effect of this lever arm, vertebral strength of a generic finite element model (with constant geometry and mechanical properties for all subjects) was successively computed applying the personalized lever arm of each subject. A combination of an increase in thoracic kyphosis, cervical lordosis, and pelvic tilt with a loss of lumbar lordosis was observed between the young and the older groups. Sagittal alignment parameters indicated a more forward position as age increased. The lever arm of the CoM above L1 varied from an average of 1 mm backward for the young group, to averages of 10 and 24 mm forward, respectively, for the intermediate and elderly group. As a result, vertebral strength decreased from 2527 N for the young group to 1820 N for the elderly group. The global sagittal alignment modifications observed with age were consistent with the literature. Posture alteration with age reduced vertebral strength significantly in this simplified loading model. Postural alignment seems essential to be considered in the evaluation of osteoporotic patients.

Three-dimensional bone reconstructions from medical imaging are essential for biomechanical modelling and are growing tools in clinics. Several methods of lower limbs reconstruction from biplanar radiographs have been proposed in the literature but with significant operator dependence. A novel reconstruction method based on reduced manual annotation, statistical shape models and fully automatic adjustments was proposed in this study. While significantly reducing operator intervention, the proposed method demonstrated similar or better precision than previous approaches on clinical parameters. Meanwhile, shape accuracy was improved to around 1mm. By quasi-automating the 3D reconstruction without loss of accuracy and precision, the proposed approach is a considerable step towards extensive use of 3D personalized models in clinical routine and large cohort biomechanical studies. Spheroids encapsulated within alginate capsules are emerging as suitable in vitro tools to investigate the impact of mechanical forces on tumor growth since the internal tumor pressure can be retrieved from the deformation of the capsule. Here we focus on the particular case of Cellular Capsule Technology (CCT). We show in this contribution that a modeling approach accounting for the triphasic nature of the spheroid (extracellular matrix, tumor cells and interstitial fluid) offers a new perspective of analysis revealing that the pressure retrieved experimentally cannot be interpreted as a direct picture of the pressure sustained by the tumor cells and, as such, cannot therefore be used to quantify the critical pressure which induces stress-induced phenotype switch in tumor cells. The proposed multiphase reactive poro-mechanical model was cross-validated. Parameter sensitivity analyses on the digital twin revealed that the main parameters determining the encapsulated growth configuration are different from those driving growth in free condition, confirming that radically different phenomena are at play. Results reported in this contribution support the idea that multiphase reactive poro-mechanics is an exceptional theoretical framework to attain an in-depth understanding of CCT experiments, to confirm their hypotheses and to further improve their design.

Background: Balanced global spinopelvic balance is important in the maintenance of the physiological alignment of all body segments above the pelvis with minimum energy expenditure. The key determinants affecting the 3D index-odontoid-hip axis (OD-HA) angle, and in particular its alterations, have not been clearly defined. The aim of this study is to identify the determinants of the 3D OD-HA angle in maintaining global spinopelvic balance in a large Chinese adult cohort of different gender and age groups. Methods: A total of 516 asymptomatic adults were enrolled in this study. Biplanar radiographies were performed to reconstruct the subject’s inter-acetabular axis and C2 odontoid process. The 3D angle formed by the vertical and the line between odontoid and mid-interacetabular axis (OD-HA angle) was computed and projected in the subject’s sagittal and coronal planes. Thoracic kyphosis (TK), lumbar lordosis (LL), T1 pelvic angle (TPA) and sagittal vertical axis (SVA) were measured. Classical evaluation and follow-up are usually based on full-body static radiographs and health related quality of life questionnaires. Despite being an essential daily life activity, formal gait assessment lacks in clinical practice.Research Question:What are the main alterations in gait kinematics of ASD and their radiological determinants?Methods:52 ASD and 63 control subjects underwent full-body 3D gait analysis with calculation of joint kinematics and full-body biplanar X-rays with calculation of 3D postural parameters. International audience; This study aims to describe the spinopelvic sagittal alignment in transfemoral amputees (TFAs) from a radiologic study of the spine with a postural approach to better understand the high prevalence of low back pain (LBP) in this population.METHODS:TFAs underwent X-rays with 3-D reconstructions of the full spine and pelvis. Sagittal parameters were analyzed and compared to the literature. Differences between TFAs with and without LBP were also observed.RESULTS:Twelve subjects have been prospectively included (TFA-LBP group (n = 5) and TFA-NoP group (n = 7)). Four of the five subjects of the TFA-LBP group and two of the seven in TFAs-NoP group had an imbalanced sagittal posture, especially regarding the T9-tilt, significantly higher in the TFA-LBP group than in the TFA-NoP (p = 0.046). Eight subjects (6 TFA-NoP and 2 TFA-LBP) had abnormal low value of thoracic kyphosis (TK). Moreover, the mean angle of TK in the TFA-NoP group was lower than in the TFA-LBP group (p = 0.0511).CONCLUSION:In the considered sample, TFAs often present a sagittal imbalance. A low TK angle seems to be associated with the absence of LBP. It can be hypothesized that this compensatory mechanism of the sagittal imbalance is the most accessible in this population. This study emphasizes the importance of considering the sagittal balance of the pelvis and the spine in patients with a TFA to better understand the high prevalence of LBP in this population. To validate the predictive power and reliability of a novel quasi-automatic method to calculate the severity index of adolescent idiopathic scoliosis (AIS). Fifty-five AIS patients were prospectively included (age 10–15, Cobb 16° ± 4°). Patients underwent low-dose biplanar X-rays, and a novel fast method for 3D reconstruction of the spine was performed. They were followed until skeletal maturity (stable patients) or brace prescription (progressive patients). The severity index was calculated at the first examination, based on 3D parameters of the scoliotic curve, and it was compared with the patient’s final outcome (progressive or stable). Three operators have repeated the 3D reconstruction twice for a subset of 30 patients to assess reproducibility (through Cohen’s kappa and intra-class correlation coefficient). Eighty-five percentage of the patients were correctly classified as stable or progressive by the severity index, with a sensitivity of 92% and specificity of 74%. Substantial intra-operator agreement and good inter-operator agreement were observed, with 80% of the progressive patients correctly detected at the first examination. The novel severity index assessment took less than 4 min of operator time. The fast and semiautomatic method for 3D reconstruction developed in this work allowed for a fast and reliable calculation of the severity index. The method is fast and user friendly. Once extensively validated, this severity index could allow very early initiation of conservative treatment for progressive patients, thus increasing treatment efficacy and therefore reducing the need for corrective surgery. Study design: Case-Control Study Objectives: To analyse global sagittal alignment including the cranial center of mass (CCOM) and proximal junctional kyphosis (PJK) in Adolescent Idiopathic Scoliosis (AIS) patients treated with posterior instrumentation. Summary of background data: PJK plays an important role in the global sagittal alignment in AIS patients. Maintaining the head above the pelvis allows for a minimization of energy expense in ambulation and upright posture. Numerous studies have been performed to understand PJK phenomena in AIS patients. However, to our knowledge, no study performed on AIS patients included the head in the analysis of global sagittal alignment and PJK. Methods: This study included 85 AIS patients and 51 asymptomatic adolescents. Low-dose bi-planar X-rays were acquired for each subject preoperatively and at two years follow up. Two global sagittal alignment parameters were calculated, i.e. the angle between the vertical and the line joining the center of the bi-coxofemoral axis (HA) and either the most superior point of the dentiform apophysis of C2 (OD) or the cranial center of mass (CCOM). Results: Among normal adolescents, the average OD-HA and CCOM-HA angles were -2.3°±2° and -1.5°±1.8°, respectively. Among AIS patients the average OD-HA and CCOM-HA angles were respectively -2.3°±1.9° and -1.3°±1.8° preoperatively and -2.8°±1.7° and -1.9°±1.7° at the last follow up. 13% of the patients developed PJK postoperatively. Case-by-case analysis showed that adjusting the thoracic kyphosis and the compensations required to maintain this constant could provide explanatory elements. Conclusions: OD-HA and CCOM-HA angles remain almost constant among normal group and patients, pre- and post-operatively, whether PJK or non-PJK. Five patients without PJK and only one patient with PJK produced abnormal values relative to the asymptomatic subjects. Therefore, PJK could be a compensation mechanism, which allows for CCOM-HA and, to a lesser extent, OD-HA to remain invariant. International audience; Objective: This paper reports on the quantification of passive wrist joint stiffness and investigates the potential influence of handedness and gender on stiffness estimates.Methods: We evaluated the torque-angle relationship during passive wrist movements in 2 degrees of freedom (into flexion-extension and radial-ulnar deviation) in 13 healthy subjects using a wrist robot. Experimental results determined intrasubject differences between dominant and nondominant wrist and intersubject differences between male and female participants.Results: We found differences in the magnitude of passive stiffness of left- and right-hand dominant males and right-hand dominant females suggesting that the dominant hand tends to be stiffer than the nondominant hand. Left-hand stiffness magnitude was found to be 37% higher than the right-hand stiffness magnitude in the left-handed male group and the right-hand stiffness magnitude was 11% and 40% higher in the right-handed male and female groups, respectively. Other joint stiffness features such as the orientation and the anisotropy of wrist stiffness followed the expected pattern from previous studies.Conclusion: The observed difference in wrist stiffness between the dominant and nondominant limb is likely due to biomechanical adaptations to repetitive asymmetric activities (such as squash, tennis, basketball, or activities of daily living such as writing, teeth brushing, etc.). Significance: Understanding and quantifying handedness influence on stiffness may have critical implication for the optimization of surgical and rehabilitative interventions.

International audience; Background: Biplanar X-ray system providing anteroposterior and sagittal plane with an ultra-low radiation dose and in weight-bearing position is increasingly used for spine imaging. The original three-dimensional (3D) reconstruction method from biplanar X-rays has been widely used for clinical parameters, however, the main issue is that manual adjustments of the 3D model was quite time-consuming and limited to thoracolumbar spine. A quasi-automated 3D reconstruction method of the spine from cervical vertebra to pelvis was proposed, which proved fast and accurate in 57 patients with adolescent idiopathic scoliosis. The aim of this study was to compare the newly developed technique of quasi-automatic 3D measurement with classical 2D measurements in a large cohort.Methods: A total of 494 adults with biplanar EOS X-ray scanning were included in this study and divided into health and deformity group according to the presence of spinal deformity. The proposed method of quasi-automatic 3D measurement was applied to all these subjects. The radiographic parameters included: thoracic kyphosis (TK), lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), sagittal vertical axis (SVA), T1 pelvic angle (TPA) in sagittal plane, and cobb angle in coronal plane. Comparison was made between quasi-automatic and manual measurement.Results: The mean age was 53.7±19.9 years old. In the whole population, the mean differences between the two methods were 3.9° for TK (30.5°±9.9° vs. 26.5°±9.3°, P30°, respectively. Correlation analysis showed r2 for all clinical parameters ranged from 0.667 to 0.923. On average, the new method takes 5 minutes to compute all the parameters for one case.Conclusions: In conclusion, this ergonomic and efficient quasi-automatic method for full spine proved fast and accurate measurement in a large population, which showed great potential in extensive clinical application

International audience; Objective: Our objective was to establish a corridor of normality for the external shape 3D parameters and then to assess these variables in adolescent idiopathic scoliosis (AIS).Methods: Adolescent with mild and severe AIS were included prospectively, as well as a control group of asymptomatic subjects. A quasi‐automatic 3D reconstruction of the spine and manual 3D reconstruction of the external envelope was performed from biplanar radiography. The center of mass position, the axial intersegmental moment resulting at the apex and junctional vertebrae, and the coronal trunk balance were automatically computed. A normality corridor of asymptomatic subjects was calculated as the range [5th-95th percentiles] for external shape parameters at each vertebral level.Results: Forty-one asymptomatic subjects (19 females; 22 males; 21 yo, SD=4) and sixty AIS (56 females; 4 males; 13 years old, SD=1.9; 30 mild and 30 severe; 34 thoracic curves and 26 thoraco-lumbar or lumbar curves) were included. All parameters based on the external shape showed differences between AIS and controls, as well as between mild and severe scoliosis. For instance, the intersegmental moment applied to the upper junctional vertebra was above the 95th percentile of controls in 70% of AIS patient.
